技术领域
本公开概括而言涉及电外科手术器械(electrosurgicalinstruments),更具体而言涉及具有被配置用于双极切除(bipolarresection)的电极体(electrodebody)的电外科手术器械。
背景技术
电外科手术器械近年来已变得被外科医生广泛使用了。相应地,对于易于操作、可靠且在手术环境中安全的设备和器械已形成了需求。大体上,大多数电外科手术器械是将射频(RF)电能量或电外科手术能量传送到组织部位的手持器械,例如电外科手术笔(electrosurgicalpencil)。电外科手术能量经由定位在患者下方的返回电极板(即单极系统配置)或者可定位为与手术部位身体接触或紧邻手术部位的更小的返回电极(即双极系统配置)被返回到电外科手术源。由RF源产生的波形产生一般称为电外科手术切割(cutting)和电灼(fulguration)的预定电外科手术效果。
具体地,电外科手术电灼包括向例如人类肉体或内部器官的组织之类的生物组织施加电火花,而没有显著的切割。火花是由从适当的电外科手术发生器(electrosurgicalgenerator)生成的射频电能量或电外科手术能量的脉冲串产生的。凝结(coagulation)被定义为使组织变干的过程,其中组织细胞被割裂并被脱水/变干。另一方面,电外科手术切割/解剖包括向组织施加电火花以便产生切割、解剖和/或分割效果。混合包括切割/解剖的功能与止血效果的产生相结合。同时,封合/止血被定义为溶解组织中的胶原质以使其形成为熔融体的过程。

典型的电外科手术发生器具有用于选择电外科手术输出的许多控制。例如,外科医生可选择各种外科“模式”来处置组织:切割、混合(混合级别1-3)、低切割、干燥、电灼、喷射等等。外科医生还有选择通常从1-300W不等的功率设定的范围的选项。可以明白,这给予了外科医生在处置组织时的很丰富的多样性。外科医生通常遵循预设的控制参数并且保持在已知的模式和功率设定内并且电外科手术笔包括简单且人体工学友好的控制,这些控制易被选择来调控各种模式和功率设定。
现有的电外科手术器械系统允许外科医生经由部署在电外科手术笔上的两个分立的开关在两种预配置的设定(即凝结和切割)之间改变。其他电外科手术器械系统允许外科医生通过调整或闭合电外科手术发生器上的开关来递增当器械的凝结或切割开关被按下时施加的功率。外科医生随后需要通过查看电外科手术发生器上的各种显示屏和/或计量仪来从视觉上核实所施加的功率的变化。换言之,所有对电外科手术器械的调整和在电外科手术器械的使用期间监视的参数通常都是位于电外科手术发生器上的。这样,外科医生在外科手术过程期间必须不断地从视觉上监视电外科手术发生器。
电外科手术器械系统已越来越多地设有耦合和/或连接系统(例如插头),用于将电外科手术器械可移除地连接到电外科手术发生器。通常,电外科手术器械设有所谓的“雄性”连接器,而电外科手术发生器设有相应的“雌性”连接器。连接器可包括标识装置以提供关于连接到其上的装置的特定标识信息。其他系统包括装置检测算法,这些算法与标识装置之间具有接口或者通过测量电外科手术器械的特定特性来确定装置的类型。
由于电外科手术术要求将射频能量受控制地施加到手术组织部位,所以针对特定的电外科手术程序将适当的电外科手术发生器与电外科手术器械正确和/或适当地匹配,是重要的。电外科手术器械通常被配置成使得外科医生不必将其视线从手术部位转开并转向电外科手术发生器,功率输出就能得到调整。
发明内容
本公开涉及一种被配置用于执行双极切除手术程序的电外科手术器械。该电外科手术器械包括被配置为连接到双极电外科手术能量源的壳体和耦合到该细长壳体的电极体。电极体包括电极面、第一和第二双极电极和刀片。电极面被形成在电极体的远端并且包括纵向轴和穿过它的横向轴。第一和第二电极具有前缘和后缘,被形成在纵向轴的相对两侧并且连接到相反的电势。刀片被定位在电极之间,刀片的至少一部分延伸到超出电极面。
刀片可被沿着纵向轴和横向轴两者定位,以使得第一和第二双极电极在刀片横切组织之前和之后处置组织。

在另一实施例中,刀片可包括前切割刃和后切割刃并且该方法还可包括以下步骤:沿着纵向轴在与第一方向相对的第二方向上将电外科手术器械在切除线上进行牵引并且同时利用刀片在第二方向上切割组织,从而沿着切除处置未切割的组织并沿着切除线处置已切割的组织。
在另一实施例中,该方法可包括选择第一刀片位置并且将刀片定位在第一刀片位置的步骤,其中在第一刀片位置中刀片的至少一部分延伸到超出电极面。

从图3可见,设置了第四和第五导线(例如第一RF线416d和第二RF线416e),它们电连接到形成在电极体112上的双极电极138。由于第一RF线416d和第二RF线416e是直接连接到电极体112上的双极电极138的,所以第一RF线416d和第二RF线416e绕开VDN127并且与VDN127和控制线416a-416c隔离。通过将第一RF线416d和第二RF线416e直接连接到电极体112上的相应双极电极138并且将VDN127与RF能量传输隔离,电外科手术电流不会流经VDN127。这进而增长了VDN127和/或激活开关120a、120b、120c的寿命和有效期。
参考图4,VDN127被示出并且包括被配置为操作电外科手术笔100的各种模式的第一传输线127a;被配置为操作电外科手术笔100的各种强度的第二传输线127b;被配置为充当VDN127的地的第三传输线127c;以及向VDN127传输最高达大约+5伏特的第四传输线127d。
第一RF线416d和第二RF线416e与VDN127隔离或以其他方式与VDN127完全分隔。具体地,第一RF线416d和第二RF线416e从RF输入或发生器“G”直接延伸到电极体112上的双极电极。
仅作为示例,VDN127可包括以第一串联连接在第三传输线127c和第四传输线127d之间的多个电阻器“R1”(例如六个电阻器)。第一串联的电阻器“R1”可组合成总共大约1000欧姆的电阻。第一串联的电阻器“R1”可各自由第一组开关“S1”分隔。第一组开关“S1”中的每个开关可电连接在相邻电阻器“R1”与VDN127的第一传输线127a之间。在操作中,取决于第一组开关“S1”中的哪个或哪些开关闭合,电外科手术笔100的不同操作模式被激活。
可利用包括具有从大约10%到大约100%的占空因数的波形的电外科手术能量来执行切除。可利用具有从大约10%到大约100%的占空因数的波形来执行如这里所述的凝结和烧灼的双重效果。要增大凝结的深度,可能需要具有从大约50%到100%的占空因数的波形。重要的是要注意,这些百分比是近似的并且可针对各种组织类型和特性被定制来递送期望的外科效果。
在一个实施例中,提供给双极电外科手术笔100的波形可由发生器“G”动态控制。例如,开关S1、S2、S3提供的操作的模式可指示发生器“G”的操作的范围。发生器“G”提供指定的操作范围内的波形,其中波形基于参数而动态改变,其中该参数可与能量递送、目标组织和能量递送的持续时间之一有关。该参数可以是从发生器“G”外部的源获得的,例如测量到的参数或临床提供的参数,或者该参数可包括由发生器“G”获得、测量或确定的内部参数。
从整个图2可见,电外科手术笔100还包括滑动地支撑在细长壳体102上或其中的强度控制器128。强度控制器128可被配置为充当在VDN127上沿着VDN127滑动的滑动式电位计,其中最远侧位置对应于较高强度设定,最近侧位置对应于低强度设定,其间有多个中间位置。可以明白,从近端到远端的强度设定可以反过来,例如从高到低。
强度设定通常是预设并基于以下各项从查找表中选择的:对电外科手术器械/附件的选择、期望的外科效果、外科专业和/或外科医生偏好、电极体112的类型以及电极面105上的双极电极的布置。对电极体112、强度设定和占空因数的选择决定了外科效果。设定可由用户手动选择或者是自动选择的。例如,电外科手术发生器“G”可自动确定可更换末端执行器108的远端上的电极体112的类型并且预定的强度值可由用户或电外科手术发生器“G”选择并随后调整。
现转到图3,提供对插头组件400的详细论述。插头组件400包括壳体部分402和将壳体部分402和电外科手术笔100中的控制端子215(参见图2)电互连的连接导线424。壳体部分402包括经由例如卡扣啮合而操作性地相互啮合的第一半部402a和第二半部402b。第一半部402a和第二半部402b被配置并适合于在其间容纳公共电源引脚404和多个电触点406。
插头组件400的公共电源引脚404在第一半部402a和第二半部402b之间的位置从壳体部分402向远侧延伸。公共电源引脚404可被定位为偏离中心,即与壳体部分402的另一侧边缘相比,更接近于一侧边缘。插头组件400还包括也从壳体部分402延伸的至少一对位置引脚412。位置引脚412可被定位在壳体部分402的第一半部402a和第二半部402b之间并且与公共电源引脚404朝向相同方向。第一位置引脚412a被定位得紧邻壳体部分402的中心,并且第二位置引脚412b被定位为偏离中心并且与公共电源引脚404相比紧邻壳体部分402的相反侧边缘。第一位置引脚412a、第二位置引脚412b和公共电源引脚404可位于壳体部分402上的与电外科手术发生器“G”(参见图1)的连接器接收器“R”的引脚接收位置(未示出)相对应的位置处。
插头组件400还包括从壳体部分402延伸的插脚414。具体地,插脚414包括从壳体部分402的第二半部402b延伸的主体部分414a和从壳体部分402的第一半部402a延伸的封盖部分414b。这样,当第一半部402a和第二半部402b相互接合在一起时,插脚414的封盖部分414b封闭主体部分414a。插脚414可被定位在公共电源引脚404和第一位置引脚412a之间。插脚414被配置并适合于在其中保持电触点406,以使得每个电触点406的一部分沿着其正面或远侧边缘暴露。虽然示出了五个电触点406,但可设置任意数目的电触点406,包括但不限于两个、六个和八个。插脚414可位于壳体部分402上与电外科手术发生器“G”的连接器接收器“R”(参见图1)的插脚接收位置(未示出)相对应的位置处。
由于插脚414从壳体部分402的第二半部402b延伸,所以除非壳体部分402处于适当的朝向,否则插头组件400的壳体部分402不会进入电外科手术发生器“G”的连接器接收器“R”。换言之,插脚414充当极化构件。这确保了在电外科手术发生器“G”的连接器接收器“R”中适当地接收公共电源引脚404。
连接导线424包括电连接到公共电源引脚404的电源线420、电连接到相应电触点406的控制线416a-416c以及电连接到相应电触点406的第一RF线416d和第二RF线416e。
现参考图5,电外科手术笔100的远侧部分被概括示为100d。杆柄110被配置为与末端执行器接收器104啮合。杆柄110和末端执行器接收器104被配置为在其间提供多个适当的电连接以促进电外科手术能量从电外科手术发生器“G”(参见图1)到电极体112的递送。
杆柄110的至少一部分被插入到细长壳体102的远侧开口103a中以啮合末端执行器接收器104。末端执行器接收器104被配置为将杆柄110机械且电气地耦合到细长壳体102。电连接可包括为了供电、控制和/或感测而连接到电极体112的一个或多个电连接器(或电连接器对)。例如,末端执行器接收器104可提供为刀片致动和/或定位和位置感测提供双极电外科手术能量、电功率和控制连接的一个或多个电连接器对和用于一个或多个感测装置(例如温度传感器、压力传感器、位置传感器等等)的电连接器。杆柄110和末端执行器接收器104可包括锁定装置,例如滑入到杆柄锁销接收器(未明确示出)中并与其啮合的杆柄锁销。可使用任何适当的紧固和/或锁定装置来将杆柄110可释放地紧固到细长壳体102。
如这里所述,电极体112被附接到其远端的杆柄110与细长壳体102是可互换的。在其他实施例中,杆柄110被集成到细长壳体102中并且是不可更换的。例如,杆柄110可包括集成到细长壳体102中的、被配置为调整刀片106相对于电极面105的深度和/或位置的至少一个机械联动装置。机械联动装置可连接在壳体上的调整机构(例如滑动装置、拨盘或选择器)和在杆柄110的远端或电极体112中的关节连接的机构(展开致动器和/或位置控制器)之间。
图6是图示切除手术程序期间的电极体112的部分截面立体图。在电极体112的远端的电极面105被定位为沿着切除线160c与组织160接触。随着电极体112在行进方向“D”上在组织160上被牵引,电极面105上的双极电极(参见图7A-10B)通过递送双极电外科手术能量来沿着切除线160a处置组织160并且刀片106切除被处置的组织160a的至少一部分。如下所述,刀片106相对于电极体的位置被选择为使得电极体112在切除之前对未切割的组织进行电外科手术处置并且在切除之后对已切割的组织进行电外科手术处置。
图7A和7B是各自图示具有基本上对称的双极电极布置的相应电极面105a、105b的不同电极体112a、112b的底视图。图7A图示了第一双极电极138a和第二双极电极138b,它们各自基本上是矩形的并且被定位为与电极面105a的相应边缘平行且邻近。第一双极电极138a和第二双极电极138b彼此关于纵向中心线“C”基本上对称并且彼此关于横向线“CC”基本上对称。机械刀片106a被定位在纵向中心线“C”上或与其邻近并且在第一双极电极138a和第二双极电极138b之间。刀片106a被定位为与后缘“TE”邻近,其中后缘“TE”与前缘“LE”相对,并且前缘“LE”被定位为朝着行进方向“DA”。
在使用中,如以上所述以及图6中所示,电极面105a在行进方向“DA”上在组织160上被牵引。第一和第二双极电极138a、138b的前部(例如双极电极138a、138b的与电极面105a的前缘“LE”邻近的部分)接触未处置和/或处置不足的组织并且传导双极电外科手术电流经过它。第一和第二双极电极138a、138b的中部(即与横向线“CC”邻近)和双极电极138a、138b的与后缘“TE”邻近的部分也在其间传导电外电流。随着电极面105a在行进方向“DA”上在组织160上被牵引,与刀片160a邻近的组织在被相应的第一和第二双极电极138a、138b的前部和中部处置之后被刀片106a切割。
图7B图示了根据本公开的另一实施例的电极体112b,其具有包括第一双极电极148a和第二双极电极148b的电极面105b。每个双极电极148a、148b包括与电极体112b的相应纵向侧基本平行且邻近的向外纵向边缘。第一双极电极148a和第二双极电极148b彼此关于纵向中心线“C”基本上对称并且彼此关于横向线“CC”基本上对称。具有双向切割表面的刀片106b被定位在第一和第二双极电极148a、148b之间并且邻近纵向中心线“C”和横向的中心线“CC”的交点(例如,被定位在前缘“LE”和后缘“TE”之间的大致中途处)。第一双极电极148a和第二双极电极148b各自包括一个或多个弓形边缘,这些弓形边缘朝着纵向中心线“C”定向并且沿着电极面105b的较大部分向内朝着纵向中心线“C”径向弯曲。
在使用中,可沿着行进方向“DB”双向使用电极面105b。电极面105b关于纵向中心线“C”和横向线“CC”对称并且刀片106b基本上在其间的中心处,电极面105b的对称性在任一行进方向“DB”上产生了基本上相似的结果。
图8A和8B是分别图示具有单向双极布置的电极面105c、105d的电极体112c、112d的底视图。图8A图示了第一双极电极158a和第二双极电极158b,它们各自具有内部和外部弓形边缘,其间的间距及其横向宽度被选择为沿着纵向长度在组织中生成均匀的电流密度。
在沿着第一和第二双极电极158a、158b的纵向长度的任何给定点,组织中的电流密度与组织的阻抗、组织与第一和第二双极电极158a、158b之间的接触表面以及其间的间距有关。随着电极面105c在方向“DC”上在组织上被牵引,由经过第一和第二双极电极158a、158b之间的电外科手术电流生成的双极电外科手术能量处置组织,从而改变其至少一个组织属性(例如阻抗的增大)。组织属性的变化可改变第一和第二双极电极158a、158b与组织之间的接触质量。
就本申请而言,术语“接触质量”指的是第一和/或第二双极电极158a、158b与组织之间的电连接的整体质量和电连接在其间传导电流的能力。决定接触质量的因素可包括组织与第一和/或第二双极电极158a、158b之间的接触的量和百分比以及与接触的量和百分比相关联的阻抗和/或电阻。
为了针对组织属性的任何变化进行补偿,第一和第二双极电极158a、158b的横向宽度和其间的间距都是在纵向上变化的。第一和第二双极电极158a、158b的邻近前缘“LE”的部分的宽度是最小的。第一和第二双极电极158a、158b的横向宽度在邻近前缘“LE”的部分和邻近刀片106c的部分之间逐渐增大。第一和第二双极电极158a、158b的最大横向宽度是邻近刀片106c的后缘(即在组织被刀片106c切割之后)。
第一和第二双极电极158a、158b之间的横向间距从第一和第二双极电极158a、158b的邻近前缘“LE”的部分逐渐增大到邻近刀片106c的前缘处的最大距离。横向间距从刀片106c的前缘到刀片106c的后缘逐渐减小。
在第一和第二双极电极158a、158b之间经过组织生成的电流路径在未切割的组织(例如在刀片的切割表面前方的组织)和已切割的组织(例如在刀片的切割表面后方的组织)中是根本上不同的。在未切割的组织中,在第一和第二双极电极158a、158b之间流动的电外科手术电流在沿着或邻近表面组织的电流路径中行进。随着表面组织和邻近表面的组织的阻抗增大,寻求电阻更小的路径的电外科手术电流透入表面组织的更下方。理想情况下(但不是必要的),在被刀片106c切割之前的组织中生成的电外科手术电流完全凝结组织到超过刀片106c的深度的深度。
在刀片106c切割组织之后,沿着切除线的组织结构变化(由于被切割)并且切割前的电外科手术电流所使用的电流路径(沿着和邻近组织的表面)被切除线所切断。这样,经过已切割的组织并且在第一和第二双极电极158a、158b的定位在已切割的组织之间的部分之间(例如在电极面105c的后缘“TE”和刀片106c的切割刃之间)生成的电外科手术电流必须寻求新的路径经过组织。例如,切割后的组织中的一个电路路径可包括沿着新形成的切除线在其下行进(例如电流沿着切口的一侧、在切割线下并沿着切口的另一侧行进),从而处置新切割的组织。切割后的组织中的另一电流路径可包括电流朝着前缘“LE”纵向行进,并且绕过刀片106c的切割刃(经过未切割的组织)。
这样,这里所述的电外科手术笔100、200和双极切除装置300提供了在组织被刀片106c切割之前和之后沿着切除线对组织的电外科手术处置。
在第一双极电极158a和第二双极电极158b之间经过已切割的组织(例如沿着已被刀片106c的切割刃所切割的切除线的组织)的电流路径(例如在距离上)比经过未切割的组织的电流路径长。例如,经过未切割的组织的电流路径通常是沿着表面组织的并且这些路径的长度大约等于第一和第二双极电极158a、158b之间的横向距离。刀片106a沿着切除线切断表面组织并且新的电流路径沿着切除线和/或在切除线下方形成。例如,一个新的电流路径可沿着新切割的组织形成,该电流路径的长度等于沿着切除线的第一侧的切口的深度、沿着切除线的第二侧的距离以及第一和第二双极电极158a、158b之间的距离。
为了针对在刀片106c沿着切除线切割组织之后经过组织的电流路径的整体长度的增大进行补偿,第一和第二双极电极158a、158b在后缘“TE”和刀片106c的切割刃之间的表面面积增大并且第一和第二双极电极158a、158b之间的横向距离减小。这样,由于在组织中形成的切口而引起的组织路径的长度(例如距离)的增大至少部分被表面面积增大和/或第一和第二双极电极158a、158b之间的横向距离的减小而抵消。
图8B图示了与图8A中所示以及上文所述的电极面105c结构上相似的电极面105d上的布置。虽然结构布置可能是相似的,但电连接是不同的。在图8B中,电极面105b包括一对相似极性的电极168a以及刀片106d,其中刀片106d的至少一部分形成相反极性的电极。如图8B中所示以及这里所述,整个刀片106d形成第一电极。在此特定实施例中,组织中的所有电流路径都是在形成为一个电极的刀片106d与第一双极电极168a的一者或两者之间形成的。
在此特定实施例中,电流密度在邻近刀片106d的组织中是最大的。具体地,组织中的电流密度可沿着刀片106d的与组织的接触质量最高的部分(例如刀片106d的切割刃和/或刀片106d的邻近最大横向宽度的部分)集中。
在切割之前未完全处置的任何组织将随着刀片106d在方向“DD”上经过组织而被凝结和/或烧灼。类似地,在切割期间从组织释放的液体可接触刀片106d,从而改善刀片106d与组织之间的接触质量,从而在液体释放点促进组织处置。
图9是图示具有单向双极电极布置的电极面105e的电极体112e的底视图。电极面105e包括第一双极电极178a、第二双极电极178b和刀片106e。第一双极电极178a和第二双极电极178b彼此关于纵向中心线“C”基本上对称。刀片106e被定位为邻近电极面105e的后缘“TE”(即远离行进方向“DE”)并且在第一和第二双极电极178a和178b之间。第一双极电极178a和第二双极电极178b之间的横向距离在前缘“LE”附近最小,从而在位于其间的组织中生成高电流密度。在朝着后缘“TE”纵向移动时,第一和第二双极电极178a和178b之间的横向距离逐渐增大,并且在后缘“TE”附近达到其间的最大横向距离。
在使用中,虽然第一和第二双极电极178a和178b的配置可被配置为将电外科手术处置的绝大部分递送到定位于刀片106e的切割刃前方的未切割组织,但电外科手术处置的一部分仍沿着切除线被提供给已切割的组织(即处置切割之后的组织)。
图10A和10B是分别图示具有双向双极布置的电极面105f、105g的电极体112f、112g的底视图。图10A包括与图7B中所示的电极面105b相似的电极面105f,其包括具有双向切割表面的刀片106f。刀片106f被配置为在第一方向上利用第一刀片切割刃切割并且在第二方向上利用第二刀片切割刃切割。刀片106f还包括弓形纵向侧边缘和最大厚度“T”。第一双极电极188a和第二双极电极188b基本上彼此关于纵向中心线“C”对称并且彼此关于横向线“CC”对称。双向双极布置可被在第一方向上在组织上牵引以切割组织并且被在与第一方向相反的第二方向上在组织上牵引以沿着已切割组织的壁聚集电流以凝结组织和/或止血,如上文所述。
图10B包括电极体112g的电极面105g,该电极体112g包括具有双向切割表面的刀片106g、后双极电极对198和前双极电极对199。后双极电极对198包括后第一双极电极198a和后第二双极电极198b。前双极电极对199包括前第一双极电极199a和前第二双极电极199b。后双极电极对198和前双极电极对199可被电连接并配置为接收来自同一源的电外科手术信号并且它们一起可按与这里所述的包括一个双极电极对的电极体112、112a-112g基本上类似的方式操作。
在另一实施例中,后双极电极对198和前双极电极对199被配置为在其间接收被独立控制的电外科手术信号。在一个实施例中,电外科手术发生器“G”(参见图1)在后双极电极对198之间提供第一电外科手术信号并且在前双极电极对199之间提供第二电外科手术信号。第一和第二电外科手术信号可以是在后双极电极对198和前双极电极对199之间按时间分摊的单个电外科手术信号。或者,第一和第二电外科手术信号可包括提供给各个双极电极对198、199的两个单独的电外科手术信号。发生器“G”可被配置为提供第一和第二电外科手术信号,或者电外科手术信号可由独立地或相互联合地工作的第一和第二发生器提供。
可能需要额外的导体来提供第二电外科手术信号。例如,连接导线、插头组件400、424、电外科手术笔100、杆柄110和电极体112可能需要一个或多个额外的导体以便把第二电外科手术信号提供到电极面105g。在一个实施例中,第一和第二电外科手术信号是经由三导体三轴线缆提供的。
在另一实施例中,发生器“G”(参见图1)被配置为在双极电极198a、198b、199a、199b的任何两者之间选择性地施加电外科手术信号。例如,发生器“G”可将双极电外科手术信号按时间分摊在双极电极对198、199中的任一对上,施加时间为第一时间段,然后在四个电极198a、198b、199a、199b中的任两者之间提供电外科手术信号,施加时间为第二时间段。
这里描述和图示的电外科手术笔100、200和双极切除装置300被配置为在切除之前和/或之后沿着切除线凝结和/或烧灼组织。这里描述的实施例被配置为针对由于临床医生的个人风格或技术而发生的不可控制且变化的操作参数进行补偿。在本申请的上下文中,“不可控制且变化的操作参数”(下文称为“不可控参数”)被定义为由于临床医生的动作而变化的操作条件。
两个这种不可控参数包括当电外科手术笔100、200被沿着切除线牵引时临床医生所施加的速度和向下的力。这些不可控参数,即速度和向下的力,经常在一次手术程序期间和/或两次单独的手术程序期间变化。
这里描述的电外科手术笔100、200在宽范围的不可控参数上提供了一致的操作。关于变化的速度,电外科手术笔100被配置为在切割之前和/或之后处置组织。根据本公开的一个实施例的电外科手术笔100被以第一速度沿着切除线在组织上牵引,随后被以第二速度沿着第二切除线牵引(并且经过基本上相似的组织),其中第二速度大于第一速度。在第一速度下,组织处置的百分之九十发生在切割之前并且剩余的百分之十发生在切割之后。在第二速度下,组织处置的百分之七十发生在切割之前并且剩余的百分之三十发生在切割之后。这样,电外科手术笔100通过改变在切除之前和切除之后提供的处置的比率而在每个速度下提供了充分量的处置。可以使用加速度计来对作为处置的速度或加速度的函数的、提供给双极电极的电外科手术能量的量进行计算、控制或校正。电外科手术笔100、200还可包括拖动感测能力,如在本申请的申请人拥有的发明人为Buchman等人的第7,393,354号美国专利中所记载,这里通过引用将该美国专利全部并入。
拖动感测能力可用于确定行进速度和方向并且提供反馈来调整与能量递送或刀片位置有关的一个或多个参数。可调整能量递送来在递送给刀片前方的组织的能量与递送给刀片后方的组织的能量之间提供期望的比率,并且/或者可对刀片的位置/方向进行动态调整。
关于临床医生所施加的向下的力的量,增大向下的力可增大切除的深度。如上文所述,如果沿着切除线的组织在切割之前未被处置或处置不足,则未处置或处置不足的组织在切割之后接受进一步处置。可以使用压力传感器根据由临床医生施加的压力和/或由压力传感器测量到的压力,来计算、控制或校正提供给双极电极的电外科手术能量的量。
图11A-11F是包括具有多向双极布置的圆形电极面305a、305c-305f的大体上圆柱状的电极体312a、312c-312f的各种视图。图11A是包括具有双向切割表面的刀片306a、绕着刀片306a同心部署的第一双极电极338a和绕着第一双极电极338a同心部署的第二双极电极338b的大体上圆柱状的电极体312a的底视图并且图11B是其立体图。刀片306a和旋转的刀片壳体307a被配置为相对于第一双极电极338a、第二双极电极和/或杆柄308旋转,如箭头“A”所指示。第一双极电极338a和第二双极电极338b被使第一双极电极338a和第二双极电极338b绝缘和/或分开的介电间隔器307b分隔。
旋转刀片壳体307a的近端形成球窝关节(ball-and-socketjoint)309的球309a。球309a与窝309b匹配并且被配置为在窝309b中自由旋转,如箭头“A”所指示。球窝关节309被配置为允许旋转刀片壳体307a相对于电极体312a的其余部分旋转。
球窝关节309可包括提供球309a和窝309b之间的润滑的液体。密封物可防止润滑液体从球窝关节309流到电极面305a。液体可与外科部位相容并且经过电极体312a向远侧流动,从而润滑旋转刀片壳体307a和电极体312a的其余部分之间的界面。
虽然这里描述并在图中图示了球窝关节309,但可使用其他适当的装置来提供旋转刀片壳体和电极体312a的其余部分之间的旋转。例如,滚柱轴承、滚珠止推轴承或滚柱止推轴承可被安放在旋转刀片壳体307a的近端或者滚珠轴承组件可被安放在旋转刀片壳体307a的侧面。
第一双极电极338a连接到第一导电体308a并且第二双极电极338b连接到第二导电体308b。第一双极电极338a和第二双极电极338b可被同轴地定位在电极体312a内,如图11B中所示,或者电极338a、338b可被同心地形成在电极面305a上。当被激活时,第一和第二导电体308a、308b向第一和第二双极电极338a、338b提供电外科手术能量,从而在其间提供相反的电势。
在一个实施例中,刀片306a和旋转刀片壳体307a相对于电极体312a和杆柄308的旋转位置由电极体312a的行进方向(例如电极体312a在患者组织上被牵引的路径)来控制。在刀片306a被定位在患者组织中之后,刀片306a的位置由组织路径来控制,并且电极体312a的方向的任何变化都使得刀片306a和电极体312a绕着球窝关节309旋转,从而随着电极体312a的方向改变位置。这样,当电极体312在组织上牵引时,刀片306a的位置保持与电极体312a的路径基本平行。

当被激活时,第一导电体308a和第二导电体308b分别在第一双极电极348a与多个第二双极电极348b-348i之间提供电外科手术能量信号,从而在第一双极电极348a与多个第二双极电极348b-348i中的每一个之间产生电势。刀片306d和旋转刀片壳体307d被配置为相对于第一双极电极348a和多个第二双极电极348b-348i旋转,如上文所述的图11B和11C中所示。
在另一实施例中,第二双极电极348b-348i中的至少两个被选择性地连接到第二导电体308b。所选择的连接到第二导电体308b的双极电极348b-348i可由刀片306b的位置、行进方向和/或施加的力(例如施加到电外科手术器械的力)确定。例如,如图11F中所示,圆柱状电极体312f被配置为基于刀片306f的位置选择性地将双极电极348b-348i中的两个或更多个连接到第二连接器308b。第一电连接器308a连接到第一双极电极348a。第二电连接器308b通过连接在旋转刀片壳体307f与电极体312f之间的滑动连接器308d形成到旋转刀片壳体307f的电连接。滑动连接器308d可包括旋转式电干扰连接器、旋转电连接器、集电器(collector)、转环、电旋转接头或者能够通过旋转组件形成电连接的任何其他适当的连接器。
第一滑动连接器308d连接到形成在旋转刀片壳体307f上或形成在旋转刀片壳体307f中的第一和第二电迹线308e、308f。第一和第二电迹线308e、308f分别电连接到第一和第二电极选择器308g、308h。第一和第二电极选择器308g、308h被附接到旋转刀片壳体307f的相对侧并且各自与第二双极电极348b-348i中的一个或多个形成电连接。如图11G中所示,所选择的第二双极电极(例如由第一电极选择器308g选择的第二双极电极348b-348d和由第二电极选择器308h选择的第二双极电极348f-348h)可被定位在刀片305f的每一侧,但任何适当的布置都可确定第一和第二电极选择器308g、308h的位置和周长。
在另一实施例中,电极体可包括单向刀片(例如参见具有单向电极体的图8A和8B)并且第一和第二电极选择器的位置朝着预期的行进方向偏移。
图11G是图11的电极体的部分立体图,其图示了滑环308d、第一和第二电迹线308e、308f、第一和第二电极选择器308g、308h以及多个第二双极电极348b-348i之间的互连。在刀片306f的一侧,第一电极选择器308g接触三个双极电极348b-348d。在刀片306f的相对一侧,第二电极选择器308h接触另外三个双极电极348f-348h。所选择的第二双极电极的位置由第一和第二电极选择器308g、308h的位置和刀片306b的位置确定。
第一双极电极348a和第二双极电极348b-348i的数目和位置以及电极选择器308g、308h的数目和周长是设计选择事项,并且可随着特定的电极体和/或其预期用途而变。
现转到图12,根据本公开的另一实施例的电外科手术笔200包括关节连接的电极体212。电外科手术笔200与这里所述和所示的电外科手术笔100相似并且被详细论述到标识其中的构造和操作的差异所必要的程度。电外科手术笔200包括控制机构262、263、264以分别实现关节连接的电极体212的“滚动”(Roll)、“偏转”(Yaw)和“俯仰”(Pitch)。滚动控制器262实现杆柄和连接到它的关节连接的电极体212的“滚动”。偏转控制器263和俯仰控制器264连接到并致动腕状关节261以分别实现“偏转”和“俯仰”。对“滚动”、“偏转”和“俯仰”的控制可用于相对于目标组织定位电极面205。
关节连接的电极体212可包括这里描述的电极体112、112a-112h中的任何一个。
关节连接的电极体212的“滚动”、“偏转”和“俯仰”是分别通过操纵滚动控制器262、偏转控制器263和俯仰控制器264来实现的。滚动控制器262连接到细长壳体202的远侧开口203a和杆柄210并被定位在它们之间。滚动控制器262被配置为提供杆柄210和关节连接的电极体212相对于细长壳体202的旋转。
关节连接的电极体212的“偏转”和“俯仰”是由定位在杆柄210和关节连接的电极体212之间的腕状关节261实现的。腕状关节261的“偏转”运动由偏转控制器263控制并且“俯仰”运动由俯仰控制器264控制。偏转控制器263和俯仰控制器264被安放在细长壳体202中并通过细长壳体202的远侧开口203a和杆柄210连接到腕状关节261。
偏转控制器263是被安放在壳体的上半外壳部分202a和下半外壳部分202b之间的机械滑动装置。对于偏转控制器263可以利用任何适当的机械和/或电气调整机构。偏转控制器263通过一个或多个偏转细长构件(未明确示出)连接到腕状关节261,这一个或多个偏转细长构件具有充分的刚性以便对腕状关节261进行反复双向操纵。或者,偏转细长构件可包括偏转推索和偏转拉索,其中每条索被配置为实现单个方向上的运动。
俯仰控制器264是被安放在壳体202的上半外壳部分102a中的机械滑动装置。对于俯仰控制器264可以利用任何适当的机械和/或电气调整机构。俯仰控制器264利用一个或多个俯仰细长构件(未明确示出)连接到腕状关节261,这一个或多个俯仰细长构件具有充分的刚性以便对腕状关节261进行双向操纵。或者,俯仰细长构件可包括俯仰推索和俯仰拉索,其中每条索被配置为实现单个方向上的运动。
对“偏转”、“滚动”和“俯仰”的控制在具有关节连接的电极体212的电外科手术笔200被用在腹腔镜型手术程序中时尤其有用。关节连接的电极体212、腕状关节261和杆柄210可以是从头至尾具有基本上均一的厚度的细长构件,被配置用于插入和穿过身体内部接入装置(即导管、内窥镜或插管)。在使用中,对关节连接的电极体212部分的操纵独立于壳体202的位置。
现转到图13A和13B,与这里论述和图示的电极体112、112a-112g基本上相似的电极体112h被示出并将被详细论述到标识其中的构造和操作的差异所必要的程度。电极体112h的构造和操作可被包含到这里描述的电极体112、112a-112g的任何一个中。电极体112h包括被配置为在如图13A中所示的收起的第一位置和如图13B中所示的展开的第二位置之间展开的可展开刀片106h。如这里所述,“展开位置”包括除了收起的第一位置以外的可展开刀片106h的所有位置。展开位置包括“完全展开位置”,其中可展开刀片106h被伸展到最远侧位置,并且包括多个“切割位置”,在这些位置中可展开刀片106h能够切割组织。
包括可展开刀片106h的电极体112h向临床医生提供了在可展开刀片106h不处于切割位置的情况下使用电极面105h来进行凝结和/或烧灼的灵活性。例如,作为切除手术程序的预处置或作为切除手术程序中的一个步骤,临床医生可收起可展开刀片106h并且在向组织递送电外科手术能量同时的沿着切除线牵引电极面105h。或者,在切除手术程序期间,临床医生可收起可展开刀片106h到第一位置,或者不是切割位置的位置,并且利用电极面105h来沿着切除线凝结和/或烧灼处置不足的组织。
包括可展开刀片106h的电极体112h还可被临床医生利用为测量装置来测量与目标组织有关的一个或多个参数。在可选择的操作模式中,第一和第二双极电极208a、208b被发生器“G”(参见图1)利用作为传感器并且发生器“G”测量与组织有关的一个或多个属性。例如,在可展开刀片106h不处于切割位置的情况下,临床医生可沿着切除线牵引电极面105h并且测量一个或多个组织属性。例如,发生器“G”可测量沿着切除线的组织阻抗并且指示血管和/或动脉、液囊和/或具有不符合目标组织的一个或多个属性的组织的存在(或不存在)。
可展开刀片106h可滑动地啮合在电极体112h中限定的导槽141。导槽141也可以是被容纳于在电极体112h中限定的腔中的管状构件,其中该管状构件的内部形状和尺寸被配置为容适可展开刀片106h的形状和尺寸(即,单切割刃、双切割刃和/或其宽度和形状)。可展开刀片106h连接到由致动器143致动的活塞142。致动器143被配置为将活塞142致动到两个或更多个位置,其中这两个或更多个位置包括收起的第一位置和至少一个切割位置。
在另一实施例中,致动器143可被配置为选择性地控制可展开刀片106h的位置。致动器143可包括位置控制系统以动态地测量并控制活塞142的位置。或者,可展开刀片106h可包括由位置传感器152测量的标记151。位置传感器152向控制器144提供与标记的位置有关的位置信号。控制器144驱动致动器143并控制活塞142的位置。位置传感器152还可被配置为感测可展开刀片106h的一部分,例如远侧的尖端或者近端部分。控制器144可被定位在电极体112h中,如图13A、13B中所示,或者控制器144可被包含到细长壳体102或发生器“G”(参见图1和2)中。
现转到图14,包括与这里论述和图示的电外科手术笔100、200相似的功能的双极切除装置被示为300并且将被详细论述到标识其中的构造和操作的差异所必要的程度。双极切除装置300包括通过滚动控制器362、杆柄310和肘状关节361连接到关节连接的电极体312的壳体302。关节连接的电极体312还包括可展开刀片306。双极切除装置300包含了分别与如上文所述的图12、13A和13B中所示的可展开刀片106h和电极体212相似的关节连接的电极体312和可展开刀片306。
壳体302包括主体302a和手柄302b。手柄302b包括把手302c、俯仰控制器臂364和刀片触发器343。把手302c被配置为符合临床医生的手以促进单手操作、对关节连接的电极体312的单手操纵和/或对可展开刀片306的单手操纵。俯仰控制器臂364被配置为控制关节连接的电极体312相对于杆柄310的“俯仰”。刀片触发器343被配置为操纵被容纳在关节连接的电极体312内的可展开刀片306。被容纳在主体302a中的刀片锁343a被配置为将刀片触发器343的位置(以及可展开刀片306的位置)锁定在期望的位置以防止可展开刀片306相对于电极面305的无意间运动或不合需要的调整。
偏转控制器363、功率控制器329和刀片锁343a被容纳在主体302a中。偏转控制器363被配置为控制关节连接的电极体312相对于杆柄310的“偏转”。功率控制器329控制电极面305上的第一和第二双极电极(未明确示出)的功率输出。
滚动控制器362被安放在主体302a的远侧开口303a与杆柄310之间。滚动控制器362被配置为提供杆柄310相对于壳体302的旋转运动。
用于控制“偏转”、“滚动”和“俯仰”的各种控制机构是本领域已知的并且可包括一系列缆索、致动器、齿轮、驱动杆和其他机械和/或机电元件。
这里公开的电外科手术器械可利用电极体112a-112h中的任何一个或这里描述的特征的任何组合。
电外科手术笔100、200和双极切除装置300可包括智能识别技术,该技术与发生器通信以标识装置、电极体112、112a-112h和/或与装置相关联的一个或多个参数或参数212。由该智能识别技术识别出的与处置组织有关的身份和/或任何外科参数被传达到发生器“G”。例如,电外科手术笔100可配备有发生器“G”可读取的条码或Aztec码。发生器“G”可选择与利用所标识的装置处置组织相关联的预设和/或默认的参数。条码或Aztec码还可包括可编程数据,该可编程数据可被发生器读取并且在使用前将发生器编程到特定的电气参数。
也设想了其他智能识别技术,其使得发生器能够确定正在利用的器械的类型或作为安全机制确保器械到发生器的适当附接。一个这种安全连接器在2003年11月20日提交的序列号为10/718,114的美国专利申请中提出。例如,除了以上所述的智能识别技术以外,这种安全连接器还可包括与电外科手术笔100、200或双极切除装置300操作性地关联的插头或雄性部分和与电外科手术发生器操作性地关联的互补的插孔或雌性部分。
虽然已针对所描述的实施例描述了作为主题的装置,但相关领域的普通技术人员容易清楚,在不脱离作为主题的装置的精神或范围的情况下,可对其进行改变和修改。

Claims (9)

1.一种电外科手术系统,包括:
电外科手术能量源;以及
可操作地耦合到所述电外科手术能量源的电外科手术器械,该电外科手术器械包括:
壳体,具有耦合到该壳体的电极体,该电极体包括:
形成在所述电极体的远端的电极面,该电极面包括纵向轴和穿过所述纵向轴限定的横向轴;以及
形成在所述纵向轴的相对两侧并被配置为连接到所述电外科手术能量源的相反电势的第一电极和第二电极,该第一电极和第二电极中的每一个具有前缘和后缘,
所述电外科手术器械还包括:
定位在所述第一电极和所述第二电极之间的刀片,所述刀片的至少一部分能移动以向远侧延伸到超出所述电极面;以及
被支撑在所述壳体上的至少一个激活开关,该至少一个激活开关被配置为选择性地完成向所述第一电极和第二电极提供相反电势的控制回路。
2.根据权利要求1所述的电外科手术系统,其中,所述刀片被沿着所述纵向轴和所述横向轴两者定位,以使得所述第一电极和第二电极可操作以在所述刀片横切组织之前和之后处置组织。
3.根据权利要求2所述的电外科手术系统,其中,所述第一电极和第二电极是沿着所述纵向轴对称的。
4.根据权利要求3所述的电外科手术系统,其中,所述刀片还包括前切割刃和后切割刃。
5.根据权利要求4所述的电外科手术系统,其中,所述第一电极和所述第二电极是关于所述横向轴对称的。
6.根据权利要求5所述的电外科手术系统,其中,所述刀片是可在未展开位置和多个展开位置之间展开的。
7.根据权利要求6所述的电外科手术系统,其中,所述多个展开位置中的至少一个是切割位置。
8.根据权利要求7所述的电外科手术系统,其中,所述电极体还包括可操作地连接在所述刀片和所述电极体之间的致动器,该致动器被配置为在所述未展开位置和所述多个展开位置之间致动所述刀片。
9.根据权利要求8所述的电外科手术系统,其中,所述电外科手术器械还包括:
至少一个致动器,该至少一个致动器在相对于由将所述电极体耦合到所述壳体的杆柄限定的纵向轴的偏转、俯仰和旋转方向之中的至少一个方向上致动所述电极体。
