背景技术
呼吸系统疾病是一种常见病、多发病，主要病变在气管、支气管、肺部及胸腔，病变轻者多咳嗽、胸痛、呼吸受影响，重者呼吸困难、缺氧，甚至呼吸衰竭而致死。在城市的死亡率占第3位，而在农村则占首位。更应重视的是，由于大气污染、吸烟、人口老龄化及其他因素，使国内外的慢性阻塞性肺病(简称慢阻肺，包括慢性支气管炎、肺气肿、肺心病)、支气管哮喘、肺癌、肺部弥散性间质纤维化，以及肺部感染等疾病的发病率、死亡率有增无减。
根据2006年全国部分城市及农村前十位主要疾病死亡原因的统计数，呼吸系统疾病(不包括肺癌)在城市的死亡病因中占第四位(13.1％)，在农村占第三位(16.4％)。由于大气污染、吸烟、工业经济发展导致的理化因子、生物因子吸人以及人口年龄老化等因素，使近年来呼吸系统疾病如肺癌、支气管哮喘的发病率明显增加，慢性阻塞性肺疾病居高不下(40岁以上人群中超过8％)。肺结核发病率虽有所控制，但近年又有增高趋势。肺血栓栓塞症已经构成了重要的医疗保健问题，肺动脉高压近年来也日益受到关注。肺部弥漫性间质纤维化及免疫低下性肺部感染等疾病发病率日渐增多。艾滋病的主要死亡原因为肺部感染，特别是卡氏肺囊虫肺炎。从2002年底以来，在我国及世界范围内暴发的传染性非典型肺炎(严重急性呼吸综合征，SARS)疫情，由于多发生于中青年，其传染性强，病死率高，又缺乏针对性的药物，因而引起了群众的恐慌，同时给国民经济造成巨大损失。目前在多个国家出现的人禽流感病死率超过60％。而禽流感病毒侵人体内主要的靶器官也是肺。这正说明呼吸系统疾病对我国人民健康危害仍是很大的，其防治任务艰巨。
盐酸氨溴索(Ambroxol Hydrochloride)又称盐酸溴环己胺醇，化学名为反式-4-[(2-氨基3，5-二溴苄基)氨基]环已醇盐酸盐，它是祛痰药溴己新的活性代谢物(N-去甲基，环己基对位引入反式羟基)，毒性低于溴己新，而活性高于溴己新。盐酸氨溴索是由德国勃林格殷格翰公司研发的黏液溶解剂，该药于20世纪80年代初首先在德国上市，随后在法国、意大利、日本、西班牙等许多国家相继上市，是新一代黏痰溶解剂，可以改善排痰，并具有促进肺表面活性物质和气道分泌及纤毛运动的作用。盐酸氨溴索在临床上可以调节黏液与黏浆分泌，活化纤毛摆动，易于稀释痰液，强化黏液向外运输，易于排出，它也可以促进肺表面活性物质合成，以维持肺泡张力，保证肺部功能指标；促进抗生素向组织渗透，以提高浓度，增强杀菌作用；抗氧化，减少炎性介质释放，以减轻炎症反应；与支气管解痉物质协同，以提高解痉药物的疗效。因此，该药在临床上可以广泛用于伴有呼吸道异常分泌的急慢性呼吸道疾患，特别是慢性支气管炎的祛痰治疗，新生儿呼吸窘迫症以及肺部手术的辅助治疗，具有毒性低、疗效确切、并能与抗生素并用产生良好的协同效果等优点，为最常用的祛痰药之一。近几年在我国主要城市的重点医院用药排名中，它一直居前。
硫酸沙丁胺醇(Salbutamol Sulfate)又名舒喘灵，其主要成分是沙丁胺醇，化学名为1-(4-羟基-3-羟甲基苯基)-2-(叔丁胺基)乙醇，是一种高选择性的激动支气管平滑肌的β-肾上腺受体兴奋剂，使支气管平滑肌松弛，从而解除支气管平滑肌痉挛。对支气管扩张作用较强，而对心脏的β1-受体作用较弱，是目前较安全、最常用的平喘药。适用于防治支气管哮喘，喘息性支气管炎与肺气肿病人的支气管痉挛，缓解因支气管哮喘、慢性支气管炎和肺气肿等气道阻塞性疾病引起的呼吸困难等症状，其优点是起效快，可迅速改善病人症状，解痉、平喘、祛痰，缺点是作用不持久，仅仅起到缓解病人哮喘症状的作用；较长时间应用可导致β-受体向下调节，使患者出现对β受体激动剂失敏，甚至对哮喘治疗无效的耐药现象。
现有技术中已经存在含有盐酸氨溴索和硫酸沙丁胺醇的溶液剂和颗粒剂等剂型，但是未见散剂的报道。
此外，硫酸沙丁胺醇和盐酸氨溴索为活性成分在同一个最小制剂单位中，长期保存时，由于各成分间可能发生化学配伍变化，容易产生副产物，因此会降低效果或增加副作用，产品稳定性不够理想。
发明内容
考虑到盐酸氨溴索和硫酸沙丁胺醇的协同作用，本发明的目的即是提供一种安全有效，质量稳定，患者适应性强，效果显著，可有效治疗急慢性支气管炎和哮喘等呼吸系统疾病的氨溴索沙丁胺醇散剂。
在研究中，我们意外发现，在制剂的制备时适量地加入的泊洛沙姆188、葡甲胺和甘露醇，能够显著提高制剂中盐酸氨溴索和硫酸沙丁胺醇的稳定性和生物利用度，对于药品的临床使用具有重要意义。
本发明解决该技术问题的技术方案是：
一种氨溴索沙丁胺醇散剂，由下列重量的组分制成：
硫酸沙丁胺醇：8-10g；
盐酸氨溴索：15-20g；
泊洛沙姆188:8-10g；
葡甲胺：6-8g；
甘露醇：2-4g；
填充剂：500-600g；
润滑剂：适量。
优选处方为：
硫酸沙丁胺醇：10g；
盐酸氨溴索：20g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：4g；
填充剂：600g；
润滑剂：适量。
所述的填充剂选自淀粉、乳糖、糊精、可压性淀粉和糖粉中的一种或几种。
所述的润滑剂选自硬脂酸镁、滑石粉和微粉硅胶中的一种或几种。
其制备方法为：
1)称取处方量的原辅料，分别过80-120目筛备用；
2)将过筛后的硫酸沙丁胺醇、盐酸氨溴索、泊洛沙姆188、葡甲胺、甘露醇和填充剂按等量递增法搅拌混合均匀，加入润滑剂混匀后分装即得。
优选的制备方法为：
1)称取处方量的原辅料，分别过100目筛备用；
2)将过筛后的硫酸沙丁胺醇、盐酸氨溴索、泊洛沙姆188、葡甲胺、甘露醇和填充剂按等量递增法搅拌混合均匀，加入润滑剂混匀后分装即得。
本发明的有益效果是所述氨溴索沙丁胺醇散剂的活性成分含量显著增加；稳定性大幅提高；因此质量稳定，效果显著，可有效治疗急慢性支气管炎和哮喘等呼吸系统疾病。

下面结合具体实施例，进一步阐述本发明。应理解，这些实施例仅用于说明本发明而不用于限制本发明的范围。下列实施例中未注明具体条件的实验方法，通常按照常规条件或按照制造厂商所建议的条件。除非另外说明，否则所有的百分数、比率、比例或份数按重量计。
除非另行定义，文中所使用的所有专业与科学用语与本领域熟练人员所熟悉的意义相同。此外，任何与所记载内容相似或均等的方法及材料皆可应用于本发明方法中。文中所述的较佳实施方法与材料仅作示范之用。
实施例1
处方：
硫酸沙丁胺醇：10g；
盐酸氨溴索：20g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：4g；
乳糖：600g；
硬脂酸镁：适量。
其制备方法为：
1)称取处方量的原辅料，分别过100目筛备用；
2)将过筛后的硫酸沙丁胺醇、盐酸氨溴索、泊洛沙姆188、葡甲胺、甘露醇和乳糖按等量递增法搅拌混合均匀，加入硬脂酸镁混匀后分装即得。

2)将过筛后的硫酸沙丁胺醇、盐酸氨溴索、泊洛沙姆188、葡甲胺、甘露醇和淀粉按等量递增法搅拌混合均匀，加入硬脂酸镁混匀后分装即得。
实施例3
硫酸沙丁胺醇：8g；
盐酸氨溴索：16g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：2g；
糊精：500g；
微粉硅胶：适量。
其制备方法为：
1)称取处方量的原辅料，分别过100目筛备用；
2)将过筛后的硫酸沙丁胺醇、盐酸氨溴索、泊洛沙姆188、葡甲胺、甘露醇和糊精按等量递增法搅拌混合均匀，加入微粉硅胶混匀后分装即得。
实施例4稳定性试验
对比例1采用制备方法与实施例相同；处方如下：
硫酸沙丁胺醇：10g；
盐酸氨溴索：20g；
泊洛沙姆188:8g；
乳糖：600g；
硬脂酸镁：适量。
对比例2采用制备方法与实施例相同；处方如下：
硫酸沙丁胺醇：10g；
盐酸氨溴索：20g；
乳糖：600g；
硬脂酸镁：适量。
1.加速稳定性试验
将实施例1、对比例1和对比例2所得氨溴索沙丁胺醇散剂分别置光照强度4500lx条件下10天，分别于第0、5、10天取样，检测含量、有关物质等各项质量指标，结果见表1。

结果表明：本发明制备的氨溴索沙丁胺醇散剂在强光条件下各成分含量无明显变化，稳定性明显好于对比例。究其原因，是因为对比例中的普通辅料无法较好的对活性成分起到支持和稳定作用，进而导致各成分间可能发生化学配伍变化，容易产生副产物；并且由于活性成分的含量大于一般现有技术水平，导致稳定性更大大降低。
2.加速试验
将实施例1所得氨溴索沙丁胺醇散剂置于40℃、RH20％的恒温、恒湿箱中6个月，分别于第0、1、2、3、6个月取样，测定含量、有关物质等各项质量指标，结果见表2。

结果表明：氨溴索沙丁胺醇散剂在40℃、RH20％的条件下放置6个月，与0个月比较，除有关物质略有增加外，其他各项质量指标均无明显变化，质量稳定可靠，符合规定。

硫酸沙丁胺醇：10g；
盐酸氨溴索：20g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：4g；
乳糖：600g；
硬脂酸镁：适量。
2.一种氨溴索沙丁胺醇散剂，由下列重量的组分制成：
硫酸沙丁胺醇：8g；
盐酸氨溴索：15g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：4g；
淀粉：500g；
硬脂酸镁：适量。
3.一种氨溴索沙丁胺醇散剂，由下列重量的组分制成：
硫酸沙丁胺醇：8g；
盐酸氨溴索：16g；
泊洛沙姆188:8g；
葡甲胺：8g；
甘露醇：2g；
糊精：500g；
微粉硅胶：适量。
