Abstract
本发明涉及一种胆汁及结石收集装置,具体地说是一种保胆手术中胆汁及结石收集装置。本发明收集装置由双层聚乙烯塑料膜压制而成,包括若干相连通的气柱,两个支撑杆,充气管;充气管一端连接任意一气柱,经充气管充气后,气柱撑开使所述收集装置呈盒状,且盒状一端为凸出的筒形气柱。本发明制备工艺简单,成本低廉,在手术中可防止胆囊切口出血,避免胆汁流入腹腔内,可防止碎石及胆泥进入污染腹腔,保证手术顺利进行,且操作简便。
Description
技术领域
本发明涉及一种胆汁及结石收集装置,具体地说是一种保胆手术中胆汁及结石收集装置。
背景技术
胆囊结石是我国的常见病和多发病,可引起急性胆囊炎、胆源性胰腺炎,严重影响人类生活健康。胆囊结石传统的治疗方案为胆囊切除术,随着内镜技术的发展及人们对胆囊功能保护意识的逐渐加强,取出结石、保留有功能的胆囊是目前治疗胆囊结石的主要手术方案之一,其中腹腔镜下内镜微创保胆取石术具有创伤小、恢复快等优点。
腹腔镜下内镜微创保胆取石术的实施方法是先行在剑突下、肚脐及右上腹三个切口,分别置入一个0.5cm及两个1.0cm戳卡,腹腔镜经肚脐处戳卡置入,电离钩经剑突下戳卡置入,先在胆囊体部做一直径与胆囊最大结石直径一致的切口,然后经右上腹戳卡置入胆道镜,胆道镜经胆囊切口进入胆囊腔内,使用取石网篮及吸附器取出胆囊内的结石、泥沙样结石及胆泥,最后缝合胆囊切口。
目前临床采取腹腔镜下内镜微创保胆取石术中存在以下不足:1.胆囊切开后,胆囊内胆汁可经胆囊切口流入腹腔,进而污染腹腔,是引起术后感染、腹腔粘连、腹痛的原因之一,为了降低上述并发症的发生率,术后往往需行腹腔冲洗并放置腹腔引流管,以引流残留的胆汁及冲洗液;2.胆囊壁切开呈条状,胆道镜每次经胆囊切口进入胆囊时,需反复调整角度,而且胆道镜经胆囊切口反复进出胆囊过程中,可引起胆囊创面的损伤及出血;3.使用胆道镜对胆囊腔进行探查、取石过程中,需向胆囊内注入生理盐水,方可保持胆道镜视野的清楚,治疗过程中大量的冲洗液及胆汁可经胆囊切口流入腹腔内,进而无法腹腔组织,导致患者腹腔的腹软及体热的丢失;4.胆道镜检查及碎石过程中,胆囊内胆泥、小结石、碎石及血块容易经胆囊切口进入腹腔内,上述物质难以清除干净,最终被大网膜包括,是导致术后腹腔粘连、感染、慢性腹痛等的重要原因。
发明内容
为解决现有技术中存在的缺陷,本发明提供一种保胆手术中胆汁及结石收集装置。
为实现上述发明目的,本发明采用如下技术方案:
保胆手术中胆汁及结石收集装置,所述收集装置由双层聚乙烯塑料膜压制而成,包括若干相连通的气柱,两个支撑杆,充气管;充气管一端连接任意一气柱,经充气管充气后,气柱撑开使所述收集装置呈盒状,且盒状一端为凸出的筒形气柱。
上述保胆手术中胆汁及结石收集装置,所述装置远离筒形气柱一端设有牵引线。
上述保胆手术中胆汁及结石收集装置,所述充气管为双通道管,通道Ⅰ与气柱相通,另一个通道Ⅱ与胆汁引流管连通。
上述保胆手术中胆汁及结石收集装置,所述胆汁引流管头端封闭,尾端与通道Ⅱ相连通,侧壁带孔。
上述保胆手术中胆汁及结石收集装置,所述充气管一端连接有接头,接头上分别有注气接头和吸引接头,注气接头与通道Ⅰ相连通,吸引接头与通道Ⅱ相连通。
上述保胆手术中胆汁及结石收集装置,所述盒状收集装置底部有带弧状角度的气柱。
与现有技术相比,本发明具有以下优点:
1.本发明装置由双层聚乙烯塑料膜压制而成,其制作工艺较为简单,成本较低;
2.本发明装置在充气前呈柱形,可通过支撑杆,经右上腹1.0戳卡孔将其轻松置入腹腔内;
3.本发明装置充气后自然弹开呈盒装,通过双成塑料膜之间的气柱及两端的支撑杆,保持其盒装形态;
4.本发明装置一端设有筒形气柱,经胆囊切口置入,充气后呈圆筒形,对胆囊切口具有支撑作用,具有压迫止血及防止胆汁流入腹腔的作用,可防止切口迟发性出血,对胆汁收集盒具有固定作用,而形成的圆筒形气柱,可方便胆道镜进出胆囊;筒形气柱外径为1-3cm,适合支撑不同大小的切口;
5.本发明装置盒状底部形状符合胆囊周围组织的生理结构,对胆囊周围组织刺激小,便于结石及胆汁流入胆汁引流管头一侧;
6.本发明中引流管头端游离在胆汁收集盒上方,可避免小结石对引流管侧孔的堵塞,同时能对结石上方的胆汁进行充分引流;
7.本发明中胆汁收集盒口内有收紧线,收紧牵引线后,可将本装置经腹腔镜戳卡取出。

具体实施方式
以下结合具体实施方式对本发明做进一步说明。
保胆手术中胆汁及结石收集装置,由双层聚乙烯塑料膜压制而成,设置若干相连通的气柱1,并将气柱制成所需形状,以满足充气后所需要形成的盒状,所述收集装置有两个支撑杆2支撑,在未充气的状态,收集装置通过支撑杆将其卷成柱状,可经右上腹1.0戳卡孔将其置入腹腔内;所述收集装置还设置有充气管3,充气管为双通道管,通道Ⅰ6可与任意一气柱相通,另一个通道Ⅱ7与胆汁引流管8连通,胆汁引流管头端封闭,尾端与通道Ⅱ7相连通,侧壁带孔9,胆汁引流管固定在盒状收集装置底部位置,以胶粘住使引流管头端向上,充气管3一端连接任意一气柱,充气管另一端连接有接头10,接头上分别有注气接头11和吸引接头12,注气接头11与通道Ⅰ6相连通,吸引接头12与通道Ⅱ7相连通。经注气接头11充气后,所有气柱撑开使所述收集装置呈敞口盒状,且盒状一端为凸出的圆筒形气柱4,以供胆道镜进出,筒形气柱外径为1-3cm,适合支撑不同大小的切口;根据胆囊周围组织的生理结构,盒状收集装置的底部为斜坡装,底部位置的气柱制作成带弧状角度的气柱13,以满足充气撑开后底部成斜坡状。所述装置远离圆筒形气柱一端设有牵引线5,使收集装置收紧,便于取出。
使用时,经口右上腹胆道镜用戳卡孔将胆汁及结石收集装置置入到腹腔内,经注射接头置入适量空气或生理盐水,其弹开呈盒状。将收集装置的筒形气柱经胆囊切口置入到胆囊腔内,经注射继续置入适量空气或生理盐水至筒形气柱呈充盈状态,于本装置盒底部放置一块纱布。胆道镜经本装置圆筒形气柱进入胆囊内检查、冲洗、取石及碎石,当胆汁、冲洗液、胆泥及碎石进入盒内后,通过吸引接头,对冲洗液进行负压吸引,流入盒内的液体经胆汁引流管头端负压吸出,结石、胆泥等有形物质则过滤到盒内纱布上,胆道镜操作结束后,直径 1cm以上的结石,在盒内使用异物钳或碎石网篮勒碎,直径1cm以下的结石及纱布经取石袋取出。收紧牵引线,并抽净盒内气体,将本装置经1.0腹腔镜戳卡引出。

1.保胆手术中胆汁及结石收集装置,其特征在于,所述收集装置由双层聚乙烯塑料膜压制而成,包括若干相连通的气柱,两个支撑杆,在未充气的状态,收集装置通过支撑杆将其卷成柱状,可经右上腹1.0戳卡孔将其置入腹腔内;所述收集装置还设有充气管,充气管一端连接任意一气柱,经充气管充气后,气柱撑开使所述收集装置呈盒状,且盒状一端为凸出的筒形气柱,以供胆道镜进出,筒形气柱外径为1-3cm,适合支撑不同大小的切口;所述充气管为双通道管,通道Ⅰ与气柱相通,另一个通道Ⅱ与胆汁引流管连通;所述盒状收集装置底部有带弧状角度的气柱。
2.如权利要求1所述的保胆手术中胆汁及结石收集装置,其特征在于,所述装置远离圆筒形气柱一端设有牵引线。
3.如权利要求1所述的保胆手术中胆汁及结石收集装置,其特征在于,所述胆汁引流管头端封闭,尾端与通道Ⅱ相连通,侧壁带孔。
4.如权利要求1所述的保胆手术中胆汁及结石收集装置,其特征在于,所述充气管一端连接有接头,接头上分别有注气接头和吸引接头,注气接头与通道Ⅰ相连通,吸引接头与通道Ⅱ相连通。
