Instrument zur Erleichterung von Gebärmutteruntersuchungen und zur Durchführung von Einspritzungen in dieselbe
Bei Durchführung von Untersuchungen der Gebärmutter und Eileiter verwendet man je nach den Verhältnissen Flüssigkeit- oder Gaseinspritzungen als Hilfsmittel, welche Einspritzungen durch den Gebärmuttermund geschehen. Bei Röntgenuntersuchungen geschehen beispielsweise Voreinspritzungen von Kon trastflüssigkeiten und bei Operationen Nacheinspritzungen von Spülflüssigkeiten. Gaseinspritzungen, beispielsweise mit Kohlensäure, geschehen zur Feststellung, ob die Eileiter freien Durchgang aufweisen.
Es ist dabei wichtig, dass der Gebärmuttermund in unveränderter Lage festgehalten wird, so dass man ohne Schwierigkeit die Öffnung des Gebärmuttermundes zur Einführung der Einspritzkanüie finden kann; dies ist bisher mittels Greifzangen zustande gebracht worden. Die Greifzangen verursachen indessen nicht selten Schäden und Verletzungen an den empfindlichen Teilen, ferner verursachen sie Schwierigkeiten bezüglich freier Durchsicht zum Gebärmuttermund und durch denselben hindurch.
Diesen Ungelegenheiten weicht man durch die vorliegende Erfindung aus, die sich auf ein Instrument zur Erleichterung von Gebärmutteruntersuchungen und zur Durchführung von Einspritzungen in diese bezieht, welches Instrument dadurch gekennzeichnet ist, dass es aus einer Saugglocke zur Umfassung und zum Festhalten des Gebärmuttermundes besteht, die mit einem in Axialrichtung verstellbaren Spreizorgan für den letzteren zusammenwirkt, wobei sowohl die Saugglocke als das Spreizorgan in ihrer Wirkungslage zusammen mit dem Gebärmuttermund Dichtungen gegen das Eindringen von Luft durch die dem Gebärmuttermund zugewandte Saugöffnung der Saugglocke bilden.
In beigefügter Zeichnung ist eine Ausführungsform des erfindungsgemässen Instrumentes veranschaulicht, wobei
Fig. 1 einen Längsschnitt des Instrumentes in Lage gegen den Gebärmuttermund zeigt, sowie
Fig. 2 eine entsprechende Ansicht des Instrumentes in Wirkungslage mit eingesaugtem Gebärmuttermund.
Das Instrument enthält ein Kanülrohr 1, das so mit Spielraum von einem Aussenrohr 2 umgeben ist, dass ein Durchlaufkanal 3 zwischen ihnen vorhanden ist. Das Aussenrohr 2 ist im vorderen Teil 4a eines ungefähr an seiner Mitte quergeteiken Handgriff es befestigt, dessen hinterer Teil 4b mittels eines gewindegeschnittenen Endzapfens 5 in den vorderen Handgriffs teil einschraubbar ist zwecks Andrücken gegen einen um das den Handgriff durchziehende Kanul- rohr 1 herum verlegten, elastischen Dichtungsring 6.
Dieser Dichtungsring, der gegen den Boden einer dem Endzapfen 5 entsprechenden Ausnehmung im vorderen Handgriffsteil anliegt, verhindert beim Anpressen Lufteindringungen längs des Kanülrohres in das weiter vorn im Handgriff liegende Hinterende des Aussenrohres und hält dabei auch das Kanülrohr mit gewisser Reibungskraft in eingestellter Lage.
Im vorderen Handgriffsteil reicht ein Querrohr 7 vom Aussenrohr hinaus zum Anschluss an eine nicht gezeigte Vakuumpumpe oder dergleichen zum Aussaugen der Luft aus der Saugglocke. Das Aussenrohr hat an seinem Vorderende eine mit Flansch versehen, innen gewindegeschnittene Muffe 8, deren Endflansch gegen eine Gummipackung 9 oder dergleichen anliegt, die ihrerseits gegen die Aussenseite der Saugglocke anliegt.
In die Endmuffe ist von innen aus der Saugglocke heraus ein flanschversehener Nippel 10 eingeschraubt, durch welchen das Kanülrohr mit Spielraum in die Saugglocke hineingeht, welches Kanülrohr an seinem hierliegenden Ende einen Endkopf 11 beispielsweise in Form eines Kegels trägt, wie die Fig. I andeutet. Der Nippel ist mittels passendem Schlüssel einschraubbar, und sein Flansch drückt eine Ringscheibe gegen die Innenseite der Saugglocke. Das Aussenrohr wird hierdurch lösbar an der Saugglocke festgehalten, während es mittels der erwähnten Gummipackung dichtet, und hat mit dem Innern der Saugglocke Verbindung durch den Spielraum im Nippel.
Die zweckmässig kreisrunde Saugöffnung 12 der Saugglocke hat die Öffnungskante übergehend in eine Wandpartie der Saugglocke, die wesentlich nach der Seite und aussen gerichtet ist, und mit der Verlängerung der Saugöffnungsebene einen sehr kleinen Winkel bildet. Die Saugglocke erhält hierdurch eine auf die Saugöffnung bezogen, stark nach der Seite und nach aussen gerichtete Ausbuchtung 13. Die Saugglocke ist mit Vorteil ganz durchsichtig, beispielsweise aus Kunststoff, ausgeführt.
Der hintere Handgriffsteil 4b hat im Aussenende eine langgestreckte axiale Ausbohrung 14, und es ist, falls als zweckmässig befunden, auch hier ein elastischer Dichtungsring 15 am Boden der Ausbohrung um das Kanülrohr 1 herumgelegt. In die Ausbohrung ist ein in seiner Länge der Länge der Ausbohrung entsprechender Kanülrohrhalter 16 eingeschraubt, der an seinem Aussenende einen Kopf 17 hat, der als Handrad zum Ein- und Ausschrauben des Kanülrohrhalters in den hinteren Handgriffsteil 4b dient. Auch dieser Halter hat, vom Aussenende an gerechnet, eine axiale Ausbohrung, in welche ein Anschlussrohr 18 eingeschraubt und so lang ist, dass es normal bis zum Boden der Ausbohrung reicht, um hier durch Anpressen eines weiteren um das Kanülrohr herumgelegten elastischen Dichtungsringes 19 einen Reibungsschluss des Kanülrohres zustande zu bringen.
Dieses reicht dabei ein Stück in das Anschlussrohr 18 hinein, mit seinem Aussenende im Anschlag gegen einen Absatz 20 darin. Das Anschlussrohr 18 kann irgendein beliebiges äusseres Kupplungsende für Gas- oder Flüssigkeitsrohranschluss haben. In der Zeichnung ist ein Kupplungsteil 21 für eine Spritze angedeutet. Der Kanülrohrhalter 16 hat in seinem Aussengewinde drei Einpressmarken A, B, C, welche im Anschluss an die äussere Endkante des hinteren Handgriffs teiles 4b beim Einschrauben des Kanülrohrhalters die Lagen des Ka nülendkopfes 11 innerhalb der Saugglocke ausweisen, mit A die Bodeniage des Spreizkopfes zeigend, mit C dessen maximale Aussenlage sowie mit B die Mittellage zwischen ihnen anzeigend.
Bei der Anwendung des Instrumentes wird die Saugglocke mit dem Endkopf des darin befindlichen Kanülrohres gegen den Gebärmuttermund geführt, so dass die Saugglockenöffnung den Ringmuskel 25 des Gebärmuttermundes umgibt, und das vordere Ende des Spreizkopfes wird in den Gebärmuttermund hin einorientiert, wie es in Fig. 1 gezeigt ist. Dies wird durch die Durchsichtigkeit der Saugglocke erleichtert. Der Spreizkopf befindet sich dabei in Bodenlage mit ausgeschraubtem Kanülrohrhalter 16, so dass die Marke A gerade eben ausserhalb der äusseren Endkante des hinteren Handgriffteiles 4b liegt. In der Saugglocke wird danach Vakuum durch das Ansaugen der Innenluft durch das Aussenrohr mittels der dazu angeschlossenen Vakuumpumpe erzeugt.
Zufolge des Vakuums wird der Ringmuskel durch die Saugglockenöffnung zwischen deren Ringkante und dem Spreizkopf des Kanülrohres eingesaugt. Wie in Fig. 2 gezeigt, wird dies eine Ausbuchtung des Ringmuskds 25 in Seitenrichtung innerhalb der Saugglocke mit sich führen, und ein Eingleiten längs des Spreizkopfes, so dass die Saugglocke und der Spreizkopf Dichtungsflächen gegen den eingesaugten Muskel bilden werden, und ein starkes Festhalten der Saugglocke wird erhalten.
Während dieser Zeit und danach wird der Spreizkopf des Kanülrohres in den Gebärmuttermund hineingeschoben, indem man den Kanülrohrhalter 16 mit dem Handgriff hineinschraubt, bei welcher Bewegung der innere Absatz 20 des Anschlussrohres 18 das Kanülrohr vorwärts schiebt, und diese Grösse der Einschiebung wird an den Verschiebungslagen der Einpressmarken B und C beurteilt. Der Gebärmuttermund wird dabei so geöffnet, dass ein unbehindertes Einspritzen von Flüssigkeit oder Gas stattfinden kann. Bei grösseren Einschiebungen kann der Gebärmuttermund auch zum Umlegen um die Basis des Spreizkopfes herum geformt werden, wie in Fig. 2 angedeutet, mit weiter verbesserten Dichtverhältnissen als Folge.
Soll der Spreizkopf des Kanülrohres zurückgezogen oder rückwärts justiert werden, so wird der hintere Handgriffsteil gelöst, so dass die Spannung des Dichtungsringes 6 sich vermindert. Wenn danach der Kanülrohrhalter 16 nach aussen geschraubt wird, folgt das Kanülrohr dieser Bewegung wegen der Reibungswirkung des Dichtungsringes 19.
Die oben beschriebene Vorrichtung stellt ein einfaches und wirksames Instrument dar, das den Gebärmuttermund ohne Schmerzen und Schadenwirkungen festhält.
Je nach den vorhandenen Verhältnissen werden Spreizköpfe in verschiedenen Grössen erfordert. Der Spreizkopf kann auch eine andere Form haben, beispielsweise abgeplattete Form, Kugelform oder dergleichen. Die Ausführung des Instrumentes in der beschriebenen Ausführungsform ist so, dass das Kanülrohr mit samt dem Spreizkopf in einfachster Weise ausgetauscht werden kann, weil das Kanülrohr durch die Saugglocke herausgezogen werden kann und danach ein Kanülrohr mit einem anderen Spreizkopf unmittelbar durch genannte Saugglocke in das Instrument eingeschoben werden kann. Das Austauschen der Spreizköpfe kann somit ohne Hilfe von Werkzeugen und ohne nennenswerten Zeitverlust zustande gebracht werden.
Die beschriebene Handgriffsausführung mit der Verschiebungsanordnung des Kanülrohres kann durch eine andere ersetzt sein. Das Kanülrohr mit dem Spreizkopf kann durch ein anderes Einschiebeorgan ersetzt werden, das den Gebärmuttermund auch für andere Zwecke öffnet, beispielsweise für eine Ausleuchtung, in welchem Fall die Durchsichtigkeit der Saugglocke auch von Bedeutung ist.
Instrument for facilitating uterine examinations and for making injections into the same
When carrying out examinations of the uterus and fallopian tubes, depending on the circumstances, injections of liquid or gas are used as an aid, which injections are made through the cervix. In X-ray examinations, for example, there are pre-injections of contrast fluids and post-injections of rinsing fluids in operations. Gas injections, for example with carbon dioxide, are used to determine whether the fallopian tubes are free.
It is important that the cervix is held in an unchanged position so that the opening of the cervix for the introduction of the injection cannula can be found without difficulty; this has so far been achieved using grippers. The grasping forceps, however, not infrequently cause damage and injuries to the sensitive parts, and they also cause difficulties with regard to a clear view of the cervix and through it.
These inconveniences are avoided by the present invention, which relates to an instrument for facilitating uterine examinations and for carrying out injections into them, which instrument is characterized in that it consists of a suction cup for enclosing and holding the cervix, which with an axially adjustable expansion element for the latter cooperates, with both the suction cup and the expansion element in their operative position together with the cervix forming seals against the ingress of air through the suction opening of the suction cup facing the cervix.

The instrument contains a cannula tube 1 which is surrounded by an outer tube 2 with enough space so that a flow channel 3 is present between them. The outer tube 2 is fastened in the front part 4a of a handle that is transverse to its center approximately, the rear part 4b of which can be screwed into the front handle part by means of a thread-cut end pin 5 for the purpose of pressing it against a cannula tube 1 that passes through the handle, elastic sealing ring 6.
This sealing ring, which rests against the bottom of a recess corresponding to the end pin 5 in the front handle part, prevents air penetration along the cannula tube into the rear end of the outer tube, which is further forward in the handle, and also holds the cannula tube in the set position with a certain frictional force.
In the front handle part, a cross tube 7 extends from the outer tube for connection to a vacuum pump (not shown) or the like for sucking the air out of the suction bell. At its front end, the outer pipe has a flange 8, internally threaded, the end flange of which rests against a rubber packing 9 or the like, which in turn rests against the outside of the suction cup.
A flange-provided nipple 10 is screwed into the end sleeve from the inside of the suction bell, through which the cannula tube goes into the suction bell with clearance, which cannula tube carries an end head 11, for example in the form of a cone, at its end here, as FIG. The nipple can be screwed in using a suitable key, and its flange presses an annular disk against the inside of the suction bell. The outer tube is thereby releasably held on the suction cup, while it seals by means of the rubber packing mentioned, and is connected to the inside of the suction cup through the clearance in the nipple.
The suitably circular suction opening 12 of the suction bell has the opening edge merging into a wall section of the suction bell which is directed substantially to the side and outward and forms a very small angle with the extension of the suction opening plane. As a result, the suction bell is provided with a bulge 13 that is directed strongly to the side and outward in relation to the suction opening. The suction bell is advantageously made completely transparent, for example made of plastic.
The rear handle part 4b has an elongated axial bore 14 in the outer end and, if found to be expedient, an elastic sealing ring 15 is placed around the cannula tube 1 at the bottom of the bore. A cannula tube holder 16 corresponding in its length to the length of the bore is screwed into the bore and has a head 17 at its outer end which serves as a handwheel for screwing and unscrewing the cannula tube holder in the rear handle part 4b. This holder, calculated from the outer end, also has an axial bore into which a connection tube 18 is screwed and is so long that it normally extends to the bottom of the bore to create a frictional connection by pressing another elastic sealing ring 19 around the cannula tube to bring about the cannula tube.
This extends a piece into the connecting pipe 18, with its outer end abutting against a shoulder 20 therein. The connecting pipe 18 can have any desired outer coupling end for gas or liquid pipe connection. In the drawing, a coupling part 21 for a syringe is indicated. The cannula tube holder 16 has three press-in marks A, B, C in its external thread, which, following the outer end edge of the rear handle part 4b when screwing in the cannula tube holder, identify the positions of the cannula end head 11 within the suction bell, with A showing the bottom position of the expanding head, with C its maximum outer position and with B the middle position between them.
When using the instrument, the suction bell with the end head of the cannula tube located in it is guided against the cervix so that the suction bell opening surrounds the circular muscle 25 of the cervix, and the front end of the expanding head is oriented into the cervix, as shown in FIG. 1 is shown. This is made easier by the transparency of the suction cup. The spreader head is in the bottom position with the cannula tube holder 16 screwed out, so that the mark A is just outside the outer end edge of the rear handle part 4b. A vacuum is then generated in the suction bell by sucking in the internal air through the external tube by means of the vacuum pump connected to it.
As a result of the vacuum, the sphincter muscle is sucked in through the suction bell opening between its annular edge and the expanding head of the cannula tube. As shown in Fig. 2, this will result in a bulge of the sphincter 25 laterally within the suction cup, and a sliding along the expansion head, so that the suction cup and expansion head will form sealing surfaces against the sucked in muscle, and a strong hold of the Suction bell is obtained.
During this time and afterwards, the expanding head of the cannula tube is pushed into the cervix by screwing in the cannula tube holder 16 with the handle, during which movement the inner shoulder 20 of the connecting tube 18 pushes the cannula tube forward, and this amount of insertion is based on the displacement positions of the Press-in marks B and C assessed. The cervix is opened in such a way that liquid or gas can be injected unhindered. In the case of larger indentations, the cervix can also be shaped to fold around the base of the expanding head, as indicated in FIG. 2, with further improved sealing conditions as a result.
If the expanding head of the cannula tube is to be withdrawn or adjusted backwards, the rear part of the handle is released so that the tension on the sealing ring 6 is reduced. When the cannula tube holder 16 is then screwed outwards, the cannula tube follows this movement because of the frictional effect of the sealing ring 19.
The device described above is a simple and effective instrument that holds the cervix in place without pain or harm.
Depending on the existing conditions, expanding heads of different sizes are required. The expansion head can also have a different shape, for example flattened shape, spherical shape or the like. The design of the instrument in the embodiment described is such that the cannula tube together with the expansion head can be exchanged in the simplest possible way because the cannula tube can be pulled out through the suction bell and then a cannula tube with another expansion head is inserted directly into the instrument through said suction bell can be. The exchange of the expansion heads can thus be accomplished without the aid of tools and without any significant loss of time.
The handle design described with the displacement arrangement of the cannula tube can be replaced by another. The cannula tube with the expanding head can be replaced by another insertion element that opens the cervix for other purposes, for example for illumination, in which case the transparency of the suction cup is also important.
