本发明涉及一种鼻内治疗泵,尤其是一种鼻腔双囊可调泵,属于医疗器械的技术领域。按照本发明提供的技术方案,鼻腔双囊可调泵包括用于填塞在鼻腔内对鼻腔内表面缓释给药并加压的加压给药囊、治疗时位于口腔内后鼻孔开口处用于防止所述加压给药囊从鼻腔内脱出的固定囊及与加压给药囊和固定囊分别连通的加药管,固定囊的位置能根据不同患者解剖特点精确调整位置。本发明在鼻腔手术后或治疗鼻出血能在压迫创面同时持续给药,结构简单,实施方便,疗效增加,安全可靠。

技术领域
本发明涉及一种治疗泵,尤其是一种鼻腔双囊可调泵,属于医疗器械的技术领域。
背景技术
鼻腔手术后常规填塞鼻腔压迫止血,给病人带来较大强度的不适,如:头痛、头晕、恶等。鼻出血的填塞疗法也有同样弊端,急需解决。目前最常见的解决方案是使用常规的静脉镇痛泵,但静脉镇痛泵提供的是全身作用的镇痛效果,但对鼻腔填塞局部不适治疗效果欠佳还伴随诸多副作用,如恶心呕吐、呼吸抑制、皮肤瘙痒等。不适当的加大镇痛药物剂量只会带来更大风险,甚至危及患者生命安全。
同时,非麻醉的鼻出血患者鼻腔内填塞压迫及填塞物的固定也是较为棘手的问题,因为疼痛,患者不能配合,造成填塞失败。
临床需要一种能方便填塞及固定,即使患者在非麻醉状态下也能轻松填塞压迫并固定且能减轻填塞后不适的耳鼻喉科手术耗材。
发明内容
本发明的目的是克服现有技术中存在的不足,提供一种鼻腔双囊可调泵,在非麻醉状态的患者也可应用,填塞鼻腔后的压迫治疗同时局部能泵入局麻药或镇痛药,固定方便可靠,减轻患者痛苦,结构简单,成本低,安全可靠。
按照本发明提供的技术方案,一种鼻腔双囊可调泵,包括用于填塞在鼻腔内对鼻腔加压并对鼻腔内表面缓释给药的加压给药囊、治疗时位于后鼻孔开口处口腔内用于防止所述加压给药囊从鼻腔内脱出的固定囊及与加压给药囊和固定囊分别连通的加药管。所述加药管为“Y”形,加压给药囊和固定囊分别设置在“Y”形加药管的两分叉的头端。加药管主干及“Y”形主分叉头端内包埋设置用于对加压给药囊加注药液的加压囊加药管,加药管主干内设置固定囊加药腔,用于活动埋置固定囊加药管。所述固定囊加药管能在固定囊加药腔内滑动,所述加压囊加药管的尾端连通设置加压囊加药口,用于对所述加压给药囊加注药液。所述固定囊加药管出固定囊加药腔的尾端设置固定囊加药口,用于对固定囊加注药液。所述加药管尾端在固定囊加药管的出口处设置固定块,所述固定块能将固定囊加药管临时固定在加药管的固定囊加药腔内。
所述加压给药囊包括半滤膜套囊或微孔膜套囊,所述加压给药囊内注满药液时,药液能经囊壁缓慢渗出。所述加压给药囊包括弹性较差的预塑形低压高容量套囊,不受外力时注入药液后加压给药囊扩张后为头端截面较小尾端截面较大的圆锥柱形囊。
所述固定囊为囊壁厚薄一致的较强弹性的套囊,不受外力时注入药液囊壁沿固定囊加药管垂直方向均匀扩张。
所述加压囊加药口连接设置用于对所述加压给药囊持续可控加注药液的加药泵,所述加药泵包括机械泵及电子泵。
所述固定块为设置在加药管尾端固定囊加药管出口处外壁的螺旋环,所述螺旋环内壁设置与固定囊加药腔外侧管壁匹配的内径逐层变小的凸出的环形螺纹。当螺旋环沿固定囊加药腔外侧管壁旋入时,固定囊加药管被临时固定在加药管尾端的固定囊加药腔内;当螺旋环沿固定囊加药腔外侧管壁旋出时,临时固定在加药管尾端的固定囊加药腔内的固定囊加药管被松解,固定囊加药管能在固定囊加药腔内滑动。
本发明的优点:在非麻醉的患者鼻腔出血需填塞固定压迫时也能方便应用;在填塞固定压迫的同时持续对鼻腔内表面给药,在填塞止血同时缓解患者局部疼痛,能根据不同患者的解剖特点具体调整固定囊的位置,达到最佳固定效果,结构简单,成本低,使用方便,效果安全可靠。

下面结合具体附图对本发明作进一步说明。
如图1所示,所述一种鼻腔双囊可调泵,包括用于填塞在鼻腔内对鼻腔加压并对鼻腔内表面缓释给药的加压给药囊1、治疗时位于后鼻孔开口处口腔内用于防止所述加压给药囊1从鼻腔内脱出的固定囊2及与加压给药囊1和固定囊2分别连通的加药管3。所述加药管3为“Y”形,加压给药囊1和固定囊2分别设置在“Y”形加药管3的两分叉的头端。加药管3主干及“Y”形主分叉头端内包埋设置用于对加压给药囊1加注药液的加压囊加药管31,加药管3主干内设置固定囊加药腔33用于活动埋置固定囊加药管32,所述固定囊加药管32能在固定囊加药腔33内滑动。所述加压囊加药管31的尾端连通设置加压囊加药口41,用于对所述加压给药囊1加注药液;所述固定囊加药管32出固定囊加药腔33的尾端设置固定囊加药口42,用于对固定囊2加注药液。所述加药管3尾端在固定囊加药管32的出口处设置固定块5,所述固定块5能将固定囊加药管32临时固定在加药管3的固定囊加药腔33内。
具体使用时:首先将“Y”形加药管3的两分叉的头端分别置入鼻腔及鼻咽腔内,即:将设置加压给药囊1的分叉端置入鼻腔,将设置固定囊2的分叉端置入鼻咽腔内,同时,加药管3的尾端暴露在鼻孔外。当然,此时加药管3尾端的加压囊加药口41及固定囊加药口42军暴露在鼻孔外。然后,调节活动埋置在固定囊加药腔33内的固定囊加药管32暴露长度,使实际置入鼻咽腔通道的固定囊加药管32的长度适宜,固定囊2位于鼻腔腔内的最佳位置,调节固定块5使固定囊加药管32临时固定在加药管3的固定囊加药腔33内。连接暴露在鼻孔外的固定囊加药口42通过固定囊加药管32对固定囊2加注药液,使固定囊2饱满充涨并卡置在后鼻孔开口处及鼻咽腔内。
再连接暴露在鼻孔外的加压囊加药管31对加压给药囊1加注药液,使加压给药囊1的压力达到适当范围,能对鼻腔创面出血或出血点进行有效压迫,此时,由于固定囊2的固定作用,加压给药囊1不会脱出。
如图2所示,为了实现加压给药囊1对鼻腔缓释药液进一步治疗,所述加压给药囊1包括半滤膜套囊或微孔膜套囊,所述加压给药囊1内注满药液时,药液能经囊壁上的渗药微孔01缓慢渗出。具体药液包括用于治疗出血的止血药或用于减轻疼痛的局部麻醉药或镇痛药或镇静药等等。其中,局麻药或止血药通过局部作用达到治疗效果,镇痛药及镇静药除局部作用外更多的是通过粘膜或创面吸收如血,通过中枢系统起作用。
为了保证所述半滤膜套囊或微孔膜的药液经渗药微孔01渗出时速度不致过快,所述加压给药囊1的材质优选弹性较差的预塑形低压高容量套囊,即:套囊膜本身弹性较差,陶囊的囊腔大小预先已确定;当套囊充满药液后继续向套囊内充入药液套囊内的液压急剧增加,同时,套囊囊腔的容积变化极为有限。当然,为了确保所述加压给药囊1的压迫效果,预先确定的加压给药囊1囊腔容量要适度大于鼻腔内的容积,准确的说加压给药囊1在长度粗度都要适当大于鼻腔内需要压迫区域的容积。
为了使加压给药囊1囊腔充入药液后的外形更好的与鼻腔内空间外形相匹配达到较好的压迫效果,加压给药囊1优选预先制成头端较细,尾端较粗的圆锥柱形囊。这样,加压给药囊1充入药液扩张饱满后外形为头端截面较小尾端截面较大的圆锥柱形囊,更好的压迫鼻腔内创面或出血点。而当加压给药囊1没加注药液置入鼻腔内之前,应经加压囊加药管31抽出囊内的残余气体,使整个加压给药囊1的囊壁能紧贴在设置了加压囊进药孔10的加压囊加药管31的头端外部。
更进一步的,加压给药囊1的囊壁外表面优选凹凸相间的结构,避免鼻腔内某些凹陷部位压迫效果不佳。这样,鼻腔内表面平滑部位压迫时囊壁外表面凹凸相间互相挤压不影响压迫效果,而鼻腔内表面凹陷部位压迫时就能达到较佳的压迫治疗效果。
同理,为了使固定囊2在鼻咽腔内达到稳妥的固定效果,所述固定囊2为囊壁厚薄一致的较强弹性的套囊,不受外力时注入药液囊壁沿固定囊加药管32垂直方向均匀扩张,使固定囊2和鼻咽腔内表面的摩擦力达到最佳。同时,固定囊2的外表面应做成磨砂面处理的软囊,增加摩擦系数。
具体实施时,随着加压给药囊1内药液渗出,加压给药囊1对鼻腔内表面的创面或出血点的压迫的压力缓慢降低,为了保证治疗效果,需间断适时对加压给药囊1内追加药液补充流出的药液。
如图3所示,为了便易对加压给药囊1内追加药液,所述加压囊加药口41连接设置用于对所述加压给药囊1持续可控加注药液的加药泵6,所述加药泵包括机械泵及电子泵。加药泵6加药的速度因和加压给药囊1内药液渗出速度相当,确保加压给药囊1内药液的压力在适宜的范围之内。
具体的,所述固定块5为设置在加药管3尾端固定囊加药管32出口处外壁的螺旋环,所述螺旋环内壁设置与固定囊加药腔33外侧管壁匹配的内径逐层变小的凸出的环形螺纹。当螺旋环沿固定囊加药腔33外侧管壁旋入时,固定囊加药管32被临时固定在加药管3尾端的固定囊加药腔33内;当螺旋环沿固定囊加药腔33外侧管壁旋出时,临时固定在加药管3尾端的固定囊加药腔33内的固定囊加药管32被松解,固定囊加药管32能在固定囊加药腔33内滑动。
当然,固定块5的具体实施方式有许多,此处不再一一列举。所述固定囊2内不是一定要充注药液,还可以充注气体、药液甚至其他液体。
在非麻醉患者,仅需将没有加注药液的固定囊2置入鼻咽腔内,同时将没有加注药液的加压给药囊1置入鼻腔,此时,两个囊的体积小,患者不适感并不时难以忍受,可以完成操作。当向固定囊2内加注适量药液后,加压给药囊1被固定在鼻腔内;然后仅需持续的向加压给药囊1内缓慢加注药液,加压给药囊1不断胀起向鼻腔内创面及出血点不断加压,此时的压力是缓慢增加;同时也没有手术器械的点状碰触及钳夹,其痛楚是有限的,患者一般情况下可以耐受。
在压迫治疗结束时,仅需从固定囊加药口42抽出固定囊2内的药液使固定囊2瘪陷;同时,从加压囊加药口41抽出加压给药囊1内的药液,使加压给药囊1瘪陷,就可轻松将本治疗装置从患者治疗侧鼻孔抽出,完成治疗。
总之:该鼻腔双囊可调泵通过加压给药囊1对鼻腔创面或出血点压迫给药,通过固定囊2位置调整使加压给药囊1被稳妥的固定在鼻腔内。需要时可以在加压给药囊1加药口连接加药泵6,使治疗轻松可靠。本装置结构简单,成本低,使用方便,效果安全可靠。
