Abstract

Description
本发明的技术背景
本发明涉及需要注射胰岛素的糖尿病患者的夜间低血糖的控制。本发明采用可以在入夜之前使用的食品条来控制血糖的水平。
一项包括1400名需要定期注射胰岛素的糖尿病患者的重大研究的结果(Diabetes Control and Complication Trial)(Dawson,Clinical Diabetes 11:88-96,1993)已经指出,在那些能够尽量将血糖水平保持接近于正常水平的患者中,发生神经病、视网膜病、以及肾脏疾病的可能性较小。为了获得这种结果,现在的众多糖尿病患者都严格地遵守一种称为“严谨控制”的方案。该方案要求患者严密监视自己的血糖水平(例如,每天进行多次检验),多次注射胰岛素,并且严格控制饮食。但是,这种对血糖的严格控制的副作用是这些遵守该方案的患者中发生低血糖的比率升高3倍,其中的至少一半发生在夜间。
低血糖是一种危险的病状,可以迅速发展并引起对神经系统的损害并使患者进入失去意识的状态,而且,在某些不多的情况下,会发生昏迷。医生都希望患者意识到低血糖的症状,例如,发冷、发抖、心悸、精神活动的改变,从而预防低血糖的发生。然而,在睡眠中,患者无法意识到低血糖的发展,因此,非常需要找到可以预防夜间发生低血糖的处理。
当今,为了防止低血糖,糖尿病患者一般都在入夜前注射胰岛素并伴随进食。注射胰岛素为的是将睡眠中的血糖水平保持在最接近正常的水平上。现在可用的常规食品提供糖源(一般为蔗糖和复合糖),它们在被消化后释放到血液中,并在胰岛素的影响下被细胞吸收作为能量或以糖原或脂肪的形式被保存起来。由于这些形式的糖类的一部分会在消化后迅速地被释放到血液中,所以在夜晚的其它时间内,就会有一部分的活性胰岛素残存在于体内。这些残存的胰岛素就会因为葡萄糖的释放的降低而没有可以作用的碳水化合物,从而导致夜间的低血糖。
欧洲专利EP127287号揭示了一种谷阿胶食品条,用于II形糖尿病患者。该食品条含有的糖量高,但是脂肪量低,并且是安排在患者在进餐前或进餐中食用来降低血液中的胰岛素含量。其中加入的单糖是为了增甜,但是,不鼓励使用蔗糖、葡萄糖和右旋糖。
因此,就需要对现有的食品进行改进,使其可以在夜间延续地释放葡萄糖到血液中,从而维持正常的血糖水平,防止糖尿病患者发生夜间的低血糖。
本发明的目的之一是提供新型的糖尿病患者的补充食品条,该食品条可以保持夜间的稳定的血糖水平,从而防止糖尿病患者在夜间发生低血糖。
本发明的另一个目的是提供一种提供采用进食本发明的糖尿病患者的补充食品条来防止或处理糖尿病患者的夜间的低血糖的方法。
本发明的再一个目的是在入夜前提供本发明的食品条从而提供阶段性释放葡萄糖来控制糖尿病患者的夜间血糖水平。
本发明的目的和特点将在下面的描述和权利要求书中给予更为细致的描述。
本发明概述
本发明提供了一种新型的糖尿病患者的补充食品条,用于防止和处理糖尿病患者的夜间低血糖。在优先的实施例中,糖尿病患者的食品条提供了100千卡/条的养分,并且具有1“面包交换”和1/2“脂肪交换”(1“bread,exchange”and 1/2“fat exchange”,这些术语的定义如美国糖尿病学会所规定)。
糖尿病患者的补充食品条包括约10-60%(重量)的单糖,更为优选的是这些单糖提供每100千卡中的约4-55千卡。最优选的是可以提供每100千卡中的40千卡的单糖。优选的单糖是蔗糖,还可以是葡萄糖或右旋糖。
糖尿病患者的补充食品条还包括约1-25%(重量)的蛋白质,优选为每100千卡中为10-20千卡,最优选为每100千卡中为12千卡。在优选的实施例中,仅仅使用生物学价值最高的蛋白质,例如,乳清、乳清蛋白、酪蛋白、蛋清、蛋粉、大豆、以及去乳糖的奶粉。优选的蛋白质是不含乳糖的。
糖尿病患者的补充食品条进一步包括月2-40%(重量)的脂肪,优选的脂肪提供每100千卡中的约10-40千卡,最优选的提供每100千卡中的约27千卡。在优选的实施例中,脂肪包括中等长度链的三酸甘油酯以及长链三酸甘油酯。中等长度链的三酸甘油酯优选椰油、夏威夷坚果油、棕榈油、棕榈胚油,或它们的混合物。长链的三酸甘油酯优选卡诺拉油、红花籽油、葵花籽油、玉米油、橄榄油、鲱鱼油、花生油、或它们的混合物。在更为优选的实施例中,所提供的脂肪量要足以推迟空胃的到来。
糖尿病患者的补充食品条还进一步包括约1-60%,优选约5-35%(重量)的复合糖,优选的复合糖提供每100千卡中的约10-35千卡,最优选的提供每100千卡中的约20千卡。最为优选的复合糖是未经烹煮的玉米淀粉。在另外的优选实施例中,复合糖可以选自坚果、大麦、通心粉、速煮米、干豆、或它们的混合物。
在另一方面,本发明提供了糖尿病患者的补充食品条,该食品条除了包括上述的成分(例如,单糖、蛋白质、脂肪、复合糖)以外,还包括维生素、和矿物质,依据的是《规定饮食推荐》(RDA,Recommended Dietary Allowance)和《每日进食参考》(RDI,Reference Daily Intake)。
本发明还涉及糖尿病患者补充食品条,该食品条除了包括上述成分外,还包括非活性成分,例如,乳化剂、人造甜味剂和/或调味剂。
本发明还涉及了采用向糖尿病患者提供补充食品条来处理或防止夜间发生低血糖的方法,该糖尿病患者补充食品条包括约10-60%(重量)的单糖、约1-25%(重量)的蛋白质、约2-40%(重量)的脂肪、和约1-60%(重量)且优选约5-35%(重量)的复合糖。
在本发明的另一个方面,本发明提供了控制糖尿病患者夜间血糖水平的方法,采用的是在入睡前给患者提供补充食品条,该食品条包括约10-60%(重量)的单糖、约1-25%(重量)的蛋白质、约2-40%(重量)的脂肪、和约1-60%(重量)且优选约5-35%(重量)的复合糖。
下面的描述和非限制性的实施例将进一步介绍本发明。本发明的详细叙述
本发明糖尿病患者补充食品条的制造,是将单糖、蛋白质、脂肪、复合糖、以及其它的辅助成分一起混合,然后将该混合物均化制成食品条。该食品条可以含有不同数量的卡路里,根据最终用户来确定。
尽管不想被任何的具体理论所限制,据信,本发明的糖尿病患者补充食品条对夜间低血糖的处理和防止,是通过使糖被在夜间按三个阶段释放从而提供了延续性的释放葡萄糖到血液中来供与胰岛素反应。这三个阶段是:1)快速释放单糖,例如释放蔗糖,蔗糖易于吸收并且在食品条被消化后不久就作为葡萄糖进入到血液中;2)较为缓慢的将通过肝的葡糖生成作用从蛋白质中产生的糖释放到血液中的阶段,蛋白质葡糖生成作用可以在单糖中释放的葡萄糖被消耗完了的时候,以缓慢和平稳的流量将葡萄糖释放到血液中;以及3)将对复合糖例如玉米淀粉进行水解得到的单糖缓慢地释放到血液中。复合糖被小肠酶的水解比其它的糖类慢,结果是小肠的吸收发生在该食品条被消化之后的长时间后。中等长度链和长链形式的三酸甘油酯被加入到食品条中是作为能源并且推迟整体的空胃的到来,因此,前述各个过程都缓慢地发生。
在实践本发明的过程中,蔗糖是优选的单糖。虽然可以使用葡萄糖和右旋糖,但是,应该避免使用果糖,因为果糖对代谢产生不利的副作用,包括产生乳酸,影响肝的葡糖生成作用。
本发明的糖尿病患者补充食品条中的脂肪包括中等长度链的和长链三酸甘油酯的混合物。适宜的中等长度的三酸甘油酯的例子包括椰油、夏威夷坚果油、棕榈油、棕榈胚油、以及它们的混合物。适宜的长链的三酸甘油酯的例子包括卡诺拉油、红花籽油、葵花籽油、玉米油、橄榄油、鲱鱼油、夜来香油、花生油、以及它们的混合物。加入到糖尿病患者的补充食品条中的脂肪量应该足以延迟空胃的到来。其它种类的中等长度链和长链的脂肪在美国专利地4,703,062号中有讨论,该篇文献通过在此引述而合并于本文。
蛋白质可以是任何适用于营养配方的蛋白质,例如大豆蛋白、酪蛋白、乳清蛋白、动物和植物蛋白、以及它们的混合物。为了使食品条的用途最大,应该使用不含乳糖的蛋白质,从而使其可以用于对乳糖过敏的患者。在选择蛋白质时,首先应该考虑蛋白质的生物学价值,现在发现的是,在酪蛋白、乳清、乳清蛋白、蛋清和全蛋蛋白的生物学价值最高。然后,再考虑价格因素,最好的结合是价格低但生物学价值高。
优选的复合糖是未经烹煮的玉米淀粉,因为其糖份的含量比较均匀,其代谢率是已知的和一致的,还可以被加工为易于食用的食品条。在实践本发明中还可以使用的复合糖包括,坚果、大麦、蒸谷麦、通心粉、速煮米、干豆、以及它们的混合物。未经烹煮的玉米淀粉的市售种类有,商品名为“mellojel”的来自国家淀粉和化学公司的产品。此外,对于不到两岁的儿童,优选的复合糖是随着胰酶一起提供给儿童的,而且,胰酶的用量要能够促进对复合糖的消化。胰酶是作为附剂使用的,一般的用量为约1/4茶勺胰酶对5克复合糖。
加入乳化剂的目的是稳定本发明的糖尿病补充食品条,乳化剂的例子有卵磷脂。
在本发明的糖尿病患者补充食品条中还可以加入调味剂来使其更为可口。调味剂的形式可以是浓缩物,挥发性油,巧克力风味,花生酱风味,糖果风味,香草风味,或者是商业上可以购买的调味剂。例如,如果使用的调味剂中含有脂肪(例如,巧克力风味会含有黄油脂肪),则应该对糖尿病患者补充食品条的脂肪成分进行相应的调整,使最终的脂肪含量不变,例如,最终脂肪浓度不超过脂肪重量的40%。
可以在本发明的糖尿病患者食品条中加入防腐剂来增加其商品寿命。优选使用的防腐剂包括,山梨酸钾、山梨酸钠、苯甲酸钾、苯甲酸钠、EDTA等。
除了糖以外,还可以在本发明的糖尿病患者补充食品条中加入人造增甜剂,例如,人造糖类、环己基氨基磺酸钠、甜味素。应当避免使用山梨糖作为甜味剂,因为山梨糖不能够被糖尿病患者很好地代谢,而且在数量较大时,会引起腹泻。
为了在整夜睡眠期间避免低血糖,本发明的糖尿病患者补充食品条应该在入睡前提供给患者。例如,食品条可以设计成为提供100千卡营养的形式为1面包交换和1/2脂肪交换。然而,本发明还包括其它的交换值的食品条,依据的是每一条的具体卡路里数值。这些交换值可以由美国糖尿病学会以及美国糖尿病患者协会来很快地确定。本发明的食品条提供了足够的循环糖类,所以,患者在超过正常的睡眠时间后,不需要补充葡萄糖,而且在经过约6小时至8小时的睡眠后,其葡萄糖水平在60mg/分升或更高。此外,本发明的糖尿病患者补充食品条可以被分成小块儿,例如,分成4块儿,从而可以每次补充控制量的卡路里。本发明还包括其它形式的对食品条的分割,例如,按照每块儿提供10、20、25、50卡路里的要求来分割。因此,在入睡前进食的食品条的数量,可以根据患者每日已经摄入的食品量来调整,例如,根据患者入睡前消化的食品交换值来确定。
在本文中使用的一些术语的定义如下。
术语“糖尿病患者”指的是那些需要注射胰岛素来将血糖水平维持在接近正常水平的人们,例如维持血糖水平在80-120mg/分升。例如,患有I型或II型糖尿病并且需要定期注射胰岛素的患者。
术语“低血糖”指的是由于血液中葡萄糖含量低而造成的临床现象,例如,血液中全血的葡萄糖水平低于或等于3.0mmol/升或60mg/分升。低血糖的症状可以被划分为两类。肾上腺素的快速释放引起出汗、发抖、心动过速、急噪和饥饿。中枢神经系统的症状包括头昏、头痛、视觉模糊、思维的锐利性下降、思维混淆、行为异常、惊厥、失去知觉。

本实施例给出制造本发明的糖尿病患者补充食品条的一种加工方法。
该方法可以由药剂师或其它的受过训练的人员来手工配制。制造本发明的糖尿病患者补充食品条的方法可以是将所有的成分放入混合器中进行混合,然后将它们制成条。每条的总重量最好不超过25克,包括添加剂在内。等价方案

10-60%重量的单糖,选自蔗糖、葡萄糖、所组成的一组;
1-25%重量的蛋白质;

1-60%重量的复合糖,
其中所述的糖尿病的补充食品条用于防止依赖胰岛素
的糖尿病患者的夜间低血糖,并且其中不含有瓜儿胶。
