突眼症,即眼睛相对于其在眼窝中正常位置的前移,是一种更为严重的情况。突眼症最常见的原因是甲状腺眼病,甲状腺功能异常会导致眼睛周围的肌肉、脂肪和其他组织肿胀。如果不加以控制,突眼症会导致眨眼和睡觉时眼睑无法正常闭合,进而造成角膜损伤。眼睛周围的组织肿胀(包括脂肪沉积)可能会妨碍其正常工作,从而影响眼睛的位置和眼球运动,从而导致复视。在严重的情况下,眼睛后面的组织、肌肉和脂肪沉积物的炎症和肿大会压迫视神经,导致视力丧失。
突眼症可以通过眼眶减压手术治疗。与此手术相关的风险包括复视、溢泪、鼻泪管阻塞、瘢痕形成、鼻子或眼睛周围出血、眼睛或鼻窦感染、视力丧失、视神经损伤、脑脊液漏、眼睑错位、眼睛周围疼痛或麻木、角膜刮伤和结膜肿胀。
另一种眼球突出症治疗方法是静脉注射利妥昔单抗(Teprotumumab,TEPEZZATM)。副作用包括肌肉痉挛或痉挛、恶心、脱发、腹泻、感觉疲劳、高血糖、听力问题、味觉变化、头痛和皮肤干燥。替普妥单抗也可能加重现有的炎性肠病(如克罗恩病或溃疡性结肠炎)的症状。
此外,与上呼吸道脂肪沉积过多相关的阻塞性睡眠呼吸暂停没有特定的治疗方法。对于肥胖患者,通常会进行减肥手术。虽然手术可能会改善阻塞性睡眠呼吸暂停,但它伴随着任何手术的典型风险。
有必要开发一种针对诸如脂肪性眼睑(Steatoblepharon)、突眼症和阻塞性睡眠呼吸暂停等疾病的治疗方法,以消除多余的脂肪沉积且没有现有方法的副作用。
发明内容
为了满足上述需要,提供了一种减少体内皮下脂肪沉积物或内脏脂肪沉积物的方法。所述方法通过将脂肪沉积物与有效量的包封在脂质体中的拉坦前列素组合物接触来实现。所述脂质体仅由蛋黄磷脂酰胆碱(EggPC)或棕榈酰磷脂酰胆碱(POPC)构成,有效量的组合物使脂肪沉积物中脂肪细胞的数量减少5%~90%。
提供了一种通过向受试者眼睑的脂肪沉积物中注射包封在EggPC或POPC脂质体中的拉坦前列素组合物来治疗受试者脂肪性眼睑的类似方法。所述组合物使脂肪沉积物中的脂肪细胞数量减少5%~90%。
还公开了一种通过向受试者的锥内空间(intraconal space)注射上述包封在EggPC或POPC脂质体中的拉坦前列素组合物来治疗突眼症的方法。应用所述方法可使锥内空间的脂肪细胞数量至少减少35%。
此外,提供了一种治疗与上呼吸道脂肪过量相关的阻塞性睡眠呼吸暂停的方法。所述方法是通过将有效量的包封在EggPC或POPC脂质体中的拉坦前列素组合物注射到上呼吸道脂肪沉积物中来实现的,从而使上呼吸道脂肪沉积物中的脂肪细胞数量减少5%~90%。

具体实施方式
如上所述,提供了一种减少体内脂肪沉积物(例如皮下脂肪沉积物或内脏脂肪沉积物)的方法。所述方法通过将脂肪沉积物与包封在脂质体中的拉坦前列素组合物接触来实现。接触步骤可以通过将组合物直接注射到脂肪沉积物中来完成。或者,可以将组合物注射到与脂肪沉积物相邻的胞间隙中。在特定方法中,脂肪沉积物是皮下脂肪沉积物,并且将组合物皮下注射到皮下脂肪沉积物中或其附近。将组合物局部施用到覆盖脂肪沉积物的皮肤上也在本发明的范围内。
上述方法中使用的组合物包括包封在仅由蛋黄磷脂酰胆碱(EggPC)或棕榈酰磷脂酰胆碱(POPC)构成的脂质体中的拉坦前列素。换言之,脂质体不含任何其它成分,如脂类或甾醇。包封拉坦前列素的EggPC和POPC脂质体可以是分别为美国专利10,272,040和9,956,195中描述的脂质体。所述组合物也可以按照这两项美国专利所述来制备。
组合物中拉坦前列素的浓度范围为可以为0.5mg/mL~3.0mg/mL(如0.5、1.0、1.5、2.0、2.5和3.0mg/mL)。在一个示例范围内,拉坦前列素的浓度为1.0mg/mL~2.3mg/mL(如1.0、1.1、1.2、1.3、1.4、1.5、1.6、1.7、1.8、1.9、2.0、2.1、2.2和2.3mg/mL)。
减少脂肪细胞数量所需的组合物的量根据脂肪沉积物的大小而改变。在特定方法中,每0.5cm3~1.0cm3的脂肪沉积物,注射上述组合物的体积为25μL~75μL。
以上述方法施加到脂肪沉积物上的组合物的量可有效地将脂肪细胞的数量减少5%~90%(如5%、10%、15%、20%、25%、30%、35%、40%、45%、50%、55%、60%、65%、70%、75%、80%、85%和90%)。脂肪细胞数量的减少范围可以为10%~90%或35%~90%。
发明内容部分描述了一种通过将上述拉坦前列素脂质体组合物注射到受试者眼睑中的脂肪沉积物中来治疗脂肪性眼睑(Steatoblepharon)的方法。如上所述方法,注射所述组合物可使脂肪沉积物中的脂肪细胞数量减少5%~90%(如5%、10%、15%、20%、25%、30%、35%、40%、45%、50%、55%、60%、65%、70%、75%、80%、85%和90%)。拉坦前列素的组成及其浓度也与上述减少脂肪的方法中所述的相同。例如,组合物中拉坦前列素的浓度在0.5mg/mL~3.0mg/mL的范围内,并且每0.5cm3~1.0cm3的脂肪沉积物,注射上述组合物的体积为25μL~75μL。

在这种方法的一个实例中,突眼症是甲状腺相关眼病的结果。在另一个实例中,患有突眼症的受试者患有格雷夫斯病或桥本甲状腺炎。
在治疗突眼症的方法中,注射组合物中拉坦前列素的浓度范围为0.5mg/mL~3.0mg/mL(如1.0mg/mL~2.3mg/mL)。注射导致锥内空间的脂肪细胞数量至少减少35%,最高减少90%(如35%、40%、45%、50%、55%、60%、65%、70%、75%、80%、85%和90%)。
最后,上述治疗与上呼吸道脂肪沉积相关的阻塞性睡眠呼吸暂停的方法是通过将拉坦前列素组合物注射到上呼吸道脂肪沉积物中来实现的,所述组合物为包封在EggPC或POPC脂质体中具有浓度为0.5mg/mL~3.0mg/mL(如1.0mg/mL~2.3mg/mL)的拉坦前列素。

实施例
实施例1:抑制前脂肪细胞分化
按照既定规程培养脂肪间充质干细胞(ADMSC),即前脂肪细胞,并使其分化为脂肪细胞。简言之,脂肪间充质干细胞从ATCC获得(PCS500011TM,马纳萨斯,弗吉尼亚州),并在37℃5%的CO2中用生长试剂盒(PCS500040TM)在间充质干细胞基础培养基(PCS500030TM)中培养。为了诱导分化,ADMSC培养物生长至70%~80%汇合度时,替换为成脂分化培养基(成脂分化试剂盒;Gibco Life TechnologiesTM,美国)。培养基每周更换两次。
9天和15天后,平板中细胞分别部分分化和完全分化为脂肪细胞。部分分化和完全分化的脂肪细胞都积累了脂滴。在接种后第15天,用赋形剂(vehicle)或用包封在EggPC脂质体中的拉坦前列素(EggPC-拉坦前列素,相当于10μg/mL拉坦前列素)处理脂肪细胞24小时。油红O染色后通过光学显微镜观察脂质的存在,并通过测量492nm处的吸光度进行定量。结果如图1A和图1B所示。
与赋形剂(vehicle)处理的细胞相比,用EggPC-拉坦前列素处理脂肪细胞可使脂滴中储存的油脂量减少5倍。
通过定量实时PCR分析检测已处理和未经处理的脂肪细胞中的基因表达。根据供应商的说明,使用核糖核酸逆转录试剂(分子研究中心有限公司,美国俄亥俄州辛辛那提市)从脂肪细胞中分离总RNA,并使用QuantiTect逆转录试剂盒(Qiagen Inc.,美国加利福尼亚州查茨沃斯市)进行逆转录。通过在480实时PCR系统(德国罗氏应用科学公司)中使用FastStartUniversal SYBR GreenMasterMix(罗氏应用科学公司)对实时PCR(RT-PCR)进行定量分析,确定促脂肪生成基因C/EBPα,PPARγ和LPL的信使RNA表达。热循环仪条件为95℃,持续10分钟。随后进行55个循环(95℃,10秒;50℃,5秒,72℃,13秒)。所有样品一式两份进行测试,通过△△Ct方法计算相对mRNA表达,以看家基因的表达进行归一化处理。结果如图2A~图2C所示。
Egg PC-拉坦前列素、POPC-拉坦前列素和游离拉坦前列素分别抑制脂肪细胞促进基因C/EBPα、PPARγ和LPL的表达。
实施例2:体内脂肪减少
通过直接注射入小鼠腹股沟脂肪垫中来测试脂质体包封的拉坦前列素减少体内脂肪沉积的能力。
在三只动物中每只的一个腹股沟脂肪垫中注射100μL赋形剂(vehicle)、100μL含100μg拉坦前列素的拉坦前列素溶液、100μL含100μg拉坦前列素的EggPC-拉坦前列素或100μL含100μg拉坦前列素的POPC-拉坦前列素。7天后,处死小鼠,测量注射的腹股沟脂肪垫和对侧未注射的脂肪垫的重量。结果见图3。
与对侧未注射的腹股沟脂肪垫相比,单次注射游离拉坦前列素并未导致注射7天后腹股沟脂肪垫的重量减少。事实上,注射拉坦前列素后脂肪垫重量增加。
令人惊讶的是,与未注射的脂肪垫相比,注射EgPC-拉坦前列素7天后,注射的腹股沟脂肪垫的重量减少了9%。
实施例3:锥内区域脂肪细胞计数的短期减少
游离拉坦前列素、EggPC-拉坦前列素和POPC-拉坦前列素分别通过眼球后注射引入大鼠眼眶内,以确定其对锥内脂肪沉积的作用。注射脱氧胆酸(DCA;)作为阳性对照。DCA已经被美国食品药品监督管理局批准用于减少下颌的脂肪细胞。
六只大鼠中每只单眼注射治疗。每只动物的对侧眼睛不注射,以此作为阴性对照。各组动物的药物剂量和注射体积如下表1所示。

注射后第7天,通过组织病理学图像的计算机分析对存在于锥内脂肪沉积物中的脂肪细胞的数量进行计数,并与对侧未注射眼睛中的脂肪细胞数量进行比较。结果如图4A~图4F和下表2所示。

结果表明,在单次注射后,注射EggPC-拉坦前列素和POPC-拉坦前列素对减少脂肪细胞数量的效果最佳。
实施例4:向大鼠模型的眼眶脂肪进行眼球后注射
材料
将游离拉坦前列素、EggPC-拉坦前列素、POPC-拉坦前列素和DCA分别通过眼球后注射引入大鼠眼眶,以确定其在三周内对锥内脂肪沉积物的影响。
将EggPC-拉坦前列素和POPC-拉坦前列素制备为含有1mg/mL拉坦前列素的无菌溶液,并将0.5mL各溶液储存在2~8℃的小瓶中。
使用前,将试验药物在室温下加热至少30分钟。使用无菌技术从储存瓶中直接填充至定量注射器。
实验方案
在研究中使用了24只8~10周龄的雄性Lewis大鼠(Rattus norvegicus)。研究开始时大鼠的体重在250~300克之间。
在给药前至少一天,每六只大鼠作为四组中的一组,每组均设计成达到相似的平均体重。将这四组按照实施例3中的方法进行处理,如上面的表1所示。A-D组的动物在注射前和注射后每周称重。
将大鼠麻醉,用30号针头将药物注射到每只动物右眼眶的球后间隙中。左眼眶作为对侧对照。所有大鼠均从麻醉中醒来,无并发症。
对所有24只大鼠的每只眼睛注射前和注射后3周测量眼内压(IOP)。
在第21天,注射后3周,用4%多聚甲醛心脏灌注对大鼠实施安乐死。灌注后,将每只大鼠斩首,并将头部置于10%福尔马林溶液中放置2天。用解剖刀小心地将每个眼眶骨膜下剥离,并放回10%福尔马林溶液中放置1天(每组3只动物,共6个眼眶),其余眼眶放回10%福尔马林溶液中放置2周。切除的眼眶用石蜡包埋,并切成5μm切片。由受过训练的眼部病理学家和第二位观察者以盲法对以用苏木精-伊红染色的锥内脂肪和视神经为中心的切片进行组织学检查。
样本分析
对切片进行视神经损伤、眼眶脂肪萎缩、脂肪细胞密度和细胞毒性的组织学检查,例如脂肪细胞溶解、血管损伤和修复、中性粒细胞炎症、局部中性粒细胞溶解、出血、脂质湖形成和新血管形成。
在光学显微镜下检查眼眶切片的锥内脂肪,并扫描图像用于进一步分析。切割锥内脂肪区域,并使用ImageJ软件(Rasband W.S.,美国国立卫生研究院,贝塞斯达,马里兰州)对脂肪细胞进行计数,以使用先前描述的方法确定脂肪细胞密度。参见Park等,2011,Jpn.J.Ophthalmol.55:22-27。计算锥内脂肪区域中脂肪细胞的密度,并通过双尾学生t检验进行组间比较。
利用MATLAB实现的基于最大稳定极值区域的分割算法(Mathworks,Natick,MA),分析锥内脂肪光切割图像中脂肪细胞大小的异质性。使用名为VL_FEAT的最大稳定极值区域算法来寻找参数δ=1、最大变化=0.2、最小差异=0.5的极值区域。尺寸小于10像素或偏心率大于0.9的最大稳定极值区域确定的区域作为假阳性丢弃。计算每幅图像的平均分割的脂肪细胞大小,并使用学生t检验对各组进行比较。
结果
在为期三周的实验方案中,未处理(未注射)对照眼的眼压保持稳定,眼压无显著变化。如图5A所示。眼球后注射EggPC-拉坦前列素、POPC-拉坦前列素和拉坦前列素后的眼压也保持了三周的稳定。如图5B所示。注射DCA的大鼠显示眼压显著增加。参见Id。
定量图像分析显示,在注射POPC-拉坦前列素、拉坦前列素和DCA后,眼球球后脂肪复合物的面积显著增加,但在注射EggPC-拉坦前列素后没有增加。如下表3所示。

不受理论限制,认为眼球球后脂肪复合物面积的增加是由于眼外肌体积的增加同时脂肪细胞、成纤维细胞或T细胞的减少所致。注射不同制剂后面积的不同变化可能是由于肌肉和脂肪组织的重塑以不同的速率发生。
定量图像分析还显示所有注射的眼睛的脂肪细胞密度显著增加。结果如下表4所示。

在4个试验组中,注射POPC-拉坦前列素的组增长百分率最高(64%)。
通过图像分析也获得绝对脂肪细胞计数。结果如下表5所示。

与任何其他试验相比,POPC-拉坦前列素的注射引起的脂肪细胞计数减少最多(68%)。
进一步的统计分析表明,POPC-拉坦前列素引起的脂肪细胞减少与任何其他治疗引起的减少显著不同。如下表6所示。

眼眶组织的组织病理学检查显示,无论是注射的眼眶还是未注射的对照组,均未见炎性细胞浸润。定性光学显微镜分析表明,在未注射的对照标本中,眼眶脂肪未受干扰。如图6E所示。
相比之下,在注射的眼睛中可以观察到小的、不规则形状的、碎片状的脂肪细胞,每单位面积脂肪细胞密度高。如图6A~图6D所示。这一观察表明锥内脂肪萎缩。
对注射眼睛的标本进行计算机辅助的形态学分析表明,与单纯对照组相比(例如图7E所示),眶内脂肪萎缩,锥内间隙脂肪细胞密度和异质性显著增加(如图7A~图7D所示)。
总之,与未注射的对侧对照眼眶相比,眼眶注射EggPC-拉坦前列素、POPC-拉坦前列素、拉坦前列素和DCA导致密集堆积在锥内空间中的脂肪细胞之间的异质性增加。令人意想不到的是,在为期7天的研究(实施例3)中,EggPC-拉坦前列素在减少脂肪细胞数量方面比POPC-拉坦前列素更有效,然而,在为期21天的研究(实施例4)显示了相反的结果,即POPC-拉坦前列素比EggPC-拉坦前列素更有效,尽管这两种包封形式的拉坦前列素之间有相似之处。
实施例5:脂肪性眼睑(steatoblepharon)治疗的临床试验
在一个中心招募了20名受试者。每名受试者都有临床上显著的下眼睑脂肪变性,即眼袋。
将120~240μg POPC-拉坦前列素(1mg/mL拉坦前列素)单次注射入较严重的眼睛的下眼睑。向相对不严重的眼睑注射相同体积的溶液,所述溶液除不含拉坦前列素外具有与对侧注射的溶液相同的成分。
对下眼睑注射POPC-拉坦前列素的安全性和耐受性进行为期12周的分析。在同一时期检测POPC-拉坦前列素的药代动力学。
其他实施例

1.一种减少体内脂肪沉积物的方法,所述方法包括将脂肪沉积物与有效量的包封在脂质体中的拉坦前列素的组合物接触,其中所述脂质体仅由蛋黄磷脂酰胆碱或棕榈酰磷脂酰胆碱构成,所述脂肪沉积物为皮下脂肪沉积物或内脏脂肪沉积物,有效量的组合物使脂肪沉积物中脂肪细胞的数量减少5%~90%。
2.根据权利要求1所述的方法,其中,所述脂肪沉积物中脂肪细胞的数量减少10%~90%。
3.根据权利要求2所述的方法,其中,所述脂肪沉积物中脂肪细胞的数量减少35%~90%。
4.根据权利要求1所述的方法,其中所述接触步骤为将所述组合物直接注射到所述脂肪沉积物中。
5.根权利要求4所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
6.根权利要求5所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
7.根据权利要求4所述的方法,其中,每0.5cm3~1.0cm3的脂肪沉积物,注射所述组合物的体积为25μL~75μL。
8.根据权利要求1所述的方法,其中所述接触步骤为将所述组合物注射到与脂肪沉积物相邻的胞间隙中。
9.根据权利要求8所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
10.根据权利要求9所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
11.根据权利要求8所述的方法,每0.5cm3~1.0cm3的脂肪沉积物,注射所述组合物的体积为25μL~75μL。
12.根据权利要求1所述的方法,其中,所述脂肪沉积物是皮下脂肪沉积物,并且所述接触步骤为将所述组合物皮下注射到所述皮下脂肪沉积物中或其附近。
13.根据权利要求12所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
14.根据权利要求13所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
15.根据权利要求12所述的方法,其中每0.5cm3~1.0cm3的脂肪沉积物,注射所述组合物的体积为25μL~75μL。
16.一种用于治疗受试者脂肪性眼睑的方法,所述方法包括确定需要治疗脂肪性眼睑的受试者,并将有效量的包封在脂质体中的拉坦前列素组合物注射到所述受试者眼睑的脂肪沉积物中,其中所述脂质体仅由蛋黄磷脂酰胆碱或棕榈酰磷脂酰胆碱构成,有效量的组合物使脂肪沉积物中脂肪细胞的数量减少5%~90%。
17.根据权利要求16所述的方法,其中,所述脂肪沉积物中脂肪细胞的数量减少10%~90%。
18.根据权利要求17所述的方法,其中,所述脂肪沉积物中脂肪细胞的数量减少35%~90%。
19.根据权利要求16所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
20.根据权利要求19所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
21.根据权利要求16所述的方法,其中每0.5cm3~1.0cm3的脂肪沉积物,注射所述组合物的体积为25μL~75μL。
22.一种用于治疗受试者突眼症的方法,所述方法包括确定需要治疗突眼症的受试者,并将有效量的包封在脂质体中的拉坦前列素组合物注射到所述受试者的锥内空间,其中所述脂质体仅由蛋黄磷脂酰胆碱或棕榈酰磷脂酰胆碱构成,有效量的组合物使锥内空间的脂肪细胞数量至少减少35%。
23.根据权利要求22所述的方法,其中,所述突眼症是甲状腺相关眼病的结果。
24.根据权利要求23所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
25.根据权利要求24所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
26.根据权利要求22所述的方法,其中所述受试者患有格雷夫斯病或桥本甲状腺炎。
27.根据权利要求26所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
28.根据权利要求27所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
29.一种用于治疗与过量的上呼吸道脂肪相关的阻塞性睡眠呼吸暂停的方法,所述方法包括确定需要治疗阻塞性睡眠呼吸暂停的受试者,并将有效量的包封在脂质体中的拉坦前列素组合物注射到受试者的上呼吸道脂肪沉积物中,其中所述脂质体仅由蛋黄磷脂酰胆碱或棕榈酰磷脂酰胆碱构成,有效量的组合物使上呼吸道脂肪沉积物中脂肪细胞的数量减少5%~90%。
30.根据权利要求29所述的方法,其中,所述上呼吸道脂肪沉积物位于上颚、舌头或咽部。
31.根据权利要求29所述的方法,其中,所述脂肪沉积物中的脂肪细胞的数量减少10%~90%。
32.根据权利要求30所述的方法,其中,所述脂肪沉积物中的脂肪细胞的数量减少35%~90%。
33.根据权利要求29所述的方法,其中所述组合物中拉坦前列素的浓度为0.5mg/mL~3.0mg/mL。
34.根据权利要求33所述的方法,其中拉坦前列素的浓度为1.0mg/mL~2.3mg/mL。
