Description
Technical Field
The invention relates to a respiration support device, in particular to a high-comfort respiration management tool suitable for patients in a supine position, and belongs to the technical field of nasal respiration support.
Background
Generally, for patients requiring respiratory support, the current modes of respiratory support mainly include endotracheal intubation, laryngeal mask intubation, tracheotomy intubation and noninvasive mask respiratory support.
For endotracheal intubation, the following disadvantages are mainly encountered: the operation is difficult, professional medical personnel are required to receive professional training, and the operation can be finished after the operation is qualified. The trachea cannula operation can bring certain damage to the throat of a patient, can cause corresponding complications, even can cause sequelae which can not be cured, and brings more pain to the patient. After trachea cannula accomplished, the sputum of patient lung can't effectively be discharged, needs professional medical staff to use and attracts the supplementary discharge of apparatus, complex operation, and the sputum discharging effect is poor, has not only increased medical staff's work load, has still increased the probability of patient lung infection. In long-term intubation of the trachea, the air sac presses the tracheal mucosa, which can cause ulcer, scar formation and even tracheal stenosis, and cause serious consequences. Sputum is remained on the inner wall of the tracheal catheter when being discharged through the tracheal catheter, and the tracheal catheter is blocked after accumulation, and a trachea is frequently needed to be cut, so that a patient is injured more greatly. The comfort level of the tracheal catheter intubation is poor, and when the tracheal catheter intubation is used by a patient, a large amount of sedative drugs are needed to reduce stress reaction, increase the metabolic burden of the liver and kidney, and increase the economic burden of the patient. For conscious patients, the communication with family members and medical staff cannot be realized by using language in the process of breathing support, so that the condition of the patient can be hidden, and inconvenience is brought to treatment.
For laryngeal mask cannulas, the following disadvantages are mainly present: the laryngeal mask is expensive and brings a large economic burden to the patient. The air bag of the laryngeal mask is large, and after the laryngeal mask is placed into the throat and the nasal cavity, the throat and the nasal cavity are oppressed, so that oral sputum cannot be cleared, and oppression injury can be caused. After the laryngeal mask intubation is completed, sputum of the lung of a patient can not be effectively discharged, professional medical staff needs to use suction instruments to assist in discharging, operation is complex, the sputum discharging effect is poor, the workload of the medical staff is increased, and the probability of lung infection of the patient is increased. Although the comfort level of the laryngeal mask is improved compared with that of the tracheal catheter, the laryngeal mask can still press the throat of a patient to cause nausea and vomiting of the patient, and the laryngeal mask needs to be assisted by sedative drugs, so that the laryngeal mask can tolerate the nausea and vomiting, increases the metabolic burden of the liver and kidney, and also increases the economic burden of the patient. For conscious patients, the communication with family members and medical staff cannot be realized by using language in the process of breathing support, so that the condition of the patient can be hidden, and inconvenience is brought to treatment.
For tracheostomy tubes, the following disadvantages are mainly present: the operation difficulty is high, and the operation can be completed after the otolaryngological medical staff is qualified after receiving long-term professional training. Tracheotomy risks are high and may lead to bleeding, aspiration, hypoxia and even death of the patient. The tracheotomy has more complications, often causes incision infection, scar formation at the incision part, tracheal stenosis after healing and death of patients due to serious complications. For conscious patients, the communication with family members and medical staff cannot be realized by using language in the process of breathing support, so that the condition of the patient can be hidden, and inconvenience is brought to treatment. After the tracheotomy tube is completed, sputum in the lung of a patient can be effectively discharged, but needs to be discharged by professional medical staff in an auxiliary way by using an attraction apparatus, so that the operation is complex, and the workload of the medical staff is increased. The tracheotomy respiratory support patient needs to be assisted with sedative drugs, can tolerate the sedative drugs, increases the metabolic burden of the liver and kidney and also increases the economic burden of the patient.
For noninvasive mask respiratory support, the following disadvantages are mainly included: the breathing support of the noninvasive mask is mainly used for a conscious patient, and when the noninvasive mask is used, the mask needs to be tightly attached to the face by a belt, and the contact part is sealed by double-faced adhesive tape, so that the contact part is damaged by compression and allergic, and great discomfort is brought to the patient. Although the patient can independently discharge the sputum, the noninvasive mask needs to be detached from the face, and the noninvasive mask needs to be fixed on the face again after the sputum is discharged, so that the operation is complex; the same operation is required for eating and speaking, which is inconvenient.
In view of the above disadvantages of respiratory support modalities, there is a clinical need for a respiratory support modality that satisfies the following conditions: the implantation operation is simple and convenient, the operation can be mastered only by simple training, and the popularization is convenient; further improving the comfort level during the respiratory support, reducing or avoiding the use of sedative drugs, reducing the influence of the drugs on the functions of the liver and the kidney and reducing the economic burden of patients. The pressure on the trachea and the throat of the patient is reduced or avoided, so that various complications caused by the pressure can be reduced or avoided.
For conscious patients, the mask does not invade the air passage, keeps the smoothness of the air passage and the oral cavity, can implement mechanical auxiliary ventilation, and is convenient for language communication, food intake and autonomous liquid discharge in the respiratory support process, thereby reducing the workload of medical staff, being convenient for communicating the state of illness, and effectively discharging sputum; does not invade the air passage, reduces the stimulation to the tracheal mucosa and further reduces the production of sputum. For unconscious patients, sputum in the oral cavity can be automatically discharged, and the workload of medical staff is reduced.

During the respiratory support process, gas, liquid, etc. in the esophagus may be discharged out of the body through the oral drainage branch tube 12. Fig. 2 shows that the air flow regulator is in the form of an air clamp 16, and a branch pipe connector 17 is further provided at the end of the oral drainage branch pipe 12, and the branch pipe connector 17 can be connected and fitted to a liquid container 32 for containing liquid or the like. The oral drainage check valve is typically provided in the end of the oral drainage manifold 12 and may be adjacent to the manifold connection 17. Of course, after set up branch pipe gasbag 13 on oral cavity flowing back branch pipe 12, can also set up oral cavity flowing back manifold hole on oral cavity flowing back branch pipe 12, oral cavity flowing back manifold hole link up oral cavity flowing back branch pipe 12, and the liquid in oropharynx intracavity can be through oral cavity flowing back manifold hole and get into oral cavity flowing back branch pipe 12 in, then can be through oral cavity flowing back branch pipe 12 discharge external.
As shown in fig. 6, the main body of the oral drainage tube is parallel to the nasopharyngeal airway 10, the oral drainage tube comprises an oral drainage guide part 27 located 1cm to 4cm in front of the nasopharyngeal airway 10 and an oral drainage tube body 30 communicated with the oral drainage guide part 27, and the front section of the oral drainage tube body 30 is integrally formed or fixedly connected with the nasopharyngeal airway 10; the oral cavity liquid drainage guide part 27 is provided with a plurality of liquid drainage guide openings; the nasopharyngeal cavity vent pipe 10 is isolated from the oral cavity liquid drainage guide part 27, a nasopharyngeal cavity vent pipe air vent is arranged on the nasopharyngeal cavity vent pipe 10, and the nasopharyngeal cavity vent pipe air vent is positioned behind the liquid drainage guide opening.
In the embodiment of the present invention, the oral drainage tube 33 is matched with the nasopharyngeal cavity vent tube 10 to form a respiratory support and an oral drainage, wherein the oral drainage tube 33 comprises an oral drainage guide portion 27 and an oral drainage tube portion 30, and the tube diameter of the oral drainage guide portion 27 is larger than the tube diameter of the oral drainage tube portion 30. When the oral drainage tube 33 is composed of the oral drainage guide 27 and the oral drainage tube body 30, the body of the oral drainage tube 33 is parallel to the length direction of the nasopharyngeal cavity vent tube 10, and the end of the oral drainage tube body 30 is generally located outside the end of the nasopharyngeal cavity vent tube 10, so that the oral drainage tube body 30 can be connected and matched with the liquid container 32.
In specific implementation, the drainage guide port at the end of the oral drainage guide part 27 includes a drainage guide port 28 and a guide side hole 29 penetrating the oral drainage guide part 27, and the nasal-pharyngeal airway vent on the nasal-pharyngeal airway 10 includes a vent release side hole 25 and a vent release side hole 26, wherein the oral drainage guide port 28 is located at the end of the oral drainage guide part 27, and the guide side hole 29 is located at the side of the oral drainage guide part 27. The vent release port 26 is located at the end of the nasopharyngeal cavity vent tube 10, and the vent release side hole 25 is located outside the vent release port 26 on the nasopharyngeal cavity vent tube 10.
When the oral liquid drainage device is used specifically, the air vent release port 26 can be aligned with the respiratory tract, the air released by the air vent release port 26 and the air vent release side hole 25 can be partially discharged into the oral liquid drainage guide part 27 and then discharged out of the body after passing through the oral liquid drainage tube body 30, and the air and the liquid can conveniently enter the oral liquid drainage guide part 27 due to the fact that the pipe diameter of the oral liquid drainage guide part 27 is larger than that of the oral liquid drainage tube body 30, and the air and the liquid discharging efficiency is improved. In fig. 6, which shows the case where the air flow regulator employs the air flow regulating valve 31, an oral discharge check valve may be provided at the end of the oral discharge tube body 30.
