Einrichtung zur Erzeugung von Koagulationen, insbesondere im Fundus des Auges
Bei Ablösungen der Netzhaut des Auges bzw. bei Netzhautrissen kann gemäss einer bekannten Behandlungsmethode eine Verschweissung zwischen Netzhaut und Aderhaut des Auges dadurch erzielt werden, dass die Netzhaut an der betreffenden Stelle durch Lichteinwirkung koaguliert wird. Zu diesem Zweck wird eine Lichtquelle hoher Strahlungsdichte auf dem Fundus des Auges abgebildet. Dabei kommt es vor allem im Pigmentepithel und in der Aderhaut zu einer starken Absorption von Strahlungsenergie, die zum grossen Teil in Wärme verwandelt wird. Die im Gewebe an der Auftreffstelle der Lichtstrahlen entstehende Wärme wird durch Leitung und Konvektion in die Umgebung abtransportiert.
Vor allem die Aderhaut hat infolge ihres Gefässreichtums ein gutes Wärmeleitungsvermögen. Übersteigt jedoch die Wärmezufuhr die Wärmeableitung in der Zeiteinheit, so kommt es im bestrahlten Gewebe zu einer fortlaufenden Steigerung der Temperatur, die schliesslich zur Gerinnung des Gewebeeiweisses, das heisst zur Koagulation führt. Im Verlauf von 1 bis 2 Wochen flacht der entstehende Koagulationsherd ab und verwandelt sich schliesslich in eine Narbe, innerhalb deren Netzhaut und Aderhaut eine gemeinsame mit Pigment durchsetzte Membran bilden.
Eine bekannte Einrichtung zur Durchführung dieser Behandlungsmethode enthält eine Lichtquelle hoher Strahlungsdichte, einen Kondensor zur Abbildung der Lichtquelle in eine Blende, eine Linse zur Erzeugung eines parallelen Strahlenganges hinter dieser Blende sowie einen in Strahlrichtung gesehen hinter dieser Linse angeordneten Spiegel, mittels dessen die Lichtstrahlen auf das Auge des Patienten gerichtet werden. Durch die Augenlinse werden die auftreffenden parallelen Lichtstrahlen auf dem Fundus vereinigt und können so an dieser Stelle bei genügender Strahlungsdichte der Lichtquelle eine Koagulation hervorrufen. Um die Behandlung unter ständiger Kontrolle des Arztes durchführen zu können, ist der erwähnte Spiegel mit einer zentralen Durchbohrung versehen, durch welche der Arzt das Patientenauge beobachtet.
Bei diesem bekannten Gerät sind die genannten optischen Elemente in einem Gehäuse angeordnet, welches auf Rädern bewegbar ist. Dies bedingt den Nachteil, dass das Gerät nur sehr schwer zu bewegen ist und dass dadurch eine Einstellung auf die zu behandelnde Stelle der Netzhaut ausserordentlich erschwert wird.
Dieser Nachteil wird gemäss der Erfindung dadurch vermieden, dass die genannten optischen Elemente sowie ein hinter der Lichtquelle angebrachter Hohlspiegel auf einem gemeinsamen Träger angeordnet sind, welcher im Schwerpunkt um seine Hochund Querachse frei drehbar und in Längsrichtung verschiebbar in einem Gehäuse aufgehängt ist. Dadurch wird gewährleistet, dass der die optischen Elemente enthaltende Träger um die Hoch- und Querachse gedreht sowie in Längsrichtung verschoben werden kann, ohne dass irgendwelche Gewichtsverlagerungen auftreten. Dies bedeutet, dass bei entsprechender Ausbildung der Lager eine ausserordentlich feingängige Bewegung des Trägers vorgenommen werden kann und dass der behandelnde Arzt das Gerät ohne Mühe in die gewünschte Stellung bringen kann.
Es ist dabei vorteilhaft, die optischen Elemente in einem Rohr anzuordnen, das in der Umgebung seines Schwerpunktes von einem weiteren koaxialen Rohr umgeben und in diesem um die Querachse drehbar gelagert ist, wobei dieses zweite Rohr um die Hochachse drehbar in einer Gabel aufgehängt ist, welche ihrerseits um eine zur Hochachse des Rohres parallele Achse drehbar ist.
Im folgenden wird anhand der Fig. 1 bis 4 ein Ausführungsbeispiel der erfindungsgemässen Einrichtung näher erläutert. Dabei zeigen:

In Fig. 1 ist mit 1 ein Gehäuse bezeichnet, welches auf einem weiteren Gehäuse 2 angeordnet ist, das alle zur Spannungsversorgung des Gerätes notwendigen elektrischen Teile enthält. Im Gehäuse 1 ist ein die optischen Teile des Gerätes enthaltendes Rohr 9 gelagert. Mit diesem Rohr 9 ist zugleich eine Lichtquelle 3 verbunden, deren Spannungsversorgung aus dem Gehäuse 2 entnommen wird. Ein Zündgerät 6 für die Lampe 3, welche beispielsweise zwei in einem Gas hohen Druckes angeordnete Elektroden enthält, ist getrennt vom Rohr 9 im Gehäuse 1 angeordnet. Mit dem Rohr 9 ist ferner ein Träger 26 verbunden, welcher einen Hohlspiegel 4 und ein Gegengewicht 5 trägt.
Die optischen Teile des Gerätes sind in Fig. 2 dargestellt. Vor der Lichtquelle 3 befindet sich eine Punktblende 7 sowie eine Siebblende 8. Die Lichtquelle 3 wird mittels eines Kondensors 27 in der Ebene einer Blende 28 abgebildet. Die Blende 28 ist dabei zweckmässig so gestaltet, dass ihr Öffnungsdurchmesser variiert werden kann. Das in der Ebene der Blende 28 entstehende Zwischenbild wird mittels einer Linse 29 ins Unendliche abgebildet. Das von der Linse 29 ausgehende Parallellichtbündel wird von einem Spiegel 10 in das Auge 30 eines Patienten reflektiert. Durch die Augenlinse 31 wird dabei das eintretende Licht auf die Netzhaut des Auges fokussiert. An dieser Stelle können Koagulationen hervorgerufen werden, wenn das gesamte von der Lichtquelle 3 ausgehende Licht in das Auge 30 des Patienten gelangt.
Zu diesem Zweck ist es erforderlich, während des eigentlichen Behandlungsvorganges die Siebblende 8 aus dem Strahlengang auszuschwenken. Die Siebblende 8 hat lediglich die Aufgabe, während des Absuchens der Netzhaut und der Einstellung auf die zu behandelnde Stelle, das von der Lichtquelle 3 ausgehende Licht so zu schwächen, dass keine Schädigungen des Auges 30 auftreten können.
Die peripheren Löcher der Siebblende 8 werden mit der Austrittspupille des Gerätes in der Pupille des bestrahlten Auges 30 abgebildet und sind hier als leuchtende Strahlenbündel sichtbar. Dadurch wird ein genaues Zentrieren des Strahlenganges in der Patientenpupille erleichtert. Um die Beobachtung des Patientenauges 30 ermöglichen zu können, ist der Spiegel 10 mit einer zentralen Durchbohrung 11 versehen. Durch diese Durchbohrung hindurch kann der behandelnde Arzt, dessen Auge mit 32 bezeichnet ist, während des gesamten Behandlungsvorganges das Patientenauge 30 beobachten.
Um während des Behandlungsvorganges, das heisst also in dem Zeitraum, in welchem das volle Licht der Lichtquelle 3 in das Patientenauge fällt und die Siebblende 8 aus dem Strahlengang ausgeschwenkt ist, Blendungen des Arztes zu vermeiden, ist ein Graufilter 33 vorgesehen, welcher in diesem Zeitraum automatisch vor die zentrale Durchbohrung 11 des Spiegels 10 geschoben wird.
Wie aus Fig. 1 hervorgeht, ist der Spiegel 10 zugleich an einem Handgriff 12 und an dem Rohr 9 befestigt. Zum Einstellen kann der Spiegel 10 zusammen mit dem Handgriff 12 um die Achse des Rohres 9 gedreht werden. Ebenso ist es möglich, den Spiegel 10 um die Achse des Handgriffes 12 zu drehen.
Ist das Gerät auf die richtige Stelle des Auges eingestellt, so drückt der Arzt auf den Knopf 13. Dadurch wird gleichzeitig das Graufilter 33 vor die zentrale Durchbohrung 11 des Spiegels 10 geschoben und automatisch die Siebblende 8 aus dem Strahlengang ausgeschwenkt. Nach Loslassen des Knopfes 13 schiebt sich das Graufilter 33 zurück und die Siebblende 8 wird wieder eingeschwenkt. Es hat sich dabei als zweckmässig erwiesen, zum Ein- und Ausschwenken der Siebblende 8 einen Elektromagneten zu verwenden, der über einen mittels des Knopfes 13 betätigten Schalter erregt wird.
Zum Einstellen des Gerätes ist es jedoch nicht nur notwendig, den Spiegel 10 zu verdrehen, sondern es ist ebenso notwendig, das Rohr 9 zu verschieben. Zu diesem Zweck kann es erforderlich sein, das Rohr 9 in Längsrichtung zu bewegen und gleichzeitig nach Höhe und Seite zu verschwenken. Zur Ermöglichung dieser Bewegungen dient die nachfolgend beschriebene Lagerung des Rohres 9.
Wie aus Fig. 3 hervorgeht, ist das Rohr 9 in der Umgebung seines Schwerpunktes, welcher durch Verschieben des Gegengewichtes 5 in axialer Richtung verlagert werden kann, von einem koaxialen Rohr 33 umgeben. Das Rohr 9 ist dabei mittels der Zapfen 34 und 35 im Rohr 33 um die Querachse verschwenkbar gelagert. Der radiale Abstand zwischen dem Innendurchmesser des Aussenrohres 33 und dem Aussendurchmesser des Rohres 9 sowie die Länge des Rohres 33 ist so bemessen, dass das Rohr 9 nur um einen vorgegebenen Winkel um die Zapfen 34 und 35 verschwenkt werden kann.
Das Rohr 33 ist in einer Gabel 38 aufgehängt und in dieser mittels der Zapfen 36 und 37 um die Hochachse verschwenkbar. Die Gabel 38 ihrerseits ist mittels der Lager 39 und 40 so auf einem Träger 14 gelagert, dass sie um die Achse dieses Trägers verschwenkt werden kann.
Durch diese Lagerung ist es möglich, das Rohr 9 nach der Höhe und nach der Seite zu verschwenken und ebenso in Längsrichtung zu verschieben. Um auch für die Bewegungen des Rohres 9 nach der Seite so wie für die Längsbewegungen einen Anschlag zu schaffen, ist das Rohr 33 mit einem Stift 15 versehen, welcher in eine Führungskurve 16 eingreift. Diese Führungskurve ist in ein Blech 50 eingeschnitten, welches am Träger 14 befestigt ist. Die Form der seitlichen Begrenzung der Führungskurve ist so gewählt, dass sich beim Gleiten des Stiftes 15 an dieser Kurve eine Bewegung des Rohres 9 in genau axialer Richtung ergibt. Die vordere Begrenzung der Führungskurve wird durch einen Hebel 17 gebildet, welcher um eine Achse 43 verschwenkbar ist. Dieser Hebel 17 wird mittels einer Feder 41 in Ruhestellung gehalten.
Die Spannung der Feder 41 ist dabei so gewählt, dass normalerweise durch Anschlagen des Stiftes 15 am Hebel 17 keine Drehung desselben stattfindet.
Zur Justierung des Gerätes ist eine Justierung der Lampe 3 notwendig. Da, wie schon erwähnt, die Lampe 3 mit einem unter hohem Druck stehenden Gas gefüllt ist, muss mit der Möglichkeit einer Explosion dieser Lampe gerechnet werden. Es ist deshalb notwendig, während des Justiervorganges einen Raum zu schaffen, in den keine Lampensplitter eindringen können. Zu diesem Zweck dient eine Scheibe 23, welche ebenfalls auf dem Rohr 9 angeordnet und mit diesem fest verbunden ist. Diese Scheibe 23 wirkt in der äussersten vorderen Stellung des Rohres 9 mit einer Zwischenwand 22 zusammen und schliesst dadurch den vorderen Teil 46 des Gehäuses 1 gegen den die Lampe 3 enthaltenden hinteren Teil 47 splitterdicht ab. Zur Arretierung des Rohres 9 in der vordersten Stellung dient ein koaxial angeordnetes Rohr 45, welches mit einem Splitterschutz 44 verbunden und auf dem Rohr 9 frei drehbar ist.
In das Rohr 45 ist eine Nut 20 eingefräst, in welche ein mit dem Gehäuse 1 fest verbundener Stift 21 eingreift.
Wird nun durch Kraftanwendung das Rohr 9 in die vorderste Stellung gezogen, so gleitet bei einer Drehung des Rohres 45 der Stift 21 in den Teil 48 der Nut 20. Dadurch ist das Rohr 9 in der vordersten Stellung verriegelt. Durch Anlegen der Scheibe 23 an die Zwischenwand 22 ist dabei gleichzeitig ein splittersicherer Abschluss zwischen den Räumen 46 und 47 gewährleistet. Nunmehr kann die Lichtquelle 3 durch Drehen der Justierschrauben 24 und 25, welche vom vorderen Raum 46 aus zugänglich sind, justiert werden. Um zu vermeiden, dass eine Justierung des Gerätes vorgenommen wird, wenn das Rohr 9 nicht in vorderster Stellung verriegelt ist, sind zwei elektrische Schalter vorgesehen. Diese sind so geschaltet, dass die Lampe 3 nur dann brennt, wenn mindestens einer dieser Schalter geschlossen ist. Der vordere Raum 46 ist mittels einer nicht dargestellten Türe verschlossen.
Mit dem Griff dieser Türe ist einer der elektrischen Schalter verbunden, so dass beim Öffnen der Türe auch dieser Schalter geöffnet wird. Der zweite elektrische Schalter ist durch 18 bezeichnet und wird nur dann geschlossen, wenn das Rohr 9 in vorderster Stellung verriegelt ist. In diesem Fall wird der Hebel 17 entgegen der Wirkung der Feder 41 gedreht und drückt mit seinem Stift 42 auf den Schalter 18.
Wird also die Türe des vorderen Raumes 46 geöffnet, wenn das Rohr 9 nicht in vorderster Stellung verriegelt ist, so sind beide elektrische Schalter ge öffnet und die Lampe 3 erlischt. Ist dagegen das Rohr 9 in vorderster Stellung verriegelt und der Schalter 18 geschlossen, so brennt auch nach Öffnen der Türe des Raumes 46 die Lampe 3 weiter und es kann justiert werden.
Zur Durchführung einer Behandlung wird das gesamte Gerät zunächst an das Bett des Patienten herangeschoben. Zu diesem Zweck dienen am Gehäuse 2 angeordnete Räder. Danach wird durch Höhe und Seitenverstellung bzw. Längsverschiebung des Rohres 9 bei eingeklappter Siebblende 8 auf das Auge des Patienten eingestellt. Nunmehr kann durch Verschwenken des Spiegels 10 die Netzhaut des Auges abgesucht und schliesslich auf die Koagulations. stelle eingestellt werden. Durch Druck auf den Knopf 13 wird dann die Siebblende 8 ausgeklappt und die Koagulation durchgeführt.
Device for generating coagulations, especially in the fundus of the eye
In the case of detachment of the retina of the eye or in the case of retinal tears, according to a known treatment method, welding between the retina and the choroid of the eye can be achieved in that the retina is coagulated at the relevant point by exposure to light. For this purpose, a light source of high radiation density is imaged on the fundus of the eye. In the process, the pigment epithelium and the choroid are particularly strongly absorbed by radiation energy, which is largely converted into heat. The heat generated in the tissue at the point of impact of the light rays is carried away into the environment by conduction and convection.
The choroid in particular has good thermal conductivity due to its abundance of blood vessels. However, if the heat supply exceeds the heat dissipation in the unit of time, the temperature in the irradiated tissue increases continuously, which ultimately leads to the coagulation of the tissue protein, i.e. coagulation. In the course of 1 to 2 weeks, the resulting coagulation focus flattens and finally turns into a scar, within the retina and choroid form a joint membrane permeated with pigment.
A known device for carrying out this treatment method contains a light source of high radiation density, a condenser for imaging the light source in a diaphragm, a lens for generating a parallel beam path behind this diaphragm and a mirror arranged behind this lens, viewed in the direction of the beam, by means of which the light beams onto the Eye of the patient. The incident parallel light rays are combined on the fundus through the eye lens and can thus cause coagulation at this point if the radiation density of the light source is sufficient. In order to be able to carry out the treatment under constant supervision of the doctor, the above-mentioned mirror is provided with a central through-hole through which the doctor observes the patient's eye.
In this known device, the optical elements mentioned are arranged in a housing which can be moved on wheels. This results in the disadvantage that the device is very difficult to move and that this makes it extremely difficult to adjust the position of the retina to be treated.
This disadvantage is avoided according to the invention in that the optical elements mentioned as well as a concave mirror attached behind the light source are arranged on a common carrier, which is freely rotatable about its vertical and transverse axis in the center of gravity and suspended in a housing so that it can be moved longitudinally. This ensures that the carrier containing the optical elements can be rotated about the vertical and transverse axes and displaced in the longitudinal direction without any weight shifts occurring. This means that with a corresponding design of the bearings, an extremely smooth movement of the carrier can be made and that the attending physician can bring the device into the desired position without any effort.
It is advantageous to arrange the optical elements in a tube that is surrounded by a further coaxial tube in the vicinity of its center of gravity and is rotatably mounted in this about the transverse axis, this second tube being suspended in a fork that can rotate about the vertical axis in turn is rotatable about an axis parallel to the vertical axis of the tube.
In the following, an embodiment of the device according to the invention is explained in more detail with reference to FIGS. Show:

In Fig. 1, 1 designates a housing which is arranged on a further housing 2 which contains all the electrical parts necessary for the voltage supply of the device. A tube 9 containing the optical parts of the device is mounted in the housing 1. At the same time, a light source 3 is connected to this tube 9, the voltage supply of which is taken from the housing 2. An ignition device 6 for the lamp 3, which contains, for example, two electrodes arranged in a high pressure gas, is arranged separately from the tube 9 in the housing 1. A carrier 26, which carries a concave mirror 4 and a counterweight 5, is also connected to the tube 9.
The optical parts of the device are shown in FIG. A point diaphragm 7 and a screen diaphragm 8 are located in front of the light source 3. The light source 3 is imaged in the plane of a diaphragm 28 by means of a condenser 27. The aperture 28 is expediently designed so that its opening diameter can be varied. The intermediate image created in the plane of the diaphragm 28 is imaged into infinity by means of a lens 29. The parallel light bundle emanating from the lens 29 is reflected by a mirror 10 into the eye 30 of a patient. The incoming light is focused on the retina of the eye through the eye lens 31. At this point, coagulation can be caused if all of the light emanating from the light source 3 reaches the patient's eye 30.
For this purpose it is necessary to swivel the screen diaphragm 8 out of the beam path during the actual treatment process. The screen diaphragm 8 only has the task of weakening the light emanating from the light source 3 during the search of the retina and the adjustment to the area to be treated so that no damage to the eye 30 can occur.
The peripheral holes of the screen diaphragm 8 are imaged with the exit pupil of the device in the pupil of the irradiated eye 30 and are visible here as luminous bundles of rays. This makes it easier to center the beam path precisely in the patient's pupil. To enable observation of the patient's eye 30, the mirror 10 is provided with a central through-hole 11. The attending physician, whose eye is denoted by 32, can observe the patient's eye 30 during the entire treatment process through this perforation.
In order to avoid dazzling the doctor during the treatment process, i.e. in the period in which the full light from the light source 3 falls into the patient's eye and the screen diaphragm 8 is swiveled out of the beam path, a gray filter 33 is provided, which is used during this period is automatically pushed in front of the central through hole 11 of the mirror 10.
As can be seen from FIG. 1, the mirror 10 is fastened to a handle 12 and to the tube 9 at the same time. For adjustment, the mirror 10 can be rotated together with the handle 12 around the axis of the tube 9. It is also possible to rotate the mirror 10 about the axis of the handle 12.
If the device is set to the correct position of the eye, the doctor presses the button 13. This simultaneously pushes the gray filter 33 in front of the central through-hole 11 of the mirror 10 and automatically swings the screen 8 out of the beam path. After releasing the button 13, the gray filter 33 moves back and the screen 8 is swiveled in again. It has proven to be expedient to use an electromagnet for pivoting the screen diaphragm 8 in and out, which electromagnet is excited by a switch operated by means of the button 13.
To adjust the device, however, it is not only necessary to rotate the mirror 10, but it is also necessary to move the tube 9. For this purpose, it may be necessary to move the tube 9 in the longitudinal direction and at the same time to pivot it up and down. The support of the tube 9 described below is used to enable these movements.
As can be seen from FIG. 3, the tube 9 is surrounded by a coaxial tube 33 in the vicinity of its center of gravity, which can be displaced in the axial direction by moving the counterweight 5. The tube 9 is mounted pivotably about the transverse axis in the tube 33 by means of the pins 34 and 35. The radial distance between the inner diameter of the outer tube 33 and the outer diameter of the tube 9 and the length of the tube 33 are dimensioned such that the tube 9 can only be pivoted about the pins 34 and 35 by a predetermined angle.
The tube 33 is suspended in a fork 38 and can be pivoted about the vertical axis in this by means of the pins 36 and 37. The fork 38, in turn, is mounted on a carrier 14 by means of the bearings 39 and 40 in such a way that it can be pivoted about the axis of this carrier.
This storage makes it possible to pivot the tube 9 vertically and sideways and also to move it in the longitudinal direction. In order to create a stop for the movements of the pipe 9 to the side as well as for the longitudinal movements, the pipe 33 is provided with a pin 15 which engages in a guide curve 16. This guide curve is cut into a sheet metal 50 which is fastened to the carrier 14. The shape of the lateral delimitation of the guide curve is chosen so that when the pin 15 slides on this curve, the tube 9 moves in an exactly axial direction. The front limitation of the guide curve is formed by a lever 17 which can be pivoted about an axis 43. This lever 17 is held in the rest position by means of a spring 41.
The tension of the spring 41 is selected so that normally when the pin 15 hits the lever 17, there is no rotation of the same.
To adjust the device, an adjustment of the lamp 3 is necessary. Since, as already mentioned, the lamp 3 is filled with a gas under high pressure, the possibility of this lamp exploding must be expected. It is therefore necessary to create a space during the adjustment process into which no lamp splinters can penetrate. For this purpose, a disk 23 is used, which is also arranged on the tube 9 and firmly connected to it. In the outermost front position of the tube 9, this disk 23 cooperates with an intermediate wall 22 and thereby closes the front part 46 of the housing 1 against the rear part 47 containing the lamp 3 in a splinter-proof manner. To lock the tube 9 in the foremost position, a coaxially arranged tube 45 is used, which is connected to a splinter guard 44 and is freely rotatable on the tube 9.
A groove 20 is milled into the tube 45, into which a pin 21 firmly connected to the housing 1 engages.
If the tube 9 is now pulled into the foremost position by the application of force, when the tube 45 is rotated, the pin 21 slides into the part 48 of the groove 20. As a result, the tube 9 is locked in the foremost position. By placing the disk 23 on the partition 22, a splinter-proof seal between the spaces 46 and 47 is ensured at the same time. The light source 3 can now be adjusted by turning the adjusting screws 24 and 25, which are accessible from the front space 46. In order to avoid the device being adjusted when the tube 9 is not locked in the foremost position, two electrical switches are provided. These are switched so that the lamp 3 only burns when at least one of these switches is closed. The front space 46 is closed by means of a door, not shown.
One of the electrical switches is connected to the handle of this door, so that this switch is also opened when the door is opened. The second electrical switch is indicated by 18 and is only closed when the tube 9 is locked in the foremost position. In this case, the lever 17 is rotated against the action of the spring 41 and presses the switch 18 with its pin 42.
So if the door of the front room 46 is opened when the tube 9 is not locked in the foremost position, both electrical switches are ge opens and the lamp 3 goes out. If, on the other hand, the tube 9 is locked in the foremost position and the switch 18 is closed, the lamp 3 continues to burn even after the door of the room 46 is opened, and adjustments can be made.
