超声刀以其切割精确、凝血可控、无电流通过及侧热损伤小等优势,在外科手术中正日益推广使用,大有取代电刀等手术器械的趋势。超声刀通常包括刀头部件(1)和主机(2),其中,刀头部件主要包括执行超声振动以进行切割等手术操作的端部执行器(21)、位于刀头主体(22)内部的超声换能器以及位于刀头主体(22)下部的设置有开关装置的手持柄(23)。主机(2)中的超声能量发生器输出电能通过线缆(3)输送至超声换能器,通过其中的压电陶瓷转换成机械能,驱动端部执行器(21)高频(通常为55000Hz)震荡,从而进行手术。为了增加控制维度,还可以通过设置脚踏开关或其他调节部件,通过线缆连接于刀头主体(22)和/或主机(2),方便多种运行项目的开关控制。超声刀已经能成熟地市场化应用,市售的超声刀包括强生公司的 超声刀等。
由于超声刀的振动幅度很小,通常为50~100μm,所以其端部执行器(21)表面通常经过防粘处理,使用包括聚四氟乙烯(如,特氟龙)和其他摩擦系数小的复合陶瓷材料等作为表面(如可参见中国专利申请第200880118493号),尤其是市售的 超声刀也是以其防粘的性能而著名,难以使用水溶液涂布上水溶性蛋白质或活性多肽等生物活性物质,而使用与这类表面有高附着性的专用溶剂进行涂布,则会产生安全性问题,所以至今未见有相应涂层技术的报道。
本发明人经过长期研究和实践,不但从用于涂布的水溶液入手,还从生物活性物质入手,在保持生物活性物质原有生物活性的基础上,可以将其高效率地涂布于超声刀的端部执行器表面,并可在生理条件下释放,而且该涂层是水溶性的,能方便地通过常规超声刀清洗手段清除,其中,不含生物活性物质的预涂布液是可以单独保存的,为进一步改进超声刀提供了基础。另外,本发明人根据医务工作者的反馈,对手持柄(23)上的开关装置进行了一定的改进,使其能低成本地适应不同握力的使用者。
发明内容
本发明要解决的技术问题在于提供新的超声刀,其相对于现有技术有改进,其主要改进在于超声刀的端部执行器表面设置诸如促创伤愈合的水溶性涂层,并可以改进手持柄上的开关装置。本发明还提供了该超声刀的制造及中间试剂,包括促创伤愈合的涂布液及其关键的生物活性物质和不含生物活性物质的预涂布液等。
具体而言,在第一个方面,本发明提供了促创伤愈合并能吸附于超声刀刀头部件的端部执行器表面的碱性成纤维细胞生长因子,其氨基酸序列
(1)如SEQ ID NO:1所示;或
(2)是在SEQ ID NO:1所示序列上添加、缺失和/或取代一个或几个氨基酸残基而得的,而且其具有促创伤愈合和吸附于超声刀刀头部件的端部执行器表面的功能。
本发明的第一个方面的碱性成纤维细胞生长因子具有促创伤愈合的功能,即保留了碱性成纤维细胞生长因子本身促细胞增殖的功能。本发明的第一个方面的碱性成纤维细胞生长因子还具有吸附于超声刀刀头部件的端部执行器表面的功能,其能在碳酸钙和甘露糖的帮助下,吸附在端部执行器表面并遇水释放,从而促进细胞增殖而促创伤愈合。
野生型的碱性成纤维细胞生长因子不稳定,尤其是在碱性环境中,而本发明克服了这样的缺陷。本发明的第一个方面的碱性成纤维细胞生长因子的氨基酸序列可以是在SEQ ID NO:1所示序列上添加、缺失和/或取代一个或几个(优选一个至五个,更优选一个至三个)氨基酸残基而得的氨基酸序列。本领域技术人员知晓,通过改变已知多肽的编码基因序列并将其导入表达载体,可以制备出取代、添加或缺失了氨基酸残基的多肽,这些方法广泛记载于《分子克隆实验指南》(北京:科学出版社,2002年)等本领域公知的文献中。在本发明的具体实施方式中,本发明的第一个方面的碱性成纤维细胞生长因子的氨基酸序列如SEQ ID NO:1所示。
在第二个方面,本发明提供了多核苷酸,其编码本发明的第一个方面的碱性成纤维细胞生长因子。本发明的多核苷酸,可以是DNA形式,也可以是RNA形式,优选DNA形式。DNA形式包括天然cDNA和人工合成的cDNA,DNA可以是编码链或模板链。通过常规技术,如PCR方法、重组法或人工合成的方法,本领域技术人员可以获得编码本发明的第一个方面的多肽的核酸分子或其片段。这些序列一旦获得,就可以将其克隆入载体,再转化或转染入相应的细胞,然后通过培养宿主细胞进行增殖。优选本发明的多核苷酸的核苷酸序列如SEQ IDNO:2所示。该优选序列没有受常规表达优化密码子的限制,优化了在大肠杆菌中的表达,表达效果有更大幅提升。
在第三个方面,本发明提供了一种载体,其含有本发明第二个方面所述的多核苷酸。本文中的载体包括表达载体,例如,细菌质粒、粘粒、噬菌粒、酵母质粒、植物细胞病毒、动物病毒及其它各种病毒载体。本发明的载体优选为原核载体,更优选是大肠杆菌表达载体。
在第四个方面,本发明提供了细胞,其含有本发明第二个方面所述的多核苷酸。该细胞可以用本发明第三个方面所述的载体转化或转染而获得。细胞可以是原核细胞,也可以是真核细胞,如,细菌细胞、酵母细胞、植物细胞、昆虫细胞、哺乳动物细胞等。优选本发明的细胞是大肠杆菌细胞。
在第五个方面,本发明提供了制备本发明的第一个方面的碱性成纤维细胞生长因子的方法,其包括在适合蛋白质表达的条件下培养本发明第四个方面所述的细胞,然后从培养物中分离出本发明的第一个方面的多肽。分离的方法包括但不限于:裂菌(超声波裂菌、渗透压裂菌),离心,盐析,分子筛色谱(又称为分子尺寸排阻色谱),离子交换色谱,吸附色谱(亲和层析、金属鏊合层析),反向色谱,高效液相色谱,毛细管电泳,等电聚焦以及变性/复性处理等。
在第六个方面,本发明提供了促创伤愈合的超声刀涂布液,其是包含本发明的第一个方面的碱性成纤维细胞生长因子和甘露糖的饱和碳酸钙水溶液,优选其是仅包含本发明的第一个方面的碱性成纤维细胞生长因子和甘露糖的饱和碳酸钙水溶液。本发明的第六个方面的超声刀涂布液能帮助本发明的第一个方面的碱性成纤维细胞生长因子吸附于超声刀刀头部件的端部执行器表面。
碳酸钙的溶解度在0℃至室温都很小,但是使得水溶液呈一定的弱碱性,因而野生型成纤维细胞生长因子在其中都很不稳定。微量的碳酸钙沉积在端部执行器表面,从而帮助本发明的第一个方面的碱性成纤维细胞生长因子吸附上去。优选在本发明的第六个方面的超声刀涂布液中,碱性成纤维细胞生长因子的浓度为10~1000μg/mL,优选为30~300μg/mL,更优选为50~200μg/mL,在本发明的具体实施方式中,碱性成纤维细胞生长因子的浓度为100μg/mL。
甘露糖对蛋白质有一定保护作用,但是其单独对野生型成纤维细胞生长因子和本发明的成纤维细胞生长因子的活性保持都不明显,主要用于稳定碳酸钙溶液本身。优选在本发明的第六个方面的超声刀涂布液中,甘露糖的浓度为1~10%(w/w),优选为3~8%(w/w),更优选为4~6%(w/w),在本发明的具体实施方式中,甘露糖的浓度为约5%(w/w)。
相应地,在第六个方面的另一个方面,本发明提供超声刀预涂布液,其是包含甘露糖的饱和碳酸钙溶液。优选其中,甘露糖的浓度为1~10%(w/w),优选为5%(w/w)。
本发明的超声刀预涂布液在常温下可以保存至少48小时。另外,本发明的超声刀预涂布液可以用于装载除了本发明的第一个方面的碱性成纤维细胞生长因子之外的其他物质,为继续改进超声刀系统而奠定基础。
在第七个方面,本发明提供了本发明的第六个方面的超声刀涂布液的制备方法,其包括:
(1)配制碳酸钙悬浮液;
(2)步骤(1)得到的悬浮液过滤后加入甘露糖溶解;
(3)将步骤(2)得到的溶液通过0.22μm滤膜过滤;
(4)任选将步骤(3)得到的滤液进行保存;和,
(5)用步骤(3)得到的滤液溶解本发明的第一个方面的碱性成纤维细胞生长因子。
由此,制备得到本发明的第六个方面的超声刀涂布液。
本发明第七个方面的制备方法可以在常温下进行,如在15~30℃下进行,优选在室温下(25℃)进行。制备得到的超声刀涂布液应当尽快使用,如果不使用,可以选择进行步骤(4)的保存,在常温下可以保存至少48小时。
因此,在第七个方面的另一个方面,本发明提供了本发明的第六个方面的超声刀预涂布液的制备方法,其包括:
(1)配制碳酸钙悬浮液;
(2)步骤(1)得到的悬浮液过滤后加入甘露糖溶解;
(3)将步骤(2)得到的溶液通过0.22μm滤膜过滤;和,
(4)任选将步骤(3)得到的滤液进行保存。
上述方法的步骤(3)制备得到本发明的第六个方面的超声刀预涂布液,其可以用于装载除了本发明的第一个方面的碱性成纤维细胞生长因子之外的其他物质,继续改进超声刀。
在第八个方面,本发明提供了超声刀系统,其包括刀头部件和主机以及连接两者的线缆,其中,刀头部件包括位于刀头主体前部的端部执行器、刀头主体和位于刀头主体下部的手持柄,刀头主体内设有超声换能器,手持柄上设有开关装置,其特征在于,端部执行器表面设置有促创伤愈合的水溶性涂层。
优选在本发明第八个方面的超声刀系统中,水溶性涂层是用本发明的第六个方面的超声刀涂布液涂布的。涂布的方法包括将超声刀刀头部件的端部执行器浸入权利要求3所述的超声刀涂布液中,静止后干燥。因此在第九个方面,本发明提供了该超声刀系统的涂布方法,其包括将超声刀刀头部件的端部执行器浸入权利要求3所述的超声刀涂布液中,静止后干燥。
也优选在本发明第八个方面的超声刀系统中,开关装置包括按片以及连接该按片和手持柄的弹簧,该弹簧位于手持柄外。由于在手持柄外,该弹簧可以拆卸和安装,因而十分便于更换其他与该弹簧大小相同但弹性系数不同的一个或多个分离放置的弹簧更换件。所以更优选在本发明第八个方面的超声刀系统中,还包括与该弹簧大小相同但弹性系数不同的一个或多个分离放置的弹簧更换件。这(些)弹簧更换件可以单独装在一个容器内,配套在本发明第八个方面的超声刀系统中。
本发明的有益效果在于,开创了超声刀刀头部件的端部执行器涂布生物活性物质的新思路,不局限在于改进涂布液的辅料配方,而是直接对生物活性物质进行了研究和改进,使得端部执行器能够覆盖生物活性物质层并在使用时释放,该涂层可经清洗除去,不会对超声刀的端部执行器表面造成永久影响,其中,不含生物活性物质的预涂布液可以单独保存,并为进一步改进超声刀提供了基础;另外,本发明也改进了超声刀手持柄的开关装置,适合不同医务工作者的使用。
为了便于理解,以下将通过具体的附图和实施例对本发明进行详细地描述。需要特别指出的是,这些描述仅仅是示例性的描述,并不构成对本发明范围的限制。依据本说明书的论述,本发明的许多变化、改变对所属领域技术人员来说都是显而易见的。
另外,本发明引用了公开文献,这些文献是为了更清楚地描述本发明,它们的全文内容均纳入本文进行参考,就好像它们的全文已经在本文中重复叙述过一样。

具体实施方式
以下通过实施例进一步示例性地说明本发明。如未特别指明,实施例中所用的技术手段为本领域技术人员所知晓的,包括可以商业化委托合成,所用的器械和试剂也均可通过商业渠道购买。
实施例1改进的bFGF变体及其制备
根据我们研究的氨基酸侧链基团吸附特性以及碱性成纤维细胞生长因子的活性位点,设计了氨基酸序列如SEQ ID NO:1所示的bFGF变体及其在大肠杆菌中优化表达的多核苷酸序列(参见SEQ ID NO:2),委托生工生物工程(上海)股份有限公司合成并克隆入pET28a+质粒,经测序正确后转化入大肠杆菌BL21(DE3)细胞,得到原核表达菌株。同样委托合成了表达野生型bFGF(氨基酸序列参见GenBank登录号AAA52533.1)的原核表达菌株作为阳性对照。
将委托构建的原核表达菌株涂布在LB平板上于37℃复壮过夜,挑取单菌落接种于150mL LB液体培养基中于37℃振荡培养6小时,然后加入15uL的1mol/L IPTG进行诱导,于37℃继续培养4小时。1500rpm离心收集菌体,重悬于10mL 50mM Tris-HCl(pH 7.0)中,置冰浴上超声破碎,然后于4℃10000rpm离心20分钟,收集的沉淀依次用30mL含2M尿素的50mMTris-HCl(pH 7.0)洗涤,洗涤后于10000rpm离心15分钟,收集沉淀。将洗涤好的沉淀完全溶解于30mL含8M尿素的50mM Tris-HCl(pH 7.0)中,1小时后向该包涵体溶解液缓慢加入含含0.1mMβ-巯基乙醇的50mM Tris-HCl(pH 7.0)300mL,然后室温静置过夜。将复性液上样于平衡好的Superdex-200柱含0~0.5M NaCl的50mM Tris-HCl(pH 7.0)洗脱,收集各洗脱峰,进行SDS-PAGE检测,收集显示18kD的洗脱液,然后将收集的洗脱液过Sephedex G-25柱脱去洗脱液中的盐离子,保留280nm紫外监测最大的洗脱峰,冷冻干燥备用,由此制备本发明的bFGF变体及作为阳性对照的野生型bFGF。
通过MTT法检测本发明的bFGF变体的促增殖活性。用含1%小牛血清的DMEM培养基分别溶解上述bFGF变体和野生型bFGF,浓度为500ng/ml,然后进行10倍梯度稀释,备用;用含10%小牛血清的DMEM培养基将3T3细胞稀释至5*104个/mL的浓度,加入96孔板上,每孔100μL,培养24小时后,吸除每个孔中的液体,并加入含1%小牛血清的DMEM培养基,继续培养24小时,然后分别加入等体积的不同浓度的bFGF变体和野生型bFGF稀释液(阴性对照加入含1%小牛血清的DMEM培养基),继续培养48小时,以阴性对照的光吸收度值为零计算各bFGF的促3T3细胞增殖的半有效浓度(ED50),其中,本发明的bFGF变体的ED50为0.3ng/mL,低于作为阳性对照的野生型bFGF的0.9ng/mL,但是两者活性基本保持在同一数量级。
实施例2改进的超声刀及其制造
如图1所示,超声刀系统包括刀头部件1和主机2以及连接两者的线缆3,其中,刀头部件1包括位于刀头主体22前部的端部执行器21、刀头主体22和位于刀头主体22下部的手持柄23,刀头主体22内设有超声换能器,手持柄23上设有开关装置,与现有技术(如,强生ACE+超声刀)所不同的是,即本发明的改进是:1,开关装置包括按片231以及连接该按片和手持柄23的弹簧232,该弹簧位于手持柄23外而方便拆装,由此能更换不同弹性系数的弹簧从而能适合不同医务工作者的手感力度,所以本发明的超声刀系统还包括与弹簧232大小相同但弹性系数不同的一个或多个弹簧更换件233;2,端部执行器21表面设置有促创伤愈合的水溶性涂层,该涂层包含实施例1的bFGF变体、碳酸钙和甘露糖。
该超声刀系统的改进点1的制造可以将原有超声刀刀头部件手持柄上的开关按键拆下,替换成按片231和外置的弹簧232,并添加一个容器,其中装有与弹簧232大小相同但弹性系数不同的一个或多个弹簧更换件233。
该超声刀系统的改进点2的制造步骤如下:取5克分析纯碳酸钙粉末于室温(25℃)悬浮于100mL蒸馏水中,搅拌30分钟,滤纸过滤,滤液中加入5克甘露糖,溶解后,过0.22μm滤膜过滤除菌,室温保存。涂布前,用该5%甘露糖的饱和碳酸钙溶液于室温溶解实施例1的bFGF变体,使其浓度为100μg/mL,然后取该溶液2mL置于试管中并将超声刀刀头部件1的端部执行器21浸入该液面下,并将试管通过0℃冰水浴降温,如此保持10分钟,取出超声刀刀头部件1,置于40℃温箱烘干,由此完成本发明的涂布。
另外,换用直接溶于蒸馏水的bFGF变体涂布作为对照1,换用溶于上述甘露糖的饱和碳酸钙溶液的野生型bFGF涂布作为对照2。
实施例3改进的超声刀的bFGF释放试验
将根据实施例2分别涂布的超声刀刀头部件1的端部执行器21浸入装有2mL生理盐水的试管中,启动超声波3秒,然后静置5秒,取出超声刀后通过MTT法(可参见实施例1)检测各试管中的洗脱液的促增殖活性,结果如表1所示。

结果表明,不使用本发明的bFGF变体,或者不使用甘露糖的饱和碳酸钙溶液,都几乎无法使得有活性的bFGF被涂布到端部执行器21上并被释放,其中用含野生型bFGF的甘露糖的饱和碳酸钙溶液涂布的效果差预计主要是由于野生型bFGF在此环境下不稳定造成的;而使用本发明的bFGF变体溶于甘露糖的饱和碳酸钙溶液进行涂布,能够涂布上并在使用超声刀时释放出较多活性的bFGF变体。考虑到本发明的bFGF变体本身活性较低,因此其释放量更大。
另外,用常规的超声刀清洗方法清洗本发明涂布的超声刀刀头部件1(即,先用水冲洗,再浸入多酶洗剂5min,再用高压水枪冲洗5次,然后用蒸馏水冲洗3次,擦干水迹后干燥)后,没有本发明的bFGF变体被检出,表明该涂层能方便地去除。
Claims (7)
1.超声刀系统,其包括刀头部件和主机以及连接两者的线缆,其中,刀头部件包括位于刀头主体前部的端部执行器、刀头主体和位于刀头主体下部的手持柄,刀头主体内设有超声换能器,手持柄上设有开关装置,其特征在于,端部执行器表面设置有促创伤愈合的水溶性涂层,其中,所述水溶性涂层是用促创伤愈合的超声刀涂布液涂布的,所述超声刀涂布液是包含氨基酸序列如SEQ ID NO:1所示的碱性成纤维细胞生长因子和甘露糖的饱和碳酸钙溶液。
2.权利要求1所述的超声刀系统,其中,开关装置包括按片以及连接该按片和手持柄的弹簧,该弹簧位于手持柄外。
3.权利要求2所述的超声刀系统,其中还包括与该弹簧大小相同但弹性系数不同的一个或多个分离放置的弹簧更换件。
4.权利要求1所述的超声刀系统,其中,甘露糖的浓度为1~10%(w/w)。
5.权利要求1所述的超声刀系统,其中,甘露糖的浓度为5%(w/w)。
6.权利要求1所述的超声刀系统,其中,成纤维细胞生长因子的浓度为10~1000μg/mL。
7.权利要求1所述的超声刀系统,其中,成纤维细胞生长因子的浓度为100μg/mL。
