背景技术
术后粘连是术后疼痛和其它手术并发症的重要原因。氧化应激和缺氧在粘连形成中起着重要作用。缺氧触发一系列反应,其最终导致粘连形成。粘连的发展是一个多步骤过程,包括巨噬细胞驱动的炎症反应、纤维状渗出物的形成、成纤维细胞(这些成纤维细胞被激活以形成肌成纤维细胞)的募集、过量的胶原纤维沉积和随后的血管形成。这些过程发生在低氧环境(缺氧)中,其决定性地调节炎症和愈合。在分子水平上,对缺氧的反应是由缺氧诱导因子(HIF)调控的,该因子由氧依赖性α亚基(HIF-1α,HIF-2α)和氧非依赖性β亚基组成。HIF-α亚基是组成型表达的,并在常氧下迅速降解。然而,在缺氧状态下,HIF-1α和HIF-2α被稳定并形成活性转录复合物。该复合物与许多下游靶基因的启动子区域中的缺氧反应元件(HRE)结合,这共同启动了对缺氧的适应性反应。

基于先前涉及静脉内施用L-谷氨酰胺和包含L-谷氨酰胺的二肽的研究,一剂特定的谷氨酰胺源可以提供每天每患者千克约0.01g至约1.0g L-谷氨酰胺。然而,可以选择剂量以超过这些上限和下限量。采用丙氨酰-谷氨酰胺的典型剂量可以是约0.02g至约0.5g二肽/kg/天的范围。在手术程序过程中施用谷氨酰胺源时,对于普通成人患者的典型剂量可以提供约0.02g至约2.0g、或约0.3g至约1.5g的L-谷氨酰胺或L-丙氨酰基-L-谷氨酰胺。由于谷氨酰胺是一种无害氨基酸,因此可以大量施用而不会产生任何预期的副作用。
在一些实施方案中,谷氨酰胺源包括谷氨酰胺或其任何药学上可接受的盐。在一些实施方案中,谷氨酰胺源包括L-谷氨酰胺,或是L-谷氨酰胺。已知L-谷氨酰胺具有相对低的溶解度(即,室温下约40g/L)和存储过程中较低的稳定性。因此,谷氨酰胺源还可以包括其它载体氨基酸或可以将谷氨酰胺作为寡肽的一部分结合,这可以提高L-谷氨酰胺的溶解度和稳定性中的一种或其二者。寡肽可以包括任何天然产生的或非天然产生的氨基酸。合适的寡肽包括L-谷氨酰胺并且能够代谢来提供L-谷氨酰胺。优选,这样的肽将呈现出L-谷氨酰胺提高的水溶性和提高的稳定性。常常,这样的肽还将呈现出提高的耐受性,以耐受灭菌和存储过程中的分解。包含L-谷氨酰胺的寡肽可以进一步包括本领域已知的可分裂接头肽部分。参见,和Beck-Sickinger,“Drug delivery and release systems fortargeted tumor therapy,”J.Pept.Sci.21(3):186-200(2015)。这些可分裂的接头可以用于连接各种支架和植入物以及用于本文所述的给药方法。

L-谷氨酰胺和含有L-谷氨酰胺的肽(包括L-丙氨酰-L-谷氨酰胺)的药学上可接受的制备物是可商购获得的。此外,用于本文所述的方法中的含有L-谷氨酰胺的肽也可以根据已知方法来合成并纯化和灭菌,以用于药物使用。
在一些实施方案中,将谷氨酰胺源与一种或多种其它的活性药物成分一起施用。其它的活性药物成分可以以与谷氨酰胺源相同的方式来施用,或通过不同的合适的胃肠外方法或肠内方法来使用。在一些实施方案中,其它的一种或多种活性药物成分可以在谷氨酰胺源施用之前、同时或之后通过相同的给药方式与谷氨酰胺源一起施用。在其它实施方案中,其它的一种或多种活性药物成分可以在谷氨酰胺源施用之前、同时或之后通过不同的给药方式与谷氨酰胺源一起施用。
在一些实施方案中,包括谷氨酰胺源的制剂可以是包含溶解于水相中的L-谷氨酰胺源的液体、糊剂或凝胶。本文所述的组合物可以是无水的、部分水合或完全水合的形式,并且包括谷氨酰胺源加药学上可接受的载体和/或稀释剂成分,如无菌蒸馏水、无菌等渗液、无菌生理盐水溶液,或无水的缓冲剂和/或盐混合物或浓缩物,当其稀释时形成这样的稀释剂成分。将选择组合物中谷氨酰胺源的含量,以在施用过程中提供完全溶解的氨基酸或肽。此外,为给药而配制时从组合物可利用的L-谷氨酰胺的含量和/或给药的制剂的总量将由熟练的医疗工作者来选择,以向患者提供合适剂量的L-谷氨酰胺。
在某些实施方案中,将谷氨酰胺源制剂增稠,使得制剂将呈现出提高的高于适用于静脉内注射的典型液体制剂的粘度。这样增稠的制剂将是糊剂或凝胶的形式,其可以在手术程序过程中直接施用于选定的组织或区域。合适的药学上可接受的增稠剂是已知的并可以使用。优选,这样的试剂将在水合时形成水凝胶,或接受合适的交联剂并水合时将形成水凝胶。针对其生物相容性来选择这样的凝胶形成组分并且可以是可吸收的。已经用于药物制剂中的本领域技术人员已知的合适的增稠剂和凝胶形成剂的实例包括具有亲水性成分的聚合物,如胶原;聚氧化烯聚合物,如聚环氧乙烷、聚乙烯醇、聚乙烯吡咯烷酮和聚甲基丙烯酸羟乙酯;透明质酸盐;和各种蛋白质,如白蛋白等。也可以使用止血凝胶,包括含有血纤蛋白原或血纤蛋白的那些。
水凝胶制剂允许合适的药物延长的递送,所述合适的药物包括例如L-谷氨酰胺或含有至少一个谷氨酰胺残基的二肽或三肽。水凝胶通常形成仓储(depot),其进一步允许药物的浓缩应用。可以在施用前产生包含谷氨酰胺源的水凝胶并且用谷氨酰胺源浸渍,用于进一步的植入。或者,在改变环境后在原位形成水凝胶,所述环境包括pH和温度。用于原位水凝胶形成的各种物理和化学交联聚合物是本领域已知的。示例性和非限制性水凝胶可以包括共聚物,其包含环氧丙烷(PPO)、聚(丙交酯-乙醇酸)(PLGA)、聚(N-异丙基丙烯酰胺)、聚(丙烯富马酸酯)、聚(己内酯)等的嵌段。参见,例如,Hoare和Kohane,“Hydrogels indrug delivery:Progress and challenges,”Polymer49(8):1993-2007(2008)。合适的水凝胶也可以从天然产生的蛋白质和肽生成。参见,例如,Jonker等,“Peptide-and Protein-Based Hydrogels,”Chem.Mater.24(5):759-773(2011)。特定的水凝胶制剂可以由施用部位来确定。
在一些实施方案中,作为含有谷氨酰胺源的可植入膜的一部分来配制谷氨酰胺源。谷氨酰胺源可以应用于或渗入手术可植入膜或其它手术植入物中。例如,可以作为凝胶的一部分来配制谷氨酰胺源并附着于植入物的外部。可以用本发明的液体或凝胶制剂浸渍由诸如编织的可吸收纤维素的材料构成的植入物,该材料可从Ethicon以商购获得。考虑的其它膜包括基于聚酯酰胺的膜(PEA-III)。参见例如国际申请公开No.WO 2014/053542 A1。在一些其它实施方案中,将谷氨酰胺源进行配制用于局部、透皮或离子电渗疗法施用。
在一些实施方案中,剂型内的谷氨酰胺源的载药量为约2%至约90%,包括指定范围内的每个整数。在一些实施方案中,载药量为约10%至约80%。在一些实施方案中,载药量为约20%至约60%。在一些实施方案中,载药量为约20%至约50%。在一些实施方案中,载药量为约20%至约40%。在一些实施方案中,载药量为约1%,约2%,约5%,约10%,约15%,约20%,约25%,约30%,约35%,约40%,约45%,约50%,约55%,约60%,约65%,约70%,约75%,约80%,约85%或约90%。
在一些实施方案中,将药物渗入可植入膜中。在一些实施方案中,可植入膜中的载药量为约2%至约90%,包括指定范围内的每个整数。在一些实施方案中,可植入膜中的载药量为约10%至约80%。在一些实施方案中,可植入膜中的载药量为约20%至约60%。在一些实施方案中,可植入膜中的载药量为约20%至约50%。在一些实施方案中,可植入膜中的载药量为约20%至约40%。在一些实施方案中,可植入膜中的载药量为约1%,约2%,约5%,约10%,约15%,约20%,约25%,约30%,约35%,约40%,约45%,约50%,约55%,约60%,约65%,约70%,约75%,约80%,约85%或约90%。在一个实施方案中,可植入膜中的载药量为约30%。在另一个实施方案中,可植入膜中的载药量为约50%。
在一些实施方案中,包含谷氨酰胺源的剂型稳定长达数月或数年。在一些实施方案中,本文所述的谷氨酰胺源的药物剂型在25℃和60%相对湿度(RH)下稳定约一个月,约两个月,约三个月,约四个月,约五个月,约六个月,约九个月,约十个月,约十一个月,约十二个月,或甚至更长时间。在另一个实施方案中,可以将剂型储存在冷藏条件下,如2-8℃,在这种情况下稳定长达约一个月,约两个月,约三个月,约四个月,约五个月,约六个月,约九个月,约十个月,约十一个月,约十二个月,或甚至更长时间,如约一年,约三年,或长达约五年的任何时间量。
具体实施方式
建立了正常腹膜成纤维细胞(NPF)的原代培养物,并在暴露于阶段性(episodic)低氧环境之前,在常氧条件下抽取低氧前样品以测定HIF-1α和1型胶原蛋白(COL1A1)的水平。将培养的细胞暴露于低氧条件(2%O2;0.5、1、2或4、12、24和48小时),然后恢复常氧(20%O2)并立即用L-Ala-L-Gln处理(0、1、2或10mM)。在缺氧开始后12小时和24小时测量了HIF-1α和COL1A1的水平。所有处理均一式三份进行。
通过ELISA测定了HIF-1α和COL1A1水平(分别按照制造商的实验方案,HIF-1αELISA(Thermo-Fisher Scientific)或COL1A1 ELISA(LifeSpan Biosciences))。用单向ANOVA,接着用使用Bonferroni校正的Tukey’s检验,分析了数据。针对HIF-1α的结果显示于表1-3中。
如表3中所证明的,用L-Ala-L-Gln处理的细胞显示出在所有缺氧持续时间与未处理细胞相比在12和24小时时HIF-1α降低。对于COL1A1获得了相似的结果(数据未显示)。尽管在60分钟时看到了对降低HIF-1α的最大效果,但在每个时间点都看到了一些效果,在30分钟至120分钟具有更显著的效果。所有浓度的L-Ala-L-Gln看来都是有效的,尽管较低一些的剂量(例如,1.0mM和2.0mM)显示出略好于较高(例如,10.0mM)的剂量。
