技术领域
本发明涉及营养流食,尤其是涉及一种能形成粪便并能及时促进的营养流食及其制备方法。
背景技术
流食是一种食物呈液体状态、在口腔内能融化为液体,比半流质饮食更易于吞咽和消化无刺激性的食物。流食适用于极度衰弱无力咀嚼食物的重症患者,如高烧、昏迷、脑受损、癌症、植物人、口腔、面颊部及外科手术前后以及急性胃肠炎、食道狭窄等疾病患者。此种膳食只能短期应用,作为过渡期的膳食。该营养流食可以慢慢口服,也可以借助胃管或鼻饲管滴加或泵吸饲喂。
中国有大型3甲医院近1000家(床位在500以上);2甲及以下医院约1.9万家(平均床位500以下),私营医院约1.2万家(平均床位200~300),每个月临床上有手术患者1000万左右,其中各种癌症手术人数约50万,胃肠道疾病手术患者还有50万左右。这些患者手术后3~6天几乎都不可以摄入正常固体食物,只能进流食。进流食也好,打葡萄糖、氨基酸也罢,患者都会面临一个非常难受的问题:几天甚至十几天没有大便(肠内无法形成粪便)!至今未见有流食食品企业生产相同食品。
排粪便(医学上叫排遗)是人体排代谢废物的主要途径,排尿、放屁、呼吸和出汗都不是主要途径,是一部分而已,只有排便,才是肝、胆、肠排泄的主通道。正常人如果每天只排便一次,属于轻度便秘;排2~3次/天是正常的排泄。多于3次,为腹泻,每周3次属于重度便秘,少于3次为极重度便秘。
人的肠道有8~10m长(BAYLOR医学院解剖学是人身高的7~9倍),并有千褶百皱,平均每隔3.5cm就有一个弯折,人们即使每天都排泄,也总会有一些食物残渣滞留在肠道的褶皱内,它们在细菌的作用下干结、腐败、发酵,日积月累,这些食物残渣最终形成厚达5~7cm,重达6.5kg的黑色、恶臭、有毒的物质,并像锈一样牢牢地粘连在肠壁上,其坚硬程度与轮胎相似,它们慢慢侵蚀着我们的身体,人们习惯称之为——宿便(有的学者不认同这个提法)。
其实人只要有摄入食物(流食、鼻饲也好,挂瓶滴注也好,都等同进食)就会有代谢副产物产生,形成排泄物,这些副产物必须排除体外,否则就不利于免疫力的提高,不利于电解质和酸碱平衡,不利于营养吸收和伤口愈合,最终不利于身体康复。
临床上使用的鼻饲流食,提供了部分营养,但不能够在肠道有效形成粪便;家常饮食比如补充营养瘦肉排骨汤、清热化痰白萝卜、冬瓜制作成的菜泥/汤;健脾化湿薏米红豆汤等等,对于手术后消化系统没有恢复正常的患者,都不能很好形成粪便,都无法在24h内让代谢废物有效排出。
宿便对人体健康极大的危害:
1)宿便会引起肛肠疾患。宿便和毒素长期积存在肠道里,会诱发多种肠道疾病,可直接引起或加重肛门直肠疾患,如直肠炎、肛裂、痔疮,包括结肠炎、结肠溃疡、结肠憩室炎,甚至直肠癌、结肠癌等恶性疾病。严重便秘者约10%患结肠癌。
2)宿便诱发心、脑血管疾病。肠内毒素进入血液后,会加重高血压、高血糖、高血脂、糖尿病、老年痴呆等患者症状,导致病情恶化。由于便秘使得病人排便非常困难,需要病人在排便时不断加大腹内压,使得一些高血压、心脏病和脑血栓病人由于腹内压的刺激性反射增强,导致血压升高,心率加快,回心血量和脑血流量的突然增多,导致中风和脑梗塞死亡的情况。在医学上叫做马桶上的猝死。硫化氢对大脑的危害更为严重。硫化氢进入大脑后会直接与大脑中的多巴胺受体结合,使多巴胺的结合利用率下降,导致脑神经元的传递受阻,使大脑出现萎缩,病人的认知能力大幅下降,也就是常见的老年痴呆。
3)宿便中的毒素被肠道反复吸收,通过血液循环到达人体的各个部位,导致女性面色晦暗无光、皮肤粗糙、痤疮、腹胀腹痛、口臭、痛经、月经不调、心情烦躁等。宿便易使妇女发生痛经、阴道痉挛,并产生尿潴留、尿路感染等症状。
4)宿便堆积导致人的排泄系统失调,肠道内的食物残渣不能及时排出,积存在肠道褶皱内并被肠壁吸收。渐渐地,女性纤美的体形走了样,男士的将军肚日渐隆起!
5)宿便影响大脑功能。久滞于肠道中的残渣在细菌的作用下,产生大量有害物质,如甲烷、酚、氨等,这些物质部分扩散进入中枢神经系统,干扰大脑功能,突出表现是记忆力下降、注意力分散、思维迟钝、头痛频发、失眠、烦躁易怒等。
6)对肝脏功能的危害:
毒素经大肠吸收通过门静脉进入肝脏并到全身。毒素增加了肝脏的解毒负担,大大降低了肝脏激素灭活、调节糖代谢、合成血浆蛋白及分泌胆汁的功能。由于宿便长期停留在肠道中,使得大肠在吸收水份的同时将本应通过粪便排出体外的大量甲基吲哚吸收回血液中,造成肝脏解毒功能的负担加重,导致肝功能降低,进而加速肝细胞的凋亡和变性,任其发展便形成肝硬化。由于甲基吲哚具有很强的致癌作用,如不及时治疗则易转化为肝癌。其次,当肝功能严重受损时,大肠从宿便中吸收的氨不能被肝脏及时代谢,高浓度的氨透过血脑屏障后,抑制大脑皮层的兴奋性,使病人对外界刺激的反应降低,并出现嗜睡、昏睡,甚至是深度昏迷的情况,临床上称之为肝昏迷。
7)对肾脏功能的危害:
宿便中的部分氨在血液中会与水合成为尿素、肌酐等代谢产物。由于便秘使得大量的氨在血液中合成为尿素和肌酐,加大了肾脏的负担,使肾脏的功能受损。一旦肾脏的功能被破坏,将会对人体的泌尿系统、内分泌系统和造血系统造成严重的危害。
8)对免疫系统的危害:
毒素的积累大大影响身体各个脏器的功能,降低人体免疫力,严重影响人体对疾病的抵抗力,是亚健康状态的根源。宿便的毒素可以分布到神经突触和神经——肌肉接头处,直接损害神经元、造成中枢神经受损、身体各器官免疫力下降,形成亚健康。
宿便的毒素渗入肺会使呼吸短促和口臭,引发肺病!
对于许多人患者和许多疾病情况,排泄(排余)比吃食物更加重要。尤其是全身麻醉动过手术的患者,胃肠道的苏醒需要3~7天时间。在这期间,胃肠道的蠕动非常弱,消化分泌腺体功能受损,消化液分泌不足,所以固体食物限制摄入,等到排气(放屁)现象正常了,才慢慢恢复饮食。为此针对摄入有困难的人群而言,流食不仅仅要提供维持生存的营养问题,更加要考虑到排除对生命形成威胁的不利因素,要能够帮助形成粪便和矢气(屁)并让粪便毒素和废气正常从体内排除到体外。
中国专利CN105995971A公开一种促进糖尿病康复的营养流食及其制备方法,将大豆蛋白粉、麦芽糊精烘干后过180~240目筛网;称取大豆寡肽粉、麦芽糊精、大豆蛋白粉、蔓越莓提取物、海马提取物、仙人掌提取物、综合营养素和酪蛋白,加入纯化水混合并采用磁力搅拌均匀;在搅拌均匀的物料中依次加入橄榄油、乳化剂和稳定剂,均质3~5min;均质后减压干燥灭菌,得到粉剂,粉剂以45g/袋的规格分装,每45g粉剂添加170ml温度为60℃的温水,配置成200ml营养补充或日常代餐营养流食,适用于糖尿病、应急性糖尿病患者,生物利用率高,能清除自由基,提高机体免疫力,吸收效果好,水溶性、非水溶性膳食纤维科学配比,延缓血糖吸收,平稳餐后血糖促进胰岛功能恢复,改善机体糖耐量。
中国专利CN102283342A公开一种航天用功能性全营养流食及其制备方法,其目的是为了提供一种调节脾胃功能、延缓肌肉萎缩和骨钙丢失、维持肠道菌群平衡、调节免疫功能的航天用功能性全营养流食及其制备方法。该发明每1L流食中包括下列组分:山药2~10g、党参1~8g、当归0.5~5g、阿胶0.5~5g、黄芪1~10g、炒麦芽1~8g、甘草0.2~6g、茯苓2~10g、山楂1~7g;碳水化合物100~200g;蛋白质30~80g;脂类20~60g;适量维生素、微量矿物元素及电解质;益生元1~3g:水苏糖和/或大豆低聚糖;乳化稳定剂3-13g。
中国专利CN106038559A公开一种促进腹部术后胃肠功能恢复的中药组合物以及该中药组合物在制备促进腹部术后胃肠功能恢复的药物中的应用。
发明内容
本发明的目的在于提供可恢复脾胃功能、维持肠道菌群平衡、唤醒胃肠道蠕动、调节免疫功能、非常容易形成粪便和矢气(屁)并能及时排出的一种能形成粪便并能及时促排的营养流食及其制备方法。
所述能形成粪便并能及时促排的营养流食的原料按质量比的组成为藜麦10~20g、山药5~10g、铁皮石斛2~8g、车前子壳2~8g、党参1~5g、当归0.5~5g、甘草0.2~5g、茯苓1~5g、鲍鱼多肽1~4g、鸡内金0.5~3g、莱菔子1~3g、白术1~3g、维生素5~10mg、微量矿物元素5~10mg、电解质平衡元素1~3g、牛磺酸及益生菌粉1~3g、聚葡萄糖1~3g、食品乳化稳定剂和触变剂0.1~1g。
所述维生素可选自维生素C、维生素K、维生素B、吡哆醇、生物素、叶酸、烟酸和硫胺素等中的至少一种,维生素对胃肠道粘膜(柱状上皮细胞)提供抗氧化、修复所需要的营养,促进胃肠道上皮细胞微绒毛和纤毛的生理再生(3~5天)和病理性再生(4~6天)。
所述微量矿物元素可选自葡萄糖酸锌、葡萄糖酸钙、葡萄糖酸亚铁等的至少一种;

所述食品乳化稳定剂和触变剂可为黄原胶(Xanthangum),俗称玉米糖胶,是一种糖类(葡萄糖、蔗糖、乳糖)经由野油菜黄单孢菌(en:Xanthomonascampestris)发酵产生的复合多糖体。黄原胶通常是经由玉米淀粉所制造,经好氧发酵生物工程技术,切断1,6-糖苷键,打开支链后,在按1,4-键合成直链组成的一种酸性胞外杂多糖。黄原胶分子由D—葡萄糖、D—甘露糖、D—葡萄糖醛酸、乙酰基和丙酮酸构成,相对分子质量在2×106~5×107之间,它的一级结构是由β—(1→4)键连接的葡糖基主链与三糖单位的侧链组成;其侧键由D—甘露糖和D—葡萄糖醛酸交替连接而成,分子比例为2︰1;三糖侧链由在C6位置带有乙酰基的D—甘露糖以α—(1→3)链与主链连接,在侧链末端的D—甘露糖残基上以缩醛的形式带有丙酮酸,其高级结构是侧链和主链间通过氢键维系形成螺旋和多重螺旋。黄原胶的二级结构是侧链绕主链骨架反向缠绕,通过氢键维系形成棒状双螺旋结构。黄原胶的三级结构是棒状双螺旋结构间靠微弱的非共价键结合形成的螺旋复合体。黄原胶的特殊分子三维结构,赋予它具有优良的假塑性、增稠性、悬浮性、乳化性和水溶性,并具有良好的热、酸、碱、盐稳定性,同时对酶解(蛋白酶和淀粉酶)具有稳定性,所以被广泛应用于各种食品中(国家标准GB13886-2007)。本发明充分利用黄原胶的水溶液在静态或低的剪切作用下具有高粘度,在中高剪切作用下表现为粘度急剧下降,但分子结构不变的特征,制作流质食品,这种假塑性赋予流质食品稳定成型(粪便)、同时又让饲喂方便操作。

藜麦有几个特性:第一,它不像水稻、小麦、燕麦一类主粮属于禾本科单子叶植物,它是笕科藜属(双子叶)植物(常见的有菠菜、甜菜);第二,它耐贫瘠、耐干旱、耐盐碱、耐寒,对环境的适应性非常强;第三,它的蛋白质品质与鸡蛋相当,富含球蛋白和白蛋白。它的这些特征都是由藜麦的特殊生物学构造、营养和遗传特性决定的。
藜麦是唯一的全谷全营养完全蛋白碱性食物,胚芽占种子的32%;胚乳占68%,没有外胚层和麸质,胚芽和胚乳都具有营养活性和生物活性,蛋白质含量高达16%~22%(牛肉20%),氨基酸组成与动物蛋白质接近,比例适当且易于吸收,尤其富含植物中缺乏的赖氨酸,钙、镁、磷、钾、铁、锌、硒、锰、铜等矿物质营养含量高,富含不饱和脂肪酸、类黄酮、B族维生素和维生素E、胆碱、甜菜碱、叶酸、α-亚麻酸、β-葡聚糖以及Lunasin等多种有益化合物,膳食纤维素含量高达13.4%,低脂,低热量(305kcal/100g),低升糖(GI升糖值35,低升糖标准为55),几乎都是常见食物里最优秀的。
对藜麦的加工,不仅局限于简单的加热烹饪、磨粉或干燥,而且是让其发芽,胚芽的活性物质达到最高,胚乳营养开始转化为可被胚芽发育充分利用的时候,进行远红外真空变温负压干燥技术、低温负压膨化技术加工而成。保留了营养成分,提高消化吸收率,同时增加或改善了口感。
所述鲍鱼肽是鲍鱼加工过程的副产品(内脏及粘液),经过酶解,去腥味、超滤除菌,低温冷冻真空干燥而得。分子量范围在180~1000D的占75%以上。经过细胞培养试验、人体皮肤涂抹试验、小白鼠喂养试验,证明从食物海鲜鲍鱼来源的鲍鱼肽,无毒,对细胞修复、提供营养、保湿美白效果明显。鲍鱼肽高效液相色谱仪检测的分子量分布图谱分析数据(根据GB/T22492-2008)见表1。

中医认为鲍鱼具有滋阴补阳,止渴通淋的功效,是一种补而不燥的海产,没有副作用。《食疗本草》记载,鲍鱼“入肝通瘀,入肠涤垢,不伤元气。壮阳,生百脉”。主治肝热上逆,头晕目眩,骨蒸劳热,青肓内障,高血压,眼底出血等症。鲍鱼的壳,中药称石决明,因其有明目退翳之功效,古书又称之为“千里光”。现代研究表明,鲍鱼肉和其粘液中能分离出不被蛋白酶分解的3种粘蛋白:鲍灵素I、鲍灵素II、鲍灵素III。这些生物活性物质能够提高正常细胞免疫力,破坏癌细胞代谢过程,提高抑瘤率,却不损害正常细胞,有保护免疫系统的作用。鲍鱼能“养阴,平肝,固肾”,可调整肾上腺分泌,调节血压,润燥利肠,大便秘结等疾患。药理研究还证实,鲍鱼肽及其活性物质对链球菌,葡萄球菌,流感病毒,疱疹病毒,脊髓灰质炎病毒均有抑制作用。
小肠柱状上皮细胞的游离面有许多细小突起,称为微绒毛。微绒毛能增加细胞的表面积,有利于小肠吸收营养物质和排遗。这些上皮细胞游离面伸出的微细指状突起,只能在电镜下见到,有些上皮细胞的微绒毛较少,长短不等,排列也不整齐;有些上皮细胞的微绒毛则多而长,且排列整齐。光学显微镜下见到的,小肠柱状上皮细胞表面呈纵纹状的纹状缘在电子显微镜下可见都由密集而排列整齐的微绒毛组成。微绒毛一般长0.5~1.5μm,直径80~90nm。微绒毛的表面常附有一层较厚的细胞衣。微绒毛的中轴含有许多纵行的微丝,它自微绒毛顶部向下延伸,与细胞顶部的终末网的微丝相连。终末网由与细胞游离面平行的微丝组成,其微丝常附着于细胞的中间连接。微绒毛和终末网的微丝的化学成分,分别为肌动蛋白和肌球蛋白,微绒毛的运动与微丝的相互作用有关;微丝的收缩可使微绒毛伸长或缩短。微绒毛的功能主要是扩大细胞的表面积,并参与细胞的吸收、排遗或分泌功能。例如小肠每个上皮细胞的游离面有2000~3000个微绒毛,每平方毫米小肠上皮,则可有2亿多个微绒毛,这样就大大扩大了小肠上皮表面的吸收面积和排遗面积。
此外,上皮细胞游离面还有伸出的能动的毛状突起,叫纤毛,1个上皮细胞可有数百根纤毛。纤毛比微绒毛粗大,长5~10μm。电镜下观察,纤毛的表面有1层约7nm厚的薄膜,是细胞膜的延伸部分,也是肠蠕动的功能器官。
所述通肠益生菌干粉可由武汉真福医药股份有限公司(武汉万德瑞生物技术有限公司)生产,由婴儿双歧杆菌、嗜酸乳杆菌、凝结芽孢杆菌组成,其中包含芦荟大黄素甙(aloin)、芦荟大黄素(emodin)、蒽醌苷和双蒽醌苷。这个组合可以让益生菌黏附于大肠小肠上皮细胞表面的微绒毛和粘膜层上,对致病菌如大肠杆菌有竞争和置换拟制作用,同时还有排阻拟制作用,是拮抗病原菌、提高免疫力、促进受损黏膜修复,最为关键的是对肠道内膜(包括纤毛和微绒毛)产生收敛和蠕动效果。
所述能形成粪便并能及时促排的营养流食的制备方法包括以下步骤:
1)在全部原料中加水,搅拌后得低粘度流食;
2)在低粘度流食中再加水或搅拌,静置后得高粘半流质食物,即能形成粪便并能及时排出的营养流食。
在步骤1)中,所述加水的量可为全部干粉原料总质量的12~20倍,所述搅拌的时间可为5~10min。
在步骤2)中,所述再加水的量可为低粘度流食总质量的8~12倍;所述静置的时间可大于10min。
本发明所制得的高粘半流质食物在剪切力作用下,粘度会下降,利于泵抽灌注设备鼻饲或胃管加食操作;流食到了大肠部位就可以形成松软成型粪便,并且可以短时间内排出体外。
本发明的配方中,藜麦和鲍鱼为正常营养食品;山药、甘草、茯苓、鸡内金、莱菔子、白术是药食同源目录中的食品;党参、当归、铁皮石斛、车前子壳是药食同源中的保健食品。这些食物原材料的组方,可以制作为特殊医学用途(恢复手术患者胃肠道功能)配方食品。
另外,鲍鱼多肽是活性小分子蛋白质(片段),对细胞修复细胞营养非常有利;山药健脾补气;茯苓健脾祛湿;甘草除毒调和;党参当归补气养血,提升免疫力;铁皮石斛滋阴消炎,提升内脏功能;车前子壳消炎吸水保湿,膨胀形成粪便的同时,阻断肝肠循环,加强胆汁分泌,润肠促进肠蠕动,让粪便及时排出体外。
本发明给出了一种恢复脾胃功能、维持肠道菌群平衡、调节免疫功能,非常容易形成粪便的功能性营养流食;关键的是,本发明的配方食品,热量是110大卡/100g(相当于460千焦耳);这个低热量流食非常适合刚刚手术或全身麻醉的患者,不增加胃肠道、肝肾脾负担的情况下,不仅让患者容易摄入食物,更加重要的是,提供容易消化吸收的营养同时,吃下去后要在几个小时内形成粪便,并吸收胃肠道多余渗出液,并及时排出体外。
具体实施方式
以下实施例将对本发明作进一步的说明。
所述能形成粪便并能及时促排的营养流食的原料按质量比的组成为藜麦20g、山药10g、铁皮石斛5g、车前子壳5g、党参1g、当归1g、甘草0.5g、茯苓1g、鲍鱼多肽1g、鸡内金0.5g、莱菔子1g,白术1g,维生素(复合物)5mg、微量矿物元素(复合)5mg,电解质平衡元素2g,牛磺酸及益生菌粉1g、聚葡萄糖1g、食品乳化稳定剂和触变剂(黄原胶)0.3g。
所述能形成粪便并能及时促排的营养流食的制备方法包括以下步骤:
1)在全部原料中加水,搅拌后得低粘度流食;所述加水的量可为全部原料总质量的15倍,所述搅拌的时间可为5~10min。
2)在低粘度流食中再加水或搅拌,静置后得高粘半流质食物,即能形成粪便并能及时排出的营养流食。所述再加水的量可为低粘度流食总质量的10倍;所述静置的时间超过10min。
以下给出具体实施例。
1、形成粪便的作用:200~400mL的流食可以形成200~400g的成型柔软粪便;
2、粪便颜色与内在健康情况关系:该流食正常粪便颜色是黄褐色;如果粪便黑色或柏油样粘黑色,说明上消化道还在出血;如果粪便的暗红色,下消化道出血;如果鲜红色,肛门与直肠出血;如果是果酱样,是肠套叠或阿米巴痢疾感染;如果是白陶土色,是胆道梗阻;如果是米泔水样,是霍乱与副霍乱感染;
3、粪便形状:正常情况是长条索状,比香蕉一样或更加长。如果是栗样,便秘没有完全改善好,冲泡流食时多加一些温开水,平时也多喝水;如果是稀糊状,感染与肠炎症未消除;如果是胶胨状,是慢性致病菌(大肠杆菌、沙门氏菌、志贺氏菌或阿米巴等感染;如果是细细的条带状或粗细分段,有肠道梗阻或直肠狭窄问题;
4、粪便内容物:正常情况是少许半透明水状粘膜;如果粘液多带颜色,说明还有炎症和细菌;如果带浓液,说明有痢疾杆菌和溃疡性结肠炎或大肠炎症;
5、粪便气味:正常情况没有气味;如果有恶臭,胰腺有问题或慢性肠炎;如果腐败臭味,下消化道溃疡;如果血腥臭,上消化道出血或阿米巴感染;如果出现酸败臭,消化不良,减少流食以外的食品。
案例1:罗××,女,67岁;5月31日在某妇幼治疗中心做子宫肌瘤摘除手术。5月28日开始没有吃正常固体食品,5月31日至6月3日未进食,靠注射葡萄糖与氨基酸提供能量;6月4日开始喝米汤、猪肉水,6月7日开始吃面条与稀饭(带米粒),但从5月29日至6月9日(12天)没有排便。医生吩咐起床走动,多喝水;但患者感觉疲劳,四肢无力,心情非常烦躁,也不想进食或喝水,皮肤干涩,不想走动。6月10日下午4点开始第一次服用营养流食粉冲泡至300mL流食,慢慢自己口服,顺利下咽,口感好,4h后(6月10日晚上8点)第一次排出500g左右软粪便,接着每天一次服用营养流食粉流食,排便正常化,心情改善,自己可以在平地上来回走3次,每次走15~20min;此后,每天坚持吃营养流食,保证了每天1~2次排便。
案例2:林××,男,59岁,12月1日在某肿瘤医院做食道癌切除手术。11月27日开始禁食(疼痛无法进固体食物),手术后3天挂葡萄糖与氨基酸,然后一直鼻饲流食到12月11日,从11月27日到12月11日(15天)没有排便,呼出的气有严重腐败味,表情极其痛苦,不想进任何食物。体重从60kg一下子减少至52kg。12月12日开始接受第一次营养流食(每次20g冲泡成200~400mL流食),3h后通便560g黑臭软便,12月13日开始每天吃3次营养流食,每天排便2次,到12月15日排便正常形状,颜色正常,臭味消除。12月16日开始接受米汤、瘦肉汤,营养流食一天一次,大便每天保持1~2次;维持到次年6月10日。
以下给出一般家庭流食制作参考:
一、饮食注意
伤口愈合期要多补充蛋白质,并且要非常注意饮食禁忌,按时吃消炎药。
1、饮食禁忌:忌食酱油、黑芝麻、猪头肉、腐乳、葱、辣椒、韭菜、海鲜、牛羊肉、辛辣、煎炸、豆制品(除豆浆)、鸡肉、鹅肉、食用菌、竹笋、南瓜、菠菜、桃、杏、公鸡、鸡翅、鸡爪、蒜、豆芽、莴苣、茄汁、杨梅、荔枝、甜瓜。
2、宜多食:排骨汤、小番茄、苹果、草莓、香蕉、瘦猪肉、花生、鲜果汁、稀粥、米汤、蛋花汤、蜂蜜、酸奶、黑鱼汤、鸽子汤。
3、宜适量吃(每天的食用量):鸡蛋,至多两个;枸杞,至多30g;维C,果维康一粒加复合桔味泡腾片一粒冲泡饮用;
4、当季当地盛产蔬菜水果,以蔬菜为主(2种),水果(1种)调味,榨汁喝。避免产气、泄气、寒凉的品种。
总之,以上饮食要注意搭配,比如酸奶和水果榨果汁,粥和瘦肉做瘦肉粥等。牛奶和豆浆不能多吃是因为他们属于异构蛋白,产气,效果不如分解蛋白好。
中药里的五腥包括:鱼腥、葱蒜、牛羊(牛奶)、韭菜、笋、海鲜等。
安全级食品:西红柿、黄瓜、白菜、油菜、小白菜、冬瓜、苦瓜、绿豆、薏米、红枣、香蕉、苹果、橙子、西瓜、草莓、土豆、芸豆、白萝卜、胡萝卜等。
配方可用的食物有:稠米汤,藕粉,杏仁茶,过萝麦片粥;蒸蛋羹,蛋花汤,肉汤冲鸡蛋;各种牛奶及奶制品----奶酪,杏仁豆腐、酸奶、冰淇淋、可可牛奶,牛奶冲藕粉;豆浆、过萝豆汤;菜水,过萝菜汤,西红柿汁;鲜果汁,煮果子水,果茶,果冻;清鸡汤,清肉汤,肝汤等。蔬菜、水果等富含维生素、碳水化合物、微量元素、矿物质、植物蛋白的食物,都属于轻食食物;巧克力、糖果、膨化食品、罐头水果等高热量食物则不属于。根据轻食原则,食物在加工过摄入分量要少。
流质膳食有好多种,如:米汤、蛋花汤、牛奶、菜汁、果汁、豆浆、豆腐脑、肉汤、骨头汤,一般含渣很少,呈液体状态的都可以。
半流食:半流质饮食是介于软食与流质饮食之间,外观呈半流体状态,是限量、多餐次进食形式;也称为半流或半流质。
1、食物基本特点。半流质饮食是一种食物比较稀、软、烂,易消化、易咀嚼、含粗纤维少、无强烈刺激呈半流质状态的食物。
2、适用范围。半流食适用于发烧、咀嚼吞咽困难及急性消化道炎症,手术前后以及病情危重的病人。
3、饮食原则
(1)能量需要量:全天总能量为控制在7000千焦耳(1670千卡)左右,能量过高体质虚弱的患者不易接受;蛋白质应按正常量供给;各种营养素补充要合理、充分。
(2)食物要求:食物必须呈半流体状态,食物要稀、软、烂,易于消化和吸收、易于咀嚼和吞咽、含粗纤维要少、无强烈刺激性。每日的食物品种要多样化,以增进病人食欲。
(3)餐次安排:少食多餐,病人可每隔2~3h进一次餐,每天进餐5~6次。全天主食最好不超过300g。
(4)主食选择:可食大米粥、小米粥、面条、面片、馄饨、面包、小包子、小花卷、蛋糕、苏打饼干、藕粉等。
(5)适宜的半流质食物有:肉末粥、碎菜粥、蛋花粥、面条汤、面片汤、馄饨、面包;蒸蛋羹、蛋花汤、卧鸡蛋;牛奶、酸奶;嫩豆腐、豆腐脑;果汁、果泥、果冻;西瓜、熟香蕉;菜泥、菜汁、嫩碎菜叶;各种肉汤、嫩肉丝、肉末、鱼片等。
(6)禁用食物:豆类、毛豆、大块蔬菜、大量肉类、蒸饺、油炸食品如熏鱼、炸丸子等均不宜食用;蒸米饭、烙饼等均较硬且不易消化吸收应少食或不食;刺激性调味品等亦不宜食。
胃管要求:1.妥善固定,防止打折,避免脱出。2.保证胃管的通畅,定时冲洗、抽吸胃液。3.密切观察胃液的颜色、性质、量,并做好记录。4.胃管护理。
护理要求:A.每日用棉棒沾水清洁鼻腔。B.更换胶带时,须将脸部皮肤拭净再贴,并注意勿贴于同一皮肤部位。C.鼻胃管外露部位须妥当安置,以免牵扯滑脱。D.每日注意鼻胃管刻度,若有脱出,应通知医务人员处理。E.每日清洁口腔,以棉签清洁口腔;意识清楚合作的可以牙刷清洁。鼓励病人刷牙漱口,养成良好的卫生习惯。生活不能自理的病人或昏迷的病人给予口腔护理。F.意识不清或躁动不合作的,需预防鼻胃管被拉出,必要时可将患者双手做适当的约束保护。插胃管以后要鼻饲流质饮食,要记录出入量。用注射器将食物注入。常用鼻饲喂养包括混合奶和匀浆饮食。混合奶可用的食物包括:牛奶、豆浆、熟鸡蛋、浓米汤、肉汤、甘蔗、植物油、食盐等,匀浆饮食的食物有:米饭、米粥、面条、鸡蛋、馒头、鱼、虾、鸡肉、瘦肉、猪肝、蔬菜、油、盐等。食物要冷却到38~40℃,防御前臂内侧,不觉的烫,方可注入。鼻饲病人需要一个过程,开始的时候鼻饲量应少而清淡,以后逐渐增多。昏迷或较长时间未进食者,第一、二天以混合奶为主,每次50~100mL,每4h一次,如无特殊不适,从第三天开始,就可以进食匀浆膳。长期进食匀浆膳的病人,每次注入量包括水在内一般在200~400mL,每日3~4次,加水数次,每日总量在1500~2000mL之间。
喂流食注意事项:
1、每次打流食前要回抽胃液,如胃液过多,要减少进食或停止进食。
2、病人每次打流食前一定确认胃管已经通在胃里。
喂流食注意确认胃管位置的方法如下:
(1)抽取胃液法,这是确定胃管是否在胃内最可靠的方法。
(2)听气过水声法。即将听诊器置病人胃区,快速经胃管向胃内注入10ml的空气,听到气过水声。
(3)三是将胃管末端置于盛水的治疗碗内,无气泡逸出。
Claims (10)
1.一种能形成粪便并能及时促排的营养流食,其特征在于其原料按质量比的组成为藜麦10~20g、山药5~10g、铁皮石斛2~8g、车前子壳2~8g、党参1~5g、当归0.5~5g、甘草0.2~5g、茯苓1~5g、鲍鱼多肽1~4g、鸡内金0.5~3g、莱菔子1~3g、白术1~3g、维生素5~10mg、微量矿物元素5~10mg、电解质平衡元素1~3g、牛磺酸及益生菌粉1~3g、聚葡萄糖1~3g、食品乳化稳定剂和触变剂0.1~1g。
2.如权利要求1所述一种能形成粪便并能及时促排的营养流食,其特征在于所述维生素选自维生素C、维生素K、维生素B、吡哆醇、生物素、叶酸、烟酸和硫胺素中的至少一种。
3.如权利要求1所述一种能形成粪便并能及时促排的营养流食,其特征在于所述微量矿物元素选自葡萄糖酸锌、葡萄糖酸钙、葡萄糖酸亚铁的至少一种。
4.如权利要求1所述一种能形成粪便并能及时促排的营养流食,其特征在于所述电解质平衡元素选自氯化钾、氯化钠中的一种,按质量比1︰1.5平衡添加。
5.如权利要求1所述一种能形成粪便并能及时促排的营养流食,其特征在于所述食品乳化稳定剂和触变剂为黄原胶。
6.如权利要求1~5所述能形成粪便并能及时促排的营养流食的制备方法,其特征在于包括以下步骤:
1)在全部原料中加水,搅拌后得低粘度流食;
2)在低粘度流食中再加水或搅拌,静置后得高粘半流质食物,即能形成粪便并能及时排出的营养流食。
7.如权利要求6所述能形成粪便并能及时促排的营养流食的制备方法,其特征在于在步骤1)中,所述加水的量为全部干粉原料总质量的12~20倍。
8.如权利要求6所述能形成粪便并能及时促排的营养流食的制备方法,其特征在于在步骤1)中,所述搅拌的时间为5~10min。
9.如权利要求6所述能形成粪便并能及时促排的营养流食的制备方法,其特征在于在步骤2)中,所述再加水的量为低粘度流食总质量的8~12倍。
10.如权利要求6所述能形成粪便并能及时促排的营养流食的制备方法,其特征在于在步骤2)中,所述静置的时间大于10min。
