La presente invenzione riguarda una particolare formulazione di gel acquoso a base di carragenina avente di per sè capacità emollienti e lenitive degli stati dolorifici e delle infiammazioni caratteristiche delle mucose e dell'epidermide; questa formulazione può essere utilizzata sia come tale che come veicolante di altri principi attivi per uso topico.
Nella tecnica antecedente era già noto l'uso della carragenina per la preparazione di gel destinati ad applicazioni farmaceutiche o cosmetiche, come descritto nel brevetto statunitense n. 5 246 702 che insegna un procedimento per la purificazione di carragenine commerciali da utilizzarsi come gelificanti in preparazioni farmaceutiche, cosmetiche o dietetiche. I gel così prodotti sono però caratterizzati da una bassa viscosità, da una compattezza irregolare e da un aspetto non sempre limpido ed incolore; elementi questi che ne pregiudicano spesso l'utilizzo, in particolare, appunto, in campo cosmetico e farmaceutico.
Scopo principale della presente invenzione è quello di ottenere gel, privi dei sopracitati inconvenienti, utilizzabili come veicolanti di principi attivi per uso topico. Questo risultato viene raggiunto appunto con l'utilizzo contemporaneo, vale a dire in una stessa formulazione ad uso topico, di una carragenina a basso peso molecolare ed elevato grado di purezza e di un sale dell'acido ialuronico.
E' stato infatti sorprendentemente trovato che viscosità, irregolarità di compattezza, trasparenza e colore dei gel a base di sola carragenina si modificano in modo ragguardevole con l'aggiunta di una soluzione di acido ialuronico svariatamente salificato; operando in questo modo, infatti, aumenta la viscosità della soluzione acquosa e si passa ad un gel altamente scorrevole, compatto, filamentoso, perfettamente limpido, trasparente ed incolore. Tale fenomeno sembrerebbe plausibilmente giustificabile con la formazione di un nuovo complesso chimico, solubile in acqua, attribuibile alla formazione di legami a ponte idrogeno, tra le frazioni polisaccaridiche dei due prodotti. Le caratteristiche fisico-chimiche di tale nuova associazione sono totalmente differenti da quelle ottenibili in medesime soluzioni acquose con i due componenti presi singolarmente.
In particolare, per la viscosità del nuovo gel a base di carragenina e di un sale dell'acido ialuronico, in questo caso sodio ialuronato, è stato trovato un valore di 2000 cps ad una temperatura variabile tra 20 e 25 DEG C; la determinazione è stata effettuata con un VISCOTESTER-02 della HAAKE. Le misure di viscosità sui gel a base di sola carragenina non sono invece praticamente possibili, proprio per la loro struttura irregolare, caratterizzata da viscosità e compattezza variabili e non facilmente riproducibili.
Il sale dell'acido ialuronico e la carragenina devono essere presenti singolarmente in una quantità variabile tra lo 0.2% e l'1.5% in peso; è inoltre preferibile che la carragenina sia presente con un eccesso del 20% rispetto al sale dell'acido ialuronico. Per la formazione del sale dell'acido ialuronico, possono essere utilizzati i metalli alcalini o alcalino terrosi, oppure quelle basi organiche che permettono di ottenere comunque sali perfettamente solubili in acqua o in una soluzione acquosa di glicoli, in particolare glicol etilenico e/o propilenico o glicerina; il sale preferito per la presente invenzione è il sodio ialuronato. Per quello che riguarda il peso molecolare della carragenina, invece, questo può variare tra 1000 e 1 000 000.
Per il particolare equilibrio dei suoi componenti fondamentali, questo gel fornisce una contemporanea azione lubrificante ed idratante delle mucose e dell'epidermide stessa, caratterizzata da una lunga durata e da un'ottima tollerabilità, anche dopo un numero considerevole di applicazioni. I principi attivi che eventualmente possono essere utilizzati e veicolati devono essere solubili in acqua o in una soluzione acquosa di glicoli, in particolare glicol propilenico e/o etilenico, o glicerina.
Un ulteriore oggetto della presente invenzione è costituito da una particolare formulazione di gel acquoso caratterizzata dall'utilizzo della frazione triterpenica di Centella Asiatica in combinazione con sodio ialuronato e carragenina, in cui la frazione triterpenica di Centella Asiatica è presente in una quantità pari al 20% in peso rispetto al sodio ialuronato; il sodio ialuronato e la carragenina sono invece presenti nelle quantità e nei rapporti in peso precedentemente descritti. Oltre ai sopracitati componenti principali, la formulazione può contenere eccipienti, veicolanti e conservanti standard, quali, per esempio, l'essenza di magnolia, Metilparaben, Propilparaben, clorexidina digluconato, Euxil K400, glicol propilenico o glicerina, glicol etilenico, glicole dietilenico monoetiletere o altri glicoli e acqua.
La formulazione oggetto della presente invenzione è stata sperimentata in clinica dermatologica e ginecologica sia su donne sane, che su pazienti affette da svariate e particolari patologie e disfunzioni dell'apparato vaginale, con particolare attenzione ai casi di dispareunia e craurosi vulvare. Il prodotto ha mostrato un'ottima efficacia terapeutica unita ad un'eccellente tollerabilità; dai dati clinici si evidenzia che il sodio ialuronato e la frazione triterpenica di Centella Asiatica partecipano alla risoluzione delle lesioni mucoso-epiteliali, in quanto rigenerano il tessuto riducendo nel contempo la formazione di cicatrici.
Ciò viene anche supportato da premesse farmacologiche, per cui si può ritenere che, immettendo nel gel principi attivi quali, appunto, la Centella Asiatica, si ottiene un completamento di attività degli stessi, talvolta anche in modo sinergico, sfruttando il potere veicolante della carragenina. L'uso di questo gel è risultato essere notevolmente efficace anche nei casi di patologie caratterizzati da componente algica, portando sempre ad un miglioramento, anche se a volte parziale, della sintomatologia.
ll prodotto ha mostrato un'eccellente compliance per tutte le pazienti; giudizi particolarmente buoni e soddisfacenti sono stati infatti espressi per quello che riguarda la sua efficacia terapeutica. I risultati appaiono quindi doppiamente interessanti, se si considera che la preparazione ha mostrato anche un gradevole comportamento idratante ed umettante, proprio come un prodotto cosmetico, sia nelle donne sane, che in quelle affette da patologie.
La formulazione in forma di gel oggetto della presente invenzione si presenta pertanto gradevole sia alla vista che al tatto; produce una sensazione piacevole al momento dell'applicazione e può condurre ad una rilevante azione terapeutica, lenitiva e cicatrizzante, a seconda dei principi attivi che vengono di volta in volta utilizzati. Variando il rapporto quantitativo dei componenti della formulazione, è infatti possibile associare alla stessa principi attivi differenti, scelti in base alla loro attività specifica e variabili in quantità a seconda dell'indirizzo clinico a cui possono venire preposti. Il principio attivo della formulazione, cioè la Centella Asiatica, può essere integralmente sostituito da altri principi attivi, oppure essere utilizzato in combinazione con gli stessi, a seconda delle possibili azioni terapeutiche desiderate.
I principi attivi qui sotto elencati possono essere immessi, in alternativa, o in combinazione con la Centella Asiatica, nella formulazione in forma di gel contenente un sale dell'acido ialuronico e carragenina; il seguente elenco ha unicamente scopo esemplificativo e non limitativo, per tutti quei principi attivi che possono essere utilizzati nella formulazione in forma di gel, oggetto della presente invenzione.
1. Benzidamina. La benzidamina può essere immessa nel gel quando lo stesso viene utilizzato direttamente, o in garze medicate, per la cura di mialgie, tendiniti, contusioni e distorsioni muscolari; in questo caso la quantità di benzidamina contenuta nella formulazione può variare tra l'1 ed il 10% in peso, con un valore preferito del 5%. Se invece la formulazione contenente benzidamina viene utilizzata in campo ginecologico, la quantità di benzidamina deve essere compresa tra lo 0.5 ed il 6% in peso. In questi casi è preferibile abbinare all'attività antinfiammatoria e leggermente anestetica, propria appunto del gel contenente benzidamina, un'attività antibatterica, facendo per esempio ricorso all'utilizzo di sali quaternari, come il paratoluensolfonato di trimetilcetilammonio, in quantità variabile tra lo 0.1 ed il 3% in peso, con valore preferenziale dell'1%.
2. Paraisobutilfenilpropionato di lisina. Il paraisobutilfenilpropionato di lisina può essere immesso nel gel, in quantità variabile tra l'1 ed il 10% in peso, per la cura degli stati infiammatori dell'epidermide; il gel così ottenuto può essere usato direttamente oppure in garze da applicare sulla parte lesa. Nel caso in cui il gel dovesse essere destinato ad un uso ginecologico, il paraisobutilfenilpropionato di lisina può variare tra lo 0.5 ed il 5% in peso, con valore preferenziale dell'1%.
3. Le stesse considerazioni fatte per il paraisobutilfenilpropionato di lisina valgono anche per il Naproxen se impiegato sotto forma di sale sodico o di altri sali solubili e, ugualmente, per tutti gli altri antinfiammatori non steroidei salificabili, sia con ioni inorganici, sia con basi organiche e che portano comunque a sali stabili e solubili in acqua.
4. Nonoxinolo. Il nonoxinolo può essere immesso nel gel in quantità variabile tra lo 0.1 ed il 15% in peso, preferibilmente all' 8.5%, ed utilizzato sotto questa forma per la cura delle vaginiti da Trichomonas o da altri batteri. Per questo tipo di applicazione il nonoxinolo può essere impiegato solo od in associazione con composti ad attività antibatterica, come l'alcool fenossietilico o similari.
5. Sali quaternari. Questi sali possono essere utilizzati come "alcoli che non bruciano" e sono particolarmente indicati per ferite, abrasioni, ustioni, punture di insetti e disinfezioni della cute e delle mani. Il sale quaternario più indicato per questo scopo è il cloruro di benzalconio, che può essere immesso nel gel in quantità variabile tra lo 0.1 ed il 5% in peso, con valore preferenziale intorno all'1%. Il cloruro di benzalconio può anche essere usato a questo proposito in combinazione con altri composti, come ad esempio con lo stesso nonoxinolo; nella composizione preferita sono presenti benzalconio cloruro allo 0.2% e nonoxinolo 30 all'1.5%.
Di notevole interesse è anche l'utilizzo del cloruro di dodecilbenzilammonio-N-dietil alcool da utilizzare come antibatterico ed antimicotico in concentrazione variabile tra lo 0.1 ed il 5% in peso, preferibilmente allo 0.2%.
6. Metronidazolo. Può essere utilizzato come trichomonacida topico in concentrazione variabile tra lo 0.5 ed il 10% in peso, preferibilmente al 2%.
7. Miconazolo. Può essere impiegato in alternativa ad altri antifungini solubili in concentrazione variabile tra lo 0.5 ed il 10% in peso, preferibilmente al 2%.
Possono inoltre essere utilizzati gli antibiotici solubili e stabili in soluzione acquosa, nella composizione base gelificata, purchè rimangano stabili nel tempo in queste condizioni, e, in generale, i composti di origine estrattiva sia animale che vegetale, come per esempio i glicosaminoglicani o i fosfolipidi deacetilati, purchè anch'essi solubili e stabili nel tempo. Più in generale, sono così utilizzabili tutti i principi attivi comunemente impiegati in campo farmaceutico, cosmetico, veterinario, parafarmaceutico e nel settore della strumentazione medica.
La formulazione oggetto della presente invenzione può anche essere utilizzata per la preparazione di garze, bende, bendaggi e cerotti pretrattati, da utilizzarsi localmente in campo farmaceutico, cosmetico, veterinario e dietetico. Può essere utilizzata per la disinfezione e la pulizia dei denti e del cavo orale, come un comune dentifricio, oppure utilizzata per la preparazione di soluzioni opportunamente diluite, destinate ad eventuali sciacqui sempre per la disinfezione e la pulizia dei denti e del cavo orale. Soluzioni diluite della formulazione in oggetto, possono anche essere impiegate per la disinfezione di attrezzi, pareti, pavimenti e di strumenti utilizzati in campo medico-chirurgico.

La formulazione in forma di gel qui di seguito riportata è stata oggetto di test farmacologici effettuati su 150 pazienti di sesso femminile, al fine di valutarne l'efficienza, la tollerabilità e anche l'aspetto estetico:

Sono state ammesse al test sia donne in perfette condizioni di salute, che donne affette da disfunzioni e patologie dell'apparato vaginale; i dati esatti sono riportati nella seguente tabella insieme alle relative diagnosi:

L'età delle pazienti era compresa tra i 17 ed i 50 anni con una media 31.68 +/- 8.19 anni.
Tutte le pazienti sono state sottoposte a 2 applicazioni giornaliere per un periodo di 30 giorni consecutivi. Dopo 15 e 30 giorni dall'inizio del trattamento le pazienti sono state sottoposte a visita ginecologica e/o dermatologica; inoltre, per tutta la durata del test, le pazienti sono state tenute sotto osservazione, con una particolare attenzione verso i seguenti sintomi: prurito, eritema, edema, vescicazione e dispareunia.
L'efficacia terapeutica del trattamento è stata quindi valutata in base ad una scala a 5 livelli; i risultati sono riportati in tabella 2, su base percentuale e limitatamente ai casi di dispareunia, condilectomia e craurosi vulvare, essendo poco significativi da un punto di vista statistico, i dati relativi alle altre patologie.

The present invention relates to a particular formulation of aqueous gel based on carrageenan which has per se emollient and soothing properties for painful conditions and characteristic inflammations of the mucous membranes and epidermis; this formulation can be used both as such and as a carrier of other active ingredients for topical use.
In the prior art, the use of carrageenan for the preparation of gels for pharmaceutical or cosmetic applications was already known, as described in U.S. Pat. 5 246 702 which teaches a process for the purification of commercial carrageenans to be used as gelling agents in pharmaceutical, cosmetic or dietetic preparations. However, the gels thus produced are characterized by a low viscosity, an irregular compactness and an appearance that is not always clear and colorless; these elements often jeopardize their use, in particular, in the cosmetic and pharmaceutical field.
The main object of the present invention is to obtain gels, free of the aforementioned drawbacks, usable as carriers of active principles for topical use. This result is achieved precisely with the simultaneous use, i.e. in the same topical formulation, of a carrageenan with low molecular weight and high degree of purity and a salt of hyaluronic acid.
In fact, it has been surprisingly found that the viscosity, irregularity of compactness, transparency and color of the gels based on carrageenan only change considerably with the addition of a solution of variously salified hyaluronic acid; operating in this way, in fact, increases the viscosity of the aqueous solution and passes to a highly flowing, compact, filamentous, perfectly clear, transparent and colorless gel. This phenomenon would seem plausibly justifiable with the formation of a new chemical complex, soluble in water, attributable to the formation of hydrogen bridge bonds, between the polysaccharide fractions of the two products. The physical-chemical characteristics of this new association are totally different from those obtainable in the same aqueous solutions with the two components taken individually.
In particular, for the viscosity of the new carrageenan-based gel and of a salt of hyaluronic acid, in this case sodium hyaluronate, a value of 2000 cps was found at a temperature ranging between 20 and 25 DEG C; the determination was made with a HAAKE VISCOTESTER-02. Viscosity measurements on carrageenan-based gels, on the other hand, are practically not possible, precisely because of their irregular structure, characterized by variable viscosity and compactness which are not easily reproducible.
The hyaluronic acid salt and carrageenan must be present individually in an amount ranging from 0.2% to 1.5% by weight; it is also preferable that carrageenan is present with an excess of 20% with respect to the salt of hyaluronic acid. For the formation of the salt of hyaluronic acid, alkaline or alkaline earth metals can be used, or those organic bases that allow to obtain in any case salts perfectly soluble in water or in an aqueous solution of glycols, in particular ethylene and / or propylene glycol or glycerin; the preferred salt for the present invention is sodium hyaluronate. As regards the molecular weight of carrageenan, however, this can vary between 1000 and 1 000 000.
Due to the particular balance of its fundamental components, this gel provides a simultaneous lubricating and moisturizing action of the mucous membranes and the epidermis itself, characterized by a long life and excellent tolerability, even after a considerable number of applications. The active principles that can be used and conveyed if necessary must be soluble in water or in an aqueous solution of glycols, in particular propylene and / or ethylene glycol, or glycerin.
A further object of the present invention is constituted by a particular aqueous gel formulation characterized by the use of the triterpene fraction of Centella Asiatica in combination with sodium hyaluronate and carrageenan, wherein the triterpene fraction of Centella Asiatica is present in an amount equal to 20% by weight with respect to sodium hyaluronate; sodium hyaluronate and carrageenan are instead present in the quantities and weight ratios previously described. In addition to the aforementioned main components, the formulation may contain standard excipients, carriers and preservatives, such as, for example, the essence of magnolia, Methylparaben, Propylparaben, chlorhexidine digluconate, Euxil K400, propylene glycol or glycerin, ethylene glycol, diethylene glycol monoethyl ether or other glycols and water.
The formulation object of the present invention has been tested in dermatological and gynecological clinic both on healthy women and on patients suffering from various and particular pathologies and dysfunctions of the vaginal apparatus, with particular attention to the cases of dyspareunia and vulvar craurosis. The product has shown excellent therapeutic efficacy combined with excellent tolerability; clinical data show that sodium hyaluronate and the triterpene fraction of Centella Asiatica participate in the resolution of mucosal-epithelial lesions, as they regenerate the tissue while reducing the formation of scars.
This is also supported by pharmacological premises, so it can be assumed that, by introducing in the gel active ingredients such as, Centella Asiatica, we obtain a completion of their activities, sometimes also synergistically, exploiting the carrier power of carrageenan. The use of this gel was found to be remarkably effective even in cases of pathologies characterized by an algic component, always leading to an improvement, even if at times partial, of the symptoms.
The product showed excellent compliance for all patients; particularly good and satisfactory judgments were in fact expressed with regard to its therapeutic efficacy. The results therefore appear doubly interesting, considering that the preparation has also shown a pleasant moisturizing and humectant behavior, just like a cosmetic product, both in healthy women and in those affected by pathologies.
The gel formulation object of the present invention is therefore pleasant both to the eye and to the touch; produces a pleasant sensation at the time of application and can lead to a significant therapeutic, soothing and healing action, depending on the active ingredients that are used from time to time. By varying the quantitative ratio of the components of the formulation, it is in fact possible to associate different active ingredients to the same, chosen on the basis of their specific activity and variable in quantity according to the clinical direction to which they can be assigned. The active ingredient of the formulation, i.e. Centella Asiatica, can be completely replaced by other active ingredients, or can be used in combination with them, depending on the possible therapeutic actions desired.
The active ingredients listed below can be placed, alternatively, or in combination with Centella Asiatica, in the gel formulation containing a salt of hyaluronic acid and carrageenan; the following list is for exemplificative and non-limiting purposes only, for all those active principles that can be used in the gel formulation, object of the present invention.
1. Benzidamine. Benzydamine can be introduced into the gel when it is used directly, or in medicated gauze, for the treatment of myalgias, tendinitis, bruises and muscle sprains; in this case the quantity of benzidamine contained in the formulation can vary between 1 and 10% by weight, with a preferred value of 5%. On the other hand, if the formulation containing benzidamine is used in the gynecological field, the quantity of benzidamine must be between 0.5 and 6% by weight. In these cases it is preferable to combine the anti-inflammatory and slightly anesthetic activity, typical of the gel containing benzidamine, with an antibacterial activity, making use, for example, of the use of quaternary salts, such as trimethylcetylammonium paratoluene sulfonate, in a quantity ranging from 0.1 to 3% by weight, with a preferential value of 1%.
2. Lysine paraisobutylphenylpropionate. Lysine paraisobutylphenylpropionate can be placed in the gel, in an amount ranging from 1 to 10% by weight, for the treatment of inflammatory states of the epidermis; the gel thus obtained can be used directly or in gauze to be applied on the injured part. If the gel is intended for gynecological use, the lysine paraisobutylphenylpropionate can vary between 0.5 and 5% by weight, with a preferential value of 1%.
3. The same considerations made for lysine paraisobutylphenylpropionate also apply to Naproxen if used in the form of sodium salt or other soluble salts and, equally, for all the other salable non-steroidal anti-inflammatory drugs, both with inorganic ions and with organic bases. and which however lead to stable and water-soluble salts.
4. Nonoxinol. Nonoxinol can be introduced into the gel in quantities ranging from 0.1 to 15% by weight, preferably 8.5%, and used in this form for the treatment of vaginitis caused by Trichomonas or other bacteria. For this type of application, nonoxinol can be used only or in association with compounds with antibacterial activity, such as phenoxyethyl alcohol or similar.
5. Quaternary salts. These salts can be used as "non-burning alcohols" and are particularly suitable for wounds, abrasions, burns, insect bites and disinfection of the skin and hands. The quaternary salt most suitable for this purpose is benzalkonium chloride, which can be introduced into the gel in quantities ranging from 0.1 to 5% by weight, with a preferential value of around 1%. Benzalkonium chloride can also be used in this connection in combination with other compounds, such as with the same nonoxinol; the preferred composition contains 0.2% benzalkonium chloride and 1.5% nonoxinol 30.
Of particular interest is the use of dodecylbenzylammonium-N-diethyl alcohol to be used as an antibacterial and antifungal in a concentration ranging from 0.1 to 5% by weight, preferably 0.2%.
6. Metronidazole. It can be used as a topical trichomonacid in a concentration ranging from 0.5 to 10% by weight, preferably at 2%.
7. Miconazole. It can be used as an alternative to other soluble antifungals in a concentration ranging from 0.5 to 10% by weight, preferably 2%.
Soluble and stable antibiotics in aqueous solution, in the gelled base composition, can also be used provided they remain stable over time in these conditions, and, in general, the compounds of both animal and vegetable extractive origin, such as glycosaminoglycans or deacetylated phospholipids , as long as they too soluble and stable over time. More generally, all the active principles commonly used in the pharmaceutical, cosmetic, veterinary, parapharmaceutical and in the medical instrumentation sector can be used.
The formulation object of the present invention can also be used for the preparation of gauze, bandages, bandages and pretreated plasters, to be used locally in the pharmaceutical, cosmetic, veterinary and dietetic fields. It can be used for the disinfection and cleaning of the teeth and oral cavity, like a common toothpaste, or used for the preparation of suitably diluted solutions, intended for possible rinses always for the disinfection and cleaning of the teeth and oral cavity. Diluted solutions of the formulation in question, can also be used for the disinfection of tools, walls, floors and tools used in the medical-surgical field.

The gel formulation below has been subjected to pharmacological tests carried out on 150 female patients, in order to evaluate their efficiency, tolerability and also the aesthetic aspect:

The carrageenan used in this formulation was purified following the teachings of the aforementioned United States Patent no. 5 246 702. Both women in perfect health conditions and women with vaginal dysfunction and pathologies were admitted to the test; exact data are shown in the following table together with the relative diagnoses:

The age of the patients was between 17 and 50 years with an average of 31.68 +/- 8.19 years.
All patients underwent 2 daily applications for a period of 30 consecutive days. After 15 and 30 days from the start of the treatment, the patients underwent a gynecological and / or dermatological examination; moreover, for the duration of the test, the patients were kept under observation, with particular attention to the following symptoms: itching, erythema, edema, blistering and dyspareunia.
The therapeutic efficacy of the treatment was therefore assessed on a 5-level scale; the results are shown in table 2, on a percentage basis and limited to the cases of dyspareunia, condylectomy and vulvar craurosis, being statistically insignificant, the data relating to other pathologies.
