背景技术
进行荧光引导的开放性手术常用于肿瘤切除。手术所需的荧光辐射通常由相机检测,即由芯片/光传感器检测,或者通过目镜在相应滤光片的帮助下检测,该滤光片可以分辨除荧光辐射之外的任何波长的辐射。
目前,荧光引导/支持的开放性手术以两种方式进行。
第一种方式是使用专用的相机,该相机设计成、构造成及购买来仅或至少主要于用荧光引导/支持的开放性手术。这种专用相机包括发射想要的激发辐射的辐射源,该激发辐射产生荧光。这种系统的缺点是必须要购买单独设备,才能进行荧光引导的开放性手术。
第二种方式是使用常用的内窥镜成像仪。内窥镜成像仪可以是在内窥镜手术期间与内窥镜联接(coupled)的相机,比如通过其目镜。该内窥镜成像仪用于获取荧光图像。荧光是由照明设备产生的,该照明设备可以与内窥镜成像仪连接,例如通过耦合器。通常情况下,内窥镜成像仪正好包括一个这样的耦合器,该耦合器可以在内窥镜手术期间容纳内窥镜。这通常是通过使内窥镜的目镜容纳在耦合器中来实现的。当从内窥镜成像仪中移除目镜时,上述照明设备可以与其连接。使用内窥镜成像仪,医院不需要购买额外的设备,如上述第一种方式所描述的设备,来进行荧光引导/支持的开放性手术。但是,第二种系统的缺点是需要特别设计的无菌盖,原因如下文所述。
在内窥镜手术期间,覆盖内窥镜成像仪的标准无菌盖与内窥镜一起连接到耦合器上。由于内窥镜通常在手术前是经过消毒的,只有内窥镜成像仪由于包含电子部件而无法消毒,因此其必须用无菌盖遮挡。
然而,当内窥镜成像仪不使用内窥镜进行荧光引导/支持的开放性手术时,内窥镜成像仪的耦合器或开口被照明设备占据。通常,标准无菌盖不能与照明设备一起连接到耦合器,即使可能,标准无菌盖也不能同时覆盖内窥镜成像仪和照明设备。由于内窥镜成像仪和照明设备通常都是不可消毒的,因此必须购买昂贵的专用(非标准)无菌盖来使用上述第二种系统。
本发明旨在克服上述缺点。除了其他的,本发明旨在提供一种系统,该系统可以利用大多数医院中存在的标准设备。
发明内容

当现有技术中最先进的照明设备连接到上述耦合器时,它们需要单独的无菌盖,这通常非常昂贵的,因为标准无菌盖或照明设备可以分别(但是不能同时)连接到耦合器。然而,在照明设备和无菌盖都可以连接到耦合器的情况下,由于照明设备以纵向而非侧向方式连接到内窥镜成像仪,因此标准无菌盖不能覆盖照明设备。本发明克服了这个问题。

根据本发明的照明设备的连接装置可以从以下方式中选择:摁扣装置(snap-onmeans)、卡扣装置(clip-on-menas)、捆扎装置(strapping-on-menas)、自锁装置(positive-locking means)、螺钉装置、硅橡胶以及其他非永久性粘合装置。

第二,如果照明设备和内窥镜成像仪以一种特定的方式连接,即两者都采用一个相对于彼此特定的(而不是随机或任意的)位置,外科医生会自动将内窥镜成像仪保持在正确的位置和角度。
例如,照明设备可以仅以一种方式连接到内窥镜成像仪。或者,存在指示如何连接两个部件的标记等。根据本发明的两种替代方案可以实施这。
当内窥镜成像仪包括本发明优选的芯片/光传感器时,凭直觉不清楚如何,尤其是在哪个角度下,握持内窥镜成像仪以创建所需图像,因此只能无意地将转动图像,如转动90°或180°。
然而,当外科医生没有握持内窥镜成像仪(其未显示握持角度),而是握持带有手指容纳装置的照明设备时,外科医生将直观地握住照明装置,从而也以正确的角度握住与其相连的内窥镜成像仪。
在照明设备没有手指容纳装置的情况下,外科医生可能会以某种方式“抓住”内窥镜成像仪,并使其拍摄并呈现在屏幕上的图片被旋转90°或180°。
本发明通常包括一种装置,其确保内窥镜成像仪和照明设备仅以一种特定方式和方向连接。
本发明的照明设备,优选地其主体,包括将其连接到支架(stand)的装置。该装置可以是上述手指容纳装置的替代装置。优选地,该装置位于照明装置的主体中或主体处,以便外科医生决定是将其握在手中还是将其连接到支架上。这种支架常用于拍摄定时曝光图像。
照明设备的开口中可以插入滤光片,该滤光片仅允许能够引起荧光的辐射通过。
辐射离开照明设备的方向可以相对于内窥镜成像仪的光学元件的光轴倾斜。可以选择其倾斜度或角度,使得在10到30cm,优选地15到25cm的距离内,由照明设备提供的照明的主要部分指向内窥镜成像仪的光轴。换言之,更一般地说,照明设备可以包括用于引导辐射的装置,从而使得内窥镜成像仪“看到”的区域被良好地照亮。由于内窥镜成像仪以及本发明的照明设备通常放置在距离手术区域10到30cm之间,安装或连接到内窥镜成像仪之后的照明设备可以引导大部分辐射在10到30cm的距离内,从而使光轴基本上位于辐射区域的中心。
离开照明设备的辐射/照明的方向优选参照离开照明设备的光锥/辐射锥的假想中心轴。
辐射以一定的倾斜度离开照明设备,其优点是可以降低引起不需要的反射的概率。当照明/辐射与内窥镜成像仪的光轴共轴时,反射的概率较高,而当辐射沿倾斜于光轴的方向前进时,反射的概率较低。使用根据本发明实施例的倾斜度,可以产生较低的假信号,并且增加荧光检测的深度。
根据本发明的实施例,该照明设备包括诸如铰链之类的装置,该装置用于改变倾斜度,即离开照明设备的照明/辐射的方向与内窥镜成像仪的光轴之间的角度。这些装置优选地集成在照明设备中,并且改变离开照明设备的照明方向。或者,可以改变照明设备和内窥镜成像仪之间的连接角度以改变倾斜度。
通过改变倾斜度,用户可以调整工作距离。可以安装和使用照明设备,使得光轴和假想中心轴之间的交集等于工作距离。因此,照明设备提供的照明/辐射被最有效地利用。照明设备可以设置改变倾斜度的装置,以便在距离照明设备和/或内窥镜成像仪10到50cm的距离处调整交叉点。
一般而言,照明设备可包括可改变离开照明设备的辐射的倾斜度的装置。改变倾斜度优选地导致可以在距离照明设备和/或内窥镜成像仪10到50cm之间调整其工作距离。
照明设备的主体可包括连接装置,该连接装置可以将照明设备连接到支架、机械臂或半自动臂。
本发明还包括一种系统,该系统包括上述照明设备和上述内窥镜成像仪。

图1为现有技术的内窥镜21,其包括目镜9、光导入口22和轴10。此外,所示内窥镜成像仪5包括开口17和耦合器8。
在图1b中,内窥镜21的目镜9安装在耦合器8处。
图1a与图1b的不同之处仅在于目镜9(因此整个内窥镜21)与耦合器8分离。
图1b中所示的布置用于内窥镜手术。经由连接件12,成像仪电缆(未示出)可以连接到内窥镜成像仪5,使得来自身体内部的图像被传送到屏幕(未示出)。
在图1a中最佳示出的标准内窥镜21也可以在没有内窥镜成像仪5的情况下用于内窥镜手术。然后外科医生观察目镜9,光经由光导入口22引入身体。
图1a/1b中所示的设备和布置在现有技术中是已知的。
图2-5示出了根据本发明的照明设备1的一实施例的不同立体图。
照明设备1经由连接装置4连接到常规内窥镜成像仪5。
此外,图2至图5显示出了照明设备1的开口3、弹片形式的连接装置4、手指容纳装置6、支架7、耦合器8、内成像仪电缆的连接件12、光导电缆13、用户的手14和突出部15。
如图2至图5所示,根据本发明的照明设备1工作方式如下:
通过连接装置4,照明设备1的主体2侧向连接到常规内窥镜成像仪5。参考图1a/1b,可以使用任何已知的内窥镜成像仪5。
由于根据本发明的照明设备1不占用耦合器8,因此耦合器可以容纳如下所述的无菌盖。
如图6和8所示,标准无菌盖16可以连接到耦合器8并覆盖内窥镜成像仪5和照明设备1。照明设备1发射的辐射18透过通常为透明的无菌盖16。因此,尽管照明设备1被无菌盖16覆盖,照明设备1仍可照亮手术区域(未示出)。
标准的无菌盖16设计为使其易于连接到耦合器8并覆盖整个内窥镜成像仪5。
由于内窥镜成像仪的光学器件通过开口17“看到”手术区域,覆盖开口17的无菌盖16的设计成使视线不受阻碍。可以采用如图8所示的组合式成像窗及连接件23。组合式成像窗连接件23结合了可容纳内窥镜的连接件和不阻挡或阻碍开口17的视野的透明成像窗。这样的无菌盖16为现有技术中已知的。
由图2至6中的内窥镜成像仪5拍摄的图像经由连接到连接件12的电缆11传输到处理器等并最终传输到屏幕(未示出)。
由照明设备1发送到手术区域(未示出)的辐射18由光源(未示出)提供,光源通过照明设备1的光导电缆13连接到照明设备1。
比较图3和图4,显而易见的是手指容纳装置6如何使用户本能地握住照明设备1。
图5显示了当用户不想将照明设备1握在手14上时,如何将照明设备1连接到支架7上。支架7很容易通过凹槽19与突出部15连接,如图2、3、4所示。
突出部15与其旁边的凹槽19一起,构成了将照明设备1连接至支架7的连接件以及其中一个手指容纳装置6。图2和图3所示的突出部15有很高的表面积,使其特别适合容纳手指并连接至支架7。
如图3和图6所示,离开照明设备的辐射18的方向可以在某种程度上倾斜,使得在10到30cm的距离内,内窥镜成像仪5的光轴20周围的区域得到很好地照明。
如图8所示,离开照明设备的主体2的开口3的辐射18穿过透明无菌盖16。因为主体2侧向连接到内窥镜成像仪5,所以没有部件阻挡辐射18穿过无菌盖16。
Claims (9)
1.一种照明设备(1),用于在开放性手术中用辐射照亮手术区域,以产生荧光,所述照明设备(1)包括:
主体(2),所述主体(2)包括被配置为释放辐射(18)的开口(3);
其特征在于,所述照明设备(1)还包括:
连接装置(4),所述连接装置(4)将所述主体(2)侧向连接到内窥镜成像仪(5)。
2.如权利要求1所述的照明设备(1),其中所述连接装置(4)从以下装置中选择:摁扣装置、卡扣装置、捆扎装置、自锁装置、螺钉装置、硅橡胶或其他非永久性粘合装置。
3.如权利要求1至2中的至少一项所述的照明设备(1),其中所述主体(2)包括提供辐射的辐射源。
4.如权利要求1至2中的至少一项所述的照明设备(1),其中所述主体(2)连接到提供辐射的辐射源。
5.根据上述权利要求中的至少一项所述的照明设备(1),其中所述主体(2)包括手指容纳装置(6)。
6.根据上述权利要求中的至少一项所述的照明设备(1),其中所述主体(2)包括可以连接到支架、机械臂或半自动臂的装置(15)。
7.根据上述权利要求中的至少一项所述的照明设备(1),其中所述开口(3)中插入滤光片,所述滤光片仅允许引起荧光的辐射通过。
8.根据上述权利要求中的至少一项所述的照明设备(1),其特征在于包含一装置,当所述照明设备(1)连接到所述内窥镜成像仪(5)时,所述装置用于改变离开所述照明设备(1)的辐射的倾斜度。
9.一种系统,其特征在于,包括根据上述至少一项权利要求所述的照明设备(1)和内窥镜成像仪(5)。
