电外科装置可用于许多外科手术中。电外科装置可向组织施加电能以便对组织进行处理。电外科装置可包括具有安装在远侧的端部执行器的器械,该端部执行器包括一个或多个电极。端部执行器可抵靠组织定位,使得电流可被引入到组织中。电外科装置可被构造用于单极或双极操作。在单极操作期间,电流由端部执行器上的有源(或源)电极引入到组织中,并通过返回电极返回。返回电极可以是接地垫,并且单独地位于患者的身体上。在双极操作期间,电流可分别通过端部执行器的有源电极和返回电极被引入到组织中并从组织返回。
端部执行器可包括两个或更多个钳口构件。该钳口构件中的至少一个可具有至少一个电极。至少一个钳口能够从与相反钳口间隔开以用于接收组织的位置移动到钳口构件之间的空间小于第一位置的钳口构件之间的空间的位置。可移动钳口的移动可以压缩保持在其间的组织。由流过组织的电流所产生的热结合通过钳口的移动实现的压缩可以在组织内和/或在组织之间形成止血密封并因此可尤其适用于例如密封血管。端部执行器可包括切割构件。该切割构件可相对于组织和电极运动以横切组织。
电外科装置还可包括将组织夹持在一起的机构(诸如缝合装置)和/或切断组织的机构(诸如组织刀)。电外科装置可包括轴,该轴用于将端部执行器邻近接受治疗的组织放置。轴可为直的或弯曲的、可弯曲的或不可弯曲的。在包括直的和可弯曲轴的电外科装置中,轴可具有一个或多个关节运动接头以允许轴的受控弯曲。当使用具有直的非弯曲轴的电外科装置不容易接近待处理的组织时,此类接头可允许电外科装置的使用者以与轴成角度的方式将端部执行器放置成与组织接触。
由电外科装置施加的电能可通过与手持件连通的发生器传递至器械。电能可为射频(“RF”)能量的形式。RF能量为可在200千赫兹(kHz)至1兆赫兹(MHz)频率范围内的电能形式。在应用中,电外科器械可穿过组织传递低频RF能量,这会引起离子振荡或摩擦,并实际上造成电阻性加热,从而升高组织的温度。由于受影响的组织与周围组织之间形成明显的边界,因此外科医生能够以高精确度进行操作,并在不损伤相邻的非目标组织的情况下进行控制。射频能的低操作温度适用于在密封血管的同时移除、收缩软组织、或对软组织塑型。RF能量尤其奏效地适用于结缔组织,所述结缔组织主要由胶原构成并且在接触热时收缩。
RF能量可在EN 60601-2-2:2009+A11:2011,定义201.3.218-高频率中所述的频率范围内。例如,单极RF应用中的频率通常可被限制为小于5MHz。然而,在双极RF应用中,频率几乎可为任何值。单极应用通常可使用高于200kHz的频率,以便避免由于使用低频电流而导致不希望的对神经和肌肉的刺激。如果风险分析显示神经肌肉刺激的可能性已减轻至可接受的水平,则双极应用可使用较低频率。通常,不使用高于5MHz的频率以最小化与高频渗漏电流相关联的问题。然而,在双极应用的情况下,可使用较高的频率。通常认为,10mA是组织热效应的下限阈值。
图1和图2示出了可用于执行多种不同外科手术的马达驱动外科系统10。在例示的布置中,外科系统10包括操作地联接到柄部组件500的可互换外科工具组件1000。在另一外科系统方面,可互换外科工具组件1000还可以有效地与机器人控制的外科系统或自动外科系统的工具驱动组件一起使用。例如,本文所公开的外科工具组件1000可与各种机器人系统、器械、部件和方法诸如但不限于名称为“SURGICAL STAPLING INSTRUMENTS WITHROTATABLE STAPLE DEPLOYMENT ARRANGEMENTS”的美国专利9,072,535中公开的那些一起使用,该专利申请据此全文以引用方式并入本文。

在至少一种形式中,可纵向移动的驱动构件540可以具有形成在其上的齿条542,以用于与和马达交接的对应驱动齿轮布置(未示出)啮合接合。参见图3。关于那些特征的进一步的细节可见于美国专利申请公布2015/0272575。然而,在至少一种布置中,可纵向移动驱动构件被绝缘,以保护其免受无意的射频能量的影响。至少一种形式还包括可手动致动的“救助”组件,所述组件被配置为能够允许临床医生在马达505变得停用的情况下手动地回缩可纵向移动的驱动构件。救助组件可包括杠杆或救助柄部组件,其在可释放门550下方储存在柄部组件500内。参见图2。杠杆可被配置为能够被手动枢转成与驱动构件中的齿棘轮接合。因此,临床医生可通过使用救助柄部组件手动地回缩驱动构件540,以使驱动构件在近侧方向“PD”上做棘轮运动。名称为“POWERED SURGICAL CUTTING AND STAPLINGAPPARATUS WITH MANUALLY RETRACTABLE FIRING SYSTEM”的美国专利8,608,045(该专利的全部公开内容据此以引用方式并入本文)公开了救助布置以及也可与本文所公开的各种可互换外科工具组件中的一个一起采用的其他部件、布置和系统。
在该例示方面,可互换外科工具组件1000包括外科端部执行器1500,该外科端部执行器包括第一钳口1600和第二钳口1800。在一种布置中,第一钳口包括细长通道1602,该细长通道被配置为能够将常规(机械)外科钉/紧固件仓1400(图4)或射频(RF)仓1700(图1和图2)可操作地支撑在其中。第二钳口1800包括相对于细长通道1602被枢转地支撑的砧座1810。通过致动闭合驱动系统510,砧座1810可以在打开位置和闭合位置之间选择性地朝向和远离支撑在细长通道1602中的外科仓移动。在例示的布置中,砧座1810被枢转地支撑在细长通道1602的近侧端部部分上,以用于围绕横向于轴轴线SA的枢转轴线的选择性枢转行进。闭合驱动系统510的致动可导致附接到关节运动连接器1920的近侧闭合构件或近侧闭合管1910的远侧轴向运动。

如图5所示,在至少一种布置中,可互换外科工具组件1000包括工具机架组件1200,该工具机架组件包括将喷嘴组件1240可操作地支撑其上的工具底座1210。如与本申请同日提交并据此全文以引用方式并入本文的名称为“SURGICAL INSTRUMENT WITHAXIALLY MOVABLE CLOSURE MEMBER”的美国专利申请(代理人案卷号END8209USNP/170097)中进一步所述,工具底座1210和喷嘴布置1240有利于外科端部执行器1500围绕轴轴线SA相对于工具底座1210旋转。这种旋转行进由图1中的箭头R表示。同样如图4和图5所示,可互换外科工具组件1000包括脊组件1250,该脊组件可操作地支撑近侧闭合管1910并且联接到外科端部执行器1500。在各种情况下,为了便于组装,脊组件1250可从由按扣特征部、粘合剂、焊接等互连在一起的上脊段1251和下脊段1252制成。在组装形式中,脊组件1250包括可旋转地支撑在工具底座1210中的近侧端部1253。在一种布置中,例如,脊组件1250的近侧端部1253附接到脊轴承(未示出),该脊轴承被配置为能够被支撑在工具底座1210内。这种布置有利于脊组件1250到工具底座的可旋转附接,使得脊组件1250可选择性地相对于工具底座1210围绕轴轴线SA旋转。

本文所公开的可互换外科工具组件1000的示例性型式可以与标准(机械)外科紧固件仓1400或被配置为能够有利于利用刀构件切割组织并使用射频(RF)能量密封切割的组织的仓1700结合使用。再次参见图4,其示出了常规或标准机械型仓1400。此类仓布置是已知的,并且可以包括仓体1402,该仓体的尺寸和形状设置成可移除地接纳并支撑在细长通道1602中。例如,仓体1402可以被配置为能够可移除地保持与细长通道1602卡扣接合。仓体1402包括细长狭槽1404以容纳刀构件1330穿过它轴向行进。仓体1402可操作地在其中支撑多个钉驱动器(未示出),所述钉驱动器在居中设置的细长狭槽1404的每侧上成排对准。驱动器与对应的钉/紧固件凹坑1412相关联,凹坑穿过仓体1402的上平台表面1410。每个钉驱动器在其上支撑一个或多个外科钉或紧固件(未示出)。滑动组件1420被支撑在仓体1402的近侧端部内,并且在仓1400是新的且未击发时处于起始位置,位于驱动器和紧固件的近侧。滑动组件1420包括多个倾斜的或楔形凸轮1422,其中每个凸轮1422对应于位于狭槽1404的侧面上的紧固件或驱动器的特定线。滑动组件1420被配置为能够在刀构件被朝远侧驱动通过夹持在砧座和仓平台表面1410之间的组织时被刀构件1330接触并被其驱动。当驱动器被向上推向仓平台表面1410时,支撑在其上的一个或多个紧固件被从其钉凹坑1412中驱出,并穿过夹持在砧座和仓之间的组织。
仍参见图4,在至少一种形式中,砧座1810包括砧座安装部分1820,该砧座安装部分具有从其侧向突出的一对砧座耳轴1822,这对砧座耳轴可枢转地接纳在形成于细长通道1602的近侧端部部分1610的直立壁1622中的相应耳轴支架1614中。砧座耳轴1822由通道顶盖或砧座保持器1630可枢转地保持在其对应的耳轴支架1614中。砧座安装部分1820被可移动或可枢转地支撑在细长通道1602上,以便于围绕横向于轴轴线SA的固定砧座枢转轴线相对于该细长通道选择性地枢转。如图6和图7所示,在至少一种形式中,砧座1810包括砧座主体部分1812,该砧座主体部分由例如导电金属材料制成并且具有钉成形下表面1813,该钉成形下表面具有在其中形成的一系列紧固件成形凹坑1814,这些紧固件成形凹坑在居中设置的砧座狭槽1815的每一侧上,该居中设置的砧座狭槽被配置为能够将刀构件1330可滑动地容纳在其中。砧座狭槽1815通向上部开口1816,该上部开口纵向延伸穿过砧座主体1812,以在击发期间将砧座接合特征部1336容纳在刀构件1330上。当将常规的机械外科钉/紧固件仓1400安装在细长通道1602中时,钉/紧固件被驱动穿过组织T并与对应的紧固件成形凹坑1814形成接触。砧座主体1812可在其上部部分中具有开口以例如有利于安装。砧座顶盖1818可以插入其中并且焊接到砧座主体1812以包封开口并提高砧座主体1812的整体刚度。如图7所示,为了有利于结合RF仓1700使用端部执行器1500,紧固件成形下表面1813的面向组织的段1817可在其上具有电绝缘材料1819。

仍然参见图8,击发构件闭锁系统1640还包括形成在或支撑在击发构件主体1332的远侧端部上的解锁组件1660。解锁组件1660包括向远侧延伸的凸缘1662,该凸缘被配置为能够当滑动组件1420在未击发的外科钉仓1400中处于其起始位置时接合形成在滑动组件1420上的解锁特征部1426。因此,当未击发的外科钉仓1400被适当地安装在细长通道1602中时,解锁组件1660上的凸缘1662接触滑动组件1420上的解锁特征部1426,该解锁特征部用于将刀构件1330向上偏置,使得中央通道接合特征部1137和/或脚部1338跳过通道底部1620中的直立凸缘1654,以有利于刀构件1330轴向通过细长通道1602。如果将部分击发的仓1400无意地安装在细长通道中,则滑动组件1420将不在起始位置,并且刀构件1330将保持在锁定位置。
现在将参照图3和图9描述可互换外科工具组件1000与柄部组件500的附接。要开始联接过程,临床医生可将可互换外科工具组件1000的工具底座1210定位在柄部机架506的远侧端部上方或附近,使得工具底座1210上形成的锥形附接部分1212与柄部机架506中的燕尾形狭槽507对准。然后临床医生可将外科工具组件1000沿垂直于轴轴线SA的安装轴线IA运动,以使锥形附接部分1212安置成与柄部机架506的远侧端部中的对应燕尾形接纳狭槽507“操作地接合”。这样做时,中间击发轴部分1310上的击发轴附接凸耳1314也将安置在柄部组件500内的可纵向移动的驱动构件540中的支架544中,并且闭合连接件514上的销516的部分将安置在闭合梭动件1914中的对应钩1917中。如本文所用,术语“可操作地接合”在两个部件的背景下是指这两个部件彼此充分地接合,使得一旦向其施加致动运动,这些部件便可执行其预期行动、功能和/或程序。同样在此过程中,外科工具组件1000上的机载连接器1154联接到外壳连接器562,该外壳连接器与支撑在例如柄部组件500或机器人系统控制器中的微处理器560连通。
在典型的外科手术期间,临床医生可通过患者体内的套管针或其他开口将外科端部执行器1500引入外科手术部位,以接近目标组织。当这样做时,临床医生通常沿着轴轴线SA轴向地对准外科端部执行器1500(非关节运动状态)。例如,一旦外科端部执行器1500已经通过套管针入口,则临床医生可能需要使端部执行器1500进行关节运动以有利地将其定位在目标组织附近。这先于在将砧座1810闭合到目标组织上,因此闭合驱动系统510将保持未致动。当处于该位置时,击发驱动系统530的致动将导致向近侧关节运动驱动器1370施加关节运动动作。一旦端部执行器1500已达到期望的关节运动位置,击发驱动系统530就被停用,并且关节运动锁1390可将外科端部执行器1500保持在关节运动位置。然后,临床医生可以致动闭合驱动系统510以将砧座1810闭合到目标组织上。闭合驱动系统510的这种致动还可导致换档器组件1100使近侧关节运动驱动器1370与中间击发轴部分1310脱开连接。因此,一旦目标组织已经被捕获在外科端部执行器1500中,临床医生就可以再次致动击发驱动系统530以使击发构件1330轴向前进穿过外科钉/紧固件仓1400或RF仓1700以切割被夹持的组织并将钉/紧固件击发到切割的组织T中。其他闭合和击发驱动布置、致动器布置(手持的手动式和自动或机器人式两者)也可在不脱离本公开的范围的情况下用于控制外科工具组件1000的闭合系统部件、关节运动系统部件和/或击发系统部件的轴向运动。
如上所述,外科工具组件1000被配置为能够与常规的机械外科钉/紧固件仓1400以及RF仓1700结合使用。在至少一种形式中,RF仓1700可有利于将刀构件1330夹持在砧座1810和RF仓1700之间的组织进行机械切割,同时凝结电流在电流路径中被递送至该组织。用于进行机械切割和使用电流凝结组织的另选布置在例如美国专利5,403,312、7,780,663和名称为“ELECTROSURGICAL INSTRUMENT WITH ELECTRICALLY CONDUCTIVE GAP SETTINGAND TISSUE ENGAGING”的美国专利申请序列号15/142,609中公开,每个所述参考文献的全部公开内容以引用方式并入本文。此类器械可以例如改善止血,减少手术复杂性以及缩短手术室时间。
如图10至图12所示,在至少一种布置中,RF外科仓1700包括仓体1710,该仓体的尺寸和形状设置成可移除地接纳并支撑在细长通道1602中。例如,仓体1710可以被配置为能够可移除地保持与细长通道1602卡扣接合。在各种布置中,仓体1710可以由聚合物材料制成,诸如,例如工程热塑性材料,诸如液晶聚合物(LCP)VECTRATM,并且细长通道1602可以由金属制成。在至少一个方面,仓体1710包括居中设置的细长狭槽1712,该细长狭槽纵向延伸穿过仓体以容纳刀1330穿过它纵向行进。如图10和图11所示,一对闭锁接合尾部1714从仓体1710向近侧延伸。每个闭锁接合尾部1714具有在其下侧上形成的闭锁垫1716,该闭锁垫的尺寸被设置为接纳在通道底部1620中的对应近侧开口部分1642内。因此,当仓1700被适当地安装在细长通道1602中时,闭锁接合尾部1714覆盖开口1642和凸缘1654,以将刀1330保持在解锁位置以准备击发。

在至少一种布置中,RF能量由常规的RF发生器400通过电源引线402供应到外科工具组件1000。在至少一种布置中,电源引线402包括凸形插头组件406,该凸形插头组件被配置为能够插入对应的凹形连接器410,该凹形连接器附接到机载电路板1152上的分段RF电路1160。参见图15。这样的布置通过使喷嘴组件1240旋转而不使来自发生器400的电源引线402缠绕而有利于轴和端部执行器1500围绕轴轴线SA相对于工具底座1210的旋转行进。机载开/关电源开关420被支撑在闩锁组件1280和工具底座1210上以用于打开和关闭RF发生器。当工具组件1000可操作地联接到柄部组件500或机器人系统时,机载分段RF电路1160通过连接器1154和562与微处理器560连通。如图1所示,柄部组件500还可以包括显示屏430,其用于查看关于密封、缝合、刀位置、仓的状态、组织、温度等的信息。如图15中同样可见,滑环组件1150与远侧连接器1162交接,该远侧连接器包括柔性轴电路带或组件1164,该柔性轴电路带或组件可以包括用于缝合相关活动的多个窄电导体1166和用于RF目的的较宽电导体1168。如图14和图15所示,柔性轴电路带1164被居中地支撑在形成刀杆1320的层压板或杆1322之间。这样的布置有利于在端部执行器1500的关节运动期间刀杆1320和柔性轴电路带1164的充分挠曲,同时保持足够的刚度以使得刀构件1330能够向远侧推进穿过被夹持的组织。

显示器段(段4)包括联接到主控制器717的显示器连接器。显示器连接器通过显示器的一个或多个集成电路驱动器将主控制器717联接到显示器。显示器的集成电路驱动器可与显示器集成,并且/或者可与显示器分开定位。显示器可包括任一种合适的显示器,诸如有机发光二极管(OLED)显示器、液晶显示器(LCD)和/或任何其他合适的显示器。在一些示例中,显示器段联接到安全控制器。
轴段(段5)包括用于联接到外科器械10(图1至图5)的可互换轴组件500的控件,以及/或者用于联接到可互换轴组件500的端部执行器1500的一个或多个控件。轴段包括轴连接器,该轴连接器被配置为能够将主控制器717联接到轴PCBA。轴PCBA包括具有铁电随机存取存储器(FRAM)、关节运动开关、轴释放霍尔效应开关和轴PCBA EEPROM的低功率微控制器。轴PCBA EEPROM包括特定于可互换轴组件500和/或轴PCBA的一个或多个参数、例程和/或程序。轴PCBA可联接到可互换轴组件500和/或与外科器械10一体成型。在一些示例中,轴段包括第二轴EEPROM。第二轴EEPROM包括对应于可与加电外科器械10交接的一个或多个轴组件500和/或端部执行器1500的多个算法、例程、参数和/或其他数据。
位置编码器段(段6)包括一个或多个磁性角旋转位置编码器。一个或多个磁性角旋转位置编码器被配置为能够识别外科器械10(图1至图5)的马达714、可互换轴组件500和/或端部执行器1500的旋转位置。在一些示例中,磁性角旋转位置编码器可联接到安全控制器和/或主控制器717。
马达电路段(段7)包括被配置为能够控制加电外科器械10(图1至图5)的运动的马达714。马达714通过包括一个或多个H桥场效应晶体管(FET)的H桥驱动器和马达控制器联接到主微控制器处理器717。H桥驱动器也联接到安全控制器。马达电流传感器与马达串联联接,用于测量马达的电流消耗。马达电流传感器与主控制器717和/或安全控制器进行信号通信。在一些示例中,马达714联接到马达电磁干扰(EMI)滤波器。
马达控制器控制第一马达标记和第二马达标记,以向主控制器717指示马达714的状态和位置。主控制器717通过缓冲器向马达控制器提供脉宽调制(PWM)高信号、PWM低信号、方向信号、同步信号和马达复位信号。功率段被配置为能够向电路段中的每一个提供区段电压。
功率段(段8)包括联接到安全控制器、主控制器717和附加电路段的电池。电池通过电池连接器和电流传感器联接到分段电路。电流传感器被配置为能够测量分段电路的总电流消耗。在一些示例中,一个或多个电压转换器被配置为能够向一个或多个电路段提供预先确定的电压值。例如,在一些示例中,分段电路可包括3.3V的电压转换器和/或5V的电压转换器。升压转换器被配置为能够提供最高为预先确定的量(诸如,最高至13V)的升压电压。升压转换器被配置为能够在功率密集操作期间提供附加的电压和/或电流,并且能够防止电压降低状况或低功率状况。
多个开关联接到安全控制器和/或主控制器717。开关可被配置为能够控制分段电路的外科器械10(图1至图5)的操作,并且/或者指示外科器械10的状态。用于应急的应急门开关和霍尔效应开关被配置为能够指示应急门的状态。多个关节运动开关(诸如左侧向左关节运动开关、左侧向右关节运动开关、左侧向中心关节运动开关、右侧向左关节运动开关、右侧向右关节运动开关和右侧向中心关节运动开关)被配置为能够控制互换轴组件500(图1和图3)和/或端部执行器300(图1和图4)的关节运动。左侧换向开关和右侧换向开关联接到主控制器717。左侧开关(包括左侧向左关节运动开关、左侧向右关节运动开关、左侧向中心关节运动开关和左侧换向开关)通过左挠性连接器联接到主控制器717。右侧开关(包括右侧向左关节运动开关、右侧向右关节运动开关、右侧向中心关节运动开关和右侧换向开关)通过右挠性连接器联接到主控制器717。击发开关、夹持释放开关和轴接合开关联接到主控制器717。
任何合适的机械开关、机电开关或固态开关可任意组合,用于实施所述多个开关。例如,开关可以是利用与外科器械10(图1至图5)相关联的部件的运动或存在某个物体来操作的限位开关。此类开关可用于控制与外科器械10相关联的各种功能。限位开关是由机械地连接到一组触点的致动器构成的机电装置。当某个物体与致动器接触时,该装置操作触点以形成或断开电连接。限位开关不仅耐用、安装简便,还操作可靠,故适用于多种应用和环境。限位开关可确定物体的存在或不存在、经过、定位、以及物体行程的结束。在其他具体实施中,开关可以是在磁场影响下操作的固态开关,诸如霍尔效应装置、磁阻(MR)装置、巨磁阻(GMR)装置、磁力计及其他。在其他具体实施中,开关可以是在光影响下操作的固态开关,诸如光学传感器、红外线传感器、紫外线传感器及其他。同样,开关可以是固态装置,诸如晶体管(例如,FET、结型FET、金属氧化物半导体FET(MOSFET)、双极型晶体管等)。其他开关可包括无电导体开关、超声开关、加速度计、惯性传感器等。

图18为根据本公开的一个方面的被配置为能够控制各种功能的外科器械600的示意图。在一个方面,外科器械600被编程为控制位移构件诸如I形梁614的远侧平移。外科器械600包括端部执行器602,该端部执行器可以包括砧座616、I形梁614和可移除钉仓618,该可移除钉仓可以与RF仓609(以虚线示出)互换。端部执行器602、砧座616、I形梁614、钉仓618和RF仓609可如本文所述构造,例如,参考图1至图15。为了简明和清楚起见,可参考图18描述本公开的若干方面。应当理解,结合本公开的图1至图17描述了图18中示意性示出的部件,诸如控制电路610、传感器638、位置传感器634、端部执行器602、I形梁614、钉仓618、RF仓609、砧座616。

线性位移构件诸如I形梁614的位置、移动、位移和/或平移可通过绝对定位系统、传感器布置和表示为位置传感器634的位置传感器来测量。由于I形梁614联接到可纵向移动的驱动构件540,因此I形梁614的位置可通过采用位置传感器634测量可纵向移动的驱动构件540的位置来确定。因此,在以下描述中,I形梁614的位置、位移和/或平移可通过如本文所述的位置传感器634来实现。控制电路610(诸如图16A和图16B中描述的控制电路700)可被编程用于控制位移构件(诸如I形梁614)的平移,如本文所述。在一些示例中,控制电路610可以包括一个或多个微控制器、微处理器或其他合适的处理器,以用于执行使一个或多个处理器以所述方式控制位移构件(例如,I形梁614)的指令。在一个方面,定时器/计数器电路631向控制电路610提供输出信号,诸如耗用时间或数字计数,以将如由位置传感器634确定的I形梁614的位置与定时器/计数器电路631的输出相关联,使得控制电路610可确定I形梁614在相对于起始位置的特定时间(t)处的位置。定时器/计数器电路631可被配置为能够测量所耗用的时间、计数外部事件或时间外部事件。
控制电路610可以生成马达设定点信号622。该马达设定点信号622可以被提供给马达控制器608。该马达控制器608可以包括一个或多个电路,这些电路被配置为能够向马达604提供马达驱动信号624,以驱动马达604,如本文所述。在一些示例中,马达604可以是DC直流电动马达,诸如图1所示的马达505。例如,马达604的速度可以与马达驱动信号624的电压成比例。在一些示例中,马达604可以是无刷直流(DC)电动马达,并且马达驱动信号624可以包括提供给马达604的一个或多个定子绕组的脉宽调制(PWM)信号。而且,在一些示例中,可以省略马达控制器608,并且控制电路610可以直接生成马达驱动信号624。
马达604可以从能量源612处接收电力。能量源612可以是或包括电池、超级电容器或任何其他合适的能量源612。马达604可以经由传输装置606机械联接到I形梁614。传输装置606可以包括一个或多个齿轮或其他连杆部件,以将马达604联接到I形梁614。位置传感器634可以感测I形梁614的位置。位置传感器634可以是或包括能够生成指示I形梁614的位置的位置数据的任何类型的传感器。在一些示例中,位置传感器634可包括编码器,该编码器被配置为能够在I形梁614向远侧和向近侧平移时向控制电路610提供一系列脉冲。控制电路610可以跟踪脉冲以确定I形梁614的位置。可使用其他合适的位置传感器,包括例如接近传感器。其他类型的位置传感器可提供指示I形梁614的运动的其他信号。而且,在一些示例中,可以省略位置传感器634。在马达604是步进马达的情况下,控制电路610可以通过聚合马达604已被指示执行的步骤的数量和方向来跟踪I形梁614的位置。位置传感器634可以位于端部执行器602中或器械的任何其他部分处。
控制电路610可与一个或多个传感器638通信。传感器638可定位在端部执行器602上并且适于与外科器械600一起操作以测量各种衍生参数,诸如间隙距离与时间、组织压缩与时间、以及砧座应变与时间。传感器638可包括例如磁性传感器、磁场传感器、应变仪、压力传感器、力传感器、电感式传感器(诸如涡流传感器)、电阻式传感器、电容式传感器、光学传感器、和/或用于测量端部执行器602的一个或多个参数的任何其它合适的传感器。传感器638可包括一个或多个传感器。
一个或多个传感器638可包括应变仪,诸如微应变仪,其被配置为能够在夹持条件期间测量砧座616中的应变的量值。应变仪提供电信号,该电信号的幅值随着应变量值而变化。传感器638可包括压力传感器,该压力传感器被配置为能够检测由砧座616和钉仓618之间的压缩组织的存在所生成的压力。传感器638可被配置为能够检测位于砧座616和钉仓618之间的组织区段的阻抗,该阻抗指示位于其间的组织的厚度和/或完全性。
传感器638可被配置为能够测量由闭合驱动系统施加在砧座616上的力。例如,一个或多个传感器638可位于闭合管1910(图1至图4)和砧座616之间的交互点处,以检测由闭合管1910施加到砧座616的闭合力。施加在砧座616上的力可表示在砧座616和钉仓618之间捕集的组织区段所经受的组织压缩。所述一个或多个传感器638可沿闭合驱动系统定位在各种交互点处,以检测由闭合驱动系统施加到砧座616的闭合力。一个或多个传感器638可在夹持操作期间由处理器实时取样,如图16A至图16B中所述。控制电路610接收实时样本测量值以分析基于时间的信息,并实时评估施加到砧座616的闭合力。
可以采用电流传感器636来测量由马达604消耗的电流。推进I形梁614所需的力对应于例如由马达604消耗的电流。将力转换成数字信号并将其提供给控制电路610。
当RF仓609代替钉仓618被装载在端部执行器602中时,RF能量源400联接到端部执行器602并且被施加到RF仓609。控制电路610控制RF能量到RF仓609的递送。
在一些方面,喷嘴组件中包括一个独特的电路系统,该系统可允许本文所述的模块化外科器械的使用者直接从柄部组件中所含的工具中操纵端部执行器。喷嘴组件可包括机载电路板,该机载电路板允许电外科发生器直接附接到喷嘴组件并且向端部执行器提供射频(RF)能量,同时还与柄部组件的处理器或控制电路交接。在一些方面,喷嘴组件的独特电路还允许轴旋转,同时仍为端部执行器提供适当的能量和功能性。
在一个方面,将外科器械连接到发生器能够实现某些轴功能。例如,将RF引线附接到发生器允许外科器械机载电路板将用于RF应用的一些细长轴集成电路布线隔离到与钉仓可互换使用的射频仓上。机载电路板是被配置为能够在适当的情况下将发生器输入(例如,RF能量等)与柄部电子器件隔离的分段电路。柔性电路包含具有不同几何形状的电导体以适应RF能量传递。
参考图19,在一些方面,构成外科工具组件1000的模块化部分的喷嘴组件1240可包括轴模块电路,该轴模块电路被独特地构造成能够控制轴组件中的各种功能,同时还与柄部组件500连通,并且允许从电动缝合柄部控制RF发生器400。在图19中,图15的电路在示例性喷嘴组件1240的环境中示出。根据本公开的一些方面的电路包括具有各种连接器的机载电路板1152。凹形连接器410电联接到电路板1152,该电路板允许将凸形插头组件406连接到发生器400(未示出)。
此外,根据一些方面,机载开/关电源开关420电联接到电路板1152,并且被定位成当喷嘴组件1240附接到柄部组件500时被按压。例如,当喷嘴组件锁定到位时(参见例如图9),开/关电源开关420可以被定位成面向柄部组件的近侧,并且可以随着喷嘴组件经由闭合连接件514(见图9)滑入柄部组件的狭槽中而被按压。在其他情况下,开/关电源开关420被暴露,使得其可被外科工具组件1000的操作者手动按压。
电路板1152包括机载连接器1154,该机载连接器被配置为能够与外壳连接器562(参见图9)交接,该外壳连接器与容纳在柄部组件500中的微处理器560连通。这样,柄部组件500能够命令控制喷嘴组件1240中的若干功能的电路板1152。喷嘴组件1240中的电路的设计允许操作者从柄部组件500的各种控件执行许多功能,诸如通过柄部组件500中可用的各种控件和显示控制台。
电路板1152还包括构造成能够与滑环组件1150交接的近侧连接器1153。即使当轴旋转时也可以向端部执行器提供电力,这是由于在柔性轴电路带1164在近侧封闭管1910内旋转时,在整个滑环组件1150上都提供了电力,并且远侧连接器1162与滑环组件持续接触。轴电路带1164可包括多个电导体,诸如用于缝合相关活动的窄电导体1166和用于RF目的的较宽电导体1168(参见图15)。
基于喷嘴组件1240中描述的各种部件,电路1152可以被配置为能够从电动柄部组件500控制RF发生器400,从而允许与柄部组件500的各种功能和接口进行通信,并允许从柄部组件500操作端部执行器的RF和缝合功能。其他功能可包括控制用于在端部执行器处执行各种外科手术和能量施加的算法的类型,启用在喷嘴组件1240的任何部分的柄部组件500处可见的警告功能以及改变来自RF发生器400的能量调制。在一些方面,电路板1152可以被编程为有利于这些功能,而在其他情况下,机载连接器1154可以允许柄部组件电路被编程以有利于这些功能,并且电路板1152被配置为能够相应地命令端部执行器。
在一些方面,机载电路包括分段RF电路1160,其可以允许将发生器400的RF能量经由滑环组件(参见,例如图15)供应到柔性轴电路带。RF发生器可以经由RF分段电路1160联接到机载电路板1152。开/关电源开关420可以类似地连接到分段RF电路1160。
图20示出了根据本公开的一个方面的被编程为与端部执行器3250传送功率和控制信号的外科系统3200的框图。在一个示例性方面,外科系统3200可以包括控制电路3210(例如,微处理器560、分段RF电路1160或远侧微芯片1740),该控制电路具有电外科能量控制段(或RF能量控制段)3220和轴控制段3230(例如,轴段(段5)、马达电路段(段7)或动力段(段8))。控制电路3210可以被编程为向端部执行器3250(例如,端部执行器1500)中的电极提供电外科能量(例如,RF能量)。外科系统3200可以包括用于从电外科能量发生器3240(例如,RF发生器400)向端部执行器3250提供电外科能量的一个或多个电导体3260(例如,电导体1168)。一个或多个电导体3260可以电连接在端部执行器3250和控制电路3210(例如,电外科能量控制段3220和轴控制段3230)之间。
电外科能量控制段3220可以被编程为通过一个或多个电导体3260向电极提供电外科能量。在一个示例性方面,轴控制段3230可以被编程为通过一个或多个电导体3260向端部执行器3250(和/或外科工具组件1000、轴组件704)提供控制信号和/或从该端部执行器接收控制信号。即,一个或多个电导体3260不仅可以用于向端部执行器3250提供电外科能量,而且还用于与端部执行器3250传送控制信号。在一个示例性方面,电外科能量控制段3220和轴控制段3230的至少一些部分可以彼此电隔离。
在一个示例性方面,例如,当通过一个或多个电导体3260向端部执行器3250中的电极提供电外科能量时,电外科能量控制段3220可将一个或多个电导体3260与轴控制段3230电隔离。在一个示例性方面,通过控制线3280提供用以将一个或多个电导体3260与轴控制段3230电隔离的信号,电外科能量控制段3220可以控制位于一个或多个电导体3260与轴控制段3230之间的开关3270。开关3270可以被配置为能够在打开状态和闭合状态之间切换。轴控制段3230和一个或多个电导体3260可以在开关3270处于打开状态时被电隔离,并且可以在开关3270处于闭合状态时处于电连通。在另一个示例性方面,电外科能量控制段3220可以采用任何其他合适的方式将一个或多个电导体3260与轴控制段3230电隔离。开关3270的其他构型可通过闭合开关3270来使一个或多个电导体3260与轴控制段3230电隔离。
在一个示例性方面,当控制电路3210(例如)通过连续检查连接器3265或感测电外科能量的施加而检测到电外科能量发生器3240连接到连接器3265(例如,凹形连接器410)时,电外科能量控制段3220可将一个或多个电导体3260与轴控制段3230电隔离。例如,当将凸形插头组件406插入凹形连接器410中时,电外科能量控制段3220可将电导体3260与轴控制段3230隔离。在另一个示例性方面,当将电外科能量提供给端部执行器3250时或在任何其他合适的时刻,电外科能量控制段3220可将一个或多个电导体3260与轴控制段3230电隔离。
在一个示例性方面,外科系统可包括用于操作端部执行器3250(和/或外科工具组件1000、轴组件704)的一个或多个电导体3290(例如,电导体1166)。在一个示例性方面,一个或多个电导体3290可不用于将电外科能量递送到端部执行器3250。轴控制段3230可以被编程为通过一个或多个电导体3290向端部执行器3250提供控制信号和/或从该端部执行器接收控制信号。在一个示例性方面,当开关3270处于打开状态时(例如,当电外科能量控制段3220正在通过一个或多个电导体3260将电外科能量提供给端部执行器3250时),轴控制段3230可以使用一个或多个电导体3290向端部执行器3250提供控制信号和/或从该端部执行器接收控制信号。在一个示例性方面,当开关3270处于闭合状态时,轴控制段3230还可以使用一个或多个电导体3290向端部执行器3250提供控制信号和/或从该端部执行器接收控制信号。
开关3270可以是晶体管开关、机械开关或任何其他合适的开关。在一个示例性方面,通过电导体3260、3290在控制电路3210与端部执行器3250(和/或外科工具组件1000、轴组件704)之间传递的控制信号包括但不限于用于以切割和/或凝固操作模式驱动端部执行器3250(和/或外科工具组件1000、轴组件704),测量外科系统3200和/或夹持在端部执行器3250中的组织的电特性,提供使用反馈,传递传感器信号以及识别端部执行器3250的某些特性(例如,使用/未使用状态)的信号。
因此,本公开的各方面可有利地减少在控制电路3210和端部执行器3250(和/或外科工具组件1000、轴组件704)之间传递控制信号所需的电导体的数量,具体是通过当用于递送电外科能量的电导体(例如,电导体3260)中的一些不用于电外科能量时使用这些电导体来传达控制信号。此外,通过当通过这些电导体提供电外科能量时将这些电导体与其他电路段(例如,轴控制段3230)隔离,本公开的方面可以防止电外科能量流入连接到那些电路段的其他电路段和/或电导体(例如,电导体3290),防止损坏那些电路段和/或电导体。

实施例2.根据实施例1所述的控制电路,还包括将所述控制电路电连接到端部执行器的第一电导体;其中所述轴控制段被配置为能够通过所述第一电导体将用于操作所述端部执行器的控制信号提供到所述端部执行器;并且其中所述电外科能量控制段被配置为能够通过所述第一电导体将所述电外科能量提供到至少一个电极。
实施例3.根据实施例2所述的控制电路,其中,所述电外科能量控制段被配置为能够当将所述电外科能量提供到位于所述端部执行器中的至少一个电极时,将所述第一电导体与所述轴控制段电隔离。
实施例4.根据实施例3所述的控制电路,还包括电联接在所述电外科能量控制段和所述轴控制段之间的开关,其中所述电外科能量控制段被配置为能够通过控制所述开关来将所述第一电导体与所述轴控制段电隔离。

实施例12.一种外科系统的喷嘴组件,所述喷嘴组件包括:机载电路板;机载连接器,所述机载连接器联接到所述机载电路板并且朝近侧定位在所述喷嘴组件上,所述机载连接器被配置为能够在所述喷嘴组件附接到柄部组件时与所述柄部组件的外壳连接器交接;轴附接凸耳,所述轴附接凸耳朝近侧定位在所述喷嘴组件上并且被配置为能够联接到所述柄部组件的附接支架以将所述喷嘴组件附接到所述柄部组件;以及控制电路,所述控制电路包括:轴控制段;电外科能量控制段;以及连接器,所述连接器联接到所述电外科能量控制段,所述连接器被配置为能够联接到电外科发生器;其中所述轴控制段被配置为能够:与所述外科器械的柄部部分通信;以及接收使用者输入控制;其中所述电外科能量控制段被配置为能够:检测所述电外科发生器与所述连接器的连接;与所述电外科发生器通信;当检测到所述电外科发生器与所述连接器的所述连接时,将所述柄部控制段与所述电外科能量控制段电隔离;以及通过第一组电导体将电外科能量从所述电外科发生器提供到所述外科器械的端部执行器部分;其中所述喷嘴组件能够从所述柄部组件拆卸并且能够附接到所述柄部组件。
实施例13.根据实施例12所述的喷嘴组件,还包括:电外科发生器连接器,所述电外科发生器连接器电联接到所述机载电路板并被配置为能够联接到电外科发生器的插头组件,使得所述机载电路板从所述电外科发生器接收电外科能量。
实施例14.根据实施例12至实施例13中的一项或多项所述的喷嘴组件,其中,所述机载电路板被配置为能够经由所述一个或多个电导体将所述电外科能量传输到所述端部执行器。
实施例15.根据实施例12至实施例14中的一项或多项所述的喷嘴组件,还被配置为能够通过所述柄部组件的外壳连接器和所述喷嘴组件的所述机载连接器之间的接口接收从所述柄部组件到端部执行器的指令。
实施例16.根据实施例15所述的喷嘴组件,还被配置为能够通过所述外壳连接器和所述机载连接器之间的所述接口从所述柄部组件的微处理器接收所述指令。

实施例18.根据实施例12至实施例17中的一项或多项所述的喷嘴组件,还包括滑环组件,所述滑环组件朝远侧定位到所述机载电路板并被配置为能够与所述机载电路板交接。
实施例19.根据实施例18所述的喷嘴组件,还包括:近侧连接器,所述近侧连接器联接到所述机载电路板的远侧端部和所述滑环组件的近侧端部;以及远侧连接器,所述远侧连接器被配置为能够与所述滑环组件的远侧端部交接并且联接到所述一个或多个电导体。
实施例20.根据实施例12至实施例19中的一项或多项所述的喷嘴组件,还包括被配置为能够容纳所述一个或多个电导体的柔性轴电路带。
实施例21.根据实施例12至实施例20中的一项或多项所述的喷嘴组件,其中,所述一个或多个电导体包括:第一电导体,所述第一电导体被配置为能够将能量递送到所述端部执行器以用于缝合器功能;以及第二电导体,所述第二电导体被配置为能够将电外科能量递送到所述端部执行器以用于电外科功能。

Claims (21)

1.一种用于外科器械的控制电路,所述控制电路包括:
轴控制段,所述轴控制段被配置为能够与所述外科器械的柄部部分通信以及接收使用者输入控制;
电外科能量控制段;和
连接器,所述连接器联接到所述电外科能量控制段,其中所述连接器被配置为能够将所述电外科能量控制段联接到电外科发生器;
其中所述电外科能量控制段被配置为能够:
检测所述电外科发生器与所述连接器的连接;
与所述电外科发生器通信;
当检测到所述电外科发生器与所述连接器的所述连接时,将所述轴控制段与所述电外科能量控制段电隔离;以及
通过第一组电导体将电外科能量从所述电外科发生器提供到所述外科器械的端部执行器。
2.根据权利要求1所述的控制电路,还包括第一组电导体的将所述控制电路电连接到端部执行器的第一电导体;
其中所述轴控制段被配置为能够通过所述第一电导体将用于操作所述端部执行器的控制信号提供到所述端部执行器;并且
其中所述电外科能量控制段被配置为能够通过所述第一电导体将所述电外科能量提供到至少一个电极。
3.根据权利要求2所述的控制电路,其中,所述电外科能量控制段被配置为能够当将所述电外科能量提供到位于所述端部执行器中的至少一个电极时,将所述第一电导体与所述轴控制段电隔离。
4.根据权利要求3所述的控制电路,还包括电联接在所述电外科能量控制段和所述轴控制段之间的开关,其中所述电外科能量控制段被配置为能够通过控制所述开关来将所述第一电导体与所述轴控制段电隔离。
5.根据权利要求4所述的控制电路,其中,所述电外科能量控制段被配置为能够通过打开所述开关来将所述第一电导体与所述轴控制段电隔离。
6.根据权利要求2所述的控制电路,还包括第一组电导体的第二电导体,其中所述轴控制段被配置为能够通过所述第二电导体将所述控制信号提供到所述端部执行器,并且其中所述轴控制段被配置为能够,当所述电外科能量控制段正在通过所述第一电导体将所述电外科能量提供到所述至少一个电极时,通过所述第二电导体将所述控制信号提供到所述端部执行器。
7.根据权利要求2所述的控制电路,其中,所述轴控制段被配置为能够接收用以操作所述端部执行器的可执行指令。
8.根据权利要求2所述的控制电路,其中,所述轴控制段被配置为能够协调缝合功能和能量递送功能。
9.根据权利要求1所述的控制电路,其中,所述轴控制段被配置为能够提供警告信号。
10.根据权利要求1所述的控制电路,其中,所述轴控制段被配置为能够将指令发送到端部执行器。
11.根据权利要求1所述的控制电路,还包括联接到所述轴控制段和所述电外科能量控制段的滑环组件。
12.一种喷嘴组件,所述喷嘴组件被配置为能够与外科工具组件一起使用,所述喷嘴组件包括:
轴附接凸耳,所述轴附接凸耳被配置为能够选择性地将可互换轴组件联接到工具机架组件的附接支架,其中所述可互换轴组件包括具有电极的端部执行器,和
控制电路,所述控制电路被分段并且包括:
轴控制段,所述轴控制段包括第一连接器,所述第一连接器被配置为能够将所述控制电路连接到外科工具组件的控制模块,其中所述轴控制段被配置为能够将控制信号发送到端部执行器;和
电外科能量控制段,所述电外科能量控制段包括第二连接器,
所述第二连接器被配置为能够将所述控制电路连接到电能量发生器,其中所述控制电路被配置为能够检测所述电能量发生器与所述第二连接器的连接并且在所述控制电路检测到所述电能量发生器的连接时,所述电外科能量控制段被配置为能够将所述轴控制段与从所述电能量发生器接收到的电能电隔离。
13.根据权利要求12所述的喷嘴组件,其中,所述电外科能量控制段被配置为能够将所述电能量发生器生成的电能传输到所述端部执行器。
14.根据权利要求12所述的喷嘴组件,还被配置为能够通过外科工具组件的外壳连接器和所述喷嘴组件的所述控制电路的第一连接器之间的接口从外科工具组件接收指令来控制端部执行器。
15.根据权利要求14所述的喷嘴组件,从所述外科工具组件的微处理器接收所述指令。
16.根据权利要求12所述的喷嘴组件,其中,所述喷嘴组件还包括电源开关,所述电源开关电联接到所述控制电路,其中所述电源开关被配置为能够激活和停用电外科能量的传输。
17.根据权利要求12所述的喷嘴组件,还包括滑环组件,所述滑环组件朝远侧定位到所述控制电路并被配置为能够与所述控制电路交接。
18.根据权利要求17所述的喷嘴组件,还包括:
近侧连接器,所述近侧连接器联接到所述控制电路的远侧端部和所述滑环组件的近侧端部;和
远侧连接器,所述远侧连接器被配置为能够与所述滑环组件的远侧端部交接并且联接到一个或多个电导体。
19.根据权利要求18所述的喷嘴组件,还包括被配置为能够容纳一个或多个电导体的柔性轴电路带。
20.根据权利要求19所述的喷嘴组件,其中,一个或多个电导体包括:
第一电导体,所述第一电导体被配置为能够将能量递送到所述端部执行器以用于缝合器功能;和
第二电导体,所述第二电导体被配置为能够将电外科能量递送到所述端部执行器以用于电外科功能。
21.根据权利要求20所述的喷嘴组件,还包括电联接在所述电外科能量控制段和所述轴控制段之间的开关,其中所述电外科能量控制段被配置为能够通过控制所述开关来将所述第一电导体与所述轴控制段电隔离。
