技术领域
本发明涉及一种多价疫苗,尤其涉及包含去活化的肠病毒抗原及选自:去活化流感病毒;去活化日本脑炎病毒;或去活化流感病毒混合去活化日本脑炎病毒的抗原的多价疫苗。
背景技术
肠病毒属于小RNA病毒科(Picornaviridae),为一群病毒的总称,在1997年以前,已知而被分类的肠病毒共有小儿麻痹病毒(Poliovirus)共3型(1至3型)、克沙奇病毒(Coxsackievirus),含23种A型(A1至A22型,A24型)及6种B型(B1-B6型)、伊科病毒(Echovirus)共30型(1至33型,但8、10及28型除外)及肠病毒(Enterovirus)(68型~)等60余型,近年来又陆续发现多种型别,依据基因序列分析结果将的重新归类,分为人类肠病毒A、B、C、D(Human enterovirus A、B、C、D)型,其中肠病毒71型被归类于人类肠病毒A型。
在所有肠病毒中,除了小儿麻痹病毒之外,以肠病毒71型(EnterovirusType 71)最容易引起神经系统的并发症,此病毒是在1969年美国加州的一次流行中首次被分离出来,当时引起很多无菌性脑膜炎与脑炎的病例。此后包括澳洲、日本、瑞典、保加利亚、匈牙利、法国、香港、马来西亚等地都有流行的报告,台湾在十几年前也曾经流行过,可见此型肠病毒的分布是全世界性的。
肠病毒临床上可以引起多种疾病,其中很多是无症状,有些则只有发烧或类似一般感冒症状,但有些则会出现特殊的临床表现,如手足口病(hand-foot-mouth disease)、泡疹性咽峡炎(herpangina)、无菌性脑膜炎、病毒性脑炎、肢体麻痹症候群、急性出血性结膜炎(acute hemorrhagicconjunctivitis)、婴儿急性心肌炎及成人心包膜炎、流行性肌肋痛、急性淋巴结性咽炎(acute lymphonodular pharyngitis)、发烧合并皮疹(febrile illness withrash)等。
根据临床统计数据,较常见且致死率最高的是肠病毒71型,五岁以下的幼童若感染肠病毒,会有较严重的并发症,大约有1000~10000分之一的致死率,由于人体免疫力的基因非常复杂,所以无法确实了解为何五岁以下的幼童有较高的致死率;另外,病例也有地区性集中的情况,所以居住环境应该也是肠病毒致病的重要因素之一。目前仍无疫苗可以预防肠病毒感染,感染后亦无特效药,只能加强个人卫生保健。目前已经有多家公司或研究机构取得政府许可,开始进行人体临床试验。然而,至今仍无前案已揭示将肠病毒混合其他抗原,尤其是,将肠病毒、流感病毒、日本脑炎病毒等抗原混合制成多价疫苗的构想及具体提案。
在台湾地区,关于季节型流感与新型流感的预防,目前是依据每年「流感疫苗接种计划」实施接种。然而,根据历年接种季节型流感疫苗的统计数据,以及2009年新型流感疫苗接种的经验,家长会带家中婴幼儿施打流感疫苗的意愿实在很低,因而造成流感防疫上的困难。
现行而定流感疫苗,仍以混合最有可能引发大流行的三株病毒株,并且每年更换病毒株为主。未来针对病毒变异性的问题,瑞士诺华(Novartis)药厂、荷商葛兰素史克(GSK)药厂、法国赛诺菲巴斯德(Sanofi-Pasteur)等国际药厂皆针对新型佐剂的研发作为主轴,于候选疫苗当中加入能启动先天免疫反应的免疫佐剂,从免疫反应的源头开始做起,进而达到对于新、旧病毒交叉保护的效果。
至于日本脑炎疫苗的接种,在实施上必须受到的限制非常多,例如,必须与含有佐剂的疫苗间隔一个月以上接种,以免发生过敏性「接种后脑炎」;此外台湾每年集中于3月至5月接种,许多小宝宝因此而延误了预防注射;而且施打的次数也最多(儿童接种时程达四次之多:开始间隔两周接种两剂疫苗,一年以后追加一剂,国小一年级再追加一剂)。究其原因在于,现行疫苗源自鼠脑组织萃取病毒,精制后以甲醛不活化处理而制得,且其纯化过程需使用硫酸鱼精蛋白(protamine sulfate),故残余的少量不纯物若与佐剂作用,就会不当强化其过敏反应。
再者,经由感染鼠脑所制得的日本脑炎抗原,受限于无法单独与其他抗原组合,而因此尚无法用于制备多价疫苗。然而,本发明所采用的疫苗技术为,藉由细胞培养来产制日本脑炎病毒抗原,经福尔马林不活化之后,再使用benzonase核酸酶纯化,如此可有效使所产生的副作用减小,并能克服日本脑炎病毒抗原与其他抗原疫苗一起接种的问题。
发明内容
本发明的实施例提供一种新颖抗肠病毒、流感病毒及/或日本脑炎的混合型多价疫苗。
为达到上述目的,本发明的实施例采用如下技术方案:
一种多价疫苗组成物,包含去活化的肠病毒抗原及至少一种选自:(1)去活化流感病毒;(2)去活化日本脑炎病毒;或(3)去活化流感病毒混合去活化日本脑炎病毒的抗原,及医药上可接受的载剂。
本发明所提供的一种多价疫苗组成物,能避免各类抗原间的干扰现象,产生良好的肠病毒、流感与日本脑炎的免疫抗体,并减少须多次注射不同抗原疫苗接种的次数。

图5为以去活化EV71/H5N1组合疫苗免疫后,于小鼠中所引发的(a)EV71-专一性VN,及(b)H5N1-专一性VN抗体,从经免疫的小鼠收集血清样本,并藉由免疫分析测定抗体力价,所示数据为每组六只小鼠所得的几何平均值加标准偏差,*p<0.05:与EV71+H5N1/PBS的实验组相较,#p<0.05:与EV71+H5N1+JEV/PBS的实验组相较。
图6为以无佐剂或添加铝盐(alum)、PELC或JEV的去活化EV71/H5N1组合疫苗(包含EV71去活化病毒0.2μg与H5N1去活化病毒0.5μg HA)免疫后,于小鼠脾脏细胞中所引发的细胞激素反应,于第12周施予追加剂量,并于七天后汇集取自每组六只小鼠的脾脏以纯化出T细胞,于浓度去活化病毒刺激培养三天,利用ELISpot方法测定T细胞的IFN-γ,IL-4分泌反应,所示数据为三重复组所得的平均值加标准偏差。
图7为小鼠免疫追加H5N1去活化病毒疫苗之后,每组3只小鼠取其第7天的脾脏以纯化出2x 105个T细胞,于浓度0.5μg HA/mL的去活化病毒刺激培养三天,利用ELISpot方法测定T细胞的IFN-γ分泌反应,所示数据为三重复组所得的平均值加标准偏差。
具体实施方式
下面结合附图对本发明实施例新颖抗肠病毒、流感病毒及/或日本脑炎的混合型多价疫苗进行详细描述。
应当明确,所描述的实施例仅仅是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。
在本发明中所使用的“多价(multivalent)疫苗”意指包含同一种细菌或病毒的不同亚型或血清型,或是混合其他来自不同细菌或病毒株抗原的疫苗,以使在一次施打或口服时,能同时预防多种疾病或能防治不同细菌或病毒株。例如,根据注射式白喉、破伤风、非细胞性百日咳、不活化小儿麻痹、b型嗜血杆菌五合一疫苗,预防上述等传染病。
“佐剂(adjuvant)”是疫苗内的一种成分,它可以加强疫苗内抗原所引发的免疫反应,达到比较好的效果。利用佐剂的添加,可以降低疫苗内抗原的需要量,例如,在新型H1N1流感疫苗的制造上,可以帮助将固定的抗原量制造成较多的疫苗剂数,在短时间内制造出大量的疫苗,及时提供所需。
常用的佐剂大致可区分成四类:(一)无机佐剂,利用氢氧化铝或明矾等与抗原混合制成;(二)有机佐剂,多半利用微生物及其产物如百日咳杆菌、细菌细胞壁的萃取物等;(三)合成佐剂,采人工方式合成得,例如双股多核苷酸链、异丙肌苷等;及(四)油剂,此类多半成油状,如矿物油、花生油等。
其中,铝盐已广泛被认为是一种方便使用且安全的佐剂。但是,由于其佐剂功效、异源性及再现性不理想,而在使用上受到限制。之后研发出三种具有潜力的水包油类佐剂:MF59(80/85-stabilized squaleneemulsion,Novartis)、AS03(80-stabilizedsqualene/α-tocopherol emulsion,GSK)与AF03(polyoxyethylenecetostearyl ether/85-stabilized squalene/mannitol emulsion,Sanofi-Pasteur)。乳液具有较铝盐更佳的佐剂功效,WHO建议使用水包油类佐剂做为减低抗原使用的工具。
具有CpG基序(motif)的寡脱氧核苷酸(oligodeoxynucleotides,ODN)可以刺激动物,包括牛、猪、羊等大型家畜动物的淋巴细胞及抗原呈现细胞的活性,增加树状细胞(DC)的抗原呈现活化及成熟作用,促进免疫系统对特定抗原趋向Th1细胞反应。而本发明则另意外发现,于有或无佐剂的存在下,加入日本脑炎疫苗之后,能更加增强流感与肠病毒的抗原免疫效价,显示去活化本脑炎抗原具有类似佐剂的效用。
实施例
本发明的其他特色及优点将于下列实施范例中被进一步举例与说明,而该实施例仅作为辅助说明,并非用于限制本发明的范围。
实施例一:多价疫苗的制备
疫苗抗原组成
本实例所使用的病毒粒子皆以细胞培养方式获得,疫苗组成物则是经福尔马林去活化的全病毒疫苗。各抗原的来源及制备如下:(1)去活化的H5N1亚型流感病毒疫苗(NIBRG-14),衍生自重组(reassorted)H5N1疫苗株A/越南/1194/2004,并于无血清培养基中根据微载体(microcarrier)细胞培养技术,于Madine-Darby犬肾(MDCK)细胞中进行增殖;(2)去活化的H1N1亚型流感病毒疫苗(NIBRG-121),衍生自重组(reassorted)H1N1疫苗株A/加利福尼亚/7/2009,并于无血清培养基中根据转瓶(roller bottle)细胞培养技术,于Madine-Darby犬肾(MDCK)细胞中进行增殖;(3)去活化肠病毒EV71疫苗(EV71vac)衍生自71型肠病毒株E59,并于培养基中根据无血清灌流生物反应器(perfusion bioreactor)细胞培养技术,于Vero细胞中进行增殖;(4)去活化日本脑炎病毒疫苗(JEV)衍生自日本脑炎病毒株Kitasato Beijing-1,并于无血清培养基中根据转瓶(roller bottle)细胞培养技术,于Vero细胞中进行增殖。
佐剂制备
小鼠CpG去甲基化寡脱氧核苷酸【5′-TCC ATG ACG TTC CTG ACG TT-3′,SEQ ID NO.1】由Invitrogen Taiwan Ltd合成得,且以每剂量含10μg溶于候选疫苗当中。铝盐(磷酸铝)悬浮液以每剂量含300μg溶于酸性缓冲溶液(pH=6)中。PELC乳状液为一种经由85(山梨醇酐三油酸酯,Sigma-Aldrich,Steinheim,德国),以及生物可吸收式高分子PEG-b-PLACL所乳化而成的复合相奈米乳液。其中,PEG-b-PLACL的整体分子量为7,000道耳吞,其亲水端部份(75wt-%)为水溶性聚乙二醇(PEG);疏水端部份(25wt-%)为具有生物可分解性质的聚乳酸与聚己内酯的共聚物(PLACL)。
PELC乳状液的制备,使用均质机(PT3100 polytron;Kinematica AG,瑞士)以每分钟6,000转的转速,将含有PEG-b-PLACL高分子的水溶液(120毫克高分子样品溶于0.8毫升PBS缓冲溶液)与油剂(0.165毫升85溶于0.935毫升角鲨烯)均质乳化5分钟。PELC乳状液所配制的疫苗,取200μL储备乳状液再分散于1800μL PBS缓冲溶液中,并于注射前以试管旋转器(Labinco LD-79,纽西兰)于5rpm进行混合至少1小时。再将去活化的病毒及/或CpG分别加入该缓冲溶液当中。
实施例二:疫苗接种及免疫效价分析
五周龄雌性BALB/c小鼠使用前先于动物房适应至少一周。进行疫苗接种时,将候选的组合疫苗(多价疫苗,有或无添加佐剂)以一次剂量或二次剂量时程,经肌肉内注射投药予实验小鼠。根据颌下动脉采血,从经免疫小鼠收集血清样本,并以酵素联结免疫吸附分析(ELISA)、血球凝集抑制分析(HI)及病毒中和分析(VN)测定抗体效价。统计学的显著差异(p<0.05)通过数值经对数转换再进行双尾Student′s t-试验而测得。以下叙述各例举性疫苗组合的免疫效价分析结果。
H5N1与H1N1组合疫苗
于本实验首先测试各别施打单价流感疫苗(H5N1或H1N1)是否会产生交叉中和抗体(cross-neutralizing antibodies),或者有必要施打去活化组合疫苗,始能产生对抗该二不同亚型流感病毒之中和抗体。如图1所示,接受两次H5N1去活化病毒疫苗剂量的小鼠,无法产生对抗H1N1的交叉中和抗体。同样,接打H1N1病毒的小鼠无法产生对抗H5N1的交叉中和抗体。而,混合与去活化病毒制成的疫苗组合物,不论使用铝盐或乳状液做为佐剂,皆能产生对抗此两种去活化流感病毒所诱导的病毒中和抗体。此外,本实验的结果亦显示,添加PELC乳状液的多价疫苗组合物,随着抗原组成剂量的增加,将不会影响各别去活化流感病毒所能诱发的免疫效果(p~1)。
H5N1与EV71组合疫苗
于本实验,将去活化H5N1病毒与去活化EV71肠病毒疫苗组合制成多价疫苗,并研究所得组合疫苗的免疫生成性,结果如图2所示。单独以去活化EV71病毒(无组合抗原)并未产生H5N1-专一性抗体免疫反应,甚至于有添加佐剂的情况亦然。而当去活化H5N1病毒并入去活化EV71病毒疫苗时,就会产生抗-H5N1专一性免疫反应。
本实验也发现,PELC乳状液能显著增加H5N1-专一性抗体免疫反应(血液凝集抑制,HI>40)。至于EV71专一性反应,无添加佐剂的疫苗无法诱发有效的反应,而PELC乳状液能增强-EV71专一性免疫反应。由PELC乳状液配制得的疫苗,其所诱发的抗-EV71专一性免疫反应,在并入病毒之前与之后并无显著差异,显示PELC乳状液能避免抗原竞争现象。
本实验亦将复合佐剂(CpG与乳状液及CpG与铝盐)添加于,去活化H5N1流感病毒与去活化EV71肠病毒的组合疫苗,并于施打后观察小鼠的抗-EV71专一性免疫反应。结果如图3所示,接受添加PELC/CpG所调配得的疫苗组合物的小鼠,能一并产生良好的流感与肠病毒免疫抗体,且相较于接打添加CpG及添加铝盐/CpG的疫苗组合物的动物组,添加CpG与乳状液复合佐剂,可显著强化小鼠对于流感与肠病毒混合疫苗的免疫效果。
H5N1、EV71与JEV组合疫苗
本实验将小鼠接种以去活化H5N1流感病毒、去活化EV71肠病毒与JEV去活化日本脑炎疫苗组合制成多价疫苗,并观察其于小鼠中诱发的免疫反应。结果如图4所示,在接受单一注射后第12周,本发明的疫苗组合物可使得免疫小鼠同时产生对抗H5N1、EV71及JEV的抗原专一性免疫反应。而且发现,添加佐剂后的免疫反应显著提升(p<0.05)。
为进一步评估添加去活化日本脑炎病毒,会对混合疫苗产生加成或是抑制效应,遂将去活化JEV添加至由H5N1与EV71所成的2合1疫苗组合中,并施打于小鼠观察其免疫反应,结果如图5所示。在无添加佐剂的情况,于流感与肠病毒混合疫苗中加入去活化日本脑炎病毒,可增强流感与肠病毒混合疫苗的对于肠病毒的免疫诱发效果。若使用PELC乳状液作为佐剂,于流感与肠病毒混合疫苗中加入去活化日本脑炎病毒,可同时增强混合疫苗所诱发对于肠病毒与流感病毒的免疫效果。
组合疫苗的T-细胞免疫反应
将BALB/c小鼠以肌肉(i.m.)注射有或无添加佐剂的候选疫苗。小鼠经免疫-追加疫苗之后,取其第7天的脾脏,进行T细胞分析。简言说明,脾脏细胞取出后,纯化出单一悬浮细胞,并用添加10%胎牛血清(FBS,HyClone,Perbio)的RPMI培养基将细胞浓度调整为1x106/mL,在96孔盘加入200μL细胞悬浮液,在加入去活化病毒,于37℃培养4天。收集上清液,再利用细胞激素ELISA定量套组(R&D,Abingdom),分析其细胞激素浓度。利用酵素联结免疫吸附斑点(ELISpot)定量套组(eBioscience),测定细胞培养第三天的细胞激素分泌情形。
如图6所示,小鼠接种添加PELC乳状液的去活化病毒疫苗,其诱导细胞激素分泌的效能,较无添加该佐剂的疫苗更佳;且从注射佐以PELC乳状液的去活化病毒疫苗的免疫小鼠所纯化得到的T-细胞中,制造与之细胞数显著较受无佐剂,及佐以铝盐的疫苗免疫的动物组高出甚多。此外,如图6所示,在从经H5N1/EV71/JEV组合疫苗免疫的小鼠收集得到脾脏细胞,再将该等细胞于活体外以H5N1去活化病毒刺激后的脾脏细胞上清液中,IFN-γ分泌量较H5N1/EV71疫苗免疫组显著增高,表示所增加的细胞反应因为JEV的存在所致,再次证明去活化日本脑炎疫苗在增强免疫反应方面,具有类似佐剂的功效。
如图7所示,更进一步显示,小鼠若接种添加PELC与indolicidin单棕榈酸复合佐剂(PELC/Pam-indo)、或具有左旋右旋交叉胺基酸序列抗菌胜肽indolicidin与hBD2(humanβ-defensin II)的H5N1去活化病毒疫苗,其脾脏细胞于相同病毒刺激之后,将会大量产生能分泌Th1细胞分泌素IFN-γ的T细胞。

1.一种多价疫苗,其特征在于,包括去活化肠病毒71型的抗原及去活化H5N1亚型流感病毒混合去活化日本脑炎病毒的抗原,及医药上可接受的载剂。
2.根据权利要求1所述的多价疫苗组成物,其特征在于,所述去活化的肠病毒71型根据细胞培养方式获得。
3.根据权利要求1所述的多价疫苗,其特征在于,所述去活化的日本脑炎病毒根据细胞培养方式获得。
4.根据权利要求1所述的多价疫苗,其特征在于,还包含医药上可接受的佐剂。
5.根据权利要求4所述的多价疫苗,其特征在于,所述佐剂为一种疫苗递送系统。
6.根据权利要求5所述的多价疫苗,其特征在于,所述疫苗递送系统为铝盐或乳状液。
7.根据权利要求5所述的多价疫苗,其特征在于,所述佐剂为一种复合佐剂,还包括免疫刺激物质。
8.根据权利要求7所述的多价疫苗,其特征在于,所述免疫刺激物质为能增强病毒中和效果的CpG寡脱氧核苷酸。
9.根据权利要求7所述的多价疫苗,其特征在于,所述免疫刺激物质为能增强T细胞反应的短链抗菌胜肽。
10.根据权利要求9所述的多价疫苗,其特征在于,所述短链抗菌胜肽为具有左旋右旋交叉胺基酸序列的抗菌胜肽。
11.根据权利要求9所述的多价疫苗,其特征在于,所述短链抗菌胜肽为具有脂质化片段的抗菌胜肽。
