Abstract

背景技术
低血糖事件通常与一系列的症状相关,如头晕、协调障碍、精神错乱、和改变的行为、增加的和不规则的心率、增加的排汗、震颤和沉重的呼吸。未经治疗的低血糖事件可能导致极端低血糖,其可能导致痉挛、昏迷、脑损伤甚至死亡。因此,用于检测低血糖事件的症状的非侵入性方法和系统的开发正在进行中。

具体实施方式
根据本发明,提供了用于检测低血糖事件的改进的非侵入性的系统和方法。本发明的方法提供了显著降低的错误警报率而保持了在其早期检测低血糖事件的高灵敏度。本发明的系统适于并设置用于监测以下生理参数:i、心率;ii、运动水平;iii、血管收缩和皮肤温度;和iv、皮肤的电阻。根据本发明的方法将这些生理参数的每一个的水平的变化记录并与各自的范围比较。在任意下列情况下确定低血糖事件的症状:a、心率增加,其达到并超过各自的预定阈值;b、运动水平增加,其达到或超过各自的预定阈值;c.皮肤的电阻降低,其向下达到各自的预定电阻水平;以及d、血管收缩和/或皮肤温度降低,其超过各自的预定冷却速率。认为其中测定出任何低血糖事件的症状,尽管事实上低血糖事件尚未发生的情况是错误警报。根据本发明的方法降低错误警报率是通过当至少三种不同症状被同时测定时诊断低血糖事件而完成的。然而,在这种情况下,用于确定症状的条件并不是很严格。即,替代了单独地将每个生理参数的水平的变化数量与各自的阈值相比较,第一次评估了这种偏差的组合并且将其值与不同的预定阈值进行比较,如下文进一步描述的。

监控任何上述的生理参数提供用于得出监测的参数各自的瞬时水平以及导出各自的基础水平和/或其各自的变化率。如已知的通过将预定数量的样本平均化进行监测的参数的瞬时水平和/或变化率的得出。可以利用涉及相对较少数量的部件的移动窗口技术进行平均化;和/或利用涉及沿着在下文中称为测量时间间隔的相对较小的时间间隔接收的样品的FIFO滤波器进行平均化。这些测量时间间隔分别是几秒至几分钟。根据本发明通过将沿着相对长的时间间隔(在下文中称为基础时间间隔)测量的瞬时水平平均化以及将沿着几十(a few dozens of)长度的测量间隔,如几十分钟 的范围平均化,得出各自的基础水平和/或变化率。根据本发明的方法,通过重复更新每个生理参数的特征统计和统计动差,如平均值和标准偏差,持续进行统计分析。这样的更新随着本发明的系统使用的每个和全部时间进行。因此,在每次日常操作的初始阶段,只要各自的基础水平还没有及时稳定,可以使用长期平均值替代各自的基础水平。将目前更新的基础水平、和特征统计、以及长期统计数据存储于系统处理器的存储器中以用作与瞬间水平进行比较的各自的范围。此外,根据本发明的另一个实施方式,只要任何生理参数的瞬时水平超过基础水平大于各自的长期标准偏差大小的预定部分,不管其他生理参数的测量水平,就诊断为低血糖事件。
以预定的重复率(其等于测量时间间隔的长度的倒数)反复测量皮肤温度和/或环境温度的瞬间水平。在当前测量的瞬间皮肤温度与各自的基础水平相比以快于环境温度的下降速率和/或以超过预定阈值的速率下降的情况下,根据本发明的用于确定皮肤温度降低的症状的方法的实施方式,认为这样的温度下降对低血糖事件具有指示性。
将皮肤电阻的瞬时水平与如通常记录在手册中的各自的范围进行比较。可替换地,通过进行适当的用于测定典型的电阻水平统计分析实验性地得出这样的范围,这些电阻水平分别对应于不同排汗水平下皮肤的电阻。在当前测得的电阻与此类范围相比下降并达到预定阈值时,确定与排汗相关的低血糖事件的症状。当环境温度低于预定温度时,这种排汗不太可能是由于环境温度引起的。因此,认为这种达到电阻阈值是疑似低血糖事件的排汗症状。
沿着每个轴以从几十个每秒周期(CPS)至多至比如数十个(a number of dozens of)CPS的范围内的采样频率对三轴加速器的信号重复采样。根据本发明的方法的优选实施方式得到这些信号的谱图,如以下描述的。首先,在每一秒和所有秒内对沿着预定水平的加速(沿着一秒发生)的三个轴中的任一个的到达的数量计数。然后系统对以至少发生预定数值的每秒次数(比如每秒十几次达到)的该加速水平的这种达到的那些秒进行时间 标记并记录。根据本发明,认为这些秒是运动活跃秒。然后在下文中将包括在具有预定长度的测量时间间隔(如给定的分钟数)内的运动活跃秒的数值称为活跃秒的测量数,对其值进行时间标记和记录。将活跃秒的测量数值的值沿基础时间间隔平均,以得出包括在测量时间间隔中的运动活跃秒的基础数值。基础时间间隔的长度等于前述测量时间间隔的长度乘以,比如,十几倍。当包括在当前测量时间间隔内的活跃秒的数值超过基础值至少给定数值时,确定为与增加的运动性活性相关的低血糖事件的症状。各种阈值,即确定达到,达到的数值超过被视为运动活跃秒的秒,以及用于确定症状的阈值所考虑的加速水平,只要包括在测量时间间隔内的活性秒的数值超过该阈值,都是在胰岛素依赖型糖尿病患者参加的试验中通过对测量进行统计分析而实验得出的。
可替换地,周期运动如心率和/或机械颤动的谱图可以如通过进行DFT计算而计算出。使用者的突然运动是典型的以预定频率为特征的震颤。同样地,呼吸速率可以从这种计算出的用光检测器测定的脉冲的振幅的谱图得出。通过从各自的基础量减去相关频率水平的量并将得到的差与各自的阈值水平进行比较,提供检测震颤以及检测呼吸速率的增加。因此,具有三轴加速计和光检测器的系统能够用于检测增加的运动的症状、震颤和增加的呼吸速率,以及监测血管收缩和检测与皮肤冷却相关的事件。根据本发明,光检测器与各自的安装在系统处理器中的软件程序结合替代由如利用4个独立的传感器进行的4个不同的参数的水平的测量。这种光检测器被并入并用作多功能传感器的用于检测低血糖事件的系统的实施方式的变型是根据本发明的。这种多功能传感器在下文中分别视为两个、三个和/或四个不同的传感器的组(bunch),其中每个独立地连接至系统处理器。
通过将瞬时呼吸速率平均化得出基础呼吸速率。如已知的,考虑到其振幅用如光检测器测量的血液脉冲的DFT分析的谱图的较低频率,得出 瞬时呼吸速率。将偏离各自的基础速率的瞬时水平的水平与校准范围相比较。当此偏差超过预定阈值时,确定为重呼吸的症状。
根据本发明的方法的实施方式,在所有上述的症状同时出现的情况下检测低血糖事件。可替换地,评估与至少一些上述参数相关的得分并且当它们的总和超过各自的预定阈值水平时诊断为低血糖事件。明显地,在少数几种症状被同时测定的情况下,如果保留相同的检测灵敏度,错误警报率将增加。
以下是根据本发明的优选实施方式的用于诊断低血糖事件的方法的描述。该方法适合于其中至少监测三个生理参数的情况,其中这三个选自下列生理参数:(i)运动性活动、(ii)心率、(iii)呼吸速率、(iv)皮肤温度或优选地不评估血管半径的血管收缩、以及(v)皮肤电阻。
首先,对各自被监测的生理参数(除了呼吸传感器之外)在每个和所有测量周期中测量的瞬间水平与基础水平之间的差的水平打分。根据本发明的方法,通过将任何差的各自的最高水平对应(mapped)到1进行打分。(同样地,对于考虑的生理参数,任何测量的瞬时水平,其高于此最高水平,也对应到1。)等于0或为负值的任何水平的这种差对应到0。差异的最大水平之间的整个范围线性对应到开放区间(0,1)。皮肤电阻的打分水平通过将各自的电阻的水平的范围对应到封闭区间中[0,1]中进行,如下:任何等于或超过电阻的预定最大水平的电阻,如250ΚΩ,对应到0。任何等于或低于预定最小电阻的电阻,如100ΚΩ,对应到1。这两个极限之间的所有的水平根据方程S=1-(R-100)/150对应到开放区间(0,1),其中S是预定的电阻的最大和最小的水平得到的分数;R是以ΚΩ给出的测量的电阻的水平。通过对大量的糖尿病受试者参与的实验的结果的统计学平均化得到最小和最大水平。与除皮肤电阻水平以外的生理参数相关的差如下进行打分:首先,选择瞬时水平与基础水平之间的差的范围,然后,将该范围的上端对应到1。并且为负或等于0的差对应到0。任何中间的水平通过 将其值除上限的值而分别进行打分。因此,所有的中间水平对应到开放区间(0,1)。
将所有的监测参数这样得出的分数相加在一起成为组合分数。该组合分数进一步与各自的阈值水平相比较。当组合分数超过预定阈值水平时,检测到低血糖事件。在环境温度过高可能引起使用者排汗的情况下,优选避免使用排汗传感器并且组合分数的阈值相应降低。这种方法提供了保持的灵敏度水平,然而,在另一方面,错误警报率会稍微增加。在环境温度以高于预定冷却速率的速率降低的情况下,根据本发明的方法,优选避免使用皮肤温度传感器一段时间并等待直到环境温度稳定。在此情况下对于组合分数使用不同的不考虑与皮肤温度相关分数的预定阈值水平。这种方法提供了系统的灵敏度水平的保持,然而错误警报率会稍微增加。
具体实施方式
实施例1
胰岛素依赖型糖尿病成员的组的实验,每人使用根据本发明的实施方式的用于检测低血糖事件的系统,此处参考图1至图5进行描述。运动性活动、心率、皮肤温度、皮肤电阻、和悬浮在血液中的葡萄糖的水平的时间曲线图,其所有均来自于示例性的使用者,分别示出于图1至图5中。每个图中示出的水平轴表示以小时测量的开始于夜间睡眠起始的时间。在任何图示中使用相同长度的时间间隔,所有间隔等于5分钟。使用的系统构造为如佩戴于使用者手腕上的腕表。系统包括:用于测定心率的光传感器;提供至用于测量皮肤温度的温度传感器的热敏电阻;用于测量皮肤电阻的传感器和如上文所描述的以测量运动性活动的模式操作的三轴加速计。商用Dexcom Seven Plus型血糖仪持续地侵入性监测悬浮在使用者血液中的葡萄糖的实际水平。图1的纵轴刻度无量纲。曲线10是包括在任何测量时间间隔的运动活跃秒的数量的时间曲线。峰12、14、16可以解释为给出确定的症状。即,其中与基础计数相比,当前发生活跃秒的数量 的显著变化的情况。由于与从测量起始时间开始流逝的时间相比,稳定时间可能较长,所以提供得出的峰12的基础水平的值可能是有问题的。然而,点14指示了与运动性活动相关的低血糖事件的症状,该运动性活动与另外三个生理参数的症状同时发生于同一时间点(该时间点是2点45分)。曲线20给出了脉搏速率的时间曲线,纵轴的单位是每分钟的心跳数,在每一个和全部测量时间间隔内反复进行脉搏速率的测量。通过指示与基础速率相比当前测量的脉搏速率的变化为每分钟约9次,位于靠近点22的峰相当于心率增加的症状。事实上,点22指示与增加的脉搏速率相关的低血糖事件的症状,增加的脉搏速率与由点14指示的症状同时发生。随后的峰的直到点24的连续点相当于最多每分钟3次脉动的改变。曲线30给出了皮肤温度的时间曲线,以摄氏度为单位测量。点32指示确定为其中与皮肤温度下降相关的低血糖事件的症状的状态。沿着该曲线存在另外的表现出温度降低的点,然而这些另外的点中没有与运动性活性或心率各自的症状相符合的点。因此,根据本发明的方法任何这些另外的点不能提供诊断低血糖事件,即使存在属于皮肤电阻的时间曲线的各自的点,其分别给出增强的排汗的确定症状。图4的刻度以千欧姆测量;曲线40给出了皮肤电阻的时间曲线。点42指示了以时间计的点,其中根据本发明的方法的优选实施方式的组合分数用于诊断上文中描述的低血糖事件。曲线30和40与曲线10、20和50相比振荡更显著。事实上,皮肤温度在夜间随着时间正常改变几摄氏度。盖有毯子和/或将温度传感器和该传感器附着于其上的器官暴露在周围空气中,可以容易地引起这样的变化。因此,根据本发明的方法,优选监测呼吸速率而不是皮肤温度,从而提高保持高灵敏度和相对较低的错误警报率的机会。
此外,排汗可以与低血糖事件独立发生。然而,大多数低血糖事件与大量排汗相关。因此,缺乏排汗可以需要更严格的用于诊断低血糖事件并保持允许的错误警报率的条件。
曲线50给出了悬浮在该特定使用者的血液中的葡萄糖水平的实际时间曲线。点52是该时间曲线在时间上对应点14、22、32、42的特定点。事实上,65mg/ml的葡萄糖的水平相当于经历显著时间的低血糖事件的使用者(然而,为了充分披露,应指出连续性血糖仪在相对低的葡萄糖浓度下校准较差,即与低血糖事件对应的水平可能百分之几十以上是错误的,因为在这样的水平的校准几乎是不可能的)。
Claims (4)
1.一种用于非侵入性检测低血糖事件的系统,所述系统包括:
光检测器、皮肤电阻传感器和加速计;以及
处理器,设置以:
从所述光检测器、皮肤电阻传感器和加速计接收测量结果;
从所述测量结果得出生理参数,所述生理参数选自由心率、运动性活动、外周血管收缩和/或皮肤温度、和皮肤电阻组成的组,其中所述心率和所述外周血管收缩和/或皮肤温度来源于从所述光检测器获得的测量结果;
将得出的所述生理参数的每一个与各自的范围相比较,
对得出的所述生理参数的每一个分配各自的分数;以及
当仅由得出的所述生理参数的分配的所述各自的分数计算出的组合分数达到预定值时,确定低血糖事件,
其中,所述心率、运动性活动、外周血管收缩、和皮肤电阻中的每一个的所述范围是预定的或源于实验的。
2.根据权利要求1所述的系统,进一步包括设置以在确定低血糖事件时被激活的警报。
3.根据权利要求1所述的系统,其中,所述光检测器、皮肤电阻传感器和加速计并入单一的可配戴单元中。
4.根据权利要求3所述的系统,其中,所述单一的可配戴单元是腕表。
