技术领域
本发明一般地涉及用于治疗肠渗透性过高(intestinal hyperpermeability)的组合物、药盒(kit)和方法。
背景技术
肠上皮将腔内容物与间质(interstitium)隔开。该功能主要由上皮的完整性和密封细胞旁空间的紧密连接(tight junction)决定。这些肠紧密连接是选择性渗透的。这种渗透性可响应于腔营养物的存在而生理性提高。渗透性还可以通过黏膜免疫细胞和细胞因子、肠神经系统以及通过病原体而病理性提高。认为肠黏膜防止肠腔的潜在的危险内容物(包括驻留在那里的微生物)进入内部区域和全身循环是至关重要的。肠道和全身的数种临床病症均与受损的肠屏障功能相关。
已经提出肠渗透性过高与疾病之间可能的联系。这已经导致肠渗透性过高(也称为“肠漏综合征”)的诊断急剧增多。与肠渗透性过高相关的疾病包括糖尿病、自闭症(autism)、纤维肌痛、炎性肠病(inflammatory bowel disease,IBD)、移植物抗宿主病(graft versus host disease,GVHD)、HIV/AIDS、多器官功能障碍综合征、肠易激综合征(irritable bowel syndrome,IBS)、乳糜泻、湿疹、银屑病、急性胰腺炎、帕金森病、抑郁、慢性疲劳综合征、哮喘、多发性硬化、关节炎、强直性脊柱炎、非酒精性脂肪性肝病、酒精性肝硬化、环境性肠病和夸希奥科病(kwashiorkor)。认为肠屏障的恢复将改善或治愈基础疾病(underlying disease)。已经提出了可潜在地促进屏障恢复的数种药物靶标,但没有一个被证明是安全且有效的。
因此,对于肠超敏性(hypersensitivity)和多种潜在的疾病,仍然需要开发安全且有效的治疗或治愈。
发明内容
本发明提供了用于在有此需要的对象中治疗肠渗透性过高的方法、组合物和试剂盒,所述肠渗透性过高包括基础病症(包括高血糖症),并且包括基础疾病,例如糖尿病、自闭症、纤维肌痛、炎性肠病(IBD)、移植物抗宿主病(GVHD)、HIV/AIDS、多器官功能障碍综合征、肠易激综合征(IBS)、乳糜泻、湿疹、银屑病、急性胰腺炎、帕金森病、抑郁症、慢性疲劳综合征、哮喘、多发性硬化、关节炎、强直性脊柱炎、非酒精性脂肪性肝病、酒精性肝硬化、环境性肠病或夸希奥科病。在某些实施方案中,本发明提供了包括向有此需要的对象施用有效量的酪氨酸羟化酶抑制剂的方法。在某些实施方案中,本发明提供了包括向有此需要的对象施用有效量的酪氨酸羟化酶抑制剂和p450 3A4促进剂的方法。
在另一些实施方案中,本发明提供了包含酪氨酸羟化酶抑制剂和p450 3A4促进剂的药物组合物。还提供了包含酪氨酸羟化酶抑制剂和p450 3A4促进剂及其包装的药盒。

自闭症谱系患者的一些亚群,包括诊断患有阿斯波哥尔症(Asperger’sDisorder)(即,阿斯波哥尔综合征(Asperger Syndrome))或社交障碍(SocialCommunication Disorder)的患者表现出注意缺陷多动障碍(Attention DeficitHyperactivity Disorder,ADHD)的症状(例如,注意力不集中(inattention)、多动和冲动(impulsivity))和/或抽搐(运动性抽搐(motor tic)或发声性抽搐(vocal tic))。参见例如DSM-5。
可以使用本领域已知的方法进行自闭症症状或本公开内容的任何症状的评估。例如,评估自闭症症状的一种方法是基于多种心理测试中相对于基线变化的临床总体印象(Clinical Global Impressions,CGI)量表。这些测试可包括异常行为检查表-社区版(Aberrant Behavior Checklist-Community,ABC-C)、康纳斯父母评定量表和自闭症诊断观察时间表(Autism Diagnostic Observation Schedule,ADOS)。自闭症症状还可以由临床医生的个人、临床观察、由定期安排的诊所访视采集的视频信息、以及由对象照顾者随时间提供的信息进行评估。本公开内容的组合物和方法导致康纳斯父母评定量表评估得分的至少一个维度(demension)至少降低1分(point)。
上文和整个公开内容中采用的术语“有效量”是指对于一定剂量和一定的时期内对于相关障碍、病症或副作用的治疗有效地获得所期望的结果所必需的量。应理解,本发明的组分的有效量不仅将因为所选择的特定化合物、组分或组合物,施用途径以及所述组分在个体中引起所期望结果的能力在患者之间不同,而且还与例如以下因素相关:待缓解的病症的疾病状态或严重程度,激素水平,个体的年龄、性别、体重,患者所处的状态,以及待治疗的病理状况的严重程度,当时由特定患者采用的同时用药情况或特殊饮食,以及本领域技术人员将认识到的其他因素,其中适当的剂量由主治医师酌情决定。可以调整剂量方案以提供改善的治疗响应。有效量还是其中组分在治疗上的有益作用胜过任何毒性作用或有害作用的量。
“可药用”是指在合理的医学判断范围内的适合与人和动物的组织接触而没有过度毒性、刺激性、变应性应答或其他问题并发症,同时具有相称的合理效益/风险比的那些化合物、材料、组合物和/或剂型。
“高血糖水平”与本文中的“高血糖”互换使用,并且在两个分开的时间下被确定为126mg/dl或更大的空腹血浆血糖水平。

本文中描述的化合物可以以替代形式制备。例如,可将许多含氨基化合物用作或制备为酸加成盐。通常这样的盐改善化合物的分离和处理性质。例如,根据试剂、反应条件等,本文中描述的化合物可用作或制备为例如其盐酸盐或甲苯磺酸盐。同形体晶体形式(isomorphic crystalline form)、所有手性和外消旋形式、N-氧化物、水合物、溶剂合物和酸盐水合物也被考虑在本发明的范围内。

提供了在对象中治疗肠渗透性过高的方法。这样的方法可包括向有此需要的对象施用有效量的酪氨酸羟化酶抑制剂。另一些这样的方法包括向有此需要的对象施用有效量的酪氨酸羟化酶抑制剂和p450 3A4促进剂。该酪氨酸羟化酶抑制剂和p450 3A4促进剂可同时施用。
可通过多种途径施用酪氨酸羟化酶抑制剂或酪氨酸羟化酶抑制剂和p450 3A4促进剂,包括经口、经鼻、皮下、静脉内、肌内、经皮、经阴道、经直肠或其任意组合。可使用例如油酸、1-甲基-2-吡咯烷酮、十二烷基壬氧基乙二醇单醚来实现经皮施用。

可用本发明的方法治疗的代表性病症包括高血糖。所述病症高血糖症的症状可包括:多食(polyphagia)、多饮(polydipisa)、多尿、视力模糊、疲劳(嗜睡)、体重减轻、伤口愈合不良(切口、刮伤等)、口干、皮肤干燥或皮肤瘙痒、脚或脚跟麻刺感(tingling)、勃起功能障碍、复发性感染、外耳感染(游泳性耳病(swimmer’s ear))、心律失常、木僵(stupor)、昏迷和癫痫发作。可用本发明的方法治疗的代表性疾病包括糖尿病、自闭症、纤维肌痛、炎性肠病(IBD)、移植物抗宿主病(GVHD)、HIV/AIDS、多器官功能障碍综合征、肠易激综合征(IBS)、乳糜泻、湿疹、银屑病、急性胰腺炎、帕金森病、抑郁、慢性疲劳综合征、哮喘、多发性硬化、关节炎、强直性脊柱炎、非酒精性脂肪性肝病、酒精性肝硬化、环境性肠病和夸希奥科病。
可通过多种途径施用药物活性分子(例如抑制剂和/或促进剂),包括经口、鼻内、皮下、静脉内、肌内、经皮、经阴道、经直肠或其任意组合。可使用例如油酸、1-甲基-2-吡咯烷酮、十二烷基壬氧基乙二醇单醚来实现经皮施用。
酪氨酸羟化酶抑制剂可以在由5至7天施用酪氨酸羟化酶抑制剂以及1至2天不施用酪氨酸羟化酶抑制剂组成的周期中施用。酪氨酸羟化酶抑制剂可在至少六个所述周期的过程中施用。在本发明的一个合适的实施方案中,每天施用酪氨酸羟化酶抑制剂。在本发明的另一个合适的实施方案中,每天多次施用酪氨酸羟化酶抑制剂。
提供了根据本发明的代表性治疗方法,其包括向有此需要的对象施用有效量的酪氨酸羟化酶抑制剂或酪氨酸羟化酶抑制剂和p450 3A4促进剂。

本公开内容的一些优选方面涉及通过向患者施用有效量的α-甲基-DL-酪氨酸在患者中治疗自闭症的方法。这些方法导致如在DSM-5中定义的至少一种自闭症诊断标志物的降低。这些方法还导致使用异常行为检查表-社区版(ABC-C)、自闭症诊断观察时间表(ADOS)和/或康纳斯父母评定量表(CPRS)评估的至少一个维度的降低。根据这些方面,每天施用约10至2000mg的α-甲基-DL-酪氨酸,优选地,每天施用150至300mg的α-甲基-DL-酪氨酸。α-甲基-DL-酪氨酸优选经口施用。每日剂量可以在整个一天内以单剂量或基本相等的剂量施用。特别优选三个基本相同的每日剂量的α-甲基-DL-酪氨酸。

根据本公开内容,所描述的用于治疗疾病或病症的方法可以与还已知的在治疗相同疾病或病症中有效的治疗方法组合使用。例如,自闭症行为和症状可以用影响自主神经传递的化合物(例如,安非他明、哌甲酯等),精神药物(例如,利培酮)、神经递质再摄取抑制剂(例如,氟西汀)、刺激谷氨酸能传递的化合物(例如,LY2140023)和/或影响胆碱能神经传递的化合物(例如,加兰他敏)进行治疗。因此,本公开内容还涉及通过施用有效量的α-甲基-DL-酪氨酸和有效量的影响自主神经传递的化合物、精神药物、神经递质再摄取抑制剂、刺激谷氨酸能传递的化合物和/或影响胆碱能神经传递的化合物在患者中治疗自闭症的方法。
本文中还提供了包含酪氨酸羟化酶抑制剂和p450 3A4促进剂及其包装的药盒。酪氨酸羟化酶抑制剂可以是酪氨酸衍生物。酪氨酸衍生物可包括能够以异构体形式存在的酪氨酸衍生物。酪氨酸衍生物可包括其L-型或其D-型的酪氨酸衍生物。例如,酪氨酸衍生物还可以以外消旋形式存在。代表性的酪氨酸衍生物包括一种或更多种的(2R)-2-氨基-3-(2-氯-4-羟苯基)丙酸甲酯、D-酪氨酸乙酯盐酸盐、(2R)-2-氨基-3-(2,6-二氯-3,4-二甲氧基苯基)丙酸甲酯、H-D-酪氨酸(tBu)-烯丙酯盐酸盐、(2R)-2-氨基-3-(3-氯-4,5-二甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(2-氯-3-羟基-4-甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(4-[(2-氯-6-氟苯基)甲氧基]苯基)丙酸甲酯、(2R)-2-氨基-3-(2-氯-3,4-二甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(3-氯-5-氟-4-羟基苯基)丙酸甲酯、2-(乙酰氨基)-2-(4-[(2-氯-6-氟苄基)氧基]苄基丙二酸二乙酯、(2R)-2-氨基-3-(3-氯-4-甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(3-氯-4-羟基-5-甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(2,6-二氯-3-羟基-4-甲氧基苯基)丙酸甲酯、(2R)-2-氨基-3-(3-氯-4-羟基苯基)丙酸甲酯、H-DL-酪氨酸甲酯盐酸盐、H-3,5-二碘-酪氨酸-甲酯盐酸盐、HD-3,5-二碘-酪氨酸-甲酯盐酸盐、HD-酪氨酸甲酯盐酸盐、D-酪氨酸甲酯盐酸盐、D-酪氨酸-ome盐酸盐、D-酪氨酸甲酯盐酸盐、H-D-酪氨酸-甲酯·盐酸盐、D-酪氨酸甲酯盐酸盐、H-D-酪氨酸甲酯盐酸盐、(2R)-2-氨基-3-(4-羟基苯基)丙酸、(2R)-2-氨基-3-(4-羟基苯基)甲酯盐酸盐、(2R)-2-氨基-3-(4-羟基苯基)丙酸甲酯盐酸盐、(2R)-2-氮烷基-3-(4-羟基苯基)丙酸甲酯盐酸盐、3-氯-L-酪氨酸、3-硝基-L-酪氨酸、3-硝基-L-酪氨酸乙酯盐酸盐、DL-m-酪氨酸、DL-o-酪氨酸、Boc-酪氨酸(3,5-I2)-OSu、Fmoc-酪氨酸(3-NO2)-OH、α-甲基-L-酪氨酸、α-甲基-D-酪氨酸和α-甲基-DL-酪氨酸。在本发明的某些实施方案中,酪氨酸衍生物是α-甲基-L-酪氨酸。在本发明的另一些具体实施方案中,酪氨酸衍生物是α-甲基-D-酪氨酸。在另一些实施方案中,酪氨酸衍生物是外消旋形式的α-甲基-DL-酪氨酸。
提供以下实施例以补充现有公开内容并提供对本文中所描述主题的更好理解。这些实施例不应被视为限制所描述的主题。应理解,本文中所描述的实施例和实施方案仅用于举例说明目的,并且对于本领域技术人员而言其多种修改或变化将是明显的,并且将包括在本发明的真实范围内并且可在不脱离在本发明的真实范围的情况下做出。
实施例1
最初筛选了两百名患者。符合研究标准的30个对象同意。九(9)名对象在同意研究之前具有高血糖水平(高血糖)。
高血糖水平(高血糖)被确定为在两个分开的时间下,空腹血浆血糖水平为126mg/dl或更高。患者平均年龄为六十二岁,且患者中位年龄为六十岁。6名患者为女性,且3名患者为男性。五名患者为五十至六十岁,且四名患者超过六十岁。
向研究中的患者施用包括以下的治疗方案:酪氨酸羟化酶抑制剂(即,α-甲基-DL酪氨酸)、黑色素促进剂(即,美拉诺坦II(melanotan II))、p450 3A4促进剂(即,5,5-二苯基乙内酰脲)和亮氨酸氨基肽酶抑制剂(即,N-[(2S,3R)-3-氨基-2-羟基-4-苯基丁酰基]-L-亮氨酸)。这些化合物每周五天的每天施用持续六周的时间,在每周周期之间中断一天或两天。每两周监测所有对象的血糖水平。通过每日血糖测试以及随后每两周进行实验室血糖测试来确定血糖水平。
在约2至4周后,所有9名对象具有在两个分开的时间下确定为125mg/dl或更低的空腹血浆血糖水平的正常的血糖水平。
总的来说,对象对于上述治疗耐受性良好,没有与治疗相关的不良事件,并且已经证实对治疗的响应为100%。
实施例2
对患者进行筛选,并评估其符合自闭症谱系障碍的DSM-V标准的程度。向符合标准的亚组每天三次施用包含剂量为50至100mg的酪氨酸羟化酶抑制剂(即,α-甲基-DL酪氨酸)的治疗方案。向另一个亚组施用还包含每日剂量为30mg的p450 3A4促进剂(即,5,5-二苯基乙内酰脲)的治疗方案。任选地,在睡前时间向两个亚组施用15mg剂量的γ-氨基丁酸以减轻抽搐和重复性行为(例如磨牙)。根据需要施用血管升压素以协助大脑治理。在每次施用治疗方案后,再次评估对象满足DSM-5标准的程度的变化。
实施例3:对于在自闭症的治疗中使用AMPT的盲法、随机、安慰剂对照的2期研究
研究设计:盲法,随机的,2组,8周治疗期,随后在接下来的18周内进行另外的随访,总共26周的研究参与。治疗组由单独的L1-79或安慰剂组成。
样品大小:L1-79N=30,安慰剂N=10
研究群体:符合入选标准并且调查者认为高表现性的超过12岁的自闭症患者。
主要入选标准:签署知情同意书、正常临床实验室值、符合自闭症诊断的DSM-5、合格的ADOS得分、完成方案的足够高的功能、不服用精神药物至少2周、ABC-C得分>12。
主要排除标准:脆性X综合征(fragile X syndrome)、癫痫、使用免费赠送的替代药物、有任何合并症(co-morbidity)、其他精神障碍、超出实验室值范围。
实验处理:L1-79是药物Demser的外消旋形式。每日给药×8周,然后在第10、13和26周给药。
非实验治疗:安慰剂。
剂量和施用:将经口施用90mg TID剂量的随机化的L1-79或安慰剂。
评价时间表:患者将接受8周的上述时间表,每周治疗评价,持续1至8周,然后在第10、13和26周进行治疗后随访。
安全措施:定期安排的完整病史和身体检查,生命体征,CBC,差异(differential),血小板计数,尿分析,血清酶,包括:总蛋白、白蛋白、葡萄糖、BUN、肌酐、直接胆红素和总胆红素、碱性磷酸酶、磷、钙、AST、ALT、钠、钾、氯化物、碳酸氢盐、T4、TSH和不良事件评估。独立的DMC将监督该试验的进行以确保患者的安全。
研究持续时间:最多50周(12周的入组,38周的治疗和随访)。
研究终点:主要的终点将是基于多种心理测试中现对于基线变化之临床总体印象(CGI)量表中反映出的主治医师的评估,包括异常行为检查表-社区版(ABC-C)、康纳斯父母评定量表和自闭症诊断观察表(ADOS),以及他们在诊所中的个人观察结果以及定期安排的诊所访视所采集的视频信息(根据该方案)以及由照顾者在该研究的过程中提供的信息。
实施例4
向3名患者施用α-甲基-DL酪氨酸(AMPT)。这组患者符合自闭症的DSM-5定义并进行治疗。参见表1。

使用异常行为检查表-社区版(ABC-C)、自闭症诊断观察时间表(ADOS),康纳斯父母评定量表(CPRS)和总体临床印象量表来评估疾病。每次访视拍摄视频。
在下表中给出了在该观察的前3周中这些患者中两位患者的数据。康纳斯父母评定量表数据在图1中示出。这些临床改善看来很快开始且持久。观察到在数周和数月内持续的改善。除了在两名患者中给药后的第一天有轻度疲劳之外,还没有报道不良事件。

Claims (7)
1.α-甲基-DL-酪氨酸用于制备用于治疗自闭症的药物的用途,其中所述治疗缓解或降低一种或多种症状,其为DSM-5中对于自闭症谱系障碍规定的诊断标准的主题。
2.权利要求1所述的用途,其中所述α-甲基-DL-酪氨酸配制成用于经口施用。
3.权利要求1或2所述的用途,其中150至300mg的所述α-甲基-DL-酪氨酸配制成用于每天施用。
4.权利要求3所述的用途,其中所述α-甲基-DL-酪氨酸配制成用于以三个基本相等的剂量施用。
5.权利要求1或2所述的用途,其中所述药物配制成还用于施用有效量的p450 3A4促进剂。
6.权利要求5所述的用途,其中所述p450 3A4促进剂是5,5-二苯基乙内酰脲、丙戊酸或卡马西平。
7.权利要求1或2所述的用途,其中所述药物配制成用于降低以下至少一种:
-社会-情绪互动方面的缺陷;
-用于社会互动的非言语沟通行为方面的缺陷;
-发展、维持和理解人际关系方面的缺陷,刻板或重复的运动活动、物品使用或言语;
-坚持相同性,死板地坚持例行程序,或者仪式化模式的言语或非言语行为;
-强度或专注程度异常的高度地局限、固定的兴趣;以及
-对感官输入的高反应性或低反应性或者对环境的感官方面的不寻常兴趣。
