Device for medical examination using X-rays. The invention relates to a device for medical examination with X-rays, in which the X-ray tube can be moved parallel to the plane of the radiation receiver, so that the beam can be sent in different directions through the body of the patient. The radiation receiver can be a fluorescent screen for direct observation or indirect photography of the X-ray image or a cassette with a photographic film or with a number of such films for serial exposures.
In some cases there is a grid against secondary rays in front of the film, which can be moved at the same time as the film cassette and brought to the correct position behind the body to be photographed.
Where in the following for the sake of brevity a screen is mentioned, any other of the named radiation receivers can be arranged instead. A device of this type is known in which the X-ray tube is rotatable and coupled to the screen in such a way that when the X-ray tube is displaced relative to the screen, the central beam always hits the screen at the same point.
In the known device, this condition is met both in a vertical movement (the device is only suitable for examining standing or sitting patients) and in a horizontal movement of the X-ray tube.
The invention is based on the idea that when the x-ray tube is shifted to the side, it is more a question of selecting a specific part of the patient's body than of obtaining an obliquely falling bundle. For the last-mentioned purpose, it is sufficient if the axis of the beam is pivotable, for example, in a vertical plane.
The inventive device for medical examination with X-ray sources, in which the X-ray tube can be moved parallel to the screen plane, is characterized by the fact that it has a device which couples the tube 9 and the beam receiver 11 to one another in such a way that when the relative displacement of the X-ray tube with respect to the beam receiver in the longitudinal direction of a support plate 3 for the patient,
the middle beam always hits the beam receiver at the same point and which also causes a simultaneous translation of the X-ray tube und.des beam receiver in the same sense parallel to the support plate and in the transverse direction of this support plate.
The tube and the screen can be attached to various stands that can be rolled over the floor of the room. If the Vorrich device with a rotatable about a horizontal shaft support table for the patient see ver, it is more useful if, as suggested elsewhere, the Tragvor devices for the X-ray tube and the screen are attached to the table itself and the rotation of the table make your axis with.
A rod that is useful as a coupling element between the X-ray tube and the screen can, if desired, be telescopic so that if the distance between the X-ray tube and the screen changes, the ends do not need to be moved with respect to the holder of the X-ray tube or the screen .
If such a coupling rod is seen before, it can be rotatable about its axis, in which case means are present which convert this rod about its axis into the translation of the X-ray tube and the screen. A handle can be attached to the rod to rotate it. In this case, the movement is transmitted from the rod to the screen and the X-ray tube. The umbrella itself can also be set up so that it can be grasped with the hands and moved sideways. In this case, the movement of the screen is transmitted from the screen to the coupling rod and from the latter in turn to the X-ray tube.
On the X-ray tube or on the screen or both of a rack can be fastened, which engages in a rod mounted on a coupling gear and is moved when the rod is rotated about its axis in its longitudinal direction. This is less easy with the screen than with the X-ray tube, since such a rod must be rotatable about a horizontal axis located in the plane of the screen.
To achieve this goal, the rack can be connected to the rod in such a way that it joins the movement of the rod about an axis in the plane of the screen; the rack must consequently have a connection with the screen that allows this movement, in such a way that when the rod is rotated around its own axis, it moves the screen in the transverse direction of the support plate in the same sense as the x-ray tube.
Different solutions are possible with a rack fixed immovably on the screen. Thus, the movement of a rotatable coupling rod can be transmitted to the rack by means of two gears connected to one another by means of a flexible shaft, one of which is coupled to the rack attached to the screen and the other to a gear mounted on the coupling rod. Another solution is that the coupling rod is connected to the screen by means of a cross joint coupling, the center of which is in the plane of the screen.
One hand can be attached to the universal joint coupling, but the device can in turn be designed in such a way that the movement from the screen is transmitted to the coupling rod via the universal joint coupling.
The connection between a coupling rod and the umbrella can also be achieved by a crank and a connecting rod. Unless a cross-joint coupling is used between the handle and the coupling rod, the plane of the crank and the connecting rod must go through the axis around which the coupling rod can be pivoted with respect to the screen, and the cross head must be fastened to the screen so that it can rotate about this axis.
In the accompanying drawings, for example, an embodiment of the device according to the invention is shown schematically in two pro sections, namely: Fig. 1 is a view of the device perpendicular to the table top, Fig. 2 is a side view of the device, Fig. 3 a Part of FIG. 2 omitting individual parts, and FIG. 4 shows a detailed variant.
The storage table consists of the frame 1, the base 2 and the table top 3. The frame 1 can be rotated in the foot 2 around a horizontal shaft 4. The rotation takes place elec trically by means of a motor and a gear segment, which is contained in a box 5 th. It can also be done by hand using a crank. Except for the table top 3, the columns 6 and 7 be fastened in the frame 1. A carriage slides over each column. One of the carriages (8) carries the X-ray tube 9, the other (10) the screen 11 .. By means of counterweights, which are not shown, the movement of these carriages when the frame assumes a vertical position; facilitated.
These counterweights are movable in the hollow columns 6 and 7 and can be connected to the carriage by means of chains or steel wires running over rollers. In the carriage 8, a slide 12 is perpendicular to the frame 1 plane. This slide can also be connected to a counterweight in order to facilitate movement when the table is positioned horizontally or at an angle. The X-ray tube is attached to the slide 12 so that it can rotate about an axis 13. The screen 11 is attached to two rods 14 and 15 which can slide in sockets 16 and 17 of the carriage 10 so that the distance between the projection surface and the table top can be changed.
With the aid of clamping screws 18, the carriages 8 and 10, the slide 12 and the rod 14 can be secured against displacement.
The X-ray tube 9 is clamped in a holder 19 which ends in a spindle 20 which is rotatable in the support arm 21 of the carriage 12 and has a side piece 22. The axis 13 of the spindle 20 runs through the focal point of the X-ray tube. In the side piece 22 ends a three-part, telescoping rod 23 which is rotatable about its own axis. On the thickest piece 24 of this rod 23 pins 25 are attached, the axis of which lies in the plane of the screen 11 and runs through the center of the latter. These pins are rotatable in an annular bracket 26 which is formed by the frame 27 in which the screen is vorgese hen.
Since the axis of the rod 23, which intersects the axis 13 perpendicularly, and the middle beam of the X-ray tube lie in one plane and the axis of rotation of the pin 25 is par allel to the axis of rotation 13 and runs through the center of the screen 11, the middle beam hits the screen 11 in the middle. If the carriage 10 with the screen is moved upwards from the position shown along the column 7 while the carriage 8 is held, the distance between the focal point and the center of the screen is reduced and the inclination of the rod 23 changes Rods 23 are then pushed into one another and the rod rotates in bracket 26.
The spindle 20 and thus also the central beam rotate with the rod 23. The middle beam therefore continues to run through the center of the screen, regardless of where the carriage 10 is located on the column 7.
Conversely, it is possible to determine the carriage 10 and to move the carriage 8 along the column 6. Even then, the condition that the middle beam must run through the center of the screen 11 is always met. It is therefore possible to radiate through every part of the body of one standing on the footboard 28 or, with the support table in a horizontal or inclined position, lying on the table top 3 at any angle, the shadow image is always approximately in the middle of the screen 11.
Except along the column 6 and perpendicular to the table top, the X-ray tube 9 is also horizontally displaceable in the transverse direction of the table top. The spindle 20 is not only rotatable for this purpose, but also slidable in the support arm 21. The screen 11 is not only movable along the column 7 and perpendicular to the table top, but also horizontally in the transverse direction of the table top. For this purpose it is formed as a slide located in the frame 27.
The transverse movement of the X-ray tube 9 and the screen 11 can be brought about by means of the rod 23, namely because the rod is rotated about its axis. At the end of the rod 23 the latter carries a gear 29 which engages in a toothed rack 30. This rack is attached to the spindle 20 which can be moved horizontally in the arm 21. The bearing piece 22 does not sit unmovably on the spindle 20, but rather on a bushing which is slidable over the spindle but not rotatable with respect to the latter and which is enclosed in a fork formed by the support arm 21.
If the rod 23 rotates about its axis GE, the gear 29 is set in rotation and the X-ray tube is moved in since Licher direction, the spin del 20 slides through the support arm 21 and the socket of the bearing piece 22. A handle 31 is provided at the end of the rod 23 reaching through the bracket 26, so that the rod 23 can be used as a handle when the movements of the carriages 8 and 10 are carried out along the columns. This hand also serves to rotate the rod 23.
The pins 25 are not attached directly to the part 24 of the rod 23, but to a socket 32 in which the rod is rotatable bar. The part 24 carries a gear 33 which has the same diameter as the gear 29. It engages in a toothed rack 34 which cannot be permanently attached to the screen 11, since it would otherwise leave the gearwheel as soon as the carriage 10 is moved up or prevent the carriage from moving downwards. For this reason, the rack 34 is arranged displaceably in a socket 35 which is attached to one of the pins 25 or to the socket 32 itself (see FIGS. 1 and 3).
When the rod 23 rotates about the pin 25, the toothed rack 34 rotates with it and remains in engagement with the toothed wheel 33.
One end of the rack 34 has a corresponding to the movement around the Zap fen 25 displaceable connection to the screen 11, so that the rack free runs when Dre hung around the horizontal axis, but moves the screen when moved in its longitudinal direction. For this purpose, the end of the rack extends through a circular slot 36 in a perpendicular plate 37 which is attached to the screen 11. To ensure that the rack does not slide through the slot, the end is thinner and the slot is correspondingly narrower; to ensure that it is not pulled out, it has a thickening 38 on the other side of the plate.
Instead of a plate provided with a slot, it would also be possible to use a crank attached to the screen 11, which can be rotated about an axis which coincides with that of the pin 25 and which surrounds the rack. Also two gears connected to each other by mediation of a flexible shaft can be provided, one of which is coupled to the rack of the screen and the other to the gear 33 ge. In the latter case, the rack can be immovably connected to the screen 11.
4 shows how the coupling rod 23 can be connected to the screen 11 by a crank and a drive punch. A crank 39 attached to part 24 extends outward through a slot 40 in bushing 32 and is connected to drive rod 41. The plane of the drive rod 41 and the cure bel 39 runs through the axis 42 of the Zap fen 25. The cross head 43 is attached to the screen 11 rotatably about this axis. If the rod 23 pivots about the pin 25, the plane of the crank and the drive rod also pivot. The cross head rotates around the axis 42 with respect to the screen, so that the movement of the rod 23 is not hindered.
If the handle 31 is rotated, the crank 39 pivots through the slot 40 and the drive rod 41 pushes or pulls the screen 11 in the direction of the axis 42.
If the rotation of the rod 23 about its axis is converted into a straight movement in the case of the X-ray tube with gear and rack, in the case of the screen with a crank and drive rod, the movement of the X-ray tube is only approximately the same as that of the screen, but not excessive in one short crank the approximation within the dodge of the screen is sufficient.
Another possibility is that the handle 31 is attached to the frame 27 in such a way that it assumes an unchangeable position with respect to the screen 11, and is connected to the rod 23 by a universal joint coupling whose center point lies in the axis 42 . The Drehbewe movement of the handle can be transferred to the screen by means of a gear and an immovable rack attached to the screen or by arranging a simple cure mechanism.
In all of these cases, the handle can also be attached directly to the sliding part of the screen, so that the movement from the screen is transmitted to the coupling rod 23 via a gearwheel or a crank. The slide can, for example, be provided with handles on the sides that can be handled.
