发明背景
唾液不是血清的被动"超滤液"(Rehak,N.N.等,2000 Clin Chem Lab Med38:335-343),而含有独特的酶、激素、抗体和其它分子组成。在过去10年中,将唾液用作诊断性液体已成功应用于诊断和预测处于各种疾病风险中的群体(Streckfus,CF.和Bigler,L.R.2002 Oral Dis 8:69-76)。已在唾液中鉴定到可用于监测龋齿、牙周炎、口腔癌、唾液腺疾病和全身性疾病如肝炎和HIV的诊断性生物标记(Lawrence,H.P.2002 J Can Dent Assoc 68:170-174.)。
可在细胞内和细胞外检测人体遗传改变(Sidransky,D.1997 Science278:1054-1058)。已在大多数体液如血液、尿液和脑脊液中鉴定到核酸,并已成功运用这些核酸作为疾病的诊断生物标记物(Anker,P.等,1999 CancerMetastasis Rev 18:65-73;Rieger-Christ,K.M.等,2003 Cancer 98:737-744;Wong,L.J.等,2003 Cancer Res 63:3866-3871)。近来的兴趣已发展为检测唾液中的核酸标记物。迄今为止,大多数唾液中的DNA或RNA是病毒或细菌来源(Stamey,F.R.等,2003J.Virol Methods 108:189-193;Mercer,D.K.等,2001 FEMSMicrobiol Lett 200:163-167)。有限数量的报道证明了口腔癌患者唾液中肿瘤细胞DNA的异质性(Liao P.H.等,2000 Oral Oncol 36:272-276;El-Naggar,A.K.等,2001 JMolDiagn 3:164-170)。我们尚未发现唾液中可检测的人mRNA的公开证据。
在美国,2004年预计将诊断超过130万新的癌症病例(Cancer facts andfigures 2004.Atlanta:美国癌症协会,2004)。该年癌症将导致约563,700例美国人死亡,每分钟杀死一名患者。虽然癌症治疗取得了一些进展,但这些数字在过去十年间仍稳步增长。而且,对于一些癌症如口腔癌来说,过去几十年来总的5年存活率并没有提高,仍然低至约30-50%(Epstein,J.B.等,2002 J Can DentAssoc 68:617-621;Mao,L.等,2004 Cancer Cell 5:311-316)。缺乏预后改善的关键因素在于,很大比例的癌症开始时是无症状损伤,没有诊断或治疗直到发展到晚期。癌症早期检测是降低该疾病导致的死亡的最有效途径。
癌细胞的遗传畸变导致基因表达模式改变,可在最终癌症表型显现之前的很长时间就鉴定到这种改变。与相同来源的正常组织相比,仅仅或优先在癌中出现的改变可用作分子生物标记物(Sidransky,D.2002 Nat Rev Cancer2:210-219,2002)。精确鉴定后,生物标记物可为癌症早期检测和癌症风险评估提供新的途径并构成其主要靶点。许多基于核酸的生物标记物已被证明是癌症检测的新的有效工具(Hollstein,M.等,1991 Science 253:49-53;Liu,T.等,2000Genes Chromosomes Cancer 27:17-25;Groden,J.等,1991 Cell 66:589-600)。然而,大多数这些标记物在后期侵袭性和转移性癌症的癌细胞株或活检样本中鉴定到。我们使用生物标记物在最早期癌症阶段检测癌症的能力仍然有限。而且,活检的侵入性使其不适合在高风险人群中筛选癌症。这就提示,急需开发可改善早期检测的新的诊断工具。在体液中鉴定能在癌症最早期或在癌变前期预测癌症进程的分子标记物将构成这种工具。
发明概述
本研究的目的是在取自正常对象的无细胞唾液中测定转录组分布型。使用高密度寡核苷酸微阵列进行整体转录组分布型分析。利用唾液转录组模式产生参比数据库用于唾液转录组诊断。
像其它体液一样,唾液已用于监测人体健康和疾病。本研究显示,无细胞唾液中存在可提供信息的人mRNA。唾液mRNA提供潜在的生物标记物,以鉴定口腔或全身疾病高风险的人群和患者。使用高密度寡核苷酸微阵列描绘唾液mRNA分布。结果表明,无细胞唾液中存在数以千计的人mRNA。定量PCR(Q-PCR)分析证实通过本微阵列研究鉴定的mRNA的存在。基于正常唾液中mRNA的分布得到参比数据库。在本发明的一个实施方式中,使用唾液转录组诊断(Salivary Transcriptome Diagnostics)(STD)来诊断疾病和进行正常健康监护。
在另一个实施方式中,开发了用于唾液转录组诊断的唾液RNA最佳保存的实用的、用户友好的室温方案。

图4:评价口腔鳞状细胞癌(OSCC)的唾液mRNA预测因子的分类和回归树(CART)模型。选作初始区分标准的IL-8(截止值=3.14E-18)从共包括64个样品的母组产生两个子组。通过SAT(截止值=1.13E-14)进一步区分正常-1组,而癌症-1组通过H3F3A(截止值=2.07E-16)区分。通过CART将本研究中涉及的64个样品分为最终癌症或正常组。对于OSCC分类,总灵敏度为90.6%(29/32,正常组中),特异性为90.6%(29/32,癌症组中)。

优选实施方式的详细描述
本发明涉及人体疾病状态的早期检测、诊断和预测。公开了疾病状态的标记物,它们是来自具有疾病状态的个体唾液的特定序列的表达的RNA分子或由这些RNA分子表达的多肽。这些标记物是疾病状态的指标,当相对于正常对象中的表达发生差异性表达时,这些标记物可诊断患者中疾病状态的存在。这种标记物提供了相对于本领域现有技术的显著优点。由于它们在唾液样品中检测,因而无需在取样之前怀疑个体具有疾病状态(例如肿瘤),此外,获取唾液样品比组织活检或取血对患者的侵入性和痛苦要小得多。因此,本文公开的检测方法适用于无症状个体的广泛筛选。
实施例1
使用微阵列技术的无细胞唾液中RNA的分布型分析
材料与方法
正常对象
按照UCLA伦理委员会(UCLA Institutional Review Board)批准的方法获得加州大学洛杉矶分校(UCLA)医学中心,耳鼻喉科、头颈外科十位正常供者的唾液样品。采用以下准入标准:年龄≥30岁;无恶性肿瘤史、免疫缺陷史、自身免疫病史、肝炎史、HIV感染史或吸烟史。研究群体由6名男性和4名女性组成,平均年龄42岁(范围是32-55岁)。
唾液收集与处理
按公开的方法(Navazesh,M.1993 Ann N Y Acad Sd 694:72-77),在上午9-10点收集唾液样品。唾液收集前至少一小时要求对象禁止饮食、饮水、吸烟或进行口腔卫生操作。将唾液样品在2,600×g4℃下离心15分钟。将唾液上清液与细胞相分离。然后将RNA酶(RNase)抑制剂(Superase-In,Ambion Inc.,Austin,TX,USA)和蛋白酶抑制剂(抑肽酶,Sigma,St.Louis,MO,USA)加入无细胞唾液上清液中。
从无细胞唾液分离RNA
用厂商的改良方法(QIAamp病毒RNA试剂盒,Qiagen,Valencia,CA,USA)从无细胞唾液上清液分离RNA。将唾液(560μL)与AVL缓冲液(2,240μL)充分混合,室温孵育10分钟。加入无水乙醇(2,240μL),6,000×g下离心1分钟使溶液通过二氧化硅柱。然后洗涤该柱两次,20,000×g下离心2分钟,用30μL无RNA酶的水在9,000×g下洗脱2分钟。根据厂商说明书,用无RNAase的DNAase(DNAase I-无DNA,Ambion Inc.,Austin,TX,USA)处理RNA各等份。通过以下三种管家基因转录物的RT-PCR检验分离的RNA的质量:甘油醛-3-磷酸脱氢酶(GAPDH)、肌动蛋白-β(ACTB)和核糖体蛋白S9(RPS9)。采用PRIMER3软件(genome.wi.mit.edu)设计并商业合成(Fisher Scientific,Tustin,CA,USA)如下引物:
GAPDH的引物:

5’TCTCATCAAGCGTCAGCAGTTC3’(SEQ ID NO:6)。
用 RNA定量试剂盒(Molecular Probes,Eugene,OR,USA)估计RNA的量。
靶cRNA的制备
根据公开的方法(Ohyama,H.等,2000 Biotechniques 29:530-536),对分离的RNA进行线性扩增。简要地说,用T7-寡-(dT)24作为引物进行逆转录以合成第一条cDNA链。用T7RNA聚合酶(Ambion Inc.,Austin,TX,USA)进行第一轮体外转录(IVT)。用BioArrayTM高产率RNA转录物标记系统(Enzo LifeSciences,Farmingdale,NY,USA)进行第二轮IVT,以使cRNA产物生物素化;用样品清理模块(Affymetrix,Santa Clara,CA,USA)纯化标记的cRNA。用分光光度法和凝胶电泳测定cRNA的数量和质量。用RT-PCR评价各分离和扩增步骤获得的小等份试样的质量。用2100生物分析仪(AgilentTechnologies,Palo Alto,CA,USA)通过毛细管电泳评价片段化cRNA(如Kelly,2002所述制备)的质量。
HG-U133A微阵列分析
Affymetrix人基因组U133A阵列含有22,215个人基因cDNA探针组,代表约19,000个基因(即每个基因可由一个以上的探针组代表),用该阵列进行基因表达分布型分析。用Microarray Suite(MAS)软件(Affymetrix)将阵列数据标准化并进行分析。获得各探针组的检测p值。将p<0.04的任何探针组指定为"出现",表示可靠地检测到匹配的基因转录物(Affymetrix,2001)。获得各阵列上出现的探针组总数,计算出现基因的出现百分数(P%)。用Gene Ontology MiningTool(netaffx.com)对所选出的基因(在所有十个阵列上出现,p<0.01)进行功能分类。
通过Q-PCR的定量基因表达分析
用iCyclerTM热循环仪(Bio-Rad,Hercules,CA,USA)进行Q-PCR。用MuLV逆转录酶(Applied Biosystems,Foster City,CA,USA)将2μL分离的唾液RNA等份试样(未扩增)逆转录为cDNA。用iQ SYBR Green Supermix(Bio-Rad,Hercules,CA,USA)将所得cDNA(3μL)进行PCR扩增。由Sigma-Genosys(Woodlands,TX,USA)合成引物,如下所述:白介素1-β(IL1B)是5’GTGCTGAATGTGGACTCAATCC3’(SEQ ID NO:7)和5’ACCCTAAGGCAGGCAGTTG3’(SEQ ID NO:8);stratifin(SFN)是5’CCTGCGAAGAGCGAAACCTG3’(SEQ ID NO:9)和5’TCAATACTGGACAGCACCCTCC3’(SEQ ID NO:10);遍在微管蛋白α(K-α-1)是5’AGCGTGCCTTTGTTCACTG3’(SEQ ID NO:11)和5’CACACCAACCTCCTCATAATCC3’(SEQ ID NO:12)。所有反应一式三份进行,条件是为具体PCR产物定制的反应条件。用LightCycler软件3.0(Bio-Rad,Hercules,CA,USA)从标准曲线外推获得具体模板的cDNA起始量。用标准曲线进行定量的详细方法已有描述(Ginzinger,D.2002 Exp Hematol 30:503-512)。
结果
RNA分离和扩增
从560μL无细胞唾液样品中平均获得60.5±13.1ng(n=10)总RNA。RT-PCR结果表明,所有10份唾液样品含有编码管家基因GAPDH、ACTB和RPS9的mRNA。这些基因的mRNA可在-80℃下保存6个月以上而没有明显降解(图1)。两轮T7 RNA线性扩增后,生物素化cRNA的平均产量为42.2±3.9μg,A260/280=2.067(表1)。

片段化之前cRNA的长度为200bp-4kb;片段化后集中于约100bp。杂交之前用毛细管电泳证实了cRNA探针的质量。用PCR/RT-PCR检测到原始样品和以下各扩增步骤的产物中都有ACTB mRNA:第一条cDNA、第一次体外转录(IVT)、第二条cDNA和第二次IVT(图2)。
唾液mRNA的微阵列分布型分析
用含有22,283种cDNA探针的HG U133A阵列获得10位正常对象的唾液mRNA分布型。在各阵列(n=10)上发现平均有3,143±665.0个探针组(p<0.04)被指定为出现访问。这些探针组代表约3,000种不同的mRNA。平均出现访问百分数是14.11±2.98%(n=10)。产生包括这10个阵列的数据的参比数据库。代表GAPDH、ACTB和RPS9的探针组在所有10个阵列上被指定为出现访问。共有代表185个基因的207个探针组在所有10个阵列上被指定为出现访问,检测p<0.01。根据这些基因在所参与的生物过程和分子功能中的已知作用对它们进行分类(表2)。185种基因的主要功能涉及细胞生长/维持(119种基因)、分子结合(118种基因)和细胞结构组成(95种基因)。将这185种基因称为"正常唾液核心转录组(NSCT)"。

Q-PCR确认与定量分析
通过定量从185种“正常唾液核心转录组”基因中选出的基因,用实时定量PCR(Q-PCR)验证唾液中人mRNA的存在。随机选择IL1B、SFN和K-α-1并在所有10个阵列上指定为出现访问,进行验证。Q-PCR结果显示,在所有10例原始、未扩增的无细胞唾液中都可检测到IL1B、SFN和K-α-1的mRNA。这些转录物(n=10)的相对量(以拷贝数表示)为:IL1B是8.68×103±4.15×103;SFN是1.29×105±1.08×105;K-α-1是4.71×106±8.37×105。Q-PCR测定的这些基因的相对RNA表达水平与微阵列测定的类似。
唾液符合便宜、非侵入性和为可获得体液的要求,可用作理想的诊断介质。非常需要唾液中的特异性和信息性生物标记物用于诊断疾病和监测人体健康(Bonassi,S.等,2001 Mutat Res 480-481:349-358;Streckfus,CF.等,2002 OralDis 8:69-76;Sidransky,D.2000 Nat Reviews 3:210-219)。了解唾液的成分对于使用该介质来鉴定用于疾病诊断的潜在生物标记物来说是非常必要的(Pusch,W.等,2003 Pharmacogenomics 4:463-476)。在本发明之前,一种观点认为信息分子在唾液中存在的量一般较低。但是,使用新的扩增技术和高灵敏度分析,这不再是一种限制(Xiang,CC.等,2003 Nucleic Acids Res 31:e53)。在本实施例中,从无细胞唾液上清液中成功分离了人RNA。证明唾液mRNA的质量足以用于RT-PCR、Q-PCR和微阵列实验。
唾液与其它体液间的显著区别在于,唾液天然含有微生物(Sakki,T.&Knuuttila,M.1996 Eur J Oral Sci 104:619-622)。此外,一些外来物质(例如,食物残渣)使得唾液组成更加复杂。因此,使用唾液液相/上清液而不是全部唾液作为检测生物标记物的介质较简便且更加精确。在本实施例中,优化团粒与唾液上清液的分离条件,以避免细胞元素的机械破裂影响无细胞液相中检测到的RNA(St.John,M.A.R.等,2004,in press)。这些结果表明,使用无细胞唾液进行转录组分析是可行的并且非常有效。虽然无细胞唾液上清液中存在人mRNA是一种新发现,但很早就在其它无细胞体液中检测核酸,然后用作疾病诊断(Sidransky,D.1997 Science 278:1054-1058)。例如,已在患者血清中检测特定癌基因、肿瘤抑制基因和小随体(microsatellite)改变(Anker,P.等,2003 Int JCancer 103:149-152)。而且,已从患有不同恶性肿瘤的患者血清中分离和扩增了肿瘤mRNA(Kopreski,M.S.等,1999 Clin Cancer Res 5:1961-1965;Fleischhacker,M等,2001 Ann NY Acad Sd 945:179-188)。普遍认为这些细胞外检测的基因组信使可用作疾病的生物标记物(Sidransky,D.1997 Science278:1054-1058)。
就我们所知,这是关于唾液中人mRNA整体分布型分析的第一次报道。使用微阵列技术,我们发现每位正常对象的无细胞唾液中存在约3,000种不同的人mRNA。描绘了正常人群无细胞唾液中的唾液转录组模式,用作健康监测数据库。应注意,我们现在知道人基因组由30,000个以上的基因组成(Venter J.C.等,2001 Science 291:1304-1351),本实施例中使用的HG U133A微阵列上的探针组仅代表约19,000种人基因,不能用HG U133A微阵列检测的其它基因转录物预期存在于无细胞唾液中,并可使用本发明检测。本实施例中鉴定的基因转录物,尤其是正常唾液核心转录组(NSCT)mRNA,代表正常无系统唾液中的共有转录组。我们认为,可鉴定具有各种疾病状态的患者唾液中不同的信息和诊断转录组。因此,认为人唾液mRNA可用作口腔和表现在口腔中的全身疾病的诊断性生物标记物。
在本发明的一个实施方式中,使用唾液转录组诊断来监测正常患者的健康。在另一个实施方式中,使用唾液转录组诊断来检测疾病标记物,用于癌症(例如,前列腺癌、结肠癌、乳腺癌、肺癌、口腔癌等),以及全身疾病如自身免疫疾病、糖尿病、骨质疏松症;神经疾病如阿尔茨海默病、帕金森病等的早期诊断。
实施例2
用于口腔癌检测的唾液转录组诊断
目的:口腔液体(唾液)符合作为非侵入性、可获得和高度有效诊断基质的要求。我们发现唾液中能可靠地检测到大量人RNA,提示一种新的临床方法,唾液转录组诊断(Salivary Transcriptome Diagnostics)。在本实施例中,我们使用口腔鳞状细胞癌(OSCC)作为原理证明性(proof-of-principle)疾病来评价该新方法的诊断价值。
已知在受患对象的体液中可鉴定原发性肿瘤中存在的身份(identical)突变(Sidransky,D.1997 Science 278:1054-1059)。血液、尿液和脑脊液(CSF)中的癌症相关核酸已用作癌症诊断的生物标记物(Anker,P.等,1999 Cancer MetastasisRev 18:65-73;Rieger-Christ,K.M.等,2003 Cancer 98:737-744;Wong,LJ.等,2003 Cancer Res 63:3866-3871)。近来,癌症患者血清或血浆中的mRNA生物标记物已成为基于RT-PCR的检测方案的靶点(Kopreski,M.S.等,2001 Ann N YAcad Sd 945:172-178;Bunn,PJ.,Jr.2003 J Clin Oncol 21:3891-3893)。随着体液中这类生物标记物数量的增加,出现越来越多利用能大规模筛选基因改变的更强大和更具成本有效性的方法的技术。我们通过微阵列技术发现唾液中存在大量人mRNA(实施例1),提供用于疾病诊断和正常健康监测的新型临床方法,即唾液转录组诊断。这是确定唾液中RNA标签的高通量强效可重复方法。而且,使用唾液作为诊断液体符合对便宜、非侵入性和可获得的诊断方法的要求(Lawrence,H.P.2002 J Can Dent Assoc 68:170-174,2002)。在本实施例中,我们测试了以下假设:癌症患者唾液中可鉴定到独特的mRNA表达模式,差异表达的转录物可用作癌症检测的生物标记物。本研究中的原理证明性疾病是口腔鳞状细胞癌(OSCC)。原因在于,口腔癌细胞浸渍在唾液环境中并且已在OSCC患者唾液中检测到遗传异质性(El-Naggar,A.K等,200 1J Mol Diagn 3:164-170;Liao,P.H等,2000 Oral Oncol 36:272-276,2000)。
实验设计:从原发性T1/T2 OSCC患者和年龄、性别及吸烟史匹配的正常对象(n=32)收集未刺激的唾液。从唾液上清液分离RNA,然后使用T7RNA聚合酶进行两轮线性扩增。采用人基因组U133A微阵列进行人唾液转录组分布型分析。通过组合对10位匹配的癌症患者和对照的t配对检验(t test comparison)和倍数变化(fold-change)分析,分析不同的基因表达模式。使用定量PCR(qPCR)确认通过微阵列显示显著性差异(P<0.01)的选定基因。通过接受者操作特征曲线和分类模型分析这些唾液mRNA生物标记物的预测能力。
结果:微阵列分析显示,1,679种基因在癌症患者与对照之间存在显著不同的唾液表达水平(P<0.05)。七种癌症相关RNA生物标记物在OSCC唾液中升高至少3.5倍(P<0.01),与对OSCC患者(n=32)和对照(n=32)唾液样品的qPCR所确认的结果相一致。这些唾液RNA生物标记物是白介素8(IL-8)、白介素1-β(IL1B)、双特异性磷酸酶1(DUSP1)、H3组蛋白、家族3A(HA3A)、鸟氨酸脱羧酶抗酶蛋白1(OAZ1)、S100钙结合蛋白PS(100P)和亚精胺/精胺N1-乙酰转移酶(SAT)的转录物。这些生物标记物的组合产生OSCC与对照相区别的灵敏度(91%)和特异性(91%)。
结论:本研究成功地证明,唾液转录组诊断可用于口腔癌症检测。认为这种新的临床方法是强效、高通量和可重复的癌症早期检测工具。认为唾液转录组分布型分析可用于评价其它主要疾病以及正常健康监测。
患者与方法
患者选择:从加州洛杉矶市的加州大学洛杉矶分校(UCLA)医学中心、南加州大学(USC)医学中心以及加州旧金山市的加州大学旧金山分校(UCSF)医学中心征集口腔鳞状细胞癌(OSCC)患者。本研究包括32位患有原发性T1或T2OSCC的患者。所有患者近期诊断患有原发性疾病,且先前未接受诸如化疗、放疗、手术或替代药物等形式的任何治疗。选择相同数量的具有相当吸烟史的年龄和性别匹配的对象作为对照组(St.John,M.A.R等,2004 IL-6和IL-8:口腔和口咽SCCA潜在的生物标记物(IL-6and IL-8:Potential Biomarkers for OralCavity and Oropharyngeal SCCA).Archives of Otolaryngology-Head & NeckSurgery,in press)。两个对象组中,以下因素都没有显著差异:平均年龄:OSCC患者,49.8±7.6岁;正常对象,49.1±5.9岁(Student′s t检验P>0.80);性别(P>0.90);或吸烟史(P>0.75)。没人以前有恶性肿瘤史、免疫缺陷史、自身免疫病史、肝炎史或HIV感染史。所有对象都签署了伦理委员会批准的知情同意书,同意作为此项研究的唾液提供者。
唾液收集和RNA分离
用过去已建立的方法(Navazesh,M.1993Ann N Y Acad Sci 694:72-77)在上午9到10点之间收集未刺激的唾液样品。要求对象在收集前至少一小时禁止进食、饮水、吸烟或口腔卫生操作。将唾液样品在4℃2,600×g离心15分钟。将上清液与团粒分离并用RNase抑制剂(Superase-In,Ambion hie,Austin,TX)处理。用QIAamp病毒RNA试剂盒(Qiagen,Valencia,CA)从560μl唾液上清液分离RNA。根据厂商说明书,用无RNase的DNase(DNase I-无DNA,AmbionInc.,Austin,TX)处理分离的RNA各等份试样。通过对以下三种细胞维持基因转录物的RT-PCR检验分离的RNA的质量:甘油醛-3-磷酸脱氢酶(GAPDH),肌动蛋白-β(ACTB)和核糖体蛋白S9(RPS9)。只有显示所有三种基因PCR产物的样品用作后续试验。
微阵列分析 用来自10位OSCC患者(7男3女;年龄=52±9.0)以及性别和年龄匹配的10位正常对象(年龄=49±5.6)的唾液进行微阵列研究。用RiboAmpTM RNA扩增试剂盒(Arcturus,Mountain View,CA)线性扩增从唾液中分离的RNA。在五次独立实验中用已知量(0.1μg)的对照RNA与这20个样品平行电泳测定RNA扩增效率。根据实施例1所述方案,使用Affymetrix人基因组U133A阵列(HG U133A,Affymetrix,Santa,Clara,CA)进行基因表达分析。扫描阵列,用Microarray Suit 5.0软件(Affymetrix,Santa Ciara,CA)测定荧光强度,然后输入DNA芯片分析软件进行标准化和模式分析(Li,C.& Wong,W.H.2001 PNAS USA 98:31-36)。用S-plus 6.0(Insightful,Settle,WA)进行所有统计学检验。用三个标准来确定差异性表达的基因转录物。首先,排除阵列上在所有样品中认定为"无"访问的探针组。第二,用双尾student′s t检验比较OSCC(n=10)和对照(n=10)的基因表达的平均信号强度。将临界α水平0.05定义为有统计学显著性。第三,计算显示统计学显著性差异(P<0.05)的基因转录物的倍数比。只对至少改变2倍的那些基因转录物进行进一步分析。
定量PCR验证 进行qPCR,以验证微阵列分析鉴定的差异性表达转录物亚组。用MuLV逆转录酶(Applied Biosystems,Foster City,CA)和随机六聚体引物(ABI,Foster City,CA)从原始未扩增的唾液RNA合成cDNA。在iCyclerTMPCR系统中用iQ SYBR Green Supermix(Bio-Rad,Hercules,CA)进行qPCR反应。用PRIMER3软件设计引物组。用具体产物的专用条件一式三份进行所有反应。如前所述,从标准曲线外推得到具体模板的cDNA/RNA起始量(Ginzinger,D.G.2002 Exp Hematol 30:503-512)。通过测试包括之前用于微阵列研究的20个样品在内的所有样品(n=64)完成此验证。用Wilcoxon Signed-秩次检验进行统计学分析。
接受者操作特征(ROC)曲线分析和预测模型 利用RT-qPCR结果,用S plus6.0软件进行ROC曲线分析(Grunkemeier,G.L.& Jin,R.2001 Ann Thorac Surg72:323-326)以评价各个生物标记物的预测能力。通过检索能产生最大相应灵敏度和特异性的截止点来确定各生物标记的最优截止点。然后根据该组的最优灵敏度和特异性值制作ROC曲线。通过ROC曲线的数值积分(integration)计算曲线下面积。将ROC曲线下面积最大的生物标记物鉴定为检测OSCC的预测能力最强。
下一步,构建多变量分类模型以确定用于预测癌症的唾液标记物最佳组合。首先,用疾病(OSCC)和无疾病(正常)的二元结果作为独立变量,构建对数回归模型,控制患者的年龄、性别和吸烟史。用反向逐步回归(Renger,R.&Meadows,L.M.1994 Acad Med 69:738)找到最佳的最终模型。用舍去一个交叉确认来验证该对数回归模型。交叉确认方案首先去除一个观察结果,然后用所有标记拟合从剩余结果得到的对数回归模型。将逐步模型选择应用于各模型,以去除不能改善该模型的变量。然后,将标记值应用于该舍去的观察结果以计算出该观察结果的预期分类。交叉确认误差率是预计不正确的样品数除以样本总数。然后,使用该模型的拟合概率作为计算灵敏度和特异性的可能截止点,用相似方法以计算机制作该对数模型的ROC曲线。
第二,用验证的mRNA生物标记物作为预测因子,通过S-plus6.0构建树状分类模型、分类和回归树(CART)。CART通过二元递归划分来拟合该分类模型,其中各步骤包括检索能最佳地区分癌症组与正常组的预测变量(Lemon,S.C.等,2003 Ann Behav Med 26:172-181)。CART采用熵函数以及用S-plus的默认设置确定的区分标准。用此方法,将含有全部样品(n=64)的母组再分为癌症组和正常组。剪除初始树上不产生分类不同的亚分支的所有分支。
结果
平均来说,从560μl唾液上清液获得54.2±20.1ng(n=64)总RNA。OSCC和年龄性别匹配的对照之间的总RNA量没有显著性差异(t检验,P=0.29,n=64)。RT-PCR结果表明,所有唾液样品(n=64)都含有三种基因(GAPDH,ACTB和RPS9)的转录物,这三种基因的转录物用作人唾液RNA的质量对照(参见实施例1)。两轮RNA扩增后可获得一致的扩增幅度(658±47.2,n=5)。平均来说,生物素化cRNA的产量为39.3±6.0μg(n=20)。OSCC和对照之间cRNA的产量没有显著性差异(t检验,P=0.31,n=20)。
用HG U133A微阵列鉴定癌症患者与匹配的正常对象之间唾液RNA分布型的差异。在上述标准包括的10,316个转录物中,鉴定到1,679个转录物的P值小于0.05。在这些转录物中,OSCC组中有836个上调、843个下调。考虑到用P<0.05作为假阳性(的分界线),观察到的这些转录物不大可能仅仅归因于偶然发生(x2检验,P<0.0001)。采用预定标准即在所有10个OSCC唾液样品中调节改变在3倍以上、以及更严谨的截止P值<0.01,鉴定了17种转录物,如表3所示。这17种唾液mRNA在OSCC唾液中都上调,而用相同的滤过标准没有发现mRNA下调。表3显示了这些基因的生物学功能。

用人基因组U133A微阵列鉴定10位癌症患者和10位匹配正常对象唾液中RNA表达模式的差异。采用在所有OSCC唾液样本中调节改变在3倍以上和截止P值<0.01的标准,我们鉴定到17种在OSCC唾液中显著性上调的mRNA。
进行定量PCR以在包括32位OSCC患者和32位匹配对照者的唾液的扩大范围的样品上验证微阵列结果。根据报道的癌症相关性选择了9种唾液mRNA生物标记物候选物:DUSP1、GADD45B、H3F3A、IL1B、IL8、OAZ1、RGS2、S100P和SAT(表3)。表4提供了通过qPCR测定的OSCC患者唾液中上述9个候选物的定量变化。结果证实,OSCC患者唾液中,9种候选mRNA中的7种转录物(78%):DUSP1、H3F3A、IL1B、IL8、OAZ1、S100P和SAT显著升高(Wilcoxon Signed-秩次检验,P<0.05)。用qPCR没有检测到RGS2(P=0.149)和GADD45B(P=0.116)的量有统计学显著性差异。根据增加幅度可将经确认的7种基因分为三个等级:高度上调的mRNA包括IL8(24.3倍);中等上调的mRNA包括H3F3A(5.61倍)、IL1B(5.48倍)和S100P(4.88倍);和低度上调的mRNA包括DUSP1(2.60倍)、OAZ1(2.82倍)和SAT(2.98倍)。对OSCC的7种潜在唾液mRNA生物标记物各自进行接受者操作特征(ROC)曲线下面积、阈值以及相应灵敏度和特异性的详细统计学分析,见表5。数据显示,在7种潜在生物标记物中IL8mRNA预测OSCC存在的性能最好。对IL8计算的ROC曲线下面积是0.85。阈值为3.19E-18mol/L,唾液IL8mRNA区分OSCC与正常人的灵敏度为88%,特异性为81%。

进行定量PCR以在包括32位OSCC患者和32位匹配对照者的唾液的扩大范围的样品上验证微阵列结果。从表3所示的17种候选物中选出9种潜在的唾液mRNA生物标记物。通过qPCR验证出其中7种(P<0.05)。样品包括来自OSCC患者的32个唾液样品和来自匹配的正常对象的32个唾液样品。

利用该qPCR结果,我们进行ROC曲线分析以评价各生物标记的预测能力。确定能产生最大相应灵敏度和特异性的值为最优截止点。ROC曲线下面积最大的生物标记鉴定为检测OSCC的预测能力最强。
为证明唾液mRNA在区分疾病中的实用性,检测了两种分类/预测模型。根据7种已证实的生物标记中的4种:IL1B、OAZ1、SAT和IL8建立对数回归模型,它们组合起来提供了最佳预测(表6)。这4种标记的系数值为正,表明它们唾液浓度的同步增加提高了样品获自OSCC对象的概率。根据对数回归模型舍去一个交叉确认误差率是19%(12/64)。在舍去一个分析所产生的64种模型中,除了1种以外,都采用在表6列举的完全数据模型中具有显著性的同一组4种标记。用计算机绘制对数回归模型的ROC曲线。截止值概率50%,获得的灵敏度为91%,特异性为91%。该对数回归模型计算出的ROC曲线下面积是0.95(图3)。

根据7种已确认的生物标记中的四种(IL1B、OAZ1、SAT和IL8)建立对数回归模型,这四种生物标记组合起来提供了最佳预测。这4种标记的系数值为正,表明它们唾液浓度的同步增加提高了样品获自OSCC对象的概率。
产生了第二种模型,即“分离和回归树(CART)模型”(图4)。拟合的CART模型采用IL8、H3F3A和SAT的唾液mRNA浓度作为OSCC的预测变量。选作初始区分标记的IL8(阈值为3.14E-18mol/L)从共包括64个样品的母组产生两个子组。IL8浓度<3.14E-18mol/L的30个样品认定为“正常-1”组,而IL8浓度≥3.14E-18的34个样品认定为“癌症-1”。用SAT(阈值1.13E-14mol/L)进一步区分"正常-1"组。得到的亚组“正常-2”包括SAT浓度<1.13E-14mol/L的25个样品,“癌症-2”包括SAT浓度≥1.13E-14mol/L的5个样品。相似地,用H3F3A(阈值2.07E-16mol/L)进一步区分“癌症-1”组。得到的亚组“癌症-3”包括H3F3A浓度≥2.07E-16mol/L的27个样品,“正常-3”组包括H3F3A浓度<2.07E-16mol/L的7个样品。这样,用CART分析将该研究中涉及的64个唾液样品分为“癌症”组和“正常”组。“正常”组由“正常-2”组和“正常-3”组的样品组成。共有32个样品认定为“正常”组,其中29个来自正常人,3个来自癌症患者。因此,采用IL8、SAT和H3F3A组合进行OSCC预测时,总灵敏度是90.6%(29/32)。“癌症”组由“癌症-2”组和“癌症-3”组的样品组成。共有32个样品认定为最终的“癌症”组,其中29个来自癌症患者,3个来自正常人。因此,采用这三种唾液mRNA生物标记的组合进行OSCC预测时,总特异性为90.6%(29/32)。
癌症筛选方案的目标是在治疗可能成功的足够早期检测肿瘤。需要同时具有高灵敏度和高特异性的筛选工具。而且,筛选工具必须具有充分的非侵入性并且足够便宜以实现广泛应用。生物技术的显著发展以及我们对癌症发生和进程的基本理解使得能够在取自肿瘤受患器官的体液中检测肿瘤特征,例如癌基因和肿瘤抑制基因改变(Sidransky,D.1997 Science 278:1054-1059)。本实施例的结果表明,唾液转录组诊断是对开发癌症的非侵入性诊断、预后和随访试验合适的工具。
过去的研究已表明,人DNA生物标记物可在唾液中鉴定到并可用于口腔癌症检测(El-Naggar,A.K等,2001 J Mol Diagn 3:164-170;Liao,P.H.等,2000Oral Oncol 36:272-276)。唾液中人mRNA的存在扩展了翻译和临床应用的诊断分析物的组成。然而,RNA比DNA更不稳定,推测极易受RNase的降解。而且,据报道癌症患者的唾液中RNase活性升高(Kharchenko,S.V.和Shpakov,A.A.1989 Izv Akad Nauk SSSR Biol 58-63)。因此,普遍认为人mRNA不能在唾液中细胞外存在下去。意外地,采用RT-PCR,发明者们一致性地在唾液中检测到人mRNA,因而开启了基于唾液的表达分布型分析的方法。采用上述收集和处理方法,由RT-PCR/qPCR证实所有唾液(患者和对照)中存在对照RNA。RNA的质量符合PCR、qPCR和微阵列的要求。在本实施例中,我们利用迅速加入RNase抑制剂以新鲜收集口腔液体,然后超低温储存(-80℃)。
我们所报道的发现将为癌症和疾病诊断领域带来大量关注。关注不仅来源于以下事实:基于唾液的癌症诊断和筛选试验是简单且吸引人的概念,而且来源于以下事实:常规癌症诊断试验不完善。采用口腔癌症为例,明显令人失望的存活率最有可能是因为诊断延迟(Wildt,J.等,1995 Clin Otolaryngol20:21-25)。由于大多数口腔癌症在其早期为无症状小损伤形式,只有当临床或患者注意到异常组织时才开始正式诊断过程(Epstein,J.B.等,2002 J Can DentAssoc 68:617-621)。进行性癌症的微观水平对于成功干预来说常常太晚(Fong,K.M.等,1999 in:In:S.S.HD和G.AF(编),早期癌症的分子病理学(MolecularPathology of Early Cancer),第13-26页:IOS Press)。使用成像技术进行癌症筛选也不实用,因为其耗时且昂贵。该技术常常用于确认癌症,因为其对小损伤不敏感(Myers,LX.& Wax,M.K.1998 J Otolaryngol 27:342-347)。研究发现,通过用甲苯胺蓝染色口腔癌症组织可实现良好的阳性预测值(Mashberg,A.&Samit,A.1995 CA Cancer J Clin 45:328-351)。然而,应用该技术和解释结果需要大量经验。脱落细胞学是一种侵入性低的口腔癌症检测方法(Rosin,M.P.等,1997Cancer Res 57:5258-5260)。但是,脱落的癌细胞倾向于与肿瘤负荷相关,在最小或早期疾病的诊断中测出的概率较低。本研究中鉴定的唾液mRNA生物标记物提供了OSCC检测的新途径。唾液转录组诊断是具有充分预测能力的非侵入性诊断工具。
对于正常个体,唾液RNA来源可能来自以下三种来源之一:唾液腺(腮腺、颌下、舌下以及次级腺体)、龈沟液和口腔粘膜细胞(衬细胞或脱落细胞)。对于口腔癌症患者来说,检测到的癌症相关RNA特征可能来源于匹配的肿瘤和/或全身反应(局部或远端),这种全身性反应进一步反映在分别来自上述三种主要来源(唾液腺、龈沟液和口腔粘膜细胞)的全唾液中。可以想像到,疾病相关RNA可通过唾液腺或通过龈沟液循环进入口腔。一个很好的例子是乳腺癌患者唾液中HER-2蛋白的水平升高(Streckfus,C.等,2000 Clin Cancer Res 6:2363-2370)。对于口腔癌来说,局部肿瘤是唾液mRNA升高的来源。近来我们选择最显著升高的口腔组织转录物IL8,并证实其在口腔癌患者唾液中的蛋白水平(通过ELISA)也显著升高(St.John,M.A.R.等,2004 IL-6和IL-8:口腔和口咽SCCA潜在的生物标记物(IL-6 and IL-8:Potential Biomarkers for Oral Cavity andOropharyngeal SCCA)Archives of Otolaryngology-Head & Neck Surgery,出版中)。Chen等过去已独立地发现,头部和颈部癌组织中IL8蛋白表达升高(Chen,Z.等,1999 Clin Cancer Res 5:1369-1379)。这些数据联合支持以下事实:在mRNA和蛋白水平,肿瘤组织和唾液中口腔癌相关表达水平协调改变。
除了IL8,还鉴定到六种其它癌症相关基因如DUSP、H3F3A、OAZ1、SAT、S100P和IL-1B在口腔癌患者唾液中上调。DUSP1基因编码双特异性磷酸酶,已提示作为肿瘤抑制剂PTEN信号通路的介质(Unoki,M.& Nakamura,Y.2001Oncogene 20:4457-4465)。已证明DUSP1的表达在卵巢肿瘤中降低,并在DUSP1基因中鉴定了新的单核苷酸多态性(SNP)(Suzuki,C.等,2001J Hum Genet46:155-157)。H3F3A mRNA通常用作增殖标记物,已证明其水平在前列腺癌和结肠癌中上调(Bettuzzi,S.等,2000 Cancer Res 60:28-34;Torelli,G.等,1987Cancer Res 47:5266-5269)。基于OAZ1对鸟氨酸脱羧酶(ODC)已知的抑制作用,预测其是肿瘤抑制剂(Tsuji,T.等,2001 Oncogene 20:24-33)。然而,已报道OAZ1mRNA在前列腺癌中上调(Bettuzzi,S.等,2000 Cancer Res 60:28-34)。有趣的是,同样参与多胺代谢的SAT的表达在前列腺癌中显著较高(Bettuzzi,S等,2000 Cancer Res 60:28-34)。已知S100P与前列腺癌进程相关,体外和体内乳腺癌发展早期,S100P的过度表达与人乳房上皮细胞的无限增殖相关(Gribenko,A.等,1998 Protein Sci 7:211-215;Guerreiro Da Silva,I.D.等,2000 Int J Oncol16:231-240;Mousses,S.等,2002 Cancer Res 62:1256-1260;Mackay,A.等,2003 Oncogene 22:2680-2688)。近期研究发现S100P的差异性表达与胰腺癌相关(Logsdon,CD.等,2003 Cancer Res 63:2649-2657;Crnogorac-Jurcevic,T.等,2003 J Pathol 201:63-74)。IL-1B的表达也与癌症相关。在口腔鳞状细胞癌患者中IL-1B的血清水平较高(Jablonska,E.等,1997 Pathol Oncol Res 3:126-129)。并且,已报道在卵巢癌妇女的腹水中IL-IB的水平显著升高(Chen,CK.等,1999JFormos Med Assoc 98:24-30)。已报道IL-IB的遗传多态性与疾病如胃癌和乳腺癌的风险潜在相关(Hamajima,N.和Yuasa,H.2003 Nippon Koshu Eisei Zasshi50:194-207;El-Omar,E.M.等,2003 Gastroenterology 124:1193-1201)。
唾液正越来越多地用于研究全身疾病诊断,例如HIV(Malamud,D.1997Am J Med 102:9-14),糖尿病(Guven,Y.等,1996 J Clin Periodontal 23:879-881)和乳腺癌(Streckfus,C.等,2000 Clin Cancer Res 6:2363-2370)。最重要地,可容易地改进本发明实施例中使用的多种生物标记物的概念、技术和方法以筛选和监测其它疾病。对于口腔癌来说,唾液转录组诊断的最重要应用之一是检测口腔癌前病变的癌症转化。总的恶性转化率为11-70.3%(Lee,J.J.等,2000 ClinCancer Res 6:1702-1710;Silverman,S.,Jr.& Gorsky,M.1997 Oral Surg Oral MedOral Pathol Oral Radiol Endod 84:154-157)。已显示口腔白斑细胞中DNA含量分析可用于预测口腔癌的风险(Sudbo,J.等,2001 N Engl J Med 344:1270-1278)。然而,这仍然是活检后方法。我们预想“唾液转录组诊断”将成为口腔癌和其它人体疾病早期诊断的新方法。
实施例3
唾液RNA的实用性室温储存方案
开发了诊断应用的唾液RNA最佳储存的实用、用户友好的室温方案。本发明该实施方式提供了用于唾液转录组诊断的最高质量和数量的唾液RNA。
在RNALaterTM处理的唾液中进行人mRNA的检测和定量。将唾液与1或2体积的RNAlaterTM混合(泳道1或2)。用Qiagen试剂盒从140μL唾液上清液分离总的RNA。分离的RNA等份试样用DNAse I(Ambion)处理。采用RT-PCR来检测以下三种基因的转录物:β-肌动蛋白(ACTB)、甘油醛-3-磷酸脱氢酶(GAPDH)和白介素8(IL-8)(图5A)。使用Ribogreen试剂盒(Molecular Probes)定量RNA显示,经RNAlaterTM处理的样品比经Superase-In(Ambion)处理的样品具有更高的RNA产量(图5B)。用1体积RNAlaterTM(L1)或2体积RNAlaterTM(L2)比Superase-In(S)产生的RNA分别多约10倍和约3.3倍。在从相同个体不同时间点收集的样品和从5位不同个体相同时间点收集的样品中再现了上述数据。
进行定量PCR(qPCR)来定量唾液GAPDH和IL-8。将唾液样品分成用RNAlaterTM(1:1比例)或Superase-In处理的等份试样。未处理唾液(无)用作对照。将样品在室温下保存24小时,然而在4℃中储存。在连续的5天中从140μL唾液上清液分离总RNA。从第一天到第五天进行RT-qPCR,以定量GAPDH和IL-8编码的cDNA/RNA。图6所示数据表明,RNAlaterTM对唾液RNA完整性具有较好的保护作用。术语“RNAlaterTM”是Ambion,Inc.的商标(USP6,528,641和USP 6,204,375)。
使用HGU 133 plus 2.0阵列(Affymetrix)进行人唾液mRNA的分布型分析。表7中的数字代表MAS 5.0和Dchip 1.3指定存在的mRNA的数量。

数据表明,RNAlaterTM处理的样品比“Superase-In”处理的样品能收集更多的mRNA。
设想本发明实施方式可用于需要保存唾液RNA的任何情况(例如,儿科医生、家庭医生、牙医、其它健康护理提供机构、社区诊所、家庭保健药盒)。然后将保存的RNA运往诊断中心,用于如实施例1和2所述的基于RNA的特异性筛选或诊断。我们设想用于收集唾液的试剂盒,例如,如USP No:6,652,481;6,022,326;5,393,496;5,910,122;5,376,337;4,019,255和4,768,238所述,组合有RNAlaterTM型RNAse抑制组合物。

Claims (6)
1.一种鉴定人疾病状态标记物的方法,所述方法包括:
获得人唾液样品;
从所述人唾液样品获得人mRNA;
扩增所述mRNA以提供核酸扩增产物;
分离所述核酸扩增产物;以及
鉴定在正常个体和显示所述疾病状态的个体间差异性表达的mRNA。
2.如权利要求1所述的方法,其特征在于,所述疾病状态选自癌症、自身免疫疾病、糖尿病和神经疾病。
3.如权利要求1所述的方法,其特征在于,所述获得人mRNA的步骤包括用RNA酶抑制剂处理人唾液样品。
4.如权利要求3所述的方法,其特征在于,所述RNA酶抑制剂包括RNAlaterTM组合物。
5.一种保存唾液RNA的方法,所述方法包括:
获得唾液样品;
将所述样品与RNAlaterTM组合物混合。
6.一种试剂盒,所述试剂盒包括:用于收集唾液的容器,和RNAlaterTM组合物。
