Abstract
The invention relates to an impression method for accurately copying gingival morphology, and belongs to the field of dental implant repair. The method directly uses the formed temporary false tooth as the impression column, directly manufactures the artificial gum on the impression column, and takes down the temporary crown after the model is poured and is disinfected and then is worn back into the mouth of a patient. The method is simple, has no complex procedures, avoids the error of repeatedly duplicating the model, accurately transfers the gum form, effectively solves the difficult problems of complex technical procedures of the personalized impression column, easy error generation and difficult operation of a plurality of teeth, and has simple procedures and small error; the problems that the common in-place channel cannot be obtained in the traditional mode, multiple times of division and combined wearing are needed, the operation is complex, and the difficulty is high are solved; the method can obtain accurate impression effect under the condition of saving time and a large amount of material cost, and is simple to operate and easy to popularize.

Background
Implant restoration has become the main restoration mode of tooth loss, because the texture of gum is easy to deform and collapse under the condition of no temporary denture support, and the traditional impression mode (finished product transfer rod) can not copy the shape of gum, so that technicians can not master the gum penetrating form of the implant denture and the design of contact points, and can not achieve good aesthetic physiological effect.
The current method of personalized impression columns for gingival morphology replication is specifically as follows: pouring gypsum substitute, copying a temporary female die for penetrating the gum part by using silicon rubber, stacking and molding a personalized impression column by using resin (or other materials), taking a die in the mouth, and copying a final model in a laboratory. However, this method has the following problems:
(1) the process is complex, and the time cost and the material cost are high;
(2) errors exist in each operation link, the operation links are multiple, the final accumulated errors are large, and the gum penetrating form cannot be accurately copied.
(3) In the case of more than two implants or bridges, the personalized impression column is difficult to position, requires segmental positioning, and leads to more complicated operation and more difficult accurate impression.
The planting of the front tooth aesthetic region is also a difficult point in planting and repairing. How to accurately copy the transgingival shape formed by shaping the aesthetic region on a model to enable a technician to accurately manufacture the transgingival shape of a prosthesis is one of the main problems urgently needed to be solved by the current impression technology.
Disclosure of Invention
In order to solve the technical problems, the invention aims to provide an impression method for accurately copying the gingival form, which has the advantages of simple operation, cost saving, capability of accurately copying the gingival form and improvement of the success rate of planting.
The specific scheme of the invention is as follows:
an impression method for accurately reproducing a gingival shape, comprising: the method comprises the following steps:
step (1), gum shaping to obtain a formed temporary denture;
step (2), under the state of wearing the formed temporary denture in the step (1), using a silica gel secondary impression, and simultaneously ensuring that the crown part of the temporary denture is completely in place in the oral cavity;
step (3), taking a mold to obtain a precise bubble-free silica gel model;
step (4), taking out the temporary denture from the oral cavity, and connecting the columnar substitute body to the formed temporary denture to obtain the impression column;
step (5), inserting the crown of the temporary denture of the impression column in the step (4) into the corresponding position of the silica gel model in the step (3), so that the crown and the silicone rubber impression are tightly attached without gaps and shaking; due to the fact that the operation is conducted outside the reverse port, the problem of difficulty in positioning caused by inconsistency of positioning channels is solved.
Step (6), making an artificial gum around the impression column in the step (5);
step (7), pouring a second model on the artificial gum and the silica gel model, wherein the impression column is covered by the second model;
step (8), the second model, the artificial gum and the impression column are taken down from the silica gel model to obtain a gum form completely consistent with the form in the oral cavity;
and (9) taking down the formed temporary denture to obtain a complete transgingival shape.
Further, as another embodiment, in the step (1), the gum is shaped to obtain a shaped temporary denture with an extended crown.
Further, as another embodiment, in the step (4), the formed temporary denture of the lengthened crown is taken out as the impression cylinder. Under the condition that the dental crown of a certain temporary crown is short and has poor retention, the temporary dental crown can be properly lengthened, and then a silica gel model is taken, so that the temporary crown can perfectly replace a die column.
Further, in the step (2), the complete setting means that the crown part of the temporary denture is tightly combined with the implant without a gap.

Compared with the prior art, the invention has the following beneficial effects:

(2) Under the condition of multi-implant model taking, because the substitute body is reversely worn in and operated outside the mouth, the problems that the traditional model taking can not obtain a common in-place way, needs to be divided for many times, is combined to be worn in, is complex to operate and has high difficulty are solved.
(3) The method can obtain an accurate impression effect under the conditions of saving time and a large amount of material cost, and is simple to operate and easy to popularize.

Example 1
As shown in fig. 1, three anterior teeth of a patient are missing and an aesthetic region of the anterior teeth needs to be implanted, and the impression method for accurately reproducing the gingival contour of the present embodiment includes the following steps:
step (1), gum shaping to obtain a formed temporary denture 5; the molded temporary denture 5 is fitted into the vacant part 3 of the gum 4 to be fitted with the surrounding upper and lower teeth 1 and 2, as shown in fig. 2.
And (2) in the state of wearing the formed temporary denture 5 in the step (1), using a silica gel secondary impression, and simultaneously ensuring that the crown part of the temporary denture 5 is completely positioned in the oral cavity, the crown part is tightly combined with the implant without a gap, as shown in fig. 3, and the silica gel 6 and the mold 7 in fig. 3 are both the prior art.
And (3) taking a model to obtain a precise bubble-free silica gel model which is stable with the shape of the teeth of the patient, as shown in fig. 4, wherein the shape 9 corresponding to the upper teeth and the shape 8 corresponding to the formed temporary false teeth are formed on the silica gel 6.
And (4) taking the temporary denture 5 out of the oral cavity, and connecting the columnar substitute 10 to the formed temporary denture 5 to obtain the impression column, as shown in fig. 5.
And (5) inserting the crown of the temporary denture of the impression column in the step (4) into the corresponding position of the silicone rubber model in the step (3), so that the crown and the silicone rubber impression are tightly attached without gap and shaking, as shown in fig. 6. Due to the fact that the operation is conducted outside the reverse port, the problem of difficulty in positioning caused by inconsistency of positioning channels is solved.
And (6) manufacturing an artificial gum 11 around the impression column in the step (5) according to a conventional process, wherein the artificial gum 11 is covered on a silicon rubber model, covers the crown part and surrounds the columnar substitute body, as shown in fig. 7.
Step (7), a second mold 12 is cast on the artificial gum 11 and the silicone mold 6, as shown in fig. 8, and the second mold 12 covers the impression column. The casting mold is made of a Silag-plus-silicone rubber impression material, manufactured by DMG chemical medicine group of Germany, having a product number of 909036 and a light weight of 909891.
And (8) removing the second model 12, the artificial gum 11 and the impression column from the silicone model 6 to obtain a gum shape completely consistent with the shape in the oral cavity, as shown in fig. 9.
And (9) taking down the formed temporary denture 5 to obtain a complete transgingival shape, as shown in fig. 10. Further, the technician can produce a dental prosthesis having a shape corresponding to the gingival-through part, and perfectly maintain the gingival shape.
After the implantation is carried out by the method of the embodiment, the patient does not feel uncomfortable when using the method, and the re-diagnosis result is good.
Example 2
The impression method for accurately copying the gingival form in the embodiment can be used for properly lengthening the temporary dental crown and then taking the silica gel model to enable the temporary dental crown to perfectly replace an impression column aiming at the condition that the dental crown of the temporary denture is short and poor in retention, and specifically comprises the following steps:
step (1), gum shaping is carried out to obtain a formed temporary denture of the lengthened dental crown; the molded temporary denture is fitted into the vacant part 3 of the gum 4 to be fitted with the surrounding upper and lower teeth 1 and 2.
And (2) under the state that the temporary denture 5 formed in the step (1) is worn, using a silica gel secondary impression, and simultaneously ensuring that the crown of the temporary denture 5 is completely in place in the oral cavity, the crown is tightly combined with the implant without a gap, and the silica gel 6 and the mould 7 are both the prior art.
And (3) taking a model to obtain a precise bubble-free silica gel model which is stable with the shape of the teeth of the patient, wherein the shape 9 corresponding to the upper teeth and the shape 8 corresponding to the formed temporary denture are formed on the silica gel 6.
And (4) taking out the formed temporary denture with the lengthened dental crown as an impression column.
And (5) inserting the crown of the temporary denture of the impression column in the step (4) into the corresponding position of the silica gel model in the step (3), so that the crown and the silicone rubber impression are tightly attached without gaps and shaking.
And (6) manufacturing an artificial gum 11 around the impression column in the step (5) according to a conventional process, covering the artificial gum 11 on the silica gel model, covering the crown part and surrounding the columnar substitute.
And (7) casting a second mold 12 on the artificial gum 11 and the silicone mold 6, wherein the second mold 12 covers the impression column. The casting mold is made of a Silag-plus-silicone rubber impression material, manufactured by DMG chemical medicine group of Germany, having a product number of 909036 and a light weight of 909891.
And (8) taking the second model 12, the artificial gum 11 and the impression column off the silica gel model 6 to obtain a gum shape completely consistent with the shape in the oral cavity.
And (9) taking down the impression column to obtain a complete gum penetrating form. Further, the technician can produce a dental prosthesis having a shape corresponding to the gingival-through part, and perfectly maintain the gingival shape.

step (5), inserting the crown of the temporary denture of the impression column in the step (4) into the corresponding position of the silica gel model in the step (3), so that the crown and the silicone rubber impression are tightly attached without gaps and shaking;
step (6), making an artificial gum around the impression column in the step (5);
step (7), pouring a second model on the artificial gum and the silica gel model, wherein the impression column is covered by the second model;
step (8), the second model, the artificial gum and the impression column are taken down from the silica gel model to obtain a gum form completely consistent with the form in the oral cavity;
and (9) taking down the formed temporary denture to obtain a complete transgingival shape.
