Description

Traditional Chinese medicine composition for treating diabetic peripheral neuropathy
Technical Field
The invention belongs to the technical field of traditional Chinese medicines, and particularly relates to a traditional Chinese medicine composition for treating diabetic peripheral neuropathy.
Background
Diabetic Peripheral Neuropathy (DPN) is a neuropathy caused by metabolic and microvascular lesions due to chronic hyperglycemia, and is characterized clinically by symmetric or asymmetric numbness, tingling, burning, formication, hypoesthesia or loss of socks-like sensation in both lower limbs, and hyporeflexion or disappearance of achilles tendons and knee tendons. DPN has the characteristics of high morbidity, early appearance time, more clinical symptoms, difficult disease spreading and the like, and brings great mental and physical pains to patients. Because the pathogenesis is unclear, no specific treatment method exists at present. DPN is a common chronic complication of diabetics, the pathogenesis of the DPN is not completely clarified, the clinical manifestations mainly appear at the earliest in sensory neuropathy, and also involve other nervous systems of motor nerves and vegetative nerves, the main symptoms are pain, numbness, nerve dysfunction and dyskinesia of limbs, and the quality of life and the life span of the diabetics are seriously influenced. The main pathological basis of DPN is hyperglycemia caused by diabetes, and is currently believed to be mainly associated with factors such as metabolic abnormal mechanisms, microangiopathy, advanced glycosylation endproducts, nitration reaction, endoplasmic reticulum stress, oxidative stress and mitochondrial dysfunction, deficiency of growth factors (N F), inflammatory response, and the like. The Western medicine mainly treats DPN according to symptoms such as blood sugar control, risk factor control, neuropathy blockage, microcirculation improvement, pain control and the like.
Clinically, the fact that DPN is mostly seen in the middle and later stages of disease development is observed, and the theory accords with the traditional Chinese medicine theory of 'chronic disease and kidney' and 'chronic disease and stasis', although the clinical manifestations of DPN are complex and changeable, the affected parts of DPN are mainly lower limbs, except common symptoms such as numbness, pain and muscular atrophy, the DPN also has the kidney deficiency manifestations such as soreness and weakness of waist and knees, intolerance of cold and cold limbs, impotence of men and female hyposexuality, and the theory accords with the theory of 'house of waist and kidney of people', 'below waist and knee and kidney qi of traditional Chinese medicine'. DPN is mostly caused by long course of disease, or poor disease control, prolonged course of disease, gradual aggravation of kidney deficiency, insufficient functions of various organs, incapability of promoting blood circulation, slow blood flow, blood stasis in veins and pain caused by obstruction of collaterals; laboratory examination finds that the rheological property of the blood of a patient is abnormal, the blood is viscous, the deformability of red blood cells is reduced, the platelet aggregation is enhanced, the microcirculation perfusion is insufficient, and the like, namely invisible stasis. In conclusion, the modern pharmacological research results are combined, the prescription combination is optimized under the guidance of the above thought, and the kidney tonifying and collateral dredging medicine is added on the basis of the angelica sinensis Sini decoction, so that the curative effect is obviously improved. The results of the invention show that the hemorheological indexes, TCSS scores and the conduction speeds of the peroneal nerve and the tibial nerve of the patients in the traditional Chinese medicine group after treatment are all superior to those in the conventional group (P is less than 0.05)), and the traditional Chinese medicine composition has a good curative effect on treating diabetic peripheral neuropathy, can effectively improve the blood running condition of the patients, promotes the recovery of nerve functions, improves the symptoms and physical signs of the patients, and is worthy of popularization and application.

1. Data and method
1.1 general data
110 cases of diabetic peripheral neuropathy patients treated in the endocrinology department in 2015 6-2018 and 12 are selected and are averagely divided into a control group (55 cases) and a treatment group (55 cases) according to the sequence of the treatment time of the patients. In the conventional group, 30 male patients and 25 female patients are aged 45-71 years, the average (59.36 +/-3.57) years, the course of disease is 5-10 years, and the average course of disease is (5.79 +/-1.26) years. 31 male patients and 24 female patients in the traditional Chinese medicine are aged 42-72 years, the average (59.58 +/-3.42) years, the course of disease is 1-14 years, and the average course of disease is (5.64 +/-1.33) years. The general data of the patients were compared and the differences between the groups were found to be statistically insignificant (P > 0.05).
1.2 inclusion and exclusion criteria
Patients are admitted to the system and are diagnosed according to relevant diagnosis standards proposed by WHO in 1999, and the patients know the information and agree to participate in the study, and have signed informed consent, patients with mental or intellectual abnormality, patients allergic to western medicines and Chinese medicines used in the invention, and patients with malignant tumor or cardiovascular and cerebrovascular diseases.
1.3 methods of treatment
The patients are treated by conventional blood sugar control treatment including diet control and exercise, and the conventional group is administered with mecobalamin (Wei material (China) pharmaceutical industry Co., Ltd., national Standard H20030812, specification: 0.5mg × 20s) orally 1 tablet at a time 3 times a day. The traditional Chinese medicine is treated by using the hebetor granules on the basis of conventional blood sugar control measures, the oral administration dosage is 9g per day, and the traditional Chinese medicine is taken warm in the morning, at noon and at night in 3 times. Treatment was continued for 1 month in all 2 groups.
1.4, observation index
Post-treatment hemorheological indices including Plasma Viscosity (PV), Hematocrit (HCT), whole blood high-shear viscosity (HBV), whole blood high-shear viscosity (LBV) were determined and compared for 2 groups of patients. The severity of peripheral neuropathy after treatment of the patients is compared, and the TCSS scoring system is adopted for scoring, wherein the TCSS scoring system comprises 3 dimensions of symptoms (6 points), feelings (5 points) and reflexes (8 points), the total score is 19 points, and the higher the score of the patients is, the more serious the disease condition is. The sensory nerve conduction velocity (MCV) and motor nerve conduction velocity (SCV) of the peroneal nerve and the tibial nerve of 2 groups of patients after treatment were measured using an electromyogram evoked potential apparatus and compared.
1.5 statistical methods
The data from this study were analyzed using SPSS 20.0 statistical software, and the counts and measures were expressed as rates (%) and means. + -. standard deviations ( X. + -.s), respectively, and as χ2And (5) checking and t-checking. If the test result is that P is less than 0.05, the difference is statistically significant.
2. Results
2.1, 2 groups of patients have lower indexes of PV, HCT, HBV and LBV than the indexes of conventional groups after treatment compared with the indexes of the hemorheology indexes of the traditional Chinese medicine group patients, and the difference has statistical significance (P is less than 0.05), which is shown in Table 1.
2.2 and 2 groups of patients have TCSS scores after treatment, and TCSS symptom scores, feeling scores and reflex scores after treatment of traditional Chinese medicine group patients are all obviously lower than those of the conventional group, and the differences have statistical significance (P is less than 0.05), which is shown in Table 2.
2.3 and 2 groups of patients suffered from the traditional Chinese medicine group after nerve conduction velocity comparison treatment, the MCV and SCV of the peroneal nerve and the tibial nerve of the patients are obviously larger than those of the conventional group, and the difference has statistical significance (P is less than 0.05), which is shown in Table 3.
The clinical curative effect of 2.4 and 2 groups of patients is obviously better than that of a control group in comparison with the overall curative effect of a treatment group, and the clinical curative effect is better. The total effective rate of the treatment group is obviously higher than that of the control group, and the effective rate and the effective number of the treatment group are both higher than those of the control group, which is shown in table 4.
