本发明涉及一种治疗腰部疼痛的内服药,尤其涉及以纯中药为原料制成的一种治疗强直性脊柱炎的药物。
背景技术
强直性脊柱炎是一种慢性、进行性和炎性疾病。病变部位主要在骶骼关节、脊柱、脊柱旁软组织及四肢关节。病理改变为椎间盘纤维环和纤维环附近结缔组织的骨化、椎间动关节和四肢关节滑膜的炎症和增生。晚期各关节发生骨性融合,韧带骨化,脊柱呈强直状态。
幼年发病的强直性脊柱炎还可出现皮肤、粘膜、眼、心脏、肺及神经系统等病变,还伴有一些非特异性的全身症状,如低、中度发热、多汗,乏力及消瘦等。
强直性脊柱炎属中医“骨痹”范畴,病因可能是基因因素及环境因素的综合作用引起。病机特征:气血两亏,阴阳俱虚。痰瘀互结,凝聚关节。“不荣则痛″,“不通则痛”
本病如能得到早期治疗是可以治愈的,但目前还没有能根治本病的特效药。西医治疗以祛风湿止痛为主,常用药物如吲哚美辛、阿司匹林等,仅能抗炎,解热,镇痛,对病情无控制作用。大剂量使用皮质激素类药物,如地塞米松,有诸多副作用,致残或带来生命危险。改变病程的药物如柳氮磺胺吡啶,有可能控制幼年强直性脊柱炎的进展,但起效慢,通常在服药2至3个月起效,本类药物对已经发生骨质破坏的中晚期病变有无逆转作用,目前尚表示怀疑。中药新药的主要问题是如何能早中期治愈,控制并改善晚期症状,及时止痛和抑制骨质增生,是十分重要的关键。
发明内容
本发明的目的是提供一种治疗强直性脊柱炎的药物,具有气血双补,阴阳兼顾,活络止痛之功用,口服起效快,疗程短,无毒副作用,能根治强直性脊柱炎、幼年强直性脊柱炎。尤其适宜骨质已受到破坏,腰部疼痛顽固的患者。
本发明的技术方案是以下述方式实现的:
一种治疗强直性脊柱炎的药物,它是一首中药方剂加工或提炼后制成的内服药,其组方是:人参15克,白术30克,茯苓15克,炙甘草15克,狗脊(去茸毛)60克,炒杜仲15克,续断30克,鹿茸(白鹿)6克,制首乌90克,白芍45克,当归30克,枸杞子30克,丹参75克,川牛膝30克,红花9克,制乳香10克,陈皮30克,木香6克,制香附9克,桑寄生75克,独活9克,威灵仙9克,菝葜60克,薏苡仁60克,防风9克,老鹳草60克,制川乌9克,乌梢蛇15克,全蝎15克,炮山甲9克,干地黄60克,黄柏30克,知母30克。
本组方33味中药按比例组成,重一千克。有毒药物川乌头含量千分之九,全蝎千分之十五,内服不会产生毒副作用。
按照本组方药与药间的比例分量,即相对剂量,加工或提炼后制成胶囊剂、片剂或丸剂,供口服。
本组方乃标本同治之剂。强直性脊柱炎病理过程是致病因子的“邪气”和人体抗病能力的“正气”相争的正邪消长过程。因而临床上治疗本病时,要“祛邪以通”,“扶正以荣”。故扶正祛邪为本病的治则;气血双补,阴阳兼顾,活络止痛为具体治法。方中人参、白术、茯苓、甘草、狗脊、杜仲、续断、鹿茸补气兼助阳。当归、何首乌、白芍、枸杞子补血兼顾阴。以上药物气血两补,阴阳兼顾,主肝肾亏虚,腰痛脊强,为治疗强直性脊柱炎的主要药物。丹参、红花、川牛膝、乳香通利血脉,促进血行,内之脏腑而化瘀滞,外之利关节而通脉络,功在腰背疼痛。陈皮、木香、香附通三焦,解六郁,理气健脾,燥湿化痰。“气行则血行,气滞则血凝。”气血通利,则疼痛自止。桑寄生、独活、威灵仙、菝葜、薏苡仁、防风、老鹳草能解除肌肉、经络、筋骨间的风寒湿。主肢体疼痛,关节不利,筋脉拘急,下肢痿弱,尤其擅长背强腰痛,功效卓著。川乌头、乌梢蛇、全蝎、穿山甲搜风通络,增强疗效。干地黄、知母、黄柏泻火以坚阴,滋阴以清热,泻中寓补,补中寓泻,和热性药物并用,寒热互制,疏通清透共施,解表里寒热互结之痹痛。全方合用,以达气血双补,阴阳兼顾,活络止痛之功效。
本发明的积极效果是:
纯中药,成本低,药材来源广。药性平和,无毒副作用。补而不滞,润而不腻,标本同治,扶正祛邪兼施。具有气血双补,阴阳兼顾,活络止痛之功效。口服起效快,疗程短,愈后不复发。可调节免疫功能,增强机体抵抗力。抗炎、镇痛。对内分泌产生影响并改善造血系统功能,促进血红蛋白和红细胞的生成,改善微循环。从而扶助正气,调整阴阳,补偏救弊。把风寒湿邪、痰阻、气滞、血瘀逐出体外,消除脊柱、腰、胸、颈部、四肢及微动关节的病变,疼痛、肿胀发僵或肌肉萎缩等症状。在治疗骨病的同时,又能调呼吸,快脾胃,进饮食,排病毒。对强直性脊柱炎患者可早中期治愈,控制并改善晚期症状,尤其适宜已发生骨质破坏,腰腿疼痛顽固的病人。解决了目前治疗本病没有特效药的问题,同时对风湿、类风湿性关节炎、腰腿疼痛等疾病疗效甚佳,也是治风之良药。
下面就本组方33味中药的药性及功能等作进一步说明。
人参 甘、微苦,平。归肺、脾、心、肾经。大补元气,补脾益肺,安神益智。注意:反藜芦。
白术 苦、甘,温。归脾、胃经。健脾益气,燥湿利水,止汗。
茯苓 甘、淡,平。归心、肺、脾、肾经。利水渗湿,健脾和中,宁心安神。
甘草 甘,平。归脾、胃、肺、心经。益气补中,润肺止咳,泻火解毒,缓急止痛,调和药性。宜蜜炙用。
狗脊 苦、甘,温。归肝、肾经。强腰膝,祛风湿,利关节。可用作治疗强直性脊柱炎的引经药。宜去茸毛,制熟用。
杜仲 甘,温。归肝、肾经。补肝肾,强筋骨,安胎。宜炒用。
续断 苦、辛,温。归肝、肾经。补肝肾,强筋骨,通关节,和血脉,止血安胎。
鹿茸 甘、咸,温。归肝、肾经。壮肾阳,益精血,强筋骨,托疮毒,宜用白鹿鹿茸。
何首乌 甘、涩,微温。归肝、肾经。补益精血,固肾乌须。宜制用。
当归 甘、辛、苦,温。归肝、心、脾经。补血、活血,调经止痛,润燥滑肠。
白芍 苦、酸,微寒。归肝、脾经。养血和营,缓急止痛,敛汗平肝。
枸杞子 甘,平。归肝、肾、肺经。滋肝、补肾、润肺。
丹参 苦,微寒。归心、肝经。活血祛瘀,调经止痛,养血安神。注意:孕妇慎用。
川牛膝 甘、微苦,平。归肝、肾经。活血祛瘀,利关节,强腰膝。注意:孕妇忌服。
红花 辛,微苦,温。归心、肝经。活血通经,祛瘀止痛。注意:孕妇忌服。
乳香 辛、苦,微温。归心、肝、脾经。活血行气,通经止痛,消肿生肌。宜炒去油用。注意:孕妇忌服。
陈皮 辛,苦,温。归脾,肺经。行气健脾,降逆止呕,燥湿化痰。
木香 辛,苦,温。归脾,胃,大肠,三焦、胆经。行气止痛,健脾消食。
香附 辛、甘、微苦,归肝、三焦经。行气解郁,调经止痛,安胎。宜制用。
桑寄生 苦、甘,平。归肝,肾经。补肝肾,强筋骨,祛风湿,益血安胎。
独活 辛,苦,温。归肝、肾,膀胱经。祛风湿,止痛,解表。
威灵仙 辛,咸,微苦,温。有小毒。归膀胱经。祛风利湿,通络止痛。宜酒制用。
菝葜 甘、酸,平。归肝,肾经。祛风利湿,解毒消痈。
薏苡仁 甘、淡,微寒。归脾、胃、肺经。利水渗湿,健脾益胃,除痹舒筋,排脓消肿。注意:孕妇慎用。
防风 辛、甘、温。归膀胱、肺、脾经。祛风发表,胜湿止痛、止痉。
老鹳草 苦、微辛,平。归肝、大肠经,祛风利湿,通经络,活血消肿。
川乌头 苦、热,有大毒。归心、脾、肾经,祛风散寒,除湿止痛、溃坚去腐。宜制用。内服:煎汤1.5-6克。入丸散1-2克。注意事项:孕妇忌服。
毒性毒理:含毒主要成份为乌头碱,乌头碱小鼠灌胃给药的LD50为1.8mg/kg。炮制过程中生物碱含量可损失81.3%,川乌头内服中毒剂量为:3-90克。
乌梢蛇 甘、平,归肝经。祛风通络,定惊止痉,宜去头、皮,酒炙。
全蝎 辛、咸,平。有毒。归肝经。祛风止痉,通络止痛,攻毒散结。宜炒焙去毒后研末用。内服:煎汤2-5克,入丸散0.5-1克。注意:孕妇忌服。毒性毒理:蝎毒能产生宫缩,并导致早期流产。河南、辽宁产东亚钳蝎,蝎毒腹腔注射小鼠LD50为2.4mg/kg,10.3mg/kg.全蝎内服中毒量为:30-60克。
穿山甲 咸,微寒。归肝胃经。活血散结,通经下乳。
干地黄 甘、苦,微寒。归心、肝、肾经。滋阴清热,凉血补血。
知母 苦,寒。归肺、胃、肾经。清热泻火,滋阴润燥,止渴除烦。宜去毛切片用。
黄柏 苦,寒。归肾,膀胱、大肠经。清热燥湿,泻火解毒。
下面就本发明一种治疗强直性脊柱炎的药物的功能、主治等作一说明。
【功能】气血双补,阴阳兼顾,活络止痛。
【主治】用于强直性脊柱炎引起的脊柱畸形、腰、胸、颈部,四肢微动关节的病变,疼痛、肿胀、骨质破坏、发僵、肌肉萎缩等。也可用于腰腿疼痛、风湿、类风湿性关节炎等疾病。
【药物组成】人参、白术、茯苓、甘草、狗脊、杜仲、续断、鹿茸、当归、制首乌、白芍、枸杞子等33味。
【方剂剂型】胶囊剂、片剂或丸剂。丸剂应为蜜丸。
【性状】本品为棕色、气香、味苦、微甘。
【作用类别】本品为骨科,腰腿疼痛及风湿类非处方药药品。
【用法用量】口服(原生药)每次3—8克,一日二至三次或遵医嘱,儿童酌减,二个月为一疗程。服用一疗程后可根据情况继续服用。
【注意事项】幼儿、孕妇、妇女月经过多及有出血倾向者忌服。发热患者慎用。服药期间忌久坐、酗酒及劳逸失度。患者病愈一年内禁止参加重体力劳动。
每服本品5克含川乌头0.045克,全蝎0.075克,不会产生毒副作用。本品应放在儿童接触不到的地方。
具体实施方式
本发明的具体实施方式是临床应用,叙述如下:
【病例一】闫某,男,25岁,十二岁时患强直性脊柱炎,顽固性发热,疼痛达四年之久,生命垂危之际,服用本人发明的“速效退热骨痹克”(专利号:ZL2003 101 101 533),体温降至正常,活动性病变控制,可下床活动。改用本发明散剂,每次服5克,日服三次,无不良反应,服用两个月康复,血沉和C一反应蛋白也恢复到正常水平。
【病例二】王某,男,32岁,五年前患强直性脊柱炎。患者体质虚弱,腰腿疼痛顽固,行走困难,盗汗自汗,时常感冒,久治无效,服用本发明散剂,每次服用5克,日服三次,两个月后痊愈。又坚持服一疗程,巩固疗效。本品无任何副作用,不会引起不育。
【病例三】肖某,女,28岁,六年前患风湿性关节炎,正值盛夏,腰腿疼痛,不能行走,服用其它药物无效,服用本发明蜜丸,每服8克,日服三次,服用一个月痊愈。本品为夏月治风之良药。不会引起闭经,无不良反应,愈后不复发。
【病例四】潘某,男,21岁,西北科技大学学生。河南上蔡齐海乡人,上体育课时腰扭伤,数月后腰部疼痛僵硬。北京市某医院诊断为强直性脊柱炎,住院治疗。又到河南遂平医治,去周口地区求医,均无明显疗效,还花去一万多元。
2007年11月18日观察,行走困难,鸭步明显,腰脊强直,面部浮肿,像“满月脸”。自诉髋关节破坏,疼痛难忍,行走无力,虚汗不止。当日停服以前药物,改用本发明散剂,日服三次,每次8克。感冒发烧时,酌情服用“速效退热骨痹克”。服药三日起效,日见好转。40多天后,潘某精神良好,行走正常,鸭步消失,自诉行走有力,虚汗已止,腰痛好转,只是活动量大时髋关节偶有疼痛。服药期间无发现副作用,每日药费不足一元。
潘某08年元月初回学校参加考试。元月25日打来电话说,身体已康复,仍服用本品治“未病”。
Claims (2)
1.一种治疗强直性脊柱炎的药物,其特征在于它是由人参15克,白术30克,茯苓15克,炙甘草15克,去茸毛的狗脊60克,炒杜仲15克,续断30克,白鹿的鹿茸6克,制首乌90克,白芍45克,当归30克,枸杞子30克,丹参75克,川牛膝30克,红花9克,制乳香10克,陈皮30克,木香6克,制香附9克,桑寄生75克,独活9克,威灵仙9克,菝葜60克,薏苡仁60克,防风9克,老鹳草60克,制川乌9克,乌梢蛇15克,全蝎15克,炮山甲9克,干地黄60克,黄柏30克,知母30克组成。
2.根据权利要求1所述的药物,其特征在于它是胶囊剂、片剂或丸剂。
