背景技术
中枢神经系统(脊髓)的损伤涉及心理障碍,以及诸如运动障碍和泌尿系统疾病的医学障碍。在脊髓损伤的治疗中,目前正在使用手术治疗,基于类固醇的药物治疗如甲基泼尼松龙,或康复治疗来减轻神经损伤。然而,这些方法仅是减轻由神经损伤引起的障碍的治疗法,到目前为止,还没有根本的脊髓神经再生治疗法。
以诱导受损脊髓神经再生,在神经损伤部位营造诱导生物神经再生环境的方法,以及涉及施用神经干细胞,纺锤体祖细胞,间充质干细胞,施旺细胞或神经上皮细胞的细胞治疗法正在被开发。为了开发用于所述细胞治疗法的细胞治疗剂,主要使用成体干细胞,胚胎干细胞或诱导多能干细胞。
然而,成体干细胞具有稀缺性,有限的分化能力和由于使用外来细胞而引起免疫排斥的问题。此外,使用胚胎干细胞具有伦理问题以及使用外来细胞引起免疫排斥的问题。胚胎干细胞和诱导多能干细胞共同地具有致癌潜力。
在脊髓损伤的情况下,已知脊髓损伤后48小时为脊髓损伤的急性期,适合于脊髓损伤的治疗。在使用细胞治疗剂的情况下,神经再生细胞如自体神经干细胞和少突胶质前体细胞是避免免疫排斥所必需的。可是,在急性期期间,不可能从患有急性脊髓损伤的患者中收集细胞并获得适合用于细胞治疗剂的细胞群体。此外,尽管由患者的体细胞或直接杂交分化的细胞所制备的诱导多能干细胞具有自我再生能力,而可以满足制备细胞治疗剂所需的细胞群体,但其生产时间和分化成靶细胞的时间段不适合急性期的治疗。因此,在作为脊髓损伤治疗的适当时间的急性期期间,不可能进行治疗。
因此,需要开发在适当的时间段以非侵入性方式使脊髓损伤最小化并诱导脊髓中受损脊髓神经再生的脊髓神经再生技术。

透明质酸糖胺聚糖为具有二糖键的多糖,其中D-葡糖醛酸和N-乙酰葡糖胺具有β-(1→4)和β-(1→3)的变化的糖苷键,其分子量根据二糖键的长度而变化。在一个实施方式中,透明质酸的分子量可在5000Da至20000000Da的范围内。具体而言,透明质酸的分子量可为0.5至4.0×106Da、1.0至2.0×106Da、或1.5至1.8×106Da的范围内。所述透明质酸的浓度可包括10μg/ml至5mg/ml、50μg/ml至5mg/ml、100μg/ml至3mg/ml、100μg/ml至1mg/ml、200μg/ml至1mg/ml、或300μg/ml至800μg/ml的范围内。另外,所述透明质酸可以盐和药学上可接受的盐的形式提供,例如,钠盐,钾盐,钙盐,或镁盐。
在一个实施方式中,所述水凝胶贴片或组合物可进一步包含细胞生长因子。所述细胞生长因子可为神经细胞生长因子、血管内皮细胞生长因子、成纤维细胞生长因子、骨形态发生蛋白质、表皮细胞生长因子、肝细胞生长因子转化生长因子或其组合。在一个实施方式中,所述生长因子可包括胎盘生长因子、巨噬细胞集落刺激因子、粒细胞巨噬细胞集落刺激因子、神经菌毛素、成纤维细胞生长因子(FGF)-1、FGF-2(人碱性成纤维细胞生长因子(bFGF))、FGF-3、FGF-4、FGF-5、FGF-6、促红细胞生成素、骨形态发生蛋白质(BMP)-2、BMP-4、BMP-7、转化生长因子-β、类胰岛素生长因子(IGF-1)、骨桥蛋白(Osteopontin)、多效蛋白(pleiotrophin)、活化素(activin)、内皮素(endothelin-1)及其组合。所述神经细胞生长因子可包括选自脑源性神经营养因子(BDNF)、胶质细胞源性神经营养因子(GDNF)、睫状节神经细胞营养因子(CNTF)、人碱性成纤维细胞生长因子、环磷酸腺苷(cAMP)、神经营养因子(NT)、NT3、NT4、三碘甲状腺原氨酸(T3)、音猬因子(SHH)及血小板衍生生长因子(PDGF)中的至少一者。所述血管内皮细胞生长因子可包括血管内皮生长因子(VEGF)-A、VEGF-A、VEGF-B、VEGF-C、VEGF-D或VEGF-E。包含在水凝胶贴片或组合物中的细胞生长因子的浓度可根据细胞生长因子的种类而变化,并且可在1ng/ml至1000ng/ml或0.1μM至100μM的范围内。所述细胞生长因子可增加包括了纤维蛋白、层粘连蛋白及透明质酸的水凝胶贴片或组合物的受损组织恢复效果。
所述脑源性神经营养因子蛋白质由脑源性神经营养因子基因编码。已知脑源性神经营养因子蛋白质有助于中枢神经系统中的神经细胞的存活及新神经细胞的分化和生长。
所述胶质细胞源性神经营养因子蛋白质由胶质细胞源性神经营养因子基因编码。已知胶质细胞源性神经营养因子蛋白质有助于中枢神经系统的神经细胞中的多巴胺能神经细胞运动神经细胞的存活及分化。
所述睫状节神经细胞营养因子蛋白质由睫状节神经细胞营养因子基因编码。已知睫状节神经细胞营养因子蛋白质促进生产神经递质,并有助于神经细胞和少突胶质细胞的存活。
所述神经营养因子-3蛋白质由神经营养因子-3基因编码。已知神经营养因子-3蛋白质有助于中枢神经系统中的神经细胞的存活及新神经细胞的分化和生长。
所述环磷酸腺苷蛋白质通过腺苷酸环化酶的作用衍生自三磷腺苷(ATP)的蛋白质,并充当细胞的第二信使。已知环磷酸腺苷蛋白质参与神经递质的整体管制,如神经递质的生产、储存和分布,更有助于神经细胞的存活和分化,并充当血管内皮细胞的屏障和有助于增殖和生产一氧化氮。
所述音猬因子蛋白质由音猬因子基因编码。音猬因子在刺猬信号通路中起作用,并已知其在神经系统中,特别是运动神经细胞的分化中起作用。
所述三碘甲状腺原氨酸激素是一种甲状腺激素,并影响生物体内大多数圠生理作用。在神经系统中,三碘甲状腺原氨酸激素抑制少突胶质前体细胞的增殖,并促进少突胶质细胞的分化。
所述血小板衍生生长因子的亚型可为-AA、-BB、-AB、-CC和-DD,在由血小板衍生生长因子亚基A(PDGFA)基因编码的PDGFA的同型二聚体的情况下,血小板衍生生长因子的亚型可为PDGF-AA,并每种亚型结合到血小板衍生生长因子的不同受体(PDGFR)。血小板衍生生长因子-AA是糖蛋白质二聚体,在神经系统中,与激活少突胶质前体细胞的血小板衍生生长因子受体的成纤维细胞生长因子一起维持少突胶质前体细胞的增殖。
所述血管内皮细胞生长因子蛋白质是一种在血管生成和血管化中起作用的血管通透性因子(VPF)。已知血管内皮细胞生长因子对在胚胎发育和疤痕组织形成新血管中起作用,并通过防止由于毒性和应激导致的神经细胞死亡以有助神经细胞的存活。
在一个或多个实施方式中,所述水凝胶贴片或组合物可包括胶原蛋白,或可实质上不包括胶原蛋白。不受任何特定理论的限制,组合物的成分中可包括胶原蛋白,或可实质上不包含胶原蛋白。根据一个方面,与存在胶原蛋白相比,不存在胶原蛋白可为有利的。
另外,所述水凝胶贴片或组合物可实质上不包括细胞。在本说明书中,"实质上不包括"是指包括胶原蛋白或细胞至水凝胶贴片或组合物的活性或药理活性不受影响的水平或完全不包括的意义。根据一个实施方式的水凝胶贴片或组合物可由于实质上不包括细胞,与用于受损组织再生的细胞治疗剂基本上不同。
所述胶原蛋白蛋白质可分为I型,II型,III型,IV型和V型,I型胶原蛋白是存在体内最为大量的。胶原蛋白具有α1及α2的三螺旋结构。
在一个实施方式中,所述水凝胶贴片或组合物可包括纤维蛋白及/或纤维蛋白原;层粘连蛋白;及透明质酸或其盐。另外,水凝胶贴片或组合物可进一步包括说明书中描述的成分中的至少一种。例如,所述水凝胶贴片或组合物可包括纤维蛋白及/或纤维蛋白原;层粘连蛋白;透明质酸或其盐;任选的凝血酶;任选的胶原蛋白;及任选的细胞生长因子。
在一个实施方式中,所述水凝胶贴片或组合物可增强细细胞粘附。因此,本说明书进一步提供了增强个体细胞(例如,神经细胞)的粘附的方法,该方法包括将水凝胶贴片或组合物施用于个体。
在一个或多个实施方式中,所述水凝胶贴片或组合物可增加由神经干细胞向神经细胞的分化。因此,本说明书进一步提供了增强个体的神经干细胞向神经细胞的分化的方法,该方法包括将水凝胶贴片或组合物施用于个体。

在本说明书中,脊髓损伤(SCI)是指由脊髓压迫或脱位引起的损伤。脊髓损伤可包括外部脊髓损伤,脊柱退行性疾病(如脊椎病等),脊髓炎症性疾病(如脊椎炎,慢性类风湿性关节炎等),肿瘤(如脊髓肿瘤,脊椎肿瘤等),血管疾病(如脊髓出血,中风,由于髓外血管疾病引起的脊髓麻痹等),脊髓炎(如蛛网膜炎,病毒性脊髓炎,细菌性脊髓炎等),多发性硬化及肌萎缩侧索硬化等。根据一个实施式的水凝胶贴片或组合物不仅可对急性脊髓损伤具有治疗效果,而且对慢性脊髓损伤也具有治疗效果。因此,脊髓损伤可为慢性脊髓损伤。
根据一个实施方式的组合物的剂量可在0.01mg至10000mg、0.1mg至1000mg、1mg至100mg、0.01mg至1000mg、0.01mg至100mg、0.01mg至10mg、或0.01mg至1mg的范围内。然而,考虑到各种因素如制剂方法,施用方式,患者的年龄,体重,性别,病理状态,饮食,施用时间,施用途径,排泄率及反应感应性而可不同地规定剂量。本领域技术人员考虑到这些因素,将能够适当地调整剂量。剂量可施用一次或可在临床上可接受的副作用范围内,施用两次或以上,并且施用部位可是一个或两个或以上。对于除人类以外的动物,可以与人体每kg施用的剂量相同,或根据目标动物器官与人体器官(如:心脏)的体积比(例如,平均值)转换的剂量施用。可用的施用途径包括口服,舌下含服,肠胃外服(如皮下,肌内,动脉内,腹膜内,硬膜内,或静脉内),直肠和局部途径(包括经皮吸收),吸入和注射,或插入便携式设备或物质。根据一个实施方式的受治疗的动物包括人类和其他哺乳动物,如人类,猴,小鼠,大鼠,兔,绵羊,牛,狗,马,猪等。

根据施用方法或剂型,如有需要,根据一个实施方式的药物组合物可包括各种添加剂,如助悬剂,增溶剂,防腐剂,稳定剂,等渗剂,防腐剂,吸附抑制剂,表面活性剂,稀释剂,赋形剂,pH调节剂,镇痛剂,缓冲剂,还原剂,及抗氧化剂等。包括上述那些及适用于本说明书的药学上可接受的载体和制剂,在[雷明登氏药学全书,第19版,1995]中有详细描述。根据一个实施方式的药物组合物可通过使用根据本领域普通技术人员容易实施的任何方法的药学上可接受的载体及/或赋形剂,来配制成单位剂型或置于多剂量容器中。在这方面,剂型可为油或水介质中的溶液,悬浮液或乳剂液的形态,或粉末,颗粒,锭剂或胶囊的形态。
本发明另一方面提供了一种制备水凝胶贴片的方法,该方法包括将凝血酶加入溶胶组合物中,所述溶胶组合物包括纤维蛋白原;层粘连蛋白或层粘连蛋白衍生肽;及透明质酸或其药学上可接受的盐。
所述方法可进一步包括将细胞生长因子加入所述溶胶组合物中。
所述方法可进一步包括通过添加凝血酶进行凝胶化,然后向其中添加凝血酶进行第二次凝胶化。所述凝胶化可在10℃至40℃的温度下进行5分钟至3小时。另外,所述凝血酶可以约1U/ml至约10U/ml添加。所述水凝胶贴片可根据框架的形状,制成各种形状或尺寸。
所述方法还可包括在4℃至-210℃的温度下,低温保存或深低温保存水凝胶贴片在溶液中。所述溶液可包括二甲亚砜(DMSO),并且只要它基本上不改变水凝胶贴片的化学或物理性质,即可不受限制。即使在低温保存或低温保存时,所述水凝胶贴片基本上不改变其形状或活性。

本发明另一方面提供了一种在4℃至-210℃的温度下低温保存或深低温保存水凝胶贴片在溶液(如二甲亚砜)中的方法。
以所述方法在低温保存或在深低温保存的水凝胶贴片可立即用于治疗处于急性期的脊髓损伤的患者,急性期是指脊髓损伤的最佳治疗时机。即使当在低温保存或深低温保存的水凝胶贴片在室温下溶化时,水凝胶贴片也可不会经历形态变化,并且可保持对脊髓损伤的治疗效果。
有益效果
根据本说明书的实施方式的水凝胶贴片及制备水凝胶贴片的方法可通过修复受损组织,用于各种病症,包括脊髓损伤。

在下文中,将参考引用例和实施例详细描述本发明。以下引用例和实施例是对本发明的说明,不应解释为限制本发明。
<实施例>水凝胶混合物和贴片的制备
1-1.水凝胶混合物
将20mg/ml的纤维蛋白原(Sigma,F8630)在37℃的温度下溶解于DMEM/F12(1:1)(Gibco,11330-057)培养液中30分钟,将5mg/ml的透明质酸(Sigma,53747)在4℃的温度下溶解于DMEM/F12(1:1)(Gibco,11330-057)培养液中一天,以及将200U/ml的凝血酶(Sigma,T4648)溶解于DMEM/F12(1:1)(Gibco,11330-057)培养液中。将胶原蛋白(Corning,354236)中和并在10XDulbecco's磷酸缓冲盐溶液(DPBS,Sigma,D5652-10L),去离子水,及1N的氢氧化钠(Millipore,1.00983.1011)中电离,及稀释至其最终浓度达到3.00mg/mL。
然后,将上述成分混合以制备溶胶状态的水凝胶。每种成分的最终浓度如下:
纤维蛋白原(Sigma,F8630)1mg/ml、5mg/mL或10mg/ml;
层粘连蛋白(Thermofisher,23017-015)5μg/ml、10μg/ml或50μg/ml;
透明质酸(Sigma,53747)0.1mg/ml、0.5mg/ml或1mg/ml;以及
胶原蛋白(Corning,354236)1.2mg/ml。
1-2.向水凝胶混合物中添加神经细胞生长因子
将根据实施例1-1制备的溶胶状态的水凝胶与下列神经细胞生长因子及/或血管内皮细胞生长因子在培养液中混合,该培养液为将包括青霉素/链霉素(Invitrogen,15140-122),2mM的L-谷氨酰胺(invitrogen,25030-081),N2添加剂(Gibco,1750-048),及B27supplement minus vitamin A(Gibco,12587010)的DMEM/F12(Gibco 11330-057)与neurobasal medium(Gibco,21103049)以1:1的比例混合。
重组人脑源性神经营养因子(Peprotech,450-02)10ng/ml、重组人胶质细胞源性神经营养因子(Peprotech,450-10)10ng/ml、重组人神经营养因子-3(NT-3)(Peprotech,450-03)5ng/ml、db-环磷酸腺苷(Sigma,D0260)1uM、三碘甲状腺原氨酸(Sigma,T6397)60ng/ml、重组人音猬因子(SHH;Peprotech,100-45)50ng/ml、重组人血小板衍生生长因子-AA(Peprotech,100-13A-100)、重组人成纤维细胞生长因子(Peprotech,100-18B)10ng/ml及重组人血管内皮生长因子165(Peprotech,100-20)20ng/ml。
1-3.水凝胶贴片的制备
如图1所示制备水凝胶贴片。
具体而言,将5U/ml的凝血酶(Sigma,T4648)加入到根据实施例1-1或1-2制备的溶胶水凝胶中,并在37℃下培养1小时进行凝胶化。然后,在用紫外线灭菌的石腊膜上打尺寸为0.3mm至0.8mm的圆孔,并将其固定在10cm的培养皿上。随后,将5U/ml的凝血酶分配到圆孔中,并在其上分配15uL至60uL的水凝胶,然后在混合水凝胶和凝血酶的同时抑制气泡的形成,跟着在室温下凝胶化约2分钟,然后在37℃下进行二次凝胶化处理1小时。将凝胶状水凝胶与石腊膜分离,以制备各种尺寸范围为0.3mm至0.8mm的贴片形态。制备的水凝胶贴片保存在包括实施例1-2制备的神经细胞生长因子及血管内皮细胞生长因子的培养液中。
通过所述方法制备的水凝胶定义为水凝胶贴片,并且制备的水凝胶贴片的类型总结如下。
在以下描述中,这些水凝胶贴片将被称为水凝胶贴片1至6。
水凝胶贴片1:纤维蛋白;
水凝胶贴片2:纤维蛋白+层粘连蛋白+透明质酸;
水凝胶贴片3:纤维蛋白+层粘连蛋白+透明质酸+胶原蛋白;
水凝胶贴片4:纤维蛋白+神经细胞生长因子及血管内皮细胞生长因子(实施例1-2中列出的所有生长因子);
水凝胶贴片5:纤维蛋白+层粘连蛋白+透明质酸+神经细胞生长因子及血管内皮细胞生长因子(实施例1-2中列出的所有生长因子);
水凝胶贴片6:纤维蛋白+层粘连蛋白+透明质酸+胶原蛋白+神经细胞生长因子及血管内皮细胞生长因子(实施例1-2中列出的所有生长因子)。
1-4.水凝胶贴片的剂型及使用深低温保的可能性
水凝胶贴片被制备为各种剂型,并确认水凝胶贴片是否能够深低温保存。
具体而言,确认了根据实施例1-3制备的水凝胶贴片2的可逆相变,结果显示在图2中。
如图2所示,以与实施例1-1相同的方式制备液体剂型的水凝胶贴片2,然后浸入小瓶中。以与实施例1-3相同的方式将凝血酶浸入注射器中。另外,水凝胶贴片2通过在-80℃和0.33torr的冷冻干燥(Scientific,F78)24小时,被制成固体剂型,然后被粉碎以制备粉末形态。另外,也制备了粉末形态的剂型。通过以与实施例1-3中相同的方式向所述粉末形态的剂型加入凝血酶使所述粉末形态的剂型凝胶化,从而证实粉末形态的剂型可再次制备为凝胶形态。
该结果表明,根据一个实施方式的水凝胶贴片可发生可逆的相变,并能够以各种剂型提供,例如固体,半固体,液体或粉末剂型。
此外,确认了深低温保存是否可行。
具体而言,将水凝胶贴片2加入到10%(v/v)二甲基亚砜中,10%(v/v)二甲基亚砜是通过将二甲基亚砜(DMSO,Sigma,D2650)加入到根据实施例1-2制备的培养液中而制备。然后,将依次降低温度至4℃(30分钟)、-20℃(1小时)及-80℃深低温保存,并将所得物冷冻干燥。通过使用场冷冻发射扫描电子显微镜(Cold FE-SEM;Hitach High Technologies,S-4800),进行深低温保存前后的水凝胶贴片的形态分析,结果如图3所示。
如图3所示,深低温保存前与深低温保存前一个月后解冻的水凝胶贴片没有形态学差异。而且,发现水凝胶贴片的表面是多孔性的。
<实验例1>体外神经干细胞分化的评价
5日龄的C57BL/6只小鼠(C57BL/6N Japan SLC,Inc.)牺牲以被收集其脑,通过已知方法原代培养小鼠神经干细胞,并在100mm培养皿中培养。
将根据一个实施方式的材料和对比材料分别涂布于小鼠神经干细胞,以确认根据一个实施方式的材料和对比材料对小鼠神经干细胞分化的影响。
通过使用已知方法(Kim JB et al.Nat Protoc.2009)提取和培养小鼠神经干细胞,对具有1×104的群体的所述小鼠神经干细胞涂布根据实施例1-2制备的水凝胶(纤维蛋白5mg/mL;层粘连蛋白10μg/ml;及透明质酸0.5mg/ml),随后通过加入凝血酶,从而制备水凝胶贴片。然后,将细胞在包括或不包括实施例1-2的神经细胞生长因子和血管内皮生长因子的培养液中培养。为了选择最佳浓度,单独的纤维蛋白1mg/ml、5mg/mL或10mg/ml;单独的层粘连蛋白5μg/ml、10μg/ml或50μg/ml;及单独的透明质酸0.1mg/ml、0.5mg/ml或1mg/ml;或层粘连蛋白10μg/ml及透明质酸0.5mg/ml被作为比较材料。
接下来,在培养14天后,进行免疫荧光染色。具体而言,为了免疫细胞化学,以4%(w/v)的多聚甲醛将细胞在室温下固定在磷酸盐缓冲液(Wako,163-20145)10分钟。将固定的细胞用10X的磷酸缓冲盐溶液(PBS,P2007)稀释至1X,并在室温下洗涤3次,每次5分钟,然后利用0.1%(v/v)的聚乙二醇辛基苯基醚(Triton X-100,Sigma,T9284)透化10分钟。在室温下用1X的磷酸缓冲盐溶液洗涤3次5分钟后,在室温下用溶解在1X磷酸缓冲盐溶液中的4%(v/v)胎牛血清封闭细胞1小时以抑制非特异性结合。然后,通过使用第一抗体抗βIII-微管蛋白抗体(Tuj1)(1:400;Abcam)或抗胶质纤维酸性蛋白(GFAP)(1:400;Sigma)在室温下培养细胞1小时,并且通过使用在1X的磷酸缓冲盐溶液溶解三次的0.05%(v/v)吐温20(Tween-20,Sigma,P7949)(PBST)在室温下洗涤上述细胞三次共10分钟。随后,用二级荧光抗体(Alexa Fluorophore-偶联的二级抗体488)(1:1000)或Alexa594(1:1000)阻断光,然后在室温下细胞培养30分钟。当需要双重染色时,所述细胞与另一种第一抗体培养之前,在室温下进行另外的封闭30分钟。在室温下用PBST洗涤三次10分钟后,将细胞阻断光并与4',6-二脒基-2-苯基吲哚(DAPI)(1:1000;Invitrogen)一起培养15秒,以进行细胞核染色,然后用PBST在室温下洗涤三次共10分钟。将细胞储存在磷酸缓冲盐溶液中以使用荧光显微镜观察。
随后,通过使用附上数码单色相机的倒置荧光显微镜(Leica,DMI 3000B)观察神经细胞(Tuj1)和星型胶质细胞(胶质纤维酸性蛋白)。从倒置荧光显微镜观察的图像中,通过使用ImageJ(National Institute of Health(http://rsb.info.nih.gov/ij)软件计算神经细胞或星型胶质细胞的数量以计算分化率。
通过使用非配对双尾学生t检验进行所有统计学分析。显着性为*P<0.05,**P<0.005,或***P<0.0005。
如图4所示,当将根据一个实施方式的水凝胶涂布于神经干细胞时,发现与不涂布水凝胶的裸露条件相比,神经干细胞的细胞连接得到改善。
如图5及图6所示,与涂布单独的纤维蛋白、单独的层粘连蛋白、单独的透明质酸及层粘连蛋白和透明质酸的组合的情况相比,当涂布根据一个实施方式的组合物(纤维蛋白、层粘连蛋白、透明质酸)时,可看出神经干细胞分化为神经细胞发生协同效应。具体而言,没有任何处理的对照组中有5%的神经细胞出现,单独的每种成分中存在少于12%的神经细胞。然而,在透明质酸和层粘连蛋白组合的情况下,存在约15%的神经细胞,并且当本身没有实质影响的纤维蛋白与透明质酸和层粘连蛋白组合时,发现至少有20%的神经细胞出现。这表明透明质酸和层粘连蛋白的组合在分化神经细胞方面是有效的,特别地,三种组合物的组合具有协同效应,并显着增加神经细胞的分化。
如图7所示,当添加生长因子时,如图6中所示的结果,与涂布单独的纤维蛋白、单独的层粘连蛋白、单独的透明质酸及层粘连蛋白和透明质酸的组合的情况相比,当涂布根据一个实施方式的组合物(纤维蛋白、层粘连蛋白、透明质酸)时,可看出神经干细胞分化为神经细胞发生协同效应。具体而言,尽管生长因子本身增加了向神经细胞的分化,但是三种组合物和生长因子的组合产生了协同效应,导致约30%的神经细胞的出现。该结果表明,根据一个实施方案的水凝胶贴片或组合物显示出通常不可预测的协同效应。
另外,如图8所示,将每种生长因子加入到根据一个实施方式的组合物中,结果证实小鼠神经干细胞的神经细胞分化率提高了约2.5至5倍。
由所述结果可见,根据本说明书的一个实施方式的组合物单独产生增加分化率的协同效应,并当与生长因子组合时,产生更高的协同效应,而且生长因子对细胞的影响增加。
<实验例2>生物降解性的评估
为了评价根据实施例1制备的水凝胶贴片的生物降解性,用25mg/ml的阿佛丁麻醉剂麻醉6周龄的C57BL/6小鼠(C57BL/6N Japan SLC,Inc.),阿佛丁麻醉剂通过将2,2,2-三溴乙醇(Sigma,T48402)溶解于叔戊醇(Sigma,152463)而制备。在这方面,每只小鼠的施用麻醉剂量为125-250mg/kg体重。随后,水凝胶贴片5移植到皮下。移植水凝胶贴片两周后,切开皮下确认皮下移植的水凝胶贴片有否生物降解。
结果,如图9所示,,证实了根据一个实施方案的水凝胶贴片在体内生物降解。
<实验例3>确认水凝胶贴片的脊髓损伤治疗效果
4-1.利用脊髓损伤I动物模型确认水凝胶贴片的脊髓损伤治疗效果
为了确认根据一个实施方式的水凝胶贴片的脊髓损伤再生效果,将水凝胶贴片移植到脊髓损伤动物模型中,然后在其上进行行为测试8周。
一种评估官方小型脊髓损伤动物模型的方法是慢性脊髓损伤研究的行为分析,观察并评估动物的关节,后腿,步态,前腿和后腿的协调运动,腰部的位置,脚掌的支撑尾巴的位置以评估行为能力的恢复。获得的每只动物的评估结果,即以0至21的分数量化恢复能力:0至7的分数表示恢复最小行为能力的早期阶段,即后腿几乎没有恢复,8到13的分数表示恢复后腿的运动或前腿和后腿没有很好地控制的协调运动中存在间隙的中间恢复阶段。最后,14到21的分数表明前腿和后腿协调地运动的后期恢复阶段。
首先,注射10mg/rat Zoletil 50(Virbac)以麻醉成年雄性斯泼累格·多雷(Sprague-Dawley)大鼠(OrientBio),然后从中除去T9位点的脊髓薄层。然后,通过使用血管夹将脊髓压缩10分钟,从而完成制备脊髓损伤的动物模型。在脊髓损伤手术后,约2周期间,持续每天压缩膀胱两次,直到可自发排尿为止。在制备脊髓损伤动物模型后一周,如图10所示,将根据一个实施方式的水凝胶贴片直接移植到脊髓损伤部位。通过使用脊髓损伤动物模型评估方法(脊髓损伤的行为学测试(BBB test);旷场试验)进行行动评估8周,并且将两个观察者的观察结果的平均值作为测试结果,结果示于图15至图18。
图11显示了观察脊髓损伤动物模型在脊髓损伤后一周,其后腿的活动的图像。图12显示了观察在没有移植水凝胶贴片的情况下,仅将磷酸盐缓冲液(PBS)施用于移植部位的脊髓损伤动物模型在脊髓损伤后8周,其后腿的活动的图像。图13显示了观察移植了水凝胶贴片2(不包括细胞生长因子的水凝胶贴片2)的脊髓损伤动物模型在脊髓损伤后8周,其后腿的活动的图像。图13显示了观察移植了水凝胶贴片5(包括细胞生长因子的水凝胶贴片2)的脊髓损伤动物模型在脊髓损伤后8周,其后腿的活动的图像。
图15至18显示根据一个实施方式的根据水凝胶贴片的移植所获得的脊髓损伤的行为学(BBB)评分。
如图11至图14所示,已证实当在没有移植水凝胶贴片的情况下,单独将磷酸缓冲盐溶液涂布于移植部位时,脊髓损伤动物模型在脊髓损伤后8周,后腿的运动能力没有变化。然而,当移植水凝胶贴片2时,脊髓损伤动物模型在脊髓损伤后8周,恢复了左后腿的运动能力,以及当移植包括神经细胞生长因子和血管内皮生长因子的水凝胶贴片5时,脊髓损伤动物模型在脊髓损伤后8周,显着恢复其两只后腿的运动能力。
此外,通过使用脊髓损伤动物模型评估方法(脊髓损伤的行为学测试;旷场试验)评估脊髓损伤恢复能力。评估结果表明,如图15至18所示,当仅在移植部位上涂布磷酸缓冲盐溶液而不移植水凝胶贴片时,脊髓损伤动物模型的脊髓损伤的行为学评分为4至6分。这属于脊髓损伤再生过程的早期阶段。然而,当移植水凝胶贴片2时,脊髓损伤动物模型的脊髓损伤的行为学评分为约8至约10分。这表明脊髓损伤再生过程的中间恢复阶段。当移植水凝胶贴片5时,脊髓损伤动物模型的脊髓损伤的行为学评分为约10至12分。这表明脊髓损伤再生过程的后期恢复阶段。
如图15至18所示,当移植水凝胶贴片1或水凝胶贴片4时,髓损伤动物模型的脊髓损伤的行为学评分为约6至8分。这表明脊髓损伤再生过程的早期阶段。这些结果表明,与其他组合相比,含有纤维蛋白,层粘连蛋白和透明质酸的水凝胶贴片具有显着的效果。
此外,将水凝胶贴片2和水凝胶贴片5与包括胶原蛋白的水凝胶贴片3和水凝胶贴剂片6进行比较时,可见脊髓损伤的行为学评分没有显着差异。
另外,将水凝胶贴片3与水凝胶贴剂3和磷酸缓冲盐溶液的组合进行比较,发现脊髓损伤的行为学评分没有显着差异。该结果表明细胞培养液不影响水凝胶贴片的功效。
因此,所述结果证实了根据一个实施方式的水凝胶贴片具有显着恢复脊髓损伤的运动能力的治疗效果。
4-2.确认使用组织学分析的水凝胶贴片的脊髓损伤治疗效果
通过组织学分析确认了根据一个实施方式的水凝胶贴片的脊髓损伤再生效果。
具体而言,为了使用组织学分析以确认水凝胶贴片对脊髓损伤位点的神经细胞再生作用,在水凝胶贴片移植手术后8周,将大鼠牺牲,然后通过使用4%(w/v)的多聚甲醛(Merck)灌注以收集脊髓。此后,将脊髓切成10μm的厚度,并通过使用抗神精丝(NF)(1:3000,Abcam),抗胶质纤维酸性蛋白(1:1000,Abcam),抗2'3'-环核苷酸磷酸3'-磷酸二酯酶(CNPase)(1:800,Abcam),抗同源框HB9(Hb9)(1:100,DSHB),及抗离子钙结合衔接分子(Iba-1)(1:500,abcam),以与实验例1中相同的方式对切割的脊髓样品进行染色。此后,通过使用共聚焦显微镜(LSM 700,Zeiss)拍摄水凝胶贴片植部位以评估神经再生,其结果如图19所示。
结果,如图19所示,与在移植部位施用涂布磷酸缓冲盐溶液的情况相比,当移植水凝胶贴片2或水凝胶贴片5时,在脊髓损伤病变的部位,胶质纤维酸性蛋白的星型胶质细胞的分布较低,NF经细胞的分布较高。另外,当移植水凝胶贴片2或水凝胶贴片5时,环核苷酸-3'磷酸水解酶(CNPase)少突胶质细胞与NF神经细胞共定位于相同的位置。这些结果表明,由于涂布水凝胶贴片,神经细胞的再生及再生的神经细胞的髓鞘形成有显着增加。
此外,当磷酸缓冲盐溶液涂布于移植部位时,NF神经细胞的分布很小,并且显示炎症反应的Iba-1小胶质细胞的分布很高。然而,当移植水凝胶贴片2或水凝胶贴片5时,表现脊髓的Hb9及NF的运动神经细胞的分布高,并且Iba-1小胶质细胞的分布显着降低。这些结果表明,在水凝胶贴片移植后因脊髓损伤的炎症反应减弱,从而抑制了神经损伤,并促进了运动神经细胞和和少突胶质细胞的再生,以及促进了形成了髓鞘的运动神经细胞的再生。
这些结果表明,水凝胶贴片具有显着增加诱导受损脊髓神经的再生和抑制星型胶质细胞活化以恢复丧失的运动能力的治疗效果。以及可通过使用水凝胶贴片基本上抑制因使用注射器的脊髓损伤治疗而引起的继发性脊髓损伤。
Claims (28)
1.一种水凝胶贴片,其包括:
纤维蛋白、或纤维蛋白原、或纤维蛋白和纤维蛋白原;
层粘连蛋白;及
透明质酸或其盐。
2.根据权利要求1所述的水凝胶贴片,其进一步包括细胞生长因子。
3.根据权利要求2所述的水凝胶贴片,其中
所述细胞生长因子包括神经细胞生长因子、血管内皮细胞生长因子、成纤维细胞生长因子、骨形态发生蛋白质、表皮细胞生长因子、肝细胞生长因子、转化生长因子、或其组合。
4.根据权利要求3所述的水凝胶贴片,其中
所述神经细胞生长因子包括选自脑源性神经营养因子(BDNF)、胶质细胞源性神经营养因子(GDNF)、睫状节神经细胞营养因子(CNTF)、人碱性成纤维细胞生长因子(bFGF)、环磷酸腺苷(cAMP)、神经营养因子(NT)、NT3、NT4、三碘甲状腺原氨酸(T3)、音猬因子(SHH)及血小板衍生生长因子(PDGF)中的至少一者。
5.根据权利要求3所述的水凝胶贴片,其中所述血管内皮细胞生长因子为血管内皮生长因子(VEGF)。
6.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片不包括细胞。
7.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片具有多孔表面。
8.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片根据温度经历可逆相变以变成固体,半固体或液体。
9.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片涂布到受损组织部位,且所述水凝胶贴片的形状改变成符合受损部位的形状。
10.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片用于再生或修复受损组织。
11.根据权利要求10所述的水凝胶贴片,其中所述受损组织是脊髓。
12.根据权利要求10所述的水凝胶贴片,其中所述受损组织是慢性脊髓损伤。
13.根据权利要求1所述的水凝胶贴片,其中
在所述水凝胶贴片中,纤维蛋白或纤维蛋白原的浓度范围为0.5mg/ml至20mg/ml、层粘连蛋白的浓度范围为1μg/ml至100μg/ml、或透明质酸的浓度范围为10μg/ml至5mg/ml。
14.根据权利要求1所述的水凝胶贴片,其中所述水凝胶贴片以脊髓非侵入性方法用于患者的脊髓损伤部位。
15.根据权利要求1所述的水凝胶贴片,其进一步包括凝血酶。
16.一种用于治疗脊髓损伤的药物组合物,包括:
纤维蛋白、或纤维蛋白原、或纤维蛋白和纤维蛋白原;
层粘连蛋白;及
透明质酸或其药学上可接受的盐。
17.根据权利要求16所述的药物组合物,其进一步包括细胞生长因子。
18.根据权利要求17所述的药物组合物,其中
所述细胞生长因子包括神经细胞生长因子、血管内皮细胞生长因子、成纤维细胞生长因子、骨形态发生蛋白质、表皮细胞生长因子、肝细胞生长因子、转化生长因子、或其组合。
19.根据权利要求17所述的药物组合物,其中

27.根据权利要求26所述的制备水凝胶贴片的方法,其进一步包括:
将神经细胞生长因子、血管内皮细胞生长因子、成纤维细胞生长因子、骨形态发生蛋白质、表皮细胞生长因子、肝细胞生长因子、转化生长因子、或其组合混合至所述溶胶组合物中。
28.根据权利要求26所述的制备水凝胶贴片的方法,其中在4℃至-210℃的温度下在溶液中低温保存或深低温保存水凝胶贴片。
