Abstract
The cup shaped half (1) of a hip joint prosthesis has a part conical metal shell (2) containing a core (7) with a half spherical recess (4) for the ball end of the other half. The sloping external surface of the shell includes a helical thread (10) formed by short sawtoothed sections, which may be of briar tooth type. Description
Die vorliegende Erfindung bezieht sich auf eine Gelenkpfanne gemäss dem Oberbegriff des Anspruchs 1. Gelenkpfannen dieser Art sind bekannt. Ihr Nachteil besteht darin, dass sie mit vergleichsweise grossem Kraftaufwand in die am Beckenknochen ausgefräste Kavität eingedreht werden müssen.
Erfindungsgemäss wird diese Aufgabe gelöst durch die kennzeichnenden Merkmale des Anspruchs 1. Die Erfindung schafft für die beim Eindrehen der Gelenkpfanne anfallenden Knochenspäne zwischen den Zähnen genügend Aufnahmeraum, so dass die Aussenfläche mit mässigem Kraftaufwand vollständig in Berührungskontakt mit dem Beckenknochen gebracht werden kann.
Anhand der beiliegenden schematischen Zeichnung wird die Erfindung beispielsweise erläutert. Es zeigen:

Die zweiteilig gestaltete Gelenkpfanne 1 weist eine äussere Metallschale 2 und einen Kern 3 vorzugsweise aus Polyäthylen auf, welcher die halbkugelige Gelenkfläche 4 bildet, an der der nicht gezeigte Gelenkkopf des Hüftprothesengelenkes gelagert ist. Die Metallschale 2 und der Kern 3 sind spielfrei und fest durch eine Rastverbindung miteinander verbunden, welche von einer Ringnut 5 auf der Innenseite der Metallschale 2 und einer Ringrippe 6 am Kern 3 gebildet wird. Der Kern 3 greift zudem mit einem zylindrischen Zapfen 7 in eine im Querschnitt quadratische Ausnehmung 8 im Boden der Metallschale 2.
Die konische Aussenseite der Metallschale 2 ist mit einem konischen Gewinde 10 versehen, das über seine ganze Länge mit einer Sägezahnung mit einem Spanwinkel alpha von 0 DEG versehen ist. Bei zureichendem Freiwinkel alpha (hier ca. 12 DEG ) entsteht ein Selbstschneidegewinde, wodurch zum Einpflanzen im Beckenknochen lediglich eine konische Kavität mit dem Kerndurchmesser auszunehmen ist, in die die Gelenkpfanne 1 (ohne vorgängiges Gewindeschneiden) eingeschraubt werden kann. Die beim Einschrauben aufzubringende Kraft ist vergleichsweise gering. Die bevorzugte Zahnform ist die Wolfszahnung. Der Freiwinkel beta ist vorzugsweise so gewählt, dass die Freifläche 11 die Sehne an der die Schneidkanten 12 enthaltenden Schraubenlinie 13 bildet.
Dies bewirkt im eingepflanzten Zustand eine vergleichsweise grosse Auflagefläche zwischen Beckenknochen und Gelenkpfanne 1, was einer durch Überlastung verursachten Knochenresorption entgegenwirkt. Ein Kompromiss zwischen der Belastung des Beckenknochens durch die Gelenkpfanne 1 und der für das Einschrauben erforderlichen Kraft kann durch eine Änderung des Spanwinkels zwischen 0 DEG und 15 DEG oder durch die Wahl der Zahnteilung erreicht werden. Zum Schutz des Beckenknochens gegen eine mechanische Überlastung kann am äusseren Schalenrand ein Flansch (Fig. 4 bis 6) vorgesehen sein, der das Einschrauben der Gelenkpfanne begrenzt, indem er sich an den Rand der Kavität legt und einem späteren Einsinken der Gelenkpfanne unter Belastung in den Beckenknochen entgegenwirkt.
Bei der gezeigten Zahnteilung befinden sich auf einer 90 DEG entsprechenden Umfangslinie zwei Zähne, das heisst, jeder Zahn nimmt den einem Winkel gamma von 90 DEG entsprechenden Raum ein. Eine Zahnteilung entsprechend einem Winkel gamma zwischen 15 DEG und 60 DEG ist grundsätzlich möglich.
Die Höhe des Gewindeprofils kann (wie gezeigt) konstant oder auf das verjüngte Pfannenende zu abnehmend ausgebildet sein.
Wird der Konuswinkel der Gelenkpfanne 1 zwischen 32 DEG und 36 DEG anstatt der üblichen 30 DEG gewählt, ist mit weniger Knochenresektion ein passender Konus im harten Knochen (Corticalis) der Hüftgelenkspfanne erzielbar.
Der Flansch 14 kann weiter von zentralsymmetrisch verteilten Bohrungen (nicht gezeigt) durchsetzt sein, die erforderlichenfalls das Anbringen von Knochenschrauben ermöglichen.
Beim Einbau der Gelenkpfanne 1 in eine konische Knochenkavität wird zuerst die Metallschale 2 eingeschraubt, wobei ein entsprechendes Werkzeug in der quadratischen Ausnehmung 8 angesetzt wird. Ist die Metallschale 2 in die gewünschte Lage gebracht wird der Kern 3 bis zum Verrasten der Ringrippe 6 mit dem Kern 3 in diese hineingedrückt.

According to the invention, this object is achieved by the characterizing features of claim 1. The invention creates sufficient receiving space for the bone chips between the teeth that occur when the joint socket is screwed in, so that the outer surface can be brought fully into contact with the pelvic bone with moderate effort.

The two-part socket 1 has an outer metal shell 2 and a core 3, preferably made of polyethylene, which forms the hemispherical joint surface 4 on which the joint head, not shown, of the hip prosthesis joint is mounted. The metal shell 2 and the core 3 are free of play and firmly connected to one another by a snap connection, which is formed by an annular groove 5 on the inside of the metal shell 2 and an annular rib 6 on the core 3. The core 3 also engages with a cylindrical pin 7 in a recess 8 with a square cross section in the bottom of the metal shell 2.
The conical outside of the metal shell 2 is provided with a conical thread 10, which is provided over its entire length with a serration with a rake angle alpha of 0 °. If the clearance angle alpha is sufficient (here approx. 12 °), a self-tapping thread is created, so that only a conical cavity with the core diameter is to be used for implantation in the pelvic bone, into which the socket 1 can be screwed (without prior thread cutting). The force to be applied when screwing in is comparatively low. The preferred tooth shape is wolf teeth. The clearance angle beta is preferably selected such that the clearance surface 11 forms the chord on the helical line 13 containing the cutting edges 12.
In the implanted state, this causes a comparatively large contact surface between the pelvic bone and the socket 1, which counteracts bone resorption caused by overloading. A compromise between the load on the pelvic bone by the socket 1 and the force required for screwing in can be achieved by changing the rake angle between 0 ° and 15 ° or by choosing the tooth pitch. To protect the pelvic bone against mechanical overload, a flange (Fig. 4 to 6) can be provided on the outer shell edge, which limits the screwing in of the socket by lying on the edge of the cavity and later sinking of the socket under load into the Counteracts pelvic bones.
In the tooth pitch shown, there are two teeth on a circumferential line corresponding to 90 °, that is, each tooth occupies the space corresponding to an angle gamma of 90 °. A tooth pitch corresponding to an angle gamma between 15 DEG and 60 DEG is basically possible.
The height of the thread profile (as shown) can be constant or designed to decrease towards the tapered socket end.
If the cone angle of the acetabular cup 1 is chosen between 32 ° and 36 ° instead of the usual 30 °, a suitable cone in the hard bone (corticalis) of the acetabular cup can be achieved with less bone resection.
The flange 14 can also be penetrated by centrally symmetrically distributed bores (not shown) which, if necessary, enable the attachment of bone screws.
When installing the socket 1 in a conical bone cavity, the metal shell 2 is first screwed in, a corresponding tool being placed in the square recess 8. If the metal shell 2 is brought into the desired position, the core 3 is pressed into the ring rib 6 until the ring rib 6 engages.
