背景技术
脊柱融合术是一种手术技术,通过该脊柱融合术把两个或多个椎骨连结一起。该技术用于处理如脊柱畸形、损坏的脊柱盘、以及椎骨骨折的各种情况。融合可通过在待连结的椎骨之间引入新的骨组织和刺激椎骨本身固有的骨生长能力来实现。在一些操作中,脊柱盘和/或椎骨能用隔离件或笼替换,所述隔离件或笼使椎骨之间维持适当间隔且提供如下结构:椎骨通过所述结构可生长且最终融合一起。

具体实施方式
图1图示出典型的脊柱植入件100的立体图。脊柱植入件100包括笼110、骨板120、以及紧固件130。笼110和骨板120可通过紧固件130连接在一起。
笼110可被定尺寸以插入两个椎骨之间。在各种实施例中,笼110可被定尺寸用于插入邻近的椎骨之间或可被定尺寸以替换一个或多个椎骨,这样,可构成椎骨本体的替换。在各种实施例中,笼110可被定尺寸用于插入颈部、胸部和/或腰部椎骨之间。笼110可包括支架112、格子结构114、以及内缘116。
支架112可由各种材料形成。在各种实施例中,支架112可由如钛、钛合金、不锈钢、钴-铬合金、或钽的金属形成。选择地,支架112可由陶瓷或如聚醚醚酮(PEEK)或碳纤维的塑料形成。支架112可限定笼的周界,并且可至少部分地包含格子结构114和内缘116。

格子结构114可通过支架112暴露在笼110的至少一侧上。更优选地,格子结构114既暴露在笼110的顶侧又暴露在笼110的底侧。这种暴露可允许格子结构114直接地接触椎骨或想要锚固到笼110内的其它骨头。在各种实施例中,骨头生长可通过格子结构114上的涂层和/或植入件100的其它特性的使用而进一步被促进。例如,格子结构114可设置骨整合涂层或可为表面粗糙的以提供接触骨头的附加表面区域。
在各种实施例中,如图1中所图示的,笼110可包括从笼的顶面延伸至底面的贯穿孔。在这种实施例中,笼110可进一步包括布置在格子结构114和贯穿孔之间的内缘116。内缘116可由如钛、钛合金、不锈钢、钴-铬合金、或钽的金属形成。选择地,内缘116可由陶瓷或如聚醚醚酮(PEEK)或碳纤维的塑料形成。在各种可选择的实施例中,笼110可不包含这种贯穿孔。

紧固件130可由各种材料形成。在各种实施例中,紧固件130可由如钛、钛合金、不锈钢、钴-铬合金、或钽的金属形成。选择地,紧固件130可由陶瓷或如聚醚醚酮(PEEK)或碳纤维的塑料形成。
在各种实施例中,紧固件130可具有轴和扩大的头部。轴可以为带螺纹的。在各种可选择的实施例中,如那些螺钉由生物可吸收的材料形成的实施例,轴可为没有螺纹的,因为生物可吸收的材料在有流体的情况下可膨胀,以实现足够的固定。
图2和图3图示出典型的植入件100的顶视图和相应的侧视图。图示的典型的格子结构114为非任意的格子结构,这在图2中更清楚地可见。格子结构114包括多个堆叠层,每层包括格子材料。如图3中所图示,格子结构(未示出)可以或不暴露在笼110的所有侧上。
图4图示出典型的植入件100的分解图400。如图4所示,笼110可进一步包括紧固孔412以及三个线形构型414、416、418,所述三个线形构型414、416、418合作以形成笼对准结构。骨板422可进一步包括通孔422。
紧固孔412和通孔422可与紧固件130一起使用以将骨板120附接至笼110。同样地,通孔422可被定尺寸以允许紧固件130的轴来穿过,但是阻止紧固件130的头部穿过。紧固孔412可被定尺寸以接纳和啮合紧固件130的轴。在紧固件130的轴为带螺纹的那些实施例中,紧固孔412可以为互补螺纹的,以便啮合螺钉。在紧固件130的轴没有带螺纹的可选择的实施例中,紧固孔412可被定尺寸,以便当紧固件130的轴在未扩展的构造或扩展的构造(如在生物可吸收材料的螺钉的情况下)时,提供紧固件130的轴和紧固孔412之间的过渡配合或压入配合。在各种实施例中,笼110可以在没有骨板120或紧固件130的情况下使用。在这种实施例中,可以具有或不具有紧固孔412。

图9图示出在支撑材料914(以黑色示出)已经注入格子结构114之后的横切图7中的线A-A截取的典型笼110的另一截面图900。依照图7的实施例,支撑材料914可被定点注入在格子结构114表面上的许多选定位置中。可以看出,注入的支撑材料914可填充格子结构114的一些小孔,以便于骨头不能像进入未填充的小孔那样容易地进入已填充的小孔内生长。然而,支撑材料914的强度可对格子结构114增加足够的强度,以便于抵抗将造成挤压或切断格子结构114的由正常脊柱运动而产生的力。
图10图示出典型笼110的可选择实施例的截面图,其中一个或多个沟槽可被切开或以其它方式形成在格子结构114之内,用于接纳支撑材料。如所示,圆形沟槽1014可进入格子结构之内形成且用如PEEK的支撑材料填充。沟槽1014可为线形并且从笼的一侧延伸至另一侧,或沟槽1014可为完全的或部分的环形,围绕通孔和内缘弯曲且基本上与通孔和内缘同心。沟槽1014可与周围的格子结构114连通或可通过如由钛制成的实心壁的保持结构来与格子结构分离。将意识到的是各种可选择的沟槽可在格子结构114中形成。可选择的沟槽可具有不同于圆形的横截面,如菱形、正方形、长方形、椭圆形、新月形或半圆形的横截面。作为另一实例,可选择的沟槽可相对于笼从顶部延伸至底部,或从正面延伸至后面。可选择的沟槽还可与笼的内孔连通,像这样的沟槽延伸穿过内缘116,和/或可从外面另外能接近的(在用支撑材料填充之前),例如延伸穿过支架112的沟槽。进一步地,可选择的沟槽可为线形的且相对于笼的一个或多个表面以非平行角和/或非直角来形成。用于适合接纳支撑材料的沟槽的各种其它的实施例将为显而易见的。
图17图示出包括用于接纳支撑材料的沟槽1712、1714、1716、1718的可选择的笼1700的顶视图。如上文关于图10所述,沟槽1712、1714、1716、1718可形成为从笼1700的顶面延伸至底面。在各种可选择的实施例中,沟槽1712、1714、1716、1718可以不自始至终地延伸穿过这些顶面和底面,而改为,留下在沟槽1712、1714、1716、1718和笼1700的外表面之间的格子结构114的至少一层。沟槽1712、1714、1716、1718可与周围的格子结构114连通或可被如由钛制成的实心壁的保持结构所环绕。如PEEK的支撑材料可为注入成型或压入配合进入沟槽1712、1714、1716、1718来为笼1700提供想得到的支撑。
在如那些沟槽1712、1714、1716、1718没有自始至终地延伸穿过笼1700的顶面和底面的各种实施例中,笼1700可形成为两个分离件,如顶半部和底半部。在这种实施例中,支撑材料的预制杆可通过开口来压入沟槽1712、1714、1716、1718中,这些开口布置在组装之后将成为笼1700的内部处。此后,笼1700的两个件可环绕支撑材料的杆而被压一起,以形成包括支撑材料的单一笼1700。
图18图示出包括用于支撑材料的保持结构1812的笼的典型截面图1800。截面图1800可沿图7中的B-B线截取。保持结构1812可以为如钛或其它金属的实心壁。在这个实施例中,笼支架112可由PEEK形成。可以看出,保持结构1812形成穿过格子结构的圆,将在这个截面中的格子结构分成两个分离的格子结构1814、1816。通过将格子结构分成多个格子结构1814、1816,笼1800能将支撑材料接纳到一个这种格子结构1814、1816而使剩余的格子结构1814、1816保持无支撑材料,以便骨头可生长进入格子结构。例如,格子结构1814可用支撑材料填充而格子结构1816的小孔可基本上无支撑材料。
图19图示出包括用于支撑材料的保持结构1812的另一典型截面图1900。保持结构1812可具有比基本上的圆形或圆柱形更复杂的形状,这在沿图7中A-A线截取的这个截面图1900中可见。如所示,保持结构1812可形成靠近笼的顶面和底面的较大的圈。在较大的外圈和较小的内圈之间的过渡中,保持结构1812可形成架子且从而将格子结构分成四个分离的格子结构1814、1816、1914、1916。格子结构1814、1816可参考关于图18所述的两个格子结构。两个附加的格子结构1914、1916可分别靠近顶面和底面布置。如上文所述,四个格子结构1814、1816、1914、1916的任一组合可用支撑材料填充。例如,只有格子结构1814可用支撑材料填充,而格子结构1816、1914、1916可为没有填充支撑材料的以便用于骨头向内生长。外格子结构1914、1916可因而提供全部的或大部分的未分开格子结构的表面区域,来用于骨头向内生长。
植入件100的构造可使用3D打印技术至少部分地完成。例如,笼110可由钛逐层地、底部至顶部地构成,以便于支架112、格子结构114、以及内缘116为一体地连接。在各种实施例中,笼110可形成为单一件或待随后附接至彼此的多件。此后,凹槽418可被切入笼110的正面以及螺纹可被切入紧固孔412。PEEK或其它的支撑材料于是可注入或压入格子结构114的一个或多个小孔或(如果存在的)沟槽1014。
图11至图14图示出骨板120的各种视图。如以前所说明,骨板120可相对于笼110使用线形构型424、426、428对准,且随后通过将紧固件130插入通孔422而附接至笼110。线形构型424、426、428可形成在骨板120的底座1220上。底座1220可包括接触笼110正面的骨板120的表面,以及可把骨板120的剩余部分与笼110隔开,以便当植入件100固定于病人中时,骨板120可布置在脊柱的外面。骨板120可附加地包括被定位且定尺寸以接纳接骨螺钉(未示出)的螺丝孔1122、1124,所述接骨螺钉随后附接至椎骨或其它骨头。在各种实施例中,接骨螺钉和螺丝孔可为能操作的以提供固定的、半约束的、和/或动态的固定。

在各种实施例中,骨板的底座可为成角度的,以便于骨板在附接至笼110时相对于笼以角度定向。如图15中所图示,成角度的骨板1520可包含成角度的底座1522。成角度的底座1522靠近骨板的顶侧比在骨板的底侧更进一步延伸。如可理解的,当附接至笼110时,由于该可选择的楔形底座结构,成角度的骨板1520能相对于笼110以如15°角度布置。在各种可选择的实施例中,可使成角度的底座(未示出)在骨板内附加地或选择地至少部分凹进。例如,不是使靠近顶侧的端向外进一步延伸,而是使成角度的底座的端在骨板本体内凹进。在这种实施例中,靠近底侧的相对端可与骨板的余下部分齐平(如图15中所示),或可相对于成角度的底座1522的顶侧如所述地从板向外延伸。
将理解的是可提供各种可选择的角度。例如,楔形底座靠近骨板的左侧比骨板的右侧可更进一步延伸。进一步地,依照本公开的提供植入件的手术套件可提供多个具有不同的成角度的底座的骨板(和/或多个具有不同的尺寸的笼),以便于外科医生可选择具有想要角度的骨板来用于手头操作。例如,手术套件可包含零度骨板、两度骨板、四度骨板以及六度骨板。各种其它的角度对本领域中的技术人员将为显而易见的。
在各种实施例中,可通过分离的楔形部件(未示出)提供角度,而不是与骨板一体的。楔形部件可配置为布置在笼110和骨板120之间,以提供想要的角度。在各种实施例中,楔形部件可包含配置为至少部分地啮合笼对准结构和/或骨板对准结构的附加对准结构。在这种实施例中,套件可设置有具有各不相同的角度的多个楔形件。
图16图示出脊柱植入件1600的可选择的实施例。可选择的脊柱植入件1600可在多方面不同于典型脊柱植入件100。首先,可使笼1610的支架1612有差别地成形,且可不包含用于接纳皮层边缘的凹处。此外,格子结构1614可不与通孔连通,因为内缘1616可从笼1610的顶部完全地延伸至底部。
紧固件1630可不带螺纹的,且可改为由在有水的情况下膨胀的生物可吸收材料形成。同样地,笼1610的紧固孔1642可不设置任何螺纹。而且,骨板1620的通孔1652可以不包含不准紧固件1630头部通过的内脊部;改为,通孔1652可被定尺寸以接收贯穿其的紧固件1630的头部,并且还可依赖于紧固件的膨胀以实现固定。
可选择的植入件1600的相应的对准结构还可不同于典型植入件100。可以看出,笼1610的全部三个线形构型1644、1646、1648可为脊部,而骨板1620的全部三个线形构型1654、1656、1658可为凹槽。进一步地,脊部1648可靠近在笼1610正面上的中心平面被中断。对典型植入件100和可选择的植入件1600的各种其它实施例将为显而易见的。
尽管各种典型的实施例具体参考其确定的典型特征而已经详细地描述,但是应该理解的是本发明有其它的实施例,且其细节在各种明显的方面能够修改。可实现变化和修改而保留在本发明的精神和范围内的,这对那些本领域的那些技术人员为容易显而易见的。因此,前述的公开、说明以及数字仅仅是用于说明性的目的而没有以任何方式限制本发明,本发明只通过权利要求限定。

Claims (14)

1.一种植入件,包括:
笼110;以及
骨板120,其配置为附接至所述笼110和至少一个骨头,
其中所述骨板120包括骨板对准结构424、426、428,其配置为与所述笼110相配合,以在所述骨板120与所述笼110适当地对准时提供指示。
2.根据权利要求1所述的植入件,其中:
所述笼110被定尺寸以插入两个椎骨之间,并且
所述至少一个骨头包括至少一个椎骨。
3.根据权利要求1-2中任何一项所述的植入件,其中:
所述笼110进一步包括笼对准结构414、416、418,并且
配置为与所述笼110相配合的所述骨板对准结构424、426、428包括配置为与所述笼对准结构414、416、418相配合的骨板对准结构424、426、428。
4.根据权利要求3所述的植入件,其中:
所述骨板对准结构424、426、428包括下列中的至少一个:骨板凹槽424、426和骨板脊部428;
所述笼对准结构414、416、418包括下列中的至少一个:笼凹槽418和笼脊部414、416;并且
在所述骨板与所述笼适当地对准时的指示包括下列中的至少一个:
所述骨板脊部428位于所述笼凹槽418内,以及
所述笼脊部414、416位于所述骨板凹槽424、426内。
5.根据权利要求1-2中任何一项所述的植入件,其中:
所述骨板对准结构424、426、428包括第一线形构型和正交于所述第一线形构型的第二线形构型;
所述第一线形构型包括下列中的至少一个:第一凹槽424、426和第一脊部428;并且
所述第二线形构型包括下列中的至少一个:第二凹槽424、426和第二脊部428。
6.根据权利要求1-2中任何一项所述的植入件,其中所述骨板120配置为附接至所述笼110,以便所述骨板120相对于所述笼110以非零的角度定向。
7.根据权利要求6所述的植入件,其中所述骨板120包括接触所述笼110的楔形底座1522。
8.根据权利6所述的植入件,进一步包括配置为布置在所述笼110和所述骨板120之间的楔形件。
9.一种手术套件,包括:
笼110,其被定尺寸以插入两个椎骨之间;
第一骨板120,其配置为附接至所述笼110,以便所述骨板120相对于所述笼110以第一角度定向;以及
第二骨板120,其配置为附接至所述笼110,以便所述骨板120相对于所述笼110以第二角度定向,
其中所述第一角度不等于所述第二角度。
10.根据权利要求9所述的手术套件,其中所述第一骨板120包括接触所述笼的楔形底座1522。
11.根据权利要求9所述的手术套件,其中所述第一骨板120包括骨板对准结构424、426、428,其配置为与所述笼110相配合,以在所述骨板120与所述笼110适当地对准时提供指示。
12.根据权利要求11所述的手术套件,其中:
所述笼110进一步包括笼对准结构414、416、418,并且
配置为与所述笼110相配合的所述骨板对准结构424、426、428包括配置为与所述笼对准结构414、416、418相配合的骨板对准结构424、426、428。
13.根据权利要求11所述的手术套件,其中:
所述骨板对准结构424、426、428包括下列中的至少一个:骨板凹槽424、426和骨板脊部428;
所述笼对准结构414、416、418包括下列中的至少一个:笼凹槽418和笼脊部414、416;并且
在所述骨板与所述笼适当地对准时的指示包括下列中的至少一个:
所述骨板脊部428位于所述笼凹槽418内,以及
所述笼脊部414、416位于所述骨板凹槽424、426内。
14.根据权利要求11所述的手术套件,其中:
所述骨板对准结构424、426、428包括第一线形构型和正交于所述第一线形构型的第二线形构型;
所述第一线形构型包括下列中的至少一个:第一凹槽424、426和第一脊部428;并且
所述第二线形构型包括下列中的至少一个:第二凹槽424、426和第二脊部428。
