Bekanntlich lassen sich Steroide topisch verwenden. Ihre
Verwendung auf diese Weise hat aber unterschiedlichen Er folg gezeigt. Es besteht daher Bedarf für ein topisch wirksames Steroid. Es wurde nun gefunden, das Triamcinolon -acetonid-21 -valerat, d.h. 9cr,-Fluor- 11 ,B,21 -dihydroxy-l 6a, 1 7a-isopropylidendioxy-l ,4-pregnadien-3,20-dion-2l -valerat, als aktive Komponente in einer topischen Zusammensetzung ein sehr wirksames antiinflammatorisches Mittel ist. Diese Verbindung ist nach dem Vasokonstriktionsversuch wesentlich wirksamer als topisches antiinflammatorisches Mittel als viele Verbindungen, die in der Vergangenheit verwendet wurden, wie Triamcinolonacetonid.
Das Triamcinolon-acetonid-21-valerat, nämlich 9cx- -Fluor-l 1 p,21 -dihydroxy-l 6cm.,1 7,5C-isopropylidendioxy- 1,4- -pregnadien-3,20-dion-21 -valerat, wird erfindungsgemäss hergestellt, indem man n-Valerylchlorid mit Triamcinolonacetonid umsetzt, wobei man vorzugsweise in einer gekühl ten gerührten Lösung arbeitet. Die Mischung wird dann auf
Rückflusstemperatur erwärmt, und dann kann sie auf Zim mertemperatur abkühlen. Die Mischung wird mit Eiswasser verdünnt und mit einem Lösungsmittel wie Chloroform extrahiert.
Der Chloroformextrakt wird mit Wasser, verdünnter Chlorwasserstoffsäure, verdünntem Natriumbicarbonat und schliesslich mit gesättigter Kochsalzlösung behandelt. Der Chloroformextrakt wird bei vermindertem Druck eingedampft. Das Produkt wird mit Aceton/n-Hexan (1:19) aufgeschlämmt. Die Filtration ergibt das Produkt, nämlich 9alpha-Fluor-113,21 -dihydroxy-l 6alpha, 1 7aipha-isopropyli- dendioxy-l ,4-pregnadien-3 ,20-dion-21 -valerat, das in der vorliegenden Anmeldung als Triamcinolon-acetonid-21 - -valerat bezeichnet wird.
Dieses Steroidprodukt kann in eine Vielzahl bekannter pharmazeutischer Formulierungen, die topische Präparationen ergeben, bei Konzentrationen von beispielsweise 0,01% bis 0,50%, in topische Salben in ähnlichen Konzentrationen oder in Lotionen eingearbeitet werden.
Die Mittel, die den aktiven erfindungsgemässen Bestandteil enthalten, können irgendwelche pharmazeutischen topischen Standardpräparationen sein, beispielsweise: Lösungen, Suspension, Lotionen, Salben, Cremes, flüssige Salben bzw. Gelee, Sprays, Pulver, Schäume usw. Arzneimittelträger oder Verdünnungsstoffe, wie sie bei solchen Präparationen verwendet werden, können Puffer wie Phosphat Citrat- oder Tartratpuffer, oberflächenaktive Mittel wie Polyoxyäthylen-(20)-sorbitanmonooleat (Polysorbat 80) enthalten, dies ist eine komplexe Mischung aus Polyoxyäthylenäthern gemischter Teilölsäureester von Sorbitanhydriden und oxylierten tertiären Octylphenol-Formaldehyd-Polymeren, ein Mittel, das die Oberflächenspannung vermindert.
Konservierungsstoffe wie Methyl- und Propylparabene, d.h.
die Methyl- und Propylester von p-Hydroxybenzoesäure, Kaliumsorbat, Benzylalkohol und ähnliche Verbindungen können verwendet werden. Öle, Wachse, Fette usw. sind als erweichende Mittel und als Grundstoffe für Salben und Emulsionen sind Vaselin, Wollfett (wasserfreies Lanolin), Squalen, reines Walfett und ähnliche Stoffe nützlich. Stabilisatoren wie Talg, Tone, pflanzliche Kolloide, Carboxymethylcellulose, Carboxypolymethylen und ähnliche und Parfüms oder Duftstoffe wie Lavendel, Zitrone, Gardenien usw.
sind ebenfalls nützlich. Solche Präparationen können in Druckbehälter eingefüllt werden, und dann kann man dazu ein geeignetes Treibmittel wie Trichlorfluormethan, Dichlordifluormethan oder 12-Dichlor-1,12,2-tetrafluoräthan zufügen, Mittel wie sie üblicherweise verwendet werden.
Triamcinolin-acetonid-21-valerat wurde mit einer Vielzahl anderer Steroide verglichen, von denen bekannt ist, dass sie topische antiinflammatorische Aktivität aufweisen und wobei man ein Verfahren verwendete, bei dem die Vaso konstriktion bestimmt wird. Die verschiedenen Verbindungen werden für die Unter suchung durch Reihenverdünnungen in 95-prozentigem Al kohol formuliert, wobei man Konzentrationen von 1:10000 bis 1: 6 250 000 in 5 Stufen verwendet. Zwei Hundertstel eines Millimeters jeder Verdünnung werden auf die Handoberfläche des Unterarms von 10 normalen Menschen aufgebracht. Triamcinolinacetonid wird auf den anderen Unterarm in den gleichen Verdünnungen aufgebracht. Die Anwendungsflächen werden während 16 bis 20 Stunden ver bunden bzw. abgeschlossen und dann untersucht, um festzustellen, ob eine Vasokonstriktion vorhanden oder abwesend ist, wobei die Intensität der Vasokonstriktion nicht bewertet wird.
In der vorliegenden Tabelle werden die Ergebnisse ausgeführt. In der Tabelle sind die relativen Wirksamkeiten der Steroide im Vergleich mit dem Triamcinolon-acetonid aufgeführt.

Aus diesem Ergebnis ist ersichtlich,
dass verglichen mit Triamicinolon-acetonid Triamcinolon-acetonid 2-valerat bei diesen Versuchen um das 2,6-fache wirksamer ist. Alle anderen untersuchten Steroide besitzen eine wesentlich niedrigere Aktivität, und nur ein Steroid ist aktiver als Tiamcinolonacetonid.
Die folgenden Beispiele erläutern die Herstellung von Triamcinolon-acetonid-21-valerat und seine Verwendung in verschiedenen topischen Präparationen. Die Teile sind, sofern nicht anders angegeben, durch das Gewicht ausgedrückt.
Beispiel
Herstellung von 9α-Fluor-11ss-21-dihydroxy-16α,17α-iso- pro pylidendioxy-l ,4- pregnadien-3,20-dion-21 -valerat
Zu einer gerührten Lösung aus 3,0 g (6,9 mMol) Triamincinolonacetonid in 50 ml Pyridin, die in einem Eis/Wasserbad gerührt wird, fügt man tropfenweise 1,68 ml (14,1 mMol) n-Valerylchlorid. Nach Beendigung der Zugabe wird das Kühlbad entfernt. Die Reaktionsmischung wird eine Stunde am Rückfluss erwärmt, auf Zimmertemperatur abgekühlt und bei Zimmertemperatur über Nacht aufbewahrt. Die Reaktionsmischung wird in ungefähr 1,5 Liter Eis und Wasser unter Rühren gegossen. Die wässrige Lösung wird mit vier 100 ml Teilen Chloroform extrahiert.
Der Chloroformextrakt wird mit Wasser, verdünnter Chlorwasserstoffsäure, verdünntem Natriumbicarbonat und schliesslich mit gesät- tigter Salzlösung behandelt. Der Chloroformextrakte wird über wasserfreiem Magnesiumsulfat über Nacht getrocknet und dann bei vermindertem Druck eingedampft, wobei man einen Feststoff erhält.
Der Feststoff wird mit Aceton/n-Hexan (1:19), 25 ml, aufgeschlämmt. Das Produkt wird abfiltriert, wobei man 3,03 g (84,5% Ausbeute) eines farblosen Produktes erhält.
Eindampfen des Filtrats ergibt 0,33 g einer zweiten Charge (Gesamtausbeute 93,5%). Kristallisation aus Aceton/n-He- xan ergibt eine Probe, die die folgenden Eigenschaften besitzt: Schmelzpunkt 262,5 bis 263,5 C korrigierter Wert auf der Kofler-Heizstufe, DTA, ein einziger endothermer Wert 2640C, korrigiert. I.R. TKBr 3390, 1754, 1733, 1672, 1626, 1166 und 1058 cm-l; C0C1,250 + 930max (C = 0,408, CHCl3 1 dm); ###### 238 m (#16 600). Die Reinheit beträgt, bestimmt durch Verteilungssäulenchromatographie, 99%.

The various compounds are formulated for the investigation by serial dilutions in 95 percent alcohol, using concentrations from 1: 10,000 to 1: 6,250,000 in 5 stages. Two hundredths of a millimeter of each dilution is applied to the palm of the forearm of 10 normal people. Triamcinoline acetonide is applied to the other forearm in the same dilutions. The application areas are tied for 16 to 20 hours and then examined to determine whether vasoconstriction is present or absent, the intensity of vasoconstriction not being rated.
The results are shown in the table below. The table shows the relative potencies of the steroids compared to the triamcinolone acetonide.

From this result it can be seen
that compared with triamicinolone acetonide, triamcinolone acetonide 2-valerate is 2.6 times more effective in these experiments. All of the other steroids studied have much lower activity, and only one steroid is more active than tiamcinolone acetonide.
The following examples illustrate the preparation of triamcinolone acetonide-21-valerate and its use in various topical preparations. Unless otherwise indicated, parts are expressed by weight.

Preparation of 9α-fluoro-11ss-21-dihydroxy-16α, 17α-isopropylidenedioxy-1,4-pregnadiene-3,20-dione-21-valerate
To a stirred solution of 3.0 g (6.9 mmol) of triamine cinolone acetonide in 50 ml of pyridine, which is stirred in an ice / water bath, is added dropwise 1.68 ml (14.1 mmol) of n-valeryl chloride. After the addition is complete, the cooling bath is removed. The reaction mixture is refluxed for one hour, cooled to room temperature and stored at room temperature overnight. The chloroform extract is treated with water, dilute hydrochloric acid, dilute sodium bicarbonate and finally with saturated salt solution. The chloroform extract is dried over anhydrous magnesium sulfate overnight and then evaporated under reduced pressure to give a solid.
The solid is slurried with acetone / n-hexane (1:19), 25 ml. The product is filtered off, 3.03 g (84.5% yield) of a colorless product being obtained.
Evaporation of the filtrate gives 0.33 g of a second batch (overall yield 93.5%). Crystallization from acetone / n-hexane results in a sample which has the following properties: Melting point 262.5 to 263.5 C corrected value on the Kofler heating stage, DTA, a single endothermic value of 2640C, corrected. I.R. TKBr 3390, 1754, 1733, 1672, 1626, 1166 and 1058 cm-1; COC1.250 + 930max (C = 0.408, CHCl3 1 dm); ###### 238 m (# 16 600). The purity, determined by partition column chromatography, is 99%.
