背景技术
在一些环境下,内窥镜式外科器械相对于传统的开放式外科装置来说是优选的,因为较小的切口可减少术后恢复时间和并发症。因此,一些内窥镜式外科器械可适于通过套管针的插管来将远侧端部执行器放置在期望的外科手术部位。这些远侧端部执行器(例如,内镜切割器、抓紧器、切割器、缝合器、施夹器、进入装置、药物/基因治疗递送装置、以及使用超声波振动、RF、激光等的能量递送装置)可以多种方式接合组织,以实现诊断效果或治疗效果。内窥镜式外科器械可包括位于端部执行器和由临床医生操纵的柄部部分之间的轴。此种轴可使得能够插入到期望的深度并围绕轴的纵向轴线旋转,以由此有利于将端部执行器定位在患者体内。还可通过包括一个或多个关节运动接头或特征部而进一步有利于定位该端部执行器,使得端部执行器能够选择性地进行关节运动或者以其他方式相对于轴的纵向轴线偏转。

B.示例性端部执行器

II.外科缝合器的示例性支撑物组件

本领域的普通技术人员将认识到,在器械10的一些使用期间,操作者可需要在患者体内多次致动端部执行器40。每次致动可要求操作者从患者体内移除端部执行器40,将新的钉仓70重新加载到下钳口50中,将新的支撑物组件100,110施用到砧座60和钉仓70,然后将重新加载的端部执行器40插入患者体内。每次从患者体内移除端部执行器40时,砧座60可大体上被来自患者的体液和/或外科领域中的其他液体润湿。甚至当新钉仓70被安装在下钳口50中时,新钉仓70也可接收来自已被润湿的端部执行器40的其他部分的液体。在砧座60的下侧65和/或钉仓70的平台73上存在的液体可使得难以将支撑物组件100,110粘附到砧座60和钉仓70。下述示例涉及,当粘附到砧座60和钉仓70的支撑物组件100,110被一种或多种液体润湿时,可用于促进支撑物组件100,110与砧座60和钉仓70之间的适当粘附性的各种组合物和构型。
A.使用耐湿粘合剂材料将支撑物材料粘附到润湿的端部执行器

聚合物的前述物理机械特性中的一个为玻璃化转变温度(Tg)。玻璃化转变温度为共聚物的机械特性从流动性粘合剂显著地变为脆性塑料时的温度。因此,可为重要的是,玻璃化转变温度(Tg)充分低于耐湿粘合剂材料的操作温度,以便允许足够的聚合物链的运动性。聚合物的熔融温度(Tm)可指聚合物从固态转变成液态的“一级相变”。结晶聚合物具有真实熔点,其为结晶体熔融并且塑料的整体变为液体时的温度。非结晶聚合物不具有真实熔点,但它们的确具有一级相变,其中它们的机械特性从橡胶样性质转变为粘性橡胶样流动性。用于耐湿粘合剂材料的合适的聚合物可为半结晶的,即它们可具有非结晶片段和结晶片段两者。合适的聚合物可具有充分高于耐湿粘合剂材料的操作温度的熔点,以保持内聚强度并提供所施用的耐湿粘合剂材料的尺寸稳定性。
不可生物吸收的聚合物的分子量应当足够高以为所得粘合剂材料提供机械强度,以便避免内聚破坏,然而聚合物的分子量应当足够低,使得它们可被患者身体清除。就可生物降解的聚合物而言,可不需要分子量的上限来提供足够小以由患者身体清除的聚合物分解产物。
在外科手术期间可遇到的含水环境中聚合物的溶解度或溶解速率取决于多种聚合物特性,所述特性包括但不限于:聚合物组合物;聚合物体系结构;交联程度;嵌段长度;结晶度;分子量;支链;以及它们的组合。在下文所述的示例性示例中,某些聚合物和共聚物被选定并且与所考虑的这些特性结合,以便降低所得的耐湿粘合剂材料的溶解速率,所述耐湿粘合剂材料在外科手术期间用于将支撑物主体102,112粘附到润湿的端部执行器40,并且保持将支撑物主体粘附到润湿的端部执行器40,直到支撑物主体102,112从端部执行器40部署或者释放的时刻为止(参见例如,图5C)。
存在于耐湿粘合剂材料中的聚合物的表面张力和流变性也可影响其粘附特性。例如,如果聚合物的表面张力与其将粘附的表面之间存在足够大的失配,则两者之间的粘附性可为非常不有利的。相似地,聚合物的流变特性(例如体积模量)可使得耐湿粘合剂材料能够流动以适形于端部执行器40的表面形貌,同时提供足够的完整性以保持粘合强度并且抵抗来自端部执行器40的支撑物主体102,112的剪切和剥离。

2.包含聚乙二醇或者聚乙烯-聚乙二醇共聚物的示例性耐湿性粘合剂材料

4.包含水性胶体凝胶的示例性耐湿性粘合剂材料

图8示出了示例性支撑物主体802的透视图,其中耐湿粘合剂材料已经以离散的半刚性点804的图案施用。离散的半刚性点804的尺寸和位置被设定成对应于砧座60的钉成形凹坑64的定位。因此,粘合剂材料804的离散的半刚性点可被布置在四个纵向延伸的线性阵列中,其中每个纵向延伸的线性阵列对应于砧座60的钉成形凹坑64的纵向阵列式排列。另选地,可使用任何其他合适的布置。一般来讲,下述情况为理论化的,但绝不会限制本发明的范围:添加刚性点804增强了耐湿粘合剂材料的模量,同时对其内聚强度具有较小的影响。
图9示出了支撑物主体902如何提供限定多个泡孔908的栅格。由于所存在的电池908和支撑物主体902的多孔性质,当将耐湿粘合剂材料904施用到支撑物主体902时,耐湿粘合剂材料通过进入这些泡孔908中的一些内来形成栅格图案,从而利用粘合剂材料部分地灌注支撑物主体902。换句话讲,支撑物主体902充当吸收粘合剂材料的海绵,由此允许粘合剂材料在支撑物主体902内变形、包绕并且基本上附着到栅格连接部。
在一些情况下,在粘合剂材料处于相对较高粘性形式时,将耐湿粘合剂材料初始地施用到支撑物主体902。支撑物主体902随后被加热以增加粘合剂材料的粘度,由此导致粘合剂材料进入支撑物主体902的泡孔908中的一些。支撑物主体902随后被冷却或者允许冷却,由此导致粘合剂材料的粘度增加返回到其先前状态。支撑物主体902随后可在支撑物主体902施用到端部执行器40时被再次加热,如上文所述。在一些其他形式中,粘合剂材料已具有足够低的粘度以在粘合剂材料被施用时进入泡孔908,而无需粘合剂材料被加热。换句话讲,粘合剂材料可芯吸到支撑物主体902的泡孔908内。在一些此类形式中,保护膜(例如,聚四氟乙烯(PTFE))可在将支撑物主体902施用到端部执行器40之前被施用到粘合剂材料上,以保护和/或容纳粘合剂材料。可形成和提供支撑物组件902的其他合适的方法被公开于下述美国专利申请中:2015年3月25日提交的题目为“Method of Applying aButtress to a Surgical Stapler”的美国专利申请第14/667,842号,该申请的公开内容以引用方式并入本文。
C.降低支撑物与耐湿粘合剂材料的自发分离
在一些外科应用中,可希望降低支撑物主体102,112与已施用到其上的耐湿粘合剂材料的自发分离。此类自发分离或“脱湿”可因水分存在于已粘附到支撑物主体102,112的端部执行器40上而发生。在一些情况下,可通过选自由以下项组成的组的一个或多个步骤使脱湿最小化:干燥;灌注;吸收水;以及它们的组合。这些步骤中的每一者在下文中进行更详细的讨论。
在一些情况下,可通过干燥端部执行器40在将支撑物主体102,112粘附到其上之前使脱湿最小化。在示例性实施方案中,可通过将吸收剂施用到端部执行器40来实现干燥端部执行器40。例如,端部执行器40可暂时夹紧到吸收平台(例如,包括聚丙烯酸酯衬垫)上,以便大体上干燥砧座60的下侧65和钉仓70的平台73,如下述美国专利申请所述:2015年8月24日提交的题目为“Method and Apparatus for Applying a Buttress to EndEffector of a Surgical Stapler”的美国临时专利申请第62/209,041号,其公开内容以引用方式并入本文。根据本文的教导内容,端部执行器40可在将支撑物组件102,112粘附到其上之前被干燥的其他合适的方式对本领域的普通技术人员将是显而易见的。
在一些情况下,可通过在将支撑物主体102,112粘附到其上之前利用疏水层灌注端部执行器40来另外或另选地使脱湿最小化。在示例性实施方案中,可将具有可混溶粘合剂的疏水物的海绵夹紧到端部执行器40上,以在将支撑物主体102,112粘附到端部执行器之前使其暂时疏水。在示例性实施方案中,可混溶粘合剂的疏水物可选自由以下项组成的组:柠檬酸乙酯;甘油三乙酸酯;甘油三油酸酯;以及它们的组合。
在一些型式中,可如下施用可混溶粘合剂的疏水物。可将一种或多种可混溶粘合剂的疏水物预先加载到开孔泡沫层中。在支撑物主体102,112已粘附到端部执行器40的砧座60和下钳口50上之后,可将开孔泡沫层加载和挤压在支撑物主体102,112之间。在挤压开孔泡沫层期间,可混溶粘合剂的疏水物中的至少部分可从开孔泡沫,穿过支撑物主体102,112迁移至耐湿粘合剂材料和端部执行器40的砧座60或钉仓70之间的界面,从而产生可有利于保持良好粘附特性的暂时疏水性环境。
在一些情况下,可通过利用包含硬脂酸钙或硬脂酸镁的疏水性润滑涂层来涂覆端部执行器40中的全部或部分,例如砧座60和/或钉仓70,来另外或另选地使脱湿最小化。
在一些情况下,可通过吸收远离端部执行器40的表面的水分来另外或另选地使脱湿最小化。在一些此类型式中,这可通过在制造粘合剂时将水性胶体混合到耐湿粘合剂材料中来实现。一般来讲,下述情况为理论化的,但绝不限制本发明的范围:水性胶体提供允许粘合剂材料能够粘着到湿表面和干表面两者上的具有湿粘特性的所得耐湿粘合剂材料。可形成水性胶体的合适组合物可选自由以下项组成的组:羧甲基纤维素(CMC);明胶;透明质酸酯;以及它们的组合。
在一些情况下,可通过将亲水性嵌段添加至形成耐湿粘合剂材料的聚合物或共聚物中的一种,来远离端部执行器40的表面另外地或另选地吸收水分。合适的亲水性嵌段可选自由以下项组成的组:聚乙二醇(PEG);聚乙烯吡咯烷酮(PVP);以及它们的组合。
D.减少支撑物主体抵靠组织力
如上文参见图6所述,一系列钉90可抵靠组织T1,T2的层捕获和保持支撑物组件100,110,从而将支撑物组件100,110固定到组织T1,T2。在一些外科手术中,可希望减少抵靠支撑物组件100,110施加的力(当抵靠组织层T1,T2放置支撑物组件或者支撑物组件处于适当位置时)。这种减小的力可允许支撑物主体102,112更好地粘附到端部执行器40。此外或另选地,可希望减小可损坏待固定或已固定的组织T1,T2的层的力。在示例性示例中,这可通过处理一个或多个支撑物组件100,110的组织接触表面来实现,以使得它们变得光滑。一般来讲,下述情况为理论化的,但绝不限制本发明的范围:润滑的支撑物主体102,112表面可减小施用于组织T1,T2的层和支撑物组件100,110之间的剪切力,即拖拽力。可施用于一个或多个支撑物组件100,110的组织接触表面的示例性物质可选自由以下项组成的组:聚乙二醇200(PEG 200);有机硅;油;以及它们的组合。
在另外的示例性示例中,可通过修改支撑物主体组件100,110的边缘来另外或另选地减小力。一般来讲,下述情况为理论化的,但绝不会限制本发明的范围:支撑物组件100,110的边缘的此类修改可由支撑物组件100,110在放置支撑物组件或者将其放置在适当位置时抵靠组织T1,T2的层在组织T1,T2的层上的勾持和/或夹持最小化,反之亦然。修改支撑物主体组件100,110的边缘的有用方法可选自由以下项组成的组:圆角化;去角斜切;以及它们的组合。

仅以举例的方式,本文描述的型式可在手术之前和/或之后消毒。在一种消毒技术中,将该装置被放置在合适的闭合且密封的容器中。然后可将容器和装置放置在可穿透容器的辐射场中,诸如γ辐射、x射线、或高能电子。辐射可杀死装置上和容器中的细菌。经消毒的装置随后可存储在无菌容器中,以供以后使用。还可使用本领域已知的任何其他技术对装置进行消毒,所述技术包括但不限于β辐射或γ辐射、环氧乙烷或蒸汽。
已经示出和阐述了本发明的各种实施方案,可在不脱离本发明的范围的情况下由本领域的普通技术人员进行适当修改来实现本文所述的方法和系统的进一步改进。已经提及了若干此类可能的修改,并且其他修改对于本领域的技术人员而言将显而易见。例如,上文所讨论的实施例、实施方案、几何形状、材料、尺寸、比率、步骤等均是例示性的而非必需的。因此,本发明的范围应根据以下权利要求书来考虑,并且应理解为不限于说明书和附图中示出和描述的结构和操作的细节。
Claims (18)
1.一种利用耐湿粘合剂将支撑物施用到润湿的外科缝合器端部执行器的方法,所述方法包括以下步骤:
(a)将支撑物组件定位在所述端部执行器的砧座和钉仓之间,其中所述支撑物组件包括:
(i)支撑物主体,和
(ii)耐湿粘合剂材料,其被施用到所述支撑物主体的至少一侧,
其中所述耐湿粘合剂材料面对所述砧座的下侧或所述钉仓的平台,其中所述耐湿粘合剂材料延伸穿过所述支撑物主体的所述至少一侧的整个表面,
其中在将所述支撑物组件定位在所述砧座与所述钉仓之间的动作期间,所述砧座相对于所述钉仓处于打开位置;
(b)使所述砧座朝向所述钉仓运动;以及
(c)使所述砧座运动返回到所述打开位置,其中在所述砧座运动返回到所述打开位置的情况下,所述支撑物组件经由所述耐湿粘合剂材料粘附到所述砧座的下侧或所述钉仓的平台;
其中所述耐湿粘合剂材料包括与一种或多种吸附剂结合的聚合物,以在100%相对湿度的环境中将所述支撑物主体保持到所述砧座的下侧或所述钉仓的平台至少五分钟,并且
其中,利用疏水性润滑涂层涂覆砧座和/或钉仓。
2.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含泊洛沙姆的共混物。
4.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含具有约200至约20,000的分子量的聚乙二醇的混合物。
5.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含共聚物,所述共聚物选自由以下项组成的组:聚乙烯-聚乙二醇共聚物;聚(己内酯)-乙交酯共聚物;以及它们的组合。
6.根据权利要求5所述的方法,其中所述耐湿粘合剂材料包含聚乙烯-聚乙二醇共聚物和聚(己内酯)-乙交酯共聚物的共混物,所述聚乙烯-聚乙二醇共聚物与聚(己内酯)-乙交酯共聚物的配比为约40:60至约60:40。
7.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含聚乙二醇和聚(己内酯)-乙交酯共聚物的嵌段共聚物。
8.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含甘油三酯。
9.根据权利要求1所述的方法,其中所述耐湿粘合剂材料包含水性胶体凝胶。
10.一种支撑物组件,所述支撑物组件被配置成暂时粘附到润湿的外科缝合器端部执行器,所述支撑物组件包括:
(a)支撑物主体;以及
(b)耐湿粘合剂材料,其中所述耐湿粘合剂材料被施用到所述支撑物主体的至少一侧,其中所述耐湿粘合剂材料包括与一种或多种吸附剂结合的聚合物,以在大约37℃在100%相对湿度的环境中将所述支撑物主体保持到砧座的下侧或钉仓的平台至少五分钟,其中所述耐湿粘合剂材料延伸穿过所述支撑物主体的所述至少一侧的整个表面,
其中,利用疏水性润滑涂层涂覆砧座和/或钉仓。
11.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含泊洛沙姆的共混物。
12.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含具有约200至约20,000的分子量的聚乙二醇的混合物。
13.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含共聚物,所述共聚物选自由以下项组成的组:聚乙烯-聚乙二醇共聚物;聚(己内酯)-乙交酯共聚物;以及它们的组合。
14.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含聚乙烯-聚乙二醇共聚物和聚(己内酯)-乙交酯共聚物的共混物,所述聚乙烯-聚乙二醇共聚物与聚(己内酯)-乙交酯共聚物的配比为约40:60至约60:40。
15.根据权利要求10所述的支撑物组件,所述耐湿粘合剂材料包含聚乙二醇和聚(己内酯)-乙交酯共聚物的嵌段共聚物。
16.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含甘油三酯。
17.根据权利要求10所述的支撑物组件,其中所述耐湿粘合剂材料包含水性胶体凝胶。
18.一种支撑物组件,所述支撑物组件被配置成暂时粘附到润湿的外科缝合器端部执行器,所述支撑物组件包括:
(a)支撑物主体;以及
(b)耐湿粘合剂材料,其中所述耐湿粘合剂材料被施用到所述支撑物主体的至少一侧,其中所述耐湿粘合剂材料包括与一种或多种吸附剂结合的聚合物,以在大约37℃在100%相对湿度的环境中将所述支撑物主体保持到砧座的下侧或钉仓的平台至少五分钟,其中所述耐湿粘合剂材料包含油中固体甘油三酯与一种或多种吸附剂的组合,其中所述耐湿粘合剂材料延伸穿过所述支撑物主体的所述至少一侧的整个表面,
其中,利用疏水性润滑涂层涂覆砧座和/或钉仓。
