背景技术
软组织修复为通常涉及以下若干时期的复杂过程,即出血期、炎症期、增殖期和重塑期(Watson,T,(2003)"Soft Tissue Healing"In Touch 104:2-9)。这些时期中每一个进而涉及生物化学事件的复杂相互作用(Watson,2003,同上)。例如,炎症期尤其涉及血管事件和细胞事件(Watson,2003,同上)。血管事件的结果包括血管化,而细胞事件的结果包括细胞迁移,两者对于组织修复都很重要。

因此,需要用于软组织修复的基于纤调蛋白的疗法。

定义

在一些实施方案中,生物相容性设备具体排除了与皮肤伤口愈合或角膜伤口愈合相关的WO2004053101 A3或WO2009135135 A3中所公开的此类设备。
生物相容性设备的一些其他实例为支架,诸如在胆道重建后吻合伤口愈合的蛋白质洗脱生物可降解聚合物支架,或在手术期间植入狭窄冠状动脉中的冠状动脉支架。
纤调蛋白
纤调蛋白(FMOD)(也称为SLRR2E)为小间质蛋白聚糖家族的成员。所述蛋白质为59kDa,具有与二硫化物键合的末端结构域侧接的富含亮氨酸的重复序列,其拥有高达4个硫酸角蛋白链(Takahashi,T.,Cho,H.I.,Kublin,C.L.&Cintron,C.(1993)J HistochemCytochem 41:1447-57)。纤调蛋白表现出在关节软骨、腱和韧带中发现的具有最高浓度的广泛组织分布。纤调蛋白的亚细胞位置在具有分泌序列但不具有跨膜或细胞外结构域的胞质蛋白内。

已经发现纤调蛋白基因为B细胞慢性淋巴细胞白血病和慢性淋巴细胞白血病(CLL)中的过表达基因。它可以充当CLL中潜在的肿瘤相关抗原(TAA)(Mayr,C.等(2005)Blood 105:1566-73;Mayr,C.等(2005)Blood 106:3223-6)。人、牛、大鼠和鼠纤调蛋白的氨基酸序列示出了90%的总体同源性,从而允许人和鼠实验模型之间的紧密转译(Antonsson,P.,Heinegard,D.&Oldberg,A.(1993)Biochim Biophys Acta 1174:204-6)。
纤调蛋白多肽
可以将纤调蛋白多肽(也不时地称为纤调蛋白蛋白质)或其促进血管生成功能的部分施用于有需要的个体。在一种方法中,可以将例如在携带重组纤调蛋白表达载体的培养细胞中产生的可溶性纤调蛋白多肽施用于个体。纤调蛋白多肽或其部分将通常静脉内施用。这种方法可快速将蛋白质递送到整个系统,并且最大化蛋白质在递送时为完整的可能性。可替代地,考虑治疗性蛋白质施用的其他途径,诸如通过吸入。作为气雾剂施用药剂(包括蛋白质药剂)的技术为熟知的并且继续发展。可替代地,可以配制多肽药剂用于局部递送,包括例如脂质体中的制剂。还考虑的是例如经皮施用和经直肠施用或经阴道施用。本文所述的递送纤调蛋白多肽的其他选择在下文“药物组合物”一节中讨论。

这些载体容易适用于本发明的方法。通过使用重组DNA/分子生物学技术的适当操作将可操作地连接的纤调蛋白编码核酸片段插入到所选择的表达/递送载体中,可以生成用于实践本文所述方法的许多等效载体。本领域的技术人员将理解,可以容易地操作克隆基因以改变蛋白质的氨基酸序列。

在一些实施方案中,聚乙烯被用于构建设备的至少一部分。例如,聚乙烯可以用于表面上的整形外科植入物,其被设计成因此在关节或髋替代中接触另一植入物。当聚乙烯与其他材料接触时,聚乙烯非常耐用。当金属植入物在聚乙烯表面上移动时,如在大多数关节替代中一样,接触为非常平滑的并且磨损量最小。更年轻或更有活力的患者可能受益于具有甚至更强的耐磨损性的聚乙烯。这可以通过称为交联的方法来实现,所述交联在构成聚乙烯的元素之间产生更强的结合。交联的适当的量取决于植入物的类型。例如,髋植入物的表面可能需要与膝盖植入物的表面不同程度的交联。

剂量和施用
通常,剂量范围为0.0005mg/kg体重至25g/k体重。在一些实施方案中,剂量范围为0.001mg/kg体重至0.5g/kg体重、0.0005mg/kg体重至0.1g/kg体重、0.001mg/kg体重至0.05g/kg体重。作为另一个替代方案,剂量针对局部递送来选择,并且不一定针对体重或为了实现一定的血清水平来选择,而是为了实现局部作用,例如对于局部注射、植入或其他局部施用于眼睛。
上述剂量的施用可以在有限的时间段内重复。在一些实施方案中,剂量被每天给予一次或每天多次,例如但不限于每天三次。在优选的实施方案中,上述剂量被每天施用持续若干周或若干月。治疗的持续时间取决于受试者的临床进展和对疗法的反应性。在初始较高的治疗剂量之后考虑连续的、相对低的维持剂量。
可用于本文所述的方法和组合物中的药剂可以通过局部、静脉内(通过快速注射或连续输注)、口服、通过吸入、腹膜内、肌内、皮下、腔内施用,并且可以通过蠕动方式(若期望)或通过本领域技术人员已知的其他方式递送。优选将用于本文所述的方法的药剂局部施用于眼睛。对于肿瘤的治疗,药剂可以全身施用,或可替代地,可以直接施用于肿瘤,例如通过瘤内注射或通过注射到肿瘤的初级血液供给中。
含有本文公开的至少一种药剂的治疗组合物可以常规地以单位剂量施用。当关于治疗组合物使用时,术语“单位剂量”是指适于作为单一剂量用于受试者的物理上离散的单位,每个单位含有与所需生理学上可接受的稀释剂(即载体或媒介物)缔合的经计算可产生期望治疗效果的预定量的活性材料。
组合物以治疗有效量、以与剂量制剂相容的方式施用。待施用的量和时间取决于待治疗的受试者、受试者系统利用活性成分的能力以及期望的治疗效果的程度。可以通过靶向部分(诸如像抗体或靶向脂质体技术)来靶向药剂。在一些实施方案中,纤调蛋白活性抑制剂可以通过使用双特异性抗体(例如通过反配体抗体(Ab)和针对特异性靶标的Ab的化学连接产生的抗体)靶向组织特异性靶标或肿瘤特异性靶标。为了避免化学缀合物的限制,抗体的分子缀合物可用于产生位于细胞表面分子处的重组双特异性单链Ab引导配体和/或嵌合抑制剂。向纤调蛋白活性抑制剂中添加抗体允许附着的药剂在期望的靶位点处叠加性地累积。可以采用基于抗体或基于非抗体的靶向部分将配体或抑制剂递送到靶位点。优选地,出于这种目的,使用用于未调节的抗原或疾病相关的抗原的天然结合剂。
需要施用的活性成分的精确量取决于从业者的判断,并且对每个个体都是特定的。然而,本文公开了用于全身施加的合适的剂量范围,并且所述剂量范围取决于施用途径。施用的合适方案也是可变的,但典型的是初始施用、接着通过随后的注射或其他施用在一个或多个间隔处重复剂量。可替代地,考虑足以在血液中将浓度维持在体内疗法指定的范围内的连续静脉内输注。
药剂可以适用于基于导管的递送系统,所述递送系统包括涂覆的球囊、缓释药物洗脱支架或其他药物洗脱形式、微胶囊PEG脂质体或用于使用或不使用辅助技术(诸如超声波)的直接机械干预来递送的纳米珠粒。
药物组合物
本发明涉及可用于实践本文所述的治疗方法的治疗组合物。治疗组合物含有生理上可耐受的载体以及作为活性成分溶解或分散在其中的本文所述的活性剂。在优选的实施方案中,当出于治疗目的施用于哺乳动物或人患者时,治疗组合物不具有免疫原性。如本文所用,术语“药学上可接受的”、“生理上可耐受的”和其语法变化,它们是指可互换使用的组合物、载体、稀释剂和试剂,并且代表能够施用于哺乳动物或施用在哺乳动物上而不产生不期望的生理作用(诸如恶心、头晕、胃痛等)的材料。药学上可接受的载体不会促进增加对与其混合的药剂的免疫应答,除非如此期望。含有溶解或分散在其中的活性成分的药物组合物的制备在本领域中为熟知的,并且不需要基于制剂来限制。通常,此类组合物被制备成作为液体溶液或混悬液的可注射物;然而,也可以制备成适于在使用之前在液体中溶解或混悬的固体形式。所述制备也可为乳化的或作为脂质体组合物呈现。活性成分可以与药学上可接受的并且与活性成分相容的并且以适用于本文所述治疗方法的量的赋形剂混合。合适赋形剂包括,例如水、盐水、右旋糖、甘油、乙醇等以及其组合。另外,若期望,组合物可以含有少量辅助物质,诸如,增强活性成分的有效性的润湿剂或乳化剂、pH缓冲剂等。本发明的治疗组合物可以包括其中组分的药学上可接受的盐。药学上可接受的盐包括酸加成盐(由多肽的游离氨基形成),所述酸加成盐由无机酸(诸如像盐酸或磷酸)或有机酸(诸如醋酸、酒石酸、扁桃酸等)形成。由游离羧基形成的盐也可以衍生自无机碱(诸如像钠、钾、铵、钙或铁的氢氧化物)和这样的有机碱(诸如异丙胺、三甲胺、2-乙氨基乙醇、组氨酸、普鲁卡因等)。生理上可耐受的载体在本领域中为熟知的。示例性液体载体为无菌水溶液,其不含有除了活性成分和水之外的材料,或含有缓冲液,诸如在生理pH值下的磷酸钠、生理盐水或两者(诸如磷酸盐缓冲盐水)。此外,含水载体可以含有多于一种缓冲盐,以及盐(诸如氯化钠和氯化钾)、右旋糖、聚乙二醇和其他溶质。液体组合物还可以含有除了水之外以及排除掉水的液相。示例性此类另外的液相为甘油、植物油(诸如棉籽油)和水-油乳剂。在治疗具体病症或病状中有效的本文所述的方法中使用的活性药剂的量将取决于病症或病状的性质,并且可以通过标准临床技术来确定。

实施例1:纤调蛋白增强皮肤伤口愈合期间的血管化
概述
方法:通过鸡胚绒毛尿囊膜(CAM)测定、MatrigelTM栓塞植入物测定和啮齿动物初级闭合伤口模型来评估FMOD在体内的血管生成作用。通过细胞侵袭以及人脐静脉内皮细胞(HUVEC)的尺寸参数和拓扑参数来记录FMOD在体外的血管生成作用。
结果:我们提供了FMOD显著增强血管化的证据:首先,FMOD在CAM上推进血管形成;其次,FMOD明显地刺激毛细血管浸润到皮下植入在成年小鼠中的MatrigelTM栓塞中;并且最后,FMOD在多种成年啮齿动物皮肤伤口模型中稳健地促进血管生成。而且,FMOD施用还恢复了fmod-/-小鼠伤口的血管分布。为了支持这一点,FMOD不仅通过上调血管生成基因而且通过下调抑制血管生成基因,保证了成年啮齿动物伤口中的血管生成优势微环境。另外,FMOD显著增强了在体外的HUVEC侵袭和管状结构(TLS)形成。
结论:总之,我们证明,除了减少瘢痕形成之外,FMOD还促进血管生成。由于血管组织并且调节伤口愈合,所以FMOD潜在的血管生成特性将进一步扩大FMOD的临床应用以用于不良血管化的伤口的皮肤愈合。
引言
皮肤伤口愈合为涉及细胞事件的复杂级联以产生损伤的皮肤的表面重塑、重建和恢复拉伸强度的自然反应。不幸的是,由于伤口愈合为复杂的多方面的过程(1,2)的事实,因此一些皮肤伤口愈合失败的原因仍然很少被理解。延缓伤口愈合的根本问题为缺乏功能性细胞外基质(ECM)来刺激、引导并且协调愈合。例如,在成年小鼠皮肤伤口模型中,单个ECM分子纤调蛋白(FMOD)的缺乏导致成年小鼠皮肤伤口模型中延迟的真皮成纤维细胞迁移、延迟的肉芽组织形成、延迟的伤口闭合以及随后增加的瘢痕形成(3)。FMOD为广泛分布的小的富含亮氨酸的蛋白聚糖(SLRP),其通过与胶原结合来调节ECM组装、组织和降解(4-10)。FMOD在细胞命运决定和胎儿无瘢痕伤口愈合中起重要作用(5,11-14)。此外,我们先前的研究已经证明FMOD控制成人皮肤伤口愈合的重要方面。与它们的野生型(WT)对应物相比,FMOD无效(fmod-/-)小鼠具有降低的纤连蛋白沉积、无组织的胶原构造、改变的转化生长因子(Tgf)β信号传导以及减少的真皮成纤维细胞浸润和接着的阻碍血管生成(3,4,16)。另一方面,以腺病毒和蛋白质形式的FMOD施用减少了成人皮肤伤口中的瘢痕形成(17,18)。具体地,我们已经证明FMOD显著促进成纤维细胞迁移到伤口区域,这有助于及时的伤口闭合并且减少瘢痕形成(3,16,19)。因为新生成的血管提供营养以支持活细胞、促进肉芽组织形成并且有利于碎片的清除(20-22),所以伤口愈合不会在无血管生成(内皮细胞(EC)的新血管形成过程)的情况下发生。我们先前的研究显示,延缓的fmod-/-小鼠伤口愈合明显地与减少的血管再生有关(3),这表明了FMOD与血管生成之间的直接关系。在本研究中,调查了FMOD在未损伤和受伤的情况下对血管生成的作用。
材料和方法
卵内鸡胚绒毛尿囊膜(CAM)测定
如先前所述进行卵内CAM测定(23,24)。将受精鸡蛋(Charles River Labs,NorthFranklin,CT)在37℃和60%相对湿度下在鸡蛋孵育器中进行孵育。在第3天,从鸡蛋的尖端取出5ml白蛋白。在壳上切出矩形窗口作为CAM的入口。在第10天,将30μl的l:3稀释的生长因子减少的MatrigelTM(BD Bioscience,Franklin Lakes,NJ)中的2.0mg/ml的FMOD装载在高压灭菌的5x 5mm聚酯网层(网格尺寸:530μm;Component Supply Company,Fort Meade,FL)上,并且在移植到CAM上之前在37℃下孵育45分钟用于凝胶形成。将非FMOD磷酸盐缓冲盐水(PBS)对照以1cm距离移植到相同的CAM上。在第13天,切除CAM并且拍照。通过ImageJ(NIH,Bethesda,MD)直接在网下测量毛细血管区域密度(25)。
MatrigelTM栓塞测定
将含有0或4.0mg/ml FMOD的400μl生长因子减少的MatrigelTM皮下注射到成年129/sv雄性小鼠的腹部,在注射后14天收获覆盖皮肤(26)。
伤口生成
从每只动物中切下4块(每只成年雄性129/sv小鼠)或6块(每只成年雄性Sprague-Dawley大鼠)全部厚度10mm x 3mm的皮肤椭圆与下面的脂膜肌。所有伤口分开至少2cm以最小化相邻伤口的影响。每个开放伤口边缘在初次闭合之前注射25μl PBS或PBS中的0.4mg/ml重组人FMOD(25μl x 2个边缘=50μl/伤口)。在损伤后第7天除掉缝合线,并且在损伤后第14天收获伤口。组织被中央平分用于组织学或基因表达分析(3,4,14,16)。
组织学和免疫组织化学(IHC)染色
在4%多聚甲醛中固定后,将样品脱水、石蜡包埋并且以5μm的增量切片以用于苏木精和曙红(H&E)染色、天狼猩红(PSR)染色和IHC染色(3,4)。使用PSR耦合偏振光显微镜(PSR-PLM)来鉴定伤口区域(3)。血管由血管假性血友病因子(vWF;(Abeam Inc.,Cambridge,MA)来鉴定和定量。
基因表达测定
使用具有DNA酶处理的迷你试剂盒(Qiagen)分离RNA(3,16)。将1.0μg小鼠RNA用于使用RT-qPCR的iScriptTM逆转录超级混合物(Bio-Rad Laboratories,Hercules,CA)的逆转录。使用基因表达测定(Life Technologies)和具有ROX(Bio-Rad Laboratories)的SsoFastTM探针超级混合物(Life Technologies)在7300实时PCR系统(Life Technologies)上进行定量RT-PCR(qRT-PCR)。同时,将从成年大鼠伤口分离的2.5μg RNA注射到RT2第一链试剂盒(Qiagen)中用于逆转录。根据制造商的方案以大鼠伤口愈合RT2PCR阵列(Qiagen)的96孔形式进行qRT-PCR。测试了三种不同的cDNA模板。伴随的甘油醛-3-磷酸脱氢酶(gapdh)用作看家基因标准。数据分析由制造商的在线服务实现(http://pcrdataanalysis.sabiosciences.com/pcr/arrayanalysis.php).
细胞培养
根据制造商说明书(Life Technologies),将传代3-6次的人脐静脉内皮细胞(HUVEC)培养在增补有低血清生长补充物的培养基200PRF中。
管状结构(TLS)形成分析
使用Life Technologies提供的技术内皮管形成测定方案(http://www.lifetechnologies.com/us/en/home/references/protocols/cell-and-tissue-analysis/cell-profilteration-assay-protocols/angiogenesis-protocols/endothelial-cell-tube-formation-assay.html)在体外测定TLS。简单来说,在用提供有不同剂量的FMOD的培养基200PRF中的2.5x104个HUVEC接种之前,将24孔板用100μl/孔的减少的生长因子基底膜基质在37℃下涂覆持续1小时。4小时后通过使用奥林巴斯荧光显微镜(Center Valley,PA)记录每孔5个图像和每个处理4个孔。通过用Image J记录尺寸分析和拓扑分析来评估图像(http://image.bio.methods.free.fr/IrnageJ/7Angiogenesis-Analyzer-for-ImageJ.html&lang=en#outil_sommaire_0)。
细胞侵袭测定
使用具有8μm孔径荧光阻断PET径迹蚀刻膜(BD Bioscience)的HTS Fluoroblok插入物在24孔组织培养板中进行细胞侵袭测定。将插入物的上表面用100μl 2mg/ml减少的生长因子基底膜基质(Life Technologies)涂覆,并且置于含有750μl培养基的24孔组织培养板中。在每个插入室中添加在具有不同剂量的FMOD的500μl培养基中的2.5x104个HUVEC,并且使其侵袭膜的下侧24小时。通过用棉签擦拭膜的上侧来除去非侵袭细胞。固定侵袭细胞并且在计数之前用0.4mg/ml的4',6-二氨基-2-苯基吲哚(DAPI;Sigma-Aldrich,St.Louis,MO)染色(3)。
统计分析
统计显著性由OriginPro 8(Origin Lab Corp.,Northampton,MA)来进行,包括单因素方差分析、配对t-检验、双样本t-检验和Mann-Whitney分析。小于0.05的P值被认为是统计学上显著的。
结果
FMOD在未损伤的情况下促进血管化。施用FMOD的CAM示出比PBS对照高1.5倍比例的大直径血管(图1),这证实FMOD在发育期间促进血管生成。由于成人的血管生成可能与发育过程中的重要方式不同(27),所以使用了预先记录的MatrigelTM栓塞测定(26)来证实FMOD在体内的促血管生成作用。FMOD在MatrigelTM栓塞皮下植入的成年小鼠中明显地提高血管生成,这些小鼠的毛细血管密度为非FMOD栓塞的小鼠的4倍(图8)。因此,FMOD为未损伤的情况下的促血管生成因子。
FMOD对于伤口愈合期间的血管生成是很重要的。与我们先前在损伤后第7天的研究(3)一致,在损伤后第14天成年fmod-/-小鼠皮肤伤口的血管生成与年龄匹配的WT伤口相比减少了约50%(图2)。相反,外源性FMOD施用使fmod-/-伤口的血管分布恢复到与FMOD处理的WT伤口相同的水平,这进一步表明FMOD缺乏为造成fmod-/-小鼠伤口中血管生成减少的原因(图2)。另外,FMOD处理的成年WT小鼠皮肤伤口的毛细血管密度比PBS对照组的毛细血管密度高约2.6倍(图2)。这与FMOD施用到建立的成年大鼠皮肤伤口模型导致伤口血管分布的显著增加的发现一致(图3)。因此,这些结果强烈地赞同了我们的假设,即FMOD在未损伤和受伤的情况下都为血管生成性的。
FMOD广泛增强血管生成基因的转录并且阻碍抑制血管生成基因的表达
在皮肤中过表达血管内皮生长因子(Vegf)和血管生成素1(Angpt1)的双转基因小鼠示出更高的血管数量和大小(28)。Vegf在伤口愈合期间由表皮大量产生,并且通过增强微血管通透性以及刺激EC增殖和迁移对血管生成具有强烈的刺激作用(29-33)。成年WT和fmod-/-小鼠未受伤的皮肤组织之间vegf的表达不存在有意义的差异;然而,在损伤后第7天和第14天,WT伤口中的vegf水平显著增加(图4,左)。相比之下,在整个14天伤口愈合时期中fmod-/-伤口中的vegf表达一直保持在低水平(图4,左)。同时,FMOD在WT和fmod-/-成年小鼠伤口中都显著刺激vegf表达(图4,左)。像Vegf一样,Angpt1对血管内皮具有高度特异性。Angpt1由周细胞分泌,是EC生存和增殖以及血管成熟所需要的(28,32,34)。尽管在成年WT和fmod-/-小鼠之间没有观察到未受伤的皮肤组织中angpt1表达的值得考虑的差异,但是在伤口闭合后fmod-/-伤口中angpt1的转录水平显著低于年龄匹配的WT小鼠伤口的angpt1的转录水平(图4,右)。有趣的是,FMOD处理的WT和fmod-/-成年小鼠伤口在损伤后第14天具有相似的vegf和angpt1水平(图4),这与它们相似的伤口毛细血管密度相关(图2)。考虑到fmod-/-伤口具有减少的血管分布(其可以通过外源性FMOD施用来拯救)的事实,这些数据与肉芽组织形成期间的Vegf的关键的血管生成功能以及Angpt1的重要的对血管重塑和成熟的介导高度相关(28,34-36)。
过去已经鉴别了许多血管生成因子和抑制血管生成因子(37,38)。为了进一步丰富我们对于在伤口愈合期间FMOD如何影响血管生成相关基因的知识,在成年大鼠皮肤伤口模型中采用大鼠伤口愈合的RT2PCR阵列(Qiagen,Valencia,CA)用于高通量基因表达分析。如上示出的成年小鼠数据所见,FMOD施用提高了angpt1和vegf表达(图5)。此外,FMOD不仅上调angpt1和vegf的表达,而且上调其他血管生成基因的表达,诸如tgfα[其刺激Es的趋化反应、增殖和Vegf表达(39-41)]、成纤维细胞生长因子(fgf)2[其诱导EC增殖、迁移和Vegf分泌(32,42)]、血小板衍生的生长因子(pdgf)α[其将结缔组织细胞(诸如成纤维细胞和肥大细胞)护送到伤口区域中以产生血管生成因子,并增强Vegf和Fgf2的血管生成作用(43-46)]以及集落刺激因子(csf)3[其招募单核细胞以触发血管生成细胞因子的合成(33)](图5)。另一方面,FMOD在伤口闭合后降低抑制血管生成的基因的水平,包括干扰素(ifn)γ[其抑制EC生长和Vegf表达(47-49)并且阻断Fgf和Pdgf诱导的毛细血管生长(50)]、tgfβ1[其阻碍分化的EC出芽的激活,并且因此维持内皮静止(51)]以及纤溶酶原[plg;其抑制EC增殖(52)以及EC对Fgf和Vegf(53)的反应](图5)。因此,FMOD在成年啮齿动物伤口模型中保证了血管生成有利的基因表达网络。
FMOD在体外促进EC管状结构(TLS)形成
为了探索FMOD对EC出芽(spouting)(血管生成的初始步骤(21,54))的直接作用,将原代人脐静脉内皮细胞(HUVEC)接种在含有层粘连蛋白、胶原IV、巢蛋白和硫酸肝素蛋白聚糖的基质(Life Technologies,Grand Island,NY)中,以便模拟伤口愈合血管生成情况。HUVEC自发获得伸长的形态,并且在接种后3小时在凝胶中形成清晰可见的毛细血管网络(图6,上)。广泛范围的FMOD(10-250μg/ml)明显地增强了HUVEC TLS形成,并且随后建立了称为复杂网的多边形结构(图6,上)。定量分析证明,FMOD显著增加了HUVEC TLS的尺寸参数(每区域的细胞TLS网络的总长度)和拓扑参数(每区域的连接点、分支和网的个数)(图6,下)。与先前显示了EC迁移和多边形结构形成之间的正相关的研究一致(55),我们发现FMOD在体外通过基质显著刺激HUVEC侵袭(图7)。因此,FMOD通过促进EC迁移/侵袭至少部分地表现出其血管生成功能。

讨论
血管生成(通过原始血管网络的出芽、分裂和重塑由先前存在的血液脉管系统形成新血管的过程)由作用于由不同组的生长因子和ECM分子调节的EC的多种信号所引起(32,44,54)。到目前为止,大多数关于血管生成的研究集中于可溶性因子,诸如Vegf和Fgf2(30-33,42)。然而,不断增加的报道显示,细胞-ECM相互作用对于EC生长、分化、凋亡和对可溶性生长因子的反应也是关键性的(10,56,57)。例如,EC-ECM相互作用的阻断抑制了体内的新血管化和体外的TLS形成(58-60)。这些发现指示成功的血管生成需要EC、血管生成因子和周围ECM分子(诸如SLRP)之间的动态时间和空间调节的相互作用(21,22,32)。
SLRP为存在于所有组织的ECM内的蛋白质家族,包括饰胶蛋白聚糖、光蛋白聚糖和FMOD(4-10)。由于最近的研究已经示出,SLRP与多种细胞表面受体、细胞因子、生长因子和其他ECM组分相互作用,这导致细胞-ECM串扰和多种生物过程的调整(10-15),SLRP的常见功能远远超出其在ECM中的结构功能(10,15,61)。具体地,深入研究提出了饰胶蛋白聚糖在血管生成中的有争议的功能:饰胶蛋白聚糖在发育和正常伤口愈合期间为促进血管生成的,但是在肿瘤血管生成期间为抗血管生成的,这是因为饰胶蛋白聚糖干扰血小板反应蛋白-1、抑制内源性肿瘤Vegf产生并且诱发细胞外周纤维基质的稳定化的能力(10)。另外,Niewiarowska等揭示光蛋白聚糖通过降低EC的蛋白水解活性来抑制血管生成(62)。然而,不同于饰胶蛋白聚糖和光蛋白聚糖,我们当前的研究显示FMOD为血管生成ECM分子。尽管FMOD和光蛋白聚糖呈现密切的同源性并且在I型胶原上共享相同的结合区域(63-65),但它们对血管生成以及上皮迁移(3,16,66)的不同影响进一步支持了FMOD和光蛋白聚糖似乎没有功能冗余(特别在皮肤伤口愈合期间)的假设。
在本研究中,我们证明了FMOD不仅在发育期间明显地增强血管生成,如在卵内CAM测定中所记录的,而且还显著地刺激血管生成,如MatrigelTM栓塞测定以及成年啮齿动物皮肤伤口模型中的毛细血管密度测量所确认的。另外,fmod-/-小鼠中损害的伤口的血管生成可以通过外源性FMOD施用来恢复。在细胞水平上,我们证实FMOD在体外推进HUVEC迁移/侵袭和TLS形成。我们先前的研究还发现,FMOD在对EC增殖没有值得考虑的影响的情况下,在体外促进EC细胞粘附、扩散和肌动蛋白应力纤维形成以用于血管化(24)。因此,FMOD为直接调整EC行为的血管生成ECM分子。除EC之外,壁细胞(诸如成纤维细胞和周细胞)和炎性细胞(诸如单核细胞和肥大细胞)也有助于伤口血管生成(54,67)。通过刺激各种血管生成因子(包括angpt1、vegf、tgfα、fgf2、pdgfα和csf3)的表达,FMOD还在伤口愈合过程期间在体内激活这些血管生成相关的细胞。相比之下,Ifnγ和Plg为涉及伤口愈合的抑制血管生成的促炎症分子(47-50,52,53,68,69)。另外,Plg特别是在再上皮化中也起重要作用,因为在Plg缺乏的小鼠中伤口上的角质化细胞迁移被延迟(70)。
在本研究中,FMOD施用在成年啮齿动物伤口中降低了ifnγ和plg水平并且增加了血管生成,这与我们先前的观察,即fmod-/-小鼠的皮肤伤口表现出延长的炎症、升高的上皮迁移和不足的血管生成(3,16)高度相关。而且,调节伤口愈合的多能生长因子Tgfβ1,在发育的早期阶段以Vegf非依赖性方式促进内皮细胞分化,但在分化的EC中抑制出芽血管生成(51)。因此,伤口闭合后较低的tgfβ1转录也可有助于在FMOD治疗的伤口中增强血管生成。与先前的研究(24,71)一致,FMOD施用通过刺激血管生成因子并且降低抑制血管生成分子在体内诱导用于伤口愈合的促血管生成微环境。
总之,作为调查SLRP对伤口愈合的影响的先驱组之一,我们阐明了FMOD在受伤情况下的血管生成特性,其至少部分通过促进EC激活和伤口区域的浸润来发挥功能。虽然由于外部因素(诸如细菌抑制)的数量增加从临床前到临床环境的转译可能是很困难的,但总的来说,当前的研究表明,FMOD维持作为伤口管理的有吸引力的治疗候选者的潜力,特别是对于由于异常细胞浸润和血管生成不足而患有伤口愈合受损(诸如在糖尿病性伤口的情况下)的患者(72-74)。
