Abstract
本发明公开一种烧烫伤医用复合罗非鱼皮替代敷料,按质量份数计,包括:4‑12份罗非鱼皮冻干粉、0.6‑1.3份蒲公英全草冻干粉、1.5‑3份可溶性丝素蛋白冻干粉,将罗非鱼皮冻干粉、蒲公英全草冻干粉、可溶性丝素蛋白冻干粉混合均匀,得复合罗非鱼皮替代敷料粉;将所述复合罗非鱼皮替代敷料粉和无菌水按质量份数0.6‑1.8:1‑3混合,制备得复合罗非鱼皮替代敷料液,灭菌,凝胶,无菌罐装。本发明还公开了其配套使用工具。本复合替代敷料具有较好的对烧烫伤伤口治疗保护作用,且造价合理。其配套使用工具符合使用要求,可方便地挤出凝胶状敷料并可根据创面需要选择敷料尺寸,调换配套喷头,使用简单方便。
Description
技术领域
本发明涉及医用敷料技术领域,具体涉及一种烧烫伤医用复合罗非鱼皮替代敷料及其配套使用工具。
背景技术
对于浅表烧烫伤,最适合自身皮肤覆盖治疗;而对于深度烧伤病人,因伤口深度和广度,皮肤自身无法修复,需在创面覆敷料干预治疗。
传统敷料主要是干纱布和油纱。
传统的纱布敷料吸液量低、易干燥,需经常更换,使用后易与伤口发生粘连而造成二次创伤,存在换药痛苦、愈合情况不理想等缺点。
目前,人们研发各类更优的现代敷料,但皆有价高、操作难、技术复杂等缺点。为此,有必要设计一种价低、易操作、技术简单的烧烫伤医用新型替代敷料,其可对伤口全面保护,防止二度感染、防止干燥和体液丢失,创造伤口微环境,可一直作用至伤口愈合。
依据中医“因象用形”、“以皮治皮”经典理论,且烫伤为热毒,选取寒性的鱼皮敷料,应是符合需求的。
罗非鱼皮多产、易取、价低,鱼皮胶原对创面治愈发挥作用,其湿且透气,可促进创面肉芽和上皮形成;与创面契合度高,能避免清创损伤痛苦,治疗中少更换、少疼痛,治愈时间短,在治疗烧烫伤用作替代敷料中有其优势。
然而,由于直接使用罗非鱼皮存在取皮、消毒、存放及创面加施其他药品困难的影响因素。因此,有必要设计一种烧烫伤医用复合罗非鱼皮替代敷料及其配套使用工具。
发明内容
本发明的目的是提供一种烧烫伤医用复合罗非鱼皮替代敷料及其配套使用工具,以解决现有技术的不足。
本发明采用以下技术方案:
一种烧烫伤医用复合罗非鱼皮替代敷料,按质量份数计,包括:
罗非鱼皮冻干粉4-12份
蒲公英全草冻干粉0.6-1.3份
可溶性丝素蛋白冻干粉1.5-3份
将罗非鱼皮冻干粉、蒲公英全草冻干粉、可溶性丝素蛋白冻干粉混合均匀,得复合罗非鱼皮替代敷料粉;将所述复合罗非鱼皮替代敷料粉和无菌水按质量份数0.6-1.8:1-3混合,制备得复合罗非鱼皮替代敷料液,灭菌,凝胶,无菌罐装。
进一步地,将复合罗非鱼皮替代敷料液放入器皿,留器皿1/3-1/5体积作为凝胶成型空间,灭菌,常温静置3-8h充分吸水,后器皿置于恒温振荡器中,37℃恒温无菌孵育,外加剪切力,加快凝胶,30-60min凝胶成型,无菌灌装。
进一步地,还可根据需要包括蜂胶冻干粉或/和银离子粉或/和艾草冻干粉。
上述烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,包括管体、圆形推杆,
管体分为前部、过渡部和后部,前部管体外为长方体,内为长方体腔,后部管体内为圆腔,长方体腔截面长边小于圆腔直径,前部管体和后部管体通过过渡部管体连通,前部管体一侧设有若干凸起扣,后部管体后端设有外螺纹;
圆形推杆包括圆形推板、圆形旋钮、后部圆形旋钮、螺杆,圆形推板设在螺杆的前端,圆形推板与螺杆固定一体,圆形推板直径尺寸和后部管体圆腔匹配,圆形旋钮后端挡板中心设有一通孔,通孔设有和螺杆上外螺纹匹配的内螺纹,圆形旋钮通过通孔穿设在螺杆上,圆形旋钮圆周内侧设有和后部管体后端外螺纹匹配的内螺纹,后部圆形旋钮固定设在螺杆后端;
管体内灌装入凝胶状敷料后,前部管体出口封膜,圆形推板插入到后部管体内,旋转圆形旋钮逆时针固定在后部管体后端,未使用时,圆形推板为未推入状态,螺杆外螺纹处于逆时针旋转咬合圆形旋钮通孔内螺纹状态;使用时,拆除封膜并通过顺时针旋转螺杆带动圆形推板在后部管体内向前活动从而将凝胶状敷料挤出。
进一步地,还包括鸭嘴喷头和切割部件,
鸭嘴喷头和前部管体匹配,鸭嘴喷头前端上部设有一凹槽,鸭嘴喷头一侧设有和前部管体凸起扣匹配的凸起扣,使用时鸭嘴喷头套在前部管体上,通过扣板扣入凸起扣固定;
切割部件包括刀片、顶部固定扣、底部固定扣,刀片设在顶部固定扣、底部固定扣之间,顶部固定扣设有和鸭嘴喷头的凹槽匹配的凸块;
使用时将顶部固定扣的凸块放在鸭嘴喷头的凹槽内,凸块带动切割部件在凹槽内移动,达到切割凝胶状敷料的目的。

1、本发明烧烫伤医用复合罗非鱼皮替代敷料,按质量份数计,包括:4-12份罗非鱼皮冻干粉、0.6-1.3份蒲公英全草冻干粉、1.5-3份可溶性丝素蛋白冻干粉,主要用于烧烫伤创面处理,鱼皮+丝素蛋白中胶原成分可促进创面愈合,鱼皮+丝素蛋白为胶浆+凝胶可保护创面、防感染,蒲公英全草可杀菌、消炎、化烫伤热毒。以下从中医药性配伍、西医生物药性分析两个角度说明这三种成分效果:
基于中医配伍角度的分析:
本发明的复合罗非鱼皮替代敷料按照“君臣佐使”配伍,君臣有序。
“君药”:罗非鱼皮对烧烫伤主证起保护创面、止血、促进创面愈合、修复与再生作用,体现本发明复合敷料主攻方向,其药力居本复合敷料中之首,本复合敷料中所占分量重;
“臣药”:丝素蛋白辅助和加强罗非鱼皮对烧烫伤治疗,药力稍小于君药,据古方记载,其兼有亲肤、生肌之功效,甚至对伤口新生肌肤润泽度和白净度有一定功效,本复合敷料中所占分量稍轻;
蒲公英全草为“佐药”,分量更轻,化热毒,杀菌,兼治次要兼证—伤口发炎,消恶肿,助“君药”罗非鱼皮治疗烧烫伤热毒。
丝素蛋白同时因其极易由溶液转化为凝胶的特性,兼具行“使”调和诸药的作用。
基于西医生物药性的分析:
鱼皮胶原蛋白、丝素蛋白无毒安全无副作用,具有良好的生物相容性,其与人体皮肤蛋白结构接近,与人体皮肤有极好的亲和性,含有多种氨基酸;柔韧性、透气透湿性、缓释性能良好,在伤者体内外可被缓慢降解和吸收;能促进受损肌肤吸湿,促进皮肤胶原蛋白形成,同时分解色素,改善新生肌肤肤色。
蒲公英,甘,无毒,杀菌、抑菌作用明显,在本发明复合敷料中充当纯天然广谱抗菌剂,为本发明复合敷料实时杀菌、抑菌,使罗非鱼皮和丝素蛋白能够更长效、高效发挥治疗作用。
本发明复合敷料中罗非鱼皮和丝素蛋白作为基础治疗材料,而蒲公英的杀菌、消炎效果作为复合敷料附加效果。
可溶性丝素蛋白通常情况下,10-15天后变性成凝胶,这特性原本是常规使用中要避免的不良特性,但在本发明中相比较溶液状态,形成凝胶更利于敷料使用,本发明选择利用这一特性,本发明敷料为凝胶状不容易滑动,与创面粘附性强,不易自行脱落。
2、本发明复合敷料属新型材料,原料均为纯天然材料,设定严格参照国家卫生材料的各项灭菌标准生产。
3、本发明复合敷料属新型材料具有消炎、抗菌、无毒、性质稳定;透湿性能好,能有效地吸渗伤口分泌物。
4、本发明复合敷料,有一定弹性及强度,使用时,易操作,受敷料规格尺寸限制小,与创面契合度高,裂缝少有生成,不易剥落。换药时与创面不易产生粘连,治疗过程因其特性减少更换次数,能降低清创损伤痛苦。抗微生物性能良好,改善伤口微循环、促进伤口愈合。
5、本发明复合敷料,选用材料药源广、不良反应少,同时还有易于贮存、使用方便、价低的优点。材料中蛋白与人体皮肤结构蛋白接近,具有极佳的生理功能和理化特性,是传统棉纱敷料的新型敷料替代产品,能促进伤口愈合及缩短伤口的愈合时间。
6、本发明复合敷料,选用产量大的罗非鱼,规避了历史上曾使用陆生动物敷料的安全性和宗教问题,提升罗非鱼皮的合理利用价值和效益。鱼皮与人体表皮近似的气体渗透率,能使创面有一定湿度,可促进创面肉芽增生和上皮形成;有利药物向创面的释放;能有效阻止细菌和微生物的侵入。
7、本发明复合敷料,选用的蚕丝丝素蛋白属天然高分子材料,其生物相容性和生物降解性极佳;与罗非鱼皮配伍制备复合敷料后,能有效地修正湿态丝素蛋白较弱的力学性能;利用丝素蛋白溶液容易变性成为凝胶的特性,与罗非鱼皮又可互相调节促进复合敷料力学强度。
8、本发明复合敷料,选用的蒲公英是我国常见的野生蔬菜和中草药,清热解毒,消肿散结,具有广谱抑菌作用,从中国古代医学专著《唐本草》、《本草纲目》、《中药大辞典》及其生物活性成分上均可见,其疗效优于日常使用的抗生素,且不存在耐药性的隐患,不良反应小、药效持久,调节改善机体免疫力。
9、本发明复合敷料,选用材料均为冻干处理,有助于改变其原始状态下的溶解性能。
10、本复合替代敷料具有较好的对烧烫伤伤口治疗保护作用,且造价合理。其配套使用工具符合使用要求,可方便地挤出凝胶状敷料并可根据创面需要选择敷料尺寸,调换配套喷头,使用简单方便。

下面结合实施例和附图对本发明做更进一步地解释。下列实施例仅用于说明本发明,但并不用来限定本发明的实施范围。
一种烧烫伤医用复合罗非鱼皮替代敷料,按质量份数计,包括:
罗非鱼皮冻干粉4-12份
蒲公英全草冻干粉0.6-1.3份
可溶性丝素蛋白冻干粉1.5-3份。
将罗非鱼皮冻干粉、蒲公英全草冻干粉、可溶性丝素蛋白冻干粉混合均匀,得复合罗非鱼皮替代敷料粉;将所述复合罗非鱼皮替代敷料粉和无菌水按质量份数0.6-1.8:1-3混合,制备得复合罗非鱼皮替代敷料液,将复合罗非鱼皮替代敷料液放入器皿,留器皿1/3-1/5体积作为凝胶成型空间,灭菌,常温静置3-8h充分吸水,后器皿置于恒温振荡器中,37℃恒温无菌孵育,外加剪切力,加快凝胶,30-60min凝胶成型,无菌灌装。
罗非鱼皮冻干粉制备:将罗非鱼皮清洗去鳞取皮,去除附着鱼肉和鳍,切块、消毒;利用已成熟的真空冷冻干燥技术(将湿物料在-10℃--50℃下冻结成固态,然后在真空1.3-13帕下使其中水分不经液态直接升华成气态,最终使物料脱水干燥的技术)冷冻罗非鱼皮、真空升华干燥、研碎、灭菌保存待下步工序。
蒲公英全草冻干粉:取新鲜蒲公英全草清洗,消毒后利用上述真空冷冻干燥技术冷冻,真空升华干燥、研碎、灭菌保存待下步工序。
可溶性丝素蛋白冻干粉可购入成品。
本发明烧烫伤医用复合罗非鱼皮替代敷料中可根据实际情况拓展为利于烧烫伤治疗的其他品种鱼皮,不仅限于罗非鱼皮。
本发明烧烫伤医用复合罗非鱼皮替代敷料中可根据需要制备过程中添加如蜂胶冻干粉等成分,强化抑菌、止痛、止血、止痒、消炎、化淤、分解色素、促进上皮组织细胞修复再生和肉芽形成作用。
本发明烧烫伤医用复合罗非鱼皮替代敷料在含有蒲公英消炎抗菌基础上,可根据需要制备过程中添加如银离子粉抗菌成分,制成强化抗菌复合罗非鱼皮替代敷料,当添加银离子粉时,应充分记录和效果评估,进行辩证选择。
本发明烧烫伤医用复合罗非鱼皮替代敷料也可根据病症需要在制备过程中,添加如艾草冻干粉等止血中药,但应随药性进行辩证选择。
上述烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,如图1-12所示,包括管体1、圆形推杆2、鸭嘴喷头3和切割部件,
管体1分为前部、过渡部和后部,前部管体外为长方体,内为长方体腔,后部管体内为圆腔1.6,长方体腔截面长边小于圆腔1.6直径,前部管体和后部管体通过过渡部管体1.1连通,前部管体一侧设有若干凸起扣1.3,后部管体后端设有外螺纹1.5,后部管体上设有视窗1.2,供观察管体1内敷料量;
圆形推杆2包括圆形推板2.1、圆形旋钮2.2、后部圆形旋钮2.7、螺杆2.6,圆形推板2.1设在螺杆2.6的前端,圆形推板2.1与螺杆2.6固定一体,圆形推板2.1直径尺寸和后部管体圆腔1.6匹配,圆形旋钮2.2后端挡板2.4.1中心设有一通孔,通孔设有和螺杆2.6上外螺纹2.5匹配的内螺纹2.4,圆形旋钮2.2通过通孔穿设在螺杆2.6上,圆形旋钮2.2圆周内侧设有和后部管体后端外螺纹1.5匹配的内螺纹2.3,后部圆形旋钮2.7固定设在螺杆2.6后端;其中,螺杆2.6上可全部或部分设有外螺纹2.5,部分设有外螺纹2.5可更方便推进;

鸭嘴喷头3和前部管体匹配,鸭嘴喷头3前端上部设有一凹槽3.2,鸭嘴喷头3一侧设有和前部管体凸起扣1.3匹配的凸起扣3.1,使用时鸭嘴喷头3套在前部管体上,通过扣板3.4固定,扣板3.4上设有和鸭嘴喷头3凸起扣3.1、前部管体凸起扣1.3匹配的扣孔3.5,扣板需略挤压凸起扣扣入固定;鸭嘴喷头3的喷嘴尺寸可选;
切割部件包括刀片3.6、顶部固定扣3.7、底部固定扣3.8,刀片3.6设在顶部固定扣3.7、底部固定扣3.8之间,顶部固定扣3.7设有和鸭嘴喷头3的凹槽3.2匹配的凸块;
使用时将顶部固定扣3.7的凸块放在鸭嘴喷头3的凹槽3.2内,凸块带动切割部件在凹槽3.2内移动,达到切割凝胶状敷料的目的。
将上述敷料包装入医用灭菌包装袋,灭菌、封口,后可放入药品保存柜中低温(0-5℃)储存;鸭嘴喷头3和切割部件装入另一医用灭菌包装袋,灭菌、封口;医用灭菌包装袋由医用吸塑纸加复合塑膜组成,一面医用吸塑纸,一面复合塑膜。
管体1、圆形推杆2、鸭嘴喷头3、除刀片3.6外的切割部件的材质均为医用级聚丙烯PP;考虑扣板扣入固定和解扣方便,扣板为医用级硅胶。
实施例1
1、建立烧(烫)伤动物实验模型
选取与人体皮肤组织结构相似的豚鼠,建立动物实验模型。
选取生理状况相同且良好的10只成年同龄雄性豚鼠,随机分为2组,编号A、B,记录实验,观察指征。选取低温(0-5℃)储存备用的凝胶状复合罗非鱼皮替代敷料(罗非鱼皮冻干粉8质量份、蒲公英全草冻干粉0.8质量份、可溶性丝素蛋白冻干粉2质量份、无菌水20质量份)及无菌型纱布内加医用黄纱布(医用黄纱布是用消毒后的纱布放入2%~3%乳酸依沙吖啶溶液浸润)各5份。
建立两组实验模型:
A组为复合罗非鱼皮替代敷料烧(烫)伤治疗实验模型
B组为对照组纱布敷料烧(烫)伤治疗实验模型
考虑实验过程温度控制、创面深度、创面面积易控制的要求,同时还需确保实验结果稳定、准确,为能够将动物体表烧伤面积的准确估计,于是选取热金属烫灼法,建立烫伤实验模型。
A、B组选取健康同龄雄性豚鼠各5只作为实验对象。实验步骤如下:利用热金属烫灼法,选取10cm*10cm金属,利用烤箱90度均匀加热10分钟后,取出,贴紧已麻醉的豚鼠一侧身体,移除金属。
2、建立豚鼠损伤伤口愈合模型,清创后,A组采用复合罗非鱼皮替代敷料的治疗,B组采用无菌型纱布内加医用黄纱布治疗。
3、动物实验结果:
经观察,在基本同时、同程度的创面情况下,经定期观察实验对比详细记录内容:
A组缺血度“小于”B组;
对比A、B组治疗后,伤口肉芽生长情况、分泌物吸收情况、创面干洁度、敷料与组织相融度、敷料更换时的完好率、伤口愈合时间等:
A组应用复合敷料治疗后,伤口肉芽生长优于对照组B组;
A组敷料湿润度、收缩度、粘性程度、可自行脱落度“大于”B组;
伤口渗液可渗出,分泌物不会在伤口内积聚,A组较B组创面干洁良好;
A组与创面粘连小,优于B组,更换频率A组小于B组;换药时,对伤口二次损伤小,A组患者痛苦度小于B组;
伤口愈合时间较对照组B组快1-3天;
A组使用含有蒲公英抗菌,能有效地控制创面感染而促进创面愈合,抑菌作用明显。B组使用无菌型纱布内加医用黄纱布敷料,实验初期,消炎抗菌见效快于A组,3-5天后,效果明显低于初期,综合全程消炎抗菌疗效低于A组。
创面愈合后疤痕(含色泽)A组轻于B组,创面排汗分泌功能尚可。
A组敷料力学机械性能低于B组,但可满足使用需求。

罗非鱼皮冻干粉4-12份
蒲公英全草冻干粉0.6-1.3份
可溶性丝素蛋白冻干粉1.5-3份
将罗非鱼皮冻干粉、蒲公英全草冻干粉、可溶性丝素蛋白冻干粉混合均匀,得复合罗非鱼皮替代敷料粉;将所述复合罗非鱼皮替代敷料粉和无菌水按质量份数0.6-1.8:1-3混合,制备得复合罗非鱼皮替代敷料液,灭菌,凝胶,无菌罐装。

鸭嘴喷头和前部管体匹配,鸭嘴喷头前端上部设有一凹槽,鸭嘴喷头一侧设有和前部管体凸起扣匹配的凸起扣,使用时鸭嘴喷头套在前部管体上,通过扣板扣入凸起扣固定;
切割部件包括刀片、顶部固定扣、底部固定扣,刀片设在顶部固定扣、底部固定扣之间,顶部固定扣设有和鸭嘴喷头的凹槽匹配的凸块;
使用时将顶部固定扣的凸块放在鸭嘴喷头的凹槽内,凸块带动切割部件在凹槽内移动,达到切割凝胶状敷料的目的。
6.根据权利要求4所述的烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,其特征在于,后部管体上设有视窗。
7.根据权利要求4所述的烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,其特征在于,螺杆上全部或部分设有外螺纹。
8.根据权利要求5所述的烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,其特征在于,鸭嘴喷头的喷嘴尺寸可选。
9.根据权利要求4或5所述的烧烫伤医用复合罗非鱼皮替代敷料的配套使用工具,其特征在于,管体、圆形推杆、鸭嘴喷头、除刀片外的切割部件的材质均为医用级聚丙烯PP,扣板为医用级硅胶。
