Abstract

Description

巴瑞替尼是属于Janus激酶(JAK)抑制剂的药理学类别的获批药物。Janus激酶是四(4)种蛋白酪氨酸激酶(JAK1,JAK2,JAK3和酪氨酸激酶2 [TYK2])的家族,其在细胞因子信号转导中起作用。巴瑞替尼证实了对于JAK1和JAK2的选择性和抑制作用,伴随针对JAK3或TYK2抑制的较低效力(Fridman JS等人,“Selective inhibition of JAK1 and JAK2 isefficacious in rodent models of arthritis: preclinical characterization ofINCB028050,” J Immunol. 2010;184(9):5298-5307.)。在分离的酶测定中,巴瑞替尼抑制JAK1、JAK2、TYK2和JAK3的活性,其半数最大抑制浓度值分别为5.9、5.7、53和>400 nM(参见同上)。

奥贝胆酸(以商标OCALIVA®出售)被批准作为对UDCA具有不充分应答的患者中UDCA的附加药物,或作为对UDCA不耐受的患者的单一疗法。(Nevens等人,POISE StudyGroup,“A Placebo-Controlled Trial of obeticholic Acid in Primary BiliaryCholangitis,” N Engl J Med. 2016 Aug 18;375(7):631-43)。然而,UDCA和OCALIVA®均无法有效地解决PBC的其它主要症状,例如瘙痒或疲劳。事实上,奥贝胆酸(OCALIVA®)与瘙痒症增加相关,并且达到和维持有效剂量受到这种不良作用的限制(Nevens等人2016)。此外,奥贝胆酸的使用似乎仅在55%左右的患者群体中是有效的。
因此,仍存在部分患者群体,其不响应UDCA,并且无法通过奥贝胆酸(或单独或与UDCA组合)充分地治疗。更进一步地,与PBC相关的瘙痒、疲劳和疼痛无法通过这些当前药物进行治疗。另外,UDCA和奥贝胆酸均通过调节肝脏中胆汁酸的产生和处置来起作用,但无法治疗潜在状况—例如,引起T淋巴细胞介导的对小叶内胆管的攻击的自身免疫应答。出于这些和其它原因,需要用于PBC的新治疗。相应地,尽管在与奥贝胆酸的批准相关的领域中有所进展,显然仍需要用于PBC的新治疗。

当前不存在用于PSC的已知有效治疗;相反,患有PSC的患者可能最终需要肝脏移植。UDCA已尝试作为用于PSC的治疗,但大多数患者中仍发生肝脏中的主要胆管闭塞。(Stiehl A等人, “Development of dominant bile duct stenoses in patients withprimary sclerosing cholangitis treated with ursodeoxycholic acid: outcomeafter endoscopic treatment,” J Hepatol.;36: 151–56,2002.)因此,存在关于新的、批准的和临床上有效的PSC治疗的未满足的医学需要。
至少由于本文提供的原因,存在用于PBC和PSC两者的改善治疗的未满足需要。此类治疗应该解决PBC和PSC的自身免疫原因,且优选地预防或治疗引起肝脏中的PSC或PBC的自身免疫应答。另外,用于PBC的此类治疗应该治疗对UDCA的无应答者。进一步地,(尤其是对于PBC),治疗应该解决与这些状况相关的瘙痒(瘙痒症)、疲劳和/或疼痛。与所有治疗性处理一样,安全性和毒性仍然是一种限制,因此,任何改善的治疗都不得伴随着不可接受的安全性和毒性概况,并且提供临床益处(尤其是对于其中不存在批准的治疗的PSC)。本发明提供了用于治疗PBC和/或PSC的治疗性处理,其克服了上文认识到的一个或多个挑战。

巴瑞替尼是已知的药物,其在美国和欧洲(和其它国家)批准用于治疗类风湿性关节炎,并且以商标OLUMIANT®商购可得。在某些辖区,OLUMIANT®可以丸剂形式获得,其中所述丸剂包括指定量的巴瑞替尼和下述赋形剂:交联羧甲基纤维素钠、硬脂酸镁、甘露糖醇、微晶纤维素、三氧化二铁、卵磷脂(大豆)、聚乙二醇、聚乙烯醇、滑石和二氧化钛。在本发明的优选实施方案中,用于治疗患者的巴瑞替尼的量通过向患者给予一个或多个OLUMIANT®丸剂来施用。当然,也可以使用巴瑞替尼的其它剂型、药物组合物等。已进行了巴瑞替尼对于肝病患者的I期研究。

在一些本文的实施方案中,用巴瑞替尼治疗的患者在接受巴瑞替尼之前已接受了UDCA。在一些实施方案中,患者在开始用巴瑞替尼的疗法之前已接受了UCDA共52周或更长时间。在其它实施方案中,患者在开始用巴瑞替尼的疗法之前已接受了UDCA至少12周。在进一步的实施方案中,接受用于PBC或PSC的巴瑞替尼疗法的患者是对UDCA具有不充分应答或不耐受的患者。为了确定患者是否对UDCA具有不充分应答或不耐受,如通过下述两(2)篇文章中教导的方法概述的,对患者进行评估并确定为具有肝脏生物化学中的改善缺乏:

还应该指出的是,在美国,巴瑞替尼的标签指示下述:
肝酶升高 – 与安慰剂相比,用OLUMIANT的治疗与肝酶升高的发生率增加相关。在OLUMIANT临床试验中的患者中,对于ALT和AST均观察到至大于或等于正常上限(ULN)的5x、或者大于或等于10x的增加。在基线时以及其后根据常规患者管理进行估计。推荐肝酶升高原因的及时调查,以鉴定药物诱发性肝损伤的潜在情况。如果观察到ALT或AST中的增加并且怀疑药物诱发性肝损伤,则中断OLUMIANT直至排除该诊断[参见不良反应(6.1)]。
然而,尽管这种肝酶升高,巴瑞替尼可能是用于PBC和/或PSC的有效治疗。
体内研究
将患者分成由双盲安慰剂和巴瑞替尼疗法组组成的治疗组。巴瑞替尼疗法组施用一定量的巴瑞替尼(例如,以本文概述的方式的4 mg丸剂或片剂),而安慰剂组施用仅具有安慰剂的丸剂(例如,安慰剂的4 mg丸剂或片剂)。
研究中一般存在3个时期,例如“筛查期”、“治疗期”和“随访期”。所有患者都经历这些时期中的每一个。
在治疗开始前发生的筛查期内,患者就下述中的一种或多种进行评估:胆红素水平、ALP、瘙痒NRS评分、疲劳NRS评分和/或其它测量。该筛查期可能构成“第0天”。这些评估在患者接受治疗之前完成。
在治疗期过程中,患者给予安慰剂或巴瑞替尼(取决于他们所在的研究组)。在一些情形下,随机化的患者将在诊所服用第一剂量的调查产品,并且在剂量后15分钟和1小时抽取药代动力学(PK)样品。巴瑞替尼可能每天一次,共12周。在治疗期过程中,在预定就诊时获得临床评估和实验室样品,包括另外的PK采样。在治疗期过程中,除随机化治疗之外,患者还将维持其用于PBC的通常用药方案,包括由皮质类固醇和/或止痒疗法组成的背景疗法。在治疗期过程中的任何时间都可能发生患者的测量(例如上述那些测量)的重新评估。
在治疗期(其可能持续例如12周)后,患者进入随访期。在该时期过程中,将重新评估(或进一步重新评估)患者的测量。
Claims (30)
1.一种治疗需要治疗原发性胆汁性胆管炎(PBC)的患者的方法,其包括向所述患者施用一定量的巴瑞替尼或其药物制剂。
2.一种治疗需要治疗原发性硬化性胆管炎(PSC)的患者的方法,其包括向所述患者施用一定量的巴瑞替尼或其药物制剂。
3.权利要求1或2的方法,其中所述一定量的巴瑞替尼是经口施用的。
4.权利要求1-3中任一项的方法,其中所述患者先前对熊去氧胆酸具有不充分应答或不耐受。

在所述施用巴瑞替尼的步骤之后,重新评估患者的瘙痒NRS评分。

在所述施用巴瑞替尼的步骤之后,重新评估患者的疲劳NRS评分。

在第0天评估患者的ALP;和
在所述施用巴瑞替尼的步骤之后,重新评估患者的ALP。
8.权利要求5-7中任一项的方法,其中重新评估患者的瘙痒NRS评分或疲劳NRS评分或ALP的步骤在第12周期间或之后发生。
9.权利要求1-8中任一项的方法,其中所述施用巴瑞替尼的步骤包括以日剂量向患者施用巴瑞替尼。
10.权利要求9的方法,其中所述日剂量为4 mg。
11.巴瑞替尼在制备用于治疗PBC的药物中的用途。
12.巴瑞替尼在制备用于治疗PSC的药物中的用途。
