背景技术

在最近的一项专门针对MCC的2期临床研究中,25名晚期MCC患者接受了至少一剂派姆单抗并进行治疗反应的评估(Nghiem,N Engl J Med.374:2542-52(2016))。所有患者均具有远处转移的或局部复发的MCC,其不适合进行确定性手术或放疗。每三周以2mg/kg的剂量静脉施用派姆单抗,并且治疗持续最长2年,或直到出现完全反应、进行性疾病或剂量限制性毒性作用。本研究的客观反应率为56%,其中4名患者显示出完全反应,且10名患者显示出部分反应。在表现出客观反应的患者中,反应持续时间的范围从最小的至少2.2个月至最大的至少9.7个月的持续时间。反应在MCV-阳性肿瘤(16名患者中的10名)和MCV-阴性肿瘤(9名患者中的4名)中观察到。在4名患者中观察到3级或4级的治疗相关的不良反应事件,其中最严重的AE包括心肌炎以及天冬氨酸和丙氨酸转氨酶水平升高。
当前的MCC治疗是无效、部分有效的或导致不良副作用。因此,需要MCC的其他治疗方法。
发明内容

具体实施方式
由于这种癌症的病毒起源,免疫疗法可能是用于研究治疗梅克尔细胞癌的有希望的途径。本文提供了治疗梅克尔细胞癌的方法。该方法包括选择患有梅克尔细胞癌的受试者,并向该受试者施用治疗有效量的NK-92细胞,其中该施用治疗该受试者中的梅克尔细胞癌。

NK-92细胞表现出对多瘤病毒阳性MCC细胞系的细胞毒性活性。图1和2显示了NK-92细胞以不同的效应-靶比率过夜暴露于三种MCC细胞系(MKL-1、WaGa和MS-1)后,NK-92细胞的细胞毒性的结果。人CML细胞系K562作为对照,因为它总是被NK-92细胞杀死。具体而言,根据制造商的说明(Sigma Aldrich,St.Louis,MO)将K562、MKL-1、MS-1和WaGa细胞(靶细胞)用膜染料PKH67-GL预染色,然后以10e5/ml的细胞密度重悬于RPMI 1640+10%FBS中。将NK-92细胞(效应细胞)以10e6/ml的细胞密度重悬于X-Vivo10+5%HS+IL-2(500IU/ml)中。将靶细胞和效应细胞在96孔板中以10:1、5:1、2.5:1、1.25:1的效应-靶(E:T)比率混合,其最终体积为200ul/孔。包括仅靶细胞对照以测定自发死亡背景。将该平板在37℃的CO2培养箱中孵育4小时或24小时,然后用碘化丙锭(0.1μg/ml)将细胞染色10分钟。通过流式细胞术分析样品,并按如下方式计算细胞毒性百分比:%杀伤=[((样品中的%PKH+/PI+)–(仅靶细胞中的%PKH+/PI+)]/[100-(仅靶细胞中的%PKH+/PI+)]*100。图1显示4小时时的细胞毒性,和图2显示24小时时的细胞毒性。
实施例2.使用NK-92细胞在体内治疗梅克尔细胞癌(MCC)。
用NK-92细胞治疗一位81岁的男性患者,该患者患有头皮上的复发性MCC,具有至少3个皮肤转移。先前的疗法包括手术、辅助放疗(RT)、病灶内干扰素(IFN)加RT加局部咪喹莫特、抗PD-1治疗、病灶内TLR-4激动剂、中子和奥曲肽(octreotide)长效释放(LAR)的RT。在第一周期中第1天,患者接受了2×109细胞/m2的NK-92静脉输注。在第一周期的第2天,患者接受了2×109细胞/m2的第二次NK-92输注。该周期重复八次,每个周期之间间隔两周。患者实现了完全反应(CR),MCC肿瘤得到了完全解决。NK-92治疗耐受,无明显不良反应。
用NK-92细胞治疗了一位患有大腿的进行性MCC的75岁男性。先前的疗法包括化疗和抗PD-1疗法。在第一周期第1天,患者接受了2×109细胞/m2的NK-92静脉输注。在第一周期的第2天,患者接受了2×109细胞/m2的第二次NK-92输注。该周期重复第二次;然而,由于缺乏疾病状态的显著变化,治疗被中止。
实施例3.MCC患者的治疗
用NK-92细胞治疗三名患者。根据实体肿瘤反应评估标准(Response EvaluationCriteria in Solid Tumors,RECIST),这些患者均患有不可切除的III期(IIIB)或远处转移(IV期)MCC。每2周在连续的两天(=1个周期)通过静脉输注以2×10e9细胞/m2的剂量向患者给予NK-92细胞,进行共8个周期(16次输注)。自治疗开始的4个月,对患者进行监测并评估无进展生存率(Progression Free Survival)。初步数据表明,NK-92细胞疗法取得了有益的临床结果。
Claims (18)
1.一种治疗受试者中梅克尔细胞癌的方法,其中所述方法包括:
(a)选择患有梅克尔细胞癌的受试者;
(b)向所述受试者施用治疗有效量的NK-92细胞,其中所述施用治疗所述受试者中的梅克尔细胞癌。
2.如权利要求1所述的方法,其中所述受试者先前已接受放射疗法、手术、化学疗法、抗-PD-1疗法或其任何组合。
3.如权利要求1或2所述的方法,其中所述梅克尔细胞癌是转移性的。
4.如权利要求1-3中任一项所述的方法,其中向所述受试者施用1×103至1×1010/m2的所述NK-92细胞。
5.如权利要求1-3中任一项所述的方法,其中向所述受试者施用2×109/m2的所述NK-92细胞。
6.如权利要求1-5中任一项所述的方法,其中所述NK-92细胞肠胃外施用。
7.如权利要求1-5中任一项所述的方法,其中所述NK-92细胞静脉内施用。
8.如权利要求1-5中任一项所述的方法,其中所述NK-92细胞肿瘤周施用。
9.如权利要求1-8中任一项所述的方法,其中所述NK-92细胞通过一段时间内的输注施用于所述受试者。
10.如权利要求9所述的方法,其中所述时间在5至130分钟之间。
11.如权利要求9所述的方法,其中所述时间在90至120分钟之间。
12.如权利要求9所述的方法,其中所述时间在15至30分钟之间。
13.如权利要求1-12中任一项所述的方法,其中所述梅克尔细胞癌是由梅克尔细胞多瘤病毒引起的。
14.如权利要求1-12中任一项所述的方法,其中所述梅克尔细胞癌不是由梅克尔细胞多瘤病毒引起的。
15.如权利要求1-14中任一项所述的方法,其中所述受试者中的梅克尔细胞癌对化学疗法具有抗性。
16.如权利要求1-15中任一项所述的方法,其中所述NK-92细胞每天向所述受试者施用一次,持续1、2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20天或更多天。
17.如权利要求1-15中任一项所述的方法,其中所述NK-92细胞以每天一次持续两天的周期施用。
18.如权利要求17所述的方法,其中所述NK-92细胞以1、2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20或更多个周期施用。
