本发明公开了一种治疗慢性肾病的药物组合物,其特征在于,它是由下列重量份数配比的原料制备而成:余甘子、素馨花、凤尾草、黄蜀葵花、藁本、猪苓、黄蜀葵花、扛板归、红背菜、八月札、甲珠、淫羊藿、紫河车、金毛木通、莱菔子、黄芪、佩兰、车前子、假酸浆子、七叶一枝花、白花蛇舌草、爵床、桑寄生、补骨脂、官桂、皂矾、积血草、六月雪、胆南星、虎杖、败酱草、菝葜;双氯芬酸钠、右旋糖酐、酪蛋白磷酸肽、聚维酮、薄荷脑、维生素D、泛酸、枸橼酸、维生素E、维生素K、纤维素。该复合制剂具有清热解毒、利尿消肿、温肾渗湿之功效。

技术领域
本发明涉及医学药物领域,具体是涉及一种治疗慢性肾病的药物组合物及制备方法。
背景技术
慢性肾病,深深地困扰着患者。慢性肾病发病特点有“三高”、“三低”:发病率高、伴发的心血管病患病率高、病死率高;全社会对慢性肾病的知晓率低、防治率低、伴发心血关病的知晓率低。调查显示,我国40岁以上人群慢性肾病的患病率大于10%,知晓率却不足5%。在我国,发生于肾小球肾炎的慢性肾病患者最多,占40%。随我国人群饮食结构和生活习惯的改变,其他的继发性慢性肾病正不断增多。加上我国现有医疗水平尚有限,导致我国慢性肾病的防治工作形势面临严峻挑战。以本病属于中医学的“淋证”、“腰痛”的范畴;临床表现为发热,伴有头痛、全身不适、恶心呕吐等全身症状和尿频、尿急、尿痛。临床中,诊断为肾小球肾炎、隐匿性肾炎、肾盂肾炎、过敏性紫癜肾炎、红斑狼疮肾炎、痛风肾、肾病、肾病综合征、膜性肾病、肾病综合征、糖尿病肾病、高血压肾病、多囊肾,等等,当这些肾病的发病迁延难愈,时间超过三个月,病人尿液和相关的血液指标出现异常,肾脏病理学、影像学发现异常,或肾脏的肾小球有效滤过率低于60%,都可统称为“慢性肾病”。慢性肾病如未能及时有效救治,导致病情恶化进展,则随病程迁延,慢性肾病患者将发展成为慢性肾功能不全、肾衰竭,最终形成尿毒症。
慢性肾病发病特点有“三高”、“三低”:发病率高、伴发的心血管病患病率高、病死率高;全社会对慢性肾病的知晓率低、防治率低、伴发心血关病的知晓率低。调查显示,我国40岁以上人群慢性肾病的患病率大于10%,知晓率却不足5%。在我国,发生于肾小球肾炎的慢性肾病患者最多,占40%。随我国人群饮食结构和生活习惯的改变,其他的继发性慢性肾病正不断增多。加上我国现有医疗水平尚有限,导致我国慢性肾病的防治工作形势面临严峻挑战。首先说水肿,肾炎的水肿一般是眼睑水肿,以清晨起床后最为常见。但正常人在睡眠不足的情况下,清晨起床后也会出现眼睑水肿,起来活动后就会消退。这样的日常生活经验很容易使患者及其家属警惕性降低,笔者对此深有感触。以前我起床后有时也出现眼睑水肿,但一般都是在工作或学习紧张睡眠不足的情况下发生的,而在睡眠充足时,很少出现这种情况,所以也没有放在心上。这种情况在后来治疗的过程中询问许多病友,他们当中许多人也有相似的体会。另一个症状是高血压。的确,有一部分慢性肾病患者是以高血压为表现的,但由于发病年龄集中在青壮年,心脏和血管条件都很好,代偿能力很强,许多人都是在偶然间发现自己血压高的,而更多人特别是年轻人,没有头晕、心悸的感觉,是不会特意去量血压的,因此血压高了也不知道。
另外,还有一部分患者血压高的表现是在疾病的晚期由于肾脏功能失代偿才表现出来。对于另外两个体征血尿、蛋白尿来说,这些名词更容易让人误解,血尿是不是就是小便中带血呢?蛋白尿是不是就是小便中有蛋白呢?这样说对也不对。说对是因为实际上是这样的,说不对是现实中人们对于这两种异常状态的了解很少,一般人们以为血尿就是小便中可以看到血,而蛋白尿是什么状态就更模糊了。
目前对于慢性肾病临床治疗方法主要有:1、一般治疗要加强护理,注意保暖,戒除吸烟,减少和避免各种可能的致病诱因。2、肾上腺皮质激素和免疫抑制剂。3、血浆置换与免疫吸附疗法,血浆置换或免疫吸附可去除抗GBM抗体。以及一些其他的治疗手段,但是上述疗法副作用多,疗效欠满意,患者不易接受。中医内治法遵循整体观念,通过调节机体全身功能治疗局部疾病,且无副作用,服用方便,患者容易接受。但是疗程长,为了加强治疗作用,现在在中医的基础上结合西药,制成复合制剂,大大提高了中药的疗效,减小西药的副作用。该复合制剂具有治疗效果好,不易复发,价格低廉,无毒副作用的特点。

本发明解决的技术问题是提供一种治疗慢性肾病的药物组合物及制备方法。具体方案如下:
一种治疗慢性肾病的药物组合物,其特征在于:它是由下列重量份数配比的原料制备而成:30~50份余甘子、30~50份素馨花、30~50份凤尾草、20~30份黄蜀葵花、20~30份藁本、20~30份猪苓、5~10份黄蜀葵花、5~10份扛板归、5~10份红背菜、5~10份八月札、5~10份甲珠、3~5份淫羊藿、3~5份紫河车、3~5份金毛木通、3~5份莱菔子、3~5份黄芪、3~5份佩兰、3~5份车前子、3~5份假酸浆子、3~5份七叶一枝花、3~5份白花蛇舌草、3~5份爵床、3~5份桑寄生、3~5份补骨脂、3~5份官桂、3~5份皂矾、3~5份积血草、3~5份六月雪、3~5份胆南星、3~5份虎杖、3~5份败酱草、3~5份菝葜;1~3份双氯芬酸钠、0.5~1份右旋糖酐、0.1~0.2份酪蛋白磷酸肽、0.1~0.2份聚维酮、0.1~0.2份薄荷脑、0.1~0.2份维生素D、0.1~0.2份泛酸、0.1~0.2份枸橼酸、0.1~0.2份维生素E、0.1~0.2份维生素K、0.1~0.2份纤维素。
进一步地,所述的一种治疗慢性肾病的药物组合物,其特征在于还可以包括按重量份计的以下化学成分:0.2~0.3份超氧化物歧化酶(SOD)、0.1~0.2份葡萄糖淀粉酶、0.1~0.2份蚓激酶、0.1~0.2份古洛糖酸、0.1~0.2份支链淀粉糖。
优选地,所述的一种治疗慢性肾病的药物组合物:所述纤维素为大豆纤维素、玉米纤维素、抗性淀粉、爱尔兰苔菜胶、水溶性膳食纤维素中的一种。
进一步地,所述的一种治疗慢性肾病的药物组合物,所述中药成分中的黄蜀葵花采用黄蜀葵花单独提取,步骤如下:
(1)用80℃~100℃热水淋洗未粉碎的黄蜀葵花;
(2)将洗净的黄蜀葵花沥干或晒干后,破壁粉碎成黄蜀葵花粗粉;
(3)将黄蜀葵花粗粉用水冷浸12~24小时;
(4)酶解
1)黄蜀葵花碱调pH值至7-8,按每克黄蜀葵花生料加纤维素酶15~30U,在40~50℃的温度下酶解2小时,间歇搅拌、过滤、弃滤渣a,得滤液A;
2)黄蜀葵花滤渣a中加3~5重量份水,在75~85℃的温度下浸提1~2小时,过滤、弃滤渣b,得滤液B;
3)黄蜀葵花的滤渣b中加3~5重量份水,在90~100℃温度下浸提30~45分钟,过滤、弃滤渣c,得滤液C;
4)合并A、B、C三次的滤液,再过滤,收集微滤液;
(5)减压浓缩
将微滤液在蒸汽压力≤0.15Mpa、真空度≤700mmhg、温度80~100℃的条件下,采用真空减压浓缩成膏状,即得膏状黄蜀葵花浸提物。
一种治疗慢性肾病的药物组合物的制备方法,包括以下步骤:
(1)按所述配比称取:30~50份余甘子、30~50份素馨花、30~50份凤尾草、20~30份藁本、20~30份猪苓、5~10份黄蜀葵花、5~10份扛板归、5~10份红背菜、5~10份八月札、5~10份甲珠、3~5份淫羊藿、3~5份紫河车、3~5份金毛木通、3~5份莱菔子、3~5份黄芪、3~5份佩兰、3~5份车前子、3~5份假酸浆子、3~5份七叶一枝花、3~5份白花蛇舌草、3~5份爵床、3~5份桑寄生、3~5份补骨脂、3~5份官桂、3~5份皂矾、3~5份积血草、3~5份六月雪、3~5份胆南星、3~5份虎杖、3~5份败酱草、3~5份菝葜;置于盛有0.3~0.35%碳酸氢钠溶液的超声波清洗机中于100~180W、33~36KHz清洗15~30min,沥干,真空微波干燥,放入微粉机粉碎至80~100目,置于4~10倍量去离子水中浸泡2.5~10小时,水温度为50~55℃,冷却至常温,加混合溶剂A提取,所述混合溶剂A是正己烷、乙酸乙酯和丙酮的混合溶液,混合体积比为正己烷:乙酸乙酯:丙酮=(5~10):(2~6):(1~2);所得提取液中加入1~2%澄清剂,澄清处理后离心,离心后得到上清液,即得到中药成分的提取液;
(2)按所述配比分别称取1~3份双氯芬酸钠、0.5~1份右旋糖酐、0.1~0.2份酪蛋白磷酸肽、0.1~0.2份聚维酮、0.1~0.2份薄荷脑、0.1~0.2份维生素D、0.1~0.2份泛酸、0.1~0.2份枸橼酸、0.1~0.2份维生素E、0.1~0.2份维生素K、0.1~0.2份纤维素;用去离子水于25~28℃溶解,搅拌1~2小时,过滤,收集滤液;再在滤液中加入步骤(1)所得的中药成分的提取液,在70~80℃搅拌2~10小时,速度为100~150rpm/分,将所得溶液经离心脱气,于0~4℃低温放置12~24小时,分装,密封保存即得所述的一种治疗慢性肾病的药物组合物。
进一步地,所述的一种治疗慢性肾病的药物组合物的制备方法,所述澄清剂为:果胶酶、明胶、硅胶、单宁或膨润土其中的一种。
优选地,所述的一种治疗慢性肾炎的多酶中西药复合制剂的制备方法,所述澄清剂为ZTC1+1Ⅱ型的天然澄清剂。
一种治疗慢性肾病的药物组合物的制备方法,步骤如下:
(1)按所述配比称取:30~50份余甘子、30~50份素馨花、30~50份凤尾草、20~30份藁本、20~30份猪苓、5~10份扛板归、5~10份红背菜、5~10份八月札、5~10份甲珠、3~5份淫羊藿、3~5份紫河车、3~5份金毛木通、3~5份莱菔子、3~5份黄芪、3~5份佩兰、3~5份车前子、3~5份假酸浆子、3~5份七叶一枝花、3~5份白花蛇舌草、3~5份爵床、3~5份桑寄生、3~5份补骨脂、3~5份官桂、3~5份皂矾、3~5份积血草、3~5份六月雪、3~5份胆南星、3~5份虎杖、3~5份败酱草、3~5份菝葜;置于盛有0.3~0.35%碳酸氢钠溶液的超声波清洗机中于100~180W、33~36KHz清洗15~30min,沥干,真空微波干燥,放入微粉机粉碎至80~100目,置于4~10倍量去离子水中浸泡2.5~10小时,水温度为50~55℃,冷却至常温,加混合溶剂A提取,所述混合溶剂A是正己烷、乙酸乙酯和丙酮的混合溶液,混合体积比为正己烷:乙酸乙酯:丙酮=(5~10):(2~6):(1~2);所得提取液中加入1~2%澄清剂,澄清处理后离心,离心后得到上清液,即为上述中药成分的提取液;
(2)按重量份数比,称取5~10份黄蜀葵花、利用权利要求4的提取方法单独提取所述黄蜀葵花得黄蜀葵花提取液备用;
(3)按所述配比分别称取1~3份双氯芬酸钠、0.5~1份右旋糖酐、0.1~0.2份酪蛋白磷酸肽、0.1~0.2份聚维酮、0.1~0.2份薄荷脑、0.1~0.2份维生素D、0.1~0.2份泛酸、0.1~0.2份枸橼酸、0.1~0.2份维生素E、0.1~0.2份维生素K、0.1~0.2份纤维素;用去离子水于25~28℃溶解,搅拌1~2小时,过滤,收集滤液;再在滤液中加入步骤(1)所得的中药成分的提取液,再加入步骤(2)所得的黄蜀葵花提取液,在70~80℃搅拌2~10小时,速度为100~150rpm/分,将所得溶液经离心脱气,于0~4℃低温放置12~24小时,分装,密封保存,即得所述的一种治疗慢性肾病的药物组合物。
本发明的有益效果是,该复合制剂具有清热解毒、利尿消肿、温肾渗湿之功效,能有效治疗急性、慢性肾炎,疗程短,不易复发,不会产生耐药性,且无毒副作用,用于治疗肾炎,兼治秘尿道感染,效果好、效果快无副作用等,本发明组方严谨,配伍合理,西药成分能够弥补中药的不足,中药成分又能够中和解除西药成分的副作用,二者合用能够从根源上治疗该病,疗效确切,起效快,治愈率高。

临床案例第一批:
治疗方法:按上述实施例1的制备方法制备样品,口服,一次5克,一日三次,一个月为一个疗程,间隔1周再行第2个疗程。治疗2个疗程后统计疗效。
1.实验对象:选取某医院收治的慢性肾病患者200例,随机分成4组,每组50人,分别为组1、组2、组3、对照组。
2.实验方法:组1、组2、组3分别使用本发明实施例1、实施例2、实施例3的药剂进行治疗,对照组使用一般的用于肾炎的药剂治疗。
3.疗效标准:
痊愈:水肿等症状与体征完全消失,尿蛋白检查持续阴性,或24小时尿蛋白定量持续小于0.2克,高倍镜下尿红细胞消失,尿沉渣计数正常,肾功能正常。
显效:水肿等症状与体征基本消失,尿蛋白检查持续减少50%以上,高倍镜下尿红细胞不超过3个,尿沉渣计数接近正常。肾功能正常或基本正常(与正常值相差不超过15%)。
有效:水肿等症状与体征明显好转,尿蛋白持续减少1个(+)或24小时尿蛋白定量持续减少25%以上,高倍镜下尿红细胞不超过5个,肾功能正常或有改善。
无效:临床表现与上述实验室检查均无明显改善或者加重。

5.结论:由上述统计结果可见,本发明治疗慢性肾病的药剂有效率和治愈率均较高,与一般的肾病用药相比,效果更突出,达到了标本兼治的目的。
本发明效果显著,疗程短,不易复发,不会产生耐药性,且无毒副作用,用于疗肾炎,兼治秘尿道感染,效果好、效果快无副作用等。

病例1:崔某某,47岁,患有慢性肾病半年,尿液中出现大量泡沫以及尿中见血色或夜尿增,小便频数,淋涩不已,伴有呕恶纳呆,腹胀便搪,畏寒肢冷,面浮肢肿,腰酸膝软,全身疲惫;服用本发明的中药组合物,一次1-2包,一日2-3次,连服十余天后,症状明显减轻,继续服用30天,症状消失,半年后追访,无复发。
病例2:齐某某,38岁,患有慢性肾病3年,常伴有小便频数、尿中有血色、淋涩不已、高热、寒战,发热等症状,严重时伴有食欲不振、恶心、呕吐等症状,服用本发明的中药组合物,一次1-2包,一日2-3次,连服20天后,症状明显减轻,无恶心、呕吐等症状,继续服用两周,症状消失,治愈。
病例3:王某某,男,45岁,近15个月以来常感尿频、尿中有泡沫,尿急、尿痛,伴有寒战、发烧、腰痛、腹痛等症状,去医院检查确诊为肾盂肾炎,服用本发明的中药组合物,一次1-2包,一日2-3次,连服10天后,症状明显减轻,继续服用10天,症状消失,治愈,无复发。

(1)用80℃~100℃热水淋洗未粉碎的黄蜀葵花;
(2)将洗净的黄蜀葵花沥干或晒干后,破壁粉碎成黄蜀葵花粗粉;
(3)将黄蜀葵花粗粉用水冷浸12~24小时;
(4)酶解
1)黄蜀葵花碱调pH值至7-8,按每克黄蜀葵花生料加纤维素酶15~30U,在40~50℃的温度下酶解2小时,间歇搅拌、过滤、弃滤渣a,得滤液A;
2)黄蜀葵花滤渣a中加3~5重量份水,在75~85℃的温度下浸提1~2小时,过滤、弃滤渣b,得滤液B;
3)黄蜀葵花的滤渣b中加3~5重量份水,在90~100℃温度下浸提30~45分钟,过滤、弃滤渣c,得滤液C;
4)合并A、B、C三次的滤液,再过滤,收集微滤液;
(5)减压浓缩
将微滤液在蒸汽压力≤0.15Mpa、真空度≤700mmhg、温度80~100℃的条件下,采用真空减压浓缩成膏状,即得膏状黄蜀葵花浸提物。
5.如权利要求1-2所述的一种治疗慢性肾病的药物组合物的制备方法,其特征在于,包括以下步骤:
(1)按所述配比称取:30~50份余甘子、30~50份素馨花、30~50份凤尾草、20~30份藁本、20~30份猪苓、5~10份黄蜀葵花、5~10份扛板归、5~10份红背菜、5~10份八月札、5~10份甲珠、3~5份淫羊藿、3~5份紫河车、3~5份金毛木通、3~5份莱菔子、3~5份黄芪、3~5份佩兰、3~5份车前子、3~5份假酸浆子、3~5份七叶一枝花、3~5份白花蛇舌草、3~5份爵床、3~5份桑寄生、3~5份补骨脂、3~5份官桂、3~5份皂矾、3~5份积血草、3~5份六月雪、3~5份胆南星、3~5份虎杖、3~5份败酱草、3~5份菝葜;置于盛有0.3~0.35%碳酸氢钠溶液的超声波清洗机中于100~180W、33~36KHz清洗15~30min,沥干,真空微波干燥,放入微粉机粉碎至80~100目,置于4~10倍量去离子水中浸泡2.5~10小时,水温度为50~55℃,冷却至常温,加混合溶剂A提取,所述混合溶剂A是正己烷、乙酸乙酯和丙酮的混合溶液,混合体积比为正己烷:乙酸乙酯:丙酮=(5~10):(2~6):(1~2);所得提取液中加入1~2%澄清剂,澄清处理后离心,离心后得到上清液,即得到中药成分的提取液;
(2)按所述配比分别称取1~3份双氯芬酸钠、0.5~1份右旋糖酐、0.1~0.2份酪蛋白磷酸肽、0.1~0.2份聚维酮、0.1~0.2份薄荷脑、0.1~0.2份维生素D、0.1~0.2份泛酸、0.1~0.2份枸橼酸、0.1~0.2份维生素E、0.1~0.2份维生素K、0.1~0.2份纤维素;用去离子水于25~28℃溶解,搅拌1~2小时,过滤,收集滤液;再在滤液中加入步骤(1)所得的中药成分的提取液,在70~80℃搅拌2~10小时,速度为100~150rpm/分,将所得溶液经离心脱气,于0~4℃低温放置12~24小时,分装,密封保存即得所述的一种治疗慢性肾病的药物组合物。
6.如权利要求1所述的一种治疗慢性肾病的药物组合物的制备方法,其特征在于,所述澄清剂为:果胶酶、明胶、硅胶、单宁或膨润土其中的一种。
7.如权利要求1-4任意一项所述的一种治疗慢性肾病的药物组合物的制备方法,其特征在于:
(1)按所述配比称取:30~50份余甘子、30~50份素馨花、30~50份凤尾草、20~30份藁本、20~30份猪苓、5~10份扛板归、5~10份红背菜、5~10份八月札、5~10份甲珠、3~5份淫羊藿、3~5份紫河车、3~5份金毛木通、3~5份莱菔子、3~5份黄芪、3~5份佩兰、3~5份车前子、3~5份假酸浆子、3~5份七叶一枝花、3~5份白花蛇舌草、3~5份爵床、3~5份桑寄生、3~5份补骨脂、3~5份官桂、3~5份皂矾、3~5份积血草、3~5份六月雪、3~5份胆南星、3~5份虎杖、3~5份败酱草、3~5份菝葜;置于盛有0.3~0.35%碳酸氢钠溶液的超声波清洗机中于100~180W、33~36KHz清洗15~30min,沥干,真空微波干燥,放入微粉机粉碎至80~100目,置于4~10倍量去离子水中浸泡2.5~10小时,水温度为50~55℃,冷却至常温,加混合溶剂A提取,所述混合溶剂A是正己烷、乙酸乙酯和丙酮的混合溶液,混合体积比为正己烷:乙酸乙酯:丙酮=(5~10):(2~6):(1~2);所得提取液中加入1~2%澄清剂,澄清处理后离心,离心后得到上清液,即为上述中药成分的提取液;
(2)按重量份数比,称取5~10份黄蜀葵花、利用权利要求4的提取方法单独提取所述黄蜀葵花得黄蜀葵花提取液备用;
(3)按所述配比分别称取1~3份双氯芬酸钠、0.5~1份右旋糖酐、0.1~0.2份酪蛋白磷酸肽、0.1~0.2份聚维酮、0.1~0.2份薄荷脑、0.1~0.2份维生素D、0.1~0.2份泛酸、0.1~0.2份枸橼酸、0.1~0.2份维生素E、0.1~0.2份维生素K、0.1~0.2份纤维素;用去离子水于25~28℃溶解,搅拌1~2小时,过滤,收集滤液;再在滤液中加入步骤(1)所得的中药成分的提取液,再加入步骤(2)所得的黄蜀葵花提取液,在70~80℃搅拌2~10小时,速度为100~150rpm/分,将所得溶液经离心脱气,于0~4℃低温放置12~24小时,分装,密封保存,即得所述的一种治疗慢性肾病的药物组合物。
