Abstract

Description
技术领域
本发明涉及肿瘤靶向治疗领域,具体涉及靶向肿瘤相关巨噬细胞(TAM)所分泌的促肿瘤进展因子CCL18来抑制乳腺癌细胞浸润和转移的方法及其应用。
背景技术
乳腺癌转移是肿瘤治疗失败的主要原因,现在研究认为肿瘤微环境中的肿瘤相关巨噬细胞(TAM)促进乳腺癌转移、与病人的预后呈负相关。目前乳腺癌的治疗主要有手术、化疗、放疗、内分泌治疗和靶向治疗几大治疗手段,但多种治疗手段均不能完全阻止乳腺癌的浸润和转移。因而,目前临床上采用综合治疗手段,所采用的具体实施方案为:对于一个乳腺癌病人,首先要进行新辅助化疗,即在手术前实施的化疗,目的是降低肿瘤的分期,增加肿瘤根治手术或保乳手术的机会;清理全身其它部位的微小转移灶;观察化疗药物的疗效,为术后化疗提供经验。此后要进行乳房扩大根治术或保乳手术,降低肿瘤的局部负荷,为术后的化疗、放疗、内分泌治疗和靶向治疗提供可能。手术后再实施全身化疗和局部放疗,在快速增殖的乳腺癌细胞基本被消灭后实施约五年的内分泌治疗。在这漫长的治疗过程中或治疗结束后,许多病人复发、转移。寻其原因,目前的治疗手段靶向性比较差,内分泌治疗虽有一定的靶向性,但只对雌激素受体阳性的肿瘤有效;化疗、放疗只对快速增殖的肿瘤细胞有效,但对静止期的肿瘤细胞或对肿瘤微环境中处于分化终末期的细胞无效,而且对机体增殖较快的正常细胞同样有抑制作用,如口腔和胃肠道的粘膜细胞,从而产生呕吐、溃疡、食欲不振等众多的副作用。
乳腺癌靶向治疗最成功的范例是表皮生长因子受体的抗体赫赛汀的临床应用。表皮生长因子受体家族有四个成员,分别为HER-1、HER-2、HER-3、HER-4,在乳腺癌中都有表达,其中研究最多的是HER-2。HER-2(c-erbB-2)这种受体是具有酪氨酸激酶活性的跨膜蛋白,20-30%的晚期乳腺癌的癌组织中有HER-2基因过表达。HER-2阳性乳腺癌患者无病生存率和总生存率下降,预示对化疗和内分泌治疗药物耐药。为了解决HER-2阳性患者对许多治疗药物不敏感,预后差的难题,人们研究出曲妥珠单抗(Trastuzumab)即赫赛汀(Herceptin)投入临床使用。Herceptin是将人IgG1的稳定区和针对HER2受体胞外区的鼠源的单克隆抗体的抗原决定簇嵌合在一起的人源化单克隆抗体,该抗体是第一个用于临床的靶向治疗药物,主要用于治疗HER-2阳性的转移性乳腺癌,其作用机制为该药与HER-2结合后干扰后者的自身磷酸化及阻碍异源二聚体的形成,抑制信号传导通路的激活,从而抑制肿瘤细胞的增殖;在人体内诱导针对肿瘤细胞的抗体介导的细胞毒效应。Herceptin单用有效率为11.6%-21%,与环磷酰胺、泰素帝等联合化疗,有协同作用,与紫杉醇、阿霉素有相加作用。
Herceptin只适用于HER-2阳性的乳腺癌患者,适用范围较窄;它对心脏毒性强,从而限制了有心脏毒性化疗药物的应用剂量,另外Herceptin的有效率仍不令人满意。
目前研究认为肿瘤微环境中的肿瘤相关巨噬细胞在肿瘤细胞的调控下丧失了监视、抑制肿瘤的功能,具有免疫抑制、使肿瘤形成免疫逃逸、促进肿瘤浸润、转移。因而提出了靶向TAM的乳腺癌治疗,但对TAM分子靶点的研究还刚刚起步,TAM促进肿瘤浸润、转移的机制尚不清楚。
由于TAM大量存在于转移的或复发的乳腺癌组织微环境中,多种治疗手段不能清除TAM、阻止乳腺癌的浸润和转移。TAM对放疗不敏感,而且放疗作为一种局部治疗作用范围有限;化疗对快速增殖的乳腺癌细胞有效,但对分化终末期的TAM无效;内分泌治疗只对雌激素受体阳性的细胞有效但不能用于雌激素受体阴性的乳腺癌治疗,对TAM无效;虽然手术治疗是乳腺癌治疗的金标准,可以明显降低肿瘤的负荷,但对于微小转移灶中的TAM,手术是力所不能及的,不能完全阻止乳腺癌的复发和转移,最终使TAM成为肿瘤转移、复发的根源。
发明内容
为克服上述技术缺陷,本发明的目的是提供一种抑制乳腺癌细胞的浸润和转移的方法。
为实现上述目的,本发明提供了一种靶向促肿瘤进展因子CCL18来抑制乳腺癌细胞的浸润和转移的方法,该方法为靶向抑制肿瘤相关巨噬细胞分泌的CCL18。
具体可以利用CCL8的中和性抗体或沉默CCL18表达的siRNA抑制肿瘤相关巨噬细胞CCL18的作用。
因为目前研究证明TAM与IL-4激活巨噬细胞的表型与功能相似,在体外,把IL-4激活的巨噬细胞或TAM与乳腺癌细胞共培养可以模拟乳腺癌组织微环境,证明肿瘤相关巨噬细胞促进乳腺癌细胞浸润和迁移,本发明在共培养体系中加入CCL18的中和性抗体或用siRNA沉默IL-4激活巨噬细胞CCL18的表达,可以抑制巨噬细胞促进乳腺癌细胞浸润、迁移的作用,充分证明了TAM是通过CCL18来促进乳腺癌细胞的浸润和转移。
当选择利用CCL8的中和性抗体抑制肿瘤相关巨噬细胞分泌的CCL18作用的方式时,将CCL18的中和性抗体用于肿瘤相关巨噬细胞与乳腺癌细胞的共培养体系中,抑制CCL18的作用。
当选择利用siRNA抑制肿瘤相关巨噬细胞对CCL18表达时,将所述siRNA转染入IL-4激活的巨噬细胞,沉默CCL18的表达,然后再把巨噬细胞与乳腺癌细胞共培养,抑制CCL18促进乳腺癌细胞浸润、转移的作用。
本发明的目的之二是提供一种抑制乳腺癌细胞浸润和转移的药物,该药物含有抑制肿瘤相关巨噬细胞来源的CCL18的成分。
优选的,所述抑制肿瘤相关巨噬细胞分泌的CCL18的成分为靶向CCL18的中和性抗体。
优选的,所述抑制肿瘤相关巨噬细胞CCL18表达的成分为靶向CCL18mRNA的siRNA。
所述药物的剂型为注射剂。
与现有技术相比,由于TAM普遍存在于乳腺癌的微环境中,本发明应用CCL18的中和性抗体或siRNA抑制乳腺癌细胞浸润和转移的方法使得靶向TAM所分泌的CCL18的治疗具有普遍应用价值,使所有的患者临床获益,本发明将CCL18的中和性抗体或siRNA用于制备抑制乳腺癌细胞浸润和转移的药物,克服了赫赛汀(Herceptin)治疗对乳腺癌病例的选择性,达到治疗乳腺癌、改善患者预后的目的,弥补了手术、放疗两种局部治疗手段的不足,可以对微转移灶发挥作用;可以克服化疗对机体的毒副作用以及化疗对TAM无效的弊端;扩大靶向治疗的范围。

具体实施方式
为使本发明更加容易理解,下面结合具体实施例,进一步阐述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围,下列实施例中未提及的具体实验方法,通常按照常规实验方法进行。
实施例1:乳腺癌组织微环境中表达CCL18的TAM的数量与乳腺癌的浸润、转移相关,与患者的预后呈负相关的实验
为了研究TAM表达CCL18与乳腺癌患者预后的关系,收集了562例乳腺癌患者的手术切除标本,做了一系列的相关实验,实验结果见图1,图1是乳腺癌组织微环境中表达CCL18的TAM的数量与乳腺癌的浸润、转移相关、与患者的预后呈负相关的实验数据图。
首先,用免疫组化的方法分析乳腺癌微环境中表达CCL18的TAM的数量,结果见图1A和图1B,图1A是用免疫组化方法分析在乳腺癌的发生过程中,从良性乳腺到非典型增生、原位癌、浸润癌,肿瘤微环境中,表达CCL18的TAM的数量;由图1A可以看出,从良性乳腺到非典型增生、原位癌、浸润癌,肿瘤微环境中,表达CCL18的TAM的数量明显增多。由图1B可以看出,随病理分级的增高,表达CCL18的TAM的数量也逐渐增多。图1C是用免疫组化方法分析在乳腺癌微环境中,表达巨噬细胞特异性抗原CD68的TAM同时表达CCL18的情况;说明肿瘤微环境中的CCL18是由TAM表达的。图1D是用ELISA方法比较转移性乳腺癌病人、非转移性乳腺癌病人和良性乳腺病的病人血浆中CCL18的水平,证明转移性乳腺癌病人血浆中CCL18的水平明显高于非转移性乳腺癌病人和良性乳腺病的病人。
同时对所有病人的临床进展情况和生存情况进行随访,统计学分析表明表达CCL18的TAM的数量与乳腺癌的浸润、转移相关,与患者的预后呈负相关;图1E对562例乳腺癌病人手术切除的乳腺癌组织进行CCL18的检测,对病人的预后进行随访,生存分析表达CCL18的TAM数量与患者预后的关系;由该图可知,生存分析表明在乳腺癌微环境中表达CCL18的巨噬细胞数量与患者的预后呈负相关。
上述临床研究结合实验研究结果提示,在肿瘤微环境中,TAM分泌CCL18与乳腺癌浸润转移相关,与乳腺癌患者的预后呈负相关。
实施例2:证实CCL8促进乳腺癌细胞浸润和转移的相关体内实验
我们把乳腺癌细胞接种在NO-SCID鼠乳腺脂肪垫下,用CCL18(0.5μg/ml)、非相关的趋化因子CCL20(0.5μg/ml)、PBS在接种局部注射,同时设未处理组,每隔三天注射一次,持续约五周,然后处死小鼠,取实验鼠的肺,称取肺湿重,结果表明应用CCL18组肺湿重与其它组比有显著增大;取原发灶和转移灶作石蜡切片,进行HE染色,光镜下观察乳腺癌浸润和转移的情况,结果表明CCL18组原发肿瘤突破基底膜,出现大范围的肺转移,进一步说明CCL18促进乳腺癌浸润、转移。相关实验结果见图3。图3是CCL18促进乳腺癌细胞浸润和转移的体内证实实验的结果图,其中,图3A是光镜观察用CCL18、非相关趋化因子CCL20和PBS作用后乳腺癌浸润和转移的情况图;该图显示CCL18促进乳腺癌浸润肿瘤原发灶的基底膜,促进肿瘤远处转移到肺。图3B用实时定量PCR检测肺转移灶特异性人源化蛋白hHPRT的表达及肺的重量变化比较;该图显示远处转移灶所形成的肿瘤是人源性肿瘤;CCL18促进肿瘤肺转移,使小鼠肺的重量明显增大。
实施例3:研究IL-4激活的巨噬细胞对乳腺癌细胞作用的相关实验
目前研究认为TAM高表达CCL18,TAM的表型和功能与IL-4激活的巨噬细胞相似,我们用不同的方法证实IL-4激活的巨噬细胞与TAM相似,高表达CCL18。我们在transwell培养板中建立IL-4激活的巨噬细胞与乳腺癌细胞的共培养模型,模拟乳腺肿瘤微环境,研究IL-4激活的巨噬细胞对乳腺癌细胞的作用。
从正常成人外周血中用密度梯度离心的方法分离单个核细胞,在培养板中,去除非贴壁的淋巴细胞,加入含10%胎牛血清的DMEM培养基,37度培养箱培养3d,更换培养基后,加入IL-4(45ng/ml)诱导IL-4激活的巨噬细胞(Aa)、LPS(25ug/ml)诱导经典激活巨噬细胞(Ca)、不加任何激活因子培养没有激活的巨噬细胞(Ua),继续培养4天,48小时加一次诱导激活因子。约7天不同激活状态的巨噬细胞培养诱导完成。
图2是用不同方法检测不同激活状态的巨噬细胞CCL18的表达;其中图2A是用流式细胞技术检测不同激活状态巨噬细胞和乳腺癌细胞CCL18的表达;该图的结果表明,IL-4激活的巨噬细胞高表达CCL18,MDA-MB-231、SK-3rd不表达CCL18。图2B是用ELISA的方法检测细胞培养上清中CCL18的含量,结果表明IL-4激活的巨噬细胞培养上清中CCL18的含量最高,MDA-MB-231、SK-3rd的培养上清中检测不到CCL18。说明IL-4激活的巨噬细胞高表达CCL18。
进一步,进行了侵袭实验,不同激活状态巨噬细胞诱导完成后,胰酶消化收集不同激活状态的巨噬细胞,以1×106/ml的密度加入transwell培养板的下层,在transwell板的上层小室膜的下表面包被纤粘连蛋白(40ug/ml),在上层小室内铺基质胶,在37度培养箱放置30分钟,用无血清培养基(DMEM-F12+0.4%BSA、B272%、胰岛素1.42IU/ml)悬浮乳腺癌MAD-MB-231细胞或SK-3rd细胞,以1×106/ml的密度加入transwell的上层小室内,在transwell培养板的下层加入CCL18的中和性抗体(5μg/ml、10μg/ml),同时设置对照,37度5%二氧化碳培养箱培养12小时,用4%多聚甲醛固定穿过基质胶、黏附在transwell培养板上层小室膜的下表面的细胞15分钟、结晶紫染色15分钟,在显微镜下观察计数十个随机视野的细胞数,并进行统计学分析。结果表明,选择性激活的巨噬细胞促进乳腺癌细胞浸润,CCL18的中和性抗体抑制巨噬细胞的促浸润功能。具体结果见图4,图4是CCL18的中和性抗体明显抑制选择性激活的巨噬细胞的促MDA-MB-231乳腺癌细胞和促进SK-3rd细胞浸润功能的实验结果图;其中,图4A是不同激活状态的巨噬细胞中,应用CCL18的中和性抗体抑制IL-4激活巨噬细胞促进MDA-MB-231细胞浸润功能的观察图;图4A表明,不同激活状态的巨噬细胞中,IL-4激活的巨噬细胞促进乳腺癌细胞浸润的功能最强,应用CCL8的中和性抗体,可以抑制IL-4激活的巨噬细胞促进MDA-MB-231细胞的浸润功能。图4B是应用CCL18的中和性抗体抑制IL-4激活的巨噬细胞促进SK-3rd细胞浸润的功能的观察图;该图表明,IL-4激活的巨噬细胞促进乳腺癌细胞浸润的功能最强,应用CCL18的中和性抗体,可以抑制IL-4激活的巨噬细胞促进SK-3rd细胞浸润的功能。图4C是CCL18的中和性抗体抑制IL-4激活的巨噬细胞的促MDA-MB-231乳腺癌细胞的浸润功能的统计图;该图显示,CCL18的中和性抗体明显抑制IL-4激活的巨噬细胞的促MDA-MB-231乳腺癌细胞的浸润功能。
图2C是用western blot检测靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达。用靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达,抑制IL-4激活的巨噬细胞对乳腺癌细胞的促浸润作用。与之相关的其它实验图见图5,其中,图5A是用靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达,抑制IL-4激活的巨噬细胞对乳腺癌MAD-MB-231细胞的促浸润作用;该图表明,用靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达,可以抑制IL-4激活的巨噬细胞对乳腺癌MAD-MB-231细胞的促浸润作用。图5B是用靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达,抑制选择性激活的巨噬细胞对乳腺癌SK-3rd细胞的促浸润作用;该图表明用靶向CCL18的siRNA沉默IL-4激活的巨噬细胞中CCL18的表达,可以抑制IL-4激活的巨噬细胞对乳腺癌SK-3rd细胞的促浸润作用。
进一步研究CCL18促乳腺癌细胞浸润的功能,在没有巨噬细胞参与的情况下作侵袭实验,在培养体系中加入不同浓度的CCL18(0ug/ml、0.5ug/ml、1.0ug/ml、2.0ug/ml),实验结果见图6,图6是CCL18随浓度的增高促进乳腺癌细胞浸润的功能增强;图6A是CCL18随浓度的增高促进MDA-MB-231细胞浸润的功能逐渐增强的实验图;图6B是CCL18随浓度的增高促进SK-3rd细胞浸润的功能逐渐增强的实验图;由图6可知,CCL18随其浓度增高,促乳腺癌浸润的功能增强。
实施例4:肿瘤相关巨噬细胞(TAM)促进乳腺癌细胞浸润的相关实验
在乳腺癌组织中分离肿瘤相关巨噬细胞(TAM),把肿瘤相关巨噬细胞与原代的乳腺癌细胞或乳腺癌细胞系进行共培养,证明TAM促进乳腺癌细胞浸润。
具体实验步骤为,取手术切除的乳腺癌组织,用冰冷的PBS洗涤,在冰上把组织切为1mm3的小块,用含2%胎牛血清的冰冷的PBS洗涤组织块2次,用滤网过滤的洗涤液1000rpm 5min离心,弃上清,用含2%胎牛血清的冰冷的PBS重悬细胞,制细胞悬液(106/ml),把密度为1.055的ficoll铺于1.077ficoll之上,最上层加细胞悬液,1800rpm离心15min,1.055界面层去除肿瘤细胞,取1.077界面层细胞,洗涤后,用含20%FBS的DMEM重悬细胞,用差数贴壁的方法去除成纤维细胞,然后37度5%二氧化碳培养24小时,去除未贴壁的淋巴细胞,获得肿瘤相关巨噬细胞(TAM),用CD68标记TAM,流式检测,纯度达80%。上述洗涤后的乳腺癌组织中加入胶原酶III,37度、100rpm水浴摇床消化3-12小时,洗涤滤网过滤离心后制细胞悬液,用1.055、1.077的ficoll进行密度梯度离心,取1.055界面层细胞,获得原代乳腺癌细胞,把原代乳腺癌细胞和TAM共培养,以正常成人外周血单核细胞和乳腺癌细胞系MDA-MB-231细胞为对照,共培养12小时,原代乳腺癌细胞与原代TAM共培养60小时。结果证明TAM促进乳腺癌细胞浸润。结果见图7,图7是TAM促进原代乳腺癌细胞和MDA-MB-231细胞的浸润功能的实验图,其中,图7A是TAM促进原代乳腺癌细胞和MDA-MB-231细胞的浸润功能的实验图;图7B是TAM与正常成人外周血单核细胞比较促进原代乳腺癌细胞和MDA-MB-231细胞的浸润功能的统计图,可见TAM与正常成人外周血单核细胞比促进原代乳腺癌细胞和MDA-MB-231细胞的浸润功能有统计学显著性差异。
实施例5:利用transwell研究巨噬细胞对乳腺癌细胞迁移功能影响的实验
利用transwell研究巨噬细胞对乳腺癌细胞迁移功能的影响,按照上述方法在24孔培养板的中培养诱导巨噬细胞,同时在另外一个24孔培养板MDA-MB-231细胞和SK-3rd乳腺癌细胞,在培养乳腺癌细胞的培养板上划痕,然后消化、洗涤、计数巨噬细胞,把106/ml的巨噬细胞加入孔径为0.4um的transwell培养板的上层,在无血清的培养体系中把两种细胞共培养,在培养体系中加入CCL18中和性抗体(5ug/ml,10ug/ml),同时设置对照,观察共培养12小时,光镜下拍摄图片,图片见图8,图8是IL-4激活的巨噬细胞促进乳腺癌细胞迁移,CCL18的中和性抗体抑制巨噬细胞促迁移功能的实验图,其中,图8A是IL-4激活的巨噬细胞促进乳腺癌MDA-MB-231细胞迁移,CCL18的中和性抗体抑制巨噬细胞的促迁移功能;图8B是IL-4激活的巨噬细胞促进乳腺癌SK-3rd细胞迁移,CCL18的中和性抗体抑制巨噬细胞的促迁移功能。结果证明IL-4激活的巨噬细胞分泌CCL18促进乳腺癌细胞迁移,CCL18的中和性抗体可以抑制其迁移功能。
按照上述方法,在乳腺癌细胞的无血清的培养体系中,在没有巨噬细胞存在的情况下,加入不同浓度的CCL18,培养12小时进一步观察CCL18对乳腺癌细胞迁移功能的影响,见图9,图9是CCL18促进乳腺癌细胞迁移的观察图,其中,图9A是CCL18促进乳腺癌MDA-MB-231细胞迁移的观察图;图9B是CCL18促进乳腺癌SK-3rd细胞迁移的观察图。结果证明随CCL18浓度的增加促进乳腺癌细胞迁移的功能逐渐增强。
发明人在两个乳腺癌细胞系得到了相似的结果,充分说明结论的可信性。
最后所应当说明的是,以上实施例仅用以说明本发明的技术方案而非对本发明保护范围的限制,尽管参照较佳实施例对本发明作了详细说明,本领域的普通技术人员应当理解,可以对本发明的技术方案进行修改或者等同替换,而不脱离本发明技术方案的实质和范围。

1.靶向CCL18抑制乳腺癌细胞浸润和转移的方法,其特征在于,所述方法为靶向抑制肿瘤相关巨噬细胞分泌的CCL18。
2.根据权利要求1所述的靶向CCL18抑制乳腺癌细胞浸润和转移的方法,其特征在于,所述靶向抑制肿瘤相关巨噬细胞分泌的CCL18包括如下方法:利用CCL8的中和性抗体或沉默CCL18表达的siRNA抑制肿瘤相关巨噬细胞分泌的CCL18的作用,从而达到抑制乳腺癌细胞浸润和转移的目的。
3.根据权利要求2所述的靶向CCL18抑制乳腺癌细胞浸润和转移的方法,其特征在于,所述利用CCL8的中和性抗体抑制肿瘤相关巨噬细胞来源的CCL18的作用,为将所述CCL18的中和性抗体用于肿瘤相关巨噬细胞与乳腺癌细胞的共培养体系中,抑制CCL18的作用。
4.根据权利要求2所述的靶向CCL18抑制乳腺癌细胞浸润和转移的方法,其特征在于,所述利用沉默CCL18表达的siRNA抑制肿瘤相关巨噬细胞的CCL18表达,为将所述siRNA转染入肿瘤相关巨噬细胞,沉默CCL18的表达。
5.一种抑制乳腺癌细胞的浸润和转移的药物,其特征在于,所述药物含有抑制肿瘤相关巨噬细胞分泌的CCL18的成分。
6.根据权利要求5所述的抑制乳腺癌细胞的浸润和转移的药物,其特征在于,所述抑制肿瘤相关巨噬细胞分泌的CCL18的成分为靶向CCL18的中和性抗体。
