Abstract

背景技术

在一些实施方案中,使用VDA和AAA的组合来诱导肿瘤低氧。在一些实施方案中,VDA诱导对肿瘤血流的立即抑制以诱导低氧和 HABA的激活。在诱导肿瘤低氧的一些实施方案中,AAA将用于抑制肿瘤的代偿性低氧应答,诸如产生VEGF或其他血管生成因子,这些因子动员来自骨髓中的内皮祖细胞以修复受损的肿瘤血管。VDA 与AAA如贝伐珠单抗的组合有助于防止VEGF的代偿性效应并抑制肿瘤血管中的修复过程,从而增强VDA在使肿瘤保持在低氧状态下的效应。

在栓塞程序的一个实例中,栓塞包括在肿瘤或含有由可鉴别的动脉分支(例如,供应肝细胞癌的肝动脉)供应的肿瘤的区域中使用的局部疗法,其通过注射物质(例如碘油(Lipiodol)、明胶海绵 (gelfoam)、血凝块,特别是珠子等)来诱导供应含肿瘤区域的动脉分支发生阻塞,使得肿瘤细胞不能获得足够的血流并死亡。该区域和周围正常器官或组织的血液供应的解剖结构确定了周围的器官/组织是否由于栓塞后缺乏血液供应而可能经历显著的损伤。例如,正常肝脏由双血管——肝动脉和门静脉供应,并因此允许在不会对正常肝脏造成显著损伤的情况下阻塞肝动脉或其分支。该程序通常由介入放射科医师进行,其在荧光镜X射线引导下将导管从股动脉置于腹股沟中,并将导管的尖端推进到供应肿瘤的肝动脉分支。一旦通过注射造影剂鉴别了供应肿瘤的动脉分支,则注射栓塞剂如碘油或明胶海绵以阻塞该分支。
除了碘油之外,其他栓塞剂还包括但不限于明胶海绵、血凝块、纳米颗粒或任何可实现血管阻塞目的的在临床上得到证实的机械作用剂。栓塞剂和低氧激活的生物还原剂(HABA)的施用可以以任何合适的方式进行。例如,HABA可在施用栓塞剂之前(例如,在约1-120 分钟之前)施用,并且随后栓塞剂的施用在该区域中“捕获”HABA。或者,可以将两种药剂共同施用(例如使用包括混合物形式的两种药剂的制剂)。在一些实施方案中,对于通过该方法治疗的患者,所施用的HABA的剂量将在约1mg至约200mg(例如替拉扎明),并且优选约5mg至约80mg的范围内;并且要施用的栓塞剂的剂量将在约5-40ml,并且优选约20-30ml(例如碘油)的范围内。施用足够的栓塞剂以在荧光镜X射线检查下实现血管的预期分支的完全阻塞,从而确保在栓塞区域中产生低氧区域或状况。栓塞剂的施用通常通过动脉内注射进行。或者,可通过其他手段如特定的珠子进行栓塞以诱导阻塞。
在一些实施方案中,HABA与低氧诱导的组合诱导实体瘤的坏死。在一些实施方案中,该组合诱导一种或多种肿瘤中的至少50%、 75%、80%、85%、95%或更多的坏死。诱导坏死的程度可以是在治疗后的特定时间之后达到的程度。例如,至少50%的肿瘤坏死可在治疗后1、2、3、4周或更多周内或在治疗后1、2、3、4、5个月或更多个月内实现。在一些实施方案中,通过治疗诱导的肿瘤坏死的程度是相对于起始肿瘤大小,在治疗后观察到的最大坏死水平。

特别地,治疗癌症的功效可通过任何合适的度量标准加以衡量。在一些实施方案中,治疗功效基于治疗增生性病症如癌症的效果进行衡量。通常,本发明的方法和组合物关于治疗增生性病症(例如癌症,无论是良性的还是恶性的)的治疗功效可通过如下指标来衡量:所述方法和组合物促进抑制肿瘤细胞增殖、抑制肿瘤血管形成、根除肿瘤细胞和/或至少一个肿瘤的大小减小(从而治疗人的增生性病症)的程度。本文讨论了在确定治疗功效时要考虑的几个参数。临床医师可以确定特定情况下参数的适当组合。本发明方法在治疗癌症(例如,减小肿瘤大小或根除癌细胞)中的进展可使用任何合适的方法来确定,诸如临床上目前用于追踪肿瘤大小和癌症进展的那些方法。在一些实施方案中,用于评估癌症治疗的主要功效参数优选为肿瘤大小的减小。肿瘤大小可以使用任何合适的技术来确定,诸如尺寸的测量,或使用可用的计算机软件诸如维克森林大学(Wake Forest University)开发的FreeFlight软件(其能够精确估计肿瘤体积)来估测肿瘤体积。肿瘤大小可以通过使用例如CT、超声波、SPECT、螺旋CT、MRI、照片等等的肿瘤可视化来确定。在结束治疗期后手术切除肿瘤的实施方案中,肿瘤组织的存在和肿瘤大小可以通过待切除组织的总体分析和 /或通过切除组织的病理学分析来确定。
理想地,肿瘤的生长因治疗而稳定下来(即,一个或多个肿瘤的大小增加不超过1%、5%、10%、15%或20%,并且/或者不转移)。在一些实施方案中,肿瘤稳定至少约1、2、3、4、5、6、7、8、9、10、 11、12周或更多周。在一些实施方案中,肿瘤稳定至少约1、2、3、4、 5、6、7、8、9、10、11、12个月或更多个月。在一些实施方案中,肿瘤稳定至少约1、2、3、4、5、6、7、8、9、10年或更多年。优选地,肿瘤的大小降低至少约5%(例如,至少约10%、15%、20%或25%)。更优选地,肿瘤大小降低至少约30%(例如,至少约35%、40%、45%、 50%、55%、60%或65%)。甚至更优选地,肿瘤大小降低至少约70% (例如,至少约75%、80%、85%、90%或95%)。最优选地,肿瘤完全消除或降低到检测水平以下。在一些实施方案中,受试者在治疗后至少约1、2、3、4、5、6、7、8、9、10、11、12周或更多周保持无肿瘤(例如在缓解中)。在一些实施方案中,受试者在治疗后至少约 1、2、3、4、5、6、7、8、9、10、11、12个月或更多个月保持无肿瘤。在一些实施方案中,受试者在治疗后至少约1、2、3、4、5、6、7、8、 9、10年或更多年保持无肿瘤。
当在治疗期结束后手术切除肿瘤时,本发明方法在减小肿瘤大小中的功效可以通过测量切除的坏死(即死亡)组织的百分比来确定。在一些实施方案中,如果切除组织的坏死百分比大于约20%(例如,至少约30%、40%、50%、60%、70%、80%、90%或100%),更优选约90%或更大(例如,约90%、95%或100%),则治疗是有疗效的。最优选地,切除组织的坏死百分比约为100%,也就是说,没有肿瘤组织存在或可检测。
可采用许多次要参数来确定本发明方法的功效。次要参数的实例包括但不限于新肿瘤的检测、肿瘤抗原或标志物(例如,CEA、PSA 或CA-125)的检测、活检、外科手术降期(即,肿瘤的手术分期从不可切除转变为可切除)、PET扫描、生存期、疾病无进展生存期、到疾病进展的时间、生活质量评估如临床效益反应评估等,所有这些均可指示人类癌症的整体进展(或消退)。活检对检测组织内癌细胞的根除特别有用。采用放射免疫检测(RAID)利用由肿瘤产生和/或与之相关的血清标志物(抗原)(“肿瘤标志物”或“肿瘤相关抗原”) 水平对肿瘤进行定位及分期,并且可用作治疗前诊断预测、治疗后复发的诊断指标以及治疗后治疗功效的指标。可被评估为治疗功效指标的肿瘤标志物或肿瘤相关抗原的实例包括但不限于:癌胚抗原(CEA)、前列腺特异性抗原(PSA)、CA-125、CA19-9、神经节苷脂分子(例如,GM2、GD2和GD3)、MART-1、热休克蛋白(例如,gp96),唾液酸化Tn(STn)、酪氨酸酶、MUC-1、HER-2/neu、c-erb-B2、KSA、 PSMA、p53、RAS、EGF-R、VEGF、MAGE和gp100。其他肿瘤相关抗原是本领域已知的。RAID技术联合内窥镜检测系统也能有效地区分小肿瘤和周围组织(参见例如,美国专利号4,932,412)。
在一些实施方案中,通过一个或多个以下结果证明人类患者中癌症的治疗:(a)肿瘤完全消失(即,完全应答),(b)与治疗前的肿瘤大小相比,在治疗期结束后至少四周,肿瘤大小减小约25%至约50%,(c)与治疗期前的肿瘤大小相比,在治疗期结束后至少四周,肿瘤大小减小至少约50%,以及(d)与治疗期前的肿瘤相关抗原水平相比,在治疗期结束后约4-12周时特定肿瘤相关抗原水平降低至少2%(例如,降低约5%、10%、20%、30%、40%、50%、60%、 70%、80%或90%)。虽然优选肿瘤相关抗原水平降低至少2%,但是肿瘤相关抗原水平的任何降低均是治疗患者癌症的证据。例如,对于不可切除的局部晚期胰腺癌,治疗可如下得到证实:与治疗期之前的 CA19-9水平相比,在治疗期结束后4-12周时CA19-9肿瘤相关抗原水平降低至少10%。类似地,对于局部晚期直肠癌,治疗可如下得到证实:与治疗期之前的CEA水平相比,在治疗期结束后4-12周时CEA 肿瘤相关抗原水平降低至少10%。
关于生活质量评估,如临床效益反应标准,根据本发明的治疗的治疗益处可在疼痛强度、止痛剂使用和/或卡氏功能量表(Karnofsky Performance Scale)评分方面进行证明。卡氏功能量表允许对患者根据其功能障碍进行分类。卡氏功能量表评分为0-100。通常,较低的卡氏评分预示生存期的预后不良。因此,备选地或另外地,人类患者中癌症的治疗通过以下方面来证明:(a)与治疗前患者报告的疼痛强度相比,诸如在治疗结束后12周内的任何连续四周时间段,患者报告的疼痛强度降低至少50%(例如,降低至少60%、70%、80%、90%或100%),(b)与治疗前患者报告的止痛剂使用相比,诸如在治疗结束后12周内的任何连续四周时间段,患者报告的止痛剂使用减少至少50%(例如,减少至少60%、70%、80%、90%或100%),和/或(c)与治疗期之前患者报告的卡氏功能量表评分相比,诸如在治疗期结束后12周内的任何连续四周时间段,患者报告的卡氏功能量表评分增加至少20分(例如,增加至少30分、50分、70分或90 分)。
尽管所提到的测试和/或其他测试的备选的或另外的结果可以证明治疗功效,但对人类患者中的增生性病症(例如癌症,无论是良性的还是恶性的)的治疗理想地由一个或多个(以任何组合方式)前述结果来证实。
在一些实施方案中,优选在受试者中没有显著的不良事件的情况下减小肿瘤大小。不良事件根据国立癌症研究所(NCI)的癌症疗法评估计划(CTEP)来分类或“分级”,其中0级代表最小的不良副作用,而4级代表最严重的不良事件。NCI毒性量表(1999年4月发布)和通用毒性标准手册(1999年8月更新)可从NCI获得,例如通过NCI因特网网站www.ctep.info.nih.gov或由NCI癌症治疗和诊断部赞助的研究药剂临床试验的参与者的调查员手册(1998年3月更新) 获得。期望地,本文所述的方法与最小的不良事件相关,例如,如根据CTEP/NCI分级的0级、1级或2级不良事件。然而,尽管优选减小肿瘤大小,但这并不是必需的,原因在于尽管肿瘤细胞得到根除(诸如坏死),但肿瘤的实际大小可能不会缩小。根除癌细胞足以实现治疗效果。同样地,肿瘤大小的任何减小均足以实现治疗效果。
在Cancer Facts and Figures 2001,American Cancer Society,New York,N.Y.和国际专利申请WO 01/24684中进一步描述了人类中各种癌症的检测、监测和评定。因此,临床医师可使用标准测试来确定本发明方法的各种实施方案在治疗癌症中的功效。然而,除了肿瘤的大小和扩散之外,临床医师还可以在评估治疗功效中考虑患者的生活质量和生存期。
在一些实施方案中,施用HABA、诱导低氧和施用免疫检查点抑制剂相比于单独的任一种药剂的治疗、同时递送两种药剂的治疗和 /或以相反顺序的两种药剂的治疗提供改善的治疗功效。可使用任何合适的方法(包括但不限于本文所述的那些方法)来测量改善的功效。在一些实施方案中,使用适当的指标(例如肿瘤大小减小、肿瘤大小稳定性的持续时间、无转移事件的持续时间、无疾病生存的持续时间),改善的治疗功效为至少改善约10%、20%、30%、40%、50%、60%、 70%、75%、80%、90%、95%、100%、110%、120%、150%、200%、 300%、400%、500%、600%、700%、1000%、10000%或更多。使用适当的指标(例如肿瘤大小减小、肿瘤大小稳定性的持续时间、无转移事件的持续时间、无疾病生存的持续时间),改善的功效还可表示为改善倍数,诸如至少约2倍、3倍、4倍、5倍、6倍、7倍、8倍、9 倍、10倍、20倍、30倍、40倍、50倍、60倍、70倍、80倍、90倍、 100倍、1000倍、10000倍或更多倍。
在一个方面,本公开内容提供了用于增强对受试者中的实体瘤的免疫应答的试剂盒,诸如根据本文所述的任何用途。试剂盒可以以任何组合包括本文所述的一种或多种组合物。在一些实施方案中,该试剂盒包含(a)低氧激活的生物还原剂(HABA);(b)低氧诱导剂或栓塞剂;和(c)免疫检查点抑制剂。以上提供了HABA、低氧诱导剂、栓塞剂和免疫检查点抑制剂的非限制性实例,诸如关于本公开内容的各种方法。在一些实施方案中,该试剂盒进一步包含供临床医师、保健提供者或患者使用的说明书,例如印刷材料或包装。试剂盒中的药物组合物和其他物质可以包含在任何合适的容器中,并且可以是立即可用的形式或需要与试剂盒中的其他试剂或使用者提供的试剂来组合(例如稀释浓缩组合物或重建冻干组合物)。

以下实施例是为了说明本发明的各种实施方案的目的而给出的,并不意味着以任何方式限制本发明。本发明实施例以及目前代表优选实施方案的本文所述的方法,均为示例性的,并非旨在限制本发明的范围。本领域技术人员将会想到包含在由权利要求范围限定的本发明的精神内的变化以及其他用途。
实施例1.在肝细胞癌模型中替拉扎明和肝动脉结扎的组合。
材料和方法:将所有的HBx转基因小鼠在无特定病原体的设施中进行饲喂并随访;随后收集个体小鼠的尾巴以供3周龄断奶时的基因分型过程。肝细胞癌(HCC)在>95%的17-18月龄的雄性HBx转基因小鼠中自发产生。具有0.5-2cm直径肿瘤的转基因小鼠用于研究。
用替拉扎明和肝动脉结扎治疗荷瘤的HBx小鼠:对HBx转基因小鼠进行左肝动脉结扎40min,并随后解开丝线结扎。在药物效果的研究中,在肝动脉结扎之前将0.9%盐水、多柔比星(10mg/kg)或 TPZ(3mg/kg)注射到每只小鼠的尾静脉中,持续7min。注射结束后,进行中线剖腹手术以暴露肝左叶和肝门,从而解剖左或总肝动脉以供短暂结扎。在实验期间,通过腹膜内注射400mg/kg阿佛丁 (Sigma-Aldrich,St.Louis,MO,USA)将所有小鼠麻醉。在第一周内收集系列血清样品以供通过使用ARKRAY Spotchem EZ化学分析仪 SP-4430(Arkray,Inc.,Kyoto,Japan)评估ALT和总胆红素。1或7 天后,收集肝组织以供HE染色。
肿瘤坏死的组织学和形态学分析:对于每只小鼠,将含有整个肿瘤和相邻的非肿瘤区的肝组织在10%甲醛中固定过夜并在石蜡中包埋。跨越整个肿瘤的肝组织切片(3μm厚度)经历苏木精和伊红 (H&E)染色以用于组织学分析。通过使用ImageJ软件(Version1.46r, National Institutes of Health,Bethesda,MD,USA),通过将坏死面积除以覆盖整个肿瘤的五个横截面中的总肿瘤面积来对每个肿瘤中的坏死百分比进行定量。以下进一步讨论了详细的结果。
实施例2:在肺癌模型中替拉扎明和考布他汀A4和DMXAA的组合。
材料和方法:人类肺癌NCI-H460细胞购自ATCC并用于本研究。在接种到小鼠中之前将细胞在5次传代内进行亚培养。在动物适应期后,在用3-4%异氟烷麻醉的情况下,将200μL无血清培养基/ 基质胶(50:50v/v)中的大约1.5×106个NCI-H460细胞皮下注射到每只小鼠中。BALB/c裸鼠(雄性,约4~5周龄且体重18~20g)购自中国上海斯莱克实验动物有限公司(Shanghai SLAC Laboratory Animal Co. Ltd.,China),并按照处理组以3-5只小鼠/笼安置在动物饲养室中。小鼠可以自由获取食物(经辐照的,中国上海斯莱克实验动物有限公司) 和水(由Mol Ultrapure Water System过滤的市政自来水)。
在第一次剂量后的第18天,用CO2窒息后颈椎脱位对来自每个组的小鼠施以安乐死,收集肿瘤,称重,并拍摄照片。使用福尔马林固定的肿瘤以供组织病理学研究。福尔马林固定后,将肿瘤进行石蜡包埋以获得FFPE块。对于来自一只小鼠的每个肿瘤大块而制备一块。将FFPE块切成约4μm厚,并进行处理以供H&E染色、组织病理学检查和对肿瘤坏死的定量。以下进一步讨论了详细的结果。
实施例3:诱导肿瘤坏死并将肿瘤在体内转化为癌症疫苗。
为了解决肿瘤异质性和基因组不稳定性的问题,理想的癌症疫苗是每个患者的自体肿瘤。然而,肿瘤本身通常并非极具免疫原性,并且即使在单独的免疫检查点抑制剂的存在下,抗肿瘤免疫的程度通常也是不足的。通过诱导肿瘤坏死可以增强免疫应答,这与强烈的炎性应答相关,其导致将肿瘤相关抗原呈递给T细胞并增加肿瘤特异性 T细胞的群体。诱导肿瘤坏死的一种方法适用于例如肝脏内的癌症,肝脏是具有来自门静脉和肝动脉的双重血液供应的器官,允许在对正常肝脏没有显著损害的情况下对供应肿瘤的肝动脉进行栓塞。诱导肿瘤坏死的策略为替拉扎明(低氧激活的药剂)和经动脉栓塞(TAE) 的组合以产生肿瘤低氧,肿瘤低氧激活替拉扎明以诱导肿瘤坏死。在 HBx转基因小鼠模型中证实了该组合的功效。
用于该研究的动物模型为表达HBx的转基因小鼠,HBx为乙型肝炎病毒的致癌基因,其显示能够诱导肝细胞转化为肿瘤。HBx转基因小鼠在大约18个月时自发地产生肝细胞癌。肿瘤病变没有基线肿瘤坏死,并且理想地用于研究由替拉扎明和肝动脉结扎(HAL)组合诱导的肿瘤坏死程度。首先在滴定研究中,确定了与短暂HAL相结合的HBx转基因小鼠模型中的替拉扎明的有效剂量为3mg/kg。接下来,研究了替拉扎明与HAL联合的功效,并与TACE中常用的化疗剂多柔比星进行比较。首先测试一个小组以比较盐水(n=1)、多柔比星(10mg/kg,n=1)和替拉扎明(3mg/kg,n=1)连同短暂的左 HAL对具有可触知的HCC的HBx转基因小鼠治疗的功效。在用替拉扎明和短暂的左HAL治疗后1天将小鼠处死。替拉扎明治疗的小鼠的ALT水平比多柔比星治疗的小鼠高得多。治疗后第1天的组织病理学检查显示,替拉扎明诱导HAL区域内HCC中超过99%的坏死,相反,多柔比星治疗的HCC中仅约5%的坏死。该结果表明,当与 HAL相结合来诱导肿瘤坏死时,替拉扎明比多柔比星更有效。
进行相同的研究以检查治疗后7天的组织病理学变化。每组使用的小鼠数目为盐水(n=2)、多柔比星(10mg/kg,n=2)和替拉扎明(3mg/kg,n=3)。对于用替拉扎明或多柔比星治疗的HCC,通过 oxyFlo传感器监测肿瘤血流量,并显示HAL使血流量降低至30%,这足以诱发肿瘤低氧。
小鼠体重分析显示无统计学显著变化,但多柔比星治疗的小鼠的平均体重略低(图2中 A)。在整个7天中,三组中的血清总胆红素水平都在正常范围内(图2中 B)。在用替拉扎明和短暂的左HAL治疗的组中,血清ALT水平在第1天达到峰值(图2中 C)。随后,ALT水平降低并在第3天左右恢复正常。多柔比星和短暂的HAL也在第1 天诱导ALT升高,但低于替拉扎明和左HAL,并且到第2天,两组的ALT均恢复至正常水平。在第7天的解剖过程中,因其变得苍白可以证实在用替拉扎明和短暂的HAL治疗的HCC中出现肿瘤坏死,而在来自用盐水或多柔比星处理的小鼠的HCC中则未出现肿瘤坏死 (图2中 D)。通过H&E染色的组织病理学分析证实,在替拉扎明和左 HAL的联合治疗后,左HAL区域中的HCC具有90-99%的坏死。在用盐水或多柔比星联合左HAL处理的HCC中未检测到或几乎未检测到病理学变化(图2中E)。由于这些小鼠往往具有多处HCC,因此在用作内部对照的右叶中存在肿瘤结节。在任何动物的任何右叶中均不会出现肿瘤坏死。在肝脏左叶的无肿瘤部分中未见坏死迹象,表明到第7天,替拉扎明和左HAL的组合在同一叶的正常肝脏中未导致任何显著损伤。尽管如ALT升高所提示的,可能会在第1天出现短暂性损伤,但在第7天完全恢复。
研究结果表明,替拉扎明和TAE的组合在诱导肿瘤坏死中优于单独的TAE或多柔比星与TAE的组合。多柔比星目前用作TACE(经动脉化疗栓塞)的化疗组分以供治疗中期HCC。上述数据表明,为了达到最大的肿瘤坏死以引发针对HCC的免疫力,使用替拉扎明与 TAE联合比用多柔比星的TACE好得多。
然后使用同一模型来检查治疗后的肿瘤坏死程度以及通过该方法诱导的肿瘤坏死是否与明显的炎性应答相关,这有助于用作抗原呈递细胞的巨噬细胞/树突细胞吞噬坏死碎片。在更高放大倍数的视图 (图3)中,通过组装多个组织切片来确定肿瘤坏死的百分比,以评估在用替拉扎明和HAL治疗后肿瘤坏死的总体百分比。观察到整个肿瘤的超过99%坏死。仔细检查坏死肿瘤的周围区域以寻找炎性细胞浸润。图3的插图显示,坏死肿瘤的周围区域具有非常强烈的炎性浸润,这符合坏死触发炎性应答的理论。
实施例4:在适于使用替拉扎明和经动脉栓塞(TAE)的组合进行栓塞的肝细胞癌患
者中诱导肿瘤坏死。
为了检测替拉扎明联合TAE在人类中的临床耐受性和功效,在美国主要医疗中心开始I期剂量定义研究以检查耐受性、初步功效并确定替拉扎明在联合TAE时推荐的2期剂量(RP2D)。入选的HCC 患者为Child-Pugh A级,其不是手术候选者,并且具有最多4个肿瘤病变,其中肿瘤病变大小均不大于10cm并且适合于栓塞。前12名可评估患者的结果可用于分析。
在前两个队列中,通过全身静脉内输注以5和10mg/m2施用替拉扎明,这明显低于先前3期研究中使用的250-330mg/m2的剂量。基于从替拉扎明和左肝动脉结扎的小鼠研究估计的毒性剂量的1/10 的当量和在动物模型研究中所证实的有效结果(实施例1)来选择剂量。三名患者入选任一队列。由于未观察到毒性或剂量限制性毒性 (DLT),故全体均对治疗具有耐受性。随后,基于动脉内(IA)施用的良好耐受特征,将施用途径切换至经由肝动脉的IA,之后在大鼠毒理学研究中进行肝动脉结扎。IA途径的起始临床剂量为5mg/m2,其为显示可耐受的IV剂量的50%。然后将IA队列的剂量上调至10 mg/m2,其中在任一队列中为3名患者。由于未观察到毒性或剂量限制性毒性(DLT),故所有患者对治疗均具有耐受性。
对前12名患者的初步功效进行分析,并在图4中示例说明。基于改良的实体瘤响应评价标准(RECIST),通过对比MRI读取功效结果,从而仅测量存活肿瘤的大小。初步功效分析显示,对于没有对比度增强的病变的12名可评估患者中的6名具有强有力的活性,诸如在(图4)中,其表明所有肿瘤组织已经坏死,即在用替拉扎明与 TAE单一治疗之后实现根据改良的RECIST所定义的完全应答(CR)。另外三名患者具有超过30%的坏死或部分应答(PR)。总应答率 (CR+PR)为83%。与过去对超过10,000名经历经动脉化疗栓塞 (TACE)的患者的荟萃分析所报道的50%CR+PR的历史对照相比,这一结果明显更好。
实施例5.在用替拉扎明和经动脉栓塞治疗的具有其他肿瘤病变的患者中,未治疗
的病变的肿瘤缩小。
在I期临床研究中治疗的12名患者中,几名患者具有多个肿瘤病变并且不能在单一程序中治疗。一名患者具有三个最大直径分别为 15、26、10mm大小的病变。因为26和10mm病变彼此相邻以及考虑血管供应,介入放射科医师选择对26和10mm病变进行治疗。第一个程序时15mm的第三个病变未经治疗。当在第6周进行随访MRI 时,发现两个治疗的病变达到CR;并且第三个病变(在基线处为15 mm)的最大直径缩小到仅9mm。与该发现相一致的一个潜在的解释是,这两个病变的治疗诱导抗肿瘤免疫,其使得未治疗的病变更小。换言之,两个治疗的病变潜在地用作增强患者免疫系统以控制剩余肿瘤病变的癌症疫苗。
实施例6:使用替拉扎明和血管阻断剂在非肝脏组织中诱导肿瘤坏死。
诱导肿瘤坏死的另一种方法是应用于不太适合于栓塞程序的各种实体瘤。本实施例中诱导实体瘤低氧的方法是通过使用血管阻断剂,诸如DMXAA(5,6-二甲基呫吨酮-4-乙酸,也称为ASA404或 Vadimezan)或二苯乙烯衍生物,包括考布他汀A4、考布他汀A4磷酸酯或顺式-3,4',5-三甲氧基-3'-氨基二苯乙烯(二苯乙烯4a)。已证明这些化合物能够关闭肿瘤血流并诱导肿瘤低氧(Chaplin DJ,2006) (Tozer GM,2005)。在该实施例中,在肺癌异种移植模型中检查了替拉扎明和考布他汀A4或DMXAA的组合作为实例来检测其诱导肿瘤坏死的能力。
通过在BALB/c裸鼠中皮下注射NCI-H460人肺癌细胞以形成肿瘤异种移植物来生成肺癌小鼠模型,从而检测替拉扎明和两种血管阻断剂——考布他汀A4或DMXAA的组合。当平均肿瘤体积达到约 480-550mm3(直径1cm左右)并且肿瘤是实体瘤且大小渐增时,每周一次向荷瘤小鼠给予替拉扎明(30mg/kg,腹膜内)和考布他汀 A4(10mg/kg,静脉内)或DMXAA(20mg/kg,静脉内)共两个剂量,以确保该组合能够诱导肿瘤坏死。首先给予替拉扎明,然后3-5min 后注射考布他汀A4或DMXAA。这两种血管阻断剂均导致几乎立即关闭肿瘤血管。在第一次给药后,观察动物的任何异常情况,为期最多三周。
在第3周结束时处死经治疗的小鼠,并收获肿瘤以供通过H&E 染色进行组织学检查。大多数肿瘤表现出约50-70%的肿瘤坏死,而用生理盐水处理的对照肿瘤具有小于20%的坏死。图5中显示代表性的组织病理学图片。关键发现是治疗后大面积的肿瘤坏死,并且坏死肿瘤周围有强烈的炎性浸润。该结果与先前在HCC中的发现一致,即肿瘤坏死与强烈的炎性反应相关并因此能够加强抗肿瘤免疫。
一旦通过由替拉扎明与TAE或血管阻断剂的组合诱导的坏死肿瘤来完成自体肿瘤“疫苗接种”,则长期来看,由于肿瘤微环境的免疫抑制作用,所诱导的T细胞群体可能最终仍然不能有效控制肿瘤。因此,提出使用检查点抑制剂作为维持疗法来消除肿瘤内的免疫抑制作用的组合。
实施例7:在肺癌模型中施用HABA、低氧诱导剂和免疫检查点抑制剂的组合
在研究施用免疫检查点抑制剂联合低氧激活的生物还原剂和低氧诱导剂的效果的研究中,在C57BL/6小鼠的皮下3LL肺同系癌症模型的治疗中施用抗-mPD-1、替拉扎明(TPZ)、考布他汀A4磷酸酯、5,6-二甲基呫吨酮-4-乙酸(DMXAA)及其各种组合。
雄性C57BL/6小鼠购自上海实验动物中心(SLAC,(Shanghai, China,SCXK 2012-0002);年龄:6-8周;体重:18-22g)。将小鼠保持在恒温恒湿的SPF室内,每个笼子中有4只动物。温度为 20.5-24.5℃。湿度保持在40%-75%。光照周期包括12小时的光照和 12小时的黑暗。将小鼠保持在聚碳酸酯笼(325mm×210mm×180mm) 中。笼子垫底材料是玉米芯,每周更换两次。在整个研究期间,小鼠可以自由获取经辐照灭菌的干燥颗粒食物,并且自由获取无菌饮用水。
在37℃的5%CO2的空气气氛中,将3LL肿瘤细胞作为单层培养物在补充有10%热灭活的胎牛血清和L-谷氨酰胺(2mM)的DMEM 培养基中体外维持。每周两次通过胰蛋白酶-EDTA处理将肿瘤细胞常规传代培养。收获在指数生长期生长的3LL肿瘤细胞并对接种进行计数。
C57BL/6小鼠在右胁下接种在0.05mL磷酸盐缓冲盐水(PBS) 中的3LL肿瘤细胞(2×105个细胞)以供肿瘤发育。当平均肿瘤体积达到约455.64mm3时,在接种后9天开始治疗。图6显示了每个研究组中的治疗施用(“N”表示动物编号,“i.v.”表示静脉内注射,“i.p.”表示腹膜内注射)。基于体重调节给药量(0.1mg/10g)。当3LL同系模型的肿瘤大小在试验品治疗后11天时达到大约4000mm3时,将肿瘤手术切除并在10%福尔马林中固定以供H&E染色和用F4/80进行免疫标志物染色。
在修整后尽快将整个肿瘤组织放入10%中性缓冲福尔马林中。通过脱水、清洗、浸润将固定的组织加工成块。该程序可以通过自动组织处理器进行。为了制备福尔马林固定石蜡包埋的样品,遵循以下步骤。将熔融石蜡倒入包埋模具中。将处理过的组织放入该模具中,适用的包埋表面正面朝下(以避免生成气泡)。将包含组织的石蜡模具放在冷冻式载台上。然后使用切片机将石蜡块切成具有5μm厚度的切片。将合适的切片放入45℃蒸馏水浴中。通过载玻片将展开的切片从水浴中拉出,然后将载玻片风干。
对于苏木精和伊红(H&E)染色,将石蜡载玻片放入60℃的烘箱中约2小时,随后在室温下冷却。然后对石蜡载玻片进行脱蜡、H&E 染色、脱水、清洗和封固。使用永久性封固介质施加盖玻片。最后,对载玻片进行显微镜检查并拍摄照片。通过肿瘤坏死面积除以总肿瘤载玻片面积来进行肿瘤坏死百分比(%)的计算。
图7示出了如通过H&E染色所示,用包括抗mPD-1、替拉扎明(TPZ)、考布他汀A4磷酸酯、5,6-二甲基呫吨酮-4-乙酸(DMXAA) 及其各种组合在内的试验品治疗后的肿瘤坏死率。星号(*)表示通过Mann-Whitney检验所确定的与媒介物组相比的统计学显著性 (P<0.05)。与媒介物组相比,TPZ、考布他汀A4磷酸酯和抗mPD-1 的组合显示肿瘤坏死率显著增加。图8提供了图7中描绘的数据。

Claims (4)
1.替拉扎明、考布他汀A4磷酸酯和PD-1免疫检查点抑制剂在制备增强对有需要的受试者中的实体瘤的免疫应答的方法的药物中的应用,所述实体瘤是以下肿瘤:肺癌,所述方法包括:
(a)向所述受试者施用替拉扎明;
(b)向所述受试者施用考布他汀A4磷酸酯;以及
(c)向所述受试者施用所述PD-1免疫检查点抑制剂,其中所述PD-1免疫检查点抑制剂为抗PD-1抗体;
其中所述(a)、(b)和(c)以任何次序顺序或者同时进行。
2.根据权利要求1所述的应用,其中所述抗PD-1抗体选自:纳武单抗和派姆单抗。
3.根据权利要求1所述的应用,其中步骤(b)在步骤(a)之后进行。
4.根据权利要求1所述的应用,其中步骤(c)包括在步骤(b)之后两次或更多次施用所述PD-1免疫检查点抑制剂。
