Für eine histologische Untersuchung eines Gewebestückes, insbesondere für eine elektronenoptische Untersuchung, muss die entnommene Gewebeprobe möglichst lebendfrisch und daher sehr schnell in eine Fixierflüssigkeit, z. B. Formalin, Alkohol usw., eingebracht werden, damit nicht durch autolytische Vorgänge die Gewebestruktur verändert wird und die Diagnosestellung erschwert oder unmöglich ist. Ferner darf die Gewebeprobe bei diesen Vorgängen nicht gequetscht werden.
Die Erfindung setzt sich zur Aufgabe, ein Gerät zu schaffen, das den an es gestellten Anforderungen völlig genügt.
Die Erfindung geht aus von einem Biopsie-Entnahmegerät für nicht traumatisierende Gewebeentnahme zur histologischen, insbesondere elektronenoptischen Untersuchung, mit einem rotierenden zylindrischen Messer und besteht darin, dass zum Abschneiden des vom zylindrischen Messer ausgeschnittenen Gewebezylinders innerhalb des zylindrischen Messers einschwenkbare, um die Achse des zylindrischen Messers umlaufende Messer vorgesehen sind. Durch diese einschwenkbaren Messer ist in einfacher und zuverlässiger Weise ein Abschneiden des Gewebezylinders möglich, ohne dass derselbe hiebei gequetscht wird, so dass der erhaltene Gewebezylinder in seiner ursprünglichen Gewebestruktur vollkommen unverändert bleibt.
Weitere Vorteile des erfindungsgemässen Gerätes ergeben sich aus der Beschreibung eines Ausführungsbeispieles, welches in der Zeichnung im Längsschnitt schematisch dargestellt ist.
Das dargestellte Biopsie-Entnahmegerät dient für eine nicht traumatisierende Gewebeentnahme, also für eine Gewebeentnahme, bei welcher das Organ, dem das Gewebe entnommen werden soll, nicht zerstört, zerrissen oder gequetscht wird.
Ein Elektromotor 1, der in einem pistolengriffähnlichen Handgriff 3 des Gerätes eingeschlossen ist, der zugleich das Motorgehäuse bildet, überträgt seine Drehbewegung über Kegelzahnräder 4, 4a auf eine Antriebshohlwelle 5. Mit dem Vorderende dieser Antriebshohlwelle 5 ist ein Flansch 33 verbunden, mit dem mittels eines Bajonettverschlusses 36 ein auswechselbarer Messerkopf 6 verbunden ist, in dem mittels eines Gewindes 37 ein zylindrisches Messer 7 auswechselbar eingeschraubt ist. Die einzelnen Messer 7 bzw. ihre Messerköpfe 6 haben verschiedene Kaliber, so dass aus dem Gewebe 38 Gewebezylinder 7' beliebigen Durchmessers ausgeschnitten werden können. Zweckmässig sind auch für Gewebeentnahme aus einem Knochen besonders konstruierte Messer 7 vorgesehen.
Die Tourenzahl des Elektromotors 1 und das Übersetzungsverhältnis der Kegelzahnräder 4, 4a ist so zu wählen, dass die Umdrehungszahl des Messers 7 unter der Verbrennungsgrenze des jeweiligen Gewebezylinders 7' liegt. Die Einschaltung des Elektromotors 1 erfolgt durch einen Schaltstift 8 nach kurzem Zurückziehen eines als Abzug 9 ausgebildeten Schalters am Griffende des Gerätes, wobei der Abzug 9 durch eine Druckfeder 10 in seiner vordersten Stellung gehalten wird. Mit dem Abzug 9 ist ein einen Führungskolben bildender Ansatz 11 fest verbunden, der somit bei der Betätigung des Abzuges 9 nach hinten mitgenommen wird und hiebei eine mit ihm beweglich verbundene Führungsscheibe 12 mitnimmt.
Diese Führungsscheibe 12 besitzt eine mittige Öffnung, die von der Antriebshohlwelle 5 durchsetzt wird und in der ein radialer Stift 13 angeordnet ist, welcher in einem Langloch 14 der Antriebshohlwelle 5 gleitet, so dass die Führungsscheibe 12 und der Stift 13 die Drehbewegung der Antriebshohlwelle 5 mitmachen. Der Stift 13 ist aber auch in einem Langloch 15 einer zur Antriebshohlwelle 5 koaxial in dieser angeordneten weiteren Hohlwelle 16 verschiebbar geführt, welches Langloch an seinem hinteren Ende einen schraubenlinienförmig gewundenen Abschnitt 15' besitzt. Dadurch wird die innere Hohlwelle 16 gegenüber der Antriebshohlwelle 5 entsprechend verdreht, wenn der Abzug 9 durch den Finger des Benützers zurückgezogen und dadurch der Ansatz 11 samt der Führungsscheibe 12 nach hinten verschoben wird.
Die beiden Hohlwellen 5, 16 sind somit gemeinsam drehbar gekuppelt, relativ zueinander jedoch um einen kleinen Winkel verdrehbar. Die strichlierten Linien zeigen den Schalthahn 9 mit dem Ansatz 11, die Führungsscheibe 12 mit dem Stift 13 sowie den schraubenlinienförmig gewundenen Abschnitt 15' des Langloches 15 in der hintersten Stellung.
Die innere Hohlwelle 16 trägt vorne eine Kupplungsscheibe 17 mit Ausnehmungen 17', in welche Stifte 18' einer Kupplungsgegenscheibe 18 eingreifen, die im abnehmbaren Messerkopf 6 untergebracht ist. Die Kupplungsgegen scheibe 18 macht somit die Verdrehung der Hohlwelle 16 gegenüber der Antriebshohlwelle 5 mit, wobei sich bei dieser Relativverdrehung über in die Kupplungsgegenscheibe 18 locker eingerastete, winkelig abgebogene Achsen 19 der einschwenkbaren Messer 20 eine Einschwenkung von am Vorderende des Gerätes rechtwinkelig auf die Achsen 19 aufgesetzten inneren Schneidmessern 20 ergibt.
Diese Schneidmesser 20 liegen im geöffneten Zustand innen an der Wand des äusseren zylindrischen Messers 7 an und gehen bei Be täfigung des Abzuges 9 nach Art der Lamellen einer Irisblende eines photographischen Verschlusses so zusammen, dass sie sich in der Achse des zylinderförmigen Messers 7 berühren bzw. überschneiden. Die Achsen 19 der Messer 20 sind hiebei in Längsrillen 21 des zylinderförmigen Messers 7 geführt.
Zur Einstellung der Eindringtiefe des Messers 7 in das Gewebe 38 dient ein das Vorderende des Gerätes umgebender Zylinder 23, der vorne einen Begrenzungsanschlag 34 trägt Der Zylinder 23 ist mit seinem röhrenförmigen Teil auf dem Vorderende des Gerätes verschiebbar und hat ein Fenster 24, durch das eine am Gehäuse 22 markierte Millimeterskala 25 abgelesen werden kann. Dadurch ist eine Vorbestimmung der Länge der zu gewinnenden Gewebeproben möglich. Ein Klemmring 26 hält diese Einstellung fest.
Um die Gewebeprobe 7' sofort nach ihrem Ausschneiden in ein Fixiermittel einbringen zu können, ist ein Behälter 30 für eine Fixierflüssigkeit, z. B. eine übliche Injektionsspritze, in eine Ansatzöffnung 27 des hinteren Gehäuseteiles 31 einführbar und dort in beliebiger Weise festhaltbar. An diese Ansatzöffnung 27 ist eine Einspritznadel 28 angeschlossen, die sich in den beiden Hohlwellen 5, 16 bis in den Bereich des zylindrischen Messers 7 nach vorne erstreckt und durch die die Fixierflüssigkeit in den vom zylindrischen Messer 7 umschlossenen Raum gespritzt wird. Dichtungsringe 29 dichten die Einspritznadel 28 gegenüber dem Innenmantel der inneren Hohlwelle 16 ab und verhindern ein Zurückfliessen der Fixierflüssigkeit. Auf diese Weise wird der von den Messern ausgeschnittene Gewebezylinder sofort fixiert.
Das zylindrische Messer 7 hat an seinem Vorderende eine Schneide 32, die an einer nach einwärts springenden Stufe 35 sitzt, wobei die Formgebung so getroffen ist, dass der ausgeschnittene Gewebezylinder 7' einen kleineren Querschnitt als die innere lichte Weite des zylinderförmigen Messers 7 hat, so dass noch Raum für die Fixierflüssigkeit freigelassen ist und diese daher den gesamten ausgeschnittenen Gewebezylinder 7' umspülen kann.
Die Fixierflüssigkeit, die durch die Einspritznadel 28 zugeführt wird, kann auch dazu benutzt werden, den Gewebezylinder 7' aus dem zylindrischen Messer 7 wieder auszustossen.
For a histological examination of a piece of tissue, in particular for an electron-optical examination, the tissue sample removed must be as fresh as possible and therefore very quickly in a fixing liquid, e.g. B. formalin, alcohol, etc., are introduced so that the tissue structure is not changed by autolytic processes and the diagnosis is difficult or impossible. The object of the invention is to create a device which fully meets the requirements placed on it.
The invention is based on a biopsy removal device for non-traumatizing tissue removal for histological, in particular electron-optical, examination, with a rotating cylindrical knife and consists in the fact that, in order to cut the tissue cylinder cut out by the cylindrical knife, pivotable inside the cylindrical knife, around the axis of the cylindrical knife rotating knives are provided. By means of these swiveling knives, the tissue cylinder can be cut off in a simple and reliable manner without it being squeezed, so that the tissue cylinder obtained remains completely unchanged in its original tissue structure.
Further advantages of the device according to the invention result from the description of an exemplary embodiment which is shown schematically in the drawing in longitudinal section.
The biopsy removal device shown is used for non-traumatizing tissue removal, that is, for tissue removal in which the organ from which the tissue is to be removed is not destroyed, torn or squeezed.
An electric motor 1, which is enclosed in a pistol grip-like handle 3 of the device, which at the same time forms the motor housing, transmits its rotary motion via bevel gears 4, 4a to a hollow drive shaft 5. A flange 33 is connected to the front end of this hollow drive shaft 5, with which by means of a Bayonet lock 36 is connected to an exchangeable cutter head 6, in which a cylindrical cutter 7 is screwed in an exchangeable manner by means of a thread 37. The individual knives 7 or their cutter heads 6 have different calibres, so that tissue cylinders 7 'of any diameter can be cut out of the tissue 38. Specially constructed knives 7 are also expediently provided for removing tissue from a bone.
The number of revolutions of the electric motor 1 and the transmission ratio of the bevel gears 4, 4a are to be selected so that the number of revolutions of the knife 7 is below the combustion limit of the respective fabric cylinder 7 '. The electric motor 1 is switched on by a switch pin 8 after briefly pulling back a switch designed as a trigger 9 at the handle end of the device, the trigger 9 being held in its foremost position by a compression spring 10. A projection 11 forming a guide piston is firmly connected to the trigger 9, which is thus carried along to the rear when the trigger 9 is actuated and thereby carries along a guide disk 12 movably connected to it.
This guide disk 12 has a central opening through which the hollow drive shaft 5 passes and in which a radial pin 13 is arranged, which slides in an elongated hole 14 of the hollow drive shaft 5 so that the guide disk 12 and the pin 13 join in the rotary movement of the hollow drive shaft 5 . The pin 13 is also guided displaceably in an elongated hole 15 of a further hollow shaft 16 arranged coaxially in the hollow drive shaft 5, which elongated hole has a helically wound section 15 'at its rear end. As a result, the inner hollow shaft 16 is correspondingly rotated with respect to the hollow drive shaft 5 when the trigger 9 is withdrawn by the user's finger and the projection 11 together with the guide disk 12 is thereby shifted backwards.
The two hollow shafts 5, 16 are thus coupled rotatably together, but rotatable relative to one another by a small angle. The dashed lines show the switch tap 9 with the projection 11, the guide disk 12 with the pin 13 and the helically wound section 15 'of the elongated hole 15 in the rearmost position.
The inner hollow shaft 16 carries at the front a coupling disk 17 with recesses 17 ′, in which pins 18 ′ of a coupling counter disk 18, which is accommodated in the removable cutter head 6, engage. The coupling counter-disk 18 thus takes part in the rotation of the hollow shaft 16 with respect to the hollow drive shaft 5, with this relative rotation resulting in a swiveling in from the front end of the device at right angles to the axes 19 via axles 19 of the pivotable knives 20 that are loosely locked into the clutch counter disc 18 and which are bent at an angle attached inner cutting blades 20 results.
In the open state, these cutting knives 20 rest on the inside of the wall of the outer cylindrical knife 7 and, when the trigger 9 is actuated, come together like the lamellae of an iris diaphragm of a photographic shutter so that they touch or overlap. The axes 19 of the knives 20 are guided in longitudinal grooves 21 of the cylindrical knife 7.
To adjust the depth of penetration of the knife 7 into the tissue 38 is used a cylinder 23 surrounding the front end of the device, the front end carries a limit stop 34. The cylinder 23 is displaceable with its tubular part on the front end of the device and has a window 24 through the one on the housing 22 marked millimeter scale 25 can be read. This enables the length of the tissue samples to be obtained to be determined in advance. A clamping ring 26 holds this setting in place.
In order to be able to insert the tissue sample 7 'into a fixing agent immediately after it has been cut out, a container 30 for a fixing liquid, e.g. B. a conventional injection syringe, insertable into a neck opening 27 of the rear housing part 31 and held there in any way. An injection needle 28 is connected to this attachment opening 27, which extends forward in the two hollow shafts 5, 16 into the area of the cylindrical knife 7 and through which the fixing liquid is injected into the space enclosed by the cylindrical knife 7. Sealing rings 29 seal the injection needle 28 from the inner jacket of the inner hollow shaft 16 and prevent the fixing liquid from flowing back. In this way, the cylinder of tissue cut out by the knives is fixed immediately.
The cylindrical knife 7 has a cutting edge 32 at its front end, which is seated on an inwardly jumping step 35, the shape being such that the cut-out tissue cylinder 7 'has a smaller cross section than the inner clear width of the cylindrical knife 7, see above that there is still space left for the fixing liquid and this can therefore wash around the entire cut-out tissue cylinder 7 '.
The fixing liquid, which is supplied through the injection needle 28, can also be used to eject the tissue cylinder 7 ′ from the cylindrical knife 7 again.
