更具体地,本发明涉及睡眠障碍和原发性失眠、抑郁症、精神病以及帕金森病和阿尔茨海默病的治疗。
背景技术
应当指出,有两种类型的睡眠:生理睡眠和麻醉睡眠。只有生理睡眠(可以通过其EEG追踪识别),才能恢复身体健康。
生理睡眠通过特征性EEG追踪识别。它包括恢复性的深度慢波睡眠,以及对于记忆来说重要的异相睡眠(也称为快速眼球运动或REM睡眠)。

VLT和6-MH的消除动力学(T1/2z和CL)及其分布动力学(MRT和Vdss)的药代动力学参数值,很可能在人类和所有其他昼行性动物(例如狗)中是相同的。

值得注意的是,在分泌期结束时,约上午6点,由于6-MH的消除(T1/2z=2.27h)比VLT的消除(T1/2z=0.70h)慢得多,因此6-甲氧基哈马兰的浓度迅速变得高于瓦伦托宁的浓度。在上午6点至下午10点之间,6-MH超过VLT的这种盛行导致5-HT2C羟色胺受体的阻断,当警觉性超过具体到每个个体的特定阈值时,发生觉醒。因此,5-HT2C羟色胺受体、α2-肾上腺素能受体和D1和/或D2多巴胺受体的选择性阻断,使得警觉性有可能增加,从而确保觉醒,以增加血压和心率,并增加肌肉紧张。
人工或麻醉睡眠与自然或生理睡眠完全不同。其EEG追踪主要包括对身体没有恢复力的轻度快波睡眠,几乎没有深度睡眠。在患者中观察到了异相睡眠比例的下降,与顺行性遗忘的副作用相关。
人工或麻醉睡眠是,例如,由苯二氮卓类(地西泮等)和相关药物(唑吡坦、佐匹克隆)引起的。所有这些化合物具有以下共同的药理学性质:
●它们是通过变构调节GABAA能受体的激活剂,其激活产生神经抑制作用。其结果是降低警觉性,就像在麻醉状态下一样,以及,

因此,不幸的是,这些化合物具有两个相互矛盾的药理作用:
●通过它们的GABAA能刺激,它们产生差睡眠,即,几乎没有深睡眠且异相睡眠降低;以及,
●它们通过阻断5-HT2C羟色胺受体,通过用作觉醒激素6-MH来拮抗生理睡眠。因此,它们具有抑郁和焦虑作用,并且以这种方式拮抗其GABAA能作用的抗焦虑效果和抗抑郁药的效果。
之前发现(FR 2 898 358),不管季节如何,在夜间睡眠期间(在下午10点(分泌开始)和上午6点(分泌结束)之间),羟色胺经第一次乙酰化(由N-乙酰基转移酶促使)后,由松果体产生的褪黑激素,在其产生的时刻,经历第二次由N-乙酰基转移酶促使的酶促乙酰化,成功得到两种β-咔啉衍生物,即6-甲氧基-1-甲基-3,4二氢-β-咔啉,称为6-甲氧基哈马兰(6-MH),和2-乙酰基-6-甲氧基-1-亚甲基-3,4-二氢-β-咔啉,称为瓦伦托宁(VLT)。
因此,睡眠-觉醒系统与这三种激素有关:褪黑激素(MLT)、瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)。褪黑激素是VLT和6-MH的前体,也是标记物。因此,通过测量患者的MLT血浆浓度可以知道睡眠-觉醒系统的状态。12名年轻受试者和12名年长受试者的松果体分泌的药代动力学研究(Am.J.Physiol.Endocrinol.Metab.,280,E11–E22(2001))显示出MLT的分泌率(比例为1~10,取决于受试者)有高差异性,因此分泌量也有高差异性。这种高差异无疑是在人类中观察到的许多神经系统疾病的原因,如原发性失眠、睡眠障碍、反应性和内源性抑郁症、神经退行性疾病(帕金森病、阿尔茨海默病)和精神病。
抑郁症、精神病状态、帕金森病和阿尔茨海默病的治疗必须包括对相同睡眠-觉醒系统的理解。这些疾病的机理及其新疗法的作用方式的解释,并不是基于生理睡眠机理的知识。实际上,曾认为过量的多巴胺是精神病的原因,然而刚刚发现,过量的6-甲氧基哈马兰(觉醒激素)才是精神病状态的原因。抑郁症,至少反应性抑郁症,不是不治之症,因为它总是通过加强睡眠-觉醒系统来缓解。神经退行性疾病,例如帕金森病和阿尔茨海默病,实际上是由睡眠-觉醒系统的定量缺失造成的。
因此,这些病症无疑将在它们的进展中被阻止和停止。在阿尔茨海默病中,可以通过施用valentonergic和6-甲氧基哈马兰来恢复睡眠和认知。
因此,在一昼夜的24小时期间,[(VLT)-(6-MH)]系统通过调节身体的精神和植物状态起到重要的作用。
该系统的协调功能直接涉及:
●睡眠激素(VLT)和觉醒激素(6-MH)的化学结构,使其能够在夜间休息期间刺激(VLT),或选择性阻断(6-MH)参与器官和生理过程调节的受体,其在休息和活动期间的功能不同;
●瓦伦托宁和6-甲氧基哈马兰的药代动力学特性,使得可以确保在睡眠期间的8小时内(下午10点和上午6点之间)VLT的盛行,以及在觉醒或活动期间(上午6点和下午10点之间)6-MH的盛行;
●松果体分泌两种激素的8小时期间内VLT和6-MH的浓度变化,相当于恒定速率输注到血流中。
发明内容
因此,本申请试图实现基于VLT与6-MH组合的激素替代疗法,其目的是根据每个患者的特征来调节机体的功能,其可以通过测定下午10点至上午6点之间,优选从上午1点起采集的血液样本中血浆褪黑激素浓度的方法来建立。
与大多数内源性化合物一样,由于瓦伦托宁不能口服施用,本发明涉及一种或两种上述激素的透皮治疗系统,优选地,是单室或双室贴剂的形式,以用于治疗由[(VLT)-(6-MH)]系统的定量缺失或过量引起的神经系统疾病。
因此,本发明涉及一种透皮治疗系统,其包含作为有效成分的瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)的组合,其可以是单或双室贴剂或基质贴剂(matrix patch)的形式(以已知的方式)。
优选地,本发明涉及一种粘合性透皮治疗系统,其包含瓦伦托宁(VLT)和6-甲氧基哈马兰的负载的组合,在贴剂的施用期间,优选约8至10个小时,其向血流递送[VLT]/[6–MH]质量比为4的瓦伦托宁和6-甲氧基哈马兰。
具体实施方式
贴剂的实际实施方案将由本领域技术人员基于他对该受试者的基本了解来确定,从而在贴剂的整个施用期间,即约8至10小时的时间内,获得瓦伦托宁和6-甲氧基哈马兰的可控的和延长的系统性施用。
通常,通过利用他对该受试者的基本了解,本领域技术人员能够将这两种活性成分整合到两个独立的贴剂中,并将这两种成分在同一基质中、或在叠加的基质中、或在同一保护膜上相邻组装的基质中混合。当然,瓦伦托宁和6-甲氧基哈马兰也可能存在于其它类型的双独立储库贴剂中。
应当指出,通常有两种主要类型的透皮治疗系统或贴剂,两者均包括:
●不可渗透的外层,
●含有活性成分的隔室,
●用于控制(多种)活性成分释放的方法,
●用于将贴剂保持在皮肤的施用区域上的粘性组分,以及
●一个设计为在贴剂施用之前去除的保护性支撑物。
因此,根据本发明的贴剂可以被设计为储库型或基质型结构。
储库型贴剂将包含在聚合物基质中的一个或两个独立的储库,所述储库包含(多种)活性成分的溶液或悬浮液,所述聚合物基质通过用于调节(多种)活性成分的释放速率的半透聚合物膜与皮肤接触。
基质型贴剂将包含聚合物,(多种)活性成分将以合适的比例溶解或分散在其中。这些活性成分经所述基质的聚合物链的扩散而释放。
根据这种类型的贴剂的具体实施方式,粘合剂覆盖整个基质的释放表面并形成其主要部分。这是本领域技术人员熟知的活性粘合剂型贴剂,其由简化的制造工艺产生,使得可以生产薄且适当柔性的贴剂,以便舒适地施用于患者皮肤。
在本发明的一个优选的实施方案中,将瓦伦托宁和6-甲氧基哈马兰置于分离的储库中,扫除了当它们被混合包装在同一基质中时,出现的任何稳定性问题,或任何其他能够改变活性成分的相对扩散通量的相互作用风险。
在本发明的双室贴剂的一个具体实施方案中,贴剂的每个隔室可以独立地包含含有两种活性成分中的每一种的聚合物基质。为了确保这些活性成分以合适的剂量令人满意地输注到血流中,本领域技术人员将能够容易地设定以下参数:
●贴剂的每个隔室的面积和体积比;
●可能添加亲水性添加剂;
●可能添加吸收促进剂;以及,
●更一般地,用于控制瓦伦托宁和6-甲氧基哈马兰的通量的,本领域技术人员熟知的任何类型的添加剂。
以已知的方式,这种贴剂包括可剥离的保护膜,其被设计成从贴剂被制造时开始到其保存期限结束时,保护待施用到皮肤上的粘合面。以已知的方式,本领域技术人员将利用例如聚酯膜,其一面可用抗粘性硅树脂处理。
含有瓦伦托宁和6-甲氧基哈马兰的粘合性聚合物基质可以是聚合物或共聚物类型,例如聚异丁烯或聚丙烯酸类型、乙烯-乙酸乙烯酯共聚物类型或硅树脂聚合物类型。
自固化丙烯酸树脂,例如Duro-型(汉高公司出售)将以常规方式使用。
如上所述,可以按照已知的方式使用吸收促进剂。该添加剂的实例包括二甘醇、饱和聚乙二醇化甘油酯、1,2-丙二醇和乙醇的单烷基醚。
由于瓦伦托宁和6-甲氧基哈马兰是亲脂性的,因此可能还需要向聚合物基质中加入亲水性添加剂,例如天然或合成聚合物,如瓜尔胶、黄原胶或聚乙烯吡咯烷酮。
根据本发明的具体实施方案,含有活性成分(本发明的主题)的粘合层含有一种或多种烷基(甲基)丙烯酸酯基共聚物,特别是甲基丙烯酸乙酯、甲基丙烯酸正辛酯和甲基丙烯酸月桂酯。可以向该烷基(甲基)丙烯酸酯共聚物中加入其它单体,特别是丙烯腈、乙酸乙烯酯、丙酸乙烯酯或1-乙烯基-2-吡咯烷酮单体。
为了确保贴剂基质中活性成分的稳定性,特别是为了避免在储存期间任何被氧化降解的风险,还可以有利地加入少量的抗氧化剂如α-生育酚和抗坏血酸棕榈酸酯。
在本发明的背景中,申请人能够确定单独施用瓦伦托宁或与6-MH联合施用的主要适应症可以分为两大类:
A)由[(VLT)-(6-MH)]系统的定量缺失(quantitative deficit)导致的病症:
-原发性失眠;
-睡眠障碍;
-内源性和反应性抑郁症;以及,
-神经退行性疾病(帕金森病、阿尔茨海默病等)。
很明显,由于[(VLT)-(6-MH)]系统的缺失导致的病症可以用瓦伦托宁(对于选定的施用途径是可生物利用的)和6-甲氧基哈马兰的组合治疗。
考虑到是包含两种激素的激素替代疗法,针对选定的施用途径,瓦伦托宁和6-MH的药物代谢动力学特性,特别是其生物利用度,必须使其能够尽可能接近地再现24小时内[(VLT)-(6-MH)]系统的两种激素的血浆浓度变化曲线。
对于由两种激素(VLT和6-MH)组成的激素替代疗法,其中之一的瓦伦托宁,由于其在酸性胃液介质中被完全水解而导致其不稳定,因此本发明的背景中所选择的施用途径是透皮途径,原因如下:
1)正如静脉内途径(因为为了重现两种激素在血浆水平的生理变化曲线,需要在8小时的夜间休息期间——下午10点至上午6点之间,静脉输入两种激素的混合物,因此限制过多),透皮途径使得可以以单一透皮贴剂的形式同时施用两种天然激素(瓦伦托宁和6-甲氧基哈马兰)。为了治疗由[(VLT)-(6-MH)]系统的定量缺失引起的病症,因此可以设想,在松果体分泌两种激素期间的8小时内——下午10点至上午6点之间,以适当的比例施用贴剂。
2)此外,使用具有两个独立储库(每种激素一个储库)的贴剂应当使得可以分别控制两种激素的释放,这样就其生物利用度和接触时这两个产品的组合的不稳定性而言,这两种激素不会相互影响。
在应用于原发性失眠等类似病症的情况下,可以观察到由于松果体褪黑激素分泌过低,而导致的瓦伦托宁(VLT)以及并行的6-甲氧基哈马兰(6-MH)的分泌不足。在这种情况下,作为夜间血浆中[(VLT)-(6-MH)]系统的两种激素的标记的褪黑激素的测定,将使得有可能定量地确定该缺失,并因此来调整组合中瓦伦托宁和6-MH的剂量。
对于其它适应症,例如睡眠障碍、抑郁症、神经退行性疾病(帕金森病、阿尔茨海默病),还需要施用瓦伦托宁和6-甲氧基哈马兰的组合。这些病理相当于VLT/6-MH系统的定量缺失。缺失的程度随病症而变化。可以通过在上午6点之前测定处于稳定状态的褪黑激素(作为[(VLT)-(6-MH)]系统的标记)来评估。
应当谨记,在生理条件下,在下午10点至上午6点之间的8个小时内,松果体将[(VLT)-(6-MH)]系统的两种激素和褪黑激素以恒定速率静脉输入(即,具有零级动力学)到血流中。血浆中两种激素的浓度变化取决于其药代动力学性质,特别是用其终末半衰期(T1/2z)表征的消除动力学。因此,对于以针对各受试者可变的方式由松果体分泌的每种激素(VLT和6-MH),血浆浓度的变化取决于:
·分泌速率,其针对每个受试者,从定量的角度,反映了所述受试者的[(VLT)-(6-MH)]系统的水平。这一数据特异于每个受试者。这一独特的数据的值随每个患者所患的神经障碍而变化;以及,
·两种激素的药代动力学特征。人类和所有昼行性动物(例如狗)的消除和分布动力学确实很有可能是相同的,其中瓦伦托宁和6-甲氧基哈马兰的半衰期(T1/2z)分别为0.70小时和2.27小时,分布容量分别为5升/kg和15升/kg。
因此,为了治疗由[(VLT)-(6-MH)]系统的缺失引起的病症,根据病症类型和在患者中观察到的褪黑激素水平,可以预期,在贴剂的两个储库中具有不同剂量的瓦伦托宁和6-甲氧基哈马兰。
B)本发明还涉及由[(VLT)-(6-MH)]系统的定量过量引起的病症的治疗,特别是精神病状态。
在白天上午6点至下午10点之间,由过量的6-甲氧基哈马兰引起的精神病的治疗,可以设想通过在该期间内,用瓦伦托宁置换过量的6-MH。这种治疗尚未被要求保护。这是一种瓦伦托宁应单独施用的适应症。
根据本发明的第一方面,粘合性透皮治疗系统含有作为活性成分的瓦伦托宁(VLT)。
在本发明的背景中,建立了透皮施用地、在两种激素的组合中的瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)的剂量。
最常见的神经障碍(原发性失眠、睡眠障碍、反应性和内源性抑郁症以及神经退行性疾病,如帕金森病和阿尔茨海默病)是由于[(VLT)-(6-MH)]系统的定量缺失引起的,其需要被恢复。
因此,本发明涉及由[(VLT)-(6-MH)]系统中激素的定量缺失引起的病症的治疗。
为了在昼夜循环的24小时内,和谐地确保和维持,与生理技能完全相似的、符合要求的对机体的精神和植物状态的调节,就必须以一种能够再现下午10点(或患者开始睡觉的时间)和上午6点(或患者起床的时间)之间两种激素的松果体分泌的形式,施用所述组合(VLT+6-MH)。这就有必要遵守以下要求:
1.施用生理激素——瓦伦托宁和6-甲氧基哈马兰,而非其合成替代物。实际上,合成替代物具有不同的(化学上不同的)药代动力学性质,使得其不可能在休息期结束时(上午6点或起床时)和活动期结束时(下午10点或睡觉时)的准确时间改变各激素的盛行期。这就迫切需要对于两种天然激素(VLT和6-MH)非常精确的且一致的药代动力学特征。由于在这两个时期,这种浓度盛行必须存在于血浆中,以及两个激素的作用位点(5-HT2C、α2、D1和D2受体等)处,因此更是如此。
2.施用方式必须重现持续8小时(下午10点或睡觉时至上午6点或起床时)的两种激素的恒速输注。由于8小时静脉输注的限制性过大,并且由于VLT在酸性胃液基质中完全降解,因此在本发明的背景中能够提供这种输注的血管外途径是透皮途径,其形式为施用8小时(睡觉时开始并于起床时去除)的双储库贴剂。
3.可以根据上午1点至6点之间,在待治疗患者中测量的血浆褪黑激素水平的值,来计算出针对每种待治疗病症的必须从贴剂释放到体内的VLT和6-MH的生物可利用剂量。
在人类中,在两组涉及男性、女性、年轻和年长的12名健康志愿受试者中进行的由松果体分泌的褪黑激素的动力学研究中(Fourtillan JB,Brisson A.M.,Gobin P.,Fourtillan M.,Ingrand I.,Decourt J.Ph.and Girault J.,Melatonin secretionoccurs at a constant rate in both young and older men andwomen.Am.J.Physiol.Endocrinol.Metab.,280,E11–E22(2001)),有可能测量出夜间(下午10点至上午6点之间)分泌的褪黑激素(MLT)的总量。在24名受试者中观察到了较大的个体间差异(从1到10)。该研究显示,夜间8小时内,对于64kg的平均体重,分泌的MLT总量平均达到28.7μg,或MLT的总分泌量为0.44μg/kg(在平均体重为74kg的健康年轻成年男性中为35.7μg,或0.48μg/kg;在平均体重为54kg的健康年轻成年女性中为21.6μg,或0.40μg/kg)。此外,在这12名年轻受试者中,夜间分泌的MLT的总量从10至60μg不等,对应于从上午1点开始观察到的最大血浆浓度,范围在19至93.7pg/ml之间,平均值为54.5pg/ml。在12名年长受试者中,从上午1点开始观察到的最大血浆浓度在9.6至124.7pg/ml之间,平均值为45.6pg/ml。尽管在年长者中观察到了最高极限值(124.7pg/ml),但平均来说,年长者的MLT最大血浆浓度较低。
这项研究使得有可能确定,在各性别的健康年轻成年受试者中,夜间(不管季节如何,平均持续时间为8h)松果体分泌的总褪黑激素的平均总量为约30μg或0.5μg/kg。应当谨记,该生物参数的个体间差异非常大。事实上,在这12个健康年轻成年志愿受试者(6名男性,6名女性)样本中,该分泌量在10至60μg之间变化,对应于从上午1点开始观察到的最大血浆浓度,范围在19至93.7pg/ml之间。
无法测量到患者夜间松果体MLT的分泌,但是在年轻和年长个体上进行的该项研究显示,在受研究的24名受试者中,从上午1点开始观察到的褪黑激素最大血浆浓度在9.6至124.7pg/ml之间变化。该水平反映了松果体MLT总分泌量。
关于这项研究的重要事项是,在健康的年轻成年受试者中,褪黑激素的总松果体分泌量范围在10至60μg之间,平均为30μg。该分泌量对应于从上午1点开始观察到的最大血浆浓度,范围在19至94pg/ml之间。通过将总松果体分泌量与褪黑激素最大血浆浓度之间的这种相关性外推到该研究的年长受试者中,可以得出以下结论:
·夜间(持续时间8小时,下午10点至上午6点之间)松果体的总褪黑激素分泌量在5μg至100μg之间;
·从上午1点开始观察到的褪黑激素最大血浆浓度反映了这一分泌情况;褪黑激素在10和125pg之间,平均值约50pg/ml;
·从1点开始,对患者褪黑激素血浆浓度的测定,将能够了解其松果体分泌MLT的多少,因为MLT是两种激素——瓦伦托宁和6-甲氧基哈马兰的生物前体,因此进而能够了解这些患者中[(VLT)-(6-MH)]系统的状态。
3.1褪黑激素分泌和由[(VLT)-(6-MH)]系统的缺失导致的神经系统疾病之间的相关性。
尽管由于技术原因,科学文献中缺乏有关似乎相关的各种神经系统疾病中褪黑激素的松果体分泌多少的数据,但是可以建立松果体MLT分泌量与这些疾病之间的相关性。如我们刚刚所示,通过从上午1点开始测定患者的褪黑激素最大血浆浓度,可以评估MLT的松果体分泌量。可以说明以下关系:
·在原发性失眠症和神经退行性疾病,例如帕金森病中观察到,MLT最大血浆浓度低于10pg/ml,相对应的MLT松果体分泌量低于5μg。在阿尔茨海默病的特殊情况下,进行的少量研究显示MLT松果体分泌量暴跌,生物体液中的MLT浓度低于MLT分析技术的检测限;
●在睡眠障碍及内源性和反应性抑郁症中观察到,MLT最大血浆浓度在10pg/ml至50pg/ml之间,相对应的MLT松果体分泌量在5μg至25μg之间。
因此,反映MLT松果体分泌量多少,进而反应了[(VLT)-(6-MH)]系统中两种激素的松果体分泌量的MLT最大血浆浓度值,能够补充神经系统疾病的临床诊断,并作为建立必须透皮施用的组合(VLT+6-MH)中激素剂量的基础。
本发明还涉及一种用于神经系统疾病的临床诊断方法,涉及测定于下午10点至上午6点之间,优选从上午1点开始,采集的血流样品中的褪黑激素(MLT)血浆浓度。
根据本发明的诊断方法,其特征还在于,所述测定通过液相色谱质谱联用(HPLC-MS/MS)进行。该测定方法的示例性实施方式将在下面列出。通过HPLC–MS/MS进行示例性褪黑激素测定。
化学物质
褪黑激素由Cerillant公司合成,购自于Promochem公司(法国,莫尔塞姆),内标——D4-褪黑激素由CDN Isotopes公司合成,购自于C.I.L.公司(法国,圣福瓦拉格兰德)。 HLB固相萃取柱(1ml,30mg)购自于沃特世公司(美国,麻省,米尔福德市)。HPLC分析纯甲醇、HPLC分析纯水、分析纯98-100%甲酸、超梯度级HPLC分析纯乙腈、HPLC分析纯二氯甲烷、2N氢氧化钠溶液和乙酸钠购自于默克(德国,达姆施塔特)。所有溶剂都是高纯度的,无需经进一步纯化即可使用。为了避免任何随后的样品污染,在该分析中使用了3.5ml和10ml的一次性玻璃试管和10ml具有特氟隆塞的一次性玻璃试管。
校准曲线
通过将每种纯的参照化合物溶解在甲醇中使其初始浓度为1000ng/μl,来制备褪黑激素和内标的储备溶液。将这些溶液储存在-20℃备用。通过将储备溶液于甲醇中进行适当稀释,来获得用于分析的工作标准溶液,并储存在20℃以下备用。
检测各对照组血浆以建立内源性褪黑激素浓度。
每天通过加载含有40μl的5pg/μl内标溶液(200pg D4-褪黑激素)和不同量(范围在1至200pg之间)的褪黑激素的无药人血浆等分试样(1ml),制备具有9个点的校准曲线。按照类似的方式,通过加载1ml仅含有内标溶液的对照血浆,来制备空白样品。
从血浆样品中萃取
固相萃取
所有固相萃取(SPE)实验均使用沃特世 HLB柱(1ml,30mg)进行。用1ml甲醇,随后用2ml水平衡柱子。将如上所述用D4-褪黑激素溶液强化的血浆(1ml)置于3.5ml玻璃试管中,并用1ml水稀释。在使用涡旋混合器进行短暂搅拌之后,将稀释后的样品加载到柱子上。用1ml水洗涤柱子后,用1ml甲醇洗脱组分。样品在温和的氮气流下蒸发至干,并溶于1ml的0.002N氢氧化钠水溶液中。
液相萃取
使用涡旋混合器短暂搅拌后,加入6ml二氯甲烷。使用交替搅拌器在10分钟期间进行萃取程序。然后将试管4000rpm离心10分钟。完全除去上部水层,将剩余的有机相转移到10ml玻璃试管中,并在温和的氮气流下于40℃蒸发至干。将干燥的残余物重新溶于200μl水中,并在注射前储存于+4℃。将75μl的固定体积注射到LC-MS/MS系统中。
验证程序
在该验证过程中,同一分析人员在同一天制备和进行了三个血浆校准曲线。计算回归参数以评估该技术的线性和再现性。此外,为了估计在一天内和该过程中的一天到下一天的精密度和准确度,在三个不同的日期,以不同浓度(1、2、25和200pg/ml)进行了重复性试验。由两个不同的操作者在1周的时间内,针对每一浓度,对所加载的血浆样品进行了分析,并计算了相对标准偏差和平均百分比误差。
必须用指示置信水平确定的定量限(LOQ),已被定义为产生显著高于在代表性对照样品中测量的平均信号的信号的最小可检测浓度。首先,为了计算该过程的LOQ,用5个无药物的空白血浆样品进行了重复性试验。在褪黑激素的保留时间观察到的平均信号(Ybl)和相关的标准偏差(Sbl)用于计算定量限的理论值(Yth)。接下来,对血浆样品进行了5次重复试验,其中血浆样品负载了浓度接近该理论定量限的褪黑激素。将信号的平均值(Yloq)与对照样品的平均信号(Ybl)进行统计学比较。在测试了方差的同质性(p<0.05)后,进行Student's t检验或Welch检验显示Yloq显著高于Ybl,概率为97.5%。
为了确定冷冻人血浆中褪黑激素的稳定性,将工作溶液加入到一批空白人血浆中,然后在-20℃的冷冻温度下储存。在两个和三个冻融循环之前和之后,萃取并分析负载有2和200pg/ml褪黑激素的6个血浆样品。与用于样品的正常分析的那些相同,在每个循环之间,样品将保存在储存温度条件(约-20℃)下。
在验证期间将重复分析血浆萃取物,以便检查在溶剂中在约+4℃储存时,化合物可能的降解。稳定性的评估基于反向计算的浓度,其应在±15%以内(LOQ±20%除外),以及可接受的色谱峰形。
为了计算过程的总回收率,将制备并分析血浆校准曲线(从定量限的下限到定量限的上限)。同时,将萃取并分析负载有2、25和200pg/ml褪黑激素的六(6)个样品。在样品的处理结束时,将计算响应的平均值,并与用空白血浆萃取物(n=6)获得的那些进行比较,其中用相同浓度的纯标准品制备和加载空白血浆萃取物。也将以用于加载样品的浓度确定内标的回收率。
LC–MS/MS分析
设备
HPLC系统由配有二元泵和恒温控制自动进样器的Agilent 1100系列系统组成。
LC-MS/MS系统由三重四极串联质谱仪(API 4000,美国应用生物系统公司)以Turbo IonSpray正电离模式运行。通过一段PEEK管将该系统连接到HPLC柱的出口。
色谱条件
色谱在C18柱(美国麻省米尔福德市沃特世公司,XBridgeTM,3.5μm,50×2.1mm内径)上进行。流动相为:乙腈—5mM乙酸铵水溶液—甲酸(25:75:0.01)。流速为0.2ml/min。
质谱条件
质谱仪在500℃下以Turbo IonSpray正电离模式运行。通过使用前体离子[M+H]+向用于分析物和内标的产物离子的转变,通过选择反应监测(SRM)分析样品。对于褪黑激素,在反应监测模式(reaction monitoring mode,RMM)中检测的离子为m/z 233.18(前体离子:[M+H]+)到174.10(产物离子),对于D4-褪黑激素(内标)为m/z 237.22(前体离子:[M+H]+)到178.10(产物离子)。通过损失片段[NH-CO-CH3]获得所形成的产物离子。
电离电压设定在+4.8kV。
TIS雾化器气体(气体1)设定为40。
TIS加热器气体(气体2)设定为50。
气帘设定为12。
碰撞激活解离(CAD)气体设定为10。
DP、EP、CE和CXP值分别设定为50、10、17和10。
每个化合物的延迟时间设置为500ms。
结果与讨论
样品的预处理和分析
对于每日负责大批量待分析样品的分析人员而言,在复杂生物流体中以非常低(毫微微克级)的浓度存在的化合物的定量分析是一个巨大的挑战。首先,血浆样本中一定不能含有可能干扰褪黑激素分析的大多数内源性物质。
我们观察到,固-液萃取程序后,在碱性水相中使用高纯度溶剂(如二氯甲烷)进行液-液萃取程序,得到干净的残留物,其不含大多数杂质。萃取过程节省了相当多的时间,使得实验室每天能够处理超过100个额外的血浆样品。将残留物溶解在非常保护色谱柱的水中,由于色谱执行时间短(4.5分钟),使得可以每天处理大量的注射样。通过使用我们的分析条件,褪黑激素和D4-褪黑激素的保留时间约为2.3分钟。两个色谱峰均为高斯形,每个分析物基线处的峰宽小于30s。半高峰宽约为5s。考虑到系统的总休止时间,每个质量范围500-ms的时间延迟导致了每个化合物测量的最少20个数据点。
回收率
萃取效率约为80%,并随后在HPLC-MS/MS验证程序中证实了该结果。
对生物基质萃取物和加载有200pg/ml褪黑激素的未处理标准品进行重复分析得到结果,基于该结果计算的总回收率:

校准曲线的线性
通过追踪褪黑激素/D4-褪黑激素的峰面积比作为已知褪黑激素浓度的函数,在同一天获得的三个血浆校准曲线在1至200pg/ml的浓度范围内是直线(平均相关系数±SD=0.9973±0.023)。对于n=3测定,回归分析与原点几乎相交,平均y截距值等于0.0035(表1)。在选择性和特异性试验中的第三组和第五组对照血浆样品中,空白样品中观察到的平均信号为8.38和1.47pg/ml的褪黑激素。由于这种高水平的内源性褪黑激素,这些对照血浆样品组不再用于随后的验证程序。基于平均斜率和相关标准偏差(0.0068±0.0001)计算的RSD值(1.96%)显示了该技术在同一天过程中的再现性。通过使用由两个不同分析者在1周内制备和进行的5个校准曲线的回归参数,来评估逐日的线性和再现性。用相同组的对照血浆制备这些校准曲线。如表2所示,斜率的平均值(0.0069±0.0001)在一天内接近“线性测试”,并且沿着回归图的5个校准点的分散极少(表2)。

精密度和准确度
由于使用纯的稳定同位素类似物作为内标,因此所有的变异性来源都显著降低。在三个不同日期计算的各血浆重复性试验的相对标准偏差低于15.2%,平均百分比误差在-9.71%至+4.61%的范围内(表3)。

逐日重复性分析的结果与同日分析的结果非常相似:对于测试的褪黑激素浓度(1至200pg/ml),RSD值低于13.68%,平均百分比误差范围为-6.13至2.86%(表4)。

定量限
许多作者仍然使用信噪比>3这一值作为显著响应的标准。不幸的是,尽管电子噪音日益相对恒定,但对于化学背景噪音来说并不总是如此,其有时一个测定与下一个测定有很大的不同。这主要是由于样品本身、溶剂、萃取物和色谱系统。为了考虑到所有这些变异的来源,我们使用了定量限的统计测定。基于对五个不同对照样品进行HPLC-MS/MS分析后获得的结果,计算了理论LOQ为Yth=0.25pg/ml。
由于空白样品中存在内源褪黑激素,因此以1pg/ml的较高浓度进行了重复性分析。实施Student's t检验表明,基于以1pg/ml进行的重复性分析而计算出的信号的平均值YLOQ±SD(1.069E-02±7.405E-04),与用5个空白样品观察到的(Ybl)(2.663E-03±2.760E-04)有显著差异。对于自由度n=8,tcal=-22.5,标准偏差=0.563E-03。在虚无假设下,结果的概率等于0.00。由于RSD(1.16%)和平均百分比误差(-0.40%)非常小,因此该浓度自然被证实为该过程的定量限(表5)。
对照血浆样品进行HPLC-MS/MS分析后,记录标准质谱图。该样品中内源性褪黑激素的浓度低于0.25pg/ml。获得了加载有1(LOQ)和200pg/ml褪黑激素的对照样品的质谱图。当分析对应于该过程的LOQ的血浆萃取物时,在褪黑激素的保留时间测量的信号相当于注入HPLC-MS/MS系统的约375fg。在对照样品中测量的内源性褪黑激素产生了信号。在临床试验期间,如用5个空白样品进行的重复性分析的结果(RSD=10.36%)所示,很明显,低于0.25pg/ml的褪黑激素浓度可以高精密度和准确度定量。

注射的再现性
还在一个浓度(200pg/ml)下研究了注射步骤的再现性,该测试的结果总结于表6中。当重复注射(n=100)相同的血浆萃取物时,RSD值低于1.0%,因此表明该方法适用于褪黑激素的精确分析。此外,在注射含有200pg/ml褪黑激素的血浆萃取物之后,再进行空白样品的分析时,没有观察到记忆效应。

稳定性试验
冻融循环中的稳定性
该稳定性试验的结果(表7和8)清楚地表明,冻融循环对于储存在约-20℃的人血浆样品中的褪黑激素的稳定性没有影响。

血浆萃取物的稳定性
表9中示出的结果表明,当第一次注射褪黑激素后,将注射小瓶在约+4℃下储存至少24小时时,注射溶剂中的样品萃取物是稳定的。没有观察到色谱追踪的信号强度的降低或修改。

3.2从下午10点(或睡觉时)开始的8个小时中,以便保证在休息期间(下午10点至上午6点之间)机体内VLT浓度的盛行,以及随后从上午6点(或起床时)直到下午10点的整个活动期间的6-MH浓度的盛行,以组合施用的VLT和6-MH的剂量比的计算。
为了保证在下午10点(或睡觉时)至上午6点(或起床时)之间的休息期间内,体内VLT浓度的盛行,以及随后在起床和睡觉之间的整个活动期间内体内6-MH浓度的盛行,需要在贴剂的使用期间,将下述VLT和6-MH剂量注射到体内:其中VLT和6-MH剂量的值必须具有一个VLT的生物可利用剂量/6-MH的生物可利用剂量比,其能确保在整个昼夜的盛行及其交替。该生物可利用剂量比可以从其药代动力学参数计算,如下所述。
通过设置VLT的生物可利用剂量为40μg,该剂量为适合治疗原发性失眠和神经退行性疾病,例如帕金森病等,对应于MLT松果体分泌量低(从上午1点起,MLT分泌量低于5μg,MLT血浆浓度低于10pg/ml)的疾病的中间剂量,可以计算出共同施用的6-MH的生物可利用剂量,以确保在休息和活动期间两种激素的盛行及其交替。
在向血流中恒速(零级药代动力学)输注VLT(或6-MH)时,VLT血浆浓度逐渐增加,直至其达到平台,其中消除速率等于输注速率。因此,输入的VLT等于输出的VLT,血浆浓度保持恒定。这就是“平台”状态。如果在8小时内输注的VLT量为40μg,我们可以在平台状态下记录:

Vdss=5升/kg体重,或对于人类体重64kg为320升。
6-MH:
T1/2z=2.27h
Vdss=5升/kg体重,或对于人类体重64kg为960升。
由于药代动力学特征,即药代动力学参数值,在人类和狗中是相同的,正如在所有昼行性动物中一样,因此可以将在狗中观察到的PK参数值外推到人类中。
不仅需要确保两种激素的浓度在休息和活动期间内,在血液(血浆浓度)中的盛行和交替,而且在其作用位点(5-HT2C、α2、D1和D2受体位点等)处的盛行和交替也要确保。因此,有必要考虑两种激素的分布容量(Vdss)值,其将激素的血浆浓度与其作用位点的浓度相关联。
向人体输注40μg的量的VLT的VLT最大血浆浓度(Css)的计算。根据上述关系1,可以记录:
对于VLT,
对于分布容量比VLT高三倍的6-MH,为了维持休息期间内,血浆和外周受体位点处的6-MH浓度的盛行,最大血浆浓度(Css)必须维持在VLT的最大血浆浓度的三分之一的值。为此,与40μg的VLT输注剂量组合的6-MH的输注剂量不应超过10μg。实际上,对于10μg的输注剂量,有:

考虑到分布容量比VLT高出三倍,在休息期间,受体附近的最大6-MH浓度将总是低于VLT。
总之,为了保证两种激素(VLT和6-MH)的盛行及其交替,必须向血流输注至少比6-MH的剂量高四倍的VLT剂量。
根据本发明的另一特定方面,粘合性透皮治疗系统包含瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)的负载的组合,其能够在贴剂的施用期间,优选约8至10小时,向血流递送[VLT]/[6-MH]质量比至少等于4的瓦伦托宁和6-甲氧基哈马兰的组合。
对于剂量的水平,即数量级,我们用在人类中测量的松果体褪黑激素分泌值作为基础。
对于睡眠障碍和内源性和反应性抑郁症等,特征为从上午1点起在患者中测定的MLT最大血浆浓度在10pg/ml和50pg/ml之间的疾病的治疗,建议在睡觉时(根据习惯,约下午10点或稍后),施用具有VLT和6-MH的负载的贴剂,在贴剂的施用期间内,根据起床时间(此刻需去除贴剂)的不同为8到10小时之间,向机体递送剂量为20μg的VLT和5μg的6-MH。
根据本发明的另一个特定方面,粘合性透皮治疗系统包含VLT和6-MH的负载,为了治疗睡眠障碍和抑郁症,在贴剂的整个施用期间,优选约8至10小时,向血流递送剂量为至少20μg的VLT和至少5μg的6-MH。
对于原发性失眠和神经退行性疾病例如帕金森症等,特征为从上午1点起在患者中测定的MLT最大血浆浓度低于10pg/ml的疾病的治疗,建议在睡觉时(根据习惯,约下午10点或稍后)施用具有VLT和6-MH的负载的贴剂,在贴剂的施用期间内,根据起床时间(此刻需去除贴剂)的不同为8到10小时之间,向机体递送剂量为40μg的VLT和10μg的6-MH。

对于阿尔茨海默病型神经退行性疾病等,特征为从上午1点起在患者中测定的MLT最大血浆浓度低于1pg/ml或甚至无法测量的疾病的治疗,建议在睡觉时(根据习惯,约下午10点或稍后)施用具有VLT和6-MH的负载的贴剂,在贴剂的施用期间内,根据起床时间(此刻需去除贴剂)的不同为8到10小时之间。
根据本发明的另一个特定方面,粘合性透皮治疗系统包含VLT和6-MH的负载,为了预防和/或治疗阿尔茨海默病,在贴剂的施用期间,优选约8至10小时,向血流递送剂量为至少60μg的VLT和至少15μg的6-MH。
如已经指出的,本发明还涉及由松果体过量分泌[(VLT)-(6-MH)]系统的激素而引起的病症的治疗。精神疾病的治疗的具体情况包括:在活动期间,施用仅负载瓦伦托宁的贴剂。VLT应当取代精神疾病患者分泌的过量6-甲氧基哈马兰。
Claims (6)
1.一种粘合性透皮治疗系统,其特征在于,其包含作为活性成分的瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)的组合。
2.如前述权利要求所述的粘合性透皮治疗系统,其特征在于,其包含瓦伦托宁(VLT)和6-甲氧基哈马兰(6-MH)的负载的组合,其能够在贴剂的施用期间,优选约8至10小时,向血流递送[VLT]/[6-MH]质量比为4的瓦伦托宁和6-甲氧基哈马兰。
3.如权利要求1或2所述的粘合性透皮治疗系统,其用作药物。
4.如权利要求1至3中任一项所述的粘合性透皮治疗系统,其用于治疗睡眠障碍和抑郁症,所述粘合性透皮治疗系统包含VLT和6-MH的负载,其在贴剂的施用期间,优选约8至10小时,向血流递送剂量至少为20μg的VLT和剂量至少为5μg的6-MH。
5.如权利要求1至3中任一项所述的粘合性透皮治疗系统,其用于治疗原发性失眠和/或帕金森病型病症,所述粘合性透皮治疗系统包含VLT和6-MH的负载,其在贴剂的施用期间,优选约8至10小时,向血流递送剂量至少为40μg的VLT和剂量至少为10μg的6-MH。
