详细描述
公开了一种可穿戴设备和与之相伴的用于连续脉动BP的确定的方法。绝对值可在初始阶段被获得,以及转移函数如何能够将在手指或手腕处获得的BP信号转换为对应于肱动脉并在心脏高度处的真实BP值。公开了一种利用NBP以便确定转移函数的一次性校准方法。
可穿戴设备包含直立高度校正元件(例如加速器或运动传感器),其可测量心脏高度与实际测量发生的手指/手腕高度之间的垂直距离。直立压力差可被计算出,并且BP信号可被校正为被用于常规BP测量的心脏高度压力值。
可穿戴设备具有不同的操作模式。一种模式用于血液动力学参数的连续测量。在该模式下,接触压力CP(t)被自适应地改变为平均动脉血压(mBP)或mBP的一部分。
另一模式是初始化模式,其中收缩压、舒张压和平均动脉BP(sBP、dBP、mBP)被确定。平均动脉BP(mBP)是连续BP测量的起始点。sBP和dBP被用于对转移函数的正确确定,以便将手指/手腕处获得的BP信号校正为中心值。这被用于BP的正确临床解释。根据该特殊的初始化模式(其中实际的sBP、dBP和mBP可被确定),在使用设备之前,对NBP进行校准的需要被消除,或至少降低到为了中心BP值的对转移函数的一次性个性化。
另一模式可以是空闲模式,其中接触压力被降低到30-40mmHg。PPG信号仍被测得,并且可被用于心率检测。当患者的跌倒被检测到时,系统可由运动传感器自动地切换到初始化模式并且进一步切换到测量模式。用于切换到初始化模式并进一步切换到测量模式的另一触发可能是心率方面的意外增加或降低或者不同的定时间隔。
此外,可穿戴设备具有用于将信号无线地传送到诸如智能电话、平板或其他计算机之类的主机设备的元件。例如,可穿戴设备可以采用戒指或手表或手镯的形式。
图1示出了被置于手指100上的现有技术的系统101。系统101包括一个或多个光源102(优选地是LED),以及一个或多个光接收器103,其生成光电体积描记(PPG)信号PPG(t)。PPG(t)是控制器104的输入。控制器104使用致动器105来改变PPG系统的接触压力CP(t)。在控制器104的帮助下,改变的接触压力CP(t)遵循真实的mBP。
图2示出了本申请的系统,包括对临床相关BP的测量。元件200至205类似于图1。为了获得临床上有用的BP值和真正的连续BP信号,手指BP通过使用数学转移函数206被校正为中心值。对于该转移函数,PPG(t)由一个或多个光接收器203产生,并且施加自致动器205的CP(t)被使用。此外,在开始使用设备之前,转移函数206的初始值(例如个体收缩和舒张BP)可以在初始阶段和/或在对患者的个体值进行一次性校准期间被获得。转移函数206可以用在微控制器或其他计算机上运行的软件来实现。
对于临床使用,如果BP传感器的位置不同,则BP必须在心脏高度被测量或者必须被校正到该高度。在日常使用中,传感器的测量点通常不同于心脏高度。为了将从手指或手腕处的本发明的传感器获得的BP信号连续地校正至心脏高度,直立高度校正元件207被使用。该直立高度校正元件207通常测量或计算心脏高度与测量点之间的压力(直立的)差。
对于该校正,转移函数206接收来自直立高度校正元件207(例如加速器或运动传感器)的信息,该直立高度校正元件207可连续地测量心脏高度与实际测量发生的手指/手腕高度之间的垂直距离。直立BP差可使用已知的血液密度和垂直距离来被计算出。因此,BP信号可被校正为心脏高度压力值,这对于常规的BP测量而言是重要的。
在另一实施例中,直立高度校正元件207可以是PPG系统和心脏高度之间的充液软管,其被用作直立高度校正元件(未示出)。该软管填充有具有类似于血液的密度的密度的流体。否则,校正因子c必须被使用,其中c是血液的密度与所使用的流体的密度之比。一端靠近自由浮动的膜,而另一端靠近标准的压力传感器。与校正因子c相乘的获得自该心脏高度压力传感器的值等于直立BP差,并且可被用于心脏高度校正。
进一步的血液动力学参数可以使用数学生物医学算法208从脉动BP信号中获得。一种类型的生物医学算法208可以是用于心输出量、心搏量、外周阻力等的确定的脉搏轮廓分析方法。其他参数(尤其是针对自主神经系统的评估的参数)可通过使用本领域已知的其他数学方法来被计算出。生物医学算法208可以用在微控制器或其他计算机上运行的软件来实现。
优选地,使用转移函数206的BP信号的计算、校正至心脏高度207以及进一步的生物医学算法208均在使用软件的一个或多个微控制器或计算机(μC)中进行。这些μC可被放置在系统201中和/或智能电话、平板或被连接到互连网的其他计算机上(参见图7的进一步解释)。
图3显示了系统的一般框图和硬件元件。系统301被连接到手指300并且包括一个或多个光发射器302(优选地是具有不同波长的LED),以及生成PPG信号PPG(t)的一个或多个光接收器303。PPG信号被发送至控制器单元304,优选地是微控制器或使用软件的其他类型的计算机。例如,微芯片AS7000(ams AG,Unterpremstaetten,奥地利)是具有数字处理器的组合式光学光接收器,其可被用于测量PPG(t)以及利用致动器305来进一步控制接触压力CP(t)。
在基本实施例中,微控制器μC 304使用致动器305来控制接触压力CP(t),通过使用压力传感器306来测量接触压力CP(t),控制一个或多个光发射器LED302并接收来自一个或多个光接收器303的PPG信号PPG(t)。
压力传感器306与致动器305组合使用。压力传感器306将直接测量CP(t),并因此增加CP(t)测量的准确度,其为最终BP信号的主要输入。在替代实施例中,接触压力CP(t)可从致动器305的控制信号中导出。
致动器305可以是步进电机、压电元件或通过向其表面施加电信号来改变其尺寸的“新材料”。理想地,“保持”压力不应该需要能量。如上所述,压力传感器306被包括以用于CP(t)的准确测量。
μC 304可进一步计算上述数学转移函数206以及生物医学算法208,如针对心搏量和心输出量测量的脉搏轮廓分析,以及例如针对自主神经系统的评估的进一步的生物医学计算。
μC 304可进一步与电源和电力管理单元307连接,其接收并管理来自优选地可加载的电池308的电力。因此,接收自电池308的电力可取决于不同操作模式下的能量需求来被优化。该单元还负责加载电池308。
对于临床使用,BP必须在心脏高度被测量,并因此直立高度校正元件309被连接到μC 304。在一个实施例中,直立高度校正元件309可以是先前描述的充液软管。
在另一实施例中,直立高度校正元件309可以是加速器或运动传感器。例如,微芯片LIS3DH(STMicroelectronics)或FXLS8471QFS(Xtrinsic)代表这样的加速器或运动传感器,其可被用于连续计算心脏和传感器的位置之间的垂直差。
为了使加速器或运动传感器归零,按钮310被连接到μC 304。在归零过程期间,传感器被保持在心脏高度,并且按钮310被推动以指示零直立差。理想地,短按或双按按钮指示归零,而长按可打开或关闭传感器。μC 304存储加速器或运动传感器309的实际垂直值,并且可接着连续地计算被存储的值与新的实际垂直值之间的差。因此,直立压力可根据垂直差和血液密度来被计算出。在另一实施例中,当BP以毫米汞柱(mmHg)被测得时,如果以cm为单位的差是已知的,则mmHg和厘米水柱(cmH20)之间的比例因子可被用于直立压力的计算。所得到的值必须用血液的相对密度来被校正。
加速器或运动传感器309可检测显要的运动伪像,并且受干扰的信号可被标记。进一步的归零过程将被需要,这可以在主机设备上被指示。针对再次归零的另一标志可以是接触压力CP(t)的交替摆动。
为了将BP信号传送到智能电话、平板或优选地可连接到互联网的另一台计算机,信号发射器311被连接到μC 304。该发射器可以是WLAN或蓝牙或任何其他可用的发送器。该单元还可从主机接收信息。
图4是示出系统的不同操作模式的流程图。在打开传感器之后,设备将处于空闲模式401。当接触压力CP(t)处于空闲位置时,传感器(戒指、手表或手镯)可相应地被置于手指或手腕上。PPG系统应该处于正常值,其约为30-40mmHg。系统开始测量现在被用于心率监测的PPG信号。主机设备将在发生按下按钮、检测到的跌倒、心率的立即增加或降低的情况下开始起始于初始阶段402的测量。
在初始阶段402中,CP(t)由控制系统和致动器调整到不同的压力水平。PPG(t)被测得,具体而言在不同的CP水平对PPG幅度进行比较。mBP现在可以在最优接触压力CPOPT处被确定,其中光电体积描记信号的最高幅度被同时测得。与常数因子相乘的该mBP现在可被用作测量模式403的起始点CPStart。
除了mBP以外,收缩和舒张血压sBP和dBP可使用PPG(t)的幅度相对于所施加的CP的分布来估计。通常,PPG幅度的分布根据包络曲线,其中包络的宽度对应于脉压(sBP减去dBP)。sBP和dBP可根据这种所谓的示波法来被估计。
在测量模式403下,PPG信号被测得,并且控制器计算用于将CP(t)连续地改变为mBP的所谓的VERIFI准则(如WO 2013178475中所描述的)。如果VERIFI指示血管舒缩变化和/或mBP变化,则CP(t)被相应地调整。转移函数被应用于PPG(t)和当前的CP(t),以便取得连续的BP。所得到的BP信号和/或根本分量PPG(t)、CP(t)以及心脏高度和传感器高度之间的连续的垂直差被发送到主机设备。生物医学算法进一步根据真实的脉动BP信号计算血液动力学参数。
设备可通过将CP(t)再次降低到约30-40mmHg来被切换回另一空闲模式404。PPG信号仍可被测得以用于心率监测,或者戒指/手表可从手指/手腕上被移除。电力管理能够降低能量,以实现延长的电池寿命。
在该空闲模式404下,系统还作为监督监视器工作。先前所描述的加速器或运动传感器可检测患者的跌倒并首先切换到初始化模式402,并接着切换到测量模式403。当BP信号被提交到智能电话时,为了患者的安危,紧急电话可能会被呼叫。归零直立高度校正元件将不会那么重要,因为能够假定患者躺在了地板上,并因此传感器很可能在心脏高度。
图5示出了被配置为佩戴在手指500上(优选地在食指、中指或无名指上)的戒指501的传感器。被置于第一指骨上的戒指很可能是最没有伪像的实现。戒指可通过使用例如致动器505(诸如“软管夹”机械中的步进电机)改变其周长、改变周长的压电元件或者改变周长的“新材料”来容易地改变其CP。该“新材料”能够通过向其表面施加电信号来改变其尺寸。致动器505被置于固定器507上。接触压力CP(t)使用压力传感器506来被测得。
根据图5,其他电子504元件(如μC、运动传感器、无线发射器、电力管理器、电池等)应被置于戒指内。一个用于启动/停止以及心脏高度调整的按钮511应当被添加。
“宝石”508可指示PPG传感器(尤其是光传感器502和503)的正确放置。如在图5中可以看到的,手指500包含两条指动脉509,它们在手指骨510的掌部/手掌侧。当光传感器502和503根据图5被置于手指的左右并且“宝石”508因此处于正确的背侧方向时,最佳的PPG信号可被获得。
图6示出了被配置为佩戴在手腕600上的手镯或手表601的传感器。在手腕上,简单地改变直径不能够改变桡动脉612的CP(t)。相反,被连接到固定器607的U形带608是手镯或手表601的一部分。手镯或手表601应以使得U形带608位于桡动脉脉搏可被触诊的位置的方式来被放置。该桡脉搏在桡动脉612中被生成,并通过桡骨613处的反射来被放大。系统601可用致动器605来改变其“U”的深度-当致动器605束紧时,U形带608的深度增加,并且CP(t)也增加。
压力传感器606可被置于U形带608的槽上。此外,必须存在用于PPG元件602和603的空间,PPG元件602和603于是接收在桡骨上被反射的光信号。PPG信号将有助于找到传感器的正确放置。其他电子元件604(如μC、运动传感器、无线发射器、电力管理器、电池等)应被置于“更刚性的外壳”609中。根据该配置,在尺骨615旁边的尺骨动脉614中的自由血液流动被保证。作为手表的实施例将进一步允许在手表显示器610上的用户交互。用于启动/停止以及心脏高度调整的按钮611应当被添加。
图7示出了BP信号和/或其分量(如PPG信号、接触压力以及从可穿戴设备701到可被连接到互联网的智能电话、平板或其他计算机702的垂直距离)的传输。APP正在进一步计算血液动力学和其他参数以及供显示的软件、偿付模型和其他功能。APP的功能如图7所示。
APP执行以下功能:
·从传感器接收未经滤波的BP信号及其分量;
·将控制信息发送到传感器;
·允许传感器的软件更新;
·计算进一步的血液动力学、自主和血管参数;
·有可能以用户友好和自解释的方式来显示这些进一步的血液动力学、自主和血管参数;
·传送并存储信息;
·允许不同的商业模型;
·支持来自健康护理系统的偿付;
如上所述,系统还包括转移函数206。该转移函数包括不同的部分。第一部分是接触压力CP(t),其自适应地被改变为mBP或者与mBP相乘的常数因子c1(如WO 2013178475中所描述的)。因此,在身体的远端部位中的通常在手指或手腕处的mBP使用CP(t)或CP(t)/c1来被测得。
接下来的部分是可被用于连续的BP信号的脉动PPG(t)。PPG(t)必须通过增益因子k来被放大。所得到的手指BP信号为:
BPf(t)=CP(t)/c1+k*PPG(t)
其中k可根据在初始模式期间被测得的sBP和dBP值来估计。PPGsys和PPGdia是与sBP和dBP对应的PPG(t)值:
k=(sBP–dBP)/(PPGsys–PPGdia)
手指BP信号随后必须通过使用先前所描述的垂直距离dv和[mmHg]到[cmH2O]的转换因子(其为0.73556)以及血液的相对密度ρBlood(其在37℃为1.0506(95%置信区间:1.0537-1.0475))来被校正到心脏高度。
BPf(t)=CP(t)/c1+k*PPG(t)+0.73556*dv*ρBlood
当充液软管被用于心脏高度校正时,真实的直立压力差po(z)被测得。
BPf(t)=CP(t)/c1+k*PPG(t)+po(z)
沿动脉途径的BP变化以及外周BPf测量需要被转移为与肱动脉处获得的标准值相当的值。转移函数206的该部分通常是频率分量H(f)的偏移,并且存在使用通用转移函数的众所周知的概念。
206的该通用转移函数H(f)可通过在首次使用之前所执行的校准来被个性化。上臂袖口BP值应当在初始模式计算手指sBP和dBP的同时被获得。这些值,即上臂袖口sBP和dBP以及手指sBP和dBP可被用于个性化的通用转移函数H(f)以获得近端BP信号。
一般来说,转移函数206以及该转移函数206的所有部分可以在微控制器304上运行的软件中或者在智能电话、平板或其他计算机702上的APP中实现。
另外的实施例是作为脉搏血氧计的附加用途。通常,系统将在波长λCNAP=805nm处的氧合血红蛋白和脱氧血红蛋白的等消光点处工作。当LED由在脉搏血氧(pulsoxy)波长(λred=660nm,λIR=910nm)处工作的双色LED交换时,传感器还可通过使用最先进的脉搏血氧计方法作为SpO2环或手表工作。
虽然上面已经讨论了多个示例性方面和实施例,但是本领域技术人员将认识到,所公开的实施例的特征的其他进一步的修改、置换、添加以及其子组合仍然是可能的。因此,以下所附权利要求和以下引进的权利要求旨在被解释成包括在其真实精神和范围内的所有这样的修改、置换、添加和子组合。
Claims (6)
1.一种用于测量血液动力学参数的可穿戴设备,包括:
光电体积描记系统,具有:
一个或多个光源;
生成光电体积描记信号的一个或多个光检测器;
用于将所述光电体积描记系统附接到包含动脉的身体部位的具有致动器的安装元件;
用于控制所述致动器以及所述光电体积描记系统的接触压力的控制系统;以及
被置于所述光电体积描记系统旁边的被用于心脏高度校正的运动传感器,其中所述控制系统能被切换至不同的操作模式A到C中的至少一种:
模式A用于血液动力学参数的连续测量,
其中所述接触压力由所述控制系统和所述致动器根据与常数因子相乘的平均动脉压来调整;并且
其中转移函数被应用于所述光电体积描记信号和所述接触压力;
模式B用于血液动力学参数的所述连续测量的初始化,
其中所述接触压力由所述控制系统和所述致动器调整到不同的压力水平;
其中所述光电体积描记信号的幅度被确定;
其中平均血压在所述接触压力处被确定,其中所述光电体积描记信号的最高幅度同时被测得;并且
其中与常数因子相乘的所述平均血压被用作所述模式A的起始点;以及
空闲模式C,
其中所述接触压力被调整到30–40mmHg的恒定压力;并且
其中所述控制系统等待用户交互以便使所述控制系统进入初始化模式并进一步进入所述模式A;并且
其中所述光电体积描记信号被测得以用于获得心率;
其中如果心率增加超过或减小低于预定义阈值,则所述控制系统能够进一步进入初始化模式并接着进入所述模式A。
2.根据权利要求1所述的可穿戴设备,其特征在于,所述接触压力由压力传感器测得。
3.根据权利要求1所述的可穿戴设备,其特征在于,还包括用于将所得到的血液动力学参数发送到被连接到互联网的主机设备的发射器。
4.根据权利要求1所述的可穿戴设备,其特征在于,至少两个或更多个光源或波长被使用;
其中所述至少一个光检测器生成对应于所述至少两个或更多个光源或波长的两个或更多个光电体积描记信号;并且
还包括用于通过使用所述两个或更多个光电体积描记信号来计算氧饱和度的装置。
5.根据权利要求1所述的可穿戴设备,其特征在于,所述设备的元件被置于戒指中,其中所述戒指的周长能够由所述致动器改变。
6.根据权利要求1所述的可穿戴设备,其特征在于,所述设备的元件被置于手镯或手表中,其中所述致动器改变所述手镯或手表在手腕处的接触压力,其中桡动脉能够被触诊。
