Abstract

在各个方面,本发明提供了用于治疗或预防包括慢性感染在内的感染性疾病、以及对免疫系统或公共健康普遍呈现持续挑战的高度传染性的感染因子(infectious agent)的组合物和方法。本文中所描述的组合物和治疗方案可以与其他抗病毒或抗微生物疗法一起使用,以及与疫苗接种(例如,非含佐剂的疫苗接种)结合使用以增强疫苗效力和/或延长保护作用的持续时间。所述治疗包括对患者体内施用糖苷酶方案(例如,一种或多种糖苷酶的方案)。在各个实施方案中,所述糖苷酶方案并不被患者的免疫系统靶向。在各个实施方案中,所述糖苷酶方案提供了一种或多种对去除哺乳动物细胞(例如,免疫细胞)、受感染的细胞或其他导致免疫活化的糖基化靶标上的一个或多个末端糖基为活性的糖苷酶。所述糖苷酶疗法通过其作用于免疫细胞而引发免疫信号传导级联。靶向末端糖基可以包括,例如,唾液酸基(sialosyl)、β-半乳糖基、N-乙酰半乳糖氨基、岩藻糖基、葡糖基、N-乙酰葡糖氨基和甘露糖基残基等等。因此,在各个实施方案中,所述糖苷酶方案可以包括神经氨酸苷酶、半乳糖苷酶、N-乙酰半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶等中的一种或多种。不希望受理论的束缚,所述方案通过从免疫细胞、受感染的细胞和/或其他糖基化靶标的表面去除有效量的糖基结构(例如,唾液酸)增加了免疫信号传导。以这种方式,所述糖苷酶方案编排或设计了有效的免疫应答,允许受感染的细胞的抗原靶向并引发适当水平的细胞因子/趋化因子级联用于治疗。在这些或其他实施方案中,所述糖苷酶包括至少一种对免疫细胞表面上的突出的末端糖基残基特异的酶(例如,神经氨酸苷酶和/或半乳糖苷酶),和至少一种对免疫细胞表面上的突出的倒数第二的糖基残基特异的酶(例如,β-半乳糖苷酶、岩藻糖苷酶或甘露糖苷酶)。在一些实施方案中,这样的酶与神经氨酸苷酶协同作用。所述方案并不会使患者的免疫系统失调(反而是协调),这在抗击感染性疾病中是至关重要的,并且即使在一定水平的细胞毒性化疗的存在下也是有效的,所述化疗可对免疫细胞具有有害的影响。此外,所述方案适用于长期疗法或反复疗法,原因是在各个实施方案中所述试剂并不被免疫系统靶向。在各个实施方案中,所述方案避免了从正常细胞过多地去除唾液酸或其他糖基结构,以便它们保持正常功能。
在某些实施方案中,本文所描述的糖苷酶方案减少或消除了对施用其他传统的抗病毒或抗微生物疗法的需求。在各个实施方案中,本发明可用于免疫受损或免疫抑制的患者中以增强免疫功能。在各个实施方案中,所述糖苷酶方案不被免疫靶向,并且因此产生的糖苷酶信号传导可用于长期疗法。
在一个方面,本发明提供了通过包含糖苷酶制剂的组合物的非急性方案治疗具有慢性病毒感染的患者的方法,所述糖苷酶制剂既为免疫系统耐受的(“免疫耐受的”)又足以刺激经协调的免疫信号传导。因此所述糖苷酶组合物提供了免疫刺激,如通过一个或多个完整的免疫调节级联,同时避免了所述糖苷酶的免疫靶向,否则这种免疫靶向会随时间推移消除其效力。
所述糖苷酶组合物和方案是具有成本效益的治疗,以逆转病毒性疾病状态或轨迹和/或过渡到长期疾病控制。在一些实施方案中,患者是症状性AIDS患者,并且所述糖苷酶组合物与ARV疗法一起提供或作为ARV疗法的替代而提供,以逆转疾病轨迹。尽管ARV可以是有效的抗病毒治疗,但是ARV具有抑制免疫系统的不利影响,即一种尤其对HIV或AIDS患者有害的影响。因此,通过本文所描述的方案的持续免疫调节具有改善这些副作用的能力,同时从长远来看过渡到主要的疾病控制。可选地或另外,在状况得到改善后,所述糖苷酶组合物允许过渡到用于长期疾病控制的具有成本效益的方案,这在一些实施方案中消除了对长期ARV疗法的需求。所述方案对于控制慢性病毒感染如HIV、HSV、EBV、HAV、HBV、HCV、HPV、腺病毒等而言是普遍有效的。
在第二方面,本发明提供了治疗和/或预防感染性疾病的方法。根据这个方面,患者接受本文所述的组合物的方案,所述方案提供并随时间推移保持有效的免疫刺激,包括作为在流感季节或其他持续的感染性疾病发作或流行病期间单独的或额外的保护。在具体的实施方案中,本发明可用于免疫受损的患者,包括老人、儿童、病人、住院治疗者以及具有免疫缺陷性病症(包括遗传免疫缺陷、药物诱导的免疫缺陷或因感染性疾病如AIDS引起)的那些人。在某些实施方案中,所述组合物和/或方案充当佐剂以增强疫苗效力,从而提供更有效的疫苗接种和/或更长的疫苗保护作用的持续时间,并在一些实施方案中,允许疫苗剂量节省。
在其他方面,本发明提供了一种药物组合物,其包含神经氨酸苷酶、半乳糖苷酶、N-乙酰氨基半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的至少两种以及药学上可接受的赋形剂。例如,所述组合物可以包含神经氨酸苷酶和β-半乳糖苷酶。所述糖苷酶可以按约10-3mg至10-8mg之间共同存在。所述组合物可以被配制用于多种施用途径,包括舌下递送。
在另外其他的方面,本发明提供了一种方便的糖苷酶剂量施用器。所述施用器递送足够数目的糖苷酶剂量用于非急性方案,例如,至少一个月的剂量,用于逆转病毒性疾病的轨迹或控制感染性疾病,或用于预防或减轻感染性疾病,或增强疫苗效力。所述施用器在所述方案的过程中保持所述组合物的稳定性,保护所述组合物免于暴露至可能的环境污染。在根据本文所描述的方案施用所述剂量所需的时间长度的过程中,所述糖苷酶组合物是稳定的。

发明详述
本发明提供了用于非急性免疫增强的组合物、方法和治疗方案,其应用于治疗慢性感染性疾病,包括疫苗(例如,不含佐剂的疫苗)增强。
在一个方面,本发明包括对具有感染性疾病的患者施用一种或多种糖苷酶的体内方案。所述体内方案通过从免疫细胞或其他靶标的表面去除有效量的糖苷酶类(在一些实施方案中特别是唾液酸)来刺激免疫信号传导,并避免从正常细胞过多地去除包括唾液酸的糖苷酶类。所述方案通过被引发的免疫级联允许受感染的细胞的持续性抗原靶向。在一些实施方案中,所述方案包括对去除受病毒感染的细胞上普遍存在的糖苷酶类以及在各个实施方案中是末端或倒数第二的糖苷酶类为活性的酶。在一些实施方案中,所述方案包括神经氨酸苷酶和对去除普遍存在的倒数第二的糖基残基特异的第二糖苷酶,这可以通过避免、预防或减缓糖基链的再唾液酸化和/或再加帽提供协同治疗。包括作为辅助疗法并包括涉及方便的患者剂量监测的实施方案的所述施用方案如本文所详细描述。根据这个方面,所述方法的效力并不决定性地依赖于感染因子的身份(identity)或患者的独特生物学,这不同于许多常规疗法。
在各个实施方案中,本发明提供了足以用于免疫刺激的糖苷酶组合物的施用方案,同时也避免了免疫系统对糖苷酶的靶向,否则这可能会减少或消除其效力。在某些方面,本发明进一步提供了用于施用本文所描述的方案的糖苷酶给药施用器。
在一些实施方案中,所述糖苷酶组合物包含神经氨酸苷酶,其是一种水解各种糖缀合物上的末端唾液酸残基的糖苷键的酶。神经氨酸苷酶被发现于哺乳动物细胞以及各种细菌、真菌和病毒源中。凭借它们在细胞膜的碳水化合物链上的末端位置,唾液酸是细胞之间的通讯以及免疫识别现象的关键调节剂。按照本发明,所述神经氨酸苷酶任选地与如本文所述的其他糖苷酶一起配制,并作为允许它充当信号传导级联免疫调节剂的剂量的方案提供,协调宿主的免疫应答,以有效地对抗感染性疾病(包括HIV与其他病毒和感染因子),同时它本身不被免疫系统(显著地)靶向。在一些实施方案中,通过糖苷酶的合适的配制和/或递送改善了所述组合物的这些特性。
在各个实施方案中,并且不希望受理论的束缚,所述糖苷酶疗法至少部分地通过补体活化增强免疫功能。例如,使用单纯疱疹(HSV-1)作为模型病毒时,体外研究表明:相对于对照,将适当水平的神经氨酸苷酶和补体一起孵育既显著地降低了HSV-1对Vero细胞的感染性又显著地减少了游离病毒自预感染的细胞的释放(图1和图2)。
在各个实施方案中,并且不希望受理论的束缚,所述糖苷酶疗法至少部分地通过细胞因子刺激增强免疫功能,所述细胞因子包括例如白细胞介素-2(IL-2)、干扰素α(IFN-α)、干扰素γ(IFN-γ)、肿瘤坏死因子α(TNF-α)、白细胞介素-4(IL-4)和白细胞介素-6(IL-6)。在体外细胞系统中使用时,相对于各自的对照,至少四种细胞因子的产生在适当水平的神经氨酸苷酶的存在下受到刺激。因此,在一些实施方案中,通过神经氨酸苷酶的被保持或永久的信号级联增强了这些细胞因子的功能,产生对免疫刺激的控制。
在各个实施方案中,并且不希望受理论的束缚,所述糖苷酶疗法至少部分地通过提高免疫细胞相互作用的效率增强免疫功能。在适当水平的神经氨酸苷酶以及其他糖苷酶的存在下,所述提高的免疫细胞相互作用的效率可能是因去除带负电荷的唾液酸分子所导致,产生更大的细胞间接触。在各个实施方案中,所述糖苷酶组合物和方案通过增强细胞介导的细胞毒性和/或免疫细胞活化增强了免疫功能。
在各个实施方案中,并且不希望受理论的束缚,所述糖苷酶方案至少部分地通过提高感染因子如病毒(例如,HIV、HSV、EBV、HCV、HPV、腺病毒))和受病毒感染的细胞对免疫机制的暴露而发挥作用。除了可能的补体活化之外,通过神经氨酸苷酶从HIV或感染HIV的细胞的gp120去除末端唾液酸残基可具有另外的免疫刺激作用。从HIV的gp120/gp160去除末端唾液酸残基和/或其他聚糖暴露了隐藏的表位或改变了糖蛋白的构象,使得所述病毒更容易受到宿主的免疫系统的各个部分的攻击。本文所述的糖苷酶制剂和方案使免疫防御和病毒感染之间的微妙平衡向有利于宿主一方倾斜。
在各个实施方案中,不希望受理论束缚,可以包括神经氨酸苷酶、β-半乳糖苷酶、α-甘露糖苷酶、岩藻糖苷酶(以及其他糖苷酶,包括本文所述的那些)中的一种或多种的糖苷酶方案将维生素D结合蛋白(也被称为组特异性组分或Gc)体内转化为有效的巨噬细胞活化因子,从而活化巨噬细胞抵抗患者的感染性疾病。参见,例如美国专利5,326,749,其在此通过引用并入;Yamamoto等人,J Immunology卷151:2794-2802(1993)。维生素D结合蛋白(也被称为DBP)是一种进化上保守的糖蛋白并且是遗传多态性的。DBP具有约52,000的相对分子量,并且通常构成约0.5%的血浆蛋白质。如本文所述的糖苷酶的适当剂量和方案可以导致抵抗特定的致病病原体(包括其抗原状态的特异性靶向)的有效、持续和长期的体内巨噬细胞活化,这对不断地改变暴露表位的病原体而言是至关重要的。因此,体内施用所述糖苷酶组合物(包括方便的舌下给药)导致抵抗病原体的有效的巨噬细胞活化。
在一个方面,本发明提供了用于治疗具有慢性病毒感染的患者的方法。所述方法包括对患者施用免疫耐受的和免疫信号传导的糖苷酶组合物的非急性方案,以治疗、改善和/或控制所述感染。在一些实施方案中,病毒是与宿主基因组整合(例如,作为原病毒)或者是可以为隐匿性的或以其他方式逃避免疫监护的病毒,从而难以或不可能完全消除。示例性的病毒包括逆转录病毒(如HIV)、单纯疱疹病毒(如HSV-1或HSV-2)、肝炎病毒(例如,HAV、HBV、HCV)和腺病毒。在这样的实施方案中,本文所述的糖苷酶组合物和方案可以取代抗病毒化疗或免疫疗法(例如,细胞因子或趋化因子(如干扰素)疗法或单克隆抗体治疗)用于长期疾病控制,或者可以与这样的疗法组合以改善结果,也就是说,或者同时组合或者按顺序组合。
在一些实施方案中,所述患者是HIV阳性,并且在一些实施方案中所述患者是症状性AIDS患者。HIV感染是一个世界性的问题,并且各国政府已经作出部署以让其国民有机会获得抗逆转录病毒药物。尽管付出这些努力,有时药物因为欠缺沟通和交通不便导致无法获得,这导致患者错过了用药。此外,很多时候,患者不能耐受这些药物。在一些实施方案中本发明提供了抗逆转录病毒药物的替代,并且在其他实施方案中提供了另外的药剂以减少对长期使用抗逆转录病毒剂或其他化疗疗法或抗病毒疗法的需求。
因此,在一些实施方案中,在所述糖苷酶治疗期间所述患者没有接受抗逆转录病毒疗法,例如,因为所述患者不能耐受抗逆转录病毒疗法,或这样的ARV疗法对患者而言无法获得。所述HIV可以是任何亚型,如HIV-1或HIV-2。在一些实施方案中,所述HIV耐抗病毒化疗,使得替代疗法的可获得性是至关重要的。在一些实施方案中,在抗逆转录病毒疗法之后施用或启动所述糖苷酶以控制慢性AIDS,由此提供更有效和成本敏感的长期疾病控制。
所述糖苷酶方案也可以与抗逆转录病毒化疗一起施用,例如,以帮助逆转疾病轨迹。一旦疾病轨迹被逆转,则所述糖苷酶方案可以任选地持续至少一个月、至少两个月、至少四个月、至少六个月、或至少一年、或至少两年、或至少五年或更长,以提供所述疾病的具有成本效益的控制。可以如本文所述调节用于长期治疗的频率或每日剂量。
在一些实施方案中,免疫耐受的糖苷酶信号传导补偿了CD4细胞的损失,同时给予宿主时间来恢复和补充它的这些关键细胞的供应。在一些实施方案中,所述糖苷酶组合物允许抗逆转录病毒疗法的受试者停止抗逆转录病毒疗法一段时间(例如,约一至六个月、或约一至四个月、或约一至两个月),由此允许所述主体从这些药物的毒性作用中恢复,同时还提供成本优势。所述方案还实现了病毒和病毒颗粒,以及优先于宿主细胞的受感染的(例如,溶原的)细胞的免疫靶向。在一些实施方案中,AVR和糖苷酶治疗的这种“周期”在整个疗法中被重复一次或多次。在另外其他的实施方案中,所述糖苷酶疗法与抗逆转录病毒疗法一起施用防止了通常由逆转录病毒疗法表现出的免疫系统的部分衰减。与其他化疗方案一起施用,如在本段落中所述,对于其他病毒感染(例如,HSV-1或HSV-2、EBV、HAV、HBV、HCV、HPV、腺病毒)可提供相同或类似的优点。
被设计成对抗HIV的药物已被集中于病毒生命周期的不同阶段。一些药物已被设计成阻断病毒对CD4T细胞的附着。其他药物以参与细胞内的组装阶段的各种酶为目标;例如,齐多夫定(zidovudine)(AZT)是一种众所周知的药物,其作用是抑制逆转录酶(即HIV使用所述酶以将其RNA基因组复制到DNA中的酶)的活性。这些合成药物中的一些药物已经减缓了至AIDS的进展,但没有药物能停止它或消除病毒。一个主要的问题是HIV的高突变率,其中最初显示出一些希望的药物变得无效,因为病毒将其形式变为不再被药物识别出的形式。所述糖苷酶方案还以允许增强ARV以及其他可能的化疗的HIV突变衍生物为目标,以降低突变病毒规避治疗剂的倾向。
如本文所用,术语抗逆转录病毒疗法指被施用于HIV和AIDS患者的活性剂(通常是活性剂的混合物)。这些包括例如包括进入抑制剂(例如,马拉韦罗(maraviroc)和/或恩夫韦地(enfuvirtide))、CCR5受体拮抗剂、逆转录酶抑制剂(例如,齐多夫定、地丹诺辛(didanosine)、扎西他滨(zalcitabine)、司他夫定(stavudine)、膦甲酸(foscarnet)和/或拉米夫定(lamivudine))、蛋白酶抑制剂(例如,利托那韦(ritonavir)、地瑞那韦(darunavir)、阿扎那韦(atazanavir)、沙奎那韦(saquinavir))、整合酶抑制剂(例如,雷特格韦(raltegravir))、成熟抑制剂(例如,α-干扰素、贝韦立马(bevirimat)和/或vivecon)的疗法。在一些实施方案中,逆转录病毒疗法包括两种核苷类似物和一种非核苷类似物或蛋白酶抑制剂的组合。三种药物的这种组合通常被称为“三药混合物(cocktail)”。实例包括COMBIVIR(齐多夫定+拉米夫定)、TRIZIVIR(阿巴卡韦(abacavir)+齐多夫定+拉米夫定)、KALETRA(洛匹那韦(lopinavir)+利托那韦)、EPZICOM(阿巴卡韦+拉米夫定)、TRUVADA(替诺福韦(tenofovir)+恩曲他滨(emtricitabine))、ATRIPLA(依非韦伦(efavirenz)+替诺福韦+恩曲他滨)。其他组合包括:替诺福韦/恩曲他滨/雷特格韦、替诺福韦/恩曲他滨/利托那韦、和地瑞那韦/替诺福韦/恩曲他滨/利托那韦/阿扎那韦。
在一些实施方案中,所述抗逆转录病毒疗法包括两种核苷逆转录酶抑制剂和一种非核苷酸逆转录酶抑制剂。在一些实施方案中,所述ARV疗法包括司他夫定、拉米夫定和/或奈韦拉平(Nevirapine)。在一些实施方案中,所述ARV疗法包括依法韦仑。这样的疗法,如果施用,在过渡到长期糖苷酶治疗之前被有节制地使用,或以降低初始疾病轨迹。这种采用糖苷酶疗法的长期疾病控制引发了抵抗游离病毒(包括突变病毒)和受感染的细胞的免疫级联。
在各个实施方案中,本文描述的方案适于治疗在AIDS的不同阶段的患者。在一些实施方案中,在所述方案开始时患者的CD4计数小于约500个细胞/mm3。例如,在所述方案开始时患者的CD4计数可以在约200和约400之间。在所述方案开始时患者的CD4计数可以小于约400、小于约350、小于约300、小于约200、小于约100或小于约50。在一些实施方案中,当CD4计数小于400时,单独地或与所述糖苷酶组合物一起施用抗逆转录病毒疗法,并且一旦CD4计数正常化(例如,高于约400或高于约500或高于约800)则将所述糖苷酶组合物用于长期护理。在一些实施方案中,患者可能继续具有低于正常的CD4计数,但所述糖苷酶组合物帮助免疫系统对此缺损发挥作用。在另外其他的实施方案中,所述患者具有如所述的低CD4计数,并且在无逆转录病毒疗法下施用所述糖苷酶组合物,可以使CD4计数从长远来看正常化。
在一些实施方案中,在所述方案开始时患者的病毒载量高于约10,000/ml。例如,在所述方案开始时所述病毒载量可以是至少约25,000/ml、至少约40,000/ml、至少约50,000/ml、至少约75,000/ml、至少约100,000/ml、至少约500,000/ml、至少约1百万/ml或至少约5百万/ml。在一些实施方案中,患者接受抗逆转录病毒疗法以将病毒载量变为小于约50,000/ml、或小于10,000/ml或检测不到的,然后再过渡到糖苷酶治疗作为长期护理。在一些实施方案中,抗病毒疗法和糖苷酶的组合可以更快地或更完全地降低病毒载量。
在一些实施方案中,糖苷酶疗法的好处可至少部分地通过患者的健康评估(包括情绪和精神状态、食欲、能量水平、继发感染/并发症、毛发生长以及其他改善整体健康的症状)来辨识。采用AIDS治疗的常规定量(例如CD4水平或病毒滴度)可辨识出或未辨识出这样的好处。
本文所描述的糖苷酶方案适用于其他慢性病毒感染。例如,在一些实施方案中,患者具有选自单纯疱疹病毒感染(例如,HSV-1或HSV-2)、Epstein-Barr病毒感染(例如,单核细胞增多症)、巨细胞病毒感染、水痘带状疱疹病毒感染、甲型、乙型或丙型肝炎、腺病毒感染或者人乳头状瘤病毒感染的慢性病毒感染。在一些实施方案中,患者被诊断为患有慢性疲劳综合征。所述糖苷酶组合物治疗和/或预防疾病的发作并减少病毒载量,并且在一些实施方案中,取代了传统的抗病毒策略,例如细胞因子和/或趋化因子疗法(例如,干扰素或白细胞介素)或小分子抗病毒药物(例如,阿昔洛韦(acyclovir)、伐昔洛韦(valaciclovir)、泛昔洛韦(famciclovir))。或者,一旦病毒感染处于常规疗法(例如,干扰素或小分子病毒抑制剂或单克隆抗体治疗)的控制之下,则施用所述糖苷酶组合物用于长期控制。在一些实施方案中,所述方案降低或消除病毒性病变(例如唇疱疹)和/或防止其复发。
例如,在一些实施方案中,患者具有HSV或水痘带状疱疹病毒感染,并且在一些实施方案中可具有带状疱疹。按照本发明,患者可能无法接受抗病毒药物,例如阿昔洛韦、伐昔洛韦和/或泛昔洛韦,或在其他实施方案中,在传统抗病毒药物失败后施用所述糖苷酶疗法以改善或消除感染或其症状,或在凭借患者耐受这些药物的能力使传统抗病毒药物成为规则之后施用。
在另外其他的实施方案中,患者具有丙型肝炎病毒感染,并接受了干扰素疗法。在这样的实施方案中,例如,一旦该疗法失去效力或不被病人耐受,则可提供所述糖苷酶方案以替代无效的INF疗法。在其他实施方案中,所述糖苷酶方案可与干扰素疗法并行提供,以增强其效力。所述糖苷酶方案可促进干扰素或抗体疗法的适当整合用于有效的免疫应答。

可通过多种途径施用所述糖苷酶制剂,包括舌下、鼻、口(port)、真皮下、灌胃、眼内、静脉内、肌内、皮下、经真皮和颊部。在各个实施方案中,通过舌下施用所述糖苷酶。在一些通过舌下施用所述糖苷酶的实施方案中,所述神经氨酸苷酶在舌下保持约1至约5分钟,并且优选为约3、约4或约5分钟。在此期间患者应避免讲话。在施用的15分钟内患者应不吃或不喝。
根据本发明的各方面,按平均每天2至6次施用糖苷酶的方案至少两周或至少一个月,特别是针对免疫受损或晚期病例。每天的施用应基本上均匀地间隔开,但在各个实施方案中间隔约15分钟至5小时。例如,剂量可以间隔约15分钟、约30分钟、约1小时、约2小时或约3小时。例如,可以按平均每天2至8次施用所述糖苷酶制剂至少约两个月、至少约四个月或至少约五个月或至少约六个月。在一些实施方案中,按每天约2、约3或约4次施用所述糖苷酶制剂至少一个月、两个月、三个月、四个月、五个月或六个月。通常,在不会表现出显著延长的关节不舒服或不适下,按有效地减少感染性疾病的病理或刺激免疫系统的剂量和频率施用所述糖苷酶制剂。当患者确实经历关节或全身不舒服时,所述患者调节剂量和/或频率(例如,跳过一天、两天或三天的神经氨酸苷酶给药,或将每日剂量减少一次或两次施用),直到不舒服消退或正常化。因此,在一些实施方案中所述施用方案在关节不舒服时暂停。
例如,治疗的第一天可以约八个剂量开始,在首先的一个或两个小时内服用首先的三至五个剂量,其余在全天大致均匀间隔地服用。然后可以通过每天约四个剂量对患者进行治疗,定期监测恶性肿瘤。即使当恶性肿瘤为检测不到时,患者仍可以保持每天2至7个剂量的方案,如基于关节不舒服或不适的表现时不时地调节。
可以如上所述地启动疗法。然而,在一些实施方案中,所述糖苷酶方案是这些常规疗法的替代。在一些实施方案中,随后通过每天约一剂量对患者长期治疗至少约六个月、或至少约一年、或至少约两年、或至少约五年或更长,或者被选择或被规定用于此类长期治疗。在一些实施方案中,在不存在化疗或其他疗法下这种后续的长期治疗用以减少复发或疾病进展的可能性。长期糖苷酶治疗,例如,为了防止疾病复发或再发,可以每天施用1或2次。
在一些实施方案中,患者被指导监测关节僵硬或不适。这样的状况表明应对糖苷酶治疗进行调节。所述调节可以包括跳过一天或两天或多至一周的用药,或可选地通过每天一次或两次施用降低剂量,直到症状消除。其他分子测定可用于同样的效果,但是关节僵硬或不适提供了患者顺应性的缓解。因此,在所述方案的过程中,可容易地针对患者对所述糖苷酶剂量进行调节,从而保持长期的最佳护理。
在本发明的另外其他方面,提供了一种药物组合物,其包含用于根据需要施用单一的糖苷酶剂量的递送媒介物,且其中所述媒介物含有至少50个剂量、或至少100个剂量、至少150个剂量、或至少200个剂量的全糖苷酶方案。施用的每个糖苷酶剂量的量为至多约10-2mg糖苷酶和如已经描述的药学上的惰性成分。所述药物组合物可包含神经氨酸苷酶、半乳糖苷酶、N-乙酰氨基半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的至少两种以及药学上可接受的赋形剂。例如,所述组合物可以包含神经氨酸苷酶和β-半乳糖苷酶。所述糖苷酶可以按约10-3mg至10-8mg之间存在(全体地),或根据上文所公开的剂量存在。所述组合物可以被配制用于多种如本文所公开的施用途径,包括舌下递送。
在各个实施方案中,所述治疗方案包括不超过约10-3mg的糖苷酶的量的分割式施用(partitional administration),但是,在某些情况下,在任何一天中施用的糖苷酶的总量可以超过此限度。
可以各种途径和形式施用所述糖苷酶制剂。例如,所述糖苷酶可以作为固体施用,其中所述酶被包埋或混合在生物可降解或生物可蚀性基质中。所述基质可以是延时释放基质(time release matrix)。这些基质是那些本领域普通技术人员所熟知的。可以通过注射或通过舌下途径施用所述糖苷酶。在一个实施方案中,所述媒介物是被包含在惰性容器中的水溶液。在另一个变型中,所述组合物是栓剂的形式。所述组合物的液体形式可以经皮下、肌内或静脉内注射。此外,可通过粘膜(如鼻膜)施用所述组合物。
在某些实施方案中,通过药物施用器施用所述糖苷酶组合物,所述施用器包括至少100个剂量、或至少150个剂量或至少200个剂量的组合物。在各个实施方案中,所述施用器用于舌下、鼻、经真皮、真皮下延时释放、眼内、灌胃、口、皮下、口服或颊部递送。例如,所述施用器用于舌下递送。
在一些实施方案中,所述施用器递送可以由患者根据需要进行调节的带计量的剂量。
所述施用器优选以维持剩余剂量的无菌条件的方式分配剂量。通过非限制性实例的方式,所述施用器可以是被描述在美国专利4,830,284;4,565,302;5,011,046;5,147,087;5,893,484;6,877,672;6,886,556和7,201,296中的那些中的任一种,所述美国专利各自通过引用在此整体并入。例如,所述施用器可以是雾化或给药泵,其可以确保存在于泵缸和排放口之间的区域中的介质不会干涸或不以其他方式受环境影响而改变。参见美国专利4,830,284,其通过引用在此并入。在一些实施方案中,所述施用器采用了0.2μm的过滤器,以维持无菌内容物。此外,所述施用器可以在单一冲程排放中分配剂量。此类施用器被描述在美国专利5,893,484中,其通过引用在此并入。所述施用器可被构造用于经鼻递送、真皮递送、咽喉递送或舌下递送。在一些实施方案中,所述施用器实现了可致动的给药机构,其允许监测精确的剂量,因此在很大程度上消除了在剂量的数目和/或持续给药方面不正确的给药。参见美国专利4,565,302,其通过引用在此并入。在一些实施方案中,所述施用器递送50至100μl的剂量,如在例如美国专利6,886,556中描述的施用器,其通过引用在此并入。
实施例
实施例1:对HSV-1感染性的影响
使用HSV-1作为模型病毒,启动体外研究以确定神经氨酸苷酶和补体对自Vero细胞的病毒感染性和病毒释放的影响。对于感染性研究而言,将补体、病毒和神经氨酸苷酶的组合一起孵育,然后加入到Vero细胞,并进一步孵育。然后进行细胞固定和染色后,对细胞中形成病毒的斑块进行计数。结果表明,相对于对照,将病毒与神经氨酸苷酶和补体一起孵育显著地(70-80%)降低了Vero细胞的病毒感染性(图1)。
对于病毒释放研究而言,使Vero细胞首先用HSV-1感染。然后将这些细胞与不同浓度的补体和神经氨酸苷酶孵育。对来自细胞培养物的上清液测试形成斑块的能力,所述斑块表明自细胞释放出的病毒。结果表明,相对于对照,在神经氨酸苷酶和补体两者的存在下,游离病毒的释放被大大降低(70-80%)(图2)。这些结果表明,神经氨酸苷酶和补体的组合既显著地降低了受HSV-1感染的Vero细胞的感染性又显著地减少了游离病毒自预感染的细胞的释放。
实施例2:对细胞因子释放的影响
研究了神经氨酸苷酶对某些体外细胞因子的产生的影响。细胞因子是由活化的淋巴细胞分泌的应答于感染的化学信使。任何给定的细胞因子(单独的或与其他细胞因子组合)可对免疫功能产生多重作用。白细胞介素-2(IL-2)、干扰素α(IFN-α)、干扰素γ(IFN-γ)和肿瘤坏死因子α(TNF-α)是参与宿主抵抗病毒的防御的重要细胞因子。IL-2、TNF-α和IFN-γ由T细胞的T辅助(TH1)子集产生,并且因此与炎症过程相关。IL-2和IFN-γ一起活化巨噬细胞,所述巨噬细胞是吞噬并消化病原体(吞噬作用)的重要的免疫细胞,并且还可以用作对T淋巴细胞的抗原提呈细胞。IL-2和IFN-γ还提高了自然杀伤(NK)细胞在清除受病毒感染的细胞中的细胞毒性。IFN-γ还增强了抗原提呈细胞上的主要组织相容性复合体(MHC)I类和II类分子的表达,从而诱导参与病毒清除的CD4+和CD8+溶细胞性细胞。IFN-γ还与TNF-α组合以刺激NK细胞。IFN-α通过阻断早期病毒蛋白的转录来抑制病毒复制。
使用细胞系统、孵育条件和其他适合于生产和测定各分子的程序对这些细胞因子进行了体外研究。将含神经氨酸苷酶的试验样品与盐水对照进行了比较。图3总结了当对用神经氨酸苷酶处理的细胞测试IL-2、IFN-α、IFN-γ和TNF-α的产生时获得的结果。相对于各自的对照,使用治疗剂量的神经氨酸苷酶刺激了所有四种细胞因子的产生。预期上面讨论的这些细胞因子的功能可以通过体内系统中的神经氨酸苷酶得到加强并会在患者中具有抗病毒效果是合理的。
Claims (48)
1.一种用于治疗具有慢性病毒感染的患者的方法,其包括:
对所述患者施用免疫耐受的和免疫信号传导的糖苷酶组合物的非急性方案,以治疗、改善和/或控制所述感染。
2.如权利要求1所述的方法,其中所述患者是HIV阳性。
3.如权利要求2所述的方法,其中所述患者具有AIDS。
4.如权利要求1至3中任一项所述的方法,其中所述患者未经受抗病毒化疗,和/或所述HIV对抗病毒疗法有抗性。
5.如权利要求3所述的方法,其中在抗病毒疗法之后启动所述方案以控制慢性AIDS。
6.如权利要求3所述的方法,其中所述组合物与抗病毒疗法一起施用以逆转疾病轨迹,以及任选地持续至少六个月、或至少一年、或至少两年、或至少五年来任选地在无ARV治疗的情况下控制所述感染。
7.如权利要求1至6中任一项所述的方法,其中在所述方案开始时所述患者的CD4计数小于500。
8.如权利要求7所述的方法,其中在所述方案开始时所述患者的CD4计数在200和400之间。
9.如权利要求7所述的方法,其中在所述方案开始时所述患者的CD4计数小于400、小于350、小于300、小于200、小于100或小于50。
10.如权利要求2至9中任一项所述的方法,其中在所述方案开始时病毒载量为大于10,000/ml。
11.如权利要求10所述的方法,其中在所述方案开始时所述病毒载量为至少25,000/ml、至少40,000/ml、至少50,000/ml、至少75,000/ml、至少100,000/ml、至少500,000/ml、至少1百万/ml或至少5百万/ml。
12.如权利要求1所述的方法,其中所述患者具有选自单纯疱疹病毒感染、水痘带状疱疹病毒感染、甲型、乙型或丙型肝炎、腺病毒感染或者人乳头状瘤病毒感染的慢性病毒感染。
13.如权利要求12所述的方法,其中所述患者具有带状疱疹。
14.如权利要求1至13中任一项所述的方法,其中所述糖苷酶方案包括施用神经氨酸苷酶。
15.如权利要求14所述的方法,其进一步包括施用半乳糖苷酶、N-乙酰半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的一种或多种。
16.一种用于治疗具有持续性或复发性细菌感染的患者的方法,其包括对所述患者施用糖苷酶的方案。
17.如权利要求16所述的方法,其中所述细菌感染与肺炎、支气管炎、鼻窦炎、肠炎、结肠炎、败血症或尿路感染相关。
18.如权利要求16所述的方法,其中所述细菌感染是持续性或复发性耳、眼、鼻和/或咽喉感染。
19.如权利要求16至18中任一项所述的方法,其中所述细菌是分枝杆菌属、假单胞菌属、嗜血杆菌属、莫拉氏菌属、衣原体属、奈瑟氏菌属、链球菌属、葡萄球菌属、博德特氏菌属或耶尔森氏菌属的种。
20.如权利要求19所述的方法,其中所述细菌是结核分枝杆菌。
21.如权利要求16至20中任一项所述的方法,其中在至少一轮的抗生素疗法未能改善或消除所述感染之后施用所述糖苷酶方案。
22.如权利要求14至18中任一项所述的方法,其中与抗生素疗法并行施用所述糖苷酶方案。
23.如权利要求20所述的方法,其中所述抗生素疗法采用氨基糖苷、碳青霉烯、头孢菌素、大环内酯、β内酰胺、喹诺酮、磺酰胺或四环素中的一种或多种。
24.如权利要求16至23中任一项所述的方法,其中所述糖苷酶方案包括施用神经氨酸苷酶。
25.如权利要求24所述的方法,其进一步包括施用半乳糖苷酶、N-乙酰半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的一种或多种。
26.一种用于治疗真菌或寄生虫感染的方法,其包括对需要其的患者施用糖苷酶方案。
27.如权利要求26所述的方法,其中所述感染是念珠菌病。
28.如权利要求26所述的方法,其中所述感染是疟疾或锥虫病。
29.如权利要求26至28中任一项所述的方法,其中所述糖苷酶方案包括施用神经氨酸苷酶。
30.如权利要求29所述的方法,其进一步包括施用半乳糖苷酶、N-乙酰半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的一种或多种。
31.一种用于增强疫苗接种的方法,其包括对所述患者施用免疫信号传导的糖苷酶的方案,所述方案在疫苗接种之前、期间或之后开始。
32.如权利要求31所述的方法,其中所述疫苗是不含佐剂的疫苗。
33.如权利要求1至32中任一项所述的方法,其中通过选自舌下、鼻、口、真皮下、灌胃、眼内、静脉内、肌内、皮下、经真皮和颊部的途径施用所述糖苷酶。
34.如权利要求33所述的方法,其中通过舌下施用所述糖苷酶。
35.如权利要求31至34中任一项所述的方法,其中所述糖苷酶方案包括施用神经氨酸苷酶。
36.如权利要求35所述的方法,其进一步包括施用半乳糖苷酶、N-乙酰半乳糖苷酶、岩藻糖苷酶、葡糖苷酶、N-乙酰氨基葡糖苷酶和甘露糖苷酶中的一种或多种。
37.如权利要求1至36中任一项所述的方法,其中所述糖苷酶是生物来源的糖苷酶或保持了免疫调节活性的其衍生物或活性部分。
38.如权利要求35或36所述的方法,其中所述糖苷酶来自于生物来源或是合成的。
39.如权利要求1至38中任一项所述的方法,其中按照平均每天2至6次施用所述糖苷酶至少一个月。
40.如权利要求39所述的方法,其中按照平均每天2至6次施用所述糖苷酶至少两个月、至少四个月或至少六个月。
41.如权利要求39所述的方法,其中随后每天用约一个剂量对所述患者长期地治疗,持续至少六个月、或至少一年、或至少两年、或至少五年或更长。
42.一种用于施用糖苷酶组合物的药物施用器,所述施用器包括至少100个剂量的所述神经氨酸苷酶组合物。
43.如权利要求42所述的药物施用器,其中所述施用器用于舌下、鼻、经真皮、真皮下延时释放、眼内、灌胃、口、皮下、口服或颊部递送。
44.如权利要求42所述的药物施用器,其中所述施用器是用于舌下递送。
45.如权利要求42至44中任一项所述的药物施用器,其中所述施用器以维持剩余剂量的无菌条件的方式分配剂量。
