背景技术
伤口愈合过程涉及白血细胞(也被称为白细胞)的参与。白细胞包括淋巴细胞、粒细胞和单核细胞。三种常见的淋巴细胞类型是T细胞、B细胞和天然杀伤细胞。T细胞和B细胞在体内抗原识别中发挥重要作用(Parkin,2001)。天然杀伤(NK)细胞通过主要组织相容性复合体(MHC)水平的变化识别被感染的细胞,并杀死被感染的细胞(Moretta,2008)。愈合过程中淋巴细胞的参与在很大程度上与其产生细胞因子和生长因子有关(Keen,2008)。已经描述了皮肤中的一类新型γ-δ-T细胞(Jameson,2002.Havran,2005)。粒细胞的不同类型是嗜中性粒细胞、嗜碱性粒细胞和嗜酸性粒细胞。单核细胞分化成为巨噬细胞,后者负责清除组织碎片或者侵入的外源物质。巨噬细胞还产生控制炎症和修复的分子(Riches,1996)。
伤口愈合过程存在3个重叠的时期(Li,2007;Broughton,2006,-Tsirogianni,2006;Singer,1999;Martin,1997)。第一个时期是炎症期。其特征是招募嗜中性粒细胞,随后招募单核细胞至伤口位点,在此处将细菌杀死并吞噬(Agaiby,1999)。
伤口愈合的第二个时期被称为增殖期,其涉及形成新的肉芽组织。成纤维细胞增殖并迁移至伤口区域,合成胶原和其他胞外基质组分(Greiling,1997)。同时发生血管新生,为代谢活跃的新肉芽组织提供营养物质和氧气(Tonnesen,2000)。完整表皮中的角化细胞开始迁移经过临时基质并开始增殖,为新的上皮组织做准备(Kim,1992)。
重塑是伤口愈合中的第三个和最后一个时期。其特征是成纤维细胞分化成为肌成纤维细胞,后者相互接触并使伤口边缘更加接近(Tomasek,2002)。通过降解和重新合成而进行的胶原纤维重塑通过胶原纤维的重新定位(由细胞因子严密控制的过程)使得伤口获得强度(Werner,2003)。
处理伤口的挑战通常在患有多种病症(例如糖尿病、冠状动脉疾病和高血压)的患者中复杂化。由于多种生理条件,这些疾病具有使血管并发症恶化的共同效应。伤口的并发症可以导致发病率和死亡率增加(Doshi,2008)。
常规的伤口处理包括外科清创术、抗生素治疗和多种敷料(Moran,2008;Fonder,2008)。抵御常规处理的伤口也被称为难治性伤口。这些伤口使得生活质量下降,并可以导致发病率和死亡率增加。因此,对有效的伤口愈合组合物和方法的需求持续存在。
发明简述
本发明的一个方面是针对制备来源于血液的活化的白细胞组合物(ALC)(例如从全血样品中可获得或者已经获得)的方法。该方法包括将白细胞(可以从人全血中获得)在一定温度下第一孵育一段时间的步骤,这使得白细胞开始活化,在优选的实施方案中,所述孵育是于室温进行大约8至大约20小时。孵育后,使白细胞与生理上可接受的水溶液(例如无菌蒸馏水)相接触,以引发低渗休克,随后使休克的白细胞与生理上可接受的盐溶液相接触,以恢复等渗性。该活化的白细胞组合物(ALC)可以用于治疗。但是,在一些实施方案中,与白细胞的第一孵育分开和基本同时,与白细胞孵育的同时使可以从相同或者不同的全血样品中(即从相同或者不同的人中)获得的血浆样品与凝固剂于37℃相接触,在优选的实施方案中,所述接触是大约8至大约20小时,随后从已凝固的血浆样品中分离血清。将白细胞重悬于从已凝固的血浆样品中收集的血清中,因而形成ALC。第一孵育后,可以将白细胞进一步于37℃第二孵育大约60至大约120分钟。
本发明的另一个方面是针对来源于血液的ALC。从其中存在的白细胞群方面来说,本发明的活化的白细胞组合物包含大约40%至大约90%的粒细胞、大约5%至大约20%的单核细胞和大约5%至大约30%的淋巴细胞(基于ALC中白细胞的总体数量)。如实施例中所显示,还可以从白细胞的最低产率(相对于全血样品)、白细胞的活力和粒细胞的最低活化水平(例如如CD11b所表示)方面对本发明的ALC进行表征和鉴定。ALC进一步包含残留水平的血小板(大约46.8+/-39.2(103/μl)的量)和ALC红血细胞(大约0.1+/-0.06(106/μl)的量)。淋巴细胞群可以包含大约7%至大约25%的B细胞(CD19+)、大约20%至大约30%的NK细胞(CD3-/CD56+)、大约40%至大约60%的T细胞(CD3+)、大约0.1%至大约30%的NKT细胞CD3+/CD56+、大约8%至大约20%的辅助性T细胞(CD4+/CD3+)和大约20%至大约30%的CD8+/CD3+细胞。可以将细胞悬浮于载体(例如血清(相对于受体可以是自体的或者同种异体的))或者适于储存和对细胞施用的某些其他生理上可接受的iso-标准液体中,例如用于恢复等渗性的溶液。
而本发明的另一个方面是针对促进伤口愈合的方法,其包括对伤口施用或者应用ALC。
与含有白血细胞的至少一种已知的伤口愈合组合物相比,本文公开的发明达到了几个出乎意料的结果。如本文实施例中所描述,这些结果包括白细胞(WBCs)的产量和活力增加,以及活化的粒细胞的百分比较高。还可以相信,本文公开的发明包括较高百分比的活化的单核细胞和与CD8 T细胞相比百分比相对较高的CD4 T细胞。

发明详述
本文中的血液定义为全血或者其任意组成成分(例如血浆、白细胞、血小板或者红血细胞)。本发明ALC中存在的血小板和红血细胞的量低于全血中存在的量。
本文中与任何和所有数值(包括数值范围的上限和下限)相连使用的术语“大约”是具有可接受偏离范围的任何数值,所述范围是+/-0.5%至+/-20%(和其间的数值,例如±1%、±1.5%、±2%、±2.5%、±3%、±3.5%、±4%、±4.5%、±5%、±5.5%、±6%、±6.5%、±7%、±7.5%、±8%、±8.5%、±9%、±9.5%、±10%、±10.5%、±11%、±11.5%、±12%、±12.5%、±13、±13.5%、±14%、±14.5%、±15%、±15.5%、±16%、±16.5%、±17%、±17.5%、±18%、±18.5%、±19%、±19.5%和±20%)。
用于生产本发明ALC的起始材料可以从几种来源获得。可以从自体或者同种异体来源获得全血或者其一种或多种组分(例如白细胞和血浆)。在本发明的一个实施方案中,从最终用ALC处理的患者中收集血液样品,所述血液样品在本文中是指自体血液样品或者来源。在从非预期ALC受体的个体中获得来源(即血液或者其组分)(是指同种异体血液样品或者来源)的实施方案中,这些起始材料可以便利地从血库中获得。由血库对样品进行血型(ABO、Rh)、红细胞抗原的不规则抗体和输血可传染性疾病(transfusion-transmittable disease)的筛选。更具体地,可以使用Abbott PRISM仪器利用乙型肝炎、丙型肝炎、HIV 1/2、HTLV和梅毒的抗体(-HCV、HbsAg、抗-HIV 1/2 O+和抗-HTLV I/II)进行筛选。还可以利用分子方法(NAT-Nucleic Acid Testing)对样品进行HIV、HCV和HBV的筛选。分子筛选可以利用市售仪器(例如Chiron的TIGRIS系统)进行。
在涉及同种异体来源的这些实施方案中,样品可以从与预期ALC受体具有相同血型的供体中获得。可选地和如本文中进一步描述的,可以从具有AB+血液的供体中获得血浆样品,从具有O-血液的患者中获得白细胞。具有AB+血液的患者是血浆的全适供体,具有O-血液的患者是白细胞的全适受体。不管来源如何,无需高度专业化的设备即可对样品进行所有必要的处理。
制备本发明ALC组合物的一种优选方法利用图1和图2进行描述,其中说明了含有两组相互连接的无菌输液袋的系统。该系统是密封的,以使其不暴露于外部环境中。具体地,利用无菌连接设备(Sterile Connecting Device,例如,TSCD-II,Terumo产品号ME-203AH)将连接两个组的导管焊接到一起,以形成一个系统。更具体地,为了保证符合无菌标准,通过将特殊晶圆预加热至300℃,来进行导管的焊接和切割。这种高温增加了焊接步骤的无菌性。为了进一步保证无菌,可以在100级的生物安全柜中在100,000级的阻隔区内进行焊接。
如这些图中所说明,该系统含有两个无菌袋组。第1组含有袋A、B和C,是通常用于输血的标准市售三袋组。通过静脉穿刺并置于袋A中,将人血液样品(通常体积为大约400至大约550ml)收集于血库中,接着利用标准技术使其分级成各组分至袋A、B和C中。例如,将含有血液样品的袋A离心。离心后,例如利用Baxter生产的血液组分提取器使血液组分分离。将含有白细胞的血沉棕黄层置于袋C中,血浆置于袋B中,红细胞保留于袋A中。因此,该过程之后,袋A含有充满的红细胞;袋B含有血浆;和袋C含有含有白细胞(还可能含有残留的血浆和红细胞)的血沉棕黄层。可选地,可以通过本领域已知的血液分离技术使血液组分分离。
接着将袋A从三袋组中分离。如图2中所说明,接着将袋B和袋C焊接至定制的输液袋1-5(第2组)上,以形成用于制备活化的白细胞组合物的系统。袋1含有第一水溶液(例如200ml无菌蒸馏水),其用于使血沉棕黄层中的细胞发生低渗休克。这使得可能存在的残留红细胞裂解。袋2含有第二溶液(例如20ml氯化钠缓冲液(8.91%NaCl,USP),或者含有无机离子的任何其他生物上可接受的溶液、有机渗透物(例如蔗糖)或者其组合(例如乳酸林格(Hartmans)溶液)),其用于使白细胞在低渗休克后恢复等渗性。当将氯化钠溶液加入到200ml蒸馏水中时,其成为0.9%的NaCl溶液。袋3含有第三溶液(例如大约60ml氯化钙缓冲液(1.17%CaCl2,二水合物,USP)),其用于使袋B中的血浆凝固,有助于分离成为血小板和血清。袋4和袋5分别含有大约60ml和大约500ml的无菌过滤空气。将该组包装成单个部件,并利用高压蒸汽灭菌,这大大降低了患者继发性感染的风险。
接着将白细胞从袋C转移至袋4(或者袋5)中并进行孵育,优选处于垂直位置,以使其活化。为了本发明的目的,细胞活化定义为使细胞(白细胞)开始活化的过程,更具体地,定义为从静态至功能活化状态的转变,其中伴有生物活性物质的合成或者预先合成的物质从胞质转位至细胞膜或者释放到细胞外。这些物质可以包括蛋白或者多肽、脂类、糖、氧自由基和用作粘附分子、细胞因子、生长因子、酶、转录因子和细胞信号受体和介导物的其他生化部分。当在细胞内部或者细胞表面被检测和鉴定时,这些分子被称为活化标志物。
在一些实施方案中,通过使白细胞保持于室温中,对其进行简单孵育。为了本发明的目的,室温是指范围为大约12℃至大约28℃的温度,在一些实施方案中,是大约16℃至大约25℃。孵育时间根据温度而变化。在较高温度下孵育时间较低。使白细胞活化所需的孵育时间大体上与进行孵育的温度成反比。例如,在使白细胞保持于室温中的实施方案中,孵育时间总体上从大约90分钟和上至2、3、4、5、6、7或者8小时至大约20小时。在优选的实施方案中,孵育时间是大约8至大约20小时。在一个更优选的实施方案中,白细胞的孵育于18℃至大约24℃进行大约8小时至大约12小时。在其他实施方案中,白细胞的孵育包括对其进行加热,例如在高于室温的温度下,高至大约37℃。较高温度下的孵育时间一般是大约5小时至大约24小时。
再次参考图2,将袋C中的白细胞转移至通常小于袋C(例如大约250ml-大约500ml)的袋4中。还可以将细胞转移至另一个500ml的袋(袋5)中。为了使白细胞活化,将袋4(或者袋5)置于垂直位置并于室温孵育大约8至大约20小时。
孵育后,对白细胞进行低渗休克,这使得红细胞溶解。在优选的实施方案中,立即进行低渗休克(即前述步骤完成后,不进行间隔步骤或者不必要的耽搁,通常少于2分钟)。可以通过将蒸馏水从袋1转移至含有白细胞的袋4(或者袋5)中,开始进行低渗休克。低渗休克处理通常进行大约45秒。该步骤后,优选其后立即通过将氯化钠溶液从袋2转移至袋4(或者袋5)中,使等渗性恢复。氯化钠溶液与细胞水悬浮液的体积比通常是大约1∶10。接着将袋4中的内含物转移至体积大于袋4的袋1(或者袋5的左侧)中。如上面所描述,该组合物在本发明的方法中可以用于治疗。
一个优选的实施方案包括至少一个额外的步骤。因此,在可以与白细胞孵育同时进行的分离步骤中,通过利用凝固剂(例如CaCl2)随后离心,使血浆分离成为血小板和血清。因此,在该代表性实施方案中,将袋3的CaCl2转移至袋B中。通常使含有血浆和CaCl2组合物的袋B在大约37℃的温度下凝固。血浆与凝固剂在基本上与白细胞孵育时间相同的一段时间内保持接触。
白细胞低渗休克和等渗性恢复并随后将袋4中的内含物转移至袋1中(或者保留于袋5中)后,以及袋B中的血浆凝固后,将整个7袋系统离心。在优选的实施方案中,将白细胞转移至袋1中(或者保留于袋5中)后立即进行离心,以使细胞不会发生溶血。离心后,将袋1(或者袋5)的上清液转移至袋C中,并将离心后袋1(或者袋5)中形成的白细胞沉淀重悬于大约20ml至大约50ml袋B中的血清中。
而在本发明优选实施方案的另一个步骤中,在制备最终组合物前,使白细胞在上面提到的已凝固的血浆中进行孵育。通常该第二孵育期于37℃进行大约1-2小时。
在其他实施方案中,可以从较小体积的血液样品中制备ALC组合物,其中所有溶液的体积相应减少并使用较小的袋。此外,使用这些大小不同的袋产生具有不同组成的ALC。甚至在这些实施方案中,可以使用同种异体或者自体的血液样品作为起始材料。使用较小体积为临床医师提供了不必利用外部血库而进行自主血液收集的能力,例如在紧急情况时对具有健康免疫系统但患有某些类型的外伤伤口的患者进行治疗(例如战场和战斗情况)。在这些实施方案中,提供对可传染疾病和抗原的测试。但是在这些情况下,具有难治性伤口的患者不是制备有效ALC的临床上可接受的血液供体。当发生这种情况时,通过本文中描述的方法从同种异体供体中产生ALC。
本发明的ALC包括白细胞,例如粒细胞、单核细胞和淋巴细胞。粒细胞包括嗜中性粒细胞、嗜酸性粒细胞和嗜碱性粒细胞。从其最广泛的意义来说,ALC的白细胞群包含大约40%至大约90%的粒细胞、大约5%至大约20%的单核细胞和大约5%至大约30%的淋巴细胞。细胞的特定量基于所实施的分析技术而不同。当利用FACS(例如利用侧向散射对前向散射散点图分析)进行分析时,白细胞组合物一般包含大约55%至大约80%的粒细胞、大约5%至大约15%的单核细胞和大约5%至大约30%的淋巴细胞。在一些实施方案中,包含大约58-76%的粒细胞、大约5-11%的单核细胞和大约9-23%的淋巴细胞。当利用Cell Dyn分析仪进行分析时,白细胞组合物一般包含大约50%至大约90%的粒细胞、大约5%至大约15%的单核细胞和大约10%至大约25%的淋巴细胞。ALC中的淋巴细胞亚群经确认包含大体范围如下的下列细胞:大约7%至大约25%的B细胞(CD19+)、大约20%至大约30%的NK细胞(CD3-/CD56+)、大约40%至大约60%的T细胞(CD3+)、大约0.1%至大约30%的NKT细胞CD3+/CD56+、大约8%至大约20%的辅助性T细胞(CD4+/CD3+)和大约20%至大约30%的CD8+/CD3+细胞。在优选的实施方案中,淋巴细胞亚群富含至少9%的CD56+细胞(CD3-/CD56+、CD3+/CD56+、CD3+/CD56+/CD8+),辅助性T淋巴细胞(CD4+/CD3+)的量降至少于20%,和/或辅助性T细胞与抑制性T细胞(CD4+/CD3+∶CD8+/CD3+)的比率小于0.8。
具有伤口的患者可以是生理上缺乏免疫力的或者是健康的。例如,由于代谢系统已经受损,糖尿病和其他医学上缺乏免疫力的患者是来源于异源血液之ALC的候选者,因为其自身的白细胞对于该方案不是最佳的。但是,如在外伤患者的实施例中,健康患者也是本发明ALC组合物的适宜候选者。
本发明可用于促进大量类型的伤口愈合。尽管在实践中可以与其他治疗形式结合使用,但是其并不需要其他治疗形式来达到有效的伤口愈合。发明人已经考虑将ALC施用于任何类型的伤口,并且预料可以治疗的伤口类型不限。容易施用(例如利用标准注射器或者类似的施用设备)使得本发明的ALC组合物安全并且易于使用。
可以用本发明处理的伤口通常是有生命的组织的刺伤、切口或者撕裂形式。皮肤伤口可以穿透表皮、真皮,或者在全皮层受损伤口的情况中穿透皮下组织。因此,可以用本发明的组合物和方法进行治疗的伤口的代表性类型包括褥疮或者压力性溃疡、糖尿病性溃疡、深部胸骨伤口(例如开胸手术(冠状血管再生并获取大隐静脉后对大隐静脉进行)后)以及腹腔手术和任何其他类型的手术后的术后伤口。其他伤口是外伤引起的那些伤口,所述外伤例如通过枪击、刀或者能够产生皮肤切口或者撕裂的任何其他物体。口腔(例如牙齿)伤口,以及作为药物的副作用或者作为多种病症的症状出现的伤口(例如与卡波剂氏肉瘤(Kaposi’s Sarcoma)相关的疮),以及内部伤口(例如肛裂和胃肠道伤口或者损伤(例如胃或肠溃疡))也可以用本发明进行治疗。
ALC还可以用于治疗由血管功能不足恶化的任何伤口。为了本发明的目的,血管功能不足是指血液循环不足,导致其不足以充满受损区域。这种不足可以由外伤(例如骨折附近的脉管损伤)或者多种病症(例如糖尿病和动脉粥样硬化)引起。在另一种情况下,无论是外伤或者疾病诱导,血管功能不足使伤口有效愈合的可能性降低。ALC可以用于促进这些患者中的伤口愈合效果,并且应当根据本文中描述的方法施用。此外,治疗方案不应当受到严重性或者伤口类型或者血管功能不足程度的限制。ALC对于表现出最严重的伤口和血管功能不足的患者更加有效。
大体上,通过将ALC一次或者多次直接注射到伤口或者伤口周围的组织中,来施用活化的白细胞组合物。可以将ALC直接施用到开放性伤口中。
对于注射到伤口中,优选使用Luer-Lock注射器或者在注射器和针头之间具有锁定机制的任何其他市售注射器。伤口特别是压力性伤口的生物学区域通常是有限。当注射到伤口中时,存在压力使注射器与针头分离的风险。利用锁定注射器消除了这种风险。
当不能注射到伤口组织时,可以将ALC直接施用到伤口的洞中。使用该方法可以通过用注射器或者管直接施用。
可以在敷料的帮助下将ALC施用至伤口位点或者其周围。干性敷料包括纱布和绷带、非胶粘网筛、膜和箔、泡沫和组织粘合剂。保湿隔离敷料包括膏、乳液和软膏、不可透或者半透膜或箔、水胶体、水凝胶和组合产品。生物活性敷料包括抗菌敷料、交互式敷料、单组分生物敷料和组合产品。在一些实施方案中,用在ALC中浸泡过的无菌纱布包扎伤口。可以用组合物(例如乳酸林格(Hartman)溶液、含有藻酸盐的敷料、聚氨酯敷料或者羟甲基化纤维素敷料)使敷料(例如无菌纱布块)浸透,所述组合物覆盖伤口,随后施用干性敷料。如果伤口对象高度感染,接着可以施用银敷料,例如Silverlon。注射后敷料的选择基于临床医生的决定。市售、以前临床成功的经验和患者耐受力均是选择伤口敷料时需要考虑的因素。可以周期性地(例如通常大约24小时后)移除敷料,以冲洗伤口(例如用无菌水和肥皂)。
在另一个实施方案中,可以将组合物置于生理惰性和/或可吸收的基质或者支架(例如胶原)上,并利用压配合插入伤口中。这使得可以将ALC持续递送到位点中,使患者受益,因为细胞在原位存活时期较长。
可以对伤口施用一次或者多于一次的ALC组合物,例如4周后,一旦临床医生确定再次施用是否是必要的。需要考虑的因素包括伤口面积(宽度、长度和深度)的增加、化脓、发热或者表明有必要进行再次治疗的顽固性感染的任何其他标志或者症状。除了再次治疗之外,在临床医生认为合适的任何时刻均可以推荐进行外科清创术。
ALC可以与任何其他常规伤口治疗方法结合使用,例如升温(治疗性加热)、电刺激、磁疗、激光冶疗、摆线振动治疗和超声波。其还可以与生物疗法结合使用,例如蛆疗、皮肤替代、培养的角蛋白形成细胞(Epicel,Genzyme biosurgery)、人真皮替换(Dermagraft,Smith and Nephew Inc.)、尸源加工真皮(Alloderm,Life Cell Corporation)、双层皮肤等同物(Apligraf,OrganogenesisInc.)、TransCyte(Smith and Nephew Inc.)、生长因子(PDGF是唯一批准局部使用的生长因子)和血纤蛋白粘合剂。在一些实施方案中,ALC与由KCI生产的市售伤口疗法VAC结合使用。VAC通过对伤口实施负压来促进伤口愈合。在这些实施方案中,伤选在VAC治疗之前对伤口施用ALC。而在其他实施方案中,ALC与高压氧治疗(Thackham,2008)结合使用。例如,在患者接受高压氧治疗之前对伤口施用ALC。ALC还可以与低能休克波治疗(例如,大约0.1mJ/mm2的脉冲;每厘米伤口长度5Hz)结合使用。参见例如Dumfarth等,Ann.Thorac.Surg.86:1909-13(2008)。
治疗后,可以对伤口的长度、宽度和高度测量值进行评估。通常,当所有这些参数测量值均可忽略不计时,认为伤口已经愈合。ALC还提供镇痛作用。
该活化的白细胞组合物特别用于包括糖尿病性足溃疡和褥疮的伤口中。褥疮是由血液流动不良引起的压力性溃疡,通常是由于特定区域上的长时间压力(Berlowitz,2007)。褥疮在老年人中发病并且致死。至少48%的IV期压力性溃疡在治疗一年后仍未治愈(Girouard,2008)。患有褥疮的患者还通常患有共病病症(co-morbid pathologies),例如糖尿病和高血压。这些病症使褥疮的成功治疗进一步复杂化。
在治疗褥疮的一个实施方案中,利用18针规(18G)的针将组合物吸入任何大小的注射器中。缓慢吸入,以使对细胞的损伤最小化。尽管注射器和针的大小不受任何限制,优选吸入时使用大针规的针。这有助于转移并且减少细胞损伤。
对溃疡施用活化的白细胞组合物包括将组合物注射到伤口中。可以施用注射器中的全部样品,而且临床医生可以选择施用额外的ALC,如果基于临床参数确定这是必要的。
将吸入时使用的18G针调换成大小为22-35G的针。可以将ALC注射到伤口的多个位点。在一个实施方案中,在伤口总长度的大约每1厘米至大约每3厘米进行注射。在每个注射位点注射0.1-0.3ml的ALC。
在另一个实施方案中,可以将整个注射器一次注射到伤口内部单个位点中。
将根据下面非限定性实施方案对本发明的方面进行描述。
实施例
实施例1-细胞活化的分析
通过分析多种细胞表面标记物,对根据本发明优选的实施方案制备的活化的白细胞组合物进行定量。根据P-选择素的表达所显示的血小板与单核细胞或者粒细胞的凝集增加是单核细胞和粒细胞活化的标志。CD62L是活化过程中消失的质膜蛋白,因而随着细胞活化而减少。CD42b是作为凝固因子参与凝固过程的血小板活化标志物。其与胞外基质以及粘附分子相互作用,并且在本发明中还用作单核细胞和粒细胞活化的指示物。
在3个时间点对细胞进行取样:新鲜的血沉棕黄层(Fr.BC)、经孵育的血沉棕黄层(IBC)和最终活化的白细胞组合物(FP)(表1)。

制备血沉棕黄层后立即提取Fr.BC。第一孵育期后提取IBC,并从终产物中提取FP。在每个时间点,用特异性单克隆抗体(别藻蓝蛋白(APC)偶联的CD14抗体、藻红蛋白(PE)偶联的CD42b抗体和异硫氰酸荧光素(FITC)偶联的CD62L抗体)对细胞进行标记,并接着用FACS进行分析。
用FACS染色液(PBS,2%的正常小鼠血清;0.02%的叠氮化钠)对每个时间点的细胞进行清洗,取样并根据染色步骤进行染色。简单地讲,0.5×106/μL的细胞与适宜的单克隆抗体在黑暗中于室温(RT)或者+4℃孵育30min。孵育后,用红细胞裂解液处理细胞组合物,清洗,并接着最终重悬于PBS中,以用于进行FACS。对于CD62L阳性(+)染色,将细胞组合物与人CD14(APC)抗体和人CD62L(FITC)抗体于+4℃进行孵育。对于CD42b+染色,将细胞组合物与CD14(APC)抗体和人CD42b(PE)抗体于室温(RT)进行孵育。阴性对照细胞组合物与人CD14抗体和适宜的同型对照抗体在相关条件下进行孵育。利用FACS CALIBUR(BectonDickinson)对细胞相关的光散射和荧光进行分析。CD14阳性细胞确定为单核细胞,并且通过特征性光散射性质鉴定粒细胞。CD62L和CD42b阳性细胞定义为荧光强于阈值之单核细胞和粒细胞的百分比,所述阈值根据与适宜的同型对照抗体孵育的单核细胞和粒细胞而测定。
如图3A和3B以及表1中所显示,结果与CD62L的表达随着活化进程减少和CD42b随着活化进程增加的预期一致。这些数据表明白细胞开始活化。
实施例2-活化的白细胞组合物的分析
表2和3描述了最终ALC的细胞组成,其利用Cell Dyn分析仪分析测定。将活化的白细胞重悬于血清后,对细胞进行分析。利用台盼蓝拒染试验对活细胞进行染色,并在显微镜下观察。

如表2中所显示,Cell Dyn结果表明,ALC包含46.8+/-39.2(103/μl)的血小板、0.1+/-.03(106/μl)的红细胞和6.8+/-3.8(103/μl)的白细胞。基于Cell Dyn分析,还测定了ALC的白细胞组成(如表3中所显示)包含52%-78%的中性粒细胞、1-2%的嗜碱性粒细胞、1-9%的嗜酸性粒细胞和6%-12%的单核细胞和13%-24%的淋巴细胞(将标准差考虑进去)。这些表中描述的范围代表利用Cell Dyn分析仪进行8次不同分析后的高值和低值。
实施例3
本发明的方法和现有技术过程之间的比较
为了强调与本发明相关的多个出乎意料的优势,将其实施方案与美国专利第6,146,890号中公开的方法相比较,即与Danon(“Danon”)相比较。
本发明的实施方案
参见图1和2,将全血分离成为其一级组分(即红血细胞、血浆和血沉棕黄层)后立即将血沉棕黄层从袋C转移至袋4中,并于室温孵育12小时±2小时。该步骤后,将蒸馏水从袋1转移至袋4(或者袋5)中,以进行低渗休克处理(使得产生溶血产物)。该处理进行大约45秒。其后立即将袋2中含有的氯化钠缓冲液转移至袋4(或者袋5)中,以使血沉棕黄层(白细胞)恢复等渗性。
上面孵育血沉棕黄层的同时,将袋3中的氯化钙缓冲液转移至含有血浆组分的袋B中,以使血浆凝固。这发生于12小时±2小时外加对白细胞进行低渗休克并接着恢复等渗性的额外短时间的过程中。
恢复等渗性后,立即将整个袋组件离心(通常大约8至大约10分钟),随后从溶血产物中分离细胞。这样做使细胞受到溶血产物影响的时间最小化,即大约10分钟。离心后,弃掉袋1(或者袋5)中溶血产物的上清液,并将新鲜培养基加入到袋1(或者袋5)中,随后于37℃孵育大约1-2小时。孵育后,将细胞清洗一次。
比较性(对比文件)步骤
根据Danon的教导,对全血进行处理并将其分成3种主要组分(即红血细胞、血浆和血沉棕黄层)后,立即对血沉棕黄层进行低渗休克。因此,与本发明截然相反,Danon的步骤在分离主要血液组分之后以及对血沉棕黄层进行低渗休克之前不必对血沉棕黄层进行孵育。进行低渗休克时血沉棕黄层处于袋PB3中。
作为一个单独的步骤,低渗休克之后,将氯化钙缓冲液从袋PB6转移至含有血浆的袋PB2中,随后使血浆在PB2中低温冻结10分钟,并接着将其置于37℃水浴孵育额外的30分钟。期间,使已经进行低渗休克的血沉棕黄层组分维持原状,其中白细胞仍然暴露于溶血产物中。
该凝固过程(总共持续大约40分钟)结束后,将整个袋系统离心,并弃掉溶血产物。因此,细胞暴露于溶血产物至少大约55分钟的时间(即其包括与血浆凝固同时发生的40分钟保持时间以及离心的额外15分钟)。相比而言,在本发明的方法中,立即将白细胞离心5分钟,因而其暴露于溶血产物的时间较短,即少于10分钟。
将袋PB3中的上清液弃掉后,其中加入新鲜培养基,随后将全部悬浮液转移回PB7,接着使其于37℃孵育大约17小时。孵育后,将细胞清洗3次。相比而言,在本发明的实施例中,已凝固血浆的孵育时间是1-2小时,并且仅清洗一次。
在进行前述孵育之前,所有先前的步骤均在收集全血的输液袋中进行。相比而言,本发明的实施方案使用通常用于施用静脉注射的溶液(例如盐溶液)的输液袋。
将Danon的步骤进行4次,并与由本发明的实施方案所产生的结果相比较。下面列出的结果是平均值。
结果
对于每一批全血,对标准血液单元中获得的白血细胞(白细胞)总量(用终浓度乘以细胞悬浮液终体积来计算)进行测定。根据本发明的方法处理的111批全血和根据Danon公开的步骤处理的4批全血的平均结果列于表4中。

该结果显示,与Danon的步骤相比,本发明从一个标准血液单元(大约450ml)中获得的白细胞总数量高大约90倍。更具体地,本发明的产量是每个标准血液单元至少大约100×106、125×106、150×106、175×106、200×106、225×106、250×106、275×106、300×106、325×106、350×106、375×106、400×106、425×106、450×106、475×106、500×106或者更多白细胞(包括其所有亚范围)。
通过台盼蓝拒染方法测定总细胞群中含有的活细胞数量(以百分比表示)。将细胞重悬于台盼蓝之后在Newbauer血细胞计数器中进行计数,以评估细胞数量和活力百分比。结果显示于表5中。

表5中的数据显示,除了产量较低(如表4中所显示)之外,Danon的步骤使得细胞悬浮液的活力显著降低。即几乎四分之一(即23%)的制备品是死的白细胞(死细胞的百分比高10倍多)。与此截然相反,根据本发明处理的几乎所有白血细胞经测定均是有活力的。更具体地,基于ALC中的白细胞总量,本发明的ALC包含至少80%、81%、82%、83%、84%、85%、86%、87%、88%、89%、90%、91%、92%、93%、94%、95%、96%、97%、98%或者更多的活的白细胞(包括其亚范围)。
通过流式细胞术对粒细胞中CD11b活化标志物的表达进行测定,并以CD11b阳性细胞在粒细胞群(CD15阳性细胞)中的百分比表示。从终产物中取样的细胞与异硫氰酸荧光素(FITC)偶联的CD11b抗体和藻红蛋白(PE)偶联的CD15抗体共染色,并利用FACSCalibur流式细胞仪(Becton Dickinson ImmunocytometrySystems,San Jose,CA,USA)进行分析。分析了根据本发明的方法进行的81批终产物和根据Danon的步骤产生的3批终产物细胞中粒细胞的CD11b表达。

如表6中的数据所显示,与Danon的步骤相比,本发明的方法所产生的活化的粒细胞高将近2倍(基于百分比)。更具体的,相对于ALC中的总粒细胞群,本发明的ALCs包含至少50%、51%、52%、53%、54%、55%、56%、57%、58%、59%、60%、61%、62%、63%、64%、65%、66%、67%、68%、69%、70%、71%、72%、73%、74%、75%、76%、77%、78%、79%、80%、81%、82%、83%、84%、85%或者更多的CD11b(+)粒细胞(包括其所有的亚范围)。
比较性实验的结果显示,与Danon公开的步骤相比,本文公开的发明产生至少3个出乎意料的结果,即活的白细胞产量较高、活细胞百分比高和如CD11b活化标志物表达较高所显示的粒细胞活化水平较高。这些增加是显著的和出乎意料的。

Claims (29)
1.一种制备活化的白细胞组合物的方法,其包括:
a.将人白细胞于室温孵育大约8至大约20小时;
b.对所述白细胞进行低渗休克;
c.将恢复等渗性有效量的生理上可接受的盐溶液加入到所述步骤b的所述白细胞中;
d.使人血浆的分离样品与凝固试剂相接触,其中所述白细胞与所述血浆可以从相同或者不同的供体中获得,并移除已凝固的固体,以获得血清;和
e.将所述步骤c的白细胞与所述步骤d的血清混合。
2.权利要求1的方法,其中所述孵育进行大约8至大约12小时。
3.权利要求1或者2的方法,其中所述白细胞和所述血清从不同的供体中获得。
4.权利要求1或者2的方法,其中所述白细胞和所述血清从相同的供体中获得。

c)大约5%至大约30%的淋巴细胞。
15.权利要求14的组合物,其进一步包含大约0.1×106μl的红细胞。
16.权利要求14或者15的组合物,其进一步包含大约的46.8×103μl血小板。
17.权利要求14-16任一项的组合物,其中所述粒细胞包含:
a)大约52%至大约78%的中性粒细胞;
b)大约1%至大约9%的嗜酸性粒细胞;和
c)大约1%至大约2%的嗜碱性粒细胞。
18.权利要求14-17任一项的组合物,其中所述淋巴细胞包含;
a)大约7%至大约25%的B细胞(CD19+);
b)大约20%至大约30%的NK细胞(CD3-/CD56+);
c)大约40%至大约60的T细胞(CD3+);
d)大约0%至大约30%的NKT细胞(CD3+/CD56+);
e)大约8%至大约20%的辅助性T细胞(CD4+/CD3+);和
f)大约20%至大约30%的CD8+/CD3+细胞。
19.一种治疗伤口的方法,其包括对伤口施用权利要求14-18任一项的活化的白细胞组合物。

21.权利要求19的方法,其中所述伤口是压力性溃疡。

27.权利要求19的方法,其中所述伤口是静脉溃疡。
28.权利要求19的方法,其中所述活化的白细胞组合物来源于自体的血液样品。
29.权利要求19的方法,其中所述活化的白细胞组合物来源于同种异体的血液样品。
