Abstract
本发明公开了头端双腔球囊导管,包括导管和设于导管上与其连通的球囊,所述导管前端口与球囊之间还设有侧腔管;所述侧腔管两端分别设有导丝出口和导丝入口;所述侧腔管平行于所述导管设置。本发明的有益效果是:通过球囊的作用,在主支血管置入支架时,可有效保护分支血管,避免分支血管出现严重的狭窄或闭塞;通过侧腔管的作用,可确保导丝顺利通过主支支架网眼重入分支血管,也能避免分支血管闭塞;另外,球囊还兼顾普通半顺应性球囊特点,对病变处进行预扩张,减少介入耗材及医疗费用,也便于导丝的顺利撤出。
Description
技术领域
本发明属于医疗器械领域,尤其涉及一种头端双腔球囊导管。
背景技术
冠状动脉分叉病变约占经皮冠状动脉介入治疗(percutaneous coronaryintervention,PCI)的15%-20%。在PCI过程中,由于冠状动脉分叉处解剖复杂且动态变化,分支血管血流受影响导致不良事件发生率较高,故分叉病变的介入治疗仍是PCI的难点和挑战。大量临床研究表明,在分叉病变介入治疗策略中,与双支架术式相比,单支架术式显著减少主要MACC事件发生率,并改善患者临床预后。然而,在冠脉主支血管置入支架时,分支血管因为斑块和嵴移位、分支血管开口夹层或痉挛等原因而引起严重狭窄或闭塞,导致PCI围术期急性心肌梗死,从而严重影响病人的远期预后。因此,分叉病变介入治疗过程中,保护分支血管防止其闭塞的策略和技巧尤为重要,最大限度地保证分支血管的开通可以改善患者的生活质量和远期预后。
目前临床上,保护分支血管技术主要有:分支血管拘禁导丝技术、分支血管拘禁不扩张球囊技术、分支血管拘禁半扩张球囊技术和分支血管拘禁corsair微导管技术。然而,这些技术尚不能完全规避分支血管慢血流或闭塞而导致不可挽回的后果。同时也伴随一些器械相关并发症,如拘禁导丝截留或断裂,拘禁球囊截留、断裂或破裂,主支支架变形,特别是导丝无法从主支支架网眼重新进入分支血管。
发明内容
本发明的目的在于克服现有分叉病变介入治疗技术无法有效保护分支血管的缺陷,提供一种用于拘禁保护分支血管,同时兼顾半顺应性球囊特点,且能确保导丝顺利通过主支支架网眼重入分支血管的头端双腔球囊导管。
为实现上述目的,本发明所设计的头端双腔球囊导管,包括导管和设于导管上与其连通的球囊,所述导管具有一个前端口,所述导管的前端口与球囊之间还设有侧腔管;所述侧腔管两端分别设有导丝出口和导丝入口;所述侧腔管平行于所述导管设置。
进一步,所述侧腔管3的所述导丝出口和导丝入口处分别设有标记。
进一步,所述导丝出口和导丝入口的端面为倾斜面。
进一步,所述导管内还设有内腔管;所述内腔管一端与导管前端口连接,另一端为开口并与设于导管上的后端口连通。
进一步,所述球囊内设有显影环。
进一步,所述侧腔管的内径为0.45mm,导管的外径为0.87mm。
本发明的有益效果是:通过球囊的作用,在主支血管置入支架时,可有效保护分支血管,避免分支血管出现严重的狭窄或闭塞;通过侧腔管的作用,可确保导丝顺利通过主支支架网眼重入分支血管,也能避免分支血管闭塞;另外,球囊还兼顾普通半顺应性球囊特点,对病变处进行预扩张,减少介入耗材及医疗费用,也便于导丝的顺利撤出。

图中,导管1、球囊2、侧腔管3、导丝出口4、导丝入口5、标记6、内腔管7、前端口8、后端口9、显影环10、第一导丝11、第二导丝12。
具体实施方式
以下结合附图和具体实施例对本发明作进一步的详细描述。
如图1~3所示的头端双腔球囊导管,包括导管1和设于导管1上与其连通的球囊2,导管1具有一个前端口8,导管1的前端口8与球囊2之间还设有侧腔管3;侧腔管3两端分别设有导丝出口4和导丝入口5;侧腔管3平行于导管1设置。为了提升手术效率,可在侧腔管3的导丝出口4和导丝入口5处分别设置标记6。另外,如图3所示,导丝出口4和导丝入口5的端面优选为都是倾斜面。
本实施例的导管1内还设有内腔管7;内腔管7一端与导管1前端口8连接,内腔管7另一端为开口并与设于导管1上的后端口9连通。球囊2内设有显影环10。侧腔管3的内径为0.45mm,导管1的外径为0.87mm。
如图4所示,手术时,将现有的主支血管支架(现有装置)置入主支血管后,并在主支血管支架球囊(现有装置)扩张前,将第一导丝11通过后端口9插入内腔管7内,并带动导管1前移,直至将球囊2送入分支血管内,使球囊2位于分支血管的分支口处;当主支血管支架球囊扩张时,球囊2可起到防止主支血管斑块和嵴移位至分支血管分支口处的目的,对分支血管起到了很好的保护作用。当主支血管支架球囊收缩后(主支血管支架已经扩张),通过主支血管支架的网眼稍回撤导管1,使侧腔管3处于分支血管的分支口处,尤其是使导丝入口5处于分支口处,并以标记6作为参照,将第二导丝12穿过主支血管支架的网眼后通过导丝入口5送入侧腔管3,并通过导丝出口4进入分支血管。低压(4~6atm)扩张球囊2以使其能顺利前送,确保第二导丝12(即重入分支导丝)的一端(后端)从导丝入口5滑出,再向前端口8推送第二导丝12至分支血管远端,最后撤出球囊2和第一导丝11,顺利完成经主支血管支架网眼重入导丝至分支血管远端操作。
虽然现有单支架式导管相比双支架式导管而言,可显著改善患者临床预后和减少主要MACC事件发生率。但单支架式又会存在相应缺陷,比如在冠脉主支血管置入支架时,分支血管因为斑块和嵴移位、分支血管开口夹层或痉挛等原因而呈现严重狭窄或闭塞,导致PCI围术期急性心肌梗死,从而严重影响病人的远期预后。本实施例的头端双腔球囊导管与现有单支架式导管相比,有效克服了上述缺陷。
Claims (6)
1.一种头端双腔球囊导管,包括导管(1)和设于导管(1)上与其连通的球囊(2),所述导管(1)具有一个前端口(8),其特征在于:所述导管(1)的前端口(8)与球囊(2)之间还设有侧腔管(3);所述侧腔管(3)两端分别为导丝出口(4)和导丝入口(5);所述侧腔管(3)平行于所述导管(1)设置。
2.根据权利要求1所述头端双腔球囊导管,其特征在于:所述侧腔管(3)的导丝出口(4)和导丝入口(5)处分别设有标记(6)。
3.根据权利要求1所述头端双腔球囊导管,其特征在于:所述导丝出口(4)和导丝入口(5)的端面为倾斜面。
4.根据权利要求1所述头端双腔球囊导管,其特征在于:所述导管(1)内还设有内腔管(7);所述内腔管(7)一端与导管(1)前端口(8)连接,另一端为开口并与设于导管(1)上的后端口(9)连通。
5.根据权利要求1所述头端双腔球囊导管,其特征在于:所述球囊(2)内设有显影环(10)。
6.根据权利要求1所述头端双腔球囊导管,其特征在于:所述侧腔管(3)的内径为0.45mm,导管(1)的外径为0.87mm。
