Abstract
本发明提供一种形成囊状动脉瘤动物模型的负压诱发装置,其特征在于,该装置包括模具,用于与动脉相接合;负压装置,用于给模具提供负压,以增大所述动脉的跨壁压差;负压调节装置,用于调节负压装置的压力。该形成囊状动脉瘤动物模型的负压诱发装置,通过外科手术的方法可以将模具固定于动物动脉上,利用负压调节装置调节连接在模具上的负压装置内的压力,以增大跨壁压差使动脉局部发生形变,从而建立囊状动脉瘤动物模型,为临床研究提供条件。
Description
技术领域
本申请涉及一种医学实验学装置,具体地说,本发明涉及到用力学方法建立囊状动脉瘤动物模型的负压诱发装置。
背景技术
动脉瘤,特别是颅内动脉瘤对人类健康危害严重,其致残致死率很高。据文献报道,成人动脉瘤的尸检阳性率达2.0%~7.6%。普通人群每年因动脉瘤破裂而致的蛛网膜下腔出血(SAH)的发生率约为10/10万,随着年龄的增加这个比例还会增高。动脉瘤破裂后病人的愈后往往不佳,据统计患SAH后3个月病死率达36%,严重致残率达18%。有些动脉瘤即使没有破裂出血,其占位效应也可能引起严重的后果。随着现代影像检查手段的广泛应用,动脉瘤的在体检出率也大大增加,临床统计表明,被检出的动脉瘤患者中,有近50%的病例发生破裂,而另外50%病例则不会破裂,也就是说,对于一个检出的动脉瘤,其转归方向为生长、破裂、稳定和血栓。对于颅内动脉瘤,如果能判定其属于易破裂的病例范畴,就应该实施手术治疗,尽管颅脑手术的风险很大,但是为了避免动脉瘤破裂导致的危害发生,医生和患者对于承担这样的手术风险都是能够认可的,但是,如果不能判定该动脉瘤的破裂可能性,医生和患者都会因为颅脑手术的高风险性和动脉瘤破裂的高危害性并存而无法作出合适的选择,特别是一些患者会因为发现自己有颅内动脉瘤又无法决定如何处理而形成严重的精神压力和心理恐惧,导致生活质量大大下降。因此,如何判断动脉瘤的转归方向,就成为患者和医生严重关切的沉重话题。
研究者对于动脉瘤的病理学过程、形态学结构、发生位置以及影响其发生发展的可能因素等进行许多有益的研究。但是,要从根本上消除动脉瘤的致命危害,必须解决以下一些关键问题:1)了解动脉瘤形成的病因和动脉瘤发展的病理过程,进而从根本上治疗动脉瘤;2)研究动脉瘤破裂的风险因素,对其破裂风险进行预测,对高危险性动脉瘤提早采取有效治疗措施;3)更加有效的动脉瘤治疗方法研究。由于不能在人体上进行任何实验,所有这些问题的研究均依赖于所建立的动脉瘤动物模型,因此近年来人们试图通过对动脉瘤动物模型的研究,揭示动脉瘤的病因发病机理,为临床研究提供条件。而如何设计和制作与人体动脉瘤生理特性相一致的动脉瘤动物模型是开展此类研究的基础、难点和关键。
动脉瘤动物模型的制作方法有病因诱导法、静脉移植法、动脉结构部分破坏法、弹性蛋白酶诱导法、改变血液动力学诱发动脉瘤等多种方法,其基本原则都是使诱导出的动脉瘤尽可能地与人自然形成的动脉瘤相似,从而增加研究结果的可信度和可推广性。
但以上方法均因存在一定的局限,所以每种模型均很难被广泛采用。
静脉移植法的局限性在于:在组织结构上静脉移植法模型的瘤壁平均厚度为290μm,具有完整的内弹力层和中层,与真实动脉瘤有很大差异。此外,瘤颈处的缝合带和动脉内膜的断裂不仅影响了动脉瘤的形态学结构,而且可以导致血小板生长因子的释放,从而导致瘢痕形成和动脉瘤闭塞。
采用病因诱发法制作的动脉瘤的生长位置和发生频率有很大的不确定性,诱发动脉瘤的时间较长;而且显著改变了动物的生理情况,致死率很高,至今尚未能在大型非灵长类动物中广泛建立,无法将其应用于动脉瘤血流动力学和栓塞材料的研究。
对于动脉结构部分破坏技术,主要的缺陷在于动脉瘤从诱发到破裂的时间非常短,酶的破坏作用过于严重和迅速。动脉瘤的生成、增大到破裂或闭塞过程均被极度的缩短。此外,所诱发的动脉瘤大小、型态具有偶然性,不能随意控制。
两种结合弹力酶的动脉瘤诱发技术,由于本身成本昂贵,目前很难推广。
由于目前存在的各种动脉瘤动物模型的制作技术均存在很难克服的缺陷,依据理想的动脉瘤动物模型应具备的特点:(1)动脉瘤的形态学结构和病理状态与人的动脉瘤相同或相似;(2)动脉瘤形态结构稳定,可重复性好;形成周期短,制作较快捷;(3)具有确定的瘤内空间,易于进行实验观察和操作;(4)制作方法较简单,费用较低。
此外,越来越多的事实证明血流动力学参数改变、高血压和动脉硬化是颅内动脉瘤发生和发展的重要后天因素,也有研究表明力学机制在动脉瘤的形成、发展和破裂过程中有重要作用。
动脉瘤动物模型的制作虽然经过不断研究和改进,所形成的动脉瘤已越来越接近人自然形成的动脉瘤,但是目前建立的动脉瘤动物模型还没有一种能够完全模拟人动脉瘤的生物学特性,距离真正意义的动脉瘤还有一定的距离,有待于进一步研究和完善。
发明内容
本发明所要解决的技术问题是根据动脉瘤产生的血流动力学原理,提供一种采用力学方法形成囊状动脉瘤动物模型的装置,以建立形态学结构和病理状态与人的动脉瘤相同或相似,稳定,可重复性好的囊状动脉瘤动物模型。
本发明的目的是为了研究建立囊状动脉瘤动物模型的力学方法的实验方面的问题,而提供一种形成囊状动脉瘤动物模型的负压诱发装置,其特征在于,该装置包括模具,用于与动脉相接合;负压装置,用于给模具提供负压,以增大所述动脉的跨壁压差;负压调节装置,用于调节负压装置的压力。该形成囊状动脉瘤动物模型的负压诱发装置,通过外科手术的方法可以将模具固定于动物动脉上,利用负压调节装置调节连接在模具上的负压装置内的压力,以增大跨壁压差使动脉局部发生形变,从而建立囊状动脉瘤动物模型,为临床研究提供条件。

具体实施方式
下面结合附图与实施例进一步说明本发明。
参见附图1,囊状动脉瘤动物模型的负压诱发装置包括模具1,负压装置2和负压调节装置3,其中模具1用于与动物的动脉4相接合,负压装置2用于给模具1提供负压;负压调节装置3用于调节负压装置2的压力。
参见附图2,本发明的模具包括:瘤体11、瘤颈12、血管套13、与负压装置连接部分14,为了进一步说明该模具,下面将以一实施例进行举例说明,所述的血管套为一横管,直接与动脉接触,其横截面为3/4圆,外径为2mm,内径为2mm,端口呈钝状,以避免将动脉管壁划破;所述的瘤体呈圆球状,外径8mm,内径为7mm;所述的瘤颈颈口为圆型,直径为2mm;所述的瘤体与负压装置连接部分为一竖管,其外径为3mm,内径为2mm。如此设计,可以利用负压装置,产生持续的负压,人为地建立囊状动脉瘤形成的力学环境,从而诱发囊状动脉瘤。当然,对于本领域技术人员来说,以上仅是举例说明,而并非局限于此,本发明中模具的血管套、瘤体、瘤颈口和与负压装置连接部分的形状、尺寸可以根据实际需要而进行相应的选择和改变,如可以是卵形、椭圆形或其他任意规则或不规则的形状。
本发明模具的材料的可以是具有生物适应性的材料,如合成高分子材料(聚氨醋、聚醋、聚乳酸、聚乙醇酸、乳酸乙醇酸共聚物及其他医用合成塑料和橡胶等)、天然高分子材料(如胶原、丝蛋白、纤维素、壳聚糖等)、金属与合金材料(如钦金属及其合金等)、无机材料(生物活性陶瓷,羟基磷灰石等)、复合材料(碳纤维/聚合物、玻璃纤维/聚合物等)等,也可以是经过表面生物相容性处理的非生物相容性材料,例如,利用特氟隆等作表面生物相容性处理,以减少或避免对材料的不适应性。
本发明的负压调节装置可以是手动的,也可以是电动的。
本发明模具1,负压装置2和负压调节装置3之间的连接方式可以是已有的任意连接方式,如螺纹连接、焊接、粘接或绳结等方式。
本发明与现有方法相比,具有如下优点:
1、成瘤时间短,在体实验五天以上,就可用肉眼观察到动脉发生异常扩张并成瘤。
2、不但保持了血管内膜完整性,而且保留了动脉壁的结构,弥补了静脉移植方法的缺陷。
3、此种方法建立的动脉瘤动物模型的形态及数量均可以控制,是病因诱导法和弹性蛋白酶破坏法不能够达到的。
4、所诱发的囊状动脉瘤瘤体腔内有初步成形的血栓,并且有少量炎性细胞浸润,与人类动脉瘤有相似之处有血栓形成,而几乎所有小动物动脉瘤模型不能模拟人体所具备的这一重要病理改变。
Claims (7)
1、一种形成囊状动脉瘤动物模型的负压诱发装置,其特征在于,
该装置包括模具,用于与动脉相接合;
负压装置,用于给模具提供负压,以增大所述动脉的跨壁压差;
负压调节装置,用于调节负压装置的压力。
2、根据权利要求1所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的模具可以选用具有生物适应性的材料。
3、根据权利要求2所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的生物适应性的材料选自合成高分子材料、天然高分子材料、金属与合金材料、无机材料或复合材料中的一种或几种的组合。
4、根据权利要求1所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的模具可以选用经过表面生物相容性处理的非生物相容性材料制成。
5、根据权利要求1所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的模具包括瘤体、瘤颈、血管套和负压装置连接部分。
6、根据权利要求5所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的血管套为一横管,直接与动脉接触,其横截面为3/4圆,外径为2mm,内径为2mm,端口呈钝状,以避免将动脉管壁划破;所述的瘤体呈圆球状,外径8mm,内径为7mm;所述的瘤颈颈口为圆型,直径为2mm;所述的负压装置连接部分为一竖管,其外径为3mm,内径为2mm。
7、根据权利要求1所述的形成囊状动脉瘤动物模型的负压诱发装置,其中所述的负压调节装置可以是手动方式或电动方式。
