背景技术
机械缝合或缝合器已经改变了外科手术的前景,使得侵入性较小并且具有更好的手术结果。特别地,在胃肠外科领域,机械缝合已经以微创方式常规执行在现今多个手术疗程。即使如此,还是无法免除副作用,其中最严重的是与机械缝合相关的两个主要并发症:切割线的出血和渗出。
自创造机械缝合以来已经引入了多种改进,这已经降低了出血和渗出的速率,但是仍然存在显着百分比的患有这类型的潜在致命并发症患者。对于病态肥胖手术疗程在国际公认的分离速度为高达5%。

另一方面,术后胃过滤的病理生理学还没有很好地定义。它已经被认为是一种多因素状况,其中汇集了组织的质量、患者的代谢状态、外科技术和所用医疗用品的故障等因素。它主要出现在手术后的第一周内,并且通常位于切割/缝合区域的上段,接近于胃食管角的角度。即便是改善了技术和患者的相关因素,渗出速率仍然是现有技术存在的问题,因此本案提出了对医疗工业的持续挑战,用以开发更多地减少上述并发症或直接消除并发症的装置。

1.美国专利公告第US 5,782,396号(玛斯崔等人)提出一种手术钉合器。所述发明公开一种外科缝合装置,其可用于顺序平行的将手术固定线施加到身体组织,并且任选地,切割腹腔镜外科手术中保持的组织。所述装置可以与不同的一次性装载单元一起使用。
2.美国专利公开第US 2010/0282816号(斯基里卡等人)提出一种钉合器匣和通道联锁。所述技术保护外科缝合器械,所述外科缝合器械由砧座、定义一纵向轴线并具有一匣信道的套筒组件组成,所述匣通道具有远程,所述远程具有横向于纵向轴线延伸的至少一个突出部,更包括位于匣通道内的钉合器盒。所述仪器与其它相关联的线性缝合装置可以在内视镜以及已知的开放结构中使用。

4.美国专利公告第US 5,911,353号(博拉诺斯等人)提出一种用于外科缝合器的一次性装载单元。所述专利提到一次性装载单元,其与外科缝合装置一起使用,用于在身体组织中依序施加多个外科钉合线。所述装载单元包括具有延伸穿过其纵向路径的钉匣主体和多个分离的维持槽,多个闭合喷射组件和支撑在钉匣主体内的致动器。
上述技术不能确保在胃套管手术或全胃切除术中完全避免切割线的出血和渗出;因此,需要开发新的医疗器材。
发明内容
有鉴于此,本发明提供一种适用于一外科缝合器的装置,用于胃套管手术或全胃切除手术。所述装置为缝钉匣类型,其具有朝向欲缝合的一片段部的五排缝钉以及朝向被抽出的一片段部的一排缝钉。
这种缝钉排的设置结合了组织的高安全性,这有利地减少了有关于消化系统一段中的仅一端的缝合其过程中的并发症(例如渗出)。
所述缝钉匣,因为其在仅存的胃的一侧具有五个缝合线,一方面可以减少出血,同时对缝合线提供更大的机械支撑,特别是在第一天,必须承受由于术后恶心及/或呕吐产生的最大压力情况。
上述由更多排的缝钉组成的强化提供更大的强度,这可能转化为对失效的第一缝钉线的支撑。另一方面,胃被切取的部份仅必须由一线缝钉缝合,这在胃被切取时不是非常相关,并且甚至其在手术中破裂也不会对病人造成负面后果。关于出血,机械强化功能件以类似的方式产生作用,因为具有一较长的缝钉段,其具有较大可能性独立闭合弯曲度较小血管。

具体实施方式
为了更好地理解本发明,参考图1和图2,其中分别示出了所述匣的上视图和下视图。可以理解在(1)为五排缝钉以及(2)为一排缝钉;(3)对应于缝钉推进器,其由所述装置控制,所述缝钉推进器可以进入所述匣中或在发射它们的缝合器中;(4)是滑动致动器,其也由所述装置控制,可以进入所述匣中或在发射它们的缝合器中,其滑动以与多个所述缝钉推进器接触以开始依序施加供应缝钉(5)的功能;以及(6)对应于由中间分开的两个片段组成的一后端,以供一刀具通过,负责进行切口。这些片段中的至少一个具有一曲线形端部以使所述匣适应于现有的缝合器。所述匣的宽度为10毫米,并且长度可以在4厘米、6厘米或8厘米之间变化,以容易适应任何缝合器。
图3示出了设置于所述匣中的多个所述缝钉推进器的放大图,其允许施加缝钉并且连接至所述匣的槽。它们被依序地放置,并且取决于它们在盒中的位置以呈现较短或较长的片段,后者的区域用于多个所述匣发射五条线的情况。这些多个所述缝钉推进器使的三种不同尺寸的缝钉被推动,其中最高的是在切割处左边的两条线,即相当于4厘米高的缝钉,然后其余两个为3.5厘米的缝钉,并且两端在切口的两侧对应使用3厘米的缝钉。缝钉的尺寸可以根据待缝合组织的厚度而变化。
图4示出了现有缝合器的示意图,其缝钉匣(7)适于5:1的缝合线的设置,其有利地避免了手术中的切割线的出血和渗出。此外,(8)标示所述刀具的位置、(9)标示所述刀具的支撑件和(10)示出了一移动杆,其为一轴向移动控制杆,传送所述缝钉的纵向驱动力用于控制其头部的轴向运动。
实施例应用
所述缝钉匣采用常见的缝钉(),以验证其在活体组织上的功能。
用所述缝钉匣控制的两个缝合器执行操作的一实施例,在图2中,我们可以看到所述缝钉匣,在其前端部分已经施加所述缝钉推进器(3),而在后端示出了已经施加的所述缝钉(5)的底部,并且在顶部中示出了所述缝钉推进器仍处于其原始位置。
可以在原始位置证实当施加五条缝钉时,在两个缝合器情况下实现了机械阻力的增加和切割线显着较少的出血。
当试图用包括量测重量的张力装置分离接合的组织时,证实了缝合线间阻力的增加。相较于现有的缝合器研究中,可以观察到出血的显着减少。其中在使用后5:1的这种设置后,在切割线上没有显示任何出血。上述意味着这种装置用于这种类型的手术会有显着改进。

Claims (2)

1.一种适用于一外科缝合器的装置,其特征在于:所述装置包含:
五排缝钉(1)朝向欲缝合的一片段部;
一排缝钉(2)朝向被抽出的一片段部;以及
在一后区(6)中,所述装置具有在中间分离的两个片段,以供一刀具通过,并且其中这些片段中的至少一个具有一曲线形端部,以使所述装置适用于所述缝合器。
2.如权利要求1所述的适用于一外科缝合器的装置,其特征在于:所述装置具有一宽度为10毫米,及一可变长度为4厘米、6厘米或8厘米。
