Abstract

技术领域
本发明特别涉及基于聚乙烯醇的医疗装置及其制备和使用方法。
背景技术
大多数长期矫形外科植入物(orthopedic implants)包含合成的疏水聚合物。例如,一些金属植入物具有由疏水聚合物例如超高分子量聚乙烯制成的关节面(articulating surface)。这些疏水聚合物的磨损颗粒经常会引起不良的免疫反应例如骨质溶解。此外,虽然这些聚合物是生物惰性的,但还是不能理想地适用作细胞支架或软组织替代物。因此,本领域需要块状形式或多孔构造的更加生物友好的植入材料。
发明内容
在一些方面,本发明涉及包含聚乙烯醇(PVA)的植入物,其中所述聚乙烯醇具有至少90%的水解度以及至少50,000的重均分子量。一些植入物进一步包含治疗成分(therapeutic composition)。在某些实施方式中,水解度是至少95%或98%。一些优选的PVA是交联的。
一些实施方式涉及矫形外科植入物。本发明的矫形外科植入物包括具有关节面的矫形外科植入物,所述关节面包含聚乙烯醇。一些植入物可以包含其它的材料例如水、增塑剂例如丙三醇、或治疗成分。
在一些方面,本发明涉及包含本文所述聚乙烯醇组合物的用于软组织修复和再生的支架。
本发明的其它方面涉及形成包含本文所述聚乙烯醇组合物的制品的方法。一种这样的方法包括:
将具有至少50,000的重均分子量以及至少90%的水解度的聚乙烯醇与一定量的一种或多种增塑剂接触,从而形成增塑材料,其中该增塑剂为聚乙烯醇的10-50%重量百分比;和
将所述增塑材料模塑以形成固结的制品。
在一些实施方式中,所述方法涉及将固结的含丙三醇的PVA制品水合至完全水饱和态。
在某些实施方式中,所述方法进一步包括通过将所述制品置于等于或低于0℃或-80℃的温度以及然后将所述制品置于低于大气压的压力来增加肖氏D硬度(Shore D hardness)。如果需要降低肖氏D硬度,包含交联PVA的制品可以在70℃至95℃的温度与水溶液接触。
在一些实施方式中,当聚乙烯醇与丙三醇接触时,聚乙烯醇是粒状的。合适的增塑剂包括多元醇例如丙三醇。增塑剂应当具有合适的热学性质以适应加工条件。
任何合适的固结方法均可用于形成制品。这样的方法包括压模和活塞式挤塑(ram extrusion)。
所述方法还可以进一步包括交联聚乙烯醇以形成交联的制品。
交联可通过本领域已知的任何方法进行。在一些实施方式中,交联是通过将聚乙烯醇暴露在高能电离辐射下完成的。
一些植入物和支架可以是多孔的。用于制备这些制品的某些方法使用可压模的材料,其进一步包含氯化钠。在一些方法中,交联制品在有效去除至少一部分丙三醇和氯化钠的条件下与水接触一段时间。在一些优选的实施方式中,通过将交联制品与水接触,至少90%的丙三醇和至少90%的氯化钠被去除。
本发明还涉及离子电渗疗装置,其包括室、在所述室中的治疗成分和与所述室相连的电源,所述室包含聚乙烯醇,该聚乙烯醇具有至少90%的水解度和至少50,000道尔顿的重均分子量。在一些实施方式中,治疗成分被透皮递送。在一些实施方式中,治疗剂具有正电荷或负电荷。

发明详述
本发明一般涉及包含聚乙烯醇的植入物,其中所述聚乙烯醇具有至少90%的水解度和至少50,000道尔顿的重均分子量。一些植入物还包含治疗成分。这种植入物可以置于动物(例如人类)体内并随时间释放治疗成分。这些过程是本领域普通技术人员公知的。
一方面,本发明涉及基于聚乙烯醇(PVA)的亲水矫形外科植入物。这些植入物不同于由疏水聚合物制造的植入物,其还适合用作细胞支架或软组织替代物。聚乙烯醇比用于制造传统植入物的聚合物更加生物友好。
在一些实施方式中,本发明的制品含有10至50重量百分比的水。在其它实施方式中,所述制品含有按重量计30%或更少的水。
本发明的优势之一是本发明的PVA结构在结构上比常规PVA水凝胶更坚固。一些结构具有至少35的肖氏D硬度。
聚乙烯醇可以是完全水解的PVA,其所有的重复单元为--CH2--CH(OH)--,或者是部分水解的PVA,其具有变化比例(1%-25%)的侧链酯基。具有侧链酯基的PVA具有结构--CH2--CH(OR)--的重复基团,其中R是COCH3基团或更长的烷基,只要所需的性质被保留。在一些实施方式中,PVA优选地具有至少98%的水解度。在某些实施方式中,PVA具有至少100,000道尔顿的分子量(Mw)。
PVA优选是交联的。PVA的交联可以通过例如高能电离辐射例如γ辐射来完成。一种这类的示意图在图5中显示。可选地,也可以使用化学交联。
本发明制品的硬度可通过将制品置于一个或多个冷冻干燥循环来调节。例如,制品可以在冷冻循环中被置于低于0℃、或-20℃、或-50℃、或-80℃的温度。制品可以置于冷冻温度几分钟到若干小时。例如,5分钟到24小时。干燥循环可以在小于大气压的压力下完成。例如,压力可以等于或小于10-2、10-4或10-6托。干燥循环可以在不同的温度下进行——在一些实施方式中低于0℃。一个或多个冷冻/干燥循环可以增加肖氏D硬度。在一些实施方式中,肖氏D硬度增加至少2或5或10个单位。
硬度还可以通过在高于70℃的温度将制品在水中浸泡来调节。在一些实施方式中,制品在高于80℃或90℃的温度浸泡。制品可进行浸泡几分钟到若干小时。例如,5分钟到24小时。在一些实施方式中,肖氏D硬度降低至少2或5、10或20个单位。
如本文所用,术语“硬度”指的是用硬度计测量的厚平板或钮扣形式的非金属材料的压痕硬度。硬度计具有弹簧加载压头,其向厚平板施加压入载荷,因而感应它的硬度。硬度可以间接反映其它材料性质,例如拉伸模量、回弹性、可塑性、耐压性和弹性。材料硬度的标准测试包括ASTM D2240。除非另有说明,本文所述的材料硬度是以肖氏D的形式。
本发明的制品(植入物和支架)可以是真空箔包装的。这些技术是本领域普通技术人员公知的。这些技术包括已知为γ真空铝箔(Gamma Vaccum Foil,GVF)的方法,如Hamilton等人在美国专利号5,577,368中所公开。
聚乙烯醇具有高熔点,并且通常认为其在熔融之前降解。一方面,本发明提供新的压模方法,其在压模以前允许通过用丙三醇增塑PVA树脂来制备PVA组分。增塑过程可以例如通过将PVA树脂浸泡在丙三醇中进行。在一些实施方式中,浸泡是这样进行的:首先将PVA在室温浸泡,随后在高于70℃(在一些实施方式中高于80℃)的温度热浸泡四小时或更长的时间来得到增塑的PVA树脂。然后,增塑的PVA树脂可以在350°F(176.7℃)和420°F(215℃)之间的温度用适当的压力进行固结。
如本文所用,增塑剂是一种成分,当其被添加到PVA时,提高PVA的柔韧性、加工性或模塑性。
一些实施方式包括使用压模来形成制品例如植入物。压模技术对本领域普通技术人员是公知的。在一些优选的实施方式中,低氧环境对增塑和/或压模是优选的。合适的低氧环境包括减压、氮气或氩气环境或其组合。
丙三醇是一种生物相容的润滑剂,可以用作矫形外科植入物的一部分。或者,PVA组分中的丙三醇可以通过在水中或盐水中的延长浸泡与水进行交换。这一后续步骤允许在PVA树脂中产生包含水或盐水而不是丙三醇的PVA组分。一些实施方式可以使用不同于丙三醇的增塑剂。在某些实施方式,使用了其它的多元醇。
丙三醇
“支架”指的是支持基质,组织可以在其中以预先确定的形状生长。这一形状是通过支架的形状预先确定的。支架发挥支持和定形再生组织的功能。支架的制造是本领域公知的。
“植入物”指的是适合于植入组织中的制品(例如移植物、装置、支架或关节替换部分(joint replacement component))。植入装置是本领域熟知的。可受益于本发明的关节包括但不限于膝盖、踝、肩、肘和腕。
如本文所用,术语“水饱和”和“完全水解”被认为等同。
治疗剂也可以是与植入物或支架共价连接或者被包含在植入物或支架中。治疗剂是被化学连接或者酶连接的。治疗剂可以不经过进一步修饰而连接,或者它可以通过间隔臂(spacer arm)结合。如果使用间隔臂,该间隔臂可以具有在谨慎的生物条件下能够使间隔臂断裂的位点。一旦间隔臂断裂,生物制剂然后就可从植入物或支架中自由扩散。可以使用与PVA材料相容的治疗药物。
合适的治疗剂包括如下的一种或多种:趋化剂、抗生素、甾体和非甾体止痛剂、消炎药、抗排斥剂例如免疫抑制剂和抗癌药、各种蛋白质(例如短链肽、成骨蛋白、糖蛋白和脂蛋白)、细胞附着介质、生物活性配体、整合蛋白结合序列、配体、各种生长和/或分化剂(例如表皮生长因子、IGF-I、IGF-II、TGF-β、生长和分化因子、成纤维细胞生长因子、血小板衍生生长因子、胰岛素样生长因子、甲状旁腺激素、甲状旁腺激素相关肽、BMP-2、BMP-4、BMP-6、BMP-7、BMP-12、音猬蛋白(sonic hedgehog)、GDF5、GDF6、GDF8、PDGF)、影响特定生长因子上调的小分子、腱生蛋白-C、透明质酸、硫酸软骨素、纤连蛋白、核心蛋白聚糖、血栓弹性蛋白、凝血酶衍生肽、肝素脂、硫酸乙酰肝素、DNA片段和DNA质粒。如果这样的其它物质在矫形外科领域具有治疗价值,可以预期这些物质中的至少一些应在本公开的概念中具有用途,并且这些物质应当被包括在“治疗剂”的含义中,除非另有明确限制。
在一些实施方式中,本发明的装置是离子电渗疗装置。这些装置将治疗剂以非入侵的方式施用至病人。在一些实施方式中,使用排斥电流力(repulsive electromotive force)将治疗剂透皮施用。这种力可以使用应用于离子电渗疗室的小电荷,所述离子电渗疗室使用本文所述的PVA材料制造。离子电渗疗装置包含至少两个电极。通常,两个电极均定位于与机体皮肤的某个部分紧密电接触。一个电极发挥室的功能或与室相连接,包含待递送的治疗剂。第二个电极发挥完成穿过机体的电回路的功能。室可以包含具有与室相同电荷的治疗剂。例如,带正电荷的室可以用于从装置发射带正电荷的治疗剂。同样地,带负电荷的室可利用带负电荷的治疗剂。在某些实施方式中,治疗剂是水溶性制剂。某些治疗剂是局部麻醉剂,例如盐酸利多卡因和盐酸芬太奴。参见例如Pakinson等人,Drug Delivery Technology,Vol.7,No.4,第54-60页(2007年4月)。
与传统的透皮贴剂(transdermal patches)相反,从离子电渗疗装置递送制剂可以通过控制施加给装置的电流来控制。除了控制施加给装置的电流,药物递送还受皮肤pH、装置中制剂的浓度、制剂的特性例如电荷、电荷浓度和分子量、以及具体病人的皮肤抗性的影响。
用于递送具有正电荷或负电荷的治疗剂的一些离子电渗疗装置包括:(i)由聚乙烯醇聚合物形成并包含带正电荷或负电荷的治疗剂和对抗离子的储存器,和(ii)包含下述材料的导电元件,该材料在导体元件接触储存器并且正电压或负电压被施加于储存器时可容易氧化形成带电的离子种类。在一些实施方式中,当包含PVA的储存器被水合时,其对治疗剂是可渗透的。
本发明通过以下实施例来说明,所述实施例的意图是说明性且非限制性的。
实施例
实施例1:交联的PVA植入材料
将15.0克PVA(99+%水解,166,000道尔顿Mw)与4.5ml丙三醇混合,并将混合物浸泡24小时。然后,将混合物在80℃热浸泡8小时。将产生的增塑PVA树脂转移至3.5”直径、3片式模具(3-piece mold)上进行固结。将PVA树脂以5-10°F/min的加热速率加热到420°F(215.5℃),并且在1,000psi压力下固结10分钟,随后以10-15°F/min的速率冷却。产生的PVA板(plaque)用真空铝箔袋包装,用于50KGyγ辐射处理。
含有丙三醇的PVA与交联的含有丙三醇的PVA的拉伸数据显示于表1。拉伸测试使用V型测试样品根据ASTM D 638进行。

在丙三醇存在的情况下,当暴露于γ辐射时,PVA交联形成网状结构。辐射交联后,所有拉伸性质都有明显改进。令人感兴趣的是,交联将断裂能从47in-lb提高至69in-lb,明显改进了韧性和结构完整性。
实施例2:水饱和的交联PVA植入材料
将30.0克PVA(99+%水解,Mw=166,000道尔顿)与9ml丙三醇混合,并将混合物浸泡过夜。然后,将混合物在194°F(90℃)热浸泡6小时。将产生的增塑PVA转移到3片式模具上进行固结。固结在400°F(204.4℃)、1200psi下进行10分钟(加热速率:5-10°F/min,冷却速率:10-15°F/min)。产生的模制板在铝箔袋中真空包装。然后,用75KGyγ辐射处理板。然后,将模制的板在蒸馏水中浸泡两天以置换丙三醇。
压缩测试使用以下方法进行。五盘测试样品(直径0.50”×高度~0.19”)在MTS Insight 5测试仪上在平行板之间以十字头速率0.4”/min被负载压缩。当压缩负载超过载荷传感器等级(load cell rating)(950Lb)的95%时,停止测试。没有未通过压模的测试样品。
双缺口Izod冲击测试使用以下方法进行。将五个矩形测试样品(0.25”×0.50”×2.5”)打上缺口,并基于ASTM F 648进行测试。这一测试用于评估水饱和聚乙烯醇的韧性,将其与最具韧性的聚合物之一的超高分子量聚乙烯比较。测试结果显示水饱和的交联聚乙烯醇在冲击强度方面可与超高分子量聚氨酯(UHMWPE)相比。
表2显示水饱和的交联PVA样品的压缩性质和抗冲击性。

以潮湿的形式,交联的PVA是易弯曲的并具有高压缩强度和抗冲击性。
实施例3:大孔PVA
将20.0克PVA(99+%水解,146,000Mw)与6.0ml丙三醇混合并浸泡过夜。然后,将混合物在105℃热浸泡6小时以产生增塑的PVA混合物。然后,使用Turbula混合器将10.0克食盐与增塑的PVA树脂混合。产生的混合物的固结使用实施例2所述的模塑循环进行。模制的制品在水中浸泡5天的延长时间以滤去盐和用水交换丙三醇。表3显示了多孔水饱和PVA的特性(拉伸测试根据ASTM D638使用V型测试样品进行)。

实施例4冷冻-干燥的PVA材料
将20.0克PVA(99+%水解,Mw=166,000道尔顿)与6ml丙三醇混合,并将混合物浸泡过夜。然后,将混合物在110℃热浸泡四小时。然后,将产生的增塑PVA树脂转移到3.5”D、3片式模具进行固结。固结在380°F、600psi的压力下进行5分钟(加热速率:5-10°F/min,冷却速率:10-15°F/min)。
然后,在室温将这种非交联的PVA材料在水中浸泡两天以用水交换丙三醇。丙三醇增塑的PVA的硬度为62(肖氏D),水饱和PVA具有34.5%的水含量(水重量/PVA重量)和38的硬度(肖氏D)。
这种水饱和PVA块(block)进一步通过在-80℃过夜冷冻并在40×10-6托下干燥6小时的冷冻干燥循环来处理。冷冻干燥的PVA具有46的硬度(肖氏D)。
实施例5降低结晶度的交联PVA
将40.0克PVA(99+%水解,Mw=166,000道尔顿)与12ml丙三醇混合,并将混合物浸泡过夜。然后,将混合物在176°F(80℃)热浸泡六小时。然后将产生的增塑PVA树脂转移到3.5”D、3片式模具进行固结。固结采用两个浸泡阶段的方法进行:在220°F(104.4℃)、1040psi下进行5分钟和在400°F(204.4℃)、1560psi下进行15分钟(加热速率:5-10°F/min,冷却速率:10-15°F/min)。产生的模制板在铝箔袋中真空包装并且进行50KGyγ辐射。
由于过程中的损失和少量丙三醇从PVA中渗出(bleeding),交联的PVA材料包含17.3%的丙三醇(丙三醇重量/PVA重量)。这种材料是相对刚性的,具有66的硬度(肖氏D)。然后,将交联的PVA材料在80℃水中浸泡两小时。热水浸泡过程去除了丙三醇并溶解了非交联的PVA。这使交联的PVA明显软化。重构的PVA具有34.4%的水含量(水含量/PVA含量)和36的硬度(肖氏D)。然后,水饱和的交联PVA经过在-80℃冷冻过夜并在40×10-6托下干燥6小时的冷冻干燥循环。冷冻干燥的PVA块具有42的硬度(肖氏D)。
Claims (15)
1.一种医疗装置,其包含:
聚乙烯醇,其中所述聚乙烯醇具有至少90%的水解度和至少50,000道尔顿的重均分子量,和
治疗成分;
所述装置具有水和增塑剂中的至少一种,其含量为10-50重量百分比。
2.权利要求1所述的医疗装置,其中所述聚乙烯醇是交联的。
3.权利要求1所述的医疗装置,其中所述聚乙烯醇是至少98%水解的。
4.权利要求1所述的医疗装置,其进一步包含增塑剂。
5.权利要求4所述的医疗装置,其中所述增塑剂包括丙三醇。
6.权利要求1所述的医疗装置,其进一步包含水。
7.权利要求1所述的医疗装置,其中所述医疗装置是矫形外科植入物。
8.权利要求1所述的医疗装置,其具有包含所述聚乙烯醇的关节面。
9.权利要求1所述的医疗装置,其中所述医疗装置是用于软组织修复和再生的支架。
10.一种形成制品的方法,其包括:
将具有至少50,000道尔顿的重均分子量和至少90%的水解度的聚乙烯醇与一定量的一种或多种增塑剂接触,从而形成增塑材料,其中所述增塑剂为聚乙烯醇的10-50%重量百分比;和
将所述增塑材料模塑以形成固结的制品。
11.权利要求10所述的方法,其中所述增塑剂包含丙三醇。
12.权利要求11所述的方法,其进一步包括交联所述聚乙烯醇以形成交联的制品。
13.权利要求12所述的方法,其进一步包括将所述交联的制品在有效去除至少一部分丙三醇的条件下与水接触一段时间。
14.权利要求10所述的方法,其进一步包括通过以下步骤改变所述制品的硬度:
(a)将所述制品置于低于0℃的温度,然后将所述制品置于小于大气压的压力,或
(b)将所述包含交联PVA的制品在高于70℃的温度置于水溶液。
15.一种离子电渗疗装置,其包括:
包含聚乙烯醇的室,其中所述聚乙烯醇具有至少90%的水解度和至少50,000道尔顿的重均分子量;
在所述室中的治疗成分;和
与所述室相连的电源。
