De uitvinding heeft betrekking op een farmaceutisch preparaat in het bijzonder voor al dan niet in een ziekenhuis opgenomen patienten, gekenmerkt door een hoge dosis gereduceerd glutathion. De uitvinding heeft meer specifiek betrekking op een farmaceutisch preparaat, in het bijzonder voor het gebruik voor al dan niet in een ziekenhuis opgenomen patienten, dat per eenheidsdosis ten minste 2, 5 g gereduceerd glutathion bevat en bij voorkeur 5 g gereduceerd glutathion, voor gebruik als beschermend middel tegen de door platinacoördinatieverbindingen, in het bijzonder"cis-platina"veroorzaakte neveneffecten (in het bijzonder nefrotoxiciteit).
Het gebruik van cis-diamine-dichloorplatina (CDDP) of"cis-platina"bij de antiblastische chemotherapie is bekend. De opvallende, door cis-platina op het niveau van de nier-tubuli geinduceerde nefrotoxiciteit is eveneens bekend. De toediening van CDDP wordt daarom gelijktijdig uitgevoerd met een duidelijke hydratatie van de patient via de intraveneuze weg, zowel voor als na toediening van het geneesmiddel en gaat dikwijls gepaard met de opwekking van geforceerde diurese door combinatie met diuretica. Niettemin kan noch door een sterke hydratatie of een eveneens sterke gedwongen diurese de opvallend ernstige schade aan de tubuli na behandeling met cis-platina voorkomen worden.
Een zekere mate van nierbeschermende werking van gereduceerd glutathion (GSH), zoals waargenomen aan de hand van enkele parameters (azotemie, creatinemie, veranderingen in lichaamsgewicht, de gewichtsverhouding van de nier tot het lichaamsgewicht) zijn eerder aangetoond in met cis-platina behandelde dieren (F. Zunino, O. Tofanetti et al., Tumor 105-111, 1983). Het werd echter opgemerkt dat zelfs de toediening van relatief enorme doses (1 g/kg lichaamsgewicht) van GSH het niet mogelijk maakte beschadigingen aan de tubuli zowel bij de rat als de muis volledig te elimineren.
Verrassenderwijze is nu gevonden dat wanneer GSH daarentegen aan de mens wordt toegediend in hoge doses (hoger waarde, en opmerkelijk hoger dan gebruikt voor andere indicaties), maar veel lager dan die afgeleid kunnen worden uit de experimenten (hoewel onbevredigend) uitgevoerd op dieren, in staat is niet alleen in bijna volledige mate te beschermen tegen de door het cis-platina geinduceerde

69,beschadigingen van de tubuli maar tegelijkerijd de therapeutische effec- ten van het cis-platina zelf onveranderd te laten.
De bovengenoemde verrassende waarneming is bovendien gedaan volgens parameters die veel gevoeliger en preciezer zijn dan die in de dierproeven gebruikt worden en in het bijzonder door in de urine van de patient de spiegel van twee microënzymen te bepalen, alanine-aminopepti- dase (AAP) en N-acetyl-ss-glucosaminidase (NAG), die dratisch toenemen (ongeveer 8 tot 10 maal de normale waarden) in het geval van beschadi- gingen van de tubuli.
Het belang en de betekenis van de AAP-en NAG-spiegels als index voor nefrotoxiciteit is aangetoond door verschillende auteurs : zie bijvoorbeeld B. R. Jones et al.,"Comparison of methods of evaluating nephrotoxicity of cis-platinum", Clin. Pharmacol. Ther 1980,557 ; R. L. De bescherming tegen door cis-platina geinduceerde beschadi- gingen van de tubuli door toediening van GSH is daarom gedocumenteerd aan de hand van de systemische dosis AAP en NAG in de urine van de pa- tient, zoals hierna vermeld.
Er werden vrouwelijke patienten beschouwd die leden aan gezwel- len van de ovaria, tijdens verschillende cycli van de behandeling met
2 cis-diamine-dichloor-platina (CDDP) bij tamelijke hoge doses (90 mg/m, via de intraveneuze weg). De AAP-en NAG-dosis in de urine van de pa- tient is bepaald in afwezigheid en aanwezigheid van de GSH-behandeling, die werd uitgevoerd vóór en/of na de toediening van het CDDP (het GSH werd steeds toegediend door middel van langzame infusie).
De dosis van het microënzyme alanine-aminopeptidase (AAP) werd bepaald door met behulp van een colorimetische bepaling het p-nitro- alanine te meten dat door dit enzym vrijgemaakt wordt door katalytische hydrolyse van alanine-p-nitroanilide (zie J. E. Peters et al., Clin. Chim. acta 37, 213 (1972) ; K. Jung et al., Cin. Chem. 26 1251 (1980). Het N-ace- tyl--glucosaminidase (NAG) is eveneens bepaald door een colorimetrische bepaling, door in een alkalisch medium het p-nitrofenol te meten dat

door het microënzym wordt vrijgemaakt uit p-nitrofenyl-N-acetyl-ss-D- glucosaminide (zie D. Marhun, Clin. Chim. Acta 73,453 (1976) ; T. D. Lockwood et al., Tox. Appl. Pharmacol. 49,323 (1979) ; A. M. Gressner et al., Clin.
Chim. Acta 124, 315 (1982)).
In totaal zijn 50 therapeutische cycli met CDDP onderzocht met de hierna vermelde resultaten : A. Alleen hydratatie, zonder GSH-behandeling. Het AAP ondergaat een gemiddelde toename van ongeveer 8 maal de normale waarde ; het NAG ondergaat een gemiddelde toename van ongeveer 10 maal de normale waarde.
B. Hydratatie + 1 gram (in totaal) GSH, toegediend 15 minuten voor of na infusie van het cis-platina ; er wordt geen beschermende werking waargenomen ; het niveau van de enzymen AAP en NAG in de urine is vrijwel vergelijkbaar met het voor groep A bepaalde punt.
C. Hydratatie + 2 gram (in totaal) GSH, toegediend met een snelheid van 1 gram 15 minuten vóór en 1 gram 15 minuten na de infusie van het cis-platina : een voldoende duidelijke bescher- mende werking wordt waargenomen aangezien de dosis van de twee enzymen een gemiddelde toename laat zien van 5 maal voor AAP en 7 maal voor NAG, ten opzichte van de normale waarde.
D. Hydratatie + 2,5 gram (in totaal) GSH, waarbij ook in dit geval de helft 15 minuten vóór en de helft 15 minuten na de infusie van het cis-platina werd toegediend : de bescherming is hoger dan in het geval C, aangezien de gemiddelde spiegels van AAP en NAG respectievelijk drie tot viermaal de normale waarden zijn.
E. Hydratatie + 5 gram (in totaal) GSH, toegediend volgens de bovengenoemde criteria : het gemiddelde niveau van de twee en- zymen is ongeveer tweemaal de normale waarde, met een vrijwel volledige bescherming van de patient.
Er wordt op gewezen dat zoals reeds vermeld zelfs in groep E (en dientengevolge ook in de eerdere groepen) de therapeutische resultaten van de toediening van het cis-platina onveranderd blijven.
De bovengenoemde resultaten maken het mogelijk de volgende conclusies te trekken :

a) toediening van 1 gram GSH is vrijwel zonder effect ; b) bij verandering van de dosis van 2 tot 2,5 gram GSH wordt een kritische dosis gevonden aangezien door verhoging van het GSH met 25% een 40% afname van de AAP-spiegel en zelfs een 50% afname in de NAG-spiegel wordt verkregen ; c) de werkzame dosis van het GSH bij de bescherming tegen door het cis-platina geinduceerde nefrotoxiciteit is opmerkelijk hoger dan de voor andere therapeutische indicaties gebruikte
GSH-doses ; tegelijkertijd is het ten minste één ordegrootte lager vergeleken met de dosis die in het dier een bescherming teweeg brengt die bovendien nog onbevredigend is.
Een wezenlijk aspect van de uitvinding wordt daarom verschaft door farmaceutische preparaten, in het bijzonder voor al dan niet in een ziekenhuis opgenomen patienten, geschikt voor de bescherming tegen door platinaverbindingen geinduceerde nevenwerkingen (in het bijzonder nefrotoxiciteit), die hoeveelheden gereduceerd glutathion bevatten van niet lager dan 2,5 g per eenheid dosis, bij voorkeur van 5 g per eenheid dosis, indien gewenst gemengd met in de farmaceutische techniek gewoonlijk gebruikte excipiëntia en/of met andere actieve bestanddelen.

The invention relates to a pharmaceutical preparation in particular for hospitalized or non-hospitalized patients, characterized by a high dose of reduced glutathione. The invention more specifically relates to a pharmaceutical composition, in particular for use in hospital or non-hospitalized patients, containing at least 2.5 g of reduced glutathione per unit dose and preferably 5 g of reduced glutathione, for use as protective agent against the side effects (in particular nephrotoxicity) caused by platinum coordination compounds, in particular "cis-platinum".
The use of cis-diamine-dichloroplatinum (CDDP) or "cis-platinum" in antiblastic chemotherapy is known. The striking nephrotoxicity induced by cis-platinum at the level of the renal tubules is also known. The administration of CDDP is therefore performed simultaneously with marked hydration of the patient by the intravenous route, both before and after administration of the drug, and is often associated with the induction of forced diuresis in combination with diuretics. Nevertheless, neither the strong hydration or the equally strong forced diuresis can prevent the remarkably serious damage to the tubules after treatment with cis-platinum.
Some degree of renal protective activity of reduced glutathione (GSH), as observed by some parameters (azotaemia, creatinaemia, changes in body weight, kidney to body weight ratio) has been previously demonstrated in animals treated with cis-platinum ( F. Zunino, O. Tofanetti et al., Tumor 105-111, 1983). However, it was noted that even the administration of relatively massive doses (1 g / kg body weight) of GSH did not allow complete elimination of tubular damage in both the rat and mouse.
Surprisingly, it has now been found that when GSH is administered to humans in high doses (higher value, and markedly higher than used for other indications), but much lower than that which can be derived from the experiments (although unsatisfactory) performed on animals, in state is not only able to protect almost completely against the induced by the cis-platinum

Moreover, the above surprising observation was made according to parameters much more sensitive and precise than those used in the animal experiments and in particular by determining the level of two microenzymes, alanine aminopeptidase (AAP) in the patient's urine. N-acetyl-ss-glucosaminidase (NAG), which increase dramatically (about 8 to 10 times normal values) in case of damage to the tubules.
The importance and significance of the AAP and NAG levels as an index for nephrotoxicity has been demonstrated by several authors: see, for example, B. R. The protection against cis-platinum induced damage to the tubules by GSH administration has therefore been documented from the systemic dose of AAP and NAG in the patient's urine, as noted below.
Female patients with ovarian tumors have been considered during several cycles of treatment with
2 cis-diamine-dichloro-platinum (CDDP) at fairly high doses (90 mg / m, by the intravenous route). The patient's AAP and NAG dose in the urine was determined in the absence and presence of the GSH treatment, which was performed before and / or after the administration of the CDDP (the GSH was always administered by slow infusion).
The dose of the microenzyme alanine aminopeptidase (AAP) was determined by measuring the p-nitroalanine released by this enzyme by catalytic hydrolysis of alanine p-nitroanilide using a colorimetic assay (see JE Peters et al. A total of 50 therapeutic cycles with CDDP have been studied with the following results: A. Hydration only, without GSH treatment. The AAP undergoes an average increase of approximately 8 times the normal value; the NAG undergoes an average increase of approximately 10 times the normal value.
B. Hydration + 1 gram (total) of GSH administered 15 minutes before or after cis-platinum infusion; no protective effect is observed; the level of the enzymes AAP and NAG in the urine is almost comparable to the point determined for group A.
C. Hydration + 2 grams (total) of GSH, administered at the rate of 1 gram 15 minutes before and 1 gram 15 minutes after the cis-platinum infusion: a sufficiently clear protective effect is observed since the dose of the two enzymes shows an average increase of 5 times for AAP and 7 times for NAG, from the normal value.
D. Hydration + 2.5 grams (in total) GSH, in which case also in this case half was administered 15 minutes before and half 15 minutes after the cis-platinum infusion: the protection is higher than in case C, since the mean levels of AAP and NAG are three to four times the normal values, respectively.
E. Hydration + 5 grams (in total) GSH, administered according to the above criteria: the average level of the two enzymes is approximately twice the normal value, with almost complete protection of the patient.
It is pointed out that, as already mentioned, even in group E (and consequently in the earlier groups) the therapeutic results of the administration of the cis-platinum remain unchanged.
The above results allow the following conclusions to be drawn:

a) administration of 1 gram of GSH has virtually no effect; b) when changing the dose from 2 to 2.5 grams of GSH, a critical dose is found since increasing the GSH by 25% results in a 40% decrease in the AAP level and even a 50% decrease in the NAG level. obtained; (c) the effective dose of the GSH in the protection against nephrotoxicity induced by the cis-platinum is markedly higher than that used for other therapeutic indications
GSH doses; at the same time, it is at least one order of magnitude lower compared to the dose providing protection in the animal which is furthermore unsatisfactory.
An essential aspect of the invention is therefore provided by pharmaceutical compositions, especially for hospitalized or non-hospitalized patients, suitable for the protection against side effects induced by platinum compounds (in particular nephrotoxicity), containing amounts of reduced glutathione not lower then 2.5 g per unit dose, preferably 5 g per unit dose, if desired mixed with excipients commonly used in the pharmaceutical art and / or with other active ingredients.
