背景技术
囊性纤维化的特征在于易感染,其导致炎症和肺部损伤。但是,炎症和肺部损伤在没有细菌感染的情况下也可能发生(Sly等人Am J Respir Crit Care Med.2009 180(2):146-52)。
炎症是人体对侵害的反应,包括感染、创伤和过敏。炎症反应是复杂的,且涉及多种防御病原体和修复组织的机制。在肺部中,炎症通常是由病原体或暴露于毒素、污染物、刺激物和过敏原引起的。
在炎症期间,多种类型的炎症细胞被激活。每种细胞均释放细胞因子和介质,以改变其它炎症细胞的活性。这些细胞和分子的编排导致炎症进展。临床上,急性炎症见于以下疾病中,如肺炎和急性呼吸窘迫综合征(ARDS),而慢性炎症则表现为以下疾病,如哮喘、囊性纤维化和慢性阻塞性肺疾病(COPD)。因为肺是进行气体交换的重要器官,所以过度的炎症会危及生命。炎症和抗炎症之间的微妙平衡对于肺部稳态至关重要。
免疫力涉及先天性和适应性系统。先天性免疫是非特异性的,可引起快速反应,包括面对病原体侵害时的炎症。适应性免疫是抗原特异性的。它首先检测特定抗原,然后动员炎症细胞靶向该特定抗原。先天性和适应性系统共享组件并协同行动以抵御病原体。
气道上皮分泌多种物质,如粘蛋白、防御素、溶菌酶、乳铁蛋白和一氧化氮,这些物质非特异性地保护呼吸道免受微生物侵袭。上皮细胞还产生多种介质,例如活性氧自由基、细胞因子(TNF-α、IL-1β、粒细胞/巨噬细胞集落刺激因子[GM-CSF])和血小板活化因子,以将炎症细胞募集到炎症部位。细胞因子刺激花生四烯酸从膜脂中释放,导致类花生酸的产生,从而进一步刺激杯形细胞和组织炎症引起的粘液分泌。
表面活性剂位于肺泡表面,并包含四种表面活性蛋白(SP A-D)。这些蛋白在表面活性剂吸收到肺泡表面中起着关键作用,这对于降低肺表面张力很重要。SP-A和SP-D也参与宿主防御。它们结合细菌表面分子,调节白细胞活性,并导致病原体调理作用。
浆细胞分泌的IgA会形成额外的上皮保护屏障,其阻止微生物粘附到上皮表面。其还与病原体结合,引起吞噬作用和抗体依赖性细胞介导的细胞毒性。免疫球蛋白E(IgE)在呼吸道中引起速发型超敏反应。其通过与肥大细胞、嗜碱性粒细胞、嗜酸性粒细胞和B淋巴细胞表面的IgE受体结合而产生严重反应。重复暴露于同一抗原会导致脱颗粒和促炎症介质(包括组胺、前列腺素、白三烯和类胰蛋白酶)的释放。这些作用会增加血管通透性、支气管收缩和炎症细胞浸润。
US20160263151教导吸入的抗生素与酸化的亚硝酸盐和铁螯合剂的组合用于治疗细菌感染的用途。存在于US20160263151的制剂中的铁螯合剂当与酸化的亚硝酸盐组合时,提供协同作用,从而增强抗生素的能力。
当前针对肺部炎症的治疗包括口服或吸入类固醇以及靶向宿主炎症反应的非类固醇药物。但是,这种作用是暂时的,需要连续治疗,并且存在明显的副作用。
需要治疗或预防肺部炎症的方法;或至少一种补充或提供先前已知治疗方法的替代的方法。

优选地,所述吸入性螯合剂的高浓度是高于37.5mg/剂。
优选地,所述吸入性螯合剂的高浓度是高于50mg/剂。
先前已证明单独使用吸入性EDTA不能治疗细菌感染(Brown等人(Am J DisChild.1985 139(8):836-9);Hassett(Front Microbiol.2016 7:291))。Brown等人(1985)用雾化的EDTA钠治疗10例慢性感染铜绿假单胞菌的CF儿童三个月,观察到肺功能无变化。其他人已报道EDTA引起浓度依赖性支气管收缩(Beasley等人(Br Med J(Clin Res Ed).1987 294(6581):1197-8));EDTA对FEV1没有影响(Asmus等人(J Allergy ClinImmunol.2001 107(1):68-72))。因此,没有理由相信螯合剂会对患有如囊性纤维化(CF)、哮喘、慢性阻塞性肺病(COPD)或其它引起或与炎症相关的肺部病状的受试者产生积极影响。然而,本发明令人惊讶地发现,吸入性螯合剂可以治疗或预防肺部炎症。
通常认为CF肺环境是酸性的。但是,最近发现CF肺的pH与正常肺的pH相同(Schultz等人“Airway surface liquid pH in children with cystic fibrosis”.Nature Communications 2017 8(1):1409)。现有技术使用酸化亚硝酸盐,如US20160263151中所描述,因此该制剂不太可能对CF在临床中起作用,因为该制剂不会保持酸化,而是会立即恢复到正常的肺部pH值的7.4。
尽管CF肺的酸度正常,已发现铁水平与正常肺显著不同。Stites等人(Am JRespir Crit Care Med.1999 160(3):796-80)的结果表明,与健康个体相比,CF患者肺部和吸烟者肺部的铁水平大幅升高。其还显示,大多数的这种铁是亚铁形式,Fe(II),且与疾病的严重程度显著相关(Hunter等人,MBio.2013 4(4):1-8)。亚铁可参与Fenton反应,产生高反应性氧自由基,其可严重破坏组织和DNA(Jomova等人Toxicology.2011 283(2-3):65-87;MacNee,Eur J Pharmacol.2001 429(1-3):195-207)。
不希望受到理论的束缚,据信本发明的方法通过以下来减轻炎症:(i)通过锌的螯合使基质金属蛋白酶(MMP)失活;(ii)通过铁的螯合减少活性氧(ROS)的产生;和/或(iii)通过剥夺细菌中必需的离子(例如铁和锌)来减少肺中的细菌负载。对于个体肺部的作用是减轻炎症的理论方法中的一种方法、或多种方法的组合。
吸入是一种局部给药方法,因此可以更有效地到达目标区域,即肺部,并提供局部高浓度的吸入性螯合剂。吸入避免了由于活性物质的全身暴露而产生的不希望看到的副作用,并降低了患者产生耐药性的风险。
本发明还提供一种通过给药高浓度的吸入性螯合剂来治疗或预防肺部炎症的方法,其中所述炎症的治疗或预防导致FEV的增加。
本发明还提供一种通过给药高浓度的吸入性螯合剂来治疗或预防肺部炎症的方法,其中所述炎症的治疗或预防与MMP活性降低相关。已知基质金属蛋白酶(MMP)引起肺损伤(Garratt等人Eur Respir J.2015 46(2):384-94)并且MMP活性是Zn2+依赖性的(Hazra等人Molecular Vision 2012;18:1701-1711)。但是,先前对靶向肺部中的MMP的尝试均未成功。本发明使用吸入性螯合剂来螯合肺中的锌,从而减少MMP诱导的肺损伤并治疗或预防炎症。
本发明还提供一种通过给药高浓度的吸入性螯合剂来治疗或预防肺部炎症的方法,其中所述炎症的治疗或预防与羟基自由基生成减少相关。铁是肺损伤的关键因素(Stites等人(Am J Respir Crit Care Med.1999 160(3):796-80),由于铁催化羟基自由基的形成。但是,迄今为止,抗氧化剂试验未能产生肺功能的显著改善。本发明使用吸入性螯合剂来螯合肺中的铁,从而减少羟基自由基引起的肺损伤并治疗或预防炎症。

本发明还提供一种通过给药高浓度的吸入性螯合剂来治疗或预防肺部炎症的方法,其中所述炎症的治疗或预防是由去除或减少细菌产生的蛋白酶而导致的,所述蛋白酶可刺激局部炎症、引起局部组织损伤,且可中和抗生素活性。这些酶很大程度上是阳离子依赖性的,从环境中去除阳离子有望使这些酶失活。

优选地,所述螯合剂具有与EDTA近似相同的铁亲和力,和/或与EDTA近似相同的锌亲和力。EDTA在25℃和0.1M的形成常数或稳定常数(log K1)为:Fe2+为14.3、Fe3+为25.1且锌为16.5。
在一个实施方案中,螯合剂是所述螯合剂的钙盐。优选地,所述螯合剂是CaEDTA。

吸入性螯合剂优选为以不多于8小时、7小时、6小时、5小时、4小时、3小时、2小时、1小时、45分钟、30分钟、20分钟、15分钟、10分钟或5分钟的时间段递送。如果通过干粉递送给药,则吸入性螯合剂可以秒为周期递送,例如1秒每“喷”气雾剂装置或干粉吸入器,其中在每个时间点给药一或多喷。
优选地,吸入的螯合剂至少连续28天给药。吸入的螯合剂可以递送2天或更长时间、3天、4天、5天、6天或7天。吸入的螯合剂可以递送2至28天、1周、2周、3周或4周。
一些受试者可从一段时间内对受试者“加载”抗生素和/或螯合剂中受益,在几天或几周内以更高剂量或更频繁地给药,然后以降低剂量或维持剂量给药。
因此本发明:
·递送吸入性螯合剂总量为37.5mg/天至1,200mg/天;
·每天至少给药一次,最多每天给药六次,优选为最多每天给药四次;
·给药不超过8小时的时间段。
优选地,本发明:

任意螯合剂的优选量可通过对比所述试剂与CaEDTA的螯合能力,然后将上述给定的剂量范围乘以该数量来计算。结果提供的螯合水平应近似等于由优选量的EDTA提供的优选螯合水平。

优选地,将所述制剂对有需要的受试者以约每天一次至约每天6次给药,更优选为约每天4次给药。
可替代地,所述制剂可以通过连续吸入、通过雾化器对有需要的受试者给药。雾化的制剂可以递送24小时、12小时、8小时、6小时、4小时、2小时或1小时,并且每一次递送(除了24小时和12小时)都可以在24小时内重复几次。
受试者通常会接受约约0.01至15mg/kg/天剂量的螯合剂,+20%或+10%。该剂量通常会雾化或通过至少一次、优选为多次“喷”的气雾剂装置给药。例如,受试者可以单剂量或一天中的多剂量接受0.1mg/kg至15mg/kg螯合剂。
每天的总剂量优选每天给药至少一次,但是可以分为每天给药两次或多次。一些受试者可从一段时间内对受试者“加载”抗生素和/或螯合剂中受益,在几天或几周内以更高剂量或更频繁地给药,然后以降低剂量或维持剂量给药。因为囊性纤维化、COPD等,通常为慢性病状,受试者期望在延长的时间段内接受该疗法。
不论药物制剂的形式如何,优选生成的用于吸入的液滴或颗粒的范围为:约0.1μm至12μm、或约0.25μm至6μm、优选1μm至6μm、更优选为约2μm至4μm。可替代地,所述颗粒可为0.1μm至1.0μm、0.2μm至0.9μm、0.3μm至0.8μm、0.4μm至0.7μm或0.5μm。通过生成尺寸范围较窄的吸入颗粒,可以进一步提高药物递送系统的效率并改善剂量的可重复性。因此,优选的是,所述颗粒不仅尺寸在0.1μm至12μm或2μm至6μm或约3至4μm的范围内,而且平均粒径在窄的范围内,因此,被递送到受试者的80%或更多的颗粒的粒径在平均粒径的±20%以内,优选为平均粒径的±10%,更优选为±5%。
“粒径”是为了比较固体颗粒、液体颗粒(液滴)的尺寸而引入的概念。对于小滴和气溶胶,使用术语如“空气动力学直径”和“质量中值空气动力学直径(MMAD)”。其定义在下文中给出。
“空气动力学直径”是单位密度球的直径,其具有与所讨论的粒子相同的最终沉降速度。它用于预测此类颗粒在呼吸道中的何处沉积。

优选地,所述制剂对有需要的受试者每天给药约一次至每天给药约6次,更优选为每天4次。
可替代地,所述制剂可通过连续吸入、通过雾化器对有需要的受试者给药。所述雾化制剂可递送24小时、12小时,优选为8小时、6小时、4小时、2小时或1小时,并且每一次递送(除了24和12小时)都可以在24小时内重复几次。
可以使用一次性包装和便携式、手持式、电池供电的装置,例如AERx装置,将本发明的制剂对受试者给药(美国专利号5,823,178,Aradigm,Hayward,Calif.)。可替代地,本发明的制剂可以使用机械(非电子)装置来获得。其他可用于递送制剂的吸入装置,包括常规的喷射雾化器、超声雾化器、软雾吸入器、干粉吸入器(DPI)、定量吸入器(MDI)、冷凝气雾发生器和其他系统。
为了作为气雾剂使用,可以将溶液或悬浮液中的本发明化合物与合适的推进剂,例如烃推进剂如丙烷、丁烷或异丁烷,与常规助剂一起包装在加压气雾剂容器中。干粉吸入器是可与压缩空气源一起操作以产生被压缩成非常小的体积的药物制剂的干粉颗粒的系统。对于吸入,所述系统具有多个腔室或泡罩(blister),每个包含单剂量的药物制剂和用于释放单剂量的选择元件。

赋形剂

此外,可以添加缓冲剂以调节制剂的pH值。优选地,本发明的制剂包含三(羟甲基)氨基甲烷(TRIS,也称为THAM或氨基丁三醇)作为缓冲剂。TRIS可在增强EDTA杀死细菌的作用方面具有进一步的作用。优选地,TRIS被添加到本发明的制剂中以缓冲制剂并增加EDTA和/或抗生素在治疗或预防细菌感染中的有效性。
此外,本发明的制剂可包含抗微生物防腐剂。
优选地,本发明制剂的pH为约6.5至8.0,更优选约7.0至7.4。以前已经发现,pH下降越多,细菌对抗菌治疗的耐性就越高。优选的pH有助于避免细菌对制剂在不存在酸化亚硝酸盐的情况下的耐性,所述制剂包含高浓度吸入性螯合剂与抗生素的组合。

可以添加抗氧化剂,如谷胱甘肽和维生素E、锌和EDTA的锌盐。
乙醇蒸气可作为肺部的消泡剂,使痰更液体化,可以帮助呼吸并减少肺水肿。可以将乙醇以0.5%至60%,更优选地1至40%,1至20%或1至10%的比例添加到本发明的制剂中。乙醇可以1%、2%、3%、4%、5%、6%、7%、8%、9%、10%、15%、20%、25%、30%、35%、40%、45%、50%、55%或60%的比例加入。

制备药物的方法

优选地,所述高浓度螯合剂以包含至少37.5mg/剂、至少50mg/剂、或50mg/剂至300mg/剂、或37.5mg/剂至300mg/剂的剂型提供。所述螯合剂每天给药一至四次,最多可以最多为约1,200mg/天,优选为至少150mg/天的总剂量给药。所述螯合剂优选为CaEDTA。
试剂盒
本发明提供了一种用于治疗或预防肺部炎症的试剂盒,其包含(i)含有高浓度螯合剂的可吸入制剂;和(ii)使用说明书。

实施例1:通过给药高浓度的吸入性螯合剂治疗或预防肺部感染的方法
使用从上皮细胞系中收获的囊性纤维化粘液的悬浮液滴,在仿真的体外模型中生长生物被膜(Haley等人BMC Microbiol 2012 12:181)。将铜绿假单胞菌临床菌株(MIC妥布霉素>256μg/ml)的培养物在无碳源的M63中生长到静止期晚期,以模拟营养限制。
从倒置的IBIDI盖玻片上悬浮粘液滴(5μl),并接种103个菌落形成单位(CFU),然后在35℃的潮湿环境中温育72小时,以使生物被膜发育。然后用雾化妥布霉素(20mg/ml)、雾化的CaEDTA颗粒(10mg/ml)或两者同时处理5分钟。对照用雾化的0.9%盐水/水的50/50溶液处理。处理后,将液滴温育16小时,然后用BacLight LIVE/DEAD(1μl)染色,并在多聚甲醛蒸气中固定30分钟。使用共聚焦显微镜观察生物被膜。
图1A显示厚而坚固的生物被膜,用雾化盐水处理后大部分是活细胞(绿色)。如对耐药性菌株所预期的那样,仅使用妥布霉素治疗对生存力影响很小。单独使用EDTA会造成一定程度的杀灭(红色细胞)。令人惊讶的是,妥布霉素和EDTA的组合杀死了绝大多数生物被膜细胞。图1B显示图1A中的显微图像的定量表示。对照生物被膜为1x108CFU/ml,而EDTA-妥布霉素处理过的生物被膜降低了>6个数量级,降至<102CFU/ml。
入院的病情加重的≥6岁CF患者,除常规静脉内注射抗生素和雾化妥布霉素治疗外,还随机接受EDTA或生理盐水(安慰剂)治疗。EDTA作为雾化溶液与妥布霉素一起给药,所述雾化溶液为4ml的0.9%盐水中的50mM CaNa2EDTA、111mM Tris,pH 7.1。随机化后,受试者在医院治疗两周,期间每天接受四次治疗(300mg EDTA/天,或最多3.3mg EDTA/kg/天)。然后患者出院并继续治疗,每天两次,持续四周。进一步监测患者四周,使总研究时间达到10周。
用雾化的3%高渗盐水以8-10L/min的速度诱导痰≥5分钟。在治疗之前以及在第2、6和10周时收集样品,然后根据相关方案进行处理,并保存于-80℃。从混有Sputalysin(每克痰1ml)的透明痰中分离粘液,涡旋并温育一小时,然后放入脱脂乳甘油储存介质中,并储存于-80℃。
在最初的筛查中、以及然后在第3次访视(约2周后)、第5次访视(6周)、最后在随访时(10周),通过排痰从受试者身上获得痰液样本。从透明的痰中分离粘液,用Sputalysin(每克痰1ml)处理,然后放入脱脂乳甘油存储介质(1ml/100mg粘液)中,涡旋混合并储存于-80℃。
将样品在冰上融化,将系列稀释液从原始浓度最多稀释至10-7,并将20μL各稀释液分别置于McConkey(McC)琼脂或Blood琼脂(BA)的三个平板中。培养板于35℃温育。
假单胞菌属被定义为Mc琼脂平板上的透明或非常浅的粉红色的乳糖阴性菌落。粗糙形态菌落具有金属光泽和粗糙的菌落边缘,光滑形态菌落生长缓慢,在Mc琼脂上具有规则的菌落边界,并且粘液样形态菌落被细菌分泌的大量藻酸盐包围。
在24小时时,分别在McC琼脂平板和BA平板上计数粗糙、光滑和粘液样菌落的数量,将板再温育24小时,并进行每个菌落形态的确认计数。挑选每个样品中存在的每种形态的单个菌落,并在BA平板上划线,以获得纯培养物。
通过革兰氏染色进一步鉴定,以确认分离物由革兰氏阴性杆状细胞组成,并通过将菌落的一小部分擦到氧化酶测试条上来确认氧化酶阳性状态。深蓝色的快速发展表明存在氧化阳性分离物。
假单胞菌属的确认通过测试对C390抗生素的抗性进行。将经过抗生素浸渍的圆盘置于营养琼脂(NA)平板上,该平板上铺有纯分离物在磷酸盐缓冲盐水(PBS)中的悬浮液,其McFarland密度为0.5。在35℃过夜温育后,圆盘周围没有任何抑制区域,表明对抗生素具有抗性。挑选每个分离物中存在的每种形态学类型的被确认的铜绿假单胞菌属单菌落(可能是铜绿假单胞菌)并将其重悬浮在甘油/血清存储介质中,并于-80℃存储。图2显示与治疗时开始相比,第2周和第6周铜绿假单胞菌(McC)的菌落计数变化。治疗两周后,EDTA组的菌落数减少>400倍,而安慰剂组为4.5倍。
实施例2:肺部炎症的治疗导致FEV1的剂量依赖性增加
入院的病情加重的≥6岁CF受试者,除常规静脉内注射抗生素和雾化妥布霉素治疗外,还随机接受EDTA或生理盐水(安慰剂)治疗。EDTA作为雾化溶液与妥布霉素一起给药,所述雾化溶液为4ml的0.9%盐水中的50mM CaNa2EDTA、111mM Tris,pH 7.1。随机化后,受试者在医院治疗两周,期间每天接受四次治疗(300mg EDTA/天)。出院后继续治疗,每天两次,持续四周。进一步监测患者四周,使总研究时间达到10周。
在每次研究访视时,通过肺活量测定法测量肺功能。记录数据为三次尝试中最好的一次,结果以预测%表示。
图3A显示治疗开始后第2、6和10周两组的FEV1的平均变化。EDTA组2周后FEV1的平均增加为16%点,而安慰剂组为5%点。这种差异在治疗完成后四周持续存在,EDTA组增加了7%点,而安慰剂组增加了2%点。这表明EDTA组的肺功能明显改善,而安慰剂组变化不大。图3B显示EDTA组FEV1改善与体重呈负相关(R2=0.70),但单独使用妥布霉素治疗的安慰剂组无相关性(R2=0.01)。这表明EDTA对肺功能具有剂量依赖性作用(mg EDTA/kg体重)。
图4显示,向肺中递送75mg CaEDTA会导致在给药后5分钟EDTA的峰值浓度为0.41-1.34mM。
实施例3:香烟烟雾引起的肺部炎症可以通过对肺部给药高剂量的螯合剂来治疗
在慢性阻塞性肺疾病(COPD)的小鼠模型中测试螯合剂对肺部炎症的作用。众所周知,香烟烟雾(CS)会诱发肺部炎症,其可以通过增加的白细胞计数和增加的肺重量来测量。
将雄性BALB/c小鼠(每组8只)在两周的时间内暴露于一定剂量的香烟烟雾(3支香烟,周一至周五每天3次)或过滤后的室内空气中。在实验过程中,每次香烟烟雾暴露前30-60分钟,每天三次用铁螯合剂去铁胺(DFO,3.8mg,50μl)或媒介物鼻内处理小鼠。然后杀死小鼠,并评估气道和肺以确定对香烟烟雾引起的炎症的影响和元素浓度。
收集支气管肺泡灌洗液(BALF)(约1ml/小鼠),并通过手术切除肺并称重。通过将等体积的锥虫蓝与BALF混合,并使用Zeiss Axioscope荧光显微镜在标准Neubauer血细胞计数器上手动计数,来确定BALF中的活细胞总数。使用激光烧蚀-电感耦合等离子体质谱法(LA-ICP-MS)通过元素分析、并与已知金属含量的标准品进行定量来测量铁。
图5表明,如预期的那样,香烟烟雾显著增加BALF白细胞的总数。用铁螯合剂DFO处理会显著降低该效果。
与此相符的是,图6显示,使用香烟烟雾处理后,平均肺部重量显著增加,但是使用CFO进行处理可以防止这种作用。
如前文提到的,Stites等人(Am J Respir Crit Care Med.1999 160(3):796-80)显示,与健康个体相比,CF患者的肺部和吸烟者的肺部的铁水平大幅升高。图7(左)证实暴露于香烟烟雾的小鼠的BALF中平均铁水平显著提高,并且用DFO处理降低了BALF中的平均铁含量。图7(右)结果表明,在用DFO治疗的7只小鼠中(其中一只因与治疗无关的原因而丢失),有6只中的BALF的铁含量与暴露于空气中的小鼠处于相同水平。
预示性实施例P1:高剂量干粉螯合剂对感染、炎症和氧化应激影响的体内研究
需要使用干粉妥布霉素治疗的CF受试者将分为四组,每天接受112mg干粉两次,持续28天。另外,组1(患者>18岁)接受递增剂量的干粉CaEDTA(37.5mg每天两次持续1周;75mg每天两次持续2周,150mg每天两次持续1周)。组2(患者>18岁)接受CaEDTA(37.5mg每天两次持续1周;75mg每天两次持续2周;75mg每天四次持续1周)。组3(患者12-18岁)接受CaEDTA(37.5mg每天两次持续1周;75mg每天两次持续2周,150mg每天两次持续1周)。最后,对照组将仅接受妥布霉素治疗28天。

可以预期,与安慰剂组相比,该实验将显示EDTA组中的炎症标志物减少,铁水平降低。可以进一步预期该实验将显示MMP和TIMP之间,尤其是MMP-9和TIMP-1之间的平衡变化,其与支气管扩张的进展有关。
进一步预期该实验将显示与对照患者相比,用EDTA治疗的受试者的FEV1增加,其痰液中细菌负载减少。
预示性实施例P2:高剂量螯合剂对炎症、肺损伤和氧化应激的影响的体外研究
肺上皮细胞在组织培养物中生长,并且通过暴露于Fe(II)或过量氧气而诱导炎症。细胞用CaEDTA(0、1、5、10、25、50mM)处理30分钟、1、3、24和48小时。
免疫测定法用于监测炎症标志物如IL-6、IL-8、TNF-α、中性粒细胞弹性蛋白酶等的变化。氧化应激和毒性是通过根据TUNEL测定法评估谷胱甘肽(GSH)水平的降低和细胞凋亡来测量的,二者均使用商业测定试剂盒如,ThermoFisher Scientific的谷胱甘肽荧光检测试剂盒和BioVision Inc.的TUNEL DNA片段(gragmentation)分析试剂盒。
可以预期,该实验将显示,与对照相比,EDTA处理的细胞中炎症标志物浓度依赖性的降低;GSH的减少指示与对照相比,EDTA处理的细胞中活性氧的减少;且与对照相比,如TUNEL测定法所测量的,EDTA处理的细胞中的细胞凋亡减少。

·细胞损伤、游离铁和氧化应激
将咳出的痰液不处理直接冷冻,并按上述方法检测炎症标志物。如前所述,作为结构损伤的度量,分别使用明胶酶谱法和免疫测定法测量基质金属蛋白酶(MMP)和金属蛋白酶组织抑制剂(TIMP)的水平(Gaggar等人,Eur Respir J.2011 38(3):721–727;Garratt等人,Eur Respir J.2015 46(2):384-94)。如前所述,痰液中铁的含量通过ICP-MS进行定量(Hunter等人,MBio.2013 4(4):1-8)。使用免疫测定法评估铁结合蛋白的量。如前所述,通过使用免疫测定法测量谷胱甘肽(GSSG和GSH)来评估氧化应激(Kettle等人,Eur RespirJ.2014 44(1):122-9)。

预示性实施例P4:高剂量螯合剂对炎症、肺部损伤和氧化应激影响的体内研究
对囊性纤维化受试者进行了单中心、随机、双盲、交叉研究。受试者随机接受吸入性CaEDTA或盐水(安慰剂)治疗两周。接下来是冲洗阶段,然后是持续两周的其它治疗(EDTA或安慰剂)。
如上所述监测铁水平、炎症标志物、MMP/TIMP和FEV1。如前所述,还测定了作为嗜中性粒细胞炎症的量度的髓过氧化物酶活性(Gaggar等人,Eur Respir J.2011 38(3):721–727)。将3-氯酪氨酸作为强氧化剂次氯酸的生物标志物来测定。使用稳定的同位素稀释气相色谱仪和质谱仪测量水平(Gaggar等人,Eur Respir J.2011 38(3):721–727)。使用商业免疫测定试剂盒测量蛋白质羰基化合物作为活性氧(ROS)的指标(Gaggar等人,EurRespir J.2011 38(3):721–727)。
可以预期,与用安慰剂治疗相比,用EDTA治疗的受试者的铁和炎症标志物水平降低、MMP/TIMP平衡改变、平均FEV1升高。可以进一步预期该实验将显示降低的髓过氧化物酶活性和较低的氯酪氨酸和羰基平均水平。
临床数据显示以300mg/天持续两周的疗效。同一项研究表明150mg/天(75mg每天两次)对肺功能和感染有益(图2为降低的细菌计数;图3A为改善的肺功能)。如本领域技术人员所理解的,根据改善的显著程度(FEV1平均为16%点),很可能更低的剂量,即75mg/天(37.5mg每天两次),是有效的,如预示性实施例P1所设想。
图4显示,单一剂量的75mg CaEDTA在30分钟后可导致在粘液栓内部高达1.34mM的EDTA。众所周知,妥布霉素等药物进入CF痰的渗透是显著受阻的(Kuhn,R.J.(2001)。Formulation of aerosolized therapeutics.Chest 120,94S-98S),因此,气道表面液体中EDTA的浓度极有可能大大高于中心。因此,可以合理地预期,37.5mg的每日剂量(比具有临床益处的较低剂量低4倍)将在一项完全研究中显示出疗效。在年轻患者的情况下尤其如此,其接受每单位体重更高的剂量且通常在FEV1中表现出更大的反应(图3B)。
