本发明公开了一种检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,包括以下步骤:通过检测TGF‑β1的试剂盒检测待测物血浆中TGF‑β1的含量,通过检测IL‑2、IL‑4、IL‑5和IL‑13的试剂盒检测待测物血浆中IL‑2、IL‑4、IL‑5和IL‑13的含量;当血浆中IL‑4、IL‑5、IL‑13比例升高,而IL‑2比例下降,造成Th1/Th2失衡,则为喘息性疾病患者;当TGF‑β1含量相比于正常人升高,则喘息性疾病患者存在气道重塑的高危因素。本发明通过检测血浆中TGF‑β1、IL‑2、IL‑4、IL‑5和IL‑13的含量,判断Th1/Th2是否失衡,进行确定是否为婴幼儿喘息性疾病患者,其检测方法灵敏度高,简单可靠。

背景技术
喘息是婴幼儿最常见的呼吸道症状,近1/3儿童在3岁之前至少有过一次喘息发作,6岁时有近一半的儿童有过喘息发作,最终高达40%的儿童可发展为支气管哮喘。目前对婴幼儿喘息性疾病概念和分类尚不统一,包括毛细支气管炎、喘息性支气管炎、哮喘等多种疾病。根据婴幼儿喘息的病因及转归,2008年中华医学会儿科学分会呼吸学组制定了《儿童支气管哮喘诊断与防治指南》,将5岁以下儿童喘息分成3种临床表型:(1)早期一过性喘息;(2)早期起病的持续性喘息(指3岁前起病);(3)迟发性喘息/哮喘。第1、2种类型的小儿喘息只能通过回顾性分析才能做出鉴别。由此可见,婴幼儿喘息与哮喘密切相关,喘息婴幼儿中存在哮喘儿童。
哮喘是儿童最常见的慢性呼吸道疾病,严重危害儿童的身心健康。近年来其发病率及患病率在全球范围内呈上升趋势,据报告,全球11.6%的6至7岁儿童存在哮喘症状。根据2010年完成的我国第三次城市儿童哮喘流行病学调查,我国城市儿童哮喘的平均患病率已达3.02%,其中,婴幼儿和学龄前儿童的总患病率分别为1.77%和4.15%,能够得到合理治疗的患儿不足30%,一些不及时和不适当的治疗最终导致这些患儿在成年后发展为成年性哮喘,给家庭和社会带来了巨大的精神和经济负担。因此,从喘息的学龄前儿童中把可能发展为哮喘的患儿识别出来,早期进行有效干预十分必要。然而,目前婴幼儿喘息性疾病的发病机制尚未清楚。
与其他年龄组相比,学龄前儿童(年龄≤5岁)有较高的哮喘发病率,据统计,学龄前喘息儿童于急诊科就诊的年增长率在23~42‰,相比之下6到70岁年龄组的年增长率为15‰。学龄前儿童的喘息占用了大量医疗资源,也给人们带来了诊断方面的挑战。在发展中国家,学龄前儿童喘息常被误诊为肺炎进行治疗,导致这个年龄组死亡率增加,这些被诊断为复发性肺炎的患儿往往在后来被诊断为哮喘。
在过去的15年中,许多基于纵向症状的分类虽然对婴幼儿喘息流行病学起到了一定的帮助,Castro-Rodriguez及同事使用Tucson出生队列,根据喘息发作的年龄、频率,父母的哮喘史,与感冒无关的喘息,湿疹,过敏性鼻炎以及外周血EOS计数提出了哮喘预测指数(the asthma predictive index,API),分别使用严格和不严格的指标以更好的预测其6岁时的结局,结果显示在6~13岁时有哮喘症状者中,不严格指标阳性的儿童有59%、严格指标阳性的儿童为76%,严格指标阴性的儿童95%以上在6~13岁时不发生哮喘。我国2008年制定的《儿童支气管哮喘诊断与防治指南》[6],也提出了哮喘预测指数以预测3岁内喘息儿童发展为持续性哮喘的危险性,包括在过去1年内喘息≥4次,具有1项主要危险因素或2项次要危险因素,主要危险因素指:(1)父母有哮喘病史,(2)经医生诊断的特应性皮炎,(3)有吸入变应原致敏的依据;次要危险因素:(1)有食物变应原过敏的依据,(2)外周血EOS≥4%,(3)与感冒无关的喘息。然而,由于婴幼儿喘息性疾病的发病机制尚未清楚,其与哮喘发病机制关系的研究也较少,缺乏前瞻性的可靠生物标志及预测因素作为确定哮喘发生的标准,使得API在临床应用存在一定的局限性,这也使从喘息婴幼儿中识别出有可能发展成为哮喘的儿童变得非常困难。
哮喘由免疫、遗传和环境因素等共同作用引起。哮喘的本质是多种炎性细胞(包括EOS、肥大细胞、T淋巴细胞、中性粒细胞)、气道结构细胞(气道平滑肌细胞、上皮细胞等)及其分泌的细胞组分(细胞因子、粘附分子、趋化因子等)共同参与的慢性气道炎症,气道重塑(airway remodeling)以慢性气道炎症为发生基础。反复多次喘息的婴幼儿发展为哮喘可能性更大,喘息婴幼儿生命早期已存在气道炎症,甚至已发生气道重塑,不管是特应性体质的喘息患儿还是非特应性体质的喘息患儿,都存在类似哮喘的气道病理改变,但其发生机制尚未完全清楚。目前可用于哮喘气道炎症评估的方法有:痰液的气道炎症标志物检测、呼出气一氧化氮(fractional exhaled nitric oxide,FENO)浓度测定、支气管肺泡灌洗及支气管粘膜活检,后两者是评估和诊断哮喘气道炎症和气道重塑的金标准,但均为侵袭性检查,临床使用受到限制;前两者为无创性检查,其中FENO测定的敏感性和特异性已得到证实,但目前在国内尚未广泛应用,且6岁以下儿童对呼气流速的控制能力欠缺,应用有一定的限制。
辅助T细胞分为1型辅助T细胞(Th1)和2型辅助T细胞(Th2),Th1诱导细胞免疫,Th2诱导体液免疫。通常认为,Th1/Th2平衡,当状态倾向于Th1时,细胞免疫变得过强,容易引起风湿性关节炎等自身免疫性相关的疾病,倾向于Th2时,体液免疫变得过强,容易陷入癌症、免疫缺陷、皮肤炎、过敏症状、肾炎、感染症等疾病。IL-2为代表性的Th1细胞因子,能促进所有亚型T细胞的增殖及产生细胞因子,增强NK细胞胞毒活性。IL-5与EOS的活化、气道炎症、气道高反应性(airway hyperresponsiveness,AHR)及IgE的合成密切相关。IL-13在哮喘炎症过程中起主导作用,它可以单独介导并引起哮喘,与气道的炎症反应和免疫调节有关。转化生长因子-β1(transforming growth factor-β1,TGF-β1)具有强大的促炎及抗炎作用,被认为是哮喘气道炎症和气道重塑中最重要的炎性介质。IL-2、IL-4、IL-5、IL-13、TGF-β1及IgE作为哮喘发病过程中重要的因子,通过错综复杂的机制及相互作用参与了哮喘免疫学发病机制、气道炎症及气道重塑的发生发展,目前尚未显现这些因子与婴幼儿喘息性疾病之间的关系。
发明内容
本发明要解决的技术问题是克服现有技术的不足,提供一种检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,该检测试剂盒以外周血为检测对象,检测灵敏度高,应用广泛。其检测方法简单可行。
为了解决上述技术问题,本发明提供了一种检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,用于婴幼儿喘息性疾病的检测试剂盒包括检测IL-2、IL-4、IL-5和IL-13含量的试剂盒;
所述检测IL-2、IL-4、IL-5和IL-13含量的试剂盒为美国Millipore公司的MILLIPLEX MAP human Th17 Magnetic Bead Panel试剂盒;
所述检测试剂盒的应用方法包括以下步骤:
S1、通过检测IL-2、IL-4、IL-5和IL-13含量的试剂盒检测待测血浆中IL-2、IL-4、IL-5和IL-13的含量;
S2、当血浆中IL-4、IL-5、IL-13比例升高,或IL-2比例下降,造成Th1/Th2失衡,则为喘息性疾病患者。
上述的应用,进一步的,所述检测试剂盒还包括检测TGF-β1含量的试剂盒;所述检测TGF-β1含量的试剂盒为美国R&D公司Human TGF-β1 ELISA试剂盒。

S3、通过检测TGF-β1含量的试剂盒检测喘息性疾病患者血浆中TGF-β1的含量,当TGF-β1含量较未患病时升高,则喘息性疾病患者存在气道重塑的高危因素。

S3-A1、在微型反应孔中加入Assay Diluent RD1-73和待测样品,进行孵育;

S3-A3、加入底物溶液,室温下避光显色;
S3-A4、加入终止液,用酶联仪在450nm波长处测定各孔的OD值。

S1-B1、分别制备IL-2、IL-4、IL-5、IL-13抗体标记的微球;将上述微球混合得到混合微球;
S1-B2、在实验滴定板的孔内加入待测样品和所述混合微球,4℃下摇床过夜;
S1-B3、向孔内加入二抗,室温下摇床孵育;
S1-B4、加入Streptavidin-Phycoerythrin,室温下摇床孵育;
S1-B5、加入鞘液,摇床上震荡使微球充分悬浮;
S1-B6、将滴定板置入Luminex 200TM内测定IL-2、IL-4、IL-5、IL-13的浓度。
上述的应用,进一步的,所述混合微球中,所述IL-2、IL-4、IL-5、IL-13抗体标记的微球的体积比为1︰1︰1︰1。
与现有技术相比,本发明的优点在于:
(1)本发明提供了一种检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,其主要检测外周血的血浆中IL-2、IL-5、IL-13、TGF-β1的含量,申请人发现,Th1/Th2失衡与婴幼儿喘息性疾病存在关联,当血浆来源是婴幼儿喘息性疾病时,白介素-2(interleukin-2,IL-2)为代表的Th1细胞因子在婴幼儿喘息性疾病患者中低表达;而Th2型细胞因子白介素-5(interleukin-5,IL-5)、白介素-13(interleukin-13,IL-13)及转化生长因子-β1(transforming growth factor-β1,TGF-β1)在婴幼儿喘息性疾病患者血清中表现为升高。通过检测这四者的含量,将IL-2的含量除以IL-5、IL-13和TGF-β1含量的总和,若Th1/Th2失衡,为婴幼儿喘息性疾病患者,Th1/Th2没有失衡,则为非婴幼儿喘息性疾病患者。本发明的检测方法,检测灵敏度高,检测方法简单可靠。

实施例
以下实施例中所采用的材料和仪器均为市售。
实施例1
一种用于检测婴幼儿喘息性疾病的检测试剂盒,包括检测TGF-β1含量的试剂盒和检测IL-2、IL-4、IL-5和IL-13含量的试剂盒。
检测TGF-β1含量的试剂盒为美国R&D公司Human TGF-β1 ELISA试剂盒,其主要成分包括:Assay Diluent RD1-73、Wash Buffer、TGF-β1结合物、底物溶液和终止溶液。
所述检测IL-2、IL-4、IL-5和IL-13含量的试剂盒为美国Millipore公司的MILLIPLEX MAP human Th17 Magnetic Bead Panel试剂盒,其主要的成分包括:WashBuffer、Assay Buffer、Streptavidin-Phycoerythrin、鞘液、含IL-2抗体的微球、含IL-4抗体的微球、含IL-5抗体的微球、含IL-13抗体的微球。
实施例2
一种实施例1的检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,包括以下步骤:
(1)研究对象的选择:选取2015年1月至2015年3月期间在中南大学湘雅医院儿科及湖南省儿童医院呼吸科住院部的婴幼儿,年龄为1月~3岁,分为喘息组及正常对照组。
喘息组:
2015年1月至2015年3月期间在中南大学湘雅医院儿科及湖南省儿童医院住院患儿50例,其中男性39例,女性11例,平均年龄14月,诊断均满足《诸福棠实用儿科学》7版中喘息样支气管炎及肺炎的诊断标准,其中诊断为肺炎的病例选取伴有喘息的患儿作为研究对象。
入选标准:(1)足月出生,年龄1月~3岁;(2)伴有咳嗽、喘息、呼气时间延长;(3)肺部可闻及哮鸣音。
排除标准:(1)入院前两周内服激素和白三烯调节剂类药物;(2)心源性哮喘、肺结核、百日咳、支气管异物、支气管肺发育不良、大气道梗阻等其他原因所致的喘息。
1.1、根据既往喘息发作次数,哮喘组又分为首次喘息组(首发组)及反复喘息组(反复组,发作次数≥2次),其中首发组25例,包括男性19例,女性6例,平均年龄10月;反复组25例,包括男性20例,女性5例,平均年龄15月;与正常对照组相比,三组患儿在性别、年龄上无统计学差异(P>0.05)具有可比性。
1.2、根据是否存在哮喘发生的高危因素,哮喘组又分为有高危因素组及无高危因素组,高危因素包括:父母哮喘病史,经医生诊断的特应性皮炎,有吸入变应原或食物变应原致敏的依据,外周血EOS≥4%,与感冒无关的喘息。有上述高危因素之一者纳入有高危因素组,共22例,包括男性16例,女性6例,平均年龄12.3月,包括有湿疹史者11例,对牛奶过敏者2例,家族中有哮喘史者1例,外周血EOS≥4%者7例,同时有湿疹及哮喘家族史者1例;无高危因素组28例,包括男性23例,女性5例,平均年龄12.2月;与正常对照组相比,三组患儿在性别、年龄上无统计学差异(P>0.05)具有可比性。
1.3、根据病原学检测结果,将血清学呼吸道合胞病毒(RSV)IgM抗体阳性、MP-IgM抗体阳性、咽拭子MP培养阳性或痰培养肺炎链球菌阳性者定为病原学阳性组,所有检测均为阴性者定为病原学阴性组。病原学阳性组23例,包括男性17例,女性6例,平均年龄11.5月;病原学阴性组27例,包括男性22例,女性5例,平均年龄13.2月;与正常对照组相比,三组患儿在性别、年龄上无统计学差异(P>0.05)具有可比性。
正常对照组:
性别、年龄相匹配的健康体检婴幼儿25例,均否认个人及家族过敏性疾病,近期无发热及感染性疾病史,其中男性19例,女性6例,平均年龄12.4月。
以上所有研究对象均征得其监护人的知情同意并签字。
(2)标本的采集与处理:
2.1、选用EDTA抗凝管,无菌采取上述研究对象清晨静脉血2ml;
2.2、采血后2小时之内进行标本处理,1600g离心15分钟,离心后管内分为两层,上层为淡黄色血浆,下层主要为红细胞和血小板等;
2.3、吸取上层血浆后,分装于多个EP管中,清楚标记每一份样品编号,在≤-20℃条件下保存。
(3)酶联免疫吸附试验(ELISAs)检测TIgE:
将500μl血浆通过酶联免疫吸附试验(采用美国R&D公司Human TGF-β1 ELISA试剂盒检测)检测外周血TIgE。
(4)外周血EOS计数及病原学检测:
喘息组患儿同时送检外周血EOS计数、血清呼吸道病原体九联检(包括嗜肺军团菌、MP、Q热立克次体、肺炎衣原体、腺病毒、RSV、甲型流感病毒、乙型流感病毒和副流感病毒1、2和3型病原体IgM抗体)、痰培养、咽拭子MP培养,由湘雅医院检验科及儿科实验室完成。
检测结果为:结合血清呼吸道病原体九联检、咽拭子MP培养及痰培养,结果发现RSV检出者6例,MP检出者9例;肺炎链球菌检出者3例,同时检出RSV及MP者3例,同时检出RSV及肺炎链球菌者2例,共计病原学检测阳性者为23例。
(5)酶联免疫吸附试验(ELISAs)检测外周血TGF-β1浓度:
将500μl血浆通过酶联免疫吸附试验(采用美国R&D公司Human TGF-β1 ELISA试剂盒检测)检测外周血TGF-β1浓度,具体步骤为:
5.1、开始试验前先在室温下均匀地充分解冻样品,准备好所有试剂、标准品;
5.2、去除板架上多余的微型反应孔,与干燥剂一同收入铝箔袋并重新密封好;
5.3、向每个孔中加入50μl的Assay Diluent RD1-73;
5.4、分别设标准孔、质控孔、待测样品孔。向相应的孔中分别加50μl标准溶液、50μl质控液、50μl待测样品,并用封板膜封住反应孔,轻轻混匀,室温下孵育2小时。
5.5、每孔加Wash Buffer 400μl洗板4次;最后1次时颠倒反应板在干净的纸巾上拍干;
5.6、依序向每孔加入100μl的TGF-β1结合物,用封板膜封住后室温下孵育2小时;
5.7、重复步骤5.5;
5.8、依序每孔加底物溶液100μl,室温避光显色30分钟;
5.9、依序每孔加终止溶液50μl,终止反应。孔的颜色由蓝色变成黄色;
5.10、在30min内用酶联仪在450nm波长处测定各孔的OD值。
上述步骤中标准溶液、质控溶液、底物溶液、终止溶液均是Human TGF-β1 ELISA试剂盒中的溶液。
(6)液相悬浮芯片技术检测IL-2、IL-4、IL-5及IL-13水平:
采集血浆通过MILLIPLEX MAP human Th17 Magnetic Bead Panel(美国Millipore公司)检测IL-2、IL-4、IL-5及IL-13的浓度,具体过程为:
6.1、充分解冻血浆/血清样品,混匀后3000g离心5分钟;
6.2、根据试剂盒说明书(美国R&D公司)制备质量控制组,标记为C1,C2;
6.3、根据试剂盒说明书(美国R&D公司)制备Wash Buffer;
6.4、根据试剂盒说明书(美国R&D公司)制备血清基质;
6.5、根据说明说书依次制备标准品,分别标记为S0,S1-S7;
6.6、制备抗体标记微球:分别从IL-2、IL-4、IL-5、IL-13的微球瓶中吸取60μl至微球混合瓶中混合,在加入适量的微球稀释液;
6.7、向实验滴定板的所有孔内中加入200μl Assay Buffer预湿,封闭后在室温下置于摇床上震荡10分钟;
6.8、用排枪移除孔中Assay Buffer;
6.9、设置好标准品孔、质量控制组孔及待测样品孔的位置,加入标准品组或质量控制组,每种浓度加25μl,以Assay Buffer作为0pg/mL标准品(背景);
6.10、向每个待测样品孔中加入25μl Assay Buffer;
6.11、向背景孔、标准品孔、质量控制组孔中加入25μl的配制好的血清基质液;
6.12、向每个待测样品孔中加入25μl样品原液;
6.13、向每个孔中加入25μl预混合微球(为防止微球沉降,加入过程中间歇震荡瓶子充分晃动混合瓶);
6.14、封闭滴定板,盖紧盖子后于4℃度条件下摇床上过夜(16-18小时);
6.15、将滴定板置于磁板上,轻轻吸走残留液体,需要移除液体的过程均需要在磁板在上完成;
6.16、向每孔加入200μl Wash Buffer清洗,共洗涤2次并轻轻移除孔内的WashBuffer;
6.17、将二抗复温至室温后,向每孔中加入25μl二抗;
6.18、封闭、室温下摇床上孵育1小时;
6.19、向含有二抗的孔中加入25μlStreptavidin-Phycoerythrin;
6.20、封闭,室温下摇床上孵育30分钟;
6.21、将滴定板置于磁板上,轻轻地移走板内所有内容物;
6.22、重复步骤16;
6.23、向每一个孔内加入150μl鞘液,摇床上震荡5分钟使微球充分悬浮;
6.24、将滴定板置入Luminex 200TM内;
6.25、保存并分析结果,计算样品中的各细胞因子浓度。
喘息组婴幼儿外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及总IgE与正常对照组相比(见表1)。

从表1和图1的结果可知:喘息组婴幼儿外周血IL-2水平较正常对照组变化不明显(P>0.05),喘息组外周血IL-4、IL-5、IL-13、TGF-β1及TIgE水平较正常对照组均明显升高,差异均有统计学意义(P<0.05)。表明IL-4、IL-5、IL-13参与了婴幼儿喘息性疾病的发病过程,喘息婴幼儿存在气道炎症。
TGF-β1是目前发现的最强的促纤维化因子,是哮喘气道炎症和气道重塑中最重要的炎性介质,TGF-β1的表达强弱被认为与哮喘发作严重程度密切相关,喘息组TGF-β1水平的升高,提示喘息患儿不仅存在气道炎症,可能存在气道重塑的高危因素。因此,无论是首次还是反复多次喘息发作的婴幼儿,都存在气道炎症。
喘息组患儿外周血总IgE水平明显高于健康对照组(P<0.05),提示IgE参与了婴幼儿喘息性疾病的发病过程。
首发组与反复组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较(见表2)。

从表2和图2的结果可知:首发组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平与反复组相比,差异均无统计学意义(P>0.05)。对于学龄前儿童,喘息次数是发展为哮喘的高危因素,但并非诊断哮喘的标准。根据Tucson儿童研究的结果,反复多次喘息的儿童发展为哮喘可能性更大。我们将急性喘息发作的婴幼儿进一步分为了首发组及反复组(发作次数≥2次),比较两组间外周血IL-4、IL-5、IL-13及TGF-β1差异,以了解反复喘息对气道炎症介质的影响。结果发现首发组及反复组外周血IL-4、IL-5、IL-13及TGF-β1水平无差异(P>0.05),未能发现不同喘息次数对IL-4、IL-5、IL-13及TGF-β1水平的影响。
无哮喘高危因素组及有哮喘高危因素组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较(见表3)。

图3为对照组、无哮喘高危因素组与有哮喘高危因素组的水平分布图。
从表3和图3的结果可知:三组间外周血IL-2、TIgE水平比较,差异无统计无统计学意义(P>0.05);有哮喘高危因素组及无哮喘高危因素组外周血IL-4、IL-5、IL-13及TGF-β1均高于正常对照组(P<0.05);有哮喘高危因素组和无哮喘高危因素组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较,差异无显著性(P>0.05)。为了解哮喘高危因素对喘息婴幼儿外周血IL-2、IL-4、IL-5,IL-13、TGF-β1及TIgE水平的影响,将喘息组患儿进一步分为有哮喘高危因素组和无哮喘高危因素组,结果显示有哮喘高危因素组及无哮喘高危因素组外周血IL-4、IL-5、IL-13及TGF-β1均高于正常对照组(P<0.05),说明两组患儿均存在气道炎症;但两组外周血IL-2、IL-4、IL-5,IL-13、TGF-β1及TIgE水平无差异(P>0.05),提示喘息婴幼儿的气道炎症与是否存在哮喘高危因素无关。
病原学阳性组与病原学阴性组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较(见表4)。

从表4和图4的结果可知:三组间外周血IL-2等水平比较,差异无统计学意义(P>0.05);病原学阳性组及病原学阴性组外周血IL-4、IL-5、IL-13、TGF-β1及TIgE均高于正常对照组(P<0.05);病原学阳性组和病原学阴性组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较,差异无统计学意义(P>0.05)。呼吸道感染是导致或加重婴幼儿喘息的原因,研究已证实,婴幼儿期呼吸道合胞病毒(Respiratory syncytial virus,RSV)感染是儿童哮喘的高危因素,在引起反复喘息的病原中,肺炎支原体(Mycoplasma pneumoniae,MP)仅次于病毒,且急性MP感染能诱发无症状患者哮喘的首次发作。因此,每位喘息患儿入院时均进行了病原学检测,检测内容包括了痰培养、咽拭子MP培养以及血清呼吸道九联检(包括RSV-IgM抗体、MP-IgM抗体)。我们发现,在50例喘息患儿中,RSV检出者6例,MP检出者9例,肺炎链球菌检出者3例,同时检出RSV及MP者3例,同时检出RSV及肺炎链球菌者2例,共计病原学检测阳性者为23例,占喘息组患儿的46%,提示感染可能是诱发及加重喘息的原因。根据病原学检测结果,将喘息组进一步分为病原学阳性组和病原学阴性组,结果显示病原学阳性组及病原学阴性组外周血IL-4、IL-5、IL-13、TGF-β1及TIgE均高于正常对照组(P<0.05),表明两组患儿体内存在多种细胞因子紊乱,均存在气道炎症,但病原学阳性组和病原学阴性组外周血IL-2、IL-4、IL-5、IL-13、TGF-β1及TIgE水平比较,差异无显著性(P>0.05),提示喘息婴幼儿的气道炎症与病原学检测是否为阳性无关。
相关性分析表明喘息组外周血EOS计数与TIgE水平呈显著正相关(Spearman相关系数rs=0.888,P<0.01);IL-4水平与IL-5、IL-13水平呈显著正相关(相关系数分别为rs=0.640,P<0.01;rs=0.614,P<0.01);IL-5水平与IL-13水平呈显著正相关(rs=0.802,P<0.01);IL-2水平与TGF-β1水平呈显著正相关(rs=0.298,P<0.05);正常对照组IL-5水平与IL-13水平呈显著正相关(rs=0.581,P<0.01)。
综上,IL-2是Th1细胞因子,哮喘患儿存在Th1/Th2失衡,哮喘患儿血清IL-2水平在急性发作期及缓解期均低于正常对照组,而Th2型细胞因子水平明显增高。虽然外周血IL-2水平在喘息组(包括首发组及反复组)与正常对照组间变化不明显(P>0.05),但与正常对照组相比,喘息组Th2细胞因子IL-4、IL-5、IL-13明显升高(P<0.05),提示喘息组婴幼儿存在Th1/Th2失衡,即Th2的优势表达。喘息婴幼儿Th2应答相对亢进,IL-4、IL-5及IL-13产生过多所促发的气道炎症及AHR可能是造成喘息发生的重要原因,这与哮喘发病的免疫学机制相似,这也提示通过药物治疗纠正喘息患儿的Th1/Th2失衡,可能对缓解急性喘息发作有一定的帮助。

Claims (6)
1.一种检测试剂盒在制备用于婴幼儿喘息性疾病的检测试剂盒中的应用,其特征在于,所述用于婴幼儿喘息性疾病的检测试剂盒包括检测IL-2、IL-4、IL-5和IL-13含量的试剂盒;
所述检测IL-2、IL-4、IL-5和IL-13含量的试剂盒为美国Millipore公司的MILLIPLEXMAP human Th17 Magnetic Bead Panel试剂盒;
所述检测试剂盒的应用方法包括以下步骤:
S1、通过检测IL-2、IL-4、IL-5和IL-13含量的试剂盒检测待测血浆中IL-2、IL-4、IL-5和IL-13的含量;
S2、当血浆中IL-4、IL-5、IL-13比例升高,或IL-2比例下降,造成Th1/Th2失衡,则为喘息性疾病患者。
2.根据权利要求1所述的应用,其特征在于,所述检测试剂盒还包括检测TGF-β1含量的试剂盒;所述检测TGF-β1含量的试剂盒为美国R&D公司Human TGF-β 1 ELISA试剂盒。
3.根据权利要求2所述的应用,其特征在于,所述应用方法还包括:
S3、通过检测TGF-β1含量的试剂盒检测喘息性疾病患者血浆中TGF-β1的含量,当TGF-β1含量较未患病时升高,则喘息性疾病患者存在气道重塑的高危因素。
4.根据权利要求3所述的应用,其特征在于,所述通过检测TGF-β1含量的试剂盒检测待测物血浆中TGF-β1的含量的方法具体为:
S3-A1、在微型反应孔中加入Assay Diluent RD1-73和待测样品,进行孵育;
S3-A2、加入Wash Buffer清洗后加入TGF-β1结合物进行孵育;
S3-A3、加入底物溶液,室温下避光显色;
S3-A4、加入终止液,用酶联仪在450nm波长处测定各孔的OD值。
5.根据权利要求1至4中任一项所述的应用,其特征在于,所述通过检测IL-2、IL-4、IL-5和IL-13含量的试剂盒检测待测物血浆中IL-2、IL-4、IL-5和IL-13的含量的方法具体为:
S1-B1、分别制备IL-2、IL-4、IL-5、IL-13抗体标记的微球;将上述微球混合得到混合微球;
S1-B2、在实验滴定板的孔内加入待测样品和所述混合微球,4℃下摇床过夜;
S1-B3、向孔内加入二抗,室温下摇床孵育;
S1-B4、加入Streptavidin-Phycoerythrin,室温下摇床孵育;
S1-B5、加入鞘液,摇床上震荡使微球充分悬浮;
S1-B6、将滴定板置入Luminex 200TM内测定IL-2、IL-4、IL-5、IL-13的浓度。
6.根据权利要求5所述的应用,其特征在于,所述混合微球中,所述IL-2、IL-4、IL-5、IL-13抗体标记的微球的体积比为1︰1︰1︰1。
