Abstract

本发明公开了一种JCI‑CCPC团队照护与病患信息管理系统，包括登录模块、患者纳入模块、病情记录模块、数据维护模块、日程安排模块、留言安排模块、状态转换模块；与现有技术相比，本发明为了方便医生对加入CCPC计划的患者进行统一管理和照顾，JCI‑CCPC团队照护与病患信息管理系统运用而生。根据临床医生的相关需求，并结合医院现有的信息系统进行开发，实现了CCPC与现有HIS系统、电子病历系统、检查与检验系统的对接，方便了临床医生对加入CCPC计划的患者管理。

技术领域
本发明涉及一种医疗管理系统，尤其涉及一种JCI-CCPC团队照护与病患信息管理系统。
背景技术
JCI是国际医疗卫生机构认证联合委员会(Joint Commission on Accreditationof Healthcare Organizations简称JCAHO)用于对美国以外的医疗机构进行认证的附属机构。JCI由医疗、护理、行政管理和公共政策等方面的国际专家组成，他们分别来自西欧、中东、拉丁美洲及中美洲、亚太地区、北美、中欧、东欧以及非洲。目前JCI已经给世界40多个国家的公立、私立医疗卫生机构和政府部门进行了指导和评审，13个国家(包括中国)的78个医疗机构通过了国际JCI认证。
CCPC，即临床服务项目认证(Clinical Care Program Certification)，是建立在国际临床实践指南基础上，以团队协作、科学管理和JCI安全文化为特征的单病种照护项目，具有国际最高水准。
其目的是提供具有实证医学、高品质的照护，其宗旨是以患者为中心，以最新的国际指南为标准，保证纳入患者人人能得到连续、平等、同质化照护。
其治疗特点为医疗机构很清楚的透过跨专业照护团队，依照先制定的照护计划书及照护指引，结合完整的跨专业团队照护团队成员(营养师、康复师、药师、心理治疗师、健康教育师等)，以提供一致的医疗服务；并定期监测疾病照护指标，有效管理疾病之照护品质。目前国内还没有关于CCPC的相关系统。
发明内容
本发明的目的就在于为了解决上述问题而提供一种JCI-CCPC团队照护与病患信息管理系统。
本发明通过以下技术方案来实现上述目的：
本发明包括登录模块、患者纳入模块、病情记录模块、数据维护模块、日程安排模块、留言安排模块、状态转换模块；
所述登录模块采用HIS系统登录接口；
所述患者纳入模块：对从HIS或电子病历系统初筛过来的患者，按照CCPC纳入标准进行筛选；
所述病情记录模块：各师对患者进行相关的病情情况、个案管理、医师管理、营养管理、药师管理、康复管理、心理管理、健教管理、知情同意书、品质指标、病历质量管理；
所述数据维护模块：CCPC小组成员管理、ICD编码初筛字典、宣教手册文件管理、病程记录模板管理；
所述日程安排模块：CKD日程安排、AMI日程安排、卒中日程安排；
所述留言安排模块：CKD留言安排；
状态转换：根据患者病情及各收案、终止、结案、删除状态条件，对患者状态进行调整。
本发明的有益效果在于：
本发明是一种JCI-CCPC团队照护与病患信息管理系统，与现有技术相比，本发明为了方便医生对加入CCPC计划的患者进行统一管理和照顾，JCI-CCPC团队照护与病患信息管理系统运用而生。根据临床医生的相关需求，并结合医院现有的信息系统进行开发，实现了CCPC与现有HIS系统、电子病历系统、检查与检验系统的对接，方便了临床医生对加入CCPC计划的患者管理。

具体实施方式
下面结合附图对本发明作进一步说明：
如图1所示：本发明包括登录模块、患者纳入模块、病情记录模块、数据维护模块、日程安排模块、留言安排模块、状态转换模块；
所述登录模块：采用HIS系统登录接口；以免账号过多给医生工作带来不便。医生或护士使用HIS系统和密码即可登录CCPC个案管理系统。
所述患者纳入模块：对从HIS或电子病历系统初筛过来的患者，按照CCPC纳入标准进行筛选。根据医生在电子病历系统、医生工作站系统中对患者所作诊断来判断，若该诊断符合“ICD初筛字典”中的诊断，则纳入到CCPC初筛列表中；其中“ICD初筛标准”，个案管理师或组长可以自行维护。为防止按“ICD初筛字典”筛选进来的患者遗漏，或者因特殊情况需要纳入CCPC的患者，本系统可以通过“手工纳入”方式进行纳入，若患者满足CCPC纳入标准，并且患者同意加入该计划，则点击“收案”按钮；收案的同时，系统会向相关CCPC小组推送该患者纳入CCPC计划的相关信息，并提醒相关医护人员关注。
所述病情记录模块：各师对患者进行相关的病情情况、个案管理、医师管理、营养管理、药师管理、康复管理、心理管理、健教管理、知情同意书、品质指标、病历质量管理；
所述数据维护模块：CCPC小组成员管理、ICD编码初筛字典、宣教手册文件管理、病程记录模板管理；
所述日程安排模块：CKD日程安排、AMI日程安排、卒中日程安排；
所述留言安排模块：CKD留言安排；
状态转换：根据患者病情及各收案、终止、结案、删除状态条件，对患者状态进行调整。

1.一种JCI-CCPC团队照护与病患信息管理系统，其特征在于：包括登录模块、患者纳入模块、病情记录模块、数据维护模块、日程安排模块、留言安排模块、状态转换模块；
所述登录模块采用HIS系统登录接口；
所述患者纳入模块：对从HIS或电子病历系统初筛过来的患者，按照CCPC纳入标准进行筛选；
所述病情记录模块：各师对患者进行相关的病情情况、个案管理、医师管理、营养管理、药师管理、康复管理、心理管理、健教管理、知情同意书、品质指标、病历质量管理；
所述数据维护模块：CCPC小组成员管理、ICD编码初筛字典、宣教手册文件管理、病程记录模板管理；
所述日程安排模块：CKD日程安排、AMI日程安排、卒中日程安排；
所述留言安排模块：CKD留言安排；
状态转换：根据患者病情及各收案、终止、结案、删除状态条件，对患者状态进行调整。
