Abstract

转染期间重排(RET)的受体酪氨酸激酶(RTK)以及神经胶质细胞系衍生的神经营养因子(GDNF)和GDNF家族受体-α(GFRα)是多种神经、神经内分泌和泌尿生殖组织类型的发育、成熟和维持所必需的。然而,越来越多的证据表明,RET的异常激活是许多实体肿瘤的肿瘤生长和增殖的关键驱动因素(Mulligan LM.,Nat.Rev.Cancer.14:173-186(2014))。
已经在1%-2%的NSCLC中发现了致癌性RET重排(Lipson,D.等人,Nat.Med.18:382-384(2012);Takeuchi,K.等人,Nat.Med.18:378-381(2012);Stransky,N.等人,Nat.Commun.5:4846(2014))。致癌性RET重排产生促进肿瘤发生的组成性活性激酶。与间变性淋巴瘤激酶(ALK)和c-ros癌基因(ROS)1重排的NSCLC一样,RET重排的NSCLC通常具有腺癌组织学(尽管偶尔为鳞状的),并且发生在年轻的非吸烟患者中。本文所述的(1S,4R)-N-((S)-1-(6-(4-氟-1H-吡唑-1-基)吡啶-3-基)乙基)-1-甲氧基-4-(4-甲基-6-((5-甲基-1H-吡唑-3-基)氨基)嘧啶-2-基)环己烷甲酰胺(化合物1)为RET激酶和致癌性RET突变体的有效且选择性抑制剂。在细胞系统中,化合物1以低纳摩尔效力抑制RET致癌性突变体的激酶活性。化合物1的体内剂量依赖性抗肿瘤功效已在几种RET驱动的模型中得到证实。值得注意的是,在首次人体试验中,化合物1在具有RET改变的NSCLC患者中诱导了持久的临床响应而没有明显的脱靶毒性(Subbiah,V.等人Cancer Disc(2018年4月15日早期在线发布))。目前正在研究化合物1用于治疗患有RET驱动的恶性肿瘤(诸如甲状腺癌、非小细胞肺癌(NSCLC)和其他晚期实体肿瘤)的患者。

在EGFR突变型患者中,由RET融合促进的EGFR TKI抗性类似于由MET扩增促进的旁路跟踪抗性。在MET扩增的情况下,临床前和临床证据均显示出通过抑制EGFR和MET的强烈响应(Engleman,J.A.等人,Science316:1039-43(2007);Gainor,J.F.等人,J.Thorac.Oncol.11(7):e83-e85(2016);Ahn,M.等人,J.Thorac.Oncol.12(11S2):pS1768(2017))。

本领域普通技术人员可例如使用选自以下的方法来确定受试者是否具有RET融合:基于杂交的方法、基于扩增的方法、微阵列分析、流式细胞术分析、DNA测序、二代测序(NGS)、引物延伸、PCR、原位杂交、荧光原位杂交、斑点印迹和Southern印迹。
为了检测融合,可从受试者收集原发性肿瘤样品。对样品进行处理,使用本领域已知的技术分离核酸,然后使用本领域已知的方法对核酸进行测序。然后将序列映射到各个外显子,并定量转录表达的量度(诸如RPKM,或每千碱基读数每百万映射读数)。原始序列和外显子阵列数据可从诸如TCGA、ICGC和NCBI基因表达综合数据库(GEO)的来源获得。对于给定的样品,用基因标识符信息注释单独的外显子坐标,并且标记属于激酶结构域的外显子。然后将外显子水平在所有肿瘤样品中进行z-得分标准化。
接下来,鉴定以与3'外显子显著不同的水平表达5'外显子的基因。滑动框用于识别单个样品中的断点。具体地,在每次迭代中,递增的断点将基因分成5'和3'区域,并且t统计量用于测量两个区域之间的表达差异(如果有的话)。选择具有最大t统计量的断点作为可能的融合断点。如本文所用,“断点”是融合两个不同基因的边界。有时将其称为“融合点”。5'与3'之间外显子表达差异最大的位置是推断的融合断点。可以此种方式快速分析成千上万个肿瘤样品,从而生成融合候选物列表(按t统计量排序)。然后可对高级候选物进行验证,并通过检查原始RNA-seq数据集并识别支持融合的嵌合对和/或拆分读数来识别融合伴侣。然后可如下所述通过实验确认候选融合。

实施例
试剂和抗体
阿法替尼、奥西替尼和卡博替尼购自Selleck Chemicals,并重悬于DMSO中。磷酸化EGFR(Y1068)、EGFR、pBRAF(Ser445)、RET、pAKT(Ser473)、AKT、pMEK1/2(Ser217/221)、MEK1/2、pERK1/2(Thr202/204)、ERK1/2和肌动蛋白抗体购自Cell Signaling Technology。pRET(Y1062)抗体获自Abcam。
RT-PCR和测序

细胞培养
PC9和MGH134细胞系在本领域中已知(Hata,A.N.等人,Nat.Med.22(3):262-69(2016)),将细胞在补充有10%FBS的RPMI1640(Life Technologies)中培养。将MGH845-1细胞另外在150nM奥西替尼中培养。对所有细胞进行常规测试,并确认无支原体污染。
表达CCDC6-RET的细胞系的生成
通过GenScript合成CCDC6-RET融合构建体,并使用ViraPower慢病毒定向TOPO表达试剂盒(Life Technologies)连接到pLENTI6/V5-D-TOPO载体中。通过转染pLENTI6构建体并将质粒包装到293FT细胞(Life Technologies)中来生成慢病毒。病毒的产生、收集和感染均按照制造商的方案完成。将转导的细胞在杀稻瘟菌素(10mg/mL-20mg/mL)中选择一周。
细胞活力测定
对于药物剂量响应测定,在添加药物之前24小时将细胞接种到96孔板中。使用标准方案,在添加药物后72-120小时通过CellTiter-Glo测定法(Promega)确定细胞增殖。对于时程实验,以相同的方式接种和注药多个板,并在指定的时间点将板在-80℃下冷冻;实验中的所有板同时用CellTiter-Glo培养。用SpectraMax i3x多模式微板读数仪(Molecular Devices)测量发光。
实施例1:奥西替尼AR活检
为了更好地表征所获得的对奥西替尼的抗性(AR),进行奥西替尼AR活检的单中心队列。在IRB批准的方案下,通过SNaPshot或Foundation One二代测序(NGS)进行奥西替尼AR活检并且通过Guardant360 NGS来测定血浆。具体地,通过组织NGS(n=22)、血浆NGS(n=9)或两者(n=10)来查询用奥西替尼治疗T790M+疾病的41名EGFR突变型患者。在32个组织样品中的2个中,观察到SCLC转化。在5个组织样品和5个血浆样品中(所有T790M均为顺式),发现EGFR C797S。另外,在7个组织和3个血浆样品中观察到MET扩增。在32个组织样品中的2个中发现BRAF重排,而在32个组织样品中的1个以及19个血浆样品中的1个中发现CCDC6-RET重排。表现出CCDC6-RET重排的组织和血浆样品来自不同的供体,这表明RET重排在具有对奥西替尼的AR的EGFR突变型患者中是低频率但反复出现的观察结果。
实施例2:患者研究
患有del 19 EGFR突变型晚期NSCLC的60岁女性接受一线阿法替尼(一年)、获得T790M并用奥西替尼治疗(18个月)。然后,她经历肺活检,通过SFA显示出CCDC6-RET融合。基线组织不足以进行固体融合测定(SFA),但RET荧光原位杂交(FISH)为阴性,这表明获得CCDC6-RET融合。为患者编写了用于用奥西替尼加化合物1治疗的个体患者研究新药(IND)方案。她开始每天80mg的奥西替尼和每天200mg的化合物1,然后在治疗2周后将化合物1增加到300mg。在治疗开始后几天内,她的呼吸困难得到改善。8周后的扫描显示出显著响应,其中RECIST肿瘤缩小78%(图1)。组合具有良好的耐受性,仅具有1级毒性,包括疲劳、白细胞减少、高血压、口干和转氨酶升高。截至2018年9月24日,治疗仍在进行中。
接受一线顺铂/培美曲塞(pemetrexed)、二线阿法替尼(一年)的患有del 19 EGFR突变型晚期NSCLC的44岁男性经历增长的肺部病变的支气管镜活检,通过SFA显示出CCDC6-RET融合。基线组织不可用于RET测试。他接受每天150mg的厄洛替尼组合每天60mg的标签外卡博替尼治疗。一个月后的扫描显示疾病稳定(RECIST 1.1),但2.5个月后的后续扫描显示疾病进展并提示治疗中止。患者有1级腹泻、皮疹和AST升高
实施例3:EGFR突变型NSCLC细胞系中的CCDC6-RET表达赋予对EGFR抑制剂的抗性。
为了确定CCDC6-RET表达是否足以引起获得性耐药,通过慢病毒感染PC9(EGFRdel 19)和MGH134(EGFR L858R/T790M)细胞来产生表达CCDC6-RET融合的细胞系模型(图2)。
在EGFR抑制剂不存在的情况下,表达CCDC6-RET的细胞与亲代细胞生长相似。当用奥西替尼(OSI)处理时,与经历了细胞活力净下降的亲代细胞(EV)相比,PC9CCDC6-RET和MGH134CCDC6-RET细胞继续增殖(图3)。在奥西替尼处理的情况下,CCDC6-RET表达细胞的增殖速率降低,这表明RET激活虽然足以驱动获得性抗性,但不能完全补偿EGFR信号转导丢失。
还检查了CCDC6-RET表达对PC9和MGH134细胞中下游信号通路激活的影响。与不表达可检测的RET蛋白的亲代细胞相比,在PC9CCDC6-RET和MGH134CCDC6-RET细胞中均检测到磷酸化RET(图4A至图4B),单独的CCDC6-RET表达并不导致基线处下游MAPK(磷酸化ERK1/2)或PI3K(磷酸化AKT)信号转导激活的增加;然而,在PC9CCDC6-RET和MGH134CCDC6-RET细胞中,在阿法替尼或奥西替尼存在的情况下,RET、ERK1/2和AKT磷酸化均得以保留(图4A至图4B)。因此,CCDC6-RET融合的表达可在EGFR突变型NSCLC中赋予对EGFR抑制剂的抗性。
实施例4:通过EGFR加RET可克服由CCDC6-RET表达引起的获得性抗性。
在不存在或存在EGFR抑制剂的情况下,用化合物1处理如上产生的PC9CCDC6-RET细胞。单独用化合物1的处理抑制了RET磷酸化,但是没有降低下游ERK或AKT磷酸化(图4A)。用化合物1与奥西替尼或阿法替尼的组合治疗完全抑制磷酸化ERK和磷酸化AKT,并将细胞活力降低至与用EGFR TKI处理的亲代细胞相似的水平(图4C)。在MGH134CCDC6-RET细胞中观察到了相似的结果(图4B、图5C)。另外,PC9CCDC6-RET和MGH134CCDC6-RET细胞对EGFR TKI+卡博替尼(具有RET活性的多激酶抑制剂)敏感(图4B、图5A至图5C)。综上所述,这些数据表明,通过双重EGFR加RET阻断可克服由CCDC6-RET融合产生的获得性抗性。
实施例5:化合物1和奥西替尼用于RET融合介导的对EGFR抑制的抗性的转移性非小细胞肺癌的研究(具有RET介导的对EGFR抑制的抗性的转移性NSCLC的组合研究)
此研究是开放标签的1/2期研究,被设计来评估有效且选择性RET抑制剂化合物1与第三代EGFR抑制剂奥西替尼组合在NSCLC患者中的安全性、耐受性、抗肿瘤活性、PK和药效动力学,所述患者已发展出与奥西替尼抗性相关的RET融合。
进行剂量递增研究以评估化合物1与奥西替尼的组合的安全性和耐受性,并确定最大耐受剂量(MTD)和/或推荐的2期剂量(RP2D)。通过任何不良事件的类型、频率、严重程度、时间和与研究药物的关系,严重不良事件,生命体征变化,ECG和安全性实验室测试来评估化合物1与奥西替尼组合治疗的总体安全特性。
研究还估计了患有在奥西替尼期间或之后进展的转移性、RET融合阳性非小细胞肺癌的患者中化合物1与奥西替尼组合治疗的总响应率(ORR)。ORR被定义为根据RECIST1.1达到确认的完全响应(CR)或部分响应(PR)的患者比例。
研究还评估了抗癌活性的另外的量度,包括响应持续时间(DOR)、疾病控制率(DCR)、临床受益率(CBR)、无进展生存期(PFS)和总体生存期(OS)。另外,研究将化合物1PK参数与安全性终点和抗肿瘤活性相关联;进一步表征化合物1与奥西替尼组合的安全性和耐受性;并评估生活质量(QoL)和症状严重程度的量度变化。
对于另外的措施,DOR被定义为从首次满足CR或PR的标准的时间到客观地记录具有确认CR或PR的患者的进行性疾病(PD)的第一日期的月数;DCR被定义为经历根据RECIST1.1的稳定疾病(SD)、部分响应(PR)或完全响应(CR);根据RECIST 1.1的部分响应(PR)或完全响应(CR)的最佳响应的患者比例;并且PFS被定义为从研究治疗的第一剂到由任何原因引起的PD或死亡中的较早者的月数。
化合物1的PK参数包括:人群衍生的估计值,包括最大血浆药物浓度(Cmax)、0时间至给药后24小时的血浆浓度与时间曲线下面积(AUC0-24)、稳态下给药后24小时的血浆药物浓度(C24);以及任何AE的类型、频率、严重程度、时间和与研究药物的关系,严重不良事件(SAE),生命体征变化,ECG和安全性实验室测试。
研究包括标准的3+3剂量递增部分,以鉴定化合物1与奥西替尼组合给与时的MTD和/或RP2D,之后进行扩展期以评估ORR和其他临床活性度量。所有参加1期研究部分的患者都必须以80mg/天的批准起始剂量开始进行奥西替尼治疗。在2期研究部分中,如果需要的话,在先前的奥西替尼治疗下出现毒性的患者可以较低的起始剂量开始服用奥西替尼。以200mg、300mg和400mg的剂量递增来评估化合物1的剂量水平。处于扩展期的患者接受根据剂量递增确定的RP2D。扩展部分遵循两阶段设计,其中最初治疗10名患者。如果此第一阶段≥2/10的患者出现客观的肿瘤响应,则第二阶段招募另外的23名患者,在扩展期治疗总共33名患者。
对于研究资格,使用在服用EGFR抑制剂的情况下疾病进展时(或之后)采集的肿瘤或血液样品通过局部或中心评估来确定RET融合状态。
研究治疗剂化合物1和奥西替尼以28天周期通过每天口服施用来给予。剂量修改是根据基于所观察到的毒性的特定标准进行的。
患者可继续接受研究治疗剂,直到因毒性、不依从、撤回同意、死亡或研究结束而被排除。经历RECIST 1.1定义的疾病进展但在治疗研究者看来仍继续经历临床上益处的患者可在获得批准的情况下继续研究治疗。如果化合物1被永久停用,则认为患者已完成研究治疗期;并且在生存随访期间接受其他抗癌治疗(如果合适的话,包括奥西替尼)作为后续治疗。需要永久停用奥西替尼的患者可在批准后继续化合物1的单一疗法。
所有研究访视旨在在门诊病人的基础上进行,但是根据需要可在住院病人的基础上进行。前两年每8周进行一次疾病评估,然后在此之后每12周进行一次。停止研究治疗后,无进行性疾病记录的患者继续接受疾病评估,直至记录到进行性疾病、另一种抗肿瘤治疗的开始、死亡或研究结束。根据RECIST 1.1评估肿瘤响应。在停止研究治疗后30天还会与患者联系以评估安全性,并继续进行生存随访直至死亡或研究结束。
患者人群包括以下参与者:签署知情同意书时年龄≥18岁;患有经病理证实、明确确诊的转移性EGFR突变型NSCLC;具有至少一个RECIST 1.1可评估的靶病变;仅针对1期:在用任何第二代或第三代EGFR抑制剂TKI进行先前治疗期间或之后具有放射学记录的疾病进展;仅针对2期:在用奥西替尼进行先前治疗期间或之后具有放射学记录的疾病进展;使用如上所述的样品通过肿瘤组织或血液中的循环肿瘤核酸的局部或中央测试检测到具有致癌性RET融合(对于局部确定RET状态资格的患者,患者还必须同意提交血液和组织样品,以通过中央测试回顾性确认RET状态);愿意提供存档肿瘤组织(如果在用奥西替尼的先前治疗期间或之后疾病进展后获得的样品可用),或者如果没有合适的存档肿瘤组织可用,则愿意接受预处理活检,并且研究者认为所述预处理活检安全且在医学上可行(如果在基线影像学成像后进行,则从非靶病变处采集预处理活检);以及东部肿瘤协作组(ECOG)的体能状态(PS)为0-1。
患者人群不包括以下参与者:具有任何另外的已知主要驱动因素改变(原始EGFR突变和RET融合除外),包括但不限于ALK、ROS1、MET和BRAF的可靶向突变;在参加之前28天内有间质性肺病(ILD)或间质性肺炎、需要类固醇治疗的放射性肺炎的任何既往病史或有临床活动性ILD的任何证据;患有中枢神经系统(CNS)转移或原发性CNS肿瘤,其与进行性神经系统症状相关或需要增加皮质类固醇剂量以控制CNS疾病(如果患者需要皮质类固醇治疗CNS疾病,则剂量必须在C1D1之前的2周内稳定);在6个月内接受过任何抗PD-1/PDL-1/CTLA治疗并且在第一剂量的研究药物(不包括可持续不间断而无需冲洗的先前的奥西替尼)之前14天或5个半衰期(以较短者为准)内接受过任何其他抗癌治疗(包括全身治疗和放疗);在参加前6个月内接受过超过30Gy的肺部放射治疗;QTcF>480毫秒、具有QT间期延长综合征或尖端扭转型室速的病史或具有QT间期延长综合征的家族病史;或在第一剂量的研究药物之前14天内具有以下中的任一项:
a.血小板计数<75x109/L;
b.绝对中性粒细胞计数(ANC)<1.0x109/L;
c.血红蛋白<9.0g/dL(红细胞输注和促红细胞生成素可用于达到至少9.0g/dL,但必须在第一剂量的研究药物之前至少2周施用;
d.如果不存在肝转移,则天冬氨酸转氨酶(AST)或丙氨酸转氨酶(ALT)>3×正常上限(ULN);如果存在肝转移,则>5×ULN;
f.估计的(Cockroft-Gault公式)或测得的肌酐清除率<40mL/min;或
g.总血清磷>5.5mg/dL。
1期剂量递增阶段中包括多达18名患者的样本量。参加剂量递增的参与者总数取决于所观察到的安全特性,所述安全特性决定了每个剂量组的参与者数以及确认推荐的2期剂量(RP2D)所需的组数。
对于2期扩展期,使用西蒙的两阶段设计(Simon,1989),通过假定无效假设响应率为5%且替代的响应率为25%(其中单边α为0.025且功率为90%),阶段1的样本量为10名患者(总样本量的30%)。1期和2期的累计样本量为33。在1期,如果响应率不超过1/10(10%),则由于无法拒绝无效假设而停止试验。否则,试验继续至2期。如果在所有33名患者中至少有5名响应者,则拒绝无效假设。
对于33名患者,观察到至少一种AE以10%的频率发生的可能性>95%;观察到至少一种AE以20%的频率发生的可能性>99%。
对于分析人群,可评估响应的人群(REP)包括患有基线时可测量的疾病、接受至少一个剂量的每种研究治疗(化合物1和奥西替尼)且具有可评估的基线后肿瘤响应评估的所有患者。REP用于ORR、DCR和CBR的初步分析。安全人群(包括接受至少一个剂量的化合物1的所有患者)用于剩余的疗效终点和安全性。
对于REP,使用精确Clopper Pearson方法,给出了具有客观响应的患者的数量和百分比以及双边95%置信区间。基于同一方法,估计DCR和CBR以及双边95%置信区间。对于达到客观响应的患者,从首次满足CR/PR标准的时间到客观地记录到进行性疾病的第一日期来计算DOR。在最后一次响应评估时,对未经历记录的进行性疾病或死亡的响应者进行审查,并且使用Kaplan-Meier法估算中位数及其95%CI。
使用Kaplan-Meier法分析无进展生存期。如果患者未经历进行性疾病或死亡,则在最后一次响应评估时对患者进行审查。
安全性分析包括临床和实验室参数以及AE的数据总结。基于NCI CTCAE v 5.0,通过与研究药物的关系和严重程度总结了经历一种或多种AE的患者数量和百分比。不良事件使用监管活动医学词典(MedDRA)进行编码。使用描述性统计、通过相对于基线的治疗后变化以及具有临床显著异常的数据列表来总结实验室参数。使用描述性统计总结生命体征和ECG数据。用描述性统计将化合物1血浆浓度数据制成表格。化合物1的暴露参数与安全性终点和抗肿瘤活性相关。
Claims (42)
1.一种用于治疗有此需要的患者的EGFR突变型癌症的方法,所述方法包括向所述患者施用治疗有效量的至少一种RET抑制剂和治疗有效量的至少一种EGFR抑制剂。
2.如权利要求1所述的方法,其中所述至少一种RET抑制剂选自化合物1及其药学上可接受的盐。
3.如权利要求1所述的方法,其中所述至少一种RET抑制剂选自:阿来替尼、阿帕替尼、BOS172738(DS-5010)、卡博替尼(XL184)、多维替尼(TKI258)、GSK3179106、GSK3352589、乐伐替尼、LOXO-292、TPX-0046、SL-1001、尼达尼布、普纳替尼、司曲替尼(MGCD516)、索拉非尼、舒尼替尼、瑞格非尼(BAY 73-4506)、RXDX-105、凡德他尼、XL999以及前述中的任一种的药学上可接受的盐。
4.如权利要求1所述的方法,其中所述至少一种RET抑制剂为选择性RET抑制剂。
5.如权利要求1至4中任一项所述的方法,其中所述至少一种EGFR抑制剂为选择性EGFR抑制剂。
6.如权利要求1至4中任一项所述的方法,其中所述至少一种EGFR抑制剂为第三代EGFR抑制剂。
7.如权利要求1至4中任一项所述的方法,其中所述至少一种EGFR抑制剂选自奥西替尼及其药学上可接受的盐。
8.如权利要求1至7中任一项所述的方法,其中所述EGFR突变型癌症的特征在于至少一个选自T790M、C797S和L792H的EGFR突变。
9.如权利要求1至8中任一项所述的方法,其中所述EGFR突变型癌症的特征还在于至少一种RET融合。
10.如权利要求9所述的方法,其中所述EGFR突变型癌症的特征还在于CCDC6-RET融合。
11.如权利要求1至10中任一项所述的方法,其中所述EGFR突变型癌症为肺癌。
12.如权利要求11所述的方法,其中所述肺癌选自小细胞肺癌和非小细胞肺癌。
13.如权利要求1至12中任一项所述的方法,其中所述患者为人。
14.如权利要求1至13中任一项所述的方法,其中所述患者先前用至少一种EGFR抑制剂治疗。
15.如权利要求1至14中任一项所述的方法,其中所述患者获得对至少一种EGFR抑制剂的抗性。
16.如权利要求1、2和4至15中任一项所述的方法,其中:
所述至少一种RET抑制剂选自化合物1及其药学上可接受的盐;
所述至少一种RET抑制剂每天一次口服施用给所述患者;并且
所述治疗有效量的所述至少一种RET抑制剂为200mg至400mg的化合物1或重量当量的其药学上可接受的盐。
17.如权利要求16所述的方法,其中所述治疗有效量的所述至少一种RET抑制剂为200mg至300mg的化合物1或重量当量的其药学上可接受的盐。
18.如权利要求7至17中任一项所述的方法,其中
所述至少一种EGFR抑制剂选自奥西替尼及其药学上可接受的盐;
所述至少一种EGFR抑制剂每天一次口服施用给所述患者;并且
所述治疗有效量的所述至少一种EGFR抑制剂为80mg的奥西替尼或重量当量的其药学上可接受的盐。
19.一种组合疗法,所述组合疗法包含至少一种RET抑制剂和至少一种EGFR抑制剂。
20.如权利要求19所述的组合疗法,其中所述至少一种RET抑制剂选自化合物1及其药学上可接受的盐。
21.如权利要求19所述的组合疗法,其中所述至少一种RET抑制剂选自:阿来替尼、阿帕替尼、BOS172738(DS-5010)、卡博替尼(XL184)、多维替尼(TKI258)、GSK3179106、GSK3352589、乐伐替尼、LOXO-292、TPX-0046、SL-1001、尼达尼布、普纳替尼、司曲替尼(MGCD516)、索拉非尼、舒尼替尼、瑞格非尼(BAY 73-4506)、RXDX-105、凡德他尼、XL999以及前述中的任一种的药学上可接受的盐。

24.如权利要求19至22中任一项所述的组合疗法,其中所述至少一种EGFR抑制剂为第三代EGFR抑制剂。
25.如权利要求20所述的组合疗法,其中化合物1以200mg至400mg的量存在。
26.如权利要求20所述的组合疗法,其中化合物1以200mg至300mg的量存在。
27.如权利要求19至22、25或26中任一项所述的组合疗法,其中所述至少一种EGFR抑制剂选自奥西替尼及其药学上可接受的盐。
28.如权利要求27所述的组合疗法,其中奥西替尼以80mg的量存在。
29.一种用于治疗患有EGFR突变型癌症的患者的方法,所述方法包括:
(a)从所述患者获得生物样品;
(b)检测所述生物样品中至少一种RET融合的存在或不存在;以及
(c)如果检测到至少一种RET融合,则向所述患者施用组合疗法,其中所述组合疗法包含至少一种EGFR抑制剂和至少一种RET抑制剂。
30.如权利要求29述的方法,其中所述至少一种RET抑制剂选自化合物1及其药学上可接受的盐。
