Abstract
Gynaecological apparatus for placing in the uterus of a woman comprises a Y-shaped flexible support organ, each arm of which is connected with a fan-shaped membrane unfolded in the half of the space between the arms so that the free edges of the membrane together define a radial split. The apparatus consists of a plastic material such as polyethylene. The apparatus is intended for use in operations, and as an anticonception device.
Description
Dispositif gynécologique destiné à être introduit dans l'utérus
Le brevet principal a pour objet un dispositif gynécologique destiné à être introduit dans l'utérus de la femme comprenant un élément de support souple en forme de Y dont chacune des branches est solidaire d'une membrane déployée en éventail dans au moins une partie de l'espace qui les sépare.
La titulaire a trouvé que dans certains cas il y avait avantage à ce que le tronc du Y soit constitué au moins en partie par un fil en matière plastique synthétique. En effet des cas d'irritation du col de l'utérus par le tronc du Y ou les traverses dont il était muni ont été constatés chez quelques patientes particulièrement délicates.
L'invention a pour objet un dispositif gynécologique selon la revendication du brevet principal, caractérisé en ce que le tronc du Y est constitué au moins en partie par un fil.
Le dessin annexé représente à titre d'exemple une forme d'exécution du dispositif objet de l'invention.
La fig. 1 en est une vue en élévation de face.
Les fig. 2 et 3 représentent en élévation avec arrachement partiel les éléments d'un instrument pour l'introduction du dispositif de la fig. 1 dans l'utérus de la femme.
Le dispositif représenté à la fig. 1 comprend un élément de support souple 1 en forme de Y dont chacune des branches 2 et 3 est solidaire de l'un des bords d'une membrane, 8 et 9 respectivement, déployée chacune dans un même plan dans la moitié de l'espace entre lesdites branches 2 et 3 dudit élément de support 1. Ces membranes 8 et 9 sont munies chacune, le long de leur bord libre en regard. d'un bourrelet raidisseur 10 délimitant une fente 11 coaxiale du tronc 5 de l'élément 1 en forme de Y. Le tronc 5 du Y est constitué essentiellement par un fil en nylon 16 noué dans un orifice 6 que présente la partie supérieure dudit tronc 5 au voisinage de la jonction des branches 2 et 3 du Y.
La mise en place par voie vaginale du dispositif représenté à la fig. 1 étant rendue difficile du fait que le tronc 5 du Y est constitué essentiellement par un fil souple, un instrument d'introduction de ce dernier dans le col de l'utérus de la patiente (voir fig 2 et 3) a été conçu. Cet instrument comprend une pièce de forme tubulaire 17 dans laquelle peut coulisser une tige pleine 18 présentant à l'une de ses extrémités un organe d'arrêt 19 et à son autre extrémité un orifice 20.
La préparation pour la mise en place et la mise en place du dispositif représenté à la fig. 1 s'effectuent de préférence de la façon suivante:
On introduit le fil de nylon double 16 noué à l'orifice 6 du dispositif dans le tube 17 de haut en bas de la fig. 3 jusqu'à ce qu'ils en ressortent par son extrémité 21.
On noue ensuite l'extrémité libre dudit fil double 16 à l'orifice 20 de la tige 18 que l'on introduit alors par l'orifice 21 dans le tube 17 jusqu'à la hauteur de l'organe d'arrêt 22. L'ensemble constitué par le dispositif de la fig. 1 et l'injecteur des fig. 2 et 3 ainsi assemblés est alors soumis à la stérilisation, éventuellement dans un sac en matière plastique scellé par exemple, et est ensuite prêt à l'emploi.
Lors de la mise en place le médecin retire l'ensemble susmentionné de son enveloppe dans des conditions aseptiques, rabat l'une contre l'autre les branches 2 et 3 du support souple 1 et enroule les membranes 8 et 9 autour desdites branches 2 et 3. I1 introduit alors le dispositif dans l'extrémité 23 du tube 17 par son extrémité munie de l'orifice 6.
L'instrument 17-18 et le dispositif gynécologique qui y est logé est alors introduit dans le vagin et dans le col de l'utérus de la patiente par l'extrémité 23 de la partie 17 de l'instrument de mise en place. La pénétration de cet instrument dans le col de l'utérus est limitée par l'organe d'arrêt 22 qui peut éventuellement prendre appui sur le bord externe du cervix. On enfonce alors la tige 18 progressivement dans le tube 17 tout en retirant progressivement ce dernier du col de l'utérus jus
qu'à ce que le dispositif gynécologique en soit expulsé
et se déploie dans l'utérus. L'instrument 17-18 est alors
retiré entièrement du col de l'utérus et du vagin et le fil
16 coupé à la longueur désirée. On laissera dépasser une
longueur de fil suffisante du col de l'utérus dans le vagin
pour permettre une extraction subséquente du dispositif
par voie vaginale.

The licensee found that in certain cases it was advantageous for the trunk of the Y to be made at least in part by a synthetic plastic thread. In fact, cases of irritation of the cervix by the trunk of the Y or the crossbars with which it was fitted have been observed in some particularly delicate patients.
The subject of the invention is a gynecological device according to the claim of the main patent, characterized in that the trunk of the Y is formed at least in part by a thread.
The appended drawing represents by way of example an embodiment of the device which is the subject of the invention.
Fig. 1 is a front elevational view thereof.
Figs. 2 and 3 show in elevation with partial cut away the elements of an instrument for the introduction of the device of FIG. 1 in the woman's womb.
The device shown in FIG. 1 comprises a flexible support element 1 in the form of a Y, each of the branches 2 and 3 of which is integral with one of the edges of a membrane, 8 and 9 respectively, each deployed in the same plane in half the space between said branches 2 and 3 of said support element 1. These membranes 8 and 9 are each provided along their facing free edge. a stiffening bead 10 delimiting a slot 11 coaxial with the trunk 5 of the Y-shaped element 1. The trunk 5 of the Y consists essentially of a nylon thread 16 tied in an orifice 6 which the upper part of said trunk has 5 near the junction of branches 2 and 3 of the Y.
The vaginal placement of the device shown in FIG. 1 being made difficult by the fact that the trunk 5 of the Y consists essentially of a flexible thread, an instrument for introducing the latter into the cervix of the patient (see FIGS. 2 and 3) has been designed. This instrument comprises a tubular-shaped part 17 in which a solid rod 18 can slide, having at one of its ends a stop member 19 and at its other end an orifice 20.
The preparation for the installation and the installation of the device shown in FIG. 1 are preferably carried out as follows:
The double nylon thread 16 tied at the orifice 6 of the device is introduced into the tube 17 from top to bottom in FIG. 3 until they come out through its end 21.
The free end of said double wire 16 is then tied to the orifice 20 of the rod 18 which is then introduced through the orifice 21 into the tube 17 up to the height of the stopper 22. L 'assembly constituted by the device of FIG. 1 and the injector of FIGS. 2 and 3 thus assembled is then subjected to sterilization, possibly in a sealed plastic bag for example, and is then ready for use.
During the installation, the doctor removes the aforementioned assembly from its casing under aseptic conditions, folds the branches 2 and 3 of the flexible support 1 against each other and wraps the membranes 8 and 9 around said branches 2 and 3. I1 then introduces the device into the end 23 of the tube 17 through its end provided with the orifice 6.
The instrument 17-18 and the gynecological device housed therein is then introduced into the vagina and the cervix of the patient through the end 23 of the part 17 of the delivery instrument. The penetration of this instrument into the cervix is limited by the stop member 22 which may possibly be supported on the outer edge of the cervix. We then push the rod 18 gradually into the tube 17 while gradually withdrawing the latter from the cervix juice

and deploys in the uterus. Instrument 17-18 is then
removed entirely from the cervix and vagina and the thread
16 cut to desired length. We will let a
sufficient length of thread from the cervix into the vagina
to allow subsequent extraction of the device
vaginally.
